1. Stellar Energetic Particle Transport in the Turbulent and CME-disrupted Stellar Wind of AU~Microscopii

Abstract: Energetic particles emitted by active stars are likely to propagate in astrospheric magnetized plasma turbulent and disrupted by the prior passage of energetic Coronal Mass Ejections (CMEs). We carried out test-particle simulations of $\sim$ GeV protons produced at a variety of distances from the M1Ve star AU~Microscopii by coronal flares or travelling shocks. Particles are propagated within the large-scale quiescent three-dimensional magnetic field and stellar wind reconstructed from measured magnetograms, and { within the same stellar environment following passage of a $10^{36}$~erg kinetic energy CME}. In both cases, magnetic fluctuations with an isotropic power spectrum are overlayed onto the large scale stellar magnetic field and particle propagation out to the two innnermost confirmed planets is examined. In the quiescent case, the magnetic field concentrates the particles onto two regions near the ecliptic plane. After the passage of the CME, the closed field lines remain inflated and the re-shuffled magnetic field remains highly compressed, shrinking the scattering mean free path of the particles. In the direction of propagation of the CME-lobes the subsequent EP flux is suppressed. Even for a CME front propagating out of the ecliptic plane, the EP flux along the planetary orbits highly fluctuates and peaks at $\sim 2 -3$ orders of magnitude higher than the average solar value at Earth, both in the quiescent and the post-CME cases., Comment: 19 pages, 14 figures, submitted

2. Space Weather-driven Variations in Ly$\alpha$ Absorption Signatures of Exoplanet Atmospheric Escape: MHD Simulations and the Case of AU Mic b

Abstract: We simulate the space environment around AU Microscopii b and the interaction between the magnetized stellar wind with a planetary atmospheric outflow for ambient stellar wind conditions and Coronal Mass Ejection (CME) conditions. We also calculate synthetic Ly$\alpha$ absorption due to neutral hydrogen in the ambient and the escaping planetary atmosphere affected by this interaction. We find that the Ly$\alpha$ absorption is highly variable due to the highly-varying stellar wind conditions. A strong Doppler blue-shift component is observed in the Ly$\alpha$ profile, in contradiction to the actual escape velocity observed in the simulations themselves. This result suggest that the strong Doppler blue-shift is likely attributed to the stellar wind, not the escaping neutral atmosphere, either through its advection of neutral planetary gas, or through the creation of a fast neutral flow via charge exchange between the stellar wind ions and the planetary neutrals. Indeed, our CME simulations indicate a strong stripping of magnetospheric material from the planet, including some of the neutral escaping atmosphere. Our simulations show that the pressure around close-in exoplanets is not much lower, and may be even higher, than the pressure at the top of the planetary atmosphere. Thus, the neutral atmosphere is hydrodynamically escaping with a very small velocity ($<15~km~s^{-1}$). Moreover, our simulations show that an MHD treatment is essential in order to properly capture the coupled magnetized stellar wind and the escaping atmosphere, despite of the atmosphere being neutral. Abstract: We investigate the space environment conditions near the Earth-size planet TOI~700~d using a set of numerical models for the stellar corona and wind, the planetary magnetosphere, and the planetary ionosphere. We drive our simulations using a scaled-down stellar input and a scaled-up solar input in order to obtain two independent solutions. We find that for the particular parameters used in our study, the stellar wind conditions near the planet are not very extreme -- slightly stronger than that near the Earth in terms of the stellar wind ram pressure and the intensity of the interplanetary magnetic field. Thus, the space environment near TOI700-d may not be extremely harmful to the planetary atmosphere, assuming the planet resembles the Earth. Nevertheless, we stress that the stellar input parameters and the actual planetary parameters are unconstrained, and different parameters may result in a much greater effect on the atmosphere of TOI700-d. Finally, we compare our results to solar wind measurements in the solar system and stress that modest stellar wind conditions may not guarantee atmospheric retention of exoplanets., Comment: accepted to ApJ

