Abstract

Mass casualty events particularly those requiring multiple simultaneous operating rooms are of increasing concern. Existing literature predominantly focuses on mass casualty care in the emergency department. Hospital disaster plans should include a component focused on preparing for multiple simultaneous operations. When developing this plan, representatives from all segments of the perioperative team should be included. The plan needs to address activation, communication, physical space, staffing, equipment, blood and medications, disposition offloading, special populations, and rehearsal.

Abstract

OBJECTIVE: US trauma centers (TCs) must remain prepared for mass casualty incidents (MCIs). However, trauma surgeons may lack formal MCI training. The recent COVID-19 pandemic drove multiple patient surges, overloaded Emergency Medical Services (EMS) agencies, and stressed TCs. This survey assessed trauma surgeons MCI training, experience, and system and personal preparedness before the pandemic compared with the pandemics third year. METHODS: Survey invitations were emailed to all 1544 members of the American Association for the Surgery of Trauma in 2019, and then resent in 2022 to 1575 members with additional questions regarding the pandemic. Questions assessed practice type, TC characteristics, training, experience, beliefs about personal and hospital preparedness, likelihood of MCI scenarios, interventions desired from membership organizations, and pandemic experiences. RESULTS: The response rate was 16.7% in 2019 and 12% in 2022. In 2022, surgeons felt better prepared than their hospitals for pandemic care, mass shootings, and active shooters, but remained feeling less well prepared for cyberattack and hazardous material events, compared with 2019. Only 35% of the respondents had unintentional MCI response experience in 2019 or 2022, and even fewer had experience with intentional MCI. 78% had completed a Stop the Bleed (STB) course and 63% own an STB kit. 57% had engaged in family preparedness activities; less than 40% had a family action plan if they could not come home during an MCI. 100% of the respondents witnessed pandemic-related adverse events, including colleague and coworker illness, patient surges, and resource limitations, and 17% faced colleague or coworker death. CONCLUSIONS: Trauma surgeons thought that they became better at pandemic care and rated themselves as better prepared than their hospitals for MCI care, which is an opportunity for them to take greater leadership roles. Opportunities remain to improve surgeons family and personal MCI preparedness. Surgeons most desired professional organization interventions include advocacy, national standards for TC preparedness, and online training. LEVEL OF EVIDENCE: VII, survey of expert opinion.

Abstract

With a looming enrollment crisis, colleges must explore new efforts for developing strategic and sustainable partnerships that support institutional advancement. As such, Kent State University (KSU) partnered with three regional, two-year institutions developing high-impact pathways to expand upon the transfer student experience and success rates. These transfer specific activities and opportunities were crafted to explore additional degree pathway opportunities with scalable transfer programs and services to increase access and success. As a large, multi-campus regional system, KSU leveraged these relationships to support its own institutional goals, while also ensuring the new regional collaborations provided value to these partners in a mutually beneficial way.

15. Building a Better Report Card.

Abstract

To assign grades that accurately communicate student performance, conscientious teachers must ensure their grading is consistent with that of other teachers, reveals student progress, and remains sensitive to impacts on student motivation and parent reaction. Teachers, parents, and administrators of the Round Rock Independent School District in Austin, Texas, met in 1997 to review and revise first-grade report cards, stimulated, in part, by confusion regarding existing report cards and successful changes implemented at the kindergarten level. Criteria emerging from discussions indicate report cards should clearly communicate achievement, provide information about progress toward exit-level standards, be easily understood, provide accurate descriptions of learning, and communicate growth over time. While offering some immediate improvements, an interim report card did not meet all the criteria, and continuing discussions were necessary to engender support for the developmental-continuum approach employed in kindergarten. A steering committee developed a time line, shared information with teachers and parents, and coordinated content-area committees. Planners developed a simple, one-page format that graphically depicts student growth over 9 weeks, progress toward grade-level standards, and information about attendance, special services, student responsibilities, promotion and retention, parent conferences, and reading levels. An Explanation of Stages guide describes key skills and processes linked to developmental progress. Teachers, administrators, and parents provided valuable input during the revision process, and the participation of all stakeholders was critical to project success. (TEJ)

17. Solar thermal storage using phase change material for space heating in residential buildings

Abstract

In 2007 the domestic sector was responsible for 27% of all energy consumed by final users in the UK, yet only 1.5% of this energy was met by renewables. The utilisation of renewable energy systems such as active solar water heating with Phase Change Material (PCM) thermal storage, offers vast potential for reducing energy use and CO2 emissions in the domestic energy sector in the UK. Previous research indicated that the incorporation of PCMs in underfloor heating had the potential to make energy savings, but their use in combination with renewable energy had not been explored in the UK. Consequently this was identified as a gap in the current knowledge that the current research would fill. A shortage was also identified in real life performance data on PCM space heating system performance in the UK. The current work successfully addresses this shortfall in data and in doing so provides a significant contribution to knowledge in the area of using solar thermal storage for space heating of residential buildings. An in depth literature review was undertaken as part of the research programme, which identified the key shortcomings in existing PCM based thermal storage systems for space heating. An underfloor space heating system for residential buildings was therefore developed that addresses the weaknesses of the existing systems highlighted in the literature review. The system stores solar thermal energy during the day and then uses this to provide space heating in the evening, thus addressing the problem of matching solar availability to demand. An experimental approach was adopted for the study as numerous researchers (Kauranen et al., 1991, Hasnian, 1998, Kenisarin and Mahkamov, 2007), have demonstrated the unreliability of manufacturer's published thermophysical properties of PCM. Therefore, this research chose to adopt an experimental model approach instead of a mathematical modelling approach. A model consisting of a full size solar collector 4m2 in area and a PCM filled underfloor heating panel was constructed in the laboratory. A methodology was developed to measure the performance of the key modules which allowed the performance of the system to be evaluated. The experimental data indicated that it was possible to use a low flow rate of 2.52 litres per minute, without a detrimental effect on the performance of the PCM panel. The use of a low flow rate minimises parasitic losses and produces significant energy savings in comparison to the use of higher flow rates. The experimental results indicated that the system was able to provide adequate thermal comfort with a maximum floor heat emission of 158 W/m using a flow rate temperature of 50°C. Comparisons of the annual space heating energy of the developed integrated system versus a wet central heating system in the UK revealed a significant reduction of energy use and associated CO2 emissions by as much as 52%.

