Background Adaptive optics scanning light ophthalmoscopy (AOSLO) enables direct visualisation of the cone mosaic, with metrics such as cone density and cell spacing used to assess the integrity or health of the mosaic. Here we examined the interobserver and inter-instrument reliability of cone density measurements. Methods For the interobserver reliability study, 30 subjects with no vision-limiting pathology were imaged. Three image sequences were acquired at a single parafoveal location and aligned to ensure that the three images were from the same retinal location. Ten observers used a semiautomated algorithm to identify the cones in each image, and this was repeated three times for each image. To assess inter-instrument reliability, 20 subjects were imaged at eight parafoveal locations on one AOSLO, followed by the same set of locations on the second AOSLO. A single observer manually aligned the pairs of images and used the semiautomated algorithm to identify the cones in each image. Results Based on a factorial study design model and a variance components model, the interobserver study's largest contribution to variability was the subject (95.72%) while the observer's contribution was only 1.03%. For the inter-instrument study, an average cone density intraclass correlation coefficient (ICC) of between 0.931 and 0.975 was calculated. Conclusions With the AOSLOs used here, reliable cone density measurements can be obtained between observers and between instruments. Purpose: Evidence suggests that smoking is detrimental to the survival of dental implants placed in grafted maxillary sinuses. Studies have shown that improving bone quantity and quality, using rough-surfaced implants, and practicing good oral hygiene may improve outcomes. In this prospective study, the long-term survival rates of implants placed simultaneously with sinus grafting in smokers and nonsmokers were compared. Materials and Methods: Implants with roughened surfaces were immediately placed into maxillary sinus grafts in patients with 1 to 7 mm of residual bone. A total of 2,132 simultaneous implants were placed into the grafted sinuses of 226 smokers (627 implants) and 505 nonsmokers (1,505 implants). A majority of the patients received a composite graft consisting of 50% autogenous bone. In both smokers and nonsmokers, approximately two thirds of the implants had microtextured surfaces; the remainder had hydroxyapatite-coated surfaces. The implants were restored and monitored during clinical follow-up for up to 9 years. Results: Cumulative survival of implants at 9 years was 97.9%. There were no statistically significant differences in implant failure rates between smokers and nonsmokers. Discussion: Implant survival was believed to depend on the following aspects of the technique used: creation of a large buccal window to allow access to a large recipient site; use of composite grafts consisting of at least 50% autogenous bone; meticulous bone condensation; placement of long implants (ie, 15 mm); use of implants with hydroxyapatite-coated or microtextured surfaces; use of a membrane to cover the graft and implants; antibiotic use and strict oral hygiene; use of interim implants and restricted use of dentures; and adherence to a smoking cessation protocol. PURPOSE: One-stage implant placement in the grafted maxillary sinus has traditionally been limited to patients with at least 5 mm of residual bone to ensure complete implant stabilization. The aim of this prospective study was to determine the long-term survival rates of implants with roughened surfaces placed immediately into maxillary sinus grafts in patients with 1 to 5 mm of residual bone. MATERIALS AND METHODS: A total of 2132 microtextured screw-type (n=1374) or hydroxyapatite-coated cylinder-type (n=758) implants were immediately placed into the grafted sinuses of 731 patients. The implants were restored and monitored for up to 9 years of clinical follow-up. RESULTS: Cumulative survival at 9 years was 97.9% (n=2091 implants); 20.4% of the implants were placed in 1 to 2 mm of residual bone. Discussion: Initial implant stability and parallelism were achieved through a combination of meticulous condensation of the particulate bone graft material around the implants, the frictional interface of the roughened implant surfaces and the host tissues, and selection of an appropriate graft material. CONCLUSIONS: Simultaneous implant placement into sinus floor grafts can be a predictable treatment option for