2. Assessing the Impact of the European Resilience Curriculum in Preschool, Early and Late Primary School Children

- Abstract
Resilience is an individual's ability to adapt successfully to and persevere during and after significant challenges. Resilience programmes based on a socioemotional learning approach have been associated with an increase in protextive factors (e.g., prosocial competencies), improvements in physical and mental health, and a decrease in internalised and externalised symptoms. The present study aimed to evaluate the impact of the RESCUR curriculum implemented in Portuguese schools on students' academic, behavioural, and socioemotional outcomes, based on child and teacher reports. Participants included 1,084 children (53.2% male) aged 3-15 (M = 7.24, SD = 2.31). A quasi-experimental study compared outcomes for an experimental intervention group (AIG) with a waiting list control group (WG). The results showed the RESCUR programme decreased mental health difficulties while increasing both prosocial behaviours and well-being. In addition, academic performance increased for those in preschool after implementation. Both teachers and children consistently reported positive behavioural changes in resilience-related competencies after implementing RESCUR. Our findings contribute to the recent research on the potential of RESCUR to address key socioemotional competencies and improve relevant protextive factors. Study limitations and future recommendations are addressed.

6. Are Healthcare Organizations Healthy Work Ecosystems? Health and Well-Being of Health Professionals.

Background: Health professionals are at high risk of poor mental health and well-being. Faced with this challenge, healthcare organizations must be healthy and safe work environments. Objectives: This study aims to take an in-depth, systemic look at whether healthcare organizations are healthy workplaces. Methods: The study involved 2190 participants aged between 19 and 71 (M = 44.73, SD = 10.29) and data were collected in 12 public hospitals between November 2021 and December 2023. The study used the Ecosystems of Healthy Workplaces instrument, which consists of a total of 62 items organized into nine dimensions based on the Healthy Workplaces model proposed by the World Health Organization. Results: Most dimensions revealed a moderate risk in terms of whether they were healthy work environments, while the dimension that revealed a high risk was related to psychosocial risks at work in relation to well-being and mental health. A total of 87% of the professionals reported at least one symptom of burnout and 61.4% reported having all three symptoms of burnout. Additionally, 25.4% reported having been victims of harassment at work. When comparing the groups, we identified that the higher risk groups were namely women, generation Z and X professionals, doctors (compared to the different professional groups under analysis such as nurses, operational assistants, psychologists, administrators, senior technicians, and managers), professionals with chronic illnesses, and those who reported harassment at work. Conclusions: We conclude that the work environment must be understood ecologically, by analyzing the different systems and their relationships. Health literacy (HL) is an essential health determinant that could encourage the adoption of individual and community protective behaviours that contribute to public health. This study aims to analyse the associations between HL and medication HL (MHL) in 4015 adolescents from the Health Behaviour in School‐aged Children (HBSC/WHO). Data were collected from the HBSC/WHO questionnaire. Descriptive statistics, bivariate analysis, and multinomial logistic regression were performed. 62.4% of participants have a moderate level of HL and 29.1% have a high level of HL. The youngest boys, and those who have a low level of MHL in the different dimensions presented (expiration date, recycling, side effects, illegal sale and safety), have a low level of HL. The oldest girls have a moderate level of HL. Also, girls with a high level of MHL, reported a high level of HL. MHL, namely, the expiration date, recycling, side effects and safety dimensions in adolescents is significantly related to and positively associated with HL. The results will enable us to highlight to family professionals and public policies the importance of HL and MHL promotion in adolescents. [ABSTRACT FROM AUTHOR]

