Abstract: Core-collapse supernovae are among the most magnificent events in the observable universe. They produce many of the chemical elements necessary for life to exist and their remnants -- neutron stars and black holes -- are interesting astrophysical objects in their own right. However, despite millennia of observations and almost a century of astrophysical study, the explosion mechanism of core-collapse supernovae is not yet well understood. Hyper-Kamiokande is a next-generation neutrino detector that will be able to observe the neutrino flux from the next galactic core-collapse supernova in unprecedented detail. We focus on the first 500 ms of the neutrino burst, corresponding to the accretion phase, and use a newly-developed, high-precision supernova event generator to simulate Hyper-Kamiokande's response to five different supernova models. We show that Hyper-Kamiokande will be able to distinguish between these models with high accuracy for a supernova at a distance of up to 100 kpc. Once the next galactic supernova happens, this ability will be a powerful tool for guiding simulations towards a precise reproduction of the explosion mechanism observed in nature., Comment: 21 pages, 7 figures. Abstract: Hyper-Kamiokande is the next generation underground water Cherenkov detector that builds on the highly successful Super-Kamiokande experiment. The detector which has an 8.4~times larger effective volume than its predecessor will be located along the T2K neutrino beamline and utilize an upgraded J-PARC beam with 2.6~times beam power. Hyper-K's low energy threshold combined with the very large fiducial volume make the detector unique, that is expected to acquire an unprecedented exposure of 3.8~Mton$\cdot$year over a period of 20~years of operation. Hyper-Kamiokande combines an extremely diverse science program including nucleon decays, long-baseline neutrino oscillations, atmospheric neutrinos, and neutrinos from astrophysical origins. The scientific scope of this program is highly complementary to liquid-argon detectors for example in sensitivity to nucleon decay channels or supernova detection modes. Hyper-Kamiokande construction has started in early 2020 and the experiment is expected to start operations in 2027. The Hyper-Kamiokande collaboration is presently being formed amongst groups from 19 countries including the United States, whose community has a long history of making significant contributions to the neutrino physics program in Japan. Abstract: Item does not contain fulltext, The majority of neoantigens arise from unique mutations that are not shared between individual patients, making neoantigen-directed immunotherapy a fully personalized treatment approach. Novel technical advances in next-generation sequencing of tumor samples and artificial intelligence (AI) allow fast and systematic prediction of tumor neoantigens. This study investigates feasibility, safety, immunity, and anti-tumor potential of the personalized peptide-based neoantigen vaccine, EVX-01, including the novel CD8(+) T-cell inducing adjuvant, CAF®09b, in patients with metastatic melanoma (NTC03715985). The AI platform PIONEER(TM) was used for identification of tumor-derived neoantigens to be included in a peptide-based personalized therapeutic cancer vaccine. EVX-01 immunotherapy consisted of 6 administrations with 5-10 PIONEER(TM)-predicted neoantigens as synthetic peptides combined with the novel liposome-based Cationic Adjuvant Formulation 09b (CAF®09b) to strengthen T-cell responses. EVX-01 was combined with immune checkpoint inhibitors to augment the activity of EVX-01-induced immune responses. The primary endpoint was safety, exploratory endpoints included feasibility, immunologic and objective responses. This interim analysis reports the results from the first dose-level cohort of five patients. We documented a short vaccine manufacturing time of 48-55 days which enabled the initiation of EVX-01 treatment within 60 days from baseline biopsy. No severe adverse events were observed. EVX-01 elicited long-lasting EVX-01-specific T-cell responses in all patients. Competitive manufacturing time was demonstrated. EVX-01 was shown to be safe and able to elicit immune responses targeting tumor neoantigens with encouraging early indications of a clinical and meaningful antitumor efficacy, warranting further study.

Abstract: Contains fulltext : 220087.pdf (Publisher’s version ) (Closed access), BACKGROUND & AIMS: The safety and effectiveness of a home parenteral nutrition (HPN) program depends both on the expertise and the management approach of the HPN center. We aimed to evaluate both the approaches of different international HPN-centers in their provision of HPN and the types of intravenous supplementation (IVS)-admixtures prescribed to patients with chronic intestinal failure (CIF). METHODS: In March 2015, 65 centers from 22 countries enrolled 3239 patients (benign disease 90.1%, malignant disease 9.9%), recording the patient, CIF and HPN characteristics in a structured database. The HPN-provider was categorized as health care system local pharmacy (LP) or independent home care company (HCC). The IVS-admixture was categorized as fluids and electrolytes alone (FE) or parenteral nutrition, either commercially premixed (PA) or customized to the individual patient (CA), alone or plus extra FE (PAFE or CAFE). Doctors of HPN centers were responsible for the IVS prescriptions. RESULTS: HCC (66%) was the most common HPN provider, with no difference noted between benign-CIF and malignant-CIF. LP was the main modality in 11 countries; HCC prevailed in 4 European countries: Israel, USA, South America and Oceania (p < 0.001). IVS-admixture comprised: FE 10%, PA 17%, PAFE 17%, CA 38%, CAFE 18%. PA and PAFE prevailed in malignant-CIF while CA and CAFE use was greater in benign-CIF (p < 0.001). PA + PAFE prevailed in those countries where LP was the main HPN-provider and CA + CAFE prevailed where the main HPN-provider was HCC (p < 0.001). CONCLUSIONS: This is the first study to demonstrate that HPN provision and the IVS-admixture differ greatly among countries, among HPN centers and between benign-CIF and cancer-CIF. As both HPN provider and IVS-admixture types may play a role in the safety and effectiveness of HPN therapy, criteria to homogenize HPN programs are needed so that patients can have equal access to optimal CIF care.

