BACKGROUND. Persistent controllers (PCs) maintain antiretroviral-free HIV-1 control indefinitely over time, while transient controllers (TCs) eventually lose virological control. It is essential to characterize the quality of the HIV reservoir in terms of these phenotypes in order to identify the factors that lead to HIV progression and to open new avenues toward an HIV cure. METHODS. The characterization of HIV-1 reservoir from peripheral blood mononuclear cells was performed using next- generation sequencing techniques, such as full-length individual and matched integration site proviral sequencing (FLIP-Seq; MIP-Seq). RESULTS. PCs and TCs, before losing virological control, presented significantly lower total, intact, and defective proviruses compared with those of participants on antiretroviral therapy (ART). No differences were found in total and defective proviruses between PCs and TCs. However, intact provirus levels were lower in PCs compared with TCs; indeed the intact/defective HIV-DNA ratio was significantly higher in TCs. Clonally expanded intact proviruses were found only in PCs and located in centromeric satellite DNA or zinc-finger genes, both associated with heterochromatin features. In contrast, sampled intact proviruses were located in permissive genic euchromatic positions in TCs. CONCLUSIONS. These results suggest the need for, and can give guidance to, the design of future research to identify a distinct proviral landscape that may be associated with the persistent control of HIV-1 without ART. HIV-1-infected cells that persist despite antiretroviral therapy (ART) are frequently considered "transcriptionally silent," but active viral gene expression may occur in some cells, challenging the concept of viral latency. Applying an assay for profiling the transcriptional activity and the chromosomal locations of individual proviruses, we describe a global genomic and epigenetic map of transcriptionally active and silent proviral species and evaluate their longitudinal evolution in persons receiving suppressive ART. Using genome-wide epigenetic reference data, we show that proviral transcriptional activity is associated with activating epigenetic chromatin features in linear proximity of integration sites and in their inter- and intrachromosomal contact regions. Transcriptionally active proviruses were actively selected against during prolonged ART; however, this pattern was violated by large clones of virally infected cells that may outcompete negative selection forces through elevated intrinsic proliferative activity. Our results suggest that transcriptionally active proviruses are dynamically evolving under selection pressure by host factors.

Increasing evidence suggests that durable drug-free control of HIV-1 replication is enabled by effective cellular immune responses that may induce an attenuated viral reservoir configuration with a weaker ability to drive viral rebound. Here, we comprehensively tracked effects of antiviral immune responses on intact and defective proviral sequences from elite controllers (ECs), analyzing both classical escape mutations and HIV-1 chromosomal integration sites as biomarkers of antiviral immune selection pressure. We observed that, within ECs, defective proviruses were commonly located in permissive genic euchromatin positions, which represented an apparent contrast to autologous intact proviruses that were frequently located in heterochromatin regions; this suggests differential immune selection pressure on intact versus defective proviruses in ECs. In comparison to individuals receiving antiretroviral therapy, intact and defective proviruses from ECs showed reduced frequencies of escape mutations in cytotoxic T cell epitopes and antibody contact regions, possibly due to the small and poorly inducible reservoir that may be insufficient to drive effective viral escape in ECs. About 15% of ECs harbored nef deletions in intact proviruses, consistent with increased viral vulnerability to host immunity in the setting of nef dysfunction. Together, these results suggest a distinct signature of immune footprints in proviral sequences from ECs.

Type-2 cannabinoid receptor (CB2R) is a member of the G protein-coupled receptor superfamily with various biological activities in pain regulation, anti-inflammation, anti-fibrosis, and bone metabolism regulation. CB2R modulators have been demonstrated to have significant regulatory effects on bone metabolism with no significant psychoactive adverse effects. In this article, we present a comprehensive review of the molecular mechanism of CB2R in regulating bone metabolism and summarize virtual screening and experimental screening methods for CB2R modulators from natural and synthetic products. Knowing that some botanical compounds are CB2R modulators involved in the regulation of bone metabolism, we also review the botanical and synthetic compounds currently known to be potential CB2R and summarize their regulatory effects on bone metabolism based on the results of animal, osteoblasts (OBs) and osteoclasts (OCs) experimental studies presently available, hoping that the findings and conclusions in this review could provide a scientific basis for the discovery of new CB2R regulators and the development of new anti-osteoporosis drugs.

Background Hemorrhoids are a highly prevalent disease, which have been a health challenge to people for a long time, seriously affecting the life, work, study, and the quality of life. The development of hemorrhoids is closely related to the imbalance of the mucosal barrier function of the anal cushion and the destruction of the mucus barrier and the mucosal barrier. How to treat hemorrhoids simply and efficiently has become a concern of clinicians. Objective To comprehensively assess the clinical efficacy of mussel adhesive protein (MAP) with fumigation and sitz bath with Tongyangxiao Lotion (a traditional Chinese medicine) in the treatment of grades 1 and 2 mixed hemorrhoids. Methods A prospective, randomized, single-blind, controlled study design was used. 298 anorectal outpatients with grades 1 and 2 hemorrhoids were selected from Hebei University of Chinese Medicine in 2021, and equally randomized into an experimental group and a control group. All patients were given general treatment such as diet and life guidance, and a 10-day fumigation and sitz bath with Tongyangxiao Lotion. The experimental group additionally received a 10-day treatment with topical use of MAP concurrently. The primary efficacy indicators include the response level (cure, marked response, response, non-response) and recurrence rate. The secondary efficacy indicators include the visual analogue scale (VAS) score, clinical symptom score, quality of life score, and safety index. Results 276 patients actually completed this study, including 140 in the experimental group and 136 in the control group; 135 males and 141 females, with a median age of 38 (29, 48) years. There were no significant differences between two groups in gender ratio, mean age, distribution of course, and grade of mixed hemorrhoids, VAS score, clinical symptom score, anal mean canal resting pressure, hemorrhoid disease and the Crohn's Anal Fistula Quality of Life (CAF-QoL) score and past history (P>0.05). The VAS score, clinical symptom score, hematochezia score, rectal tenesmus score, hemorrhoid mucosa score, hemorrhoid nucleus size score of the two groups differed between two groups on the 4th, 7th or 10th day of follow-up (P0.05) . Conclusion MAP combined with sitz bath with Tongyangxiao Lotion acted rapidly, which had better performance in repairing the barrier function of the anal cushion, improving the clinical symptoms, reducing the tension of the internal anal sphincter, and improving the quality of life in patients with grades 1 and 2 mixed hemorrhoids.

