Radiology has historically not been a very diverse field. Many steps have been taken in the past decade to increase diversity in the field and make it more inclusive. This study shows the relative trends specifically in neuroradiology trainees, and the need for reassessment and further steps to increase diversity.

2. Malpractice Litigation Related to Diagnosis and Treatment of Intracranial Aneurysms.

Abstract

BACKGROUND AND PURPOSE: Approaches to management of intracranial aneurysms are inconsistent, in part due to apprehension relating to potential malpractice claims. The purpose of this article was to review the causes of action underlying medical malpractice lawsuits related to the diagnosis and management of intracranial aneurysms and to identify the factors associated and their outcomes. MATERIALS AND METHODS: We consulted 2 large legal databases in the United States to search for cases in which there were jury awards and settlements related to the diagnosis and management of patients with intracranial aneurysms in the United States. Files were screened to include only those cases in which the cause of action involved negligence in the diagnosis and management of a patient with an intracranial aneurysm. RESULTS: Between 2000 and 2020, two hundred eighty-seven published case summaries were identified, of which 133 were eligible for inclusion in the analysis. Radiologists constituted 16% of 159 physicians sued in these lawsuits. Failure to diagnose was the most common medical malpractice claim referenced (100/133 cases), with the most common subgroups being failure to include cerebral aneurysm as a differential and thus perform adequate work-up (30 cases), and failure to correctly interpret aneurysm evidence on CT or MR imaging (16 cases). Only 6 of these 16 cases were adjudicated at trial, with 2 decided in favor of the plaintiff (awarded $4,000,000 and $43,000,000, respectively). CONCLUSIONS: Incorrect interpretation of imaging is relatively infrequent as a cause of malpractice litigation compared with failure to diagnose aneurysms in the clinical setting by neurosurgeons, emergency physicians, and primary care providers.

Abstract

A 12 year old child presented with crush injury of forearm and hand. He was diagnosed as a case of uncorrected large Ventricular Septal Defect (VSD). Tendon repair, debridement and nailing were done using regional technique. Risks associated with a cardiac patient undergoing non-cardiac emergency procedure are discussed in this report.

Abstract

Background During the COVID-19 pandemic, decreased volumes of stroke admissions and mechanical thrombectomy were reported. The study's objective was to examine whether subarachnoid haemorrhage (SAH) hospitalisations and ruptured aneurysm coiling interventions demonstrated similar declines. Methods We conducted a cross-sectional, retrospective, observational study across 6 continents, 37 countries and 140 comprehensive stroke centres. Patients with the diagnosis of SAH, aneurysmal SAH, ruptured aneurysm coiling interventions and COVID-19 were identified by prospective aneurysm databases or by International Classification of Diseases, 10th Revision, codes. The 3-month cumulative volume, monthly volumes for SAH hospitalisations and ruptured aneurysm coiling procedures were compared for the period before (1 year and immediately before) and during the pandemic, defined as 1 March-31 May 2020. The prior 1-year control period (1 March-31 May 2019) was obtained to account for seasonal variation. Abstract