patients with at least 1 to 2 mm of vertical residual bone height when careful case planning and meticulous surgical techniques are used. PURPOSE: Several nerve repositioning techniques have ben presented in the literature, each with limitations. This article presents a new technique involving the use of 2 osteotomies, with minimizes particularly the potential duration of sensory disruption and the risk of nerve paresthesia and inadvertent nerve transection or compression. MATERIALS AND METHODS: Ten patients ranging in age from 47 to 67 years were selected for nerve lateralization utilizing the modified technique. A total of 23 cylindrical implants were placed. An average follow-up period was 29.8 months. RESULTS: Of the 10 patients, 4 experienced total return of sensation within 3 to 4 weeks. One patient experienced complete recovery at 6 weeks. DISCUSSION: Creating 2 osteotomies as described minimizes the chances for postoperative neuropraxia and nerve paresthesia or anesthesia. CONCLUSION: When there is moderate-to-severe bone resorption of the mandible posterior to the mental foramen, repositioning the inferior alveolar nerve using both an anterior and posterior osteotomy allows for more bone to accommodate ideal placement and greater length of implant. Paranasal sinus mucoceles are benign, locally expansile cyst-like masses that are filled with mucus and lined with epithelium. Most occur in the frontal sinus. Maxillary sinus mucoceles are presumably uncommon in the United States and European countries, although they have been frequently reported in Japan, particularly following Caldwell-Luc surgery. Clinical symptoms may not appear for at least 10 years postoperatively. Chronic sinus inflammation and allergic disease are also common causes of paranasal mucoceles. This paper provides an overview of maxillary sinus mucoceles and presents a case study involving a 62-year-old Latin male whose asymptomatic maxillary sinus mucocele was not revealed until he presented for maxillary sinus grafting and implant placement. This study assessed the efficacy of augmentation grafting of the maxillary sinus with simultaneous placement of dental implants in patients with less than 5 mm of alveolar crestal bone height in the posterior maxilla prior to grafting, although the procedure has traditionally been contraindicated based on empirical data. A total of 160 hydroxyapatite-coated implants was placed into 63 grafted maxillary sinuses in 63 patients whose crestal bone height in this region ranged from 3 to 5 mm. Patients were followed for 2 to 4 years after the placement of definitive prostheses. There were no postoperative sinus complications. Following uncovering of the implants at 9 months after surgery, there was no clinical or radiographic evidence of crestal bone loss around the implants. Histologic examination of bone cores from the grafted sites revealed successful integration and a high degree of cellularity. All patients maintained stable implant prostheses during follow-up. These findings indicate that the single-step procedure is a feasible option for patients with as little as 3 mm of alveolar bone height prior to augmentation grafting, utilizing hydroxyapatite-coated implants and autogenous bone. Herein, conversion of nitrate (NO 3 ̅ ) to ammonia (NH 3 ) up to ∼100 mM and in-situ cascade CO 2 co-reduction to urea (up to ∼1.36 mM) has been demonstrated by thermally activated hydrogen peroxide (H 2 O 2 ) at near ambient conditions (∼100 °C). NH 3 synthesis from NO 3 ̅ has been confirmed using absorption, 1 H nuclear magnetic resonance (NMR) analysis of the reduced product and 15 N isotopic labeled nitratre ( 15 NO 3 ̅ ) experiement. The results associated with the urea synthesis have been verified using absorption, 1 H NMR, and high resolution-mass spectrometry (HR-MS) analysis.

Herein, a simple ambient conditioned sunlight promoted photochemical reduction reaction is demonstrated for the of nitrate (NO 3 - ) conversion to ammonia (NH 3 ) with the maximum conversion yield of ∼16 mM using iron filings (f-Fe) in the presence of H 2 O 2 . Based on a radical scavenging study of reactive species and the characterization of catalyst f-Fe before and after the reaction, a plausible mechanism has been proposed for the ambient conditioned synthesis of NH 3 . The results associated with the NH 3 synthesis have been verified using the 15 N isotopic labeled nitrate ( 15 NO 3 - ), which supports the simpler viability of the reported procedure.

