Abstract

Objective: Management of follow-up protocols after endovascular aortic repair (EVAR) varies significantly between centers and is not standardized according to sac regression. By designing an international expert-based Delphi consensus, the study aimed to create recommendations on follow-up after EVAR according to sac evolution. Methods: Eight facilitators created appropriate statements regarding the study topic that were voted, using a 4-point Likert scale, by a selected panel of international experts using a three-round modified Delphi consensus process. Based on the experts' responses, only those statements reaching a grade A (full agreement ≥75%) or B (overall agreement ≥80% and full disagreement <5%) were included in the final document. Results: One-hundred and seventy-four participants were included in the final analysis, and each voted the initial 29 statements related to the definition of sac regression (Q1-Q9), EVAR follow-up (Q10-Q14), and the assessment and role of sac regression during follow-up (Q15-Q29). At the end of the process, 2 statements (6.9%) were rejected, 9 statements (31%) received a grade B consensus strength, and 18 (62.1%) reached a grade A consensus strength. Of 27 final statements, 15 (55.6%) were classified as grade I, whereas 12 (44.4%) were classified as grade II. Experts agreed that sac regression should be considered an important indicator of EVAR success and always be assessed during follow-up after EVAR. Conclusions: Based on the elevated strength and high consistency of this international expert-based Delphi consensus, most of the statements might guide the current clinical management of follow-up after EVAR according to the sac regression. Future studies are needed to clarify debated issues.

Abstract

BACKGROUND: The SAfety and FEasibility of standard EVAR outside the instruction for use (SAFE-EVAR) Study was designed to define the attitude of Italian vascular surgeons towards the use of standard endovascular repair (EVAR) for infrarenal abdominal aortic aneurysm (AAA) outside the instruction for use (IFU) through a Delphi consensus endorsed by the Italian Society of Vascular and Endovascular Surgery (Società Italiana di Chirurgia Vascolare ed Endovascolare - SI CVE). METHODS : A questionnaire consisting of 26 statements was developed, validated by an 18-member Advisory Board, and then sent to 600 Italian vascular surgeons. The Delphi process was structured in three subsequent rounds which took place between April and June 2023. In the first two rounds, respondents could indicate one of the following five degrees of agreement: 1) strongly agree; 2) partially agree; 3) neither agree nor disagree; 4) partially disagree; 5) strongly disagree; while in the third round only three different choices were proposed: 1) agree; 2) neither agree nor disagree; 3) disagree. We considered the consensus reached when ≥70% of respondents agreed on one of the options. After the conclusion of each round, a report describing the percentage distribution of the answers was sent to all the participants. RESULTS: Two-hundred-forty-four (40.6%) Italian Vascular Surgeons agreed to participate the first round of the Delphi Consensus; the second and the third rounds of the Delphi collected 230 responders (94.3% of the first-round responders). Four statements (15.4%) reached a consensus in the first rounds. Among the 22 remaining statements, one more consensus (3.8%) was achieved in the second round. Finally, seven more statements (26.9%) reached a consensus in the simplified last round. Globally, a consensus was reached for almost half of the proposed statements (46.1%). Abstract

We present a complete analysis of the density distribution of particles in a two dimensional silo after discharge. Simulations through a pseudo-dynamic algorithm are performed for filling and subsequent discharge of a plane silo. Particles are monosized hard disks deposited in the container and subjected to a tapping process for compaction. Then, a hole of a given size is open at the bottom of the silo and the discharge is triggered. After a clogging at the opening is produced, and equilibrium is restored, the final distribution of the remaining particles at the silo is analyzed by dividing the space into cells with different geometrical arrangements to visualize the way in which the density depression near the opening is propagated throughout the system. Abstract

