Featured Application: The main application of this paper is to review and improve a model that enables the use of water heaters by small electricity customers in electricity markets and services in the short and medium term. For this goal, it is necessary to review the service of loads to avoid customer discomfort and the way control mechanisms work, usually thermostats. To guarantee a minimum level of accuracy in the flexibility of the response, the understanding of heat and electric processes in the load is a cornerstone for the participation of these appliances. This must be done by comprehensive validation of the model in different conditions, including water draws, temperature, and load control. To exemplify the potential of the model, an application in a prosumer scenario is presented. Residential segments are of the greatest interest from the point of view of Demand-Side Resources and Decarbonization. Main end-uses such as water heaters, heating, and cooling have interesting opportunities: first, they can store energy, and this is relevant for the integration of renewables. Second, they are candidates for efficiency and electrification, increasing their demand share and the flexibility of demand. This paper aims to formulate an elemental Physical-Based Heat Pump Water Heater model that will enable the use of these energy-efficient appliances through aggregation in complex products, considering the advantages for demand and supply sides. Simulation results show that the individual performance is quite accurate and that the proposed model is flexible enough to be used to take more profit from energy markets or to easily respond to fast-occurring events. The model can be easily aggregated and used to obtain baselines, an important point for Demand Response evaluation. Results also demonstrate that demand–supply coordination and balance can be improved using these models to reduce or mitigate the risks and volatility of renewables without inducing a noticeable loss of service. Consequently, the contribution of this responsive load can be modelled through this methodology, making the engagement of more customer segments in Demand Response policies more credible and deploying new segments, such as prosumers. Background and aims: Several methods are available to diagnose Helicobacter pylori infection. Our objective was to evaluate the tests used for both the initial diagnosis and the confirmation of eradication after treatment in Europe. Methods: The European Registry on the management of Helicobacter pylori infection is an international, multicentre, prospective, non-interventional registry aiming to evaluate the management of Helicobacter pylori-infected patients in Europe. Countries with at least 100 cases registered from June 2013 to April 2021, and with a validated diagnostic method were analysed. Data were quality reviewed. Results: A total of 34,920 adult patients from 20 countries were included (mean age 51 years; 61% women). To establish the initial diagnosis, invasive tests were performed in 19,801 (71%) patients, non-invasive in 11,369 (41%), and both in 3437 (12%). The most frequent were histology (n = 11,885; 43%), a rapid urease test (n = 10,636; 38%) and an urea breath test (n = 7577; 27%). According to the age, invasive tests were indicated in 11,179 (77%) ≥50 years, and in 8603 (65%) <50 years. Depending on the country, the use of invasive tests ranged from 29–99% in <50 years to 60–99% in ≥50. Most of the tests used to confirm eradication were non-invasive (n = 32,540; 93%), with the urea breath test being the most frequent (n = 32,540; 78%). In 2983 (9%) post-treatment tests, histology (n = 1887; 5%) or a rapid urease test (n = 1223; 4%) were performed. Conclusion: A great heterogeneity was observed for the initial diagnosis and confirmation of the eradication. Por ello, la integración de las renovables tiene que ir de la mano de un aumento de la flexibilidad del sistema, tanto en la generación como en la demanda. Las políticas de Respuesta de la Demanda se han orientado tradicionalmente a grandes consumidores, pero para que la integración se realice de forma eficiente, hay que incluir también al sector residencial, ya que explica aproximadamente el 27% del total de la energía consumida en la Unión Europea. El objetivo principal de esta tesis doctoral es el estudio y desarrollo de nuevas herramientas para la gestión de la generación y el consumo de los llamados prosumidores (consumidores que a la vez producen su propia energía). Los grupos objetivo son consumidores de los segmentos residencial y comercial, y algunos mercados de energía. Los sistemas de generación estudiados son de tipo fotovoltaico, y se ha considerado también la utilización de sistemas de almacenamiento tanto directos (baterías, supercondensadores) como indirectos (almacenamiento térmico). El trabajo engloba el análisis y la integración de las herramientas para la planificación, operación y evaluación de la Respuesta de la Demanda por parte de los consumidores y agregadores, así como su participación en los mercados de energía. Se han desarrollado modelos de cargas físicamente basados (PBLM), así como modelos de predicción del consumo y la generación renovable a corto plazo (STLF). Asimismo, se han obtenido las líneas base de consumo (CBL) que permiten la medida y verificación de las acciones de Respuesta de la Demanda, para su posterior evaluación y remuneración por parte del mercado y los operadores del sistema eléctrico. Se ha evaluado la instalación de sistemas de almacenamiento para aprovechar el frenado regenerativo de los vehículos y la aplicación de políticas de Respuesta de la Demanda que mejoren la eficiencia energética de este sector. Los resultados se presentan en forma de 5 artículos científicos publicados en revistas indexadas. Todos ellos demuestran que la Respuesta de la Demanda puede mejorar la flexibilidad del sistema eléctrico y la eficiencia energética de los consumidores, obteniéndose beneficios tanto a nivel individual (usuarios) como colectivo (sistema eléctrico). La Respuesta de la Demanda puede jugar un papel clave en el desarrollo de los futuros sistemas y mercados de energía, pero para ello es necesario un marco legislativo estable, un aumento de las inversiones, una mayor implicación de todos los agentes involucrados y el apoyo de la tecnología. [ENG] This doctoral dissertation has been presented in the form of thesis by publication. The development of renewable energy sources to achieve a sustainable scenario in the 2030-2050 horizon has led to an increase in the variability and rigidity of the Electricity Systems. Therefore, the integration of renewables should be related to an increase in the flexibility of the system, both in generation and demand sides. Demand Response policies have traditionally been oriented towards large consumers, but the residential sector should also be included as it accounts for approximately 27% of the total energy consumed in the European Union. The main objective of this Ph.D. is the study and development of new tools for the management of the generation and consumption of the so-called prosumers (consumers who produce their own energy). The target groups are customers in the residential and commercial segments, and some "special" consumers who normally are not participating actively in energy markets. The generation systems studied are photovoltaic, and the use of both direct (batteries, supercapacitors) and indirect (thermal storage systems) has also been considered. The work includes the analysis and integration of tools for the planning, operation, and evaluation of Demand Response by consumers and aggregators, as well as their participation in energy markets. Physically-Based Load Models (PBLM) and Short-Term Load and Generation Forecasting models (STLF) have been developed. For the measurement and verification of Demand Response, Customer Baselines (CBL) have been obtained in order to evaluate and calculate the incentives to be paid by the Energy Markets and the System Operators. Finally, Demand Response models have been applied to the railway electricity system. The installation of energy storage systems to take advantage of the regenerative braking of vehicles and the application of Demand Response policies to hotel loads to improve energy efficiency and demand flexibility in this sector have been evaluated. The results are presented in 5 scientific papers published in indexed journals. All of them demonstrate that Demand Response can improve the flexibility of the electricity system and the energy efficiency of consumers, with benefits at both the individual (users) and collective (electricity system) level. Demand Response can play a key role in the development of future energy systems and markets, but this requires a stable legislative framework, increased investments, greater involvement of all stakeholders, and technology support. The management of Helicobacter pylori infection has to rely on previous local effectiveness due to the geographical variability of antibiotic resistance. The aim of this study was to evaluate the effectiveness of first and second-line H. pylori treatment in Spain, where the empirical prescription is recommended. A multicentre prospective non-interventional registry of the clinical practice of European gastroenterologists concerning H. pylori infection (Hp-EuReg) was developed, including patients from 2013 until June 2019. Effectiveness was evaluated descriptively and through a multivariate analysis concerning age, gender, presence of ulcer, proton-pump inhibitor (PPI) dose, therapy duration and compliance. Overall, 53 Spanish hospitals were included, and 10,267 patients received a first-line therapy. The best results were obtained with the 10-day bismuth single-capsule therapy (95% cure rate by intention-to-treat) and with both the 14-day bismuth-clarithromycin quadruple (PPI-bismuth-clarithromycin-amoxicillin, 91%) and the 14-day non-bismuth quadruple concomitant (PPI-clarithromycin-amoxicillin-metronidazole, 92%) therapies. Second-line therapies were prescribed to 2448 patients, with most-effective therapies being the triple quinolone (PPI-amoxicillin-levofloxacin/moxifloxacin) and the bismuth-levofloxacin quadruple schemes (PPI-bismuth-levofloxacin-amoxicillin) prescribed for 14 days (92%, 89% and 90% effectiveness, respectively), and the bismuth single-capsule (10 days, 88.5%). Compliance, longer duration and higher acid inhibition were associated with higher effectiveness. &ldquo, Optimized&rdquo, H. pylori therapies achieve over 90% success in Spain.