Abstract: Heliospheric plasmas require multi-scale and multi-physics considerations. On one hand, MHD codes are widely used for global simulations of the solar-terrestrial environments, but do not provide the most elaborate physical description of space plasmas. Hybrid codes, on the other hand, capture important physical processes, such as electric currents and effects of finite Larmor radius, but they can be used locally only, since the limitations in available computational resources do not allow for their use throughout a global computational domain. In the present work, we present a new coupled scheme which allows to switch blocks in the block-adaptive grids from fluid MHD to hybrid simulations, without modifying the self-consistent computation of the electromagnetic fields acting on fluids (in MHD simulation) or charged ion macroparticles (in hybrid simulation). 5. Stellar energetic particles in the magnetically turbulent habitable zones of TRAPPIST-1-like planetary systems

Abstract: Planets in close proximity to their parent star, such as those in the habitable zones around M dwarfs, could be subject to particularly high doses of particle radiation. We have carried out test-particle simulations of ~GeV protons to investigate the propagation of energetic particles accelerated by flares or travelling shock waves within the stellar wind and magnetic field of a TRAPPIST-1-like system. Turbulence was simulated with small-scale magnetostatic perturbations with an isotropic power spectrum. We find that only a few percent of particles injected within half a stellar radius from the stellar surface escape, and that the escaping fraction increases strongly with increasing injection radius. Escaping particles are increasingly deflected and focused by the ambient spiralling magnetic field as the superimposed turbulence amplitude is increased. In our TRAPPIST-1-like simulations, regardless of the angular region of injection, particles are strongly focused onto two caps within the fast wind regions and centered on the equatorial planetary orbital plane. Based on a scaling relation between far-UV emission and energetic protons for solar flares applied to M dwarfs, the innermost putative habitable planet, TRAPPIST-1e, is bombarded by a proton flux up to 6 orders of magnitude larger than experienced by the present-day Earth. 6. The Magnetic Nature of the Cataclysmic Variable Period Gap

Abstract: One of the most important problems in the context of cataclysmic variables (CVs) is the lack of observations of systems with periods between 2 and 3.12 hours, known as the period gap. The orbital evolution of CVs with periods shorter than those in the gap is dominated by gravitational radiation while for periods exceeding those of the gap it is dominated by magnetic braking of the secondary star. Spruit & Ritter (1983) showed that as periods approach 3 hours and secondary stars become fully convective a sharp decline in magnetic dynamo and braking efficiency would result in such a gap. Recent X-ray observations finding coronal magnetic energy dissipation is similar in fully convective and partly radiative M dwarfs cast this theory into doubt. In this work, we use Zeeman-Doppler imaging observations culled from the literature to show that the complexity of the surface magnetic fields of rapidly rotating M dwarfs increases with decreasing rotation period. Garraffo et al. (2018) have shown that the efficiency of angular momentum loss of cool stars declines strongly with increasing complexity of their surface magnetic field. We explore the idea of Taam & Spruit (1989) that magnetic complexity might then explain the period gap. By generating synthetic CV populations using a schematic CV evolutionary approach, we show that the CV period gap can naturally arise as a consequence of a rise in secondary star magnetic complexity near the long period edge of the gap that renders a sharp decline in their angular momentum loss rate., Comment: 7 pages, 3 figures, accepted to ApJ

Abstract: Observations of young open clusters show a bimodal distribution of rotation periods that has been difficult to explain with existing stellar spin-down models. Detailed MHD stellar wind simulations have demonstrated that surface magnetic field morphology has a strong influence on wind-driven angular momentum loss. Observations suggest that faster rotating stars store a larger fraction of their magnetic flux in higher-order multipolar components of the magnetic field. In this work, we present a new model for stellar spin-down that, for the first time, accounts for the stellar surface magnetic field configuration. 8. Energy Dissipation in the Upper Atmospheres of Trappist-1 Planets

