6. Multidimensional assessment of social cognition using non-immersive virtual reality in dementia with Lewy bodies and Alzheimer's disease: an exploratory study.

Abstract

Introduction: Few studies have focused on social cognition in dementia with Lewy bodies (DLB), even though some brain structures being well known as underlying social cognitive processes are directly impacted in this disease. Furthermore, social cognition processes have been mostly studied independently using evaluations with poor ecological validity. We aimed at studying the ability of a new naturalistic and multidimensional social cognition task to reveal impairments in DLB patients. We chose to compare the profile of these patients with that of Alzheimer's disease (AD) patients, for which social cognition is better preserved. Method: Fifteen patients (DLB: n = 7; AD: n = 8) and 28 healthy controls underwent the REALSoCog task. They encountered several social situations (e.g. control versus transgressions) in a non-immersive virtual city environment allowing the assessment of moral cognition, cognitive and affective theory of mind (ToM), emotional empathy and behavioral intentions. Results: The main results showed (i) a lower ability to detect transgressions in DLB patients, particularly conventional ones, whereas moral cognition seemed better preserved in AD patients; (ii) a cognitive ToM impairment in both DLB and AD patients, while affective ToM is impaired only in DLB patients; (iii) a decreased emotional empathy specifically observed in DLB patients; (iv) more inappropriate behavioral intentions, mainly in DLB patients, but also in some AD patients. Conclusions: This study suggests the feasibility and potential interest of the REALSoCog task in revealing social cognition deficits, particularly for DLB patients by showing different social patterns as compared to AD patients. These results offer interesting clinical perspectives to develop more naturalistic tasks in such populations and for clinical differential diagnosis. Abstract

Vagally-mediated heart rate variability (vmHRV) is the natural fluctuation of the heartbeat underlying the regulatory interaction between the brain and the heart which is evoked by parasympathetic influences of the autonomous nervous system. The neurovisceral integration model theorizes that cardiac and cognitive regulatory mechanisms are linked as they recruit similar brain networks. This theory has been supported by numerous studies demonstrating a robust link between vmHRV and components of cognitive control such as inhibition and working memory. Preliminary findings indicate that vmHRV is also tied to cognitive control over memory. However, previous studies concentrated almost entirely on correlation analyses based on resting-state vmHRV measures. Little is known about how vmHRV assessed to during task execution relates to task performance. Moreover, to the best of our knowledge, no study has so far proposed a design to test the causal relationship between vmHRV and cognitive control by manipulating vmHRV. The latter can be non-invasively achieved by using biofeedback. In this line, subjects exercise self-regulation, typically by applying slow breathing techniques, in order to maximize vmHRV. Previous studies demonstrated that a short training induction can cause effects that last shortly after training. In this study, we propose a novel paradigm based on HRV biofeedback to investigate the link between vmHRV and cognitive control over memory through manipulation of vmHRV. Cognitive control over memory will be assessed using an adapted false memory paradigm. The study pursues four objectives: Correlation between vmHRV and cognitive control 1) Corroboration of the hypothesis that cognitive control is related with vmHRV at rest and during cognitive processing by replicating and extending previous results. HRV biofeedback stimulation Physiological analysis: 2) Feasibility check whether short-duration HRV biofeedback is suitable to increase vmHRV during subsequent memory processing (i.e., encoding, recognition). Behavioral analysis: 3) Test whether short-duration HRV biofeedback improves cognitive control over memory during memory processing. Psychophysiological analysis: 4) Investigate whether changes in vmHRV mediate changes in cognitive control over memory.

Recent theories in the field of embodied cognition have pointed out the role of the body for episodic memory, i.e. the memory for personally experienced events. Although virtual embodiment has been used traditionally to investigate the different components of bodily self, it provides great advantages to manipulate the whole embodied experience. In the current study, we manipulated three different levels of virtual embodiment (“full embodiment”, “medium embodiment”, and “low embodiment”). All participants were asked to navigate in three different virtual cities and memorize all the events that they encountered within each environment. We evaluated the effect of different level of embodiment on the main feature of the recall and recognition (i.e., what events have occurred) and sense of presence. Data emerge with interesting consequences on embodied cognition hypothesis. Accordingly, findings are discussed giving an innovative view of virtual reality as an embodied tool able to influence cognitive processes such as episodic memory.
