Contains fulltext : 305057.pdf (Publisher’s version ) (Closed access), People with diabetes often encounter stigma (ie, negative social judgments, stereotypes, prejudice), which can adversely affect emotional, mental, and physical health; self-care, access to optimal health care; and social and professional opportunities. To accelerate an end to diabetes stigma and discrimination, an international multidisciplinary expert panel (n=51 members, from 18 countries) conducted rapid reviews and participated in a three-round Delphi survey process. We achieved consensus on 25 statements of evidence and 24 statements of recommendations. The consensus is that diabetes stigma is driven primarily by blame, perceptions of burden or sickness, invisibility, and fear or disgust. On average, four in five adults with diabetes experience diabetes stigma and one in five experience discrimination (ie, unfair and prejudicial treatment) due to diabetes, such as in health care, education, and employment. Diabetes stigma and discrimination are harmful, unacceptable, unethical, and counterproductive. Collective leadership is needed to proactively challenge, and bring an end to, diabetes stigma and discrimination. Consequently, we achieved unanimous consensus on a pledge to end diabetes stigma and discrimination.

Gallstone ileus is a surgical emergency that commonly goes unnoticed due to its low incidence, a high diagnostic suspicion is necessary to carry out a timely approach and treatment. A bibliographic review was carried out on the diagnosis and treatment of this entity, to exemplify it, through the clinical case of a female patient who presented with an acute abdomen, undergoes exploratory laparotomy showing gallstone ileus, for which enterolithotomy is performed. There is a discrepancy in the literature regarding the initial surgical management of this pathology, especially if the enterolithotomy must be associated with resection of a biloenteric fistula., El íleo biliar es una emergencia quirúrgica que comúnmente pasa desapercibida debido a su baja incidencia, es necesaria una alta sospecha diagnóstica para realizar un abordaje y tratamiento oportuno. Se realizó una revisión bibliográfica sobre diagnóstico y tratamiento de esta entidad, para ejemplificarlo mediante caso clínico de un paciente femenino que se presenta con un cuadro de abdomen agudo, se somete a laparotomía exploradora evidenciando íleo biliar, por lo que se realiza enterolitotomía. Existe discrepancia en la literatura en cuanto al manejo quirúrgico inicial de esta patología, especialmente si la enterolitotomía se debe de asociar a resección de fístula biloenterica.

Sports Cardiology practice commonly involves the evaluation of athletes for genetically determined cardiac conditions that may predispose to malignant arrhythmias, heart failure, and sudden cardiac death. High-level exercise can lead to electrical and structural cardiac remodelling which mimics inherited cardiac conditions (ICCs). Differentiation between 'athlete's heart' and pathology can be challenging and often requires the whole armamentarium of available investigations. Genetic studies over the last 30 years have identified many of the genetic variants that underpin ICCs and technological advances have transformed genetic testing to a more readily available and affordable clinical tool which may aid diagnosis, management, and prognosis. The role of genetic testing in the evaluation and management of athletes with suspected cardiac conditions is often unclear beyond the context of specialist cardio-genetics centres. This document is aimed at physicians, nurses, and allied health professionals involved in the athlete's care. With the expanding role and availability of genetic testing in mind, this document was created to address the needs of the broader sports cardiology community, most of whom work outside specialized cardio-genetics centres, when faced with the evaluation and management of athletes with suspected ICC. The first part of the document provides an overview of basic terminology and principles and offers guidance on the appropriate use of genetic testing in the assessment of such athletes. It outlines key considerations when contemplating genetic testing, highlighting the potential benefits and pitfalls, and offers a roadmap to genetic testing. The second part of the document presents common clinical scenarios in Sports Cardiology practice, outlining the diagnostic, prognostic, and therapeutic implications of genetic testing, including impact on exercise recommendations.
