Abstract

Summary: Apart from serving as a Th1 lineage commitment regulator, transcription factor T-bet is also expressed in other immune cell types and thus orchestrates their functions. In case of B cells, more specifically, T-bet is responsible for their isotype switching to specific IgG sub-classes (IgG2a/c in mice and IgG1/3 in humans). In various autoimmune disorders, such as systemic lupus erythematosus and/or rheumatoid arthritis, subsets of T-bet expressing B cells, known as age-associated B cells (CD19+CD11c+CD21−T-bet+) and/or double-negative B cells (CD19+IgD−CD27−T-bet+), display an expansion and seem to drive disease pathogenesis. According to data, mostly derived from mice models of autoimmunity, the targeting of these specific B-cell populations is capable of ameliorating the general health status of the autoimmune subjects. Here, in this review article, we present a variety of therapeutic approaches for both mice and humans, suffering from an autoimmune disease, and we discuss the effects of each approach on T-bet+ B cells. In general, we highlight the importance of specifically targeting T-bet+ B cells for therapeutic interventions in autoimmunity. Targeting of ABCs/DN, in mice models of autoimmunity and/or humans suffering from autoimmune disorders, leads to diminished levels of T-bet expression in B cells and thus improves the general health status of the subjects. Abstract

Lupus nephritis (LN) is a major course of morbidity and mortality in patients with systemic lupus erythematosus (SLE), best managed by a multidisciplinary group. To this end, we gathered a group of rheumatologists, nephrologists and a nephropathologist to review current evidence regarding diagnosis and management of LN. In this consensus paper, we summarize the key points from this meeting and provide practice guidelines for the management of kidney involvement in SLE, in view of emerging new data concerning novel agents approved recently. Renal biopsy is indispensable for the management of LN. Yet, important pearls and pitfalls need to be considered regarding indications and interpretation, which are summarized in informative tables. In new-onset LN, experts agreed that, although belimumab may be added from disease onset, patients with moderate to severe proliferative nephritis (defined as: NIH activity index > 5 plus ≥ 1 of the following: (i) NIH chronicity index > 2, (ii) proteinuria > 3 g /24 h, and (iii) increase in serum creatinine > 20%) may be more likely to benefit the most. In all other patients who have already started standard-of-care treatment with either mycophenolate mofetil (MMF) or cyclophosphamide (CY), belimumab could be considered in cases with an inadequate clinical response by 3 months, or in cases that experience a nephritic flare following initial response, or have an inability to reduce the dose of glucocorticoids. In all circumstances, the drug should be given as add-on therapy, that is, in combination with a standard-of-care therapy (MMF or CY). Voclosporin could be considered for up to 3 years, in combination with MMF, in patients with heavy proteinuria (well above the nephrotic range), wherein a quick reduction of protein loss in urine is desirable to avoid the complications of the nephrotic syndrome, either as part of the initial regimen, or in cases of inadequate reduction of proteinuria with MMF. 46. Association Between Uric Acid and Worsening Peripheral Microangiopathy in Systemic Sclerosis

Abstract

Objective: The key element in the pathogenesis of systemic sclerosis (SSc) is microcirculatory changes in several vascular beds. Uric acid is associated with endothelial dysfunction and therefore, microvascular damage. The aim of this study was to examine the association between uric acid (UA) and peripheral microvascular involvement in patients with SSc.Methods: We included consecutive, consenting patients with SSc. Serum UA, urea and creatinine were measured, and glomerular filtration rate (GFR) was calculated with CKD-EPI. All participants underwent nailfold video-capillaroscopy (NVC) to evaluate the microcirculation.Results: A total of 64 patients (95.3% women) were included in the study. UA levels were significantly associated with the number of avascular areas (r = 0.290; p = 0.020), whereas no correlation was shown for the GFR (r = -0.065; p = 0.609). A significant trend of UA in the three capillaroscopic patterns was shown (3.90 +/- 1.52 vs. 4.15 +/- 0.98 vs. 5.38 +/- 2.26; for early, active, and late patterns respectively, p = 0.028). Multivariate analysis showed that male gender (beta = 3.049; 95% CI = 0.997-5.101) and UA (beta = 0.352; 95% CI = 0.117-0.588) were independently associated with the number of avascular areas.Conclusion: These data suggest that UA levels are significantly associated with the capillaroscopic patterns, reflecting a progressive microvasculopathy.

Abstract

Introduction: Venlafaxine is a prescription drug approved for the treatment of major depressive disorder, generalized anxiety disorder, panic disorder and social anxiety disorder. It is a serotonin and norepinephrine reuptake inhibitor and a weak dopamine reuptake inhibitor. The aim of the present study is to investigate the in vitro effect of venlafaxine on human genetic material, by estimating sensitive cytogenetic indices. Methods: Five venlafaxine solutions (A=15μg/ml, B=30μg/ml, C=45μg/ml, D=60μg/ml, E=75μg/ml) were added to cultures of peripheral blood lymphocytes of six healthy donors. After 72 hours of incubation, the cultured lymphocytes were plated on glass slides, stained with the Fluorescence plus Giemsa method and Sister Chromatid Exchanges (SCEs), a sensitive marker of genotoxicity, Proliferation Rate Index (PRI), a reliable marker of cytostatic activity and Mitotic Index (MI), a marker which shows precisely the ability of a cell to proliferate were measured with the optical microscope. Results: Result analysis revealed t: a) a statistically significant (p=0.001) dose-dependent increase of SCEs and b) a statistically significant (p=0.001) reduction of PRI and MI in all concentrations. Furthermore, a correlation was observed between a) SCE and PRI index variations, b) MI and SCE index variations and c) PRI and MI index variations. Conclusions: Venlafaxine exhibited dose-dependent cytogenetic activity in vitro, increasing SCE frequencies and diminishing PRI and MI levels in healthy human cultured lymphocytes. Venlafaxine as other antidepressants seems to affect human T lymphocytes by modifying epigenetic and DNA replication procedures. This may provide additional information about the mechanism of action of the drug. Abstract

We present the case of a 58-year-old woman with a renin secreting typical bronchopulmonary carcinoid. This patient showed hypotension, constipation and fatigue due to extensive hypokaliemia (K = 1.9 meq/L). Aldosterone (102.7 ng/100 mL) and renin (46 ng/mL) were excessively elevated at that time, but cortisol level was normal. Routine chest roentgenography and computed tomography revealed a nodular lesion in the upper left lung lobe, which was suspicious for a neurosecretory pulmonary tumor. The final diagnosis was made by using bronchoscopic procedures and the histologically diagnosis was compatible as a typical pulmonary carcinoid. The tumor was resected curatively, and the renin and aldosterone level became normal. A year after the patient looks healthy.