Purpose: To explore the perceived utility and effect of simplified radiology reports on oncology patients' knowledge and feasibility of large language models (LLMs) to generate such reports., Materials and Methods: This study was approved by the Institute Ethics Committee. In phase I, five state-of-the-art LLMs (Generative Pre-Trained Transformer-4o [GPT-4o], Google Gemini, Claude Opus, Llama-3.1-8B, and Phi-3.5-mini) were tested to simplify 50 oncology computed tomography (CT) report impressions using five distinct prompts with each LLM. The outputs were evaluated quantitatively using readability indices. Five LLM-prompt combinations with best average readability scores were also assessed qualitatively, and the best LLM-prompt combination was selected. In phase II, 100 consecutive oncology patients were randomly assigned into two groups: original report (received original report impression) and simplified report (received LLM-generated simplified versions of their CT report impressions under the supervision of a radiologist). A questionnaire assessed the impact of these reports on patients' knowledge and perceived utility., Results: In phase I, Claude Opus-Prompt 3 (explain to a 15-year-old) performed slightly better than other LLMs, although scores for GPT-4o, Gemini, Claude Opus, and Llama-3.1 were not significantly different ( P > .0033 on Wilcoxon signed-rank test with Bonferroni correction). In phase II, simplified report group patients demonstrated significantly better knowledge of primary site and extent of their disease as well as showed significantly higher confidence and understanding of the report ( P < .05 for all). Only three (of 50) simplified reports required corrections by the radiologist., Conclusion: Simplified radiology reports significantly enhanced patients' understanding and confidence in comprehending their medical condition. LLMs performed very well at this simplification task; therefore, they can be potentially used for this purpose, although there remains a need for human oversight.

Purpose of Review: To review the literature evaluating the effectiveness of cataract surgery alone and cataract surgery in combination with other procedures and surgeries on intraocular pressure (IOP) in patients with ocular hypertension and glaucoma., Recent Findings: Recent studies of large trials have shown the IOP-lowering effect of cataract surgery and the beneficial effect of minimally invasive glaucoma surgery (MIGS). More studies are being published on when to use cataract surgery alone, with MIGS, or with traditional glaucoma surgeries for both primary open angle glaucoma and primary angle closure glaucoma., Summary: Patients with ocular hypertension and visually significant cataracts would benefit from cataract surgery alone to lower intraocular pressure. Patients with mild to moderate glaucoma would likely benefit from cataract surgery and MIGS to achieve a lower IOP. Patients with more advanced glaucoma would benefit from cataract surgery combined with a traditional incisional glaucoma surgery. Clear lens extraction can be used in patients with primary angle closure and early primary angle closure glaucoma. In patients with more advanced disease, phacoemulsification and another glaucoma surgery is likely to be required to achieve IOP control. Primary hyperparathyroidism (PHPT) is relatively uncommon in children, and skeletal deformities due to it are even rarer in this age group. Less than 20 such cases have been reported in the English literature. We describe a case series of three patients who presented with genu valgum deformity and were found to have primary hyperparathyroidism on further evaluation. The cases were primarily managed by removing the adenoma and later taken up for correction of the skeletal deformity. Antibody therapy for HIV-1 infection exerts two broad effects: a drug-like, antiviral effect, which rapidly lowers the viral load, and a vaccinal effect, which may control the viral load long-term by improving the immune response. Here, we elucidate a trade-off between these two effects as they pertain to the humoral response, which may compromise antibody therapy aimed at eliciting long-term HIV-1 remission. We developed a multi-scale computational model that combined within-host viral dynamics and stochastic simulations of the germinal centre (GC) reaction, enabling simultaneous quantification of the antiviral and vaccinal effects of antibody therapy. The model predicted that increasing antibody dosage or antibody-antigen affinity increased immune complex formation and enhanced GC output. Beyond a point, however, a strong antiviral effect reduced antigen levels substantially, extinguishing GCs and limiting the humoral response. We found signatures of this trade-off in clinical studies. Accounting for the trade-off could be important in optimizing antibody therapy for HIV-1 remission.