Abstract: We present a method to quantify the upper-limit of the energy transmitted from the intense stellar wind to the upper atmospheres of three of the Trappist-1 planets (e, f, and g). We use a formalism that treats the system as two electromagnetic regions, where the efficiency of the energy transmission between one region (the stellar wind at the planetary orbits) to the other (the planetary ionospheres) depends on the relation between the conductances and impedances of the two regions. Since the energy flux of the stellar wind is very high at these planetary orbits, we find that for the case of high transmission efficiency (when the conductances and impedances are close in magnitude), the energy dissipation in the upper planetary atmospheres is also very large. On average, the Ohmic energy can reach $0.5-1~W/m^2$, about 1\% of the stellar irradiance and 5-15 times the EUV irradiance. Here, using constant values for the ionospheric conductance, we demonstrate that the stellar wind energy could potentially drive large atmospheric heating in terrestrial planets, as well as in hot jupiters. 9. Giant coronal loops dominate the quiescent X-ray emission in rapidly rotating M stars

Abstract: Observations indicate that magnetic fields in rapidly rotating stars are very strong, on both small and large scales. What is the nature of the resulting corona? Here we seek to shed some light on this question. We use the results of an anelastic dynamo simulation of a rapidly rotating fully-convective M-star to drive a physics-based model for the stellar corona. We find that due to the several kilo Gauss large-scale magnetic fields at high latitudes, the corona and its X-ray emission are dominated by star-size large hot loops, while the smaller, underlying colder loops are not visible much in the X-ray. 10. Simulating the environment around planet-hosting stars - II. Stellar winds and inner astrospheres

Abstract: We present the results of a comprehensive numerical simulation of the environment around three exoplanet-host stars (HD 1237, HD 22049, and HD 147513). Our simulations consider one of the latest models currently used for space weather studies in the Heliosphere. Large-scale magnetic field maps, recovered with two implementations of the tomographic technique of Zeeman-Doppler imaging, serve to drive steady-state solutions in each system. This paper contains the description of the stellar wind and inner astrosphere, while the coronal structure was previously discussed in Alvarado-G\'omez et al. (2016). The analysis includes the magneto-hydrodynamical properties of the stellar wind, the associated mass and angular momentum loss rates, as well as the topology of the astrospheric current sheet in each system. A systematic comparison among the considered cases is performed, including two reference solar simulations covering activity minimum and maximum. For HD 1237, we investigate the interactions between the structure of the developed stellar wind, and a possible magnetosphere around the Jupiter-mass planet in this system. We find that the process of particle injection into the planetary atmosphere is dominated by the density distribution rather than velocity profile of the stellar wind. In this context, we predict a maximum exoplanetary radio emission of 12 mJy at 40 MHz in this system, assuming the crossing of a high-density streamer during periastron passage. Furthermore, in combination with the analysis performed in Alvarado-G\'omez et al. (2016), we obtain for the first time a fully simulated mass loss-activity relation, which is compared and discussed in the context of the relation based on astrospheric detections proposed by Wood et al. (2005a). Finally, we provide a characterisation of the 3D properties of the stellar wind of these systems, at the inner edges of their habitable zones., Comment: 19 pages, 12 Figures. Abstract: We present the results of a detailed numerical simulation of the circumstellar environment around three exoplanet-hosting stars. A state-of-the-art global magnetohydrodynamic (MHD) model is considered, including Alfv\'en wave dissipation as a self-consistent coronal heating mechanism. This paper contains the description of the numerical set-up, evaluation procedure, and the simulated coronal structure of each system (HD 1237, HD 22049 and HD 147513). The simulations are driven by surface magnetic field maps, recovered with the observational technique of Zeeman Doppler Imaging (ZDI). A detailed comparison of the simulations is performed, where two different implementations of this mapping routine are used to generate the surface field distributions. Quantitative and qualitative descriptions of the coronae of these systems are presented, including synthetic high-energy emission maps in the Extreme Ultra-Violet (EUV) and Soft X-rays (SXR) ranges. Using the simulation results, we are able to recover similar trends as in previous observational studies, including the relation between the magnetic flux and the coronal X-ray emission. Furthermore, for HD 1237 we estimate the rotational modulation of the high-energy emission due to the various coronal features developed in the simulation. We obtain variations, during a single stellar rotation cycle, up to 15\% for the EUV and SXR ranges. Abstract: COCOA-PUFS is an energy-diverse, time-domain study of the ultra-fast spinning, heavily spotted, yellow giant FK Com (HD117555; G4 III). This single star is thought to be a recent binary merger, and is exceptionally active by measure of its intense ultraviolet and X-ray emissions, and proclivity to flare. COCOA-PUFS was carried out with Hubble Space Telescope in the UV (120-300 nm), using mainly its high-performance Cosmic Origins Spectrograph, but also high-precision Space Telescope Imaging Spectrograph; Chandra X-ray Observatory in the soft X-rays (0.5-10 keV), utilizing its High-Energy Transmission Grating Spectrometer; together with supporting photometry and spectropolarimetry in the visible from the ground. This is an introductory report on the project. FK Com displayed variability on a wide range of time scales, over all wavelengths, during the week-long main campaign, including a large X-ray flare; "super-rotational broadening" of the far-ultraviolet "hot-lines" (e.g., Si IV 139 nm (T~80,000 K) together with chromospheric Mg II 280 nm and C II 133 nm (10,000-30,000 K); large Doppler swings suggestive of bright regions alternately on advancing and retreating limbs of the star; and substantial redshifts of the epoch-average emission profiles. These behaviors paint a picture of a highly extended, dynamic, hot (10 MK) coronal magnetosphere around the star, threaded by cooler structures perhaps analogous to solar prominences, and replenished continually by surface activity and flares. Suppression of angular momentum loss by the confining magnetosphere could temporarily postpone the inevitable stellar spindown, thereby lengthening this highly volatile stage of coronal evolution., Comment: to be published in ApJS