(1) Aims: To assess the incidence of inflammatory bowel disease (IBD) in Spain, to describe the main epidemiological and clinical characteristics at diagnosis and the evolution of the disease, and to explore the use of drug treatments. (2) Methods: Prospective, population-based nationwide registry. Adult patients diagnosed with IBD—Crohn’s disease (CD), ulcerative colitis (UC) or IBD unclassified (IBD-U)—during 2017 in Spain were included and were followed-up for 1 year. (3) Results: We identified 3611 incident cases of IBD diagnosed during 2017 in 108 hospitals covering over 22 million inhabitants. The overall incidence (cases/100,000 person-years) was 16 for IBD, 7.5 for CD, 8 for UC, and 0.5 for IBD-U, 53% of patients were male and median age was 43 years (interquartile range = 31–56 years). During a median 12-month follow-up, 34% of patients were treated with systemic steroids, 25% with immunomodulators, 15% with biologics and 5.6% underwent surgery. The percentage of patients under these treatments was significantly higher in CD than UC and IBD-U. Use of systemic steroids and biologics was significantly higher in hospitals with high resources. In total, 28% of patients were hospitalized (35% CD and 22% UC patients, p <, 0.01). (4) Conclusion: The incidence of IBD in Spain is rather high and similar to that reported in Northern Europe. IBD patients require substantial therapeutic resources, which are greater in CD and in hospitals with high resources, and much higher than previously reported. One third of patients are hospitalized in the first year after diagnosis and a relevant proportion undergo surgery.