The relation between positive youth development and stress, anxiety and depression was studied considering the mediator role of psychological symptoms and life satisfaction. A total of 3109 students included in the "Psychological Health and Well-being" study of the School Observatory participated, including 1618 females and 1491 males aged between 11 and 18 years old (M = 14.45; SD = 1.88), belonging to different school groups in different regions of the country. Adolescents with higher stress, depression and anxiety levels reported lower levels of competence, confidence and connection, as well as more psychological symptoms and lower life satisfaction. The results also showed that adolescents with higher levels of competence, confidence and connection reported less psychological symptoms and more life satisfaction. Two separate mediation analyses were performed to analyse the role of mental distress and life satisfaction on the relationship between positive youth development indicators and stress, anxiety and depression. These analyses confirmed the predicted relationships and partial mediations between mental distress and life satisfaction. These results should be taken into account in public policies concerning young people's health and education that should involve both targeted and indicated prevention strategies, including school and community-based interventions, in order to be effective. - Abstract
The hospital health care professionals are the front-line fighting COVID-19 considering they are responsible for all the care provided to patients. The purpose of this study was to determine the impact of COVID-19 at the hospital management level and, also, to understand how psychosocial environment, and satisfaction of Health Professionals were affected. A case study was performed in a Portuguese Hospital. Data were collected at one hospital under study at two different occasions: the first before the pandemic (November 2019) and the second almost two years after the pandemic started (November 2021). Regarding data collection, 37.0% of participants responded in the occasion 1 (n = 296) and 63.0% responded in the occasion 2 (n = 503). The instrument for the Assessment of Global Management of Health Organizations (AGMHO) consists of 39 items organized into six dimensions (Gaspar et al. in J Occup Environ Med 63: 581–587, 2021). Comparing timings pre and during pandemic COVID-19, it was found that the participants in the pre-COVID-19 era showed stronger organizational culture, higher quality of life, better psychosocial environment regarding content/leadership and higher job satisfaction when compared to the participants during pandemic COVID-19. On the other hand, participants in the second occasion were found to have higher psychosocial risks related to mental health when compared to participants in the pre-COVID-19 phase. We conclude that the professionals’ perception of the different dimensions of the health organization worsened after 2 years of the pandemic. With special focus on psychosocial risks at work and relationship with leadership.

24. Leisure Time Activities and Subjective Happiness in Early Adolescents from Three Ibero-American Countries: The Cases of Brazil, Chile and Spain

(1) Background: The aim of the present study is to analyze subjective happiness in relation to leisure time in 10-year-old boys and girls from Brazil, Chile and Spain and to determine which leisure time activity has a greater effect on their subjective happiness and whether there are differences according to gender. (2) Methods: Data from the third wave of the Children’s Worlds Project was used, which was collected by administering self-report measures to representative samples in each country. The sample was composed of 4008 early adolescents from Brazil (22.1%, n = 886), Chile (22.8%, n = 913) and Spain (55.1%, n = 2209). The mean age of the sample was 10.17 years (SD = 0.57), and 51.7% were girls. (3) Results: In terms of subjective happiness, moderate to high means were observed, with higher scores in boys and the Spanish sample. The results showed some differences in leisure time activities among children from Brazil, Chile and Spain. Furthermore, the results highlighted the importance of relaxation time with the family to promote happiness in pre-adolescence across genders and countries. Additionally, playing outdoors and doing homework were also found to be protective factors for subjective happiness. (4) Conclusions: In Brazil, Chile and Spain the most consistent result was the positive effect of spending time relaxing with the family on subjective happiness., Children’s Worlds Project was supported by the Jacobs Foundation.

In 2020, female breast cancer was the most commonly diagnosed cancer worldwide, representing the type of cancer with the highest incidence among women and the second most common cause of cancer death among women in all OECD countries. The conventional measures addressing the burden of breast cancer by measuring mortality, incidence, and survival do not entirely reflect the quality of life and patients experience when receiving breast cancer care. The main objective of this study is to capture patient-reported outcomes and experiences in women with breast cancer in Portugal using methods developed for international benchmarking purposes, such as the OECD Patient-reported Indicators Surveys. The study included 378 women with breast cancer, with the age distribution being 19.8% aged 15 to 49 years and 80.2% aged 50 years and over. The data collection procedure and analysis followed the “OECD Breast Cancer Patient Reported Outcomes Working Group” protocol, allowing subsequent comparability with data from other OECD member countries. Most women were satisfied with the treatment outcome regarding the shape of their lumpectomy breast when wearing a bra (96.1%) and with the equal size of both breasts (78.3%). Findings on the WHO QOL-BREF showed that women manifest a lower score in well-being when compared with the general population or populations living with chronic diseases. This study shows the feasibility of implementing and using patient-reported metrics (PROM and PREM) in breast cancer services in Portugal. Measuring PROMs and PREMs from Portuguese women receiving breast cancer care provides insightful evidence into the quality and value of cancer care. publishersversion published