A 1-year international survey was carried out to investigate whether the European Society for Clinical Nutrition and Metabolism clinical classification of CIF, based on the type and volume of the intravenous supplementation (IVS), could be an indicator of CIF severity. METHODS: At baseline, participating home parenteral nutrition (HPN) centres enrolled all adults with ongoing CIF due to non-malignant disease; demographic data, body mass index, CIF mechanism, underlying disease, HPN duration and IVS category were recorded for each patient. The type of IVS was classified as fluid and electrolyte alone (FE) or parenteral nutrition admixture (PN). The mean daily IVS volume, calculated on a weekly basis, was categorised as <1, 1-2, 2-3 and >3 L/day. The severity of CIF was determined by patient outcome (still on HPN, weaned from HPN, deceased) and the occurrence of major HPN/CIF-related complications: intestinal failure-associated liver disease (IFALD), catheter-related venous thrombosis and catheter-related bloodstream infection (CRBSI). RESULTS: Fifty-one HPN centres included 2194 patients. The analysis showed that both IVS type and volume were independently associated with the odds of weaning from HPN (significantly higher for PN <1 L/day than for FE and all PN >1 L/day), patients' death (lower for FE, p=0.079), presence of IFALD cholestasis/liver failure and occurrence of CRBSI (significantly higher for PN 2-3 and PN >3 L/day). CONCLUSIONS: The type and volume of IVS required by patients with CIF could be indicators to categorise the severity of CIF in both clinical practice and research protocols.

Abstract: Remodeling of the gut microbiota is implicated in various metabolic and inflammatory diseases of the gastrointestinal tract. We hypothesized that the gut microbiota affects the DNA methylation profile of intestinal epithelial cells (IECs) which could, in turn, alter intestinal function. Here, we used mass spectrometry and methylated DNA capture to respectively investigate global and genome-wide DNA methylation of intestinal epithelial cells from germ-free (GF) and conventionally raised mice (CONV-R). In colonic IECs from GF mice, DNA was markedly hypermethylated. This was associated with a dramatic loss of Ten-Eleven-Translocation activity, a lower DNA methyltransferase activity and lower circulating levels of the one carbon metabolite folate. At the gene level, we found an enrichment for differentially methylated regions at proximity of genes regulating cytotoxicity of natural killer (NK) cells (FDR < 8.9E-6), notably genes regulating the cross-talk between NK cells and target cells such as members of the natural killer group 2 member D ligand superfamily Raet. This distinct epigenetic signature was associated with a marked decrease in Raet1 expression and a loss of CD56+/CD45+ cells in the intestine of GF mice. Thus, our results indicate that altered activity of methylation modifying enzymes in GF mice influences the IEC epigenome and modulates the crosstalk between IECs and NK cells. Epigenetic reprogramming of IECs may modulate intestinal function in diseases associated with altered gut microbiota.

Abstract: Aim: To determine the genomic mechanisms by which adipose tissue responds to acute and chronic exercise. Methods:We profiled the transcriptomic and epigenetic response to acute exercise in human adiposetissue collected before and after endurance training. Results: Although acute exercises were performed at same relative intensities, themagnitude of transcriptomic changes after acute exercise was reduced by endurancetraining. DNA methylation remodeling induced by acute exercise was more prominent in trained versus untrained state. We found an overlap between gene expression and DNA methylation changes after acute exercise for 32 genes pre-training and six post-training, notably at adipocyte-specific genes. Conclusion: Training status differentially affects the epigenetic and transcriptomic response to acute exercise in human adipose tissue.

Abstract: Pan-specific prediction of receptor–ligand interaction is conventionally done using machine-learning methods that integrates information about both receptor and ligand primary sequences. To achieve optimal performance using machine learning, dealing with overfitting and data redundancy is critical. Most often so-called ligand clustering methods have been used to deal with these issues in the context of pan-specific receptor–ligand predictions, and the MHC system the approach has proven highly effective for extrapolating information from a limited set of receptors with well characterized binding motifs, to others with no or very limited experimental characterization. The success of this approach has however proven to depend strongly on the similarity of the query molecule to the molecules with characterized specificity using in the machine-learning process. Here, we outline an alternative strategy with the aim of altering this and construct data sets optimal for training of pan-specific receptor–ligand predictions focusing on receptor similarity rather than ligand similarity. We show that this receptor clustering method consistently in benchmarks covering affinity predictions, MHC ligand and MHC epitope identification perform better than the conventional ligand clustering method on the alleles with remote similarity to the training set. Abstract: High fat feeding impairs skeletal muscle metabolic flexibility and induces insulin resistance, whereas exercise training exerts positive effects on substrate handling and improves insulin sensitivity. To identify the genomic mechanisms by which exercise ameliorates some of the deleterious effects of high fat feeding, we investigated the transcriptional and epigenetic response of human skeletal muscle to 9 days of a high-fat diet (HFD) alone (Sed-HFD) or in combination with resistance exercise (Ex-HFD), using genome-wide profiling of gene expression and DNA methylation. HFD markedly induced expression of immune and inflammatory genes, which was not attenuated by Ex. Conversely, Ex markedly remodelled expression of genes associated with muscle growth and structure. We detected marked DNA methylation changes following HFD alone and in combination with Ex. Among the genes that showed a significant association between DNA methylation and gene expression changes were PYGM, which was epigenetically regulated in both groups, and ANGPTL4, which was regulated only following Ex. In conclusion, while short-term Ex did not prevent a HFD-induced inflammatory response, it provoked a genomic response that may protect skeletal muscle from atrophy. These epigenetic adaptations provide mechanistic insight into the gene-specific regulation of inflammatory and metabolic processes in human skeletal muscle.

