Additional file 1. : Loss to follow-up and elaboration on multiple imputation methodology. This file shows the flow of the IBenC cohort for this article and how missing data was handled. It also elaborates on the MI method, such as the methods used for imputation and selected key variables.

The aim of this study was to obtain evidenced-based knowledge about older persons in home care; we conducted a population-based study at 11 sites in Europe (2001/2002). This article focuses on urinary incontinence and need for help in home care. Methods: A sample of 4010 respondents 65 years or older were assessed by the Resident Assessment Instrument for Home Care. Urinary incontinence was defined as leakage once a week or more including use of catheters. Results: A total of 1478 individuals had urinary incontinence, 45% men and 47% women. The use of pads ran from 29% to 52% between the sites. The associates of urinary incontinence were: moderate or severe cognitive impairment, dependency in toileting and other activities of daily living compared with less impaired; urinary infections, obesity and faecal incontinence. Caregivers to persons with urinary incontinence reported burden or stress more often then carers to nonurinary incontinence individuals (OR = 2.2, 95% CI 1.8-2.7). Conclusions: To enable older people with incontinence to stay at home with a better quality of life, they need caring assistance during toileting on a regular basis.

In der Bundesrepublik Deutschland werden jährlich über 48.000 Prostatakarzinome diagnostiziert. Die radikale Prostatektomie, seit vielen Jahren Standardbehandlung des lokal begrenzten Prostatakarzinoms, wird in Berlin jährlich über 1.000 Mal durchgeführt. Patienten mit Prostatakarzinom müssen einerseits eine lebensbedrohliche Erkrankung bewältigen, andererseits ist sowohl die körperliche als auch die sexuelle Identität des Mannes durch die Erkrankung bedroht. Trotz der großen Relevanz gibt es bislang keine gesundheitspolitisch verankerten Qualitätsziele und Qualitätsmaßnahmen zum Prostatakarzinom. Die vorliegende Arbeit untersucht die Frage, welche Indikatoren zur Beurteilung der Qualität der radikalen Prostatektomie in Deutschland und international angewendet werden und welche Ergebnisqualität vorliegt. Die Behandlungs-/ Versorgungssituation in Berlin wird dabei eingehend beschrieben. Außerdem werden die Kriterien untersucht, die für Patienten und Ärzte hinsichtlich der Therapie- und Klinikwahl sowie der Ergebnisqualität der radikalen Prostatektomie relevant sind. Der Patient und seine subjektive Bewertung der Operation stehen im Mittelpunkt der Analyse. Die Lebensqualität als wesentlicher Faktor des Outcomes der Operation nimmt eine gesonderte Stellung ein. Das methodische Vorgehen dieser Untersuchung steht auf drei Säulen: (i) Analyse von Leistungszahlen, Prozess- und Strukturindikatoren der urologischen Kliniken Berlins, (ii) Befragung von urologischen Klinikärzten und Patienten zu Zielgrößen und Endpunkten der radikalen Prostatektomie und schließlich (iii) Anwendung des EORTC-Lebensqualitätsfragebogens (QLQ-C30). Es wurden an neun Berliner Kliniken Patienten befragt, die im Jahr 2004 radikal operiert wurden. 48 Ärzte und 498 Patienten nahmen Stellung zu Qualitätsaspekten der radikalen Prostatektomie und zur Lebensqualität nach der Operation. Die Ergebnisse zeigen, dass das ärztliche Klinikpersonal und die Patienten eine partnerschaftliche Therapieentscheidung wünschen. Der behandelnde Arzt gilt für beide Gruppen als stärkster Partner bei der Therapieentscheidung. Die Betreuung und Information in den neun Kliniken wird von den Patienten sehr positiv bewertet. Die gewählte Klinik hat keinen Einfluss auf Bewertung der Behandlung, Komplikationen oder Lebensqualität. Die Patienten bestätigen die in urologischen Studien erwähnten funktionellen Probleme mit Kontinenz und Potenz nach der Operation. Die Ergebnisse der Befragung belegen allerdings eine deutlich stärkere Beeinträchtigung, als dies bisher in urologischen Studien angegeben wurde. Die Lebensqualität der Operierten ist abhängig von Alter, operativem Zugangsweg und Potenz-/Harninkontinenzproblemen. Jüngere Patienten sowie Patienten, die mehr als ein Jahr nach der Operation noch unter Kontinenz- und Potenzschwierigkeiten leiden, schätzen ihre Lebensqualität negativer ein. Every year, more than 48,000 prostate cancers (PCa) are diagnosed in the Federal Republic of Germany. The standard therapy for localized prostate cancer is, since many years, the radical prostatectomy (RPT) of which more than 1,000 are annually performed in Germany. PCa patients have to cope with a disease that not only is life threatening, but also jeopardizes the male physical and sexual identity. Despite the great relevance of the problem, public health policies and legislature regarding quality standards are lacking. The present investigation sets out to examine which indicators are used in Germany and internationally, and to assess the quality of outcome. It describes in detail level and quality of treatment and medical care in Berlin. It also examines the criteria which determine patients’ and doctors’ choices of specific clinics and forms of treatment, and the expected outcome, with focus on the patient and his subjective assessment of the surgical procedure. In this context, quality of life as an essential aspect of outcome receives special consideration. The methodological approach of this investigation rests on three pillars: (i) Analysis of numbers of operations performed at, and process indicators of urological clinics in Berlin; (ii) Inquiries with urological clinicians and patients regarding target data and endpoints of RPT; (iii) Use of the EORTC Quality-of-Life Questionnaire QLQ-C30. 498 patients who underwent RPT in any of the nine participating urological clinics in Berlin, in 2004, and 48 clinicians performing RPT participated in the inquiry answered questions regarding quality of RPT and postoperative quality of life. The results of the investigation show that both clinicians and patients favour a partnership when deciding on the therapy, with the attending physician being considered the main partner. Patients’ ratings of the medical care and information obtained in the nine participating clinics were very positive. The choice of clinic had no influence on the appraisal of treatment, operative complications, and quality of life. Functional problems regarding urinary continence and sexual potency as described in urological trials were confirmed, albeit to a larger extent than previously reported. Postoperative quality of life depends on patient’s age, surgical approach, and possible impairment of potency and/or urinary incontinence. Patients below the age of 60, and as well as patients who suffer from incontinenceand/or diminishes potency more than one year after operation indicate a poor quality of life.
