Objectives: The purpose of this study was to compare the immediate angiographic and long-term results of debulking versus balloon angioplasty for treatment of true bifurcation lesions., Background: Previous studies have shown true bifurcation lesions to be a high risk morphological subset for percutaneous transluminal coronary angioplasty (PTCA). Although atherectomy devices have been used to treat bifurcation lesions, no studies have compared the outcomes of these alternative treatment modalities., Methods: Between January 1992 and May 1997, we treated 70 consecutive patients with true bifurcation lesions (defined as a greater than 50% stenosis in both the parent vessel and contiguous side branch) with conventional PTCA (n = 30) or debulking (with rotational or directional atherectomy) plus adjunctive PTCA (n = 40). Paired angiograms were analyzed by quantitative angiography, and clinical follow-up was obtained in all patients., Results: Acute procedural success was 73% in the PTCA group and 97% in the debulking group (p = 0.01). Major in-hospital complications occurred in two patients in the PTCA group and one in the debulking group. Treatment with atherectomy plus PTCA resulted in lower postprocedure residual stenoses than PTCA alone (16+/-15% vs. 33+/-17% in the parent vessel, and 6+/-15% vs. 39+/-22% in the side branch; p < 0.001 for both comparisons). At 1 year follow-up, the incidence of target vessel revascularization (TVR) was 53% in the PTCA group as compared with 28% in the debulking group (p = 0.05). Independent predictors of the need for repeat TVR were side branch diameter >2.3 mm, longer lesion lengths, and treatment with PTCA alone., Conclusions: For the treatment of true bifurcation lesions, atherectomy with adjunctive PTCA is safe, improves acute angiographic results, and decreases target vessel revascularization compared to PTCA alone. The benefits of debulking for bifurcation lesions were especially seen in lesions involving large side branches.

Previous studies have shown a high rate of repeat intervention after treating diffuse in-stent restenosis with percutaneous transluminal coronary angioplasty (PTCA) alone. It is not clear whether debulking with atherectomy is more effective in this condition. Between January 1994 and February 1997, we treated 60 consecutive patients with diffuse in-stent restenosis of a native coronary artery using conventional PTCA (n=30) or debulking (with rotational or directional atherectomy) plus adjunctive PTCA (n=30). Paired angiograms were analyzed by quantitative angiography, and clinical follow-up was obtained in all patients at 1 month, 6 months, and 1 year after revascularization. The mean lesion lengths were 13.5+/-8.3 and 18.4+/-13.2 mm in the debulking and PTCA groups, respectively (p=0.09). Acute procedural success was 100% in both cohorts, with no major complications in either group. Treatment with atherectomy plus adjunctive PTCA resulted in lower postprocedure stenoses (18+/-10 vs 26+/-13%, p=0.01) than treatment with balloon angioplasty alone. At 1-year follow-up, repeat target vessel revascularization was required in 28% of patients in the debulking group compared with 46% in the PTCA group (p=0.18). Independent predictors of the need for repeat target vessel revascularization were longer lesion lengths, diabetes mellitus, and smaller postprocedure lumen diameter. Thus, the strategy of atherectomy and adjunctive PTCA for diffuse in-stent restenosis is safe, improves acute angiographic results compared with PTCA alone, and may decrease the need for target vessel revascularization.

To determine the influence of a history of restenosis on subsequent restenosis after percutaneous transluminal coronary angioplasty (PTCA) of a new significant narrowing, the records of 100 patients who underwent successful PTCA at another site ("new narrowing PTCA") greater than or equal to 2 months after successful initial PTCA were retrospectively reviewed. Patients were grouped according to whether initial PTCA resulted in restenosis, which was determined by angiographic follow-up greater than or equal to 3 months after initial PTCA. Patients in group 1 did not have restenosis after initial PTCA (n = 50), whereas patients in group 2 did (n = 40). All patients were followed for recurrent symptoms, with serial exercise tests, for greater than or equal to 6 months after new narrowing PTCA. Clinically suspected and angiographically confirmed restenosis occurred in 11 of 50 (22%) patients and 12 of 63 (19%) narrowings in group 1, and in 20 of 40 (50%) patients and 22 of 48 (46%) narrowings in group 2 (p less than 0.01 for patients, p less than 0.002 for narrowings). Multivariate analysis identified that prior restenosis (p less than 0.02, odds ratio 3.4), left anterior descending artery location of stenosis (p less than 0.04, odds ratio 3.0), and severity of stenosis before PTCA (p less than 0.02, odds ratio 1.8) were independently associated with restenosis after new narrowing PTCA. In conclusion, prior restenosis is an independent risk factor for subsequent restenosis after new narrowing PTCA.

More than 98% of coronary studies performed in our laboratory are done with the transfemoral approach. In this report, we review our experience with 67 cases in which we used the percutaneous brachial approach as an alternative for patients with peripheral vascular disease. Sixty-six cases (99%) were completed; one case (1%) was aborted due to cardiogenic shock. No death, stroke, or myocardial infarction occurred. Seven patients (10%) had minor complications; none required surgical intervention. The percutaneous brachial approach with standard preformed catheters offers a simple and satisfactory alternative to transfemoral coronary angiography.

The angiographic and histologic consequences of laser thermal angioplasty were examined and compared with those of conventional balloon angioplasty in an atherosclerotic rabbit iliac artery preparation immediately and 4 weeks after the procedure. Nineteen vessels in 13 rabbits underwent either laser thermal or balloon angioplasty in random order. Laser thermal angioplasty was performed in a total of nine vessels with either a 1.5 or 2.0 mm laser-heated metallic-capped fiber by delivery of 6 or 8 W, respectively, of argon laser energy for 5 sec duration during continuous advancement through the stenosis. Balloon angioplasty was performed in a total of 10 stenotic lesions with a 2.5 mm balloon catheter. The immediate enlargement of the angiographic luminal diameters was similar for both procedures: from 1.0 +/- 0.2 to 1.9 +/- 0.2 mm for laser thermal angioplasty vs 1.0 +/- 0.1 to 2.0 +/- 0.2 mm for balloon angioplasty. However, 4 weeks later the vessels treated with laser thermal angioplasty had less restenosis, defined as a 20% or greater reduction in luminal diameter (two of nine vessels [22%] vs 10 of 10 vessels [100%]; p less than .001), and a significantly larger mean luminal diameter (1.6 +/- 0.5 vs 1.0 +/- 0.4 mm) than those treated with conventional balloon angioplasty (p less than .02). Histologic examination 4 weeks after the procedure revealed less fibrocellular proliferation after laser thermal angioplasty, whereas those vessels treated with balloon angioplasty demonstrated evidence of prior fracture and dissection of the vessel wall with more of a fibrocellular proliferative response.(ABSTRACT TRUNCATED AT 250 WORDS)
