The follicular variant of Becker's nevus is an under‐reported entity. We present the rare occurrence of follicular Becker's nevus in 7 patients, confirmed through dermoscopy and histopathological examination. Dermoscopy shows perifollicular hypopigmentation surrounded by a well‐defined net‐like pigmentation corresponding clinically to the presence of folliculocentric macules. Histology shows prominent basal and suprabasal melanization surrounding the follicle, corresponding to well‐defined net‐like pigmentation seen on dermoscopy. However, the melanization does not extend along the entire length of the follicular epithelium leading to perifollicular hypopigmentation on dermoscopy. Background: Endodontic flareups are always very difficult to treat because of reasons like drug resistance previously sodium hypochlorite and chlorhexidine have been used in the literature but with advancements newer materials like herbal products have been tried for this purpose. Thus, present vitro study was conducted to comparatively evaluate the efficacy of herbal formulations on commensals of oral cavity and root canal. Materials and Methods: In this in vitro study antimicrobial efficacy of chlorhexidine 2%, Septilin and Triphala wer evaluated with the help of Kirby Bauer test. The microorganisms used were Staphylococcus aureus and Escherichia coli. The samples were placed in standardized wells and were subjected to incubation. Subsequently zones of inhibition were measured with the help of a caliper. One-factor analysis of variance, Tukey's least significant difference post hoc test and Student's independent t-test were performed to find a significant difference (P < 0.05) in the two groups. Results: For chlorhexidine in S. aureus mean antibacterial efficacy was 11.10 ± 1.25 and in E. coli 10.0 ± 1.33. The value for S. aureus was lesser in Triphala with a value of 10.35 ± 1.63 and in E. coli mean value was 9.05 ± 1.27. For Septilin the mean for S. aureus was 10.40 ± 1.04, and the mean antibacterial efficacy in E. coli was 9.65 ± 1.38. Conclusion: Septilin and Triphala showed remarkable efficacy concerning zones of inhibition. These herbal formulations have tremendous potential to be used as adjuncts to traditional disinfection modalities though it has scope of further research.

Chronic mucocutaneous candidiasis (CMC) is characterized by recurrent infections with Candida spp., often linked to primary immunodeficiencies. We report a case of two 8‐year‐old monozygotic twin brothers presenting with extensive dermatophytosis, later diagnosed with autoimmune polyendocrinopathy, candidiasis, and ectodermal dystrophy (APECED) syndrome due to a homozygous p.M1V mutation in the AIRE gene. The twins exhibited widespread skin and nail infection, along with malabsorption, dental caries, and other autoimmune manifestations. This case highlights the novel presentation of extensive dermatophytosis in APECED, underscoring the variability in clinical expression even within a single family. [ABSTRACT FROM AUTHOR]

ABSTRACT We report a rare case of furuncular myiasis in a 7‐year‐old boy presenting with a discharging nodule over the scalp. The report details the clinical presentation, examination findings, and dermoscopic features of furuncular myiasis. To the best of our knowledge, Chrysomya bezziana has never been reported to cause furuncular myiasis. In vivo and ex vivo dermoscopy features help in diagnosis by obviating the need for microscopy. Background: Isolated nail lichen planus (NLP) without mucocutaneous involvement is rare. Literature about the clinical profile and management is scarce. Aims/Objective: We attempted to characterize the clinico-demographic profile and analyze the management outcome of patients with isolated nail lichen planus. Methods: Records of 15 patients were evaluated, and their demographic profile, clinical features of the nail matrix and nail bed disease, onychoscopy findings, histopathological features, treatment given, and follow-up progress were analysed. Results: Data from 15 patients were collected. The mean age of the patients was 28.6 ± 19.0 years (range 3.5 years to 56 years). The gender ratio was 0.8 with 7 males and 8 females. The average disease duration at presentation was 2.8 ± 2.6 years (6 months–9 years). The average number of nails involved was 16.4 ± 4.6 (8-20 nails). All 20 nails were involved in 9 (60%) patients. Nail matrix involvement was seen in all, with onychorrhexis being the most common manifestation, which was seen in 11 (73.3%) patients. Nail bed involvement was seen in 11 (73.3%) patients, with onycholysis being the most common presentation. Severe nail disease was seen in 7 (46.7%) patients, and 5 (33.3%) had pterygium involving an average of 3.4 nails. Moderate to good improvement was observed in 9 (60%) patients after an average of 6.1 ± 2.4 (3-9) treatment sessions with intramuscular and intramatricial triamcinolone acetonide injection. Of these, 2/9 (22.2%) developed disease recurrence in a few nails after an interval of 1 and 1.5 years, respectively. Two patients achieved complete clinical cures that persisted beyond 2 years of follow-up. Limitations: Retrospective nature of the series and the small sample size are the major limitations. Conclusion: The risk of permanent disfigurement is high in NLP and calls for an early diagnosis and prompt treatment. Introduction: Viral hepatitis is a major health problem in both developing countries. Caused mainly by 5 hepatotropic viruses designated as hepatitis A, B, C, D, E. The present study was aimed to study the impact of various epidemiological factors, to compare the etiological profile of common viral agents and to study the outcome of children admitted with acute viral hepatitis. Methodology: This was a hospital based cross sectional study. All the children admitted in paediatric ward with symptoms of acute viral hepatitis during the study period were enrolled in the study. In this study data collected. Patients were treated according to protocol and final outcome was recorded. Results: Out of total 150 patients, maximum numbers of patients (45.33%) were less than 5 years of age and 108 (72%) were male. Most common complication was Fulminant Hepatic Failure (20%). Other complications were encephalopathy (5.33%), acute kidney injury (4%), DIC (2.67%) and spontaneous bacterial peritonitis (1.33%). Out of total 150, 142 (94.67%) were discharged suggesting acute viral hepatitis infection has good outcome. Conclusion: From the present study we concluded that fulminant hepatic failure due to hepatitis A virus is more common at younger age with high mortality. Safe drinking water and food and early active immunization may reduce mortality and improve outcome.

Introduction: During the last 30 years, Paediatric hepatology has been transformed by advances in our understanding and management of a wide spectrum of liver diseases. Globally however, viral hepatitis remains one of the most important causes of liver disease in children. Typical symptoms of acute hepatitis are fatigue, anorexia, nausea, and vomiting. Very high aminotransferase values (>1000 U/L) and hyperbilirubinemia are often observed. The objective of the study was to compare the clinical diagnosis and laboratory diagnosis. Methodology: The current study was conducted in Paediatric department of tertiary care hospital of Ahmedabad, Gujarat, India. It was a record based cross sectional study. Complete data of basic socio demographic profile, symptoms, signs, all laboratory investigations, outcome of patients were obtained and analysed. Results: There were total 150 cases of symptoms of acute viral hepatitis fulfilling inclusion criteria in Paediatric department. It was observed that first rise in cases was seen in the month of June and another peak was seen in the month of September. After September, there was rapid fall in total number of cases. Yellow sclera (94.67%) and yellow colour urine (93.33%) were most common symptoms reported by patients, followed by fever (88.67%) & loss of appetite (73.33%). Icterus (94.67%) and Subcostal Tenderness (66.67%) were most common sign. Conclusion: Clinical diagnosis corresponds laboratory investigations and whenever laboratory facility is not accessible, further management should be started at the earliest as per clinical diagnosis of viral hepatitis in children to prevent mortality. Majority of the patients had bilirubin below 10 mg/dl 126 patients (84%), while 12 (8%), patients had bilirubin up to 15, 7 (4.67%) between 15-20 mg/dl. Mean Serum bilirubin was 7.28 mg/dl.