The subscapularis (SSC) muscle is a crucial anterior glenohumeral stabilizer and internal rotator of the shoulder joint. The partial tears of the SSC might result from traumatic injury or intrinsic degeneration. Partial SSC tears can range in severity and be classified into different categories based on the location of the tear, size of the lesion, and associated pathology. The tear usually begins from the superolateral margin in the first facet and propagates downwards. It is frequently associated with biceps pathology or anterosuperior lesions. These tears are now increasingly recognized as distinct pathology that requires specific diagnostic and management approaches. The current management approaches are shifting towards operative, as partial SSC tears are increasingly recognized as a distinct pathology. At present, there is no consensus regarding the timing of repair, but the relative tendency of the SSC to retract much faster than other rotator cuff muscles, and difficulty in mobilization, advocates an early repair for SSC irrespective of the lesion size. An associated biceps pathology can be treated with either tenotomy (biceps delamination/erosion) or tenodesis. The techniques of partial SSC repair are constantly improving. There is no reported difference in use of 2-anchor-based conventional single-row (SR), a 3-anchor-based interconnected double-row technique, or a 2-anchor-based interconnected hybrid double-row construct in the repair construct. However, the 2-anchor-based interconnected double-row provides an advantage of better superolateral coverage with leading-edge protection, as it helps in placing the superolateral anchor superior and lateral to the original footprint. A timely intervention and restoration of the footprint will help restore and rehabilitate the shoulder. Objective: To determine the proportion of latent iron deficiency (LID) in neonates delivered at more than 34 weeks gestation with and without risk factors for in-utero iron deficiency., Method: This observational study enrolled neonates delivered at more than 34 weeks gestation. Maternal antenatal history, risk factors for placental insufficiency, hemoglobin, and serum ferritin levels were collected upon admission for confinement. Neonates were categorized based on the presence of risk factors, and further categorized based on ferritin concentrations (cord blood or venous blood < 72 hours) into those with ferritin > 75 ng/mL (normal iron status) and those with ferritin between 11-75 ng/mL (LID)., Results: Among 559 neonates, 295 had underlying risk factors for poor in-utero iron status. Overall, 45 (9.8%) neonates had LID with an incidence of 10.5% in those with risk factors and 5.3% in those without risk factors [Odds Ratio (95% CI) 2.09 (1.08, 4.03); P value = 0.02]. Maternal anemia and gestational hypertension were found to be significant independent risk factors for LID., Conclusion: This study highlights the higher LID in at-risk neonates, emphasizing the need for targeted interventions to prevent iron deficiency.

Free non-vascularised fibular grafts are an autologous grafting option that requires a simple technique and minimal expertise to replicate. However, due to limited intrinsic biological activity, they are less preferred than their vascularised counterparts for reconstructing large bone defects. We present a case of a female child in her middle childhood with limb length discrepancy due to extensive femoral diaphyseal bone loss following successful treatment of chronic osteomyelitis of the femur. In this case, successful reconstruction of the femoral diaphysis was achieved using a free non-vascularised fibular graft, resulting in a good range of motion around the knee with no graft donor site morbidity at the latest follow-up.Therefore, we conclude that with adequate soft tissue coverage and vascularity for large bone defects, even a free, non-vascularised fibular graft can yield promising results with complete union and hypertrophy at the graft host junction., Competing Interests: Competing interests: None declared., (© BMJ Publishing Group Limited 2024. Introduction: The two techniques to fix a torn ACL are the Transportal and All-Inside Techniques. However, the argument about which method is better for functional outcomes keeps going on. The aim was to look at how well these two treatments worked and their effects. We hypothesized that All-Inside and Transportal ACL reconstruction procedures would yield similar functional results., Materials and Methods: The study included 45 patients with ACL insufficiency from AIIMS, Raipur, India. Half of each ACL approach was represented by 22 AI and 23 TP participants, randomly assigned to each group. Preoperative and post-operative VAS, IKDC, and range of motion were determined at 1, 3, and 6 months. The study used IBM SPSS v20 for Data Analysis, comparing two groups using a t-test. P < 0.05 was fixed for Statistical significance., Results: The study was dominated by young males (86.7%, 18-30 years), mainly students (44.4%). Comparable IKDC scores over time (P = 0.628); significant VAS score difference (P = 0.001). A similar movement range (P = 0.153) was observed. AI group had slightly smaller grafts (non-significant, P = 0.235). Similar Lachmann Grade distributions pre- and post-surgery; no significant post-operative complication