33. A novel in vitro wound biofilm model used to evaluate low-frequency ultrasonic-assisted wound debridement

Abstract: OBJECTIVE: Bacterial biofilms remain difficult to treat. The biofilm mode of growth enables bacteria to survive antibiotic treatment and the inflammatory reaction. Low-frequency ultrasound has recently been shown to improve healing in a variety of settings. It is hypothesised that ultrasound disrupts the biofilm leaving bacteria more vulnerable to antiseptic or antibiotic treatment. The objective of this study is to develop a realistic model to elucidate the effect of ultrasound on biofilms.METHOD: A novel in vitro wound biofilm model was developed. Biofilms of Staphylococcus aureus were casted in a semi-solid agar gel composed of either tryptic soy broth (TSB) or a wound simulating media (WSM; composed of Bolton broth with blood and plasma), to resemble the non-surface attached aggregates. The model was used to evaluate the antibiofilm effect of an ultrasonic-assisted wound debridement device (UAW) in the presence of saline irrigation and treatment with a polyhexamethylene biguanide (PHMB)-containing antiseptic. Confocal microscopy was used to evaluate the effect of treatments on biofilm disruption and cell viability counting measured the antibacterial effects.RESULTS: Confocal microscopy showed that application of 10 seconds of moderate-intensity UAW could effectively disrupt semi-solid biofilms grown on both media settings. This treatment only had a small effect on the cell viability. A 24-hour treatment with PHMB was able to reduce the number of bacteria but not eradicate the biofilm in both media settings. Interestingly, the efficacy of the PHMB antiseptic was significantly higher when applied on biofilms grown in the more complex WSM media. However, we found a significant improvement in reducing the number of viable bacteria grown on both media when applying UAW before administration of the PHMB solution. Abstract: Objetivos: Describir los resultados del registro de nutrición enteral domiciliaria (NED) del grupo NADYA-SENPE del año 2010. Material y métodos: Se recopilaron los datos introducidos en el registro desde el 1 de enero al 31 de diciembre de 2010. Resultados: Se registraron 6.591 pacientes (51% varones) con 6.688 episodios de NED, procedentes de 32 hospitales. La edad media en los menores de 14 años (4%) fue de 1 ± 2 años (m ± DS) y de 69,9 ± 17,8 en los mayores de 14 años. El 76% de los pacientes recibieron la NED por un tiempo superior a 2 años. La patología más prevalente fue la neurológica 42%, seguida de la neoplasia 28% (en su mayoría cáncer de cabeza y cuello 18%). La información referente a la vía de acceso sólo se recogió en 626 casos (9,4%), el 51% de los pacientes utilizaron sonda nasogástrica, 27% gastrostomías, 10% vía oral y 3% yeyunostomías. Sólo 251 episodios finalizaron a lo largo del año, siendo el motivo más frecuente el fallecimiento del paciente en el 57% de los casos y el paso a la alimentación oral en el 14%. El 29% de los pacientes presentaban una actividad limitada y el 39% estaba confinado en cama/sillón. El 68% de los adultos requerían ayuda total o parcial. El suministro del producto se realizó desde el hospital o la farmacia de referencia en el 63% y 34%, respectivamente. El suministro del material fungible se realizó desde el hospital o atención primaria en el 83% y 16%, respectivamente. Conclusiones: Los resultados obtenidos en el registro de NED del año 2010 muestran características muy similares a las recogidas en los años previos en cuanto al número y características de los pacientes registrados. Seguimos encontrando problemas en la recogida de datos relativos a la vía de acceso y finalización de los episodios. Objective: To describe the results of the home enteral nutrition (HEN) registry of the NADYA-SENPE group in 2010. Material and methods: We retrieved the data of the patients recorded from January 1st to December 31st 2010. Results: We registered 6,591 patients (51% males) with 6,688 episodes of HEN, from 32 hospitals. The most frequent underlying disease was neurological disorders 42%, followed by cancer 28% (mostly head and neck cancer 18%). We had information related to the enteral access route in only 626 cases (9,4%), 51% of them used nasogastric tubes, 27% gastrostomies, 10% oral route and 3% jejunostomies. Only 251 episodes were closed during the year, mostly due to patient death 57% and progress to oral diet 14%. The activity level was limited in 29% of the patients and 39% of them were bed- or chairridden. Total or partial help was needed by 68% of the patients. The hospitals and the private pharmacies delivered the enteral formula in 63% and 34% of the cases, respectively. The hospitals and the primary care centres delivered the disposables in 83% and 16% of the cases, respectively. Conclusions: The results of the 2010 HEN registry are similar to those published in previous years regarding the number and characteristics of the patients. We continue finding problems in the entrance of data referred to the enteral access route and the closing of the episodes.

La utilización de heparina no está exenta de riesgos y de complicaciones a largo plazo. El uso de anticoagulantes también es discutido. Objetivos: Conocer la práctica habitual, en nuestro país, del mantenimiento de los catéteres venosos centrales para NPD, la incidencia de oclusión de los mismos y de trombosis venosa asociada. Métodos: estudio retrospectivo de los pacientes mayores de 18 años en activo en programa de NPD hasta noviembre de 2008, registrados por el grupo NADYA-SENPE. Resultados: Se incluyeron un total de 49 pacientes (16 hombres y 33 mujeres) pertenecientes a 6 hospitales, con una media de edad de 52,1 ± 13,9 años y una duración de la NPD de 57,4 ± 73,3 meses, con una media de 5,8 ± 1,8 días de NP a la semana. Las patologías mas frecuentes fueron enteritis rádica, alteraciones de la motilidad intestinal e isquemia mesentérica, (20,4% de pacientes cada una) y neoplasia activa (16,3%). Las principales indicaciones de la NPD fueron el síndrome de intestino corto (49,0%) y obstrucción intestinal (28,6%). Los principales factores predisponentes de trombosis venosa aparte de la neoplasia fueron: diátesis trombótica, enfermedad tromboembólica, y reposo en cama con un 6,1% cada una de ellas. El 77,6% de los pacientes recibía la NPD a través de un catéter tunelizado, y el resto a través de un reservorio implantado. Para el mantenimiento de la permeabilidad del catéter en el 28,6% de pacientes se usaba suero fisiológico y en el 69,4% heparina en distintas concentraciones retirándose ésta del catéter previa la administración de la NPT en el 63,3% de los casos. Las tasas de oclusión del catéter y episodios trombóticos fueron de 0,061/10³ y 0,115/10³ días de NPD respectivamente. Once pacientes (22,4%) recibían anticoagulación sistémica por episodios previos de trombosis venosa o tromboembolismo pulmonar. Conclusiones: La incidencia de complicaciones trombóticas asociadas al catéter en este grupo de pacientes con NPD es baja. Existe una gran heterogeneidad en la profilaxis de la oclusión del catéter y prevención de la trombosis venosa, siendo necesario unificar criterios en este sentido. Objectives: To be aware of the usual maintenance practice for HPN central venous catheters, catheter occlusion and related venous thrombosis incidence in our country. Methods: Retrospective study of active HPN patients older than 18 years registered by the NADYA- SENPE working group until November 2008. Results: 49 patients were registered (16 males and 33 females), with an average age of 52.1 ± 13.9 years, belonging to 6 hospitals. HPN length was 57.4 ± 73.3 months with 5.8 ± 1.8 PN days a week. The most frequent pathologies were actinic enteritis, intestinal motility disorders and mesenteric ischemia (20.4% each), and neoplasm (16.3%). The reason for HPN provision was short bowel syndrome (49.0%), and intestinal obstruction (28.6%). Neoplasm (16.3%), thrombotic diathesis, thromboembolic syndrome and bed rest (6.1% each) were the main venous thrombosis adjuvant factors. Tunnelled catheters were used in 77.6% of patients, with implanted port-catheters in the remainder. Maintenance of the line was done with saline solution flushes (28.6%) and different concentrations of heparin solutions (69.4%). When heparin was used, it was removed before PN infusion in 63.3% of patients. Catheter occlusion and venous thrombotic events rates were 0.061/10³ and 0.115/10³ HPN days respectively. Eleven patients (22.4%) were treated with anticoagulant drugs due to previous episodes of venous thrombosis or pulmonary embolism. Conclussion: The incidence of catheter related thrombotic complications incidence is low in this group of patients on HPN. There is a great variety of practices focused on the prevention of both: line occlusion and catheter related venous thrombosis. In conclusion, it would be necessary to standardize practice.

