Abstract: BackgroundProstate cancer risk stratification using single-nucleotide polymorphisms (SNPs) demonstrates considerable promise in men of European, Asian, and African genetic ancestries, but there is still need for increased accuracy. We evaluated whether including additional SNPs in a prostate cancer polygenic hazard score (PHS) would improve associations with clinically significant prostate cancer in multi-ancestry datasets.MethodsIn total, 299 SNPs previously associated with prostate cancer were evaluated for inclusion in a new PHS, using a LASSO-regularized Cox proportional hazards model in a training dataset of 72,181 men from the PRACTICAL Consortium. The PHS model was evaluated in four testing datasets: African ancestry, Asian ancestry, and two of European Ancestry-the Cohort of Swedish Men (COSM) and the ProtecT study. Hazard ratios (HRs) were estimated to compare men with high versus low PHS for association with clinically significant, with any, and with fatal prostate cancer. The impact of genetic risk stratification on the positive predictive value (PPV) of PSA testing for clinically significant prostate cancer was also measured.ResultsThe final model (PHS290) had 290 SNPs with non-zero coefficients. Comparing, for example, the highest and lowest quintiles of PHS290, the hazard ratios (HRs) for clinically significant prostate cancer were 13.73 [95% CI: 12.43-15.16] in ProtecT, 7.07 [6.58-7.60] in African ancestry, 10.31 [9.58-11.11] in Asian ancestry, and 11.18 [10.34-12.09] in COSM. Similar results were seen for association with any and fatal prostate cancer. Without PHS stratification, the PPV of PSA testing for clinically significant prostate cancer in ProtecT was 0.12 (0.11-0.14). For the top 20% and top 5% of PHS290, the PPV of PSA testing was 0.19 (0.15-0.22) and 0.26 (0.19-0.33), respectively.ConclusionsWe demonstrate better genetic risk stratification for clinically significant prostate cancer than prior versions of PHS in multi-ancestry datasets. This is promising for implementing precision-medicine approaches to prostate cancer screening decisions in diverse populations.

Abstract: BackgroundPolygenic hazard scores (PHS) can identify individuals with increased risk of prostate cancer. We estimated the benefit of additional SNPs on performance of a previously validated PHS (PHS46).Materials and method180 SNPs, shown to be previously associated with prostate cancer, were used to develop a PHS model in men with European ancestry. A machine-learning approach, LASSO-regularized Cox regression, was used to select SNPs and to estimate their coefficients in the training set (75,596 men). Performance of the resulting model was evaluated in the testing/validation set (6,411 men) with two metrics: (1) hazard ratios (HRs) and (2) positive predictive value (PPV) of prostate-specific antigen (PSA) testing. HRs were estimated between individuals with PHS in the top 5% to those in the middle 40% (HR95/50), top 20% to bottom 20% (HR80/20), and bottom 20% to middle 40% (HR20/50). PPV was calculated for the top 20% (PPV80) and top 5% (PPV95) of PHS as the fraction of individuals with elevated PSA that were diagnosed with clinically significant prostate cancer on biopsy.Results166 SNPs had non-zero coefficients in the Cox model (PHS166). All HR metrics showed significant improvements for PHS166 compared to PHS46: HR95/50 increased from 3.72 to 5.09, HR80/20 increased from 6.12 to 9.45, and HR20/50 decreased from 0.41 to 0.34. By contrast, no significant differences were observed in PPV of PSA testing for clinically significant prostate cancer.ConclusionsIncorporating 120 additional SNPs (PHS166 vs PHS46) significantly improved HRs for prostate cancer, while PPV of PSA testing remained the same.