Background: The COVID-19 pandemic led to profound changes in the organization of health care systems worldwide. Aims: We sought to measure the global impact of the COVID-19 pandemic on the volumes for mechanical thrombectomy, stroke, and intracranial hemorrhage hospitalizations over a three-month period at the height of the pandemic (1 March–31 May 2020) compared with two control three-month periods (immediately preceding and one year prior). Methods: Retrospective, observational, international study, across 6 continents, 40 countries, and 187 comprehensive stroke centers. The diagnoses were identified by their ICD-10 codes and/or classifications in stroke databases at participating centers. Results: The hospitalization volumes for any stroke, intracranial hemorrhage, and mechanical thrombectomy were 26,699, 4002, and 5191 in the three months immediately before versus 21,576, 3540, and 4533 during the first three pandemic months, representing declines of 19.2% (95%CI, −19.7 to −18.7), 11.5% (95%CI, −12.6 to −10.6), and 12.7% (95%CI, −13.6 to −11.8), respectively. The decreases were noted across centers with high, mid, and low COVID-19 hospitalization burden, and also across high, mid, and low volume stroke/mechanical thrombectomy centers. High-volume COVID-19 centers (−20.5%) had greater declines in mechanical thrombectomy volumes than mid- (−10.1%) and low-volume (−8.7%) centers (p < 0.0001). There was a 1.5% stroke rate across 54,366 COVID-19 hospitalizations. SARS-CoV-2 infection was noted in 3.9% (784/20,250) of all stroke admissions. Conclusion: The COVID-19 pandemic was associated with a global decline in the volume of overall stroke hospitalizations, mechanical thrombectomy procedures, and intracranial hemorrhage admission volumes. Despite geographic variations, these volume reductions were observed regardless of COVID-19 hospitalization burden and pre-pandemic stroke/mechanical thrombectomy volumes. © 2021 World Stroke Organization.

Abstract

OBJECTIVE: To measure the global impact of COVID-19 pandemic on volumes of IV thrombolysis (IVT), IVT transfers, and stroke hospitalizations over 4 months at the height of the pandemic (March 1 to June 30, 2020) compared with 2 control 4-month periods. METHODS: We conducted a cross-sectional, observational, retrospective study across 6 continents, 70 countries, and 457 stroke centers. Diagnoses were identified by their ICD-10 codes or classifications in stroke databases. RESULTS: There were 91,373 stroke admissions in the 4 months immediately before compared to 80,894 admissions during the pandemic months, representing an 11.5% (95% confidence interval [CI] -11.7 to -11.3, p < 0.0001) decline. There were 13,334 IVT therapies in the 4 months preceding compared to 11,570 procedures during the pandemic, representing a 13.2% (95% CI -13.8 to -12.7, p < 0.0001) drop. Interfacility IVT transfers decreased from 1,337 to 1,178, or an 11.9% decrease (95% CI -13.7 to -10.3, p = 0.001). Recovery of stroke hospitalization volume (9.5%, 95% CI 9.2-9.8, p < 0.0001) was noted over the 2 later (May, June) vs the 2 earlier (March, April) pandemic months. There was a 1.48% stroke rate across 119,967 COVID-19 hospitalizations. Severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2) infection was noted in 3.3% (1,722/52,026) of all stroke admissions. CONCLUSIONS: The COVID-19 pandemic was associated with a global decline in the volume of stroke hospitalizations, IVT, and interfacility IVT transfers. Primary stroke centers and centers with higher COVID-19 inpatient volumes experienced steeper declines. Recovery of stroke hospitalization was noted in the later pandemic months. © 2021 American Academy of Neurology.

25. Discovery, Optimization, and Characterization of ML417: A Novel and Highly Selective D3 Dopamine Receptor Agonist

Abstract

To identify novel D3 dopamine receptor (D3R) agonists, we conducted a high-throughput screen using a β-arrestin recruitment assay. Counterscreening of the hit compounds provided an assessment of their selectivity, efficacy, and potency. The most promising scaffold was optimized through medicinal chemistry resulting in enhanced potency and selectivity. The optimized compound, ML417 (20), potently promotes D3R-mediated β-arrestin translocation, G protein activation, and ERK1/2 phosphorylation (pERK) while lacking activity at other dopamine receptors. Screening of ML417 against multiple G protein-coupled receptors revealed exceptional global selectivity. Molecular modeling suggests that ML417 interacts with the D3R in a unique manner, possibly explaining its remarkable selectivity. ML417 was also found to protect against neurodegeneration of dopaminergic neurons derived from iPSCs. Together with promising pharmacokinetics and toxicology profiles, these results suggest that ML417 is a novel and uniquely selective D3R agonist that may serve as both a research tool and a therapeutic lead for the treatment of neuropsychiatric disorders.