Los principales factores predisponentes de trombosis venosa aparte de la neoplasia fueron: diátesis trombótica, enfermedad tromboembólica, y reposo en cama con un 6,1% cada una de ellas. El 77,6% de los pacientes recibía la NPD a través de un catéter tunelizado, y el resto a través de un reservorio implantado. Para el mantenimiento de la permeabilidad del catéter en el 28,6% de pacientes se usaba suero fisiológico y en el 69,4% heparina en distintas concentraciones retirándose ésta del catéter previa la administración de la NPT en el 63,3% de los casos. Las tasas de oclusión del catéter y episodios trombóticos fueron de 0,061/10³ y 0,115/10³ días de NPD respectivamente. Once pacientes (22,4%) recibían anticoagulación sistémica por episodios previos de trombosis venosa o tromboembolismo pulmonar. Conclusiones: La incidencia de complicaciones trombóticas asociadas al catéter en este grupo de pacientes con NPD es baja. Existe una gran heterogeneidad en la profilaxis de la oclusión del catéter y prevención de la trombosis venosa, siendo necesario unificar criterios en este sentido.

37. Estudio transversal sobre la prevalencia de la Enfermedad Metabólica Ósea (EMO) y Nutrición Parenteral Domiciliaria (NPD) en España: datos del Grupo NADYA

Abstract: Los pacientes con insuficiencia intestinal que reciben NPD presentan un elevado riesgo de presentar EMO. El origen de esta afectación ósea es multifactorial y depende en gran parte de la enfermedad de base que origina la necesidad del soporte. En nuestro medio no disponemos de datos acerca de la prevalencia de esta enfermedad, por lo que el grupo NADYA-SENPE ha patrocinado este estudio transversal para intentar conocer la prevalencia de la EMO. Material y métodos: Se han recogido datos retrospectivos de 51 pacientes pertenecientes a 13 hospitales. La encuesta realizada incluía datos demográficos y los datos clínicos más relevantes que pudieran influir en la aparición de EMO. También se han registrados los datos analíticos más significativos para este proceso (calciuria, PTH, 25 OH vitamina D) y los resultados de la primera y la última densitometría realizadas. Resultados: Solamente 21 pacientes tenían realizada una densitometría en el momento de iniciar la NPD. La calidad del hueso está alterada al inicio de la NPD en un porcentaje significativo de casos (52%) Tras un seguimiento medio de 6 años ese porcentaje se eleva hasta el 81%. Dado el carácter retrospectivo del estudio y el escaso número de sujetos no es posible determinar el papel que juega la NPD en la etiología de la EMO. Sólo un 35% de los pacientes presentan niveles de vitamina D por encima de los niveles recomendados y la mayoría de ellos no recibe suplementación específica. Conclusiones: La NPD se asocia a un riesgo muy elevado de presentar EMO, por tanto es necesario disponer de protocolos de actuación que permitan detectar precozmente este problema y orientar el seguimiento y tratamiento de estos pacientes. Patients with intestinal failure who receive HPN are at high risk of developing MBD. The origin of this bone alteration is multifactorial and depends greatly on the underlying disease for which the nutritional support is required. Data on the prevalence of this disease in our environment is lacking, so NADYA-SEMPE group has sponsored this transversal study with the aim of knowing the actual MBD prevalence. Material and methods: Retrospective data from 51 patients from 13 hospitals were collected. The questionnaire included demographic data as well as the most clinically relevant for MBD data. Laboratory data (calciuria, PTH, 25 -OH -vitamin D) and the results from the first and last bone densitometry were also registered. Results: Bone mineral density had only been assessed by densitometry in 21 patients at the moment HPN was started. Bone quality is already altered before HPN in a significant percentage of cases (52%). After a mean follow up of 6 years, this percentage increases up to 81%. Due to retrospective nature of the study and the low number of subjects included it has not been possible to determine the role that HPN plays in MBD etiology. Only 35% of patients have vitamin D levels above the recommended limits and the majority of them is not on specific supplementation. Conclusions: HPN is associated with very high risk of MBD, therefore, management protocols that can lead to early detection of the problem as well as guiding for follow up and treatment of these patients are needed.