27. Design and Fabrication of E-band Traveling Wave Tube for High Data Rate Wireless Links

Abstract

E-band (71 - 76 GHz and 81 - 86 GHz) is widely used for wireless point to point links with a few Gigabit/second data rate. E-band front-ends are powered by solid state amplifiers with about 1 W output power per module. This level of output power limits range and spectral efficiency in rain condition and has to be compensated by high gain antennas or backup low frequency links with reduced capacity. The availability of tens of Watts of transmission power would allow higher spectral efficiency and long range, and the use of lower gain antennas for multibeam and area coverage. Millimeter wave Traveling Wave Tubes (TWTs) are gaining inter- est due to their higher power, more than one order of magnitude, in comparison to solid state devices. Helix TWTs, typically built at microwaves, are very arduous to fabricate due to the extremely small diameter of an helix at E-band. This paper reports the design and fabrication of the first ever E- band TWT based on a full metal slow wave structure, the double corrugated waveguide. The TWT is designed to provide about 70 W power and more than 35 dB gain in the 71 - 76 GHz band. A test TWT using a single section interaction structure is in the final assembly phase as proof of concept with about 2 W output power. The E-band TWT performance will open new perspectives in the availability of long range wireless links with multi-Gb/s data rate needed for enabling 5G and 6G new networks.

28. Half-Height Pin Gap Waveguide-Based Slow-Wave Structure for Millimeter Wave Traveling-Wave Tubes

Abstract

The design of a W -band traveling-wave tube (TWT) power amplifier based on a groove gap waveguide (GW) slow-wave circuit is presented in this article. The technology of GW is analyzed to aid the design of electromagnetic band gap-based slow-wave structures (SWSs) in the upper millimeter wave range of the spectrum while alleviating some of the typical fabrication challenges at these frequencies. The results of particle-in-cell (PIC) simulations numerically demonstrate a 10-GHz instantaneous 3-dB bandwidth in the range 89–99 GHz with a minimum power gain of 25 dB. A prototype of the complete SWS is manufactured via computer numerical control (CNC) machining and measured to verify the cold simulation results. Machining tolerances and surface roughness are also investigated. The design approach via groove GW is flexible and can be extended to alternative rectangular waveguide-based SWSs.

Abstract

E-band (71 - 76 GHz, 81 - 86 GHz) front ends are commercially available for point to point links with a few gigabit per second (Gb/s) for range of about one kilometer at full capacity. The transmission power of E-band front ends is produced by solid state amplifiers, typically not higher than a few Watts. For this reason E-band front ends have to use large diameter antennas to ensure the required EIRP (Equivalent, Isotropically Radiated Power). The availability of power in the range of tens of Watts would enable long links with high signal to noise ratio for high modulation schemes and tens of Gb/s capacity. An E-band (71 - 76 GHz) traveling wave amplifiers has been designed to enhance the capacity of future E-band links. This paper discuss the first version of the TWT based on a single section double corrugated waveguide slow wave structure. This first prototype of E-band TWT will be preparatory to the fabrication of a more complex 70 W two sections E-band TWT.

Abstract

OBJECTIVES: Colorectal cancer (CRC) screening rates continue to be low among safety-net populations. We sought to elucidate the impact of social determinants of health (SDOH) on the noncompletion of fecal immunochemical tests (FITs) and colonoscopies at the Providence Community Health Centers (PCHC). STUDY DESIGN: This was a retrospective cohort review of PCHC patients with associated SDOH profiles between December 1, 2018, and December 1, 2019. METHODS: We analyzed fulfilled and unfulfilled CRC screening orders (FITs and colonoscopies) and examined associations and odds ratios between order noncompletion and the presence of SDOH variables. The study sample consisted of a total of 517 orders (fulfilled and unfulfilled; FIT, n = 348; colonoscopy, n = 169). RESULTS: FITs were completed more often than colonoscopies (81.3% vs 65.7%, respectively; P < .001). Colonoscopy noncompletion was associated with patientreported social determinants of "housing insecurity/ homelessness" (P = .0083) and "living conditions" (P = .048) and staff-reported "behavioral health problem" (P = .048). The presence of housing insecurity/homelessness increased the likelihood of an unfulfilled colonoscopy order (odds ratio, 7.5; 95% CI, 1.3-75.0). Patients who reported any SDOH need had a statistically significant increase in colonoscopy noncompletion (P = .0022), whereas FIT noncompletion was not associated with the presence of SDOH needs (P = .81).