- Abstract
La participación de los estudiantes en debates, búsqueda de información, etc., sobre asuntos sociopolíticos, la eficacia política interna y las expectativas sociopolíticas de participación son indicadores de compromiso cívico adolescente. El objetivo de este estudio fue analizar la influencia de la Participación comunicativa sociopolítica de los adolescentes de España en las Expectativas de participación sociopolítica, de forma directa y a través de la Eficacia política interna según el género. Esta investigación basada en el proyecto “Barómetro OPINA” fué transversal y participaron 4711 adolescentes con edades comprendidas entre los 13 y los 18 años. Los instrumentos utilizados fueron tres escalas que medían los indicadores de compromiso cívico propuestos. Se realizaron comparaciones de media, un modelo de ecuaciones estructurales y un análisis de invarianza según el sexo. Los resultados obtenidos mostraron diferencias significativas en las puntuaciones de Expectativas de participación sociopolítica y Eficacia política interna. - Abstract
Cardiovascular diseases (CVDs) are the number one cause of death globally and are partially due to the inability to control modifiable lifestyle risk factors. The aim of this study was to analyze the profiles of adolescents from seven Mediterranean countries (Greece, Israel, Italy, Macedonia, Malta, Portugal, Spain) according to their modifiable lifestyle risk factors for CVD (overweight/obesity, physical activity, smoking, alcohol consumption). The sample consisted of 26,110 adolescents (52.3% girls) aged 11, 13, and 15 years who participated in the Health Behavior in School-aged Children (HBSC) survey in 2018 across the seven countries. Sociodemographic characteristics (sex, age, country of residence, socioeconomic status) and CVD modifiable lifestyle risk factors (overweight/obesity, physical activity, smoking, alcohol consumption) were recorded. A two-step cluster analysis, one-way analysis of variance, and chi-square test were performed. Four different cluster groups were identified: two low-risk groups (64.46%), with risk among those with low physical activity levels; moderate-risk group (14.83%), with two risk factors (unhealthy weight and low physical activity level); and a high-risk group (20.7%), which presented risk in all modifiable lifestyle risk factors. Older adolescents reported a higher likelihood of being in the high-risk group. Given that the adolescence period constitutes an important time for interventions aimed at CVD prevention, identifying profiles of moderate- and high-risk adolescents is crucial.

El objetivo de este estudio fue analizar la influencia de la Participación comunicativa sociopolítica de los adolescentes de España en las Expectativas de participación sociopolítica, de forma directa y a través de la Eficacia política interna según el género. Esta investigación basada en el proyecto “Barómetro OPINA” fué transversal y participaron 4711 adolescentes con edades comprendidas entre los 13 y los 18 años. Los instrumentos utilizados fueron tres escalas que medían los indicadores de compromiso cívico propuestos. Se realizaron comparaciones de media, un modelo de ecuaciones estructurales y un análisis de invarianza según el sexo. Los resultados obtenidos mostraron diferencias significativas en las puntuaciones de Expectativas de participación sociopolítica y Eficacia política interna. O presente trabalho tem como objetivo principal a compreensão dos sintomas depressivos na adolescência durante a pandemia COVID-19 numa perspetiva biopsicossocial. Participaram no estudo 826 adolescentes residentes na área geográfica da Câmara Municipal de Famalicão (Portugal). Os resultados revelaram que dos adolescentes em estudo, 30,7 % referiu ter sintomas depressivos, 34,9% sentiu tristeza tão grande que parece não aguentar. Destaca-se que 80,6% sentiu preocupações e 86,2% tem uma perceção positiva da sua saúde. Ter sintomas depressivos está negativamente associado a ter competências socioemocionais, com perceção de bem-estar e, com o conjunto de sintomas físicos e psicológicos. Por outro lado, ter sintomas depressivos está positivamente associado a sentir tristeza tão grande que parece não aguentar e preocupações, ter-se autolesionado e com ter as expectativas futuras afetadas pela COVID-19., The main objective of this study is to understand the depressive symptoms in adolescence during the COVID-19 pandemic from a bio-psychosocial perspective. A total of 826 adolescents with a mean age of 14.30±2.40 years participated in the study, from Câmara Municipal de Famalicão (Portugal). The results reveal of the adolescents under study, 30.7 % reported having depressive symptoms, 34.9% felt sadness so great that it does not seem to hold. It is noteworthy that 80.6% felt worried and 86.2% have a positive perception of their health. Depressive symptoms were negatively associated with having socio-emotional skills, with perceived well-being and with the set of physical and psychological symptoms. On the other hand, depressive symptoms were positively associated with feeling so sad that they cannot seem to cope and worry, having self-injured and having future expectations affected by COVID-19.