Abstract: Objetivo: Comunicar los datos del registro de Nutrición Parenteral Domiciliaria (NPD) del grupo de trabajo NADYA-SENPE de los años 2007, 2008 y 2009. Material y métodos: Recopilación de los datos del registro "on-line" introducidos por las Unidades responsables del seguimiento de la NPD desde el 1 de enero de 2007 al 31 de diciembre de 2009 dividido por años naturales. Resultados: Año 2007: Se registraron 133 pacientes con NPD (61 hombres y 72 mujeres), de 21 hospitales. La edad media de los 119 pacientes mayores de 13 años fue de 53,7 ± 14,9 años, y de 3,6 ± 3,6 años la de los 14 pacientes que no los superaban. La patología más frecuente fue la neoplasia (24%) seguida de las alteraciones de la motilidad intestinal y la enteritis posradiación (ambas 14%). En el 43% de los casos el motivo de indicación fue el síndrome de intestino corto, seguido de malabsorción (27%) y obstrucción intestinal (23%). Los catéteres más utilizados fueron los tunelizados (69%) y los reservorios subcutáneos (27%). Las complicaciones mas frecuentes fueron las sépticas relacionadas con el catéter con una tasa de 0,92 infecciones por cada mil días de NPD. La duración de la NPD fue superior a los dos años en el 50% de los casos. Al acabar el año seguía en activo el 71,4% de los pacientes; la muerte fue la principal causa de la finalización de la NPD (57,5%). El 26% de los pacientes se consideraron candidatos al trasplante intestinal. Año 2008: Se registraron 143 pacientes con NPD (62 hombres y 81mujeres), de 24 hospitales. La edad media de los 133 pacientes mayores de 13 años fue de 54,7 ± 13,9 años, y de 3,7 ± 0,6 años la de los 10 pacientes que no los superaban. La patología más frecuente fue la neoplasia (20%) seguida de la enteritis rádica (14%) y las alteraciones de la motilidad intestinal (13%). En el 44% de los casos el motivo de indicación fue el síndrome de intestino corto, seguido de malabsorción (28%) y obstrucción Nutriintestinal (20%). Los catéteres más utilizados fueron los tunelizados (60%) y los reservorios subcutáneos (29%). El 29% de los pacientes se consideraron candidatos al trasplante intestinal. Año 2009: Se registraron 158 pacientes con NPD (62 hombres y 96 mujeres), de 24 hospitales. La edad media de los 149 pacientes mayores de 13 años fue de 55,2 ± 13,0 años. La patología más frecuente fue la neoplasia (25%) seguida de la enteritis rádica (12%) y las alteraciones de la motilidad intestinal (11%). En el 42% de los casos el motivo de indicación fue el síndrome de intestino corto, seguido de malabsorción y obstrucción intestinal (ambas 23%). Los catéteres más utilizados fueron los tunelizados (60%) y los reservorios subcutáneos (36%). Las complicaciones mas frecuentes fueron las sépticas relacionadas con el catéter con una tasa de 0,67 infecciones por cada mil días de NPD. La duración de la NPD fue superior a los dos años en el 58% de los casos. Al acabar el año seguía en activo el 79,2% de los pacientes; el paso a alimentación oral fue la principal causa de la finalización de la NPD (48%). El 23% de los pacientes se consideraron candidatos a trasplante intestinal. Conclusiones: Se observa un aumento progresivo de los pacientes registrados respecto a años anteriores con una prevalencia muy variable según comunidades autónomas. La principal patología sigue siendo la neoplasia, que ocupa el primer lugar desde 2003. Se aprecia una disminución de las complicaciones sépticas relacionadas con el catéter en los dos últimos años, siendo la tasa de 2008 la más baja desde la creación del registro. Included fields were: age, sex, diagnosis and reason for HPN, access path, complications, beginning and end dates, complementary oral or enteral nutrition, activity level, autonomy degree, product and fungible material supply, withdrawal reason and intestinal transplant indication. La enfermedad de base que se registró con más frecuencia fue la neurológica en 2.732 (41,77%) ocasiones, seguida de la neoplasia en 1838; 28,10%. La vía de acceso se registró en 1.123 (17,17%) de los episodios, siendo más frecuente la administración por sonda nasogástrica 562 (50,04%). El tiempo medio de tratamiento nutricional fue de 323 días (10,77 meses). Finalizaron 606 episodios de NED, siendo el motivo más frecuentes el fallecimiento del enfermo, lo que aconteció en 295 (48,68%) ocasiones y el paso a alimentación oral en 219 (36,14%). Los pacientes mantenían una actividad normal en 2162 episodios de NED (32,55%) y en 2468 (37,13%) hacían vida "cama-sillón". El grado de dependencia fue "total" en 2598 (39,07%) de los episodios registrado. El suministro de la fórmula nutricional se realizó desde el hospital en 4.183 (62,91%) casos y por la farmacia de referencia en 2.262 (el 34,02%) y el material fungible se suministró desde el hospital en 3.531 (53,11%) de los casos. Conclusiones: El número de pacientes con NED registrados es superior al del año 2008, continuando con el incremento progresivo desde el inicio del registro. Las características de los mismos mantiene el mismo perfil que en años anteriores con pequeñas variaciones. The base illness registered more frequently was the neurological disorders in 2,732 (41,77%) patients, followed by cancer patients in 1,838; 28,10%. The enteral access route was registered in 1,123 (17,17%) of the episodes, being more frequent the administration by nasogastric tube 562 (50,04%). The mean length of nutritional treatment by episode was 323 days (10,77 months). 606 episodes of HEN ended, being the principal reasons for discontinuing treatment the patient death in 295 (48,68%) occasions. The transition to oral feeding occurred in 219 (36,14%) cases. Patients maintained normal activity in 2162 (32,55%) HEN episodes and 2,468 (37,13%) cases were living "bed-couch". The level of dependence was "total" in 2,598 (39,07%) of the episodes recorded. The nutritional formula was provided by the hospital in 4,183 (62,91%) cases and by the reference pharmacy in 2,262 (el 34,02%). Consumables were provided by the hospital in 3,531 (53,11%) cases. Conclusions: The number of HEN patients recorded increased from the year 2008, continuing the gradual growth increase since the start of registration. The characteristics of the patients remain in the same profile as in previous years.

Abstract: Objetivo: Exponer los resultados del registro de nutrición enteral domiciliaria (NED) del año 2008 del Grupo NADYA-SENPE. Material y métodos: Se recopilan los datos introducidosen el registro desde el 1 de enero al 31 de diciembre de 2008. Resultados: Durante este periodo se registraron 6.206 pacientes (51% varones) y un total de 6.279 episodios de NED en 31 hospitales españoles. El 95% de los enfermos tenía más de 14 años. La edad media (± desviación estándar) de los menores de 14 años fue de 4,83 años (± 3,29) y la de los mayores de 70,75 años (± 18,14). Las enfermedades que con más frecuencia motivaron el uso de la NED fueron la patología neurológica (39%) y las neoplasias (27%). La vía de administración más utilizada fue la oral (43,4%), seguida de la sonda nasoenteral (40,4%) y la gastrostomía (14,7%). La duración media del soporte fue de 305,36 días (10 meses). Los motivos más frecuentes de la finalización del tratamiento fueron la muerte del paciente (43%) y el paso a vía oral (40%). El nivel de actividad de los pacientes fue normal sólo en el 33% de ellos y limitado en diverso grado en el resto. La mayoría requería ayuda (parcial 25% o total 38%). El suministro de la fórmula de NE se realizó mayoritariamente en el Hospital (65%) y en el 32 % se efectuó a través de las oficinas de farmacia. La provisión del material fungible se efectuó a través de los hospitales en el 82,4% de los casos y en atención primaria en el 17,2%. Conclusiones: El número de pacientes registrados y su edad aumentan en relación con los años anteriores, pero se mantiene con pequeñas variaciones el resto de las variables analizadas. El aumento de la duración del soporte puede estar artefactado por el tipo de cumplimentación del registro. Aim: To present the results of the Spanish home enteral nutrition (HEN) registry of the year 2008 from the NADYA-SENPE group. Material and methods: We recorded the HEN registry data from January 1st to December 31st 2008. Results: The number of patients registered in this period was 6206 (51% male) with up to 6,279 episodes of HEN, from 31 Spanish hospitals. Most of the patients (95%) were older than 14 yr. Mean age was 4.83 ± 3.29 yr in the children group, and 70.75 ± 18.14 yr in the adult group (older than 14 yr). Neurological disorders (39%) and cancer (27%) were the two most prevalent diagnoses. The oral route was the most frequently used (43,4%), followed by nasogastric tube (40,4%), and gastrostomy tube (14,7%). Mean length of treatment was 305,36 days (10 months). The principal reasons for discontinuing treatment were death (43%) and progress to oral diet (40%). Only 33% of the patients had a normal activity level, being limited in different grades in the rest of the patients. Most of the patients required partial (25%) or total help (38%). The enteral formula was provided by the hospital in 65% of the cases and by private pharmacies in 32%. The disposables were provided by the hospital (82,4%) and primary care services (17,2%). Conclusions: The number and the age of the patients registered have increased comparing to previous years, with little variations in the rest of analyzed variables. The increase in the length of treatment could reflect misreporting of the weaning process in the registry.