13. The Interaction of Venus-like, M-dwarf Planets with the Stellar Wind of Their Host Star

Abstract: We study the interaction between the atmospheres of Venus-like, non-magnetized exoplanets orbiting an M-dwarf star, and the stellar wind using a multi-species Magnetohydrodynaic (MHD) model. We focus our investigation on the effect of enhanced stellar wind and enhanced EUV flux as the planetary distance from the star decreases. Our simulations reveal different topologies of the planetary space environment for sub- and super-Alfvenic stellar wind conditions, which could lead to dynamic energy deposition in to the atmosphere during the transition along the planetary orbit. We find that the stellar wind penetration for non-magnetized planets is very deep, up to a few hundreds of kilometers. We estimate a lower limit for the atmospheric mass-loss rate and find that it is insignificant over the lifetime of the planet. Abstract: We study the magnetospheric structure and the ionospheric Joule Heating of planets orbiting M-dwarf stars in the habitable zone using a set of magnetohydrodynamic (MHD) models. The stellar wind solution is used to drive a model for the planetary magnetosphere, which is coupled with a model for the planetary ionosphere. Our simulations reveal that the space environment around close-in habitable planets is extreme, and the stellar wind plasma conditions change from sub- to super-Alfvenic along the planetary orbit. As a result, the magnetospheric structure changes dramatically with a bow shock forming in the super-Alfvenic sectors, while no bow shock forms in the sub-Alfvenic sectors. The planets reside most of the time in the sub-Alfvenic sectors with poor atmospheric protection. A significant amount of Joule Heating is provided at the top of the atmosphere as a result of the planetary interaction with the stellar wind. For the steady-state solution, the heating is about 0.1-3\% of the total incoming stellar irradiation, and it is enhanced by 50\% for the time-dependent case. The significant Joule Heating obtained here should be considered in models for the atmospheres of habitable planets in terms of the thickness of the atmosphere, the top-side temperature and density, the boundary conditions for the atmospheric pressure, and particle radiation and transport., Comment: 15 pages, 7 figures

