Frontal subcortical cognitive defects are predominant in Parkinson's disease (PD). Temporal lobe dysfunction seems more relevant for progression to dementia. We aimed to study the relative importance of temporal lobe defects versus executive impairment in the progression to dementia in PD by using proton magnetic resonance spectroscopy (1H-MRS). The 1H-MRS features of PD patients with intact cognition (PD-CgInt; n = 16), mild cognitive impairment (MCI; n = 15) and dementia (PDD; n = 15) were compared, to delineate the metabolic alterations correlating with cognitive status. Metabolite concentrations were acquired from voxels localized to the hippocampus and dorsolateral prefrontal cortex (DL-PFC). Cognitive status was established following the Movement Disorder Society PDD criteria, administering the Clinical Dementia Rating Scale and Mattis Dementia Rating Scale. The Parkinson's Disease Cognitive Rating Scale (PD-CRS) was used to correlate 1H-MRS with neuropsychology. N-acetylaspartate (NAA) concentrations in the right DL-PFC were decreased in PD-MCI compared with PD-CgInt patients (p = 0.002), and correlated with frontal subcortical tasks. Decreased NAA concentrations in the left hippocampus in PDD compared to PD-MCI (p = 0.03) correlated with confrontation naming. The present findings support that executive impairment is related to dorsolateral prefrontal dysfunction from the early stages, while progression to dementia is linked to the additional impairment of temporal lobe structures. The PD-CRS was able to capture the differential impairment of prefrontal versus temporal cortical areas. Copyright © 2012 S. Karger AG, Basel.

Background: Dementia is a frequent and devastating complication in Parkinson's disease (PD). There is an intensive search for biomarkers that may predict the progression from normal cognition (PD-NC) to dementia (PDD) in PD. Mild cognitive impairment in PD (PD-MCI) seems to represent a transitional state between PD-NC and PDD. Few studies have explored the structural changes that differentiate PD-NC from PD-MCI and PDD patients. Objectives and Methods: We aimed to analyze changes in cortical thickness on 3.0T Magnetic Resonance Imaging (MRI) across stages of cognitive decline in a prospective sample of PD-NC (n = 26), PD-MCI (n = 26) and PDD (n = 20) patients, compared to a group of healthy subjects (HC) (n = 18). Cortical thickness measurements were made using the automatic software Freesurfer. Results: In a sample of 72 PD patients, a pattern of linear and progressive cortical thinning was observed between cognitive groups in cortical areas functionally specialized in declarative memory (entorhinal cortex, anterior temporal pole), semantic knowledge (parahippocampus, fusiform gyrus), and visuoperceptive integration (banks of the superior temporal sulcus, lingual gyrus, cuneus and precuneus). Positive correlation was observed between confrontation naming and thinning in the fusiform gyrus, parahippocampal gyrus and anterior temporal pole; clock copy with thinning of the precuneus, parahippocampal and lingual gyrus; and delayed memory with thinning of the bilateral anteromedial temporal cortex. Conclusions: The pattern of regional decreased cortical thickness that relates to cognitive deterioration is present in PD-MCI patients, involving areas that play a central role in the storage of prior experiences, integration of external perceptions, and semantic processing. © 2013 Pagonabarraga et al.

The development of visual hallucinations (VH) is a frequent complication of Parkinson's disease (PD). Presence of hallucinations is one of the main risk factors associated with dementia, and severity progression of VH mainly contributes to impaired quality of life in PD. The neuropsychological features associated with severity progression of VH are unknown and might help to detect patients at risk of a more severe outcome. We aimed to explore the neuropsychological deficits associated with the different types of VH observed in PD, from minor hallucinations to well-formed VH with loss of insight. Prospective study of 57 PD patients with (n = 29) and without VH (n = 28) matched for age, education, antiparkinsonian medications, and disease duration. Description of VH was assessed by the Hallucinations and Psychosis item of the MDS-UPDRS. Cognition was assessed with the Parkinson's Disease-Cognitive Rating Scale (PD-CRS) and the Mattis Dementia Rating Scale (MDRS). Patients with minor VH did not differ from patients without VH in any cognitive domain. PD patients with major VH and insight retained performed worse on the action verbal fluency task (P < 0.04), and patients with VH and loss of insight showed a greater impairment on the PD-CRS posterior cortical score (P = 0.021) and the clock copying item (P = 0.01). A double dissociation was found in the neuropsychological profile of patients with VH with and without loss of insight. While the presence of major VH with insight retained appeared related to a predominant frontal-striatal impairment, loss of insight was characterized by further impairment of cognitive functions related to posterior cortical areas. A comprehensible continuum pattern of clinical relationships emerged among VH and cognitive functioning in PD. © 2010 Movement Disorder Society.