Objective: To describe the Home Enteral Nutrition Characteristics (HEN) recorded by the group NADYASENPE during 2009. Material and methods: collection and analysis of the data voluntary recorded in the HEN registry from the NADYASENPE group from January 1st to December 31st. Results: 6.540 HEN patients were registered, 5.11% more than the previous year and 6,649 episodes (3,135 in women, 47,93%) from 32 different hospitals. 6,238 of them (95,38%) were over 14 years. The mean age of the patients under 14 yr was 3,67 ± 2,86 and it was 72,10 ± 16,89 in those over 14 yr group. The base illness registered more frequently was the neurological disorders in 2,732 (41,77%) patients, followed by cancer patients in 1,838; 28,10%. The enteral access route was registered in 1,123 (17,17%) of the episodes, being more frequent the administration by nasogastric tube 562 (50,04%). The mean length of nutritional treatment by episode was 323 days (10,77 months). 606 episodes of HEN ended, being the principal reasons for discontinuing treatment the patient death in 295 (48,68%) occasions. The transition to oral feeding occurred in 219 (36,14%) cases. Patients maintained normal activity in 2162 (32,55%) HEN episodes and 2,468 (37,13%) cases were living “bedcouch”. The level of dependence was “total” in 2,598 (39,07%) of the episodes recorded. The nutritional formula was provided by the hospital in 4,183 (62,91%) cases and by the reference pharmacy in 2,262 (el 34,02%). Consumables were provided by the hospital in 3,531 (53,11%) cases. Conclusions: The number of HEN patients recorded increased from the year 2008, continuing the gradual growth increase since the start of registration. The characteristics of the patients remain in the same profile as in previous years.

Complicaciones mas frecuentes: sépticas relacionadas con el catéter (0,85 infecciones por cada mil días de NPD). Duración de la NPD: > 2 años en el 50% de casos. Al acabar el año seguía en activo el 70,9% de los pacientes; la muerte fue la principal causa de la finalización de la NPD (53%). El 52% de los pacientes recibíeron alimentación oral o enteral complementaria. El 48% de los pacientes tenían un nivel de actividad normal, siendo autónomos el 55% del total. El hospital suministró la formula de NP en el 82% de los casos y el material fungible en el 78%. Se consideraron candidatos a trasplante intestinal el 27% de los pacientes. Conclusiones: Se observa un aumento de los pacientes registrados respecto a años anteriores con una prevalencia muy variable según comunidades autónomas. La principal patología sigue siendo la neoplasia, que ocupa el primer lugar desde 2003. Se aprecia una disminución de las complicaciones sépticas relacionadas con el catéter respecto a 2004 y 2005. HPD was provided for more than two years in 50% of the cases. By the end of 2006, 70.9% of the patients remained active; exitus was the most frequent reason to end HPN (53%). Complementary oral or enteral nutrition was provided to 52% of the patients. 48% of the patients had a normal activity level, and a 55% were autonomous. PN formula was supplied by the hospital in 82% of the cases, while this number was 78% for fungible materials. 27% of the patients were eligible for intestinal transplant. Conclusions: We observe an increase in registered patients with respect to previous years, with a very different prevalence among regions. Neoplasia remains as the main pathology since 2003. We observe a decrease in catheter-related infections with respect to the 2004-2005 period.