Sustained, drug-free control of HIV-1 replication is naturally achieved in less than 0.5% of infected individuals (here termed 'elite controllers'), despite the presence of a replication-competent viral reservoir1. Inducing such an ability to spontaneously maintain undetectable plasma viraemia is a major objective of HIV-1 cure research, but the characteristics of proviral reservoirs in elite controllers remain to be determined. Here, using next-generation sequencing of near-full-length single HIV-1 genomes and corresponding chromosomal integration sites, we show that the proviral reservoirs of elite controllers frequently consist of oligoclonal to near-monoclonal clusters of intact proviral sequences. In contrast to individuals treated with long-term antiretroviral therapy, intact proviral sequences from elite controllers were integrated at highly distinct sites in the human genome and were preferentially located in centromeric satellite DNA or in Krüppel-associated box domain-containing zinc finger genes on chromosome 19, both of which are associated with heterochromatin features. Moreover, the integration sites of intact proviral sequences from elite controllers showed an increased distance to transcriptional start sites and accessible chromatin of the host genome and were enriched in repressive chromatin marks. These data suggest that a distinct configuration of the proviral reservoir represents a structural correlate of natural viral control, and that the quality, rather than the quantity, of viral reservoirs can be an important distinguishing feature for a functional cure of HIV-1 infection. Moreover, in one elite controller, we were unable to detect intact proviral sequences despite analysing more than 1.5 billion peripheral blood mononuclear cells, which raises the possibility that a sterilizing cure of HIV-1 infection, which has previously been observed only following allogeneic haematopoietic stem cell transplantation2,3, may be feasible in rare instances.

Graphene oxide (GO) has recently gained significant attention in electrocatalysis as a promising electrode material owing to its unique physiochemical properties such as enhanced electron transfers due to a conjugated π-electron system, high surface area, and stable support for loading electroactive species, including metal nanoparticles. However, only a few studies have been directed toward the structural characteristics of GO, elaborating on the roles of oxygen-containing functional groups, the presence of defects, interlayer spacing between the layered structure, and nonuniformity in the carbon skeleton along with their influence on electrochemical performance. In this work, we aim to understand these properties in various GO materials derived from different graphitic sources. Both physiochemical and electrochemical characterization were employed to correlate the above-mentioned features and explore the effect of the location of the palladium nanoparticles (Pd NPs) on various GO supports for the hydrogen evolution reaction (HER). The interaction of the functional groups has a crucial role in the Pd dispersion and its electrochemical performance. Among the different GO samples, Pd supported on GO derived from graphene nanoplate (GNP), Pd/GO-GNP, exhibits superior HER performance; this could be attributed to the optimal balance among particle size, defect density, less in-plane functionalities, and higher electrochemical surface area. This study, thus, helps to identify the optimal conditions that lead to the best performance of Pd-loaded GO, contributing to the design of more effective HER electrocatalysts. This study investigates the probabilistic seismic damage characteristics of a five-span RC simply supported girder bridge with double-column piers designed for a high-speed railway (HSR). The objective is to assess the bridge's fragility by developing a refined nonlinear numerical model using the OpenSEES (Version 3.3.0) platform. Incremental dynamic analysis (IDA) was conducted with peak ground accelerations (PGA) ranging from 0.05 g to 0.5 g, and fragility curves for pier columns, tie beams, and bearings were developed. Additionally, a series–parallel relationship and a hierarchically iterated pair copula model were established to evaluate system fragility. The results indicate that as PGA increases, the damage probability of all bridge components rises, with bearings being the most vulnerable, followed by pier columns, and tie beams exhibiting the least damage. The models accurately simulate the correlations between members and system fragility, offering valuable insights into the bridge's performance under seismic conditions. [ABSTRACT FROM AUTHOR]

HIV posttreatment controllers (PTCs) represent a natural model of sustained HIV remission, but they are rare and little is known about their viral reservoir. We obtained 1,450 proviral sequences after near-full-length amplification for 10 PTCs and 16 posttreatment noncontrollers (NCs). Before treatment interruption, the median intact and total reservoir size in PTCs was 7-fold lower than in NCs, but the proportion of intact, defective, and total clonally expanded proviral genomes was not significantly different between the 2 groups. Quantification of total but not intact proviral genome copies predicted sustained HIV remission as 81% of NCs, but none of the PTCs had a total proviral genome greater than 4 copies per million peripheral blood mononuclear cells (PBMCs). The results highlight the restricted intact and defective HIV reservoir in PTCs and suggest that total proviral genome burden could act as the first biomarker for identifying PTCs. Total and defective but not intact proviral copy numbers correlated with levels of cell-associated HIV RNA, activated NK cell percentages, and both HIV-specific CD4+ and CD8+ responses. These results support the concept that defective HIV genomes can lead to viral antigen production and interact with both the innate and adaptive immune systems.