Abstract: Genetic models for cancer have been evaluated using almost exclusively European data, which could exacerbate health disparities. A polygenic hazard score (PHS1) is associated with age at prostate cancer diagnosis and improves screening accuracy in Europeans. Here, we evaluate performance of PHS2 (PHS1, adapted for OncoArray) in a multi-ethnic dataset of 80,491 men (49,916 cases, 30,575 controls). PHS2 is associated with age at diagnosis of any and aggressive (Gleason score ≥ 7, stage T3-T4, PSA ≥ 10 ng/mL, or nodal/distant metastasis) cancer and prostate-cancer-specific death. Abstract: Bacillus Calmette-Guérin (BCG) therapy for patients with non-muscle invasive bladder cancer (NMIBC) faces limitations in efficacy and significant side effects, aggravated by a recent global shortage. In this prospective clinical study, we report the outcomes of sequential intravesical administration of gemcitabine and docetaxel (Gem/Doce) as a first-line treatment for BCG-naïve patients with high-risk NMIBC (HR NMIBC). From October 2019 until April 2022, we enrolled 52 patients and followed the treatment protocol set forth by the University of Iowa. Follow-up assessments were conducted every 3 months. In this cohort, 25 (48.1%) patients were diagnosed with high-grade T1 (T1HG) bladder cancer, 10 (19.2%) patients had carcinoma in situ (CIS), and 17 (32.7%) patients had a combination of T1HG+CIS. The median time to first recurrence in the T1HG, CIS, and T1HG+CIS groups was 11, 10.5, and 8.8 months, respectively. The recurrence-free survival was 98.1%, 94.2%, and 80.8% at 6, 9, and 12 months, respectively. The rate of progression-free survival was 100%, 98.1%, and 92.3% at 6, 9, and 12 months, respectively. We demonstrated the safety and efficacy of Gem/Doce therapy in BCG-naïve patients with HR NMIBC during a one-year follow-up. Further research with extended follow-ups, as well as direct comparisons of Gem/Doce with other anticancer agents, is essential. [ABSTRACT FROM AUTHOR]

Abstract: Genome-wide polygenic risk scores (GW-PRSs) have been reported to have better predictive ability than PRSs based on genome-wide significance thresholds across numerous traits. We compared the predictive ability of several GW-PRS approaches to a recently developed PRS of 269 established prostate cancer-risk variants from multi-ancestry GWASs and fine-mapping studies (PRS269). GW-PRS models were trained with a large and diverse prostate cancer GWAS of 107,247 cases and 127,006 controls that we previously used to develop the multi-ancestry PRS269. Resulting models were independently tested in 1,586 cases and 1,047 controls of African ancestry from the California Uganda Study and 8,046 cases and 191,825 controls of European ancestry from the UK Biobank and further validated in 13,643 cases and 210,214 controls of European ancestry and 6,353 cases and 53,362 controls of African ancestry from the Million Vet-eran Program. In the testing data, the best performing GW-PRS approach had AUCs of 0.656 (95% CI = 0.635-0.677) in African and 0.844 (95% CI = 0.840-0.848) in European ancestry men and corresponding prostate cancer ORs of 1.83 (95% CI = 1.67-2.00) and 2.19 (95% CI = 2.14-2.25), respectively, for each SD unit increase in the GW-PRS. Compared to the GW-PRS, in African and European ancestry men, the PRS269 had larger or similar AUCs (AUC = 0.679, 95% CI = 0.659-0.700 and AUC = 0.845, 95% CI = 0.841-0.849, respectively) and comparable prostate cancer ORs (OR = 2.05, 95% CI = 1.87-2.26 and OR = 2.21, 95% CI = 2.16-2.26, respectively). Findings were similar in the validation studies. This investigation suggests that current GW-PRS approaches may not improve the ability to predict prostate cancer risk compared to the PRS269 developed from multi-ancestry GWASs and fine-mapping.