Abstract

A benchtop brain tissue-microelectrode insertion model system was developed to aid in improving the design of cortical neural interfaces. The model partially mimics the in vivo environment via the use of human cadaver brain specimens ([n.sub.speeimen] = 6), or agar gel exposed to physiologically relevant mechanical oscillations. 150 [micro]m diameter stainless-steel microelectrode wires (TS = 600 MPa) implanted 3.0 cm within fixed human primary auditory cortex ([n.sub.trial] > 10) experienced 133 [+ or -] 8 and 64 [+ or -] 4 mN of peak and steady axial forces. When subjected to a 3 Hz, 3-mm vertical oscillation, dynamic force amplitudes ([n.sub.trial] > 10) of 148 [+ or -] 10 mN were measured. The model system allows the study and comparison of static and dynamic forces and their mechanical influences on proposed implanted microelectrode structures. Abstract

This thesis considers the tensile deformation characteristics of polypropylene fibre reinforced hardened cement paste having heterogeneous fibre geometries and a range of volume concentrations. Polypropylene fibres were prepared under various manufacturing conditions using a laboratory extruder, to ascertain the effect of these conditions on fibre characteristics. The relevant properties of cement paste likely to influence the polypropylene fibres and the eventual composite were investigated. An investigation of continuous aligned fibre composites in tension, containing various volume concentrations of fibres, showed that multiple cracking occurred despite the elastic modulus of the fibre being considerably lower than that of the hardened cement paste. Factors which enabled fibre/matrix contact to be maintained during the multiple cracking process, despite the unfavourable Poisson's ratio of polypropylene, were considered. Discontinuous aligned fibre composites were tested in tension, to ascertain the effect of volume concentration and length of fibres on the shear stress transfer between fibre and matrix and on multiple cracking. Composites containing parallel fibres, with fibre directions at varying angles to the direction of applied tensile stress, allowed an assessment to be made of the role of inclination. Crack distributions, pull-out behaviour of fibres and the ultimate stress of composites were investigated. Finally, random fibre reinforced composites were evaluated to provide a comparison with the Continuous, Discontinuous and Inclined fibre reinforced systems. In addition to determining the mechanical deformation of the various composites, the acoustic emission associated with internal deformation mechanisms was studied. This was undertaken with equipment capable of monitoring a range of acoustic pulse parameters.

Abstract

The present study explored the possibilities of using Bacillus coagulans as a probiotic culture in vacuum‐dried milk powder. The operational drying temperature at 63 ± 2 °C under 0.7 kg/cm2 pressure emerged as the best temperature at which to prepare dried milk powder within 4.5 h with a mean (±SEM) Bacillus coagulans B37 spore count of 8.78 ± 0.03 log cfu/g and moisture content of 4.82 ± 0.12%. The total spore counts in vacuum‐dried milk powder did not change (P > 0.10) at storage temperatures of 7 °C, 37 °C or 45 °C over a three‐month period. [ABSTRACT FROM AUTHOR]

Abstract

Mesoporous silica (MPS) films are attractive for isolating Cu wiring in nanodevices but are susceptible to pore wall collapse and water and metal uptake. Pore-sealing and chemical passivation with molecular surfactants are potential solutions that could address these challenges. Here, we show that silylated MPS films capped with a Cu overlayer fracture near the Cu/MPS interface at a distance that correlates with the Cu penetration depth into MPS. Pristine MPS films fracture farther from the MPS/Cu interface than silylated MPS, where silylation-induced pore passivation hinders Cu penetration. Silylation also lowers the tensile stress and the fracture toughness of MPS films, but the relative extent of the decreases in these properties decreases the overall driving force for cracking. Such effects of molecular passivation on metal penetration, film stress, and fracture toughness and pathways are important for engineering stable porous dielectrics for nanodevice wiring structures.