17. The effect of limited spatial resolution of stellar surface magnetic field maps on MHD wind and coronal X-ray emission models

Abstract: We study the influence of the spatial resolution on scales of $5\deg$ and smaller of solar surface magnetic field maps on global magnetohydrodynamic solar wind models, and on a model of coronal heating and X-ray emission. We compare the solutions driven by a low-resolution Wilcox Solar Observatory magnetic map, the same map with spatial resolution artificially increased by a refinement algorithm, and a high-resolution Solar and Heliospheric Observatory Michelson Doppler Imager map. We find that both the wind structure and the X-ray morphology are affected by the fine-scale surface magnetic structure. Moreover, the X-ray morphology is dominated by the closed loop structure between mixed polarities on smaller scales and shows significant changes between high and low resolution maps. We conclude that three-dimensional modeling of coronal X-ray emission has greater surface magnetic field spatial resolution requirements than wind modeling, and can be unreliable unless the dominant mixed polarity magnetic flux is properly resolved., Comment: 26 pages, 6 figures, accepted for publication in ApJ

Abstract: We carry out the first time-dependent numerical MagnetoHydroDynamic modeling of an extrasolar planetary system to study the interaction of the stellar magnetic field and wind with the planetary magnetosphere and outflow. We base our model on the parameters of the HD 189733 system, which harbors a close-in giant planet. Our simulation reveals a highly structured stellar corona characterized by sectors with different plasma properties. The star-planet interaction varies in magnitude and complexity, depending on the planetary phase, planetary magnetic field strength, and the relative orientation of the stellar and planetary fields. It also reveals a long, comet-like tail which is a result of the wrapping of the planetary magnetospheric tail by its fast orbital motion. A reconnection event occurs at a specific orbital phase, causing mass loss from the planetary magnetosphere that can generate a hot spot on the stellar surface. The simulation also shows that the system has sufficient energy to produce hot-spots observed in Ca II lines in giant planet hosting stars. Abstract: Junction conditions for vacuum solutions in five-dimensional Einstein-Gauss-Bonnet gravity are studied. We focus on those cases where two spherically symmetric regions of space-time are joined in such a way that the induced stress tensor on the junction surface vanishes. So a spherical vacuum shell, containing no matter, arises as a boundary between two regions of the space-time. Such solutions are a generalized kind of spherically symmetric empty space solutions, described by metric functions of the class $C^0$. New global structures arise with surprising features. In particular, we show that vacuum spherically symmetric wormholes do exist in this theory. These can be regarded as gravitational solitons, which connect two asymptotically (Anti) de-Sitter spaces with different masses and/or different effective cosmological constants. We prove the existence of both static and dynamical solutions and discuss their (in)stability under perturbations that preserve the symmetry. This leads us to discuss a new type of instability that arises in five-dimensional Lovelock theory of gravity for certain values of the coupling of the Gauss-Bonnet term., Comment: 9 pages. Abstract: We describe a case of spontaneous rupture of the splenic vein in a pregnant patient at 33 weeks gestation. For the first time in literature, we report follow-up investigations aimed at understanding the cause of the event. The woman was admitted to the emergency ward for hypovolemic shock. Maternal and fetal distress prompted an immediate cesarean section. The fetus was delivered stillborn, hemoperitoneum was present and two spontaneous splenic vein’s lacerations were found to be the source of the bleeding. The immediate splenectomy and aggressive correction of anemia and coagulopathy determined maternal survival. During the follow-up, no evidence of neither congenital nor acquired causative factors for a splenic vein rupture was found. The aim of reporting this case is to increase the awareness between clinicians of this condition since it is both rare and with an aspecific clinical picture. It presents itself in apparently low-risk patients and a good maternal-fetal outcome can only be achieved by early diagnosis and prompt treatment.