Earthquake death tolls are a basic statistical measure of the capability of a country to manage seismic risk. The extremely high Turkish death toll of 50,000 from the Kahramanmaras earthquake doublet of 6 February 2023 is the product of a cascade of detrimental factors. These need to be explained if lessons from this disaster are to be learned. This is the purpose and objective of this paper, which is a contribution to the interdisciplinary Frontiers research topic on integrated perspectives on the 2023 Turkey and Syria earthquakes: advancing understanding and preparedness across earth sciences, engineering and public health. This paper covers these three disciplines by focusing on casualties, and identifying crucial aspects of earth sciences and engineering which contributed to the high death toll. First, there was a surprising combination of multiple fault segment ruptures, and a high level of ground motion relative to the risk-based Turkish code, indicative of the under-representation of the M7.5+ earthquake doublet event in the national probabilistic seismic hazard model. This combination of fault segment ruptures was missing from all seismic source models. Furthermore, the capability of buildings to cope with strong ground motion was much reduced by informal construction methods, which eroded the margin of safety needed to avoid building collapse. The extent of building code non-compliance was widely underestimated in seismic risk models. Noncompliance is often hard to identify, but construction amnesties make noncompliance more transparent and trackable. The disastrous outcome of the Kahramanmaras earthquake doublet of 6 February 2023 has drawn global attention to systemic building code non-compliance, and the open official acceptance of informal housing. To demonstrate that this key systemic risk is far from being just a Turkish problem, notably in Istanbul, the challenge of Italian informal housing is highlighted within the context of international building code non-compliance. 01 april 2023, Item does not contain fulltext, BACKGROUND: For descending thoracic aortic aneurysms (TAA) in proximity of the aortic arch, debranching of the left subclavian artery (LSA) may be necessary to extend proximal sealing in zone 2. The aim of this study was to determine the added proximal apposition length gained from LSA debranching during thoracic endovascular aortic repair (TEVAR). METHODS: This multicenter retrospective study (2010-2020) included patients who underwent elective TEVAR in zone 2 for a degenerative TAA where the LSA was surgically debranched. The endograft position on the first postoperative computed tomography angiography (CTA) scan was assessed using post-processing software. The analysis included the shortest apposition length (SAL), the tilt of the proximal edge of the endograft, and the distance between the endograft and the left common carotid artery. Clinical endpoints (neurological complications and endoleaks) at 30 days were also reported. RESULTS: Twenty-two patients were included. The median interval between TEVAR and the first postoperative CTA was 3 days (2-10 days). Median SAL was 9.2 mm (1.3-26.4 mm), of which 8.6 mm (1.3-16.2 mm) was gained proximal of the LSA, including the LSA orifice. In 12 patients (55.5%) the SAL was <10 mm. The median tilt was 18.3° (13.9°-22.2°). Seven endoleaks were reported on the first CTA: 1 type Ia, 2 type Ib, 3 type II, and 1 type III. CONCLUSIONS: Debranching the LSA adds valuable sealing length in zone 2, but the SAL was still relatively short in many patients, putting these patients at risk for a future type Ia endoleak. Accurate assessment of the circumferential apposition on postoperative CTA follow-up in these high-risk patients with short, complex landing zones seems mandatory. Evaluation of apposition in a larger population with longer follow-up is advised.

Abstract

The European Union (EU) recently ratcheted its climate ambition to net-zero emissions by 2050, with a milestone of 55% emissions cuts in 2030. This study carries out a model inter-comparison to assess the EU's path, from “Fit for 55” in 2030 to an intermediate milestone in 2040 and onto net zero in 2050, offering insights at sectoral and member-state levels. Our model results support the bloc's ambition for its Emissions Trading System and Effort Sharing Regulation sectors while pointing to the need for near-complete decarbonization of electricity by 2040, enabled by considerable deployment of renewables (45%–65% in 2030, to 60%–70% in 2040, and to 75%–90% in 2050 in electricity generation) and carbon capture and storage (0.5–2 GtCO2/year by 2050). We also highlight the trade-offs between supply-side and harder-to-abate sectors, assess the ambition of member states for net zero and timing of coal phaseout, and reflect on the economic implications of investment, technical, and policy needs. © 2023 The Author(s)

Abstract

Pelvic exenteration is a complex operation performed for locally advanced and recurrent pelvic cancers. The goal of surgery is to achieve clear margins, therefore identifying adjacent or involved organs, bone, muscle, nerves and/or vascular structures that may need resection. While these extensive resections are potentially curative, they can be associated with substantial morbidity. Recently, there has been a move to centralize care to specialized units, as this facilitates better multidisciplinary care input. Advancements in pelvic oncology and surgical innovation have redefined the boundaries of pelvic exenterative surgery. Combined with improved neoadjuvant therapies, advances in diagnostics, and better reconstructive techniques have provided quicker recovery and better quality of life outcomes, with improved survival This article provides highlights of the current management of advanced pelvic cancers in terms of surgical strategy and potential future developments.

Abstract

The goal of MATISSE is to develop and implement technologies needed for successfully detecting and quantifying hazards connected with geo-energy operations in the sub-surface, in particular induced seismicity. Anthropogenic earthquakes constitute one of the major environmental impacts associated with geo-resources exploitation. Injection induced events are an undesired dynamic rockmass response to technological processes. Water injection operations taking place during industrial activities such as oil, gas and geothermal exploitation often induce microseismic activity and, under specific circumstances, reactivate existing faults, causing events of considerable size. These industrial activities can also determine air pollution and ground water contamination.Induced seismic hazard is evaluated through the computation and update of ground motion prediction equations, the time-lapse of velocity, attenuation and seismic noise tomography and the computation of pore pressure time variations. All these parameters are correlated with operational activities. Abstract

The current work has the royal site of the Granja de San Ildefonso as object research, near to the city of Segovia in Spain. In particular, this research in-vestigates the historical evolution of the royal site to compare it with the Jardines du château de Versailles and the Château de Marly with an archival approach. The royal site of the Granja de San Ildefonso was a farmhouse of Jeronimo’s monks and only later it was associated to Anjou’s Duke, Felipe V. The Duke who later became King of Spain, chosed this place to build his palace with a royal garden based on the model of French gardens with a ge-ometric grid. Nevertheless, the orography of this place forced architects to modify the typical model of French garden. In the beginning, this paper in-vestigates the structure of the park and its historical evolution through the comparisons between the historical cartography (1916) and the current state. Later, the structure of the royal site of the Granja de San Ildefonso was com-pared to that of the royal sites of Versailles and of Marly. The methodologies used allowed us to understand the morphological differences over the centu-ries and identify what the Granja de San Ildefonso and the typical French gardens differ.