- Abstract
Demand Response (DR) is an opportunity and a concern for markets as well as power system flexibility. The deployment of DR depends on both knowledge on its performance and how to measure it effectively to provide adequate economic feedback. DR verification requires a baseline reference. This paper introduces a new baseline that provides an evaluation of response based on simple adjustment factors through physically-based models, tools which are also used in DR. The approach includes the detection of licit and gaming responses before and after DR. Results show that errors decrease by 10–15% with respect to conventional approaches.

- Abstract
Objective The best approach for Helicobacter pylori management remains unclear. An audit process is essential to ensure clinical practice is aligned with best standards of care. Design International multicentre prospective non-interventional registry starting in 2013 aimed to evaluate the decisions and outcomes in H. pylori management by European gastroenterologists. Patients were registered in an e-CRF by AEG-REDCap. Variables included demographics, previous eradication attempts, prescribed treatment, adverse events and outcomes. Data monitoring was performed to ensure data quality. Time-trend and geographical analyses were performed. Results 30 394 patients from 27 European countries were evaluated and 21 533 (78%) first-line empirical H. pylori treatments were included for analysis. Pretreatment resistance rates were 23% to clarithromycin, 32% to metronidazole and 13% to both. Triple therapy with amoxicillin and clarithromycin was most commonly prescribed (39%), achieving 81.5% modified intention-to-treat eradication rate. Over 90% eradication was obtained only with 10-day bismuth quadruple or 14-day concomitant treatments. Longer treatment duration, higher acid inhibition and compliance were associated with higher eradication rates. Time-trend analysis showed a region-dependent shift in prescriptions including abandoning triple therapies, using higher acid-inhibition and longer treatments, which was associated with an overall effectiveness increase (84%-90%). Conclusion Management of H. pylori infection by European gastroenterologists is heterogeneous, suboptimal and discrepant with current recommendations. Only quadruple therapies lasting at least 10 days are able to achieve over 90% eradication rates. European recommendations are being slowly and heterogeneously incorporated into routine clinical practice, which was associated with a corresponding increase in effectiveness.

