Abstract Background Gastro-tracheobronchial fistula after esophagectomy is a rare but life-threatening complication associated with high mortality. Several authors reported postoperative management of tracheobronchial fistula. However, treatment is demanding and challenging, and the strategy is still controversial. Case presentation A 64-year-old man underwent thoracoscopic esophagectomy with two-field lymph node dissection and gastric conduit reconstruction by an intrathoracic anastomosis for esophageal cancer at a local hospital in June 2013. After surgery, a gastro-tracheal fistula and a gastro-bronchial fistula of the left main bronchus were diagnosed, and the patient was referred to our hospital for the management of the gastro-tracheobronchial fistula. CT and bronchoscopy and esophagogastroduodenoscopy performed at our hospital revealed that the gastro-bronchial fistula of the left main bronchus was cured by packing with the omentum from the gastric conduit and the gastro-tracheal fistula located 3 cm above the carina remained open. We concluded that the fistula would not resolve without further surgical procedure. However, such an operation was expected to be difficult and to need much time due to severe adhesion among the gastric conduit and/or trachea, bronchus, lung, and chest wall. Therefore, a two-stage operation was planned for safety and outcome certainty. The first operation was performed to close the fistula in October 2013. The gastric conduit was separated from the trachea and resected; then, the fistula was sutured and covered by intercostal muscle and latissimus dorsi muscle flaps. A month after the first operation, reconstruction with pedunculated jejunum was performed via the percutaneous route. The patient’s postoperative course was uneventful. Conclusion If the omentum is not observed between the gastric conduit and the tracheobronchus when a gastro-tracheobronchial fistula occurs after esophagectomy, surgeons should perform surgical treatment because conservative treatment is unlikely to cure. During surgery, the use of two types of muscle flaps, such as the intercostal muscle and the latissimus dorsi muscle flaps, is helpful for the closure of gastro-tracheobronchial fistulas.

Background: Benign gastric tube-tracheobronchial fistula after esophagectomy for esophageal cancer is a rare life-threatening complication observed after gastric tube reconstruction via the posterior mediastinal route. However, the literature for this complication is limited, comprising only a few case reports; therefore, its treatment strategy remains unclear. Here, we have summarized such cases that were managed at our institution and have reviewed previous literature in order to design a relevant treatment strategy for this complication. Methods: We have included nine cases of benign gastric tube-tracheobronchial fistula after esophagectomy for esophageal cancer that were encountered during 1993-2011, with their clinical features, treatment methods, and outcomes. In addition, 37 reports published during 1990-2012 from the PubMed database comprising 61 such cases have been reviewed. A total of 70 patients (published cases and our cases) were analyzed with regard to pathogenesis, treatment methods, and outcomes. Results: The fistula type was found to determine the fistula site. Surgical treatment had the highest recovery rate of any of the types of benign gastric tube-tracheobronchial fistula after esophagectomy for esophageal cancer; further, approximately 30 % of the patients who initially received conservative or endoscopic treatment required conversion to operative treatment. Cases wherein leakage occurred were more frequently managed by conservative and endoscopic treatments than other types of such fistulas, with a good recovery rate. Conclusions: We conclude that the primary treatment for benign gastric tube-tracheobronchial fistula after esophagectomy for esophageal cancer is surgery based on the fistula type and site. SUMMARY Gastro-tracheobronchial fistula (GTF) is a rare but life-threatening complication specifically observed after esophagectomy and reconstruction using posterior mediastinal gastric tube. Ten cases of GTF were encountered in three hospitals in 2000-2009. Their clinicopathological, surgical, and postoperative care are summarized, together with a review of previously reported cases. GTF was classified as anastomotic leakage ( n= 5), gastric necrosis ( n= 4), and gastric ulcer type ( n= 1). The anastomotic leakage type appeared about 2 weeks (postoperative day [POD]: 8-35) after esophagectomy, was located in the cervical or higher thoracic trachea. Breathing and pneumonia were controlled by tracheal tube placed in the distal of fistula. The gastric necrosis type was noted in patients who developed necrosis of the upper part of the gastric tube and abscess formation behind the tracheal wall, at POD 20-36 around the carina, the site of pronounced ischemia. Due to the large fistula around the carina, emergency surgery with muscle patch repair was frequently required for the control of aspiration pneumonia. Patients of the gastric ulcer type had peptic ulcer in the lesser curvature of the gastric tube, which perforated into the right bronchus long after surgery (POD 630). With respect to tracheobronchial factors, preoperative chemoradiation (three cases) and pre-tracheal node dissection (three cases) tended to increase the risk of GTF. Closure of GTF by surgery (muscle patch repair) was successful in four cases and by nonsurgical treatment in three cases. In one case, stable oral intake was achieved by bypass operation without closure of GTF. Hospital death occurred in three cases. Understanding the pathogenesis and treatment options of GTF is important for surgeons who deal with esophageal cancer. [ABSTRACT FROM AUTHOR]