Tese de doutoramento em Gestão, Universidade Lusíada de Lisboa, 2020 Exame público realizado em 22 de Outubro de 2021 As organizações de saúde do Sistema Nacional de Saúde (SNS) são complexas e estão em constante interação dinâmica com múltiplos fatores, que incluem fatores políticos, ambientais, societais, legais, organizacionais, recursos humanos, doentes e outros stakeholders. A presente investigação pretendeu estudar três organizações do SNS e através destas identificar e caracterizar os fatores das organizações de saúde e os fatores relacionados com os profissionais, e compreender a sua influência nos resultados de desempenho económico e financeiro e nos resultados relacionados com os próprios profissionais e com os doentes. O estudo foi desenvolvido em três fases e utilizou uma metodologia mista, recorrendo a métodos quantitativos e qualitativos, triangulando e integrando informação recolhida através de diversos métodos e multi-informantes. Uma primeira fase recorreu a métodos qualitativos, entrevistas a 15 especialistas na área da gestão e saúde pública. Esta fase teve como objetivo a obtenção de informação por parte dos participantes acerca dos fatores relacionados com as organizações, com os profissionais e com os doentes a incluir no estudo quantitativo, assim como, a recolha de informação sobre questões metodológicas a implementar na fase subsequente do estudo. Numa segunda fase, foi realizado um estudo quantitativo onde foram recolhidos dados através de questionários junto de diversas fontes para melhor compreender e caracterizar os fatores em estudo relacionados com as organizações, com os profissionais e com os doentes (32 administradores hospitalares e diretores clínicos, 470 profissionais de saúde e de 768 doentes). Para testar o modelo integrado de avaliação das organizações de saúde recorreu-se à path-analysis. Numa terceira fase a informação sistematizada dos resultados obtidos e o modelo compreensivo identificado foram enviados para os especialistas de modo a obter discussão da informação recolhida, com o objetivo de elaborar recomendações para a gestão das organizações de saúde e para as políticas de gestão em saúde, recorrendo ao método delphi junto de 45 especialistas na área da gestão e saúde pública. Os resultados obtidos podem ser organizados em três grupos: 1) resultados aprofundados acerca das características da cultura organizacional das organizações de saúde, da qualidade de vida dos profissionais de saúde, dos riscos psicossociais do trabalho, da gestão de desempenho, da satisfação dos profissionais, da satisfação dos doentes e dos resultados de desempenho económico e financeiro nas três organizações de saúde envolvidas; 2) modelo integrado que inclui a força e sentido das inter-relações entre inputs, processos e outcomes nas organizações de saúde; 3) construção de uma carta de consenso acerca dos fatores que influenciam os resultados e a qualidade das organizações de saúde, sugestões de melhoria da qualidade das organizações de saúde e passos para a mudança. Os resultados revelam que a Cultura Organizacional (CO) apresenta uma relação positiva com a Qualidade de Vida (QV) dos Profissionais e a Gestão do Desempenho (GD) dos Profissionais e uma relação negativa com os Riscos Psicossociais do Trabalho (RPT). Verifica-se, ainda, relação da CO com a Satisfação dos Doentes (SD), com a Satisfação dos Profissionais (SP) e resultados de desempenho económico e financeiros (RDEF). Na relação entre os Processos e os Resultados, encontram-se relações significativas entre a GD e a SP e os RPT e a SP. Ao nível dos Resultados verifica-se uma relação significativa entre os RDEF e a SD. Considera-se que o presente estudo contribui para um conhecimento mais aprofundado dos fatores que influenciam a qualidade das organizações de saúde e respetivos resultados e produz recomendações para que as organizações de saúde façam face aos atuais desafios. Foi utilizada no estudo uma perspetiva sistémica, multidimensional e integradora para caracterizar e compreender os fatores, atores e respetivas relações e influências nas organizações de saúde do SNS, ao nível político, organizacional, dos recursos humanos, dos resultados associados aos profissionais, aos doentes e de desempenho económico e financeiro. Como produto obtivemos um modelo compreensivo de diagnóstico dos fatores que influenciam os resultados nas organizações de saúde e método de avaliação integrado que permite a avaliação e monitorização regular das organizações de saúde do SNS, nomeadamente em caso de implementação das medidas de melhoria e consequentemente apoiar um processo de governance baseado na