- Abstract
Background: Experience in Helicobacter pylori eradication treatment of patients allergic to penicillin is very scarce. A triple combination with a PPI, clarithromycin (C), and metronidazole (M) is often prescribed as the first option, although more recently the use of a quadruple therapy with PPI, bismuth (B), tetracycline (T), and M has been recommended. Aim: To evaluate the efficacy and safety of first-line and rescue treatments in patients allergic to penicillin in the “European Registry of H pylori management” (Hp-EuReg). Methods: A systematic prospective registry of the clinical practice of European gastroenterologists (27 countries, 300 investigators) on the management of H pylori infection. An e-CRF was created on AEG-REDCap. Patients with penicillin allergy were analyzed until June 2019. Results: One-thousand eighty-four patients allergic to penicillin were analyzed. The most frequently prescribed first-line treatments were as follows: PPI + C + M (n = 285) and PPI + B + T + M (classic or Pylera®; n = 250). In first line, the efficacy of PPI + C + M was 69%, while PPI + B + T + M reached 91% (P <.001). In second line, after the failure of PPI + C + M, two rescue options showed similar efficacy: PPI + B + T + M (78%) and PPI + C + levofloxacin (L) (71%) (P >.05). In third line, after the failure of PPI + C + M and PPI + C + L, PPI + B + T + M was successful in 75% of cases. Conclusion: In patients allergic to penicillin, a triple combination with PPI + C + M should not be generally recommended as a first-line treatment, while a quadruple regimen with PPI + B + T + M seems to be a better option. As a rescue treatment, this quadruple regimen (if not previously prescribed) or a triple regimen with PPI + C + L could be used but achieved suboptimal (<80%) results.

- Abstract
The objective of this paper involves the analysis, identification and evaluation of different possibilities offered by technology for the improvement and the management of the use of energy and hybridization in railways: On board generation, demand response and energy storage, both in traction and auxiliary loads, considering the aggregation of resources and its stochastic nature. The paper takes into account the importance of efficient use of energy in railways, both currently (trains in service, prototypes) and in the future, considering the trends driven by energy policy scenarios (2030–2050) that will affect service and operation of units during their lifetime. A new activity has been considered that will be relevant in the future in the framework of a new electricity supply paradigm: Smart-Grids. According to this paradigm, the interaction of the Electric Power System and the Railway Supply System (somehow embedded in the Power System) will bring new opportunities for the collaboration of these two systems to perform, in a wise economic fashion, a better and more reliable operation of the complete energy system. The paper is focused on a mixed profile with low-medium traffic (passenger and freight): The first part of the route is electrified (3 kV DC catenary) whereas the second part is not electrified. Results justify that complex policies and objectives bring an opportunity to make cost-effective the hybridization of railway units, especially in low/medium traffic lines, which improves their social and economic sustainability.

El objetivo principal de este Trabajo Fin de Máster es el estudio normativo, técnico y económico del impacto de la generación distribuida en el sistema eléctrico y en su modo de operación, y especialmente de la influencia del autoconsumo con energía solar fotovoltaica. Este objetivo general se articula en una serie de objetivos más específicos: -Estudiar y conocer en profundidad el funcionamiento del sistema eléctrico y realizar un análisis técnico de cómo la inclusión de nuevos recursos de generación con fuente de energía primaria variable afectan a los sistemas eléctricos en el lado de la demanda. -Evaluar cómo la integración de la generación distribuida se beneficia al ser utilizada con otros recursos energéticos distribuidos. -Aplicar los conocimientos adquiridos a un caso práctico de usuarios que pretendan instalar generación fotovoltaica integrada en una red de distribución, mejorando el dimensionado del sistema de autoconsumo. -Evaluar económicamente los costes y las opciones que presentan los diferentes mercados eléctricos para la integración de la generación distribuida. -Adquirir conocimientos prácticos en un campo con futuro, integrando y desarrollando conocimientos de diferentes disciplinas (ingeniería eléctrica, ingeniería de sistemas e ingeniería electrónica). - Abstract
[EN] The objectives of improving the efficiency, and integration, of renewable sources by 2030-2050 are complex in practice and should be linked to an increase of demand-side flexibility. The main challenges to achieving this flexibility are the lack of incentives and an adequate framework. For instance, customers' revenue is usually low, the volatility of prices is high and there is not any practical feedback to customers from smart meters. The possibility of increasing customer revenue could reduce the uncertainty with respect to economic concerns, improving investments in efficiency, enabling technology and thus, engaging more customers in these policies. This objective could be achieved by the participation of customers in several markets. Moreover, Demand Response and Energy Efficiency can share ICT technologies but this participation needs to perform an aggregation of demand. The idea of this paper is to present some methodologies for facilitating the definition and evaluation of energy versus cost curves; and subsequently to estimate potential revenues due to Demand Response. This can be accomplished by models that estimate: demand and energy aggregation; economic opportunities and benefits; impacts on customer convenience; customer feedback and price analysis. By doing so, we would have comprehensive information that can help customers and aggregators to define energy packages and their monetary value with the objective of fostering their market participation.