Abstract: COCOA-PUFS is an energy-diverse, time-domain study of the ultra-fast spinning, heavily spotted, yellow giant FK Comae Berenices (FK Com: HD117555; G4 III). This single star is thought to be a recent binary merger, and is exceptionally active by measure of its intense ultraviolet (UV) and X-ray emissions, and proclivity to flare. COCOA-PUFS was carried out with the Hubble Space Telescope in the UV (1200–3000 Å), using mainly its high-performance Cosmic Origins Spectrograph, but also high precision Space Telescope Imaging Spectrograph; Chandra X-ray Observatory in the soft X-rays (0.5–10 keV), utilizing its High-Energy Transmission Grating Spectrometer; together with supporting photometry and spectropolarimetry in the visible from the ground. This is an introductory report on the project. FK Com displayed variability on a wide range of timescales over all wavelengths during the week-long main campaign, including a large X-ray flare; "super-rotational broadening" of the far-ultraviolet "hot lines" (e.g., Si iv 1393 Å; 8 × 10[superscript 4] K) together with chromospheric Mg ii 2800 Å and C ii 1335 Å (1–3 × 10[superscript 4] K); large Doppler swings suggestive of bright regions alternately on advancing and retreating limbs of the star; and substantial redshifts of the epoch-average emission profiles. These behaviors paint a picture of a highly extended, dynamic, hot (~10 MK) coronal magnetosphere around the star, threaded by cooler structures perhaps analogous to solar prominences and replenished continually by surface activity and flares. Abstract: International audience; Objectives: Our aim was to assess if switching from a protease inhibitors (PI)-based regimen to a PI-free one is associated with an increased risk of Kaposi Sarcoma (KS) relapse among patients living with HIV (PLHIV) with history of KS and controlled HIV replication.Methods: In a retrospective analysis of the prospectively collected Dat'AIDS database we selected patients who both had a past KS history and a HIV-1 viral load below 200 copies/mL while being PI-treated. We searched for KS relapses while persistent virological success was maintained for at least 6 months, whether patients kept taking the PI, or switched to PI-free regimen.Results: Among the 216 patients with past KS event and a history of HIV-1 infection efficiently treated by a PI-based regimen, 148 patients (68.5%) later switched to a PI-sparing regimen. Their baseline characteristics were not different from non-switching patients. We described 7 cases of relapse (3.2% of the 216 patients). Five cases of relapse occurred in switching patients (3.4%). The remaining two relapses occurred in PI-treated patients (2.9%). At KS relapse, CD4 cell count was 459 cells/μL (range 225-560) for switching patients, compared with 362 and 136 cells/μL for the other two patients.Conclusions: In this large cohort of PLHIV with a history of KS and ART-controlled HIV replication, KS relapses were described in 3.2% of the patients, and were not more frequent when a PI-containing ART regimen has been switched to a PI-free regimen. Our results do not support a specific effect of PI on KS.