in either group., Conclusion: When comparing the methods' functionalities, no statistically significant gaps emerged. All-Inside approach preserves bone and gracilis tendon, reduces post-operative pain, and strengthens knee flexors. Although, early rehabilitation favors all inside, both methods are equally effective over time. Purpose: To investigate the impact of surgical approach and post-operative prophylaxis on heterotopic ossification (HO) development after total hip arthroplasty (THA)., Methods: A retrospective analysis of 312 patients who underwent THA between January 2009 and April 2016. Patients were categorized by surgical approach (direct lateral or posterolateral), prosthesis type, and post-operative prophylaxis (Etoricoxib 60 mg daily for two weeks). Two orthopaedic surgeons independently assessed radiographs at serial intervals, and HO was graded as per Brooker classification. Bivariate analysis and regression modelling were performed to assess the associations and confounding effects of different variables, RESULTS: Bivariate analysis identified factors correlated with higher HO incidence: absence of prophylaxis, older age, longer symptom evolution, and lower pre-surgery physical activity. Regression modelling showed a correlation between the direct-lateral approach, post-operative prophylaxis, symptom evolution, and higher HO incidence., Conclusion: Patients with longer symptom evolution before surgery and without post-operative prophylaxis are at higher risk of developing HO. While the direct lateral approach showed higher HO rates, the difference was insignificant. A two-week prophylactic regimen of Etoricoxib 60 mg daily after THA effectively reduced HO formation. Purpose: To evaluate outer retinal organization in normal subjects and those using hydroxychloroquine (HCQ) with ultrahigh resolution visible light optical coherence tomography (VIS-OCT)., Methods: Forty eyes of 22 adult subjects were recruited from a tertiary care retina practice including controls (20 eyes, 12 subjects, mean age 40±22yrs, mean logMAR BCVA 0.19, 90% female) and subjects with a history of HCQ use (20 eyes, 10 subjects, mean age 62±17yrs, mean logMAR BCVA 0.03, 67% female). Each subject was imaged using a custom-built VIS-OCT device (axial resolution 1.3μm) and FDA-approved OCT devices., Results: Using VIS-OCT, control subjects demonstrate 5 and 6 hyperreflective bands in the foveal and parafoveal regions, respectively, between the outer nuclear layer and Bruch's membrane. These bands demonstrate intensity profiles complementary to the known histopathologic distribution of rods and cones. In comparison to controls, subjects taking HCQ demonstrate blunting of all bands, particularly bands 2-4. In all cases of suspected or known toxicity, VIS-OCT demonstrated attenuation of band 3i and in no cases was there attenuation of other bands that was more severe than band 3i, suggesting that changes in the reflectivity of Band 3i may be the earliest identifiable sign of HCQ toxicity., Conclusions: VIS-OCT of the outer retina demonstrates a unique outer retinal banding pattern corresponding to photoreceptor density profiles. There is a notable attenuation of the photoreceptor outer segment reflectivity profile associated with early HCQ toxicity. This finding may be an early, and possibly reversible, sign of HCQ toxicity, primarily impacting the cones.

Objective  The aim of this study was to assess efficacy of large language models (LLMs) for converting free-text computed tomography (CT) scan reports of head and neck cancer (HNCa) patients into a structured format using a predefined template. Materials and Methods  A retrospective study was conducted using 150 CT reports of HNCa patients. A comprehensive structured reporting template for HNCa CT scans was developed, and the Generative Pre-trained Transformer 4 (GPT-4) was initially used to convert 50 CT reports into a structured format using this template. The generated structured reports were then evaluated by a radiologist for instances of missing or misinterpreted information and any erroneous additional details added by GPT-4. Following this assessment, the template was refined for improved accuracy. This revised template was then used for conversion of 100 other HNCa CT reports into structured format using GPT-4. These reports were then reevaluated in the same manner. Results  Initially, GPT-4 successfully converted all 50 free-text reports into structured reports. However, there were 10 places with missing information: tracheostomy tube ( n  = 3), noninclusion of involvement of sternocleidomastoid muscle ( n  = 2), extranodal tumor extension ( n  = 3), and contiguous involvement of the neck structures by nodal mass rather than the primary ( n  = 2). Few instances of nonsuspicious lung nodules were misinterpreted as metastases ( n  = 2). GPT-4 did not indicate any erroneous additional findings. Using the revised reporting template, GPT-4 converted all the 100 CT reports into a structured format with no repeated or additional mistakes. Conclusion  LLMs can be used for structuring free-text radiology reports using plain language prompts and a simple yet comprehensive reporting template. Key Points Structured radiology reports in oncological patients, although advantageous, are not used widely in practice due to perceived drawbacks like