evidência. São, ainda, apresentadas e discutidas recomendações e pistas para a ação para os gestores e para a melhoria contínua e sustentável das organizações de saúde do SNS. National Health System (NHS) health organizations are complex and in constant dynamic interaction with multiple factors, including political, environmental, societal, legal, organizational, human resources, patients, and other stakeholders. This research aimed to study three NHS organizations and through them identify and characterize the factors of health organizations and factors related to professionals and understand their influence on economic and financial performance results and results related to the professionals and to the patients. The study was developed in three phases and used a mixed methodology, using quantitative and qualitative methods, triangulating, and integrating information gathered through different methods and multi-informants. A first phase resorted to qualitative methods, interviews with 15 specialists in the field of management and public health. This phase aimed at obtaining information from experts about factors related to organizations, professionals, and patients to be included in the quantitative study, as well as collecting information on methodological issues to be implemented in the subsequent phase of the study. In a second phase, a quantitative study was conducted where data were collected through questionnaires from various sources to better understand and characterize the factors in study related to organizations, professionals, and patients (32 hospital administrators and clinical directors, 470 health professionals and 768 patients). To test the integrated evaluation model of health organizations, path-analysis was used. In a third phase the systematized information of the results obtained, and the comprehensive model identified were sent to the specialists to obtain a discussion of the information collected, with the objective of elaborating recommendations for the management of health organizations and health management policies, using the delphi method with 45 specialists in the area of management and public health. The results obtained can be organized in three groups: 1) in-depth results about the characteristics of the organizational culture of health organizations, quality of life of health professionals, psychosocial risks of work, performance management, professional satisfaction, patient satisfaction and economic and financial performance results in the three health organizations involved; 2) integrated model that includes the strength and sense of the interrelationships among inputs, processes and outcomes in health organizations; 3) construction of a consensus letter about factors that influence the results and quality of health organizations, suggestions for improving the quality of health organizations and road maps to the future. The results reveal that the Organizational Culture (OC) presents a positive relationship with the Professional’s Quality of Life (QL) and the Performance Management (PM) of the Professionals and a negative relationship with the Psychosocial Work Risks (PWR). There is also a relationship between the OC and Patient Satisfaction (PS), Professional Satisfaction (PfS) and economic and financial performance results (EFPR). In the relationship between Processes and Results, there are significant relationships between PM and PfS and PWR and PfS. In terms of Results, there is a significant relationship between the EFPR and PS. It is considered that this study contributes to a deeper knowledge of the factors that influence the quality of health organizations and their results and produces recommendations for health organizations to face the current challenges. A systemic, multidimensional and integrating perspective was used in the study to characterize and understand the factors, stakeholders and respective relationships and influences in NHS health organizations, at the political, organizational, human resources, results associated to professionals, patients and economic and financial performance levels. As a product we have obtained a comprehensive diagnosis model of the factors that influence the results in health organizations and an integrated evaluation method that allows the regular evaluation and monitoring of NHS health organizations, namely in case of implementation of improvement measures and consequently supporting an evidence-based governance process. Recommendations and paths for action for managers and for the continuous and sustainable improvement of NHS health organizations are also presented and discussed.

