Abstract

Background & Aims: Despite several recent international guidelines, no consensus exists on the bleeding risk nor haemostatic parameter thresholds that define the safety of invasive procedures in patients with cirrhosis. The aim of this study was to establish a position paper on the bleeding risk associated with invasive procedures in patients with cirrhosis among the experts involved in various guidelines. Methods: All experts involved in recent guidelines on the management of invasive procedures in patients with cirrhosis were invited to classify 80 procedures as ''high risk'' or ''low risk'' with respect to bleeding. Procedures were considered high risk when the estimated risk of major bleeding was 1.5% or more, or when even minor bleeding might lead to significant morbidity or death. The experts were also asked to choose safety thresholds for laboratory test values at which elective invasive procedures could be safely performed. The predetermined threshold considered as “consensus” was ≥75% agreement. Results: Fifty-two experts participated in the study. Out of 80 procedures, a consensus opinion was reached for 52 procedures (65%): 17 procedures were classified as “high risk”, primarily interventional endoscopic procedures, percutaneous organ biopsies, or procedures involving the central nervous system; and 35 as “low risk”, primarily “diagnostic” procedures. The lowest platelet counts at which performance of a low-risk procedure or a high-risk procedure/surgery were deemed acceptable were 30 × 109/L and 50 × 109/L, respectively. Experts did not believe that international normalised ratio should be considered before performing low-risk procedures; 71% also indicated that it should not be considered before performing high-risk procedures. Conclusions: This experience-based classification may be helpful to refine future study designs and to guide clinical decision making regarding invasive procedures in patients with cirrhosis. Impact and implications: Several risk classifications and management guidelines for invasive procedures in patients with cirrhosis have been proposed, but with conflicting recommendations. By providing a position paper, based on the opinion of a broad panel of experts, on the bleeding risk associated with 52 invasive procedures in patients with cirrhosis, this survey will help to provide a framework for future study design. The consensus on platelet count, international normalised ratio, fibrinogen and activated partial thromboplastin time identified in this survey will inform physicians regarding the laboratory test values considered acceptable by the experts prior to the performance of an elective invasive procedure in patients with cirrhosis.

Abstract

A global review of islands and their connections with astronomy throughout history up to the contemporary times suggests eight compelling, distinct yet interlocking reasons why islands have been and remain so important to astronomy and astronomers. Islands constitute favourable locations for various types of astronomy-related activities: from tracking satellites and monitoring significant celestial events, to providing exceptional locations to jurisdictions with mandated dark and unpolluted skies. They appeal for their favourable longitude and (especially southern) latitude, as well as for their disposition towards the conditions that the scientific community may expect in an ideal world: relatively clear viewing conditions from a secure, self-contained platform that is, however, endowed with connectivity. Abstract

Fibrinogen is a soluble glycoprotein that acts as a clotting factor and acute phase reactant. Plasma fibrinogen levels are affected by environmental and demographic factors including gender, advancing age and seasonality. Polymorphisms within the three fibrinogen genes, FGA, FGB, and FGG, encoding the Aα, Bβ and γ chains respectively, also influence plasma fibrinogen levels. The fibrinogen genes have been extensively studied. However, there are conflicting results on whether individual single nucleotide polymorphisms (SNPs) within these genes confer an increased risk of Myocardial Infarction (MI). The -148C>T SNP (rs1800787) is a promoter SNP in FGB, the rate-limiting gene in fibrinogen synthesis. Abstract

We thank Ismail et al for their interest in our study (Chowdary et al, 2015) and for raising some important issues on this controversial topic. We acknowledge the limitation in our study: that this was a real life exercise and hence, the relatively wide time frame between low molecular weight heparin (LMWH) injection and blood sampling. Ismail et al reported typical anti-Xa pharmacodynamic profiles after fixed dose Tinzaparin in patients post-Caesarean section. However this is not really comparing like with like, because these are postpartum patients and observations post-surgical procedure are a study in themselves. It is interesting to note that Ismail et al reported very low peak anti-Xa levels with a concomitant reduction of thrombin-antithrombin (TAT). It would be interesting to see TAT levels in patients with a contraindication for LMWH as a control., peer-reviewed

Abstract

The Department of Radiology at St. Luke’s Hospital has provided a screening service for stroke related disease since April 1991. This consisted of Duplex Ultrasound screening (DUS) for Extracranial Carotid Artery Disease (ECAD) followed by angiography or intra-arterial digital subtraction angiography (lADSA) or digital intravenous angiography (DIVA) if ultrasound screening was positive for significant disease. The aim of this study was to evaluate in the local context, the various criteria for assessment already established overseas and to devise the best combination of these criteria to improve the detection of disease, thus improving the quality of the•local screening service. 504 patients have been screened for stroke related disease. Twelve patients (6M : 6F) with significant disease, who were considered for surgery, were referred for angiography, IADSA or DIVA. Comparison of these two modalities, DUS and vascular study, were made on 22 sides for the Multicentre Criteria (MCC), the Modified Seattle Criteria (MSC) and the Modified Washington Criteria (MWC). The accuracy, sensitivity, specificity, positive predictive value and negative predictive value were calculated for the MCC, the MSC and the MWC for peak systolic velocity. For the MCC the end diastolic velocity, the systolic velocity ratio and the diastolic velocity ratio were also compiled. The highest precision for extra cranial carotid artery disease screening can be achieved by a combination of the MCC or MSC for peak systolic velocity and with the systolic velocity ratio for the MCC., peer-reviewed
