Abstract

Genome-wide homozygosity mapping was conducted in one Dutch and one Israeli family affected by autosomal-recessive retinitis pigmentosa (arRP) to identify the RP association with C2ORF71, encoding a putative protein of 1,288 amino acids, specifically expressed in human retina. The truncating mutations in C2ORF71 identified in three unrelated families confirmed the involvement of this gene in the etiology of arRP.

Abstract

Several major eye diseases may be characterized by deficits in ocular blood flow. Maintaining adequate nutrient delivery to the choroid, retina, and optic nerve head may prevent cellular damage and loss, complementing efforts to provide neuroprotection. The ability to quantify deficits in ocular blood flow has increased in recent years with the introduction of several new techniques. 1) Scanning laser ophthalmoscopic fluorescein angiography allows the measurement of bulk retinal flow and macular capillary transit rates. 2) Scanning laser ophthalmoscopic indocyanine green angiography measures choroidal perfusion in selected areas near the optic nerve head and macula. 3) Confocal scanning laser Doppler flowmetry permits quantitation of retinal capillary perfusion. 4) Color Doppler ultrasound imaging measures flow velocities in the ophthalmic, central retinal, and short posterior ciliary arteries. While these techniques represent a major advance, significant issues regarding blood flow and ocular disease remain. First, the hemodynamic characteristics of the disease state need further definition. Second, the blood flow effects of current and future treatments must be established. Third, improvements in ocular blood flow techniques are required, both to increase anatomic precision and to allow non-invasive measurements during times of potential ischemic risk such as sleep. These developments could enhance prevention, diagnosis, and treatment of several major eye diseases.

Oren Tomkins, Hanna J GarzoziDepartment of Ophthalmology, Bnai Zion Medical Center, Haifa, IsraelAbstract: Corneal ectasia, a weakening of corneal integrity, occurs both due to acquired and congenital conditions such as keratoconus. It is a progressing condition that affects both visual acuity, and corneal stability. Various methods exist for correcting this impairment, however none address the inherit pathology, an increase laxity of the corneal stroma. Collagen crosslinking, a new, minimally invasive method, aims to strengthen the stroma by inducing cross links between neighboring collagen fibers. This method results in an increase in corneal tensile strength, with no medium term adverse effects on its normal architecture. Clinically, treated patients display improvement in both visual acuity and keratometric readings. This method may provide clinicians with easily accessible tools to stop the progression, and even correct visual deterioration due to corneal ectasia. Here we review the current information regarding this new method, as well as discuss its potential benefits and downfalls.Keywords: corneal cross-linking, corneal ectasia, keratoconus, stroma, cornea