interference with routine radiology workflow and scan interpretation.We found that GPT-4 is highly efficient in converting conventional CT reports of HNCa patients to structured reports using a predefined template.This application of LLMs in radiology can help in enhancing the acceptability and clinical utility of structured radiology reports in oncological imaging. The American Radium Society (ARS) Central Nervous System (CNS) committee reviewed literature on epidermal growth factor receptor mutated (EGFRm) and ALK-fusion (ALK+) tyrosine kinase inhibitors (TKIs) for the treatment of brain metastases (BrMs) from non-small cell lung cancers (NSCLC) to generate appropriate use guidelines addressing use of TKIs in conjunction with or in lieu of radiotherapy (RT). The panel developed three key questions to guide systematic review: can radiotherapy be deferred in patients receiving EGFR or ALK TKIs at (1) diagnosis or (2) recurrence? Should TKI be administered concurrently with RT (3)? Two literature searches were performed (May 2019 and December 2023). The panel developed 8 model cases and voted on treatment options using a 9-point scale, with 1-3, 4-6 and 7-9 corresponding to usually not appropriate, may be appropriate, and usually appropriate (respectively), per the UCLA/RAND Appropriateness Method. Consensus was achieved in only 4 treatment scenarios, all consistent with existing ARS-AUC guidelines for multiple BrM. The panel did not reach consensus that RT can be appropriately deferred in patients with BrM receiving CNS penetrant ALK or EGFR TKIs, though median scores indicated deferral may be appropriate under most circumstances. Whole brain RT with concurrent TKI generated broad disagreement except in cases with 2-4 BrM, where it was considered usually not appropriate. We identified no definitive studies dictating optimal sequencing of TKIs and RT for EGFRm and ALK+ BrM. Background Managing intertrochanteric fractures presents challenges for orthopedic surgeons, not only in fixing the fracture but also in preventing and managing associated complications, especially in the vulnerable geriatric population. Cephalomedullary nails are commonly used for surgical fixation due to their favorable functional profile, which preserves the hip's abductor lever arm and proximal femur anatomy. However, there's a lack of data comparing two major options: proximal femoral nail (PFN) and proximal femoral nail anti-rotation (PFNA). This study aimed to compare the radiological fracture reduction and fixation as well as functional outcomes of these two implants in treating intertrochanteric fractures. Methods The study, spanning 24 months, involved a prospective comparative design. Participants included patients diagnosed with intertrochanteric femur fractures classified as AO Type 31 A1, AO Type 31 A2, and AO Type 31 A3. Fifty patients were evenly distributed into PFN and PFNA groups. Preoperatively, clinical and radiological assessments were conducted, along with serum vitamin D level measurements. Surgeries, performed under anesthesia with image intensifier guidance, followed defined reduction and implant insertion protocols for each group. Postoperatively, evaluations were conducted up to six months, examining parameters such as tip-apex distance (TAD), Cleveland index, and modified Harris hip score, while documenting intraoperative duration and blood loss. Data analysis utilized the statistical software Statistical Package for Social Sciences (SPSS), version 22.0 (IBM Corp., Armonk, NY), employing descriptive statistics, chi-square tests, independent t-tests, and paired t-tests, with significance set at p < 0.05. Results In our study, 50 patients were enrolled, with equal gender distribution (64.0% male, 36.0% female, p=1.000). The mean ages in the PFN and PFNA groups were 66.2 ± 9.8 years and 66.4 ± 11.3 years, respectively (p=0.936). All fractures united by six months, with no implant-related complications reported. PFNA showed significantly lower blood loss and shorter surgery durations (p<0.001). TAD and neck shaft angle were similar between groups (p=0.826, p=0.555). Cleveland index placement and modified Harris hip score improvement were comparable (p=0.836, p<0.001). Predominant vitamin D deficiency was observed in both groups. Conclusion PFNA offers measurable intraoperative benefits over conventional PFN in terms of operative time, blood loss, and need for fluoroscopic imaging. Purpose of Review: To review the literature evaluating systemic medications for treatment of sickle cell disease (SCD) and their applications for sickle cell retinopathy., Recent Findings: Prior studies have demonstrated the efficacy of traditional systemic therapies in reducing the risk of development of sickle cell retinopathy. Since 2017, several new and promising disease-modifying therapies for sickle cell disease have been approved for clinical use, including the first genetic therapies such as exagamglogene autotemcel (exa-cel) and lovotibeglogene autotemcel (lovo-cel). These treatments have shown promising results for systemic management but are not widely utilized due to limited access and high cost. The efficacy of these therapies for the prevention of sickle cell retinopathy remains unknown and opens the door to new avenues for research. Furthermore, the role of systemic therapy for the