As práticas e políticas de gestão de recursos humanos (GRH) tem uma forte influência nos comportamentos dos profissionais da organização, assim como no seu bem-estar, gestão de desempenho e resultados. Uma abordagem de GRH focada na promoção de qualidade de vida dos profissionais e de espaços de trabalho saudáveis (Healthy Workplace) está associada a uma melhor satisfação com o trabalho, menos riscos psicossociais no trabalho, e consequentemente melhores resultados, nomeadamente um melhor desempenho dos profissionais e satisfação por parte dos doentes. A prestação de serviços de saúde de qualidade passa necessariamente por uma gestão de RH adequada. 44. Avaliação do potencial terapêutico de moduladores epigenéticos na leucemia linfocítica crónica

Dissertação de Mestrado em Química Medicinal apresentada à Faculdade de Ciências e Tecnologia A leucemia linfocítica crónica (LLC) é uma neoplasia linfoproliferativa de células B maduras, caracterizada pela acumulação de linfócitos B neoplásicos no sangue, na medula óssea e nos tecidos linfoides secundários. Na LLC estão presentes diversas alterações genéticas como alterações cromossómicas, mutações, mas também epigenéticas como alterações na expressão de miRNAs, modificações da acetilação das histonas e da metilação do DNA. As alterações cromossómicas mais frequentes incluem a deleção 13q, a deleção 17p, a deleção 11q e a trissomia do cromossoma 12. Além disso, os miRNAs 15a e 16-1 podem estar ausentes, por exemplo, na deleção 13q, ou ser regulados negativamente ou alterados em aproximadamente 60% dos casos de LLC e são disfuncionais em casos de LLC familiar. Outras alterações epigenéticas incluem a alteração da metilação do DNA e a modificação das histonas, sendo que esta metilação anómala poderá corresponder ao evento inicial desta patologia. As modificações nas histonas determinam a natureza da cromatina sendo estas alterações as responsáveis pelo empacotamento da cromatina e o acesso à informação genética. Como consequência, da desacetilação das histonas e da hipermetilação do DNA, ocorre o silenciamento de diversos genes com consequente aumento de viabilidade e resistência à apoptose características das células cancerígenas da LLC. Assim, neste trabalho foi avaliado o potencial terapêutico de moduladores epigenéticos [agentes hipometilantes (azacitidina e decitabina) e inibidores das desacetilases das histonas (vorinostat e panobinostat)] numa linha celular de LLC. Neste sentido, a linha celular HG3 foi incubada durante 72h na ausência e na presença dos moduladores epigenéticos em monoterapia e em combinação terapêutica. Procedeu-se à avaliação da atividade metabólica recorrendo ao ensaio metabólico da resazurina. O ciclo celular foi analisado através de citometria de fluxo por marcação com iodeto de propídio (PI)/RNase. O tipo de morte celular foi avaliado também por citometria de fluxo por marcação com Anexina V e 7-Aminoactinomicina D (7-AAD) e por microscopia ótica, através da análise morfológica recorrendo à coloração de Giemsa. Na avaliação da atividade metabólica, os resultados demonstraram que no caso da azacitidina, do vorinostat e do panobinostat a diminuição da atividade metabólica é dependente do tempo e da dose de fármaco, enquanto que, no caso da decitabina a diminuição da atividade metabólica é dependente apenas do tempo. Neste ensaio, no tempo de incubação de 72h, o IC50 da azacitidina foi de 2,135 μM, da decitabina de 2,948 μM, do vorinostat de 0,4709 μM e do panobinostat foi de 5,486 nM. Além disso verificou-se que, a azacitidina, a decitabina e o panobinostat demonstram ter um efeito citotóxico, enquanto o vorinostat levou a um aumento não significativo da morte celular. Além do efeito citotóxico, a azacitidina revelou um efeito citostático com bloqueio das células em fase G0/G1. De um modo semelhante, a decitabina e o vorinostat induziram bloqueio do ciclo celular em fase G0/G1 e G2/M. Por outro lado, o panobinostat não induziu efeitos muito significativos a nível do ciclo celular. No caso das combinações terapêuticas, em todas ocorreu uma maior redução da atividade metabólica em comparação com a monoterapia, tendo sido observado um efeito sinérgico de adição. Nas células tratadas com combinações terapêuticas com decitabina observou-se uma maior percentagem de células em morte celular por apoptose, enquanto que, nas combinações com azacitidina não ocorreu um aumento significativo de morte celular. Todas as combinações terapêuticas induziram bloqueio do ciclo celular em fase G0/G1 e G2/M, revelando um efeito citostático. No entanto, a associação da azacitidina com vorinostat não revelou efeitos significativos no ciclo celular. Em suma, podemos concluir que os moduladores epigenéticos estudados em monoterapia ou em combinação podem constituir uma abordagem terapêutica para o tratamento da leucemia linfocítica crónica. The chronic lymphocytic leukemia (CLL) is a mature B cell lymphoproliferative neoplasm characterized by the accumulation of neoplastic B lymphocytes in the blood, bone marrow and secondary lymphoid