Abstract: Background The arrival of highly effective, well-tolerated, direct-acting antiviral agents (DAA) led to a dramatic decrease in hepatitis C virus (HCV) prevalence. Human immunodeficiency virus (HIV)-HCV–coinfected patients are deemed a priority population for HCV elimination, while a rise in recently acquired HCV infections in men who have sex with men (MSM) has been described. We describe the variations in HIV-HCV epidemiology in the French Dat’AIDS cohort. Methods This was a retrospective analysis of a prospective cohort of persons living with HIV (PLWH) from 2012 to 2018. We determined HCV prevalence, HCV incidence, proportion of viremic patients, treatment uptake, and mortality rate in the full cohort and by HIV risk factors. Results From 2012 to 2018, 50 861 PLWH with a known HCV status were followed up. During the period, HCV prevalence decreased from 15.4% to 13.5%. HCV prevalence among new HIV cases increased from 1.9% to 3.5% in MSM but remained stable in other groups. Recently acquired HCV incidence increased from 0.36/100 person-years to 1.25/100 person-years in MSM. The proportion of viremic patients decreased from 67.0% to 8.9%. MSM became the first group of viremic patients in 2018 (37.9%). Recently acquired hepatitis represented 59.2% of viremic MSM in 2018. DAA treatment uptake increased from 11.4% to 61.5%. More treatments were initiated in MSM in 2018 (41.2%) than in intravenous drug users (35.6%). In MSM, treatment at the acute phase represented 30.0% of treatments in 2018. Conclusions A major shift in HCV epidemiology was observed in PLWH in France from 2012 to 2018, leading to a unique situation in which the major group of HCV transmission in 2018 was MSM. Abstract: International audience; Background - Although antiretroviral therapy (ART) has reduced the risk of Kaposi sarcoma (KS), KS cases still occur in HIV-infected people. Objective - To describe all KS cases observed between 2010 and 2015 in a country with high ART coverage. Methods - Retrospective study using longitudinal data from 44 642 patients in the French Dat'AIDS multicenter cohort. Patients' characteristics were described at KS diagnosis according to ART exposure and to HIV-plasma viral load (HIV-pVL) (≤50 or >50) copies/mL. Results - Among the 209 KS cases diagnosed during the study period, 33.2% occurred in ART naïve patients, 17.3% in ART-experienced patients and 49.5% in patients on ART, of whom 23% for more than 6 months. Among these patients, 24 (11.5%) had HIV-pVL ≤50 cp/mL, and 16 (66%) were treated with a boosted-PI-based regimen. The distribution of KS localization did not differ by ART status nor by year of diagnosis. Limitations - Data on human herpesvirus 8, treatment modalities for KS and response rate were not collected. Conclusion - Half of KS cases observed in the study period occurred in patients not on ART, reflecting the persistence of late HIV diagnosis. Factors associated with KS in patients on ART with HIV-pVL ≤50 cp/mL remain to be explored.

49. Pregnancy in an Asymptomatic Woman with Porencephalic and Arachnoid Cysts.

Abstract: A 25-year-old woman presented to the obstetric clinic in her first pregnancy. The patient was accompanied by her mother who reported an episode of intracerebral hemorrhage after birth and also remembered access to the emergency department after generalized tonic-clonic seizure as an infant. She was not able to describe the therapy for seizure preventions and even when and why it was suspended, but she affirmed that no residual neurological consequences were detected in the following years. Actually, the pregnant woman was in good health without neurological symptoms nor assumed any therapy. A neurologist reviewed the patient's CT scan in which arachnoid cyst and porencephalic cyst were evident, and he assessed that no abnormalities were found in motor, sensory, and mental state examination. EEG did not show any epileptiform or seizure-like activity. No antiepileptic drug was prescribed due to the absence of symptoms since many years. The patient had no neurological symptoms during pregnancy or obstetric complications and delivered at term a healthy baby through a caesarean section. Abstract: Pregnancy after bariatric surgery has some peculiarities related to obesity, type of surgery, amount of weight loss, time elapsed from the surgery and adherence to medical prescriptions. Pregnant woman is at risk of nutritional deficiencies and it is unclear whether there is an increased incidence of intestinal complications during pregnancy after bariatric surgery and whether this kind of complications are more frequent during cesarean section. The fetus is at high risk of prematurity and fetal growth restriction, but they seem not at increased risk of birth defects (DTN) except in individual cases of folic acid deficiency (DTN) or vitamin K defect (similar abnormalities in patients receiving oral anticoagulants). In addition, the incidence of gestational diabetes and hypertension results to be decreased. Other postnatal outcomes from possible epigenetic modifications need to be evaluated in the long-term postnatal follow-up.