management of hemoglobin SC (HbSC) disease, which has milder systemic effects but higher likelihood of causing retinopathy, remains poorly understood., Summary: Hydroxyurea has been a mainstay of systemic management of SCD with prior work suggesting its ability to reduce the likelihood of developing retinopathy. There are several new and potentially curative systemic therapies for SCD, though their role in retinopathy prevention and management has not been studied extensively. Purpose: With expansion of academic cancer center networks across geographically-dispersed sites, ensuring high-quality delivery of care across all network affiliates is essential. We report on the characteristics and efficacy of a radiation oncology peer-review quality assurance (QA) system implemented across a large-scale multinational cancer network., Methods and Materials: Since 2014, weekly case-based peer-review QA meetings have been standard for network radiation oncologists with radiation oncology faculty at a major academic center. This radiotherapy (RT) QA program involves pre-treatment peer-review of cases by disease site, with disease-site subspecialized main campus faculty members. This virtual QA platform involves direct review of the proposed RT plan as well as supporting data, including relevant pathology and imaging studies for each patient. Network RT plans were scored as being concordant or nonconcordant based on national guidelines, institutional recommendations, and/or expert judgment when considering individual patient-specific factors for a given case. Data from January 1, 2014, through December 31, 2019, were aggregated for analysis., Results: Between 2014 and 2019, across 8 network centers, a total of 16,601 RT plans underwent peer-review. The network-based peer-review case volume increased over the study period, from 958 cases in 2014 to 4,487 in 2019. A combined global nonconcordance rate of 4.5% was noted, with the highest nonconcordance rates among head-and-neck cases (11.0%). For centers that joined the network during the study period, we observed a significant decrease in the nonconcordance rate over time (3.1% average annual decrease in nonconcordance, P = 0.01); among centers that joined the network prior to the study period, nonconcordance rates remained stable over time., Conclusions: Through a standardized QA platform, network-based multinational peer-review of RT plans can be achieved. Purpose of Review: To review the literature evaluating the effect of cataract surgery on intraocular pressure (IOP) in patients with glaucoma., Recent Findings: Recent high-quality secondary analyses of large and primary trials continue to show IOP lowering following cataract surgery. Likewise, cataract surgery remains a key treatment for angle closure glaucoma. Some micro-invasive glaucoma surgeries (MIGS) have strong evidence to be performed at the time of cataract surgery. Data clarifying when these surgeries should be combined with cataract surgery is emerging. The mechanism underlying IOP lowering after cataract surgery remains unclear., Summary: Patients who are glaucoma suspects with visually significant cataracts would benefit from cataract surgery alone. Those with mild-moderate damage on 1-2 classes of medications would most likely benefit from additional MIGS. Patients with advanced disease would benefit from cataract surgery and a choice of additional surgery, which depends on disease status and patient factors. Clear lens extraction is becoming a more accepted practice as a primary procedure for patients with angle closure and high IOP or glaucoma. Background: India has a population capacity of 1.2 billion people. With a worldwide aging population, the prevention and management of osteoporosis has become a significant healthcare challenge. It is crucial to recognize the factors impacting poor bone health and appreciate the many hurdles to treat the disease to provide a better quality of life and decrease the financial burden on healthcare., Purpose: In this review article, we discuss the epidemiology of osteoporosis and address the definition, incidence, and prevalence, geographical variation in the occurrence of osteoporosis, and the risk factors for osteoporosis., Methods: Search terms using various combinations of the keywords 'osteoporosis,' 'epidemiology,' 'incidence,' 'prevalence,' 'fracture,' 'India,' 'world,' 'screening,' and 'FRAX' was done to review all relevant literature till June 2023., Results & Conclusion: India has a recent estimate of population capacity of 1.2 billion people. Various studies have revealed an osteoporosis prevalence in Indian women ranging from 8% to 62%. Introduction: Pipkin type 4 fracture is defined as hip dislocation with femoral head fracture and concomitant acetabular rim fracture. These fractures are rare. Neglected fractures develop adaptive changes in the acetabulum, femoral head, and soft tissues around the hip joint, leading to hip arthritis. Total hip arthroplasty is preferred in such cases. After an extensive literature review, we did not find any guidelines for managing neglected Pipkin type 4 fractures., Case Report: A 47-year-old male presented with pain in his left hip and an inability to bear weight on his left lower limb. He had a road traffic accident 1 year ago. On clinical examination, there was a shortening of 3 cm and