La enfermedad de base más frecuente fue la neoplasia (24%) seguida de la isquemia mesentérica (19%). Los catéteres más utilizados fueron los tunelizados (75%). Las complicaciones más frecuentes fueron las sépticas relacionadas con el catéter, con una tasa de 0,98 infecciones/ 103 días. El tiempo de soporte nutricional fue superior a dos años en el 69% de los casos. El suministro de las soluciones de NPD y del material fungible fue proporcionado por las farmacias hospitalarias en el 81% y el 83%, respectivamente. El 54% de los pacientes presentaban un nivel de actividad normal. En la mayoría de los pacientes la causa de finalización de la NPD fue la transición a la dieta oral (41%), o el fallecimiento (28%). Durante el año 2005 se registraron 79 pacientes con NPD (33 hombres y 46 mujeres), pertenecientes a 14 centros hospitalarios. La edad media de los mayores de 14 años fue de 52,39 ± 14,21 años y de 6,5 ± 5,21 años para los menores de 14 años. Abstract: Objetivo: Comunicar los resultados del registro sobre Nutrición Parenteral Domiciliaria (NPD) del grupo de Trabajo NADYA-SENPE, correspondientes al año 2003. Material y métodos: Recopilación de los datos del registro introducidos por las Unidades responsables de la atención de los pacientes con NPD. Se trata de un registro "on-line" disponible para los usuarios autorizados en la página web del grupo (www.nadya-senpe.com). Se recogen datos epidemiológicos, diagnóstico, vía de acceso, complicaciones, ingresos hospitalarios, grado de incapacidad y evolución a 31 de diciembre de 2003. Resultados: Se recogieron datos de 86 pacientes (62% mujeres y 38% hombres) pertenecientes a 17 centros hospitalarios. La edad media de los pacientes adultos fue de 50,7 ± 15,0 años; mientras que fue de 2,4 ± 1,5 años para los menores de 14 años (n = 5 pacientes). Las enfermedades que motivaron el uso de NPD fueron la enfermedad neoplásica (21%), seguida de la isquemia mesentérica (20%), enteritis rádica (16,3%), alteraciones de la motilidad (10,5%) y enfermedad de Crohn (4,6%). Se emplearon catéteres tunelizados en el 66,3% de los casos, frente al 29,1% de reservorios subcutáneos. La duración media del tratamiento ha sido de 8,5 ± 4,6 meses; el 67,4% de los pacientes habían permanecido con NPD por un periodo superior a los 6 meses. El seguimiento del paciente fue mayoritariamente realizado desde el hospital de referencia (88,4%). En ningún caso el paciente fue seguido por el equipo de atención primaria o por otros especialistas distintos de los que indicaron el soporte nutricional. Se presentaron complicaciones relacionadas con el tratamiento nutritivo en 98 ocasiones. De las complicaciones, las que se presentaron con más frecuencia fueron las infecciosas. Estas complicaciones significaron 1,60 hospitalizaciones por paciente. El número medio de consultas fue de 7,9 consultas por paciente (6,4 rutinarias y 1,5 urgentes). Al finalizar el año, observamos que seguían en activo en el programa el 73,3% de los pacientes; mientras que en el 23,3% la NPD había sido suspendida. Las principales causas de retirada fueron fallecimiento (11 pacientes) y pasar a depender de dieta oral (9 pacientes). En cuanto al grado de incapacidad, el 13% estaban confinados en silla o cama y sólo el 28% no presentaba ningún grado de incapacidad o sólo una leve incapacidad social Conclusiones: Observamos un ligero incremento en la tasa de prevalencia de NPD en España (2,15 pacientes/millón de habitantes). La indicación principal fue el cáncer seguido del síndrome de intestino corto secundario a patología vascular. Las complicaciones asociadas al tratamiento nutricional fueron frecuentes, especialmente las de causa infecciosa. Material and methods: Gathering of registry data introduced by all units responsible of HBPN patient care. This an on-line registry available for authorized users of the working group web page (www.nadya-senpe.com).Epidemiological data, diagnosis, access route, complications, hospital admissions, disability degree, and course at December 31st, 2003 Results: Data from 86 patients (62% female and 38% male) from 17 hospitals were gathered. Mean age of adult patients was 50.7 ± 15.0 years, whereas for patients younger than 14 years was 2.4 ± 1.5 years (n =5 patients). Diseases that prone HBPN were neoplasm(21%), followed by mesenteric ischemia (20%), radiation enteritis (16.3%), motility impairments (10.5%), and Crohn 's disease (4.6%). Tunneled catheters were used in 66.3% of the cases versus 29.1% of subcutaneous reservoirs. Mean treatment duration has been 8.5 ± 4.6 months; 67.4% of patients had been on HBPN for a period of time longer than 6 months. Patient follow-up was mostly done from the reference area hospital (88.4%). In no case patient follow-up was done by the primary care team or by specialists other than those prescribing nutritional support. Nutritional support-related complications were seen in 98 occasions. The most frequent complications were infectious ones. They represented 1.60 hospital admissions per patient. The mean number of visits was 7.9 per patient (6.4 for scheduled visits and 1.5 for emergency visits). By the end of the year, we observed that 73.3% of the patients were still on the program, whereas in 23.3% HBPN had been withdrawn. The main reasons for withdrawal were decease (11 patients), and advancing to oral diet (9 patients). As for the disability degree, 13% were confined to a wheelchair or bed, and only 28% had no disability degree or only mild social disability Conclusions: We observed a mild increase in HBPN prevalence rate in Spain (2.15 patients pmp). The main indication was cancer followed by short-bowel syndrome secondary to vascular pathology. Nutritional support-related complications were common, especially those of aninfectious origin.

Abstract: Objetivo: El objetivo de este estudio es analizar los datos registrados por el Grupo de Trabajo NADYA-SENPE de pacientes que han recibido Nutrición Parenteral Domiciliaria (NPD) en nuestro país durante el año 2001. Material y métodos: A partir de un cuestionario previamente diseñado, los médicos del grupo NADYA introducen en la página web (www.nadya-senpe.com), para su ulterior análisis, datos epidemiológicos, diagnóstico, vía de acceso, complicaciones, ingresos hospitalarios, duración del tratamiento, evolución y calidad de vida de los pacientes con NPD. Resultados: Los datos correspondían a 66 pacientes (edad media de 5,5 ± 4,9 años para pacientes < 14 años y 49,2 ± 15,8 años para ≥ 14 años) controlados por 17 centros hospitalarios. Las patologías que con más frecuencia han comportado este tratamiento fueron: isquemia mesentérica (28,9%), seguida de enfermedad neoplásica (22,7%), enteritis rádica (12,13%), alteraciones de la motilidad (4,5%) y enfermedad de Crohn (4,5%), entre otras. Se observó un predominio de los catéteres tunelizados (62,1%) sobre los implantados (33,3%) y de la administración cíclica (nocturna) de la nutrición parenteral (NP) (81,8%). Las farmacias hospitalarias suministraron las fórmulas de NPD con más frecuencia que el Nutriser-vice (75,7% versus 24,3%). La duración media del tratamiento fue de 8,43 ± 4,52 meses. Se presentaron 86 complicaciones relacionadas con el tratamiento nutritivo (1,3/paciente), con predominio de las infecciosas (0,67/paciente) (sepsis: 0,46/paciente y contaminación de catéter: 0,19/paciente), seguidas de las mecánicas (0,15/paciente), de las metabólicas (0,1/paciente) y de las hidroelectrolíticas (0,07/paciente). Estas complicaciones comportaron 1,34 hospitalizaciones /paciente. Al finalizar el año, seguían en activo 74,2% de los pacientes; mientras que en el resto (25,8%) se había retirado este tratamiento. Las principales causas de retirada fueron por éxitus (52,9%) y por pasar a depender de dieta oral (25,3%). El 10,6% estaban confinados en silla o cama y el 10,6% no presentaba ningún grado de incapacidad. Conclusiones: Aunque inferior a otros países europeos, se mantiene el número de pacientes con NPD registrados en nuestro país (1,65 pacientes/10(6) habitantes). Si bien predomina la patología vascular, sigue creciendo el peso de las neoplasias. Predominio de las complicaciones infecciosas que generan la mayoría de los ingresos hospitalarios. Aim: We analyse the registered data of home parenteral nutrition (HPN) in our country during the year 2001. Method: The data were collected through a previously designed questionnaire. Apart from epidemiological information, the form includes the disease to prescribe this treatment, the specific nutritional treatment used and its duration, access path, complications and readmission rate in hospital, follow-up of the treatment, and progress. All data were processed and analysed by the co-ordinating team. Results: Seventeen hospitals participated, and 66 patients were enrolled. Middle age was 5.5 ± 4.9 years for patients < 14 years old, and 49,2 ± 15.8 years for those ≥ 14 years old. The more prevalent diagnosis were: ischemic bowel (28.9%), neoplasm (22.7%), radiation enteritis (12.1%), motility disorders (4.5%) and Crohn's disease (4.5%). The mean time on HPN was 8.4 ± 4.5 months. Tunnnlled catheter was the preferential route (62.1%), followed by the implantated one (33.3%). The intermittent method (nocturnal) was preferential (81.8%). Patients receive the formula, mainly from hospital pharmacy (75.7%). The complications related to nutrition (1.3/patient) included the infections (0.46 sepsis/patient, and 0,19 catheter contamination/patients), mechanic (0.15/patient), metabolic (0.1/patient) and electrolytic disorders (0.07/patient). The readmission rate, for nutritional problems, was 1.34/patient. At the end of the year, 74.2% of the patients remained in the HPN program, and 25.8% abandoned the treatment (due to death: 52.9%, and to progress to oral feeding (25.3%). Conclusions: This review illustrates that the registration of HPN patients in our country is standing (1.65 patients/10(6) habitants), that vascular pathology is the more frequent diagnoses in HPN patients, and the rate of re-admission and complications and the behaviour is similar to other series making this as a safe treatment in our place.