Objetivo. Conocer la satisfacción de los alumnos de enfermería de la Universidad Católica San Antonio sobre las prácticas que desarrollan durante su formación de pregrado. Método. Se realizó un estudio descriptivo observacional trasversal realizado en la Diplomatura de Enfermería de la Universidad Católica San Antonio durante los cursos académicos 2004/05 y 2007/08. La población objeto de estudio son todos los alumnos de enfermería matriculados en asignaturas de segundo y tercer curso con créditos prácticos. Para la realización del estudio se elaboró un cuestionario específico cerrado de respuesta múltiple. Resultados. Más del 90% de los alumnos afirman que los seminarios mejoran el inicio de las prácticas clínicas, y que el estudio teórico de los seminarios prácticos es de gran ayuda para mejorar la técnica. Las técnicas aprendidas en los seminarios se corresponden con la realidad asistencial. Los alumnos de ambos cursos académicos califican la relación con los profesionales de los centros sanitarios y socio-sanitarios como buena y muy buena en más de un 90% de los casos. Más del 80% de los alumnos consideran que el profesional - tutor ha facilitado su integración en el equipo de trabajo. Conclusiones. Se puede subrayar que la satisfacción de los alumnos tanto en las prácticas internas y externas es buena y que la relación con los profesionales - tutores es adecuada. En cuanto a la opinión de los alumnos sobre las gestiones que se llevan a cabo desde la Unidad de Prácticas de Enfermería, concluimos que existen aspectos mejorables sobre los que ya hemos comenzado a trabajar. Objective: To know the satisfaction of UCAM nursing students regarding the clinical training included in the practical training they attended during the undergraduate program. Method: A descriptive, observational and transversal study was carried out with nursing students of the Catholic University of San Antonio during the academic years 2004/05 and 2007/08. The studied sample consisted of all the nursing students registered in courses of second and third year with clinical training credits. A closed specific questionnaire of multiple choice was elaborated for the accomplishment of the study. Results: More than 90% of the students state that seminars improve the clinical training and that the theoretical study of practice seminars is helpful for improving nursing techniques. Techniques learned at seminars meet real assistance. Students of both academic years describe the relation with nurses and clinical mentors at hospitals and health centers as good and very good in more than 90% of the cases. More than 80% of the students consider that clinical mentors have facilitated its integration in working team. Conclusions: It is important that the student satisfaction in internal and external practice training is good and that the relation with clinical mentors is adequate. Regarding the opinion of the students about the clinical mentor management in the Unit of Nursing Practical Training, it was concluded that there are aspects to be improved in which we have already begun to work.

Objetivo: conocer cuánta información relativa a enfermería, salud pública, medicina y/o educación se publica en la prensa diaria. Material y Método: Es un estudio descriptivo de carácter retrospectivo de todas las noticias publicadas en los periódicos regionales La Verdad de Murcia y La Opinión de Murcia. La población de estudio son las noticias publicadas en los periódicos regionales La Verdad de Murcia y La Opinión de Murcia. La muestra son 1.518 noticias recogidas desde el 1 de Enero al 31 de Diciembre de 2004. Resultados: La distribución temática de las noticias a lo largo del año 2004 en los periódicos fue del 3% enfermería, 9% Educación, 44% medicina y salud pública. En el diario La Opinión la distribución por temas es de un 4.1% enfermería, 43.38% salud pública, 11.93 % medicina y un 40.56% de educación. En el diario la Verdad la distribución por tema es de un 2 % enfermería, 43.5 % salud pública, 7.1 % medicina y un 47.3 % educación. Conclusiones: El diario La Verdad de Murcia publica, en general, un mayor número de noticias de las áreas de interés analizadas (enfermería, medicina, salud pública y/o educación) que el diario La Opinión de Murcia. Existe una baja incidencia de noticias de Enfermería respecto a noticias de salud pública, Medicina en comparación a las noticias de Educación. Las noticias de Sanidad aumentan en las publicaciones dominicales, mientras que las de Educación se agrupan en los días centrales de la semana. A low incidence exists news of Infirmary regarding news of public health, Medicine in comparison to the news of Education. The news give Sanity they increase in the Sunday publications, while them Education they group in the central days of the week.

The aim of this study was to investigate possible abnormalities of cerebral myelination in subjects with attention-deficit hyperactivity disorder (ADHD).

INTRODUCTION: Anatomical and functional neuroimaging data from subjects with Attention Deficit Hyperactivity Disorder (ADHD) have consistently implicated a reversal of cerebral asymmetry and suggested a fronto-striatal dysfunction in this disorder. OBJECTIVE: The aim of this study is to investigate the brain asymmetries in a homogeneous and non-medicated sample of adolescents with ADHD who had been previously studied in our laboratory. PATIENTS AND METHODS: T1-weighted magnetic resonance images were obtained for 11 adolescents with ADHD and 19 control subjects. Frontal and posterior brain regions, caudate nucleus, and ventricular system were quantitatively measured. RESULTS: A reversed pattern of asymmetry for the caudate nucleus (right > left) was found in ADHD when compared to the control group. We also found a reversed pattern of asymmetry for the frontal lobe (right < left) and a smaller right frontal volume (prefrontal specifically) in the ADHD subjects most severely impaired. Right caudate and frontal measures were inversely correlated. CONCLUSIONS: ADHD is associated with fronto-striatal abnormalities, which may be explicable via extant neurodevelopmental theories. Enlargement of the right caudate nucleus may suggest the failure of a process of synaptic 'pruning' by which attentional functions could be improperly transferred from the basal ganglia to frontal regions during development.

INTRODUCTION: Fragile-X syndrome is characterized by the presence of a fragile site (gap) on Xq 27.3 and the transcriptional inhibition of a mRNA protein-binding gene called FMR-1. Neuropsychological features include cognitive impairment, attention deficit disorder with and without hyperactivity, and impairment of visuospatial functions, language and frontal executive functions. In the present paper, other cytogenetic and phenotype characteristics, associate disorders, neurological and neuroimaging studies are revised. CLINICAL CASES: We describe two siblings that illustrate the pattern of neurocognitive and behavioural trends of the Fragile X syndrome, and sex differences. DISCUSSION AND CONCLUSIONS: These two cases emphasize the need for performing a cytogenetic diagnostic in patients with mental retardation, of unknown etiology, and with a familiar history of mental retardation.