tissues. In CLL, are present several genetic alterations, such as chromosomal alterations, mutations, and epigenetic alterations, such as alterations in the expression of miRNAs, changes in histone acetylation and DNA methylation. The most frequent chromosomal alterations include the 13q deletion, the 17p deletion, the 11q deletion and chromosome 12 trisomy. Furthermore, miRNAs 15a and 16-1 may be absent, for example, in the 13q deletion, or be down-regulated or altered in approximately 60% of CLL cases and are dysfunctional in familiar CLL cases. Other epigenetic alterations include the alteration of DNA methylation and the modification of histones, and this anomalous methylation may correspond to the initial event of this pathology. Histone modifications determine the nature of chromatin, and these alterations are responsible for chromatin packaging and access to genetic information. As a consequence of histone deacetylation and DNA hypermethylation, several genes are silenced, with a consequent increase in the viability and resistance to apoptosis, characteristic of CLL cancer cells. Thus, this work, evaluated the therapeutic potential of epigenetics modulators [hypomethylating agents (azacytidine and decitabine) and histone deacetylases inhibitors (vorinostat and panobinostat)] in a CLL cell line. In this sense, the HG3 cell line was incubated for 72 hours in the absence and presence of epigenetic modulators in monotherapy and in therapeutic combination. The metabolic activity was evaluated using the resazurin metabolic assay. The cell cycle was analyzed by flow cytometry by labeling with propidium iodide (PI)/RNase. The type of cell death was also evaluated by flow cytometry using labeling Annexin V and 7-Aminoactinomycin D (7-AAD) and by optical microscopy, through morphological analysis using Giemsa staining. In the evaluated of metabolic activity, the results demonstrated that, in the case of azacytidine, vorinostat and Panobinostat, the decrease in metabolic activity is time- and dose- dependent, while in the case of decitabine the decrease in metabolic activity is dependent only of time. In this assay, at an incubation time of 72 hours, the IC50 of azacytidine was 2.135 μM, decitabine 2.948 μM, vorinostat of 0.4709 μM and Panobinostat of 5.486 nM. Furthermore, azacytidine, decitabine and panobinostat were found to have a cytotoxic effect, whereas vorinostat led to a non-significant increase in cell death. In addition to the cytotoxic effect, azacytidine revealed a cytostatic effect with blocking of cells the G0/G1 phase. Similarly, decitabine and vorinostat induced cell cycle blockage in G0/G1 and G2/M phase. For the other side, panobinostat did not lead to very significant effects at the cell cycle level.In the case of therapeutic combinations, in all of them there was a greater reduction in metabolic activity compared to monotherapy, and a synergistic addition effect was observed. In cells treated with therapeutic combinations with decitabine, a higher percentage of apoptostic cells was observed, while in combinations with azacytidine there was not significant increase in cell death. All therapeutic combinations induced a arrest in G0/G1 and G2/M phases, revealing a cytostatic effect. However, the combination of azacytidine with vorinostat did not show significant effects on the cell cycle. In summary, we can conclude that epigenetic modulators in monotherapy or in combination can constitute a therapeutic approach for the treatment chronic lymphocytic leukemia.

Objective: The aim of this study was to evaluate sleep and awakening quality (SQ and AQ) during COVID-19 in a large and diversified population in order to identify significant associations and risks in terms of demography, health and health-related behaviors, sleep variables, mental health, and attitudes. Methods/Results:Online surveys were used for data collection, received from 5479 individuals from the general population, sleep disorder patients, and COVID-involved (medical doctors (MDs) and nurses) and COVID-affected professionals (teachers, psychologists, and dentists). SQ and AQ were worse in adults, females, and high-education subjects. Feeling worse, having economic problems, depression, anxiety, irritability, and a high Calamity Experience Check List (CECL) score during COVID were significantly associated with poor SQ and AQ. Shorter sleep duration, increased latency, poor nutrition, low physical activity, increased mobile and social network use, more negative and less positive attitudes and behaviors were associated with poor AQ. Conclusions: The SQ logistic regression showed gender, morbidities, CECL, and awakenings as relevant, whereas, for AQ, relevant variables further included age and physical activity. Aiming to have a high stress compliance, each individual should sleep well, have important control of their mood, practice positive behaviors while dismissing negative behaviors and attitudes, practice exercise, have adequate nutrition, and beware of technologies and dependences. publishersversion published