- Abstract
[EN] The development of renewable sources in residential segments is basic to achieve a sustainable energy scenario in the horizon 2030-2050 because these segments explain around 25% of the final energy consumption. Demand Response and its effective coordination with renewable are additional concerns for residential segments. This paper deals with two problems: the demonstration of cost-effectiveness of renewables in three different scenarios, and the application of the flexibility of demand, performing as energy storage systems, to efficiently manage the generation of renewable sources while improving benefits and avoiding penalties for the customer. A residential customer in Spain has been used as example. The work combines the use of a commercial simulator to obtain photovoltaic generation, the monitoring of customer to obtain demand patterns, and the development of a Physically-Based Model to evaluate the capability of demand to follow self-generation. As a main result, the integration of models (load/generation), neglected in practice in other approaches in the literature, allows customers to improve revenue up to 20% and reach a basic but important knowledge on how they can modify the demand, development of new skills and, in this way, learn how to deal with the characteristics and limitations of both Demand and Generation when a customer becomes a prosumer. This synergy amongst demand and generation physically-based models boosts the possibilities of customers in electricity markets.

42. Tofacitinib in Ulcerative Colitis: Real-world Evidence From the ENEIDA Registry.

- Abstract
Aim To evaluate the effectiveness and safety of tofacitinib in ulcerative colitis [UC] in real life. Methods Patients from the prospectively maintained ENEIDA registry and treated with tofacitinib due to active UC were included. Clinical activity and effectiveness were defined based on Partial Mayo Score [PMS]. Short-term response/remission was assessed at Weeks 4, 8, and 16. Results A total of 113 patients were included. They were exposed to tofacitinib for a median time of 44 weeks. Response and remission at Week 8 were 60% and 31%, respectively. In multivariate analysis, higher PMS at Week 4 (odds ratio [OR] = 0].2; 95% confidence interval [CI] = 0].1–0.4) was the only variable associated with lower likelihood of achieving remission at Week 8. Higher PMS at Week 4 [OR = 0.5; 95% CI = 0.3–0.7] and higher PMS at Week 8 [OR = 0.2; 95% CI = 0.1–0.5] were associated with lower probability of achieving remission at Week 16. A total of 45 patients [40%] discontinued tofacitinib over time. Higher PMS at Week 8 was the only factor associated with higher tofacitinib discontinuation [hazard ratio = 1.5; 95% CI = 1.3–1.6]. A total of 34 patients had remission at Week 8; of these, 65% had relapsed 52 weeks after achieving remission; the dose was increased to 10 mg/12 h in nine patients, and five of them reached remission again. Seventeen patients had adverse events. Conclusions Tofacitinib is effective and safe in UC patients in real practice, even in a highly refractory cohort. A relevant proportion of patients discontinue the drug over time, mainly due to primary failure. [ABSTRACT FROM AUTHOR]

- Abstract
ObjectiveThe best approach for Helicobacter pylorimanagement remains unclear. An audit process is essential to ensure clinical practice is aligned with best standards of care.DesignInternational multicentre prospective non-interventional registry starting in 2013 aimed to evaluate the decisions and outcomes in H. pylorimanagement by European gastroenterologists. Patients were registered in an e-CRF by AEG-REDCap. Variables included demographics, previous eradication attempts, prescribed treatment, adverse events and outcomes. Data monitoring was performed to ensure data quality. Time-trend and geographical analyses were performed.Results30 394 patients from 27 European countries were evaluated and 21 533 (78%) first-line empirical H. pyloritreatments were included for analysis. Pretreatment resistance rates were 23% to clarithromycin, 32% to metronidazole and 13% to both. Triple therapy with amoxicillin and clarithromycin was most commonly prescribed (39%), achieving 81.5% modified intention-to-treat eradication rate. Over 90% eradication was obtained only with 10-day bismuth quadruple or 14-day concomitant treatments. Longer treatment duration, higher acid inhibition and compliance were associated with higher eradication rates. Time-trend analysis showed a region-dependent shift in prescriptions including abandoning triple therapies, using higher acid-inhibition and longer treatments, which was associated with an overall effectiveness increase (84%–90%).ConclusionManagement of H. pyloriinfection by European gastroenterologists is heterogeneous, suboptimal and discrepant with current recommendations. Only quadruple therapies lasting at least 10 days are able to achieve over 90% eradication rates. European recommendations are being slowly and heterogeneously incorporated into routine clinical practice, which was associated with a corresponding increase in effectiveness.