Abstract: Introducción y objetivos: Análisis del registro de los pacientes en programa de Nutrición Enteral Domiciliaria (NED) en nuestro país durante el año 2001. Material y métodos: Estudio analítico realizado mediante una base de datos incluida en la web del Grupo NADYA (www.nadya-senpe.com) con los pacientes en NED el año 2001. Han participado 22 hospitales. Se aportan datos epidemiológicos, diagnósticos, vía de acceso, pautas de administración, fórmulas, duración, complicaciones, evolución y grado de incapacidad. Resultados: Incidencia: Se han comunicado 3.458 pacientes, 1.818 varones (53,7%) y 1.567 hembras (46,3%). Edad media 5,6 ± 4,0 años en los menores de 14 años, y 67,1 ± 19,5 en los mayores de 14 años. Patologías: Predominio de la patología neurológica (43,4%), seguida de la neoplásica (33,5%) sobre las demás. Duración: Tiempo medio en NED de 6,5 ± 4,5 meses. Vías de administración: La vía oral es la más frecuente (54,5%), seguida de la sonda naso gástrica (SNG) (32,2%) y de las ostomías (13,3%). Formas de administración: Predominio de la forma discontinua (66,1%), mediante goteo (43,4%) o bolus (22,7%). Fórmula: La fórmula polimérica fue la más utilizada (85,9%). Seguimiento: Los pacientes fueron mayoritariamente controlados por la unidad de nutrición de su hospital de referencia (69,3%). Hospitalizaciones: Se han registrado 7,3% hospitalizaciones asociadas a la nutrición [0,02 hospitalizaciones/paciente(c/p)]. Complicaciones: Las más frecuentes relacionadas con la nutrición fueron las gastrointestinales (25,9%; 0,16 c/p) y las mecánicas (24,2%, 0,15 c/p). Las diarreas han sido las complicaciones gastrointestinales más frecuentes (11,5%, 0,07 c/p). Evolución: Al finalizar el año el 48,3% de los pacientes continuaban en activo, mientras que en un 33,3% se suspendió la NE y en 18,4% no hubo continuidad de seguimiento. La causa más común de suspensión fue el paso a nutrición oral convencional (47,3%), seguido de la muerte relacionada con la enfermedad de base (43,5%). Un 29,7% de los pacientes estaban confinados en cama o silla, mientras que un 22,9% no presentaba prácticamente ningún grado de incapacidad. Conclusiones: Persiste la vía de administración oral como la más prevalente frente a la sonda y ostomía. Predominio de la patología neurológica. El bajo número de complicaciones e ingresos comportan que la NED sea un tratamiento seguro en nuestro medio. Goal: The NADYA-SENPE Working Group analyzed the registered data of patients on Home Enteral Nutrition (HEN) in our country, during year 2001. Material and methods: The data were collected through a closed questionnaire included on our web site (www.nadya-senpe.com). Apart from epidemiological information, the form includes the indication to prescribe this treatment, the specific nutritional treatment used and its duration, access path, complications and read-mission rate in hospital, follow-up of the treatment, patient's quality of life and progress. All data were precessed and analyzed by the coordinating team.Results: Twenty two hospitals participated and 3,458 patients, aged 5.6 ± 4.0 y for those younger than 14 y, and 67.1 ± 19.5 y for those older than 14 y, were enrolled. Of these patients, 43.4% were diagnosed with neurological diseases and 33.5% with cancer. The mean time on HEN was 6.5 ± 4.5 months. Oral nutrition was the preferential route (54.5%), followed by nasoenteral tube (32.3%), and in 13.3% ostomy tubes were placed. Polymeric was the formula composition mainly used(85.9%). Patients were followed (71.1%) by the hospital reference Nutritional Support Unit. The complications related to nutrition included mainly the gastrointestinal (0.16 complications/patient), and the mechanical one (0.15 complications/patient). At the end for the year, 48.3% of the patients were in the HEN program, and in 33.3% HEN was finish due to different reasons. In 22.9% of the patients no, o light, discapacity degree was found. Conclusions: Neurological diseases and cancer were the more frequent diagnoses in HEN patients. Oral access was the higher feeding route due, probably, to the high prevalence of cancer patients. In spite of the elevated prevalence of neurological diseases, a few number of patients, as previous years, were feed with ostomy tube. Due to the fwe complications observed, HEN is a safe treatment in our country.