reduced hip range of motion on 3D computed tomography posterosuperior defect of the acetabular wall along with Pipkin type 4 femoral head fracture was detected. Total hip arthroplasty with acetabular reconstruction using a femoral head autograft was done. At 1 year of follow-up, the patient was pain-free and could walk without any assistance with a normal gait., Conclusion: Uncemented total hip arthroplasty with an autologous structural femoral head graft is a suitable method of treatment for neglected Pipkin type 4 fractures. Stereotactic radiation therapy yields high rates of local control for brain metastases, but patients in rural or suburban areas face geographic and socioeconomic barriers to its access. We conducted a phase II clinical trial of frameless, fractionated stereotactic radiation therapy for brain metastases in an integrated academic satellite network for patients 18 years of age or older with 4 or fewer brain metastases. Dose was based on gross tumor volume: less than 3.0 cm, 27 Gy in 3 fractions and 3.0 to 3.9 cm, 30 Gy in 5 fractions. Median follow-up was 10 months for 73 evaluable patients, with a median age of 68 years. Median intracranial progression-free survival was 7.1 months (95% confidence interval = 5.3 to not reached), and median survival was 7.2 months (95% confidence interval = 5.4 to not reached); there were no serious adverse events. Outcomes of this trial compare favorably with contemporary trials, and this treatment strategy provides opportunities to expand stereotactic radiation therapy access to underserved populations., (© The Author(s) 2023. Cadmium sulfide (CdS)-decorated, cross-linked melamine-formaldehyde polymer-based nanocomposite (MFP-CdS) has been synthesized. MFP-CdS is utilized here as a photoactive material for the photodegradation of six model organic dyes and their mixture in an aqueous medium in the presence of sunlight. The half-life values from the kinetic study of multiple dyes strongly support the importance of sunlight on the fast degradation of all six dyes compared to bulb light and control (dark) conditions. A comparative 1 H NMR analysis of the dyes and their degraded products has been performed to support the breakdown of the aromatic framework of organic dyes using MFP-CdS in sunlight. The mechanisms involved in the photodegradation of dyes have been investigated based on radical trapping studies that support the significant involvement of superoxide radicals along with holes. Moreover, the dye removal efficiency using MFP-CdS from real industrial wastewater samples is evaluated via the external spiking of organic dyes and their mixture in unknown industrial effluents where they showed similar photodegradation results. Based on the high recyclability of MFP-CdS, these are used for multiple cycles.

Purpose: To identify posterior cruciate ligament (PCL) peel-off lesions, to separate these lesions from more common midsubstance tears, and to evaluate patient outcomes after primary open repair., Methods: Patients with acute femoral-side "peel off"-type lesions associated with multiligamentous injuries who underwent PCL repair were identified. Patients with chronic PCL injuries, midsubstance PCL tears, or PCL tibial avulsions were excluded from the study. A total of 11 patients were included in this study. All patients underwent open repair using a suture pullout technique., Results: The mean follow-up period was 18 months. The mean Lysholm score at 12 months was 87. Mean knee range of motion (flexion) achieved at 12 months was 121°. Introduction: The prevalence of avascular necrosis (AVN) of the femoral head in sickle cell anemia is 50% whereas untreated cases lead to total hip replacement. A variety of B cell clones seed the germinal centers, where a selection stringency expands the fitter clones to generate higher affinity antibodies. However, recent experiments suggest that germinal centers often retain a diverse set of B cell clones with a range of affinities and concurrently carry out affinity maturation. Amid a tendency to flourish germinal centers with fitter clones, how several B cell clones with differing affinities can be concurrently selected remains poorly understood. Such a permissive selection may allow non-immunodominant clones, which are often rare and of low-affinity, to somatically hypermutate and result in a broad and diverse B cell response. How the constituent elements of germinal centers, their quantity and kinetics may modulate diversity of B cells, has not been addressed well. By implementing a state-of-the-art agent-based model of germinal center, here, we study how these factors impact temporal evolution of B cell clonal diversity and its underlying balance with affinity maturation. While we find that the extent of selection stringency dictates clonal dominance, limited antigen availability on follicular dendritic cells is shown to expedite the loss of diversity of B cells as germinal centers mature. Intriguingly, the emergence of a diverse set of germinal center B cells depends on high affinity founder cells. Our analysis also reveals a substantial number of T follicular helper cells to be essential in balancing affinity maturation with clonal diversity, as a low number of T follicular helper cells impedes affinity maturation and also contracts the scope for a diverse B cell response.