[ESP] El objetivo principal de este Trabajo Fin de Grado es la modelización del comportamiento dinámico de un convoy ferroviario y el estudio de las posibilidades de optimización de consumo energético y aprovechamiento de la energía generada en el frenado dinámico de los trenes, para su aplicación a la eficiencia energética en líneas de bajo tráfico. El recorrido en estudio es la línea Cartagena – Alcázar de San Juan. Para alcanzar este objetivo, primero se realiza la implantación mediante el software MATLAB® de las ecuaciones que rigen el movimiento de un convoy. Posteriormente se lleva a cabo la simulación y el análisis de todos los parámetros implicados: fuerzas resistentes, fuerzas de propulsión, aceleración, velocidad, consumo energético, recuperación de energía… Finalmente se efectuará un estudio de los distintos sistemas de almacenamiento existentes, y se presentarán cuatro alternativas de almacenamiento de la energía generada, realizando el diseño del sistema de almacenamiento necesario y un estudio energético y económico de cada opción desarrollada, así como un análisis de emisiones contaminantes. [ENG]The main objective of this Final Project is the modeling of the dynamic behaviour of a railway convoy and the study of the optimization possibilities of energy consumption and exploitation of the energy generated in the trains dynamic braking for its energy efficiency application in low traffic lines. The study tour is the line Cartagena - Alcázar de San Juan. To achieve this goal, the implantation of the equations that govern the motion of a convoy is performed using MATLAB software. Subsequently the simulation and analysis of all the parameters involved is carried out: resisting forces, propulsion forces, acceleration, speed, energy consumption, energy recovery... Finally a study of the different existing storage systems will be developed and four alternative storage of energy generated are presented, making the storage system design and the energy and economic study of each option developed. Summary: Background: Cohort studies comparing the characteristics of childhood‐onset and adulthood‐onset inflammatory bowel disease (IBD) in the biologics era are scarce. Aim: To compare disease characteristics, the use of immunomodulators and biologic agents and the need for surgery between childhood‐ and adulthood‐onset IBD. Methods: Inflammatory bowel disease patients from the ENEIDA registry diagnosed between 2007 and 2017 were included. The childhood‐onset cohort comprised patients diagnosed at ≤16 years of age and the adulthood‐onset cohort those diagnosed at >16 years. The cumulative incidences of immunosuppressive therapy, biologic therapy and surgery were estimated using Kaplan‐Meier curves, compared by the log‐rank test. Cox regression analysis was performed to identify potential predictive factors of treatment with immunosuppressants, biologic agents or surgery. Results: The adulthood‐onset cohort comprised 21 200 patients out of 20 354 (96%) and the childhood‐onset cohort 846 (4%). Median follow‐up was 54 months in the childhood‐onset cohort and 38 months in the adulthood‐onset cohort (P < 0.01). Proportions of Crohn's disease, ileocolonic involvement and inflammatory behaviour at diagnosis were higher in the childhood‐onset cohort. In the multivariate analysis, after adjusting for sex, type of IBD, extraintestinal manifestations, family history and smoking habit, childhood‐onset IBD was associated with higher risk of immunomodulator use (hazard ratio [HR] = 1.2, 95% confidence interval [95% CI] = 1.1‐1.2) and higher probability of receiving biologic treatment (HR = 1.2, 95% CI = 1.1‐1.3). However, childhood‐onset IBD was not associated with higher risk of surgery (HR = 0.9, 95% CI = 0.8‐1.2). Conclusions: Childhood‐onset IBD has differential characteristics and higher risk of treatment with immunomodulators and biologic agents, compared with adulthood‐onset IBD.