19. Rare Intracranial Multifocal Non-Germinomatous Germ Cell Tumor in an 18-Year-Old Male: A Case Report

Abstract: Intracranial germ cell tumors are rare brain tumors that are distinguished based on their histology and selected tumor markers. Non-germinomatous germ cell tumors are a diverse group of such tumors having the poorest prognosis. Most commonly, they are located in the suprasellar and pineal regions. Since the exact treatment protocol has not yet been established, there is currently no standardized modality of management. We present a case of intracranial multifocal non-germinomatous germ cell tumor in an 18-year-old male, along with relevant literature review. We describe initial diagnostic and treatment procedures in a young adult presented with diplopia and ataxic gait. Neuroradiological findings and elevated alpha fetoprotein and beta chain of the human chorionic gonadotropin tumor markers indicated the possible mixed germ cell tumor. Chemotherapy regimen was adjusted accordingly, biopsy was not performed. The patient’s clinical condition improved significantly and his alpha fetoprotein values decreased remarkably after initiation of chemotherapy. In conclusion, initial evaluation with neuroimaging, tumor markers, and cytology from cerebrospinal fluid is important as guidance to further treatment and prognosis. In selected cases, biopsy may not be indicated to start adjuvant chemotherapy. We emphasize the importance of specific treatment modality selection based mainly on tumor markers, regardless of the precise histologic classification., Intrakranijski tumori zametnih stanica su rijetki tumori mozga koji se razlikuju na temelju histologije i tumorskih biljega. Ne-germinomatozni tumori zametnih stanica su raznolika podskupina tih tumora s najlošijom prognozom. Najčešće su locirani u supraselarnoj i pinealnoj regiji. S obzirom na to da točan protokol liječenja još nije utemeljen trenutno ne postoji standardizirani način liječenja. Predstavljamo slučaj intrakranijskog multifokalnog ne-germinomatoznog tumora zametnih stanica kod 18-godišnjeg mladića, popraćen relevantnim pregledom literature. Opisali smo početne dijagnostičke i terapijske postupke provedene kod mladića koji se prezentirao diplopijom i ataksičnim hodom. Neuroradiološki nalazi i povišeni tumorski biljezi, alfa fetoprotein te beta lanac humanog korionskog gonadotropina, upućivali su na mogući miješani tip tumora zametnih stanica. Propisana je odgovarajuća kemoterapija, a biopsija nije učinjena. Nakon početka kemoterapije bolesnikovo kliničko stanje se iznimno popravilo te su mu se vrijednosti alfa fetoproteina značajno snizile. Zaključno, početna neuroradiološka obrada, nalaz tumorskih biljega i citologija iz likvora važni su čimbenici u određivanju smjera liječenja i predviđanju prognoze. U određenim slučajevima biopsija ne mora biti indicirana kako bi se započelo s kemoterapijom. Naglašavamo važnost odabira specifičnog načina liječenja prvenstveno na temelju nalaza tumorskih biljega bez obzira na preciznu histološku klasifikaciju.

Abstract: Background: As in disease recurrence, providing clinicians with the exact extent of the disease at the time of initial diagnosis is key in the management and individual treatment of prostate cancer (PC) patients. Intending to examine the usefulness of gallium-68 PSMA-11 positron emission tomography/computed tomography ([68Ga]Ga-PSMA-11 PET/CT) and to determine if there is a correlation between prostate-specific antigen (PSA) serum values, WHO/ISUP (World Health Organization/International Society of Urological Pathology's) grade group of the tumor and SUVmax (maximized standardized uptake value) values we retrospectively analyzed PET/CT studies performed for initial staging of the disease. Patients and methods: We retrospectively evaluated 34 studies of patients who underwent [68Ga]Ga-PSMA-11 PET/CT as part of the initial staging of prostate cancer. All patients had prostate cancer confirmed by histological assessment after biopsy and had Gleason score and PSA serum values obtained. The mean PSA value was 33.8 ± 40.9 nmol/L (range 2.2-232). Results: Nineteen patients had extended disease (55.9%). The mean SUVmax in prostate lesions was 19.5 ± 12.6. The mean value of SUVmax of PET studies in the high-risk group was significantly higher than those of low risk (23.5 ± 13.2 and 10.6 ± 5.4, p < 0.05). A positive correlation was observed between the ISUP group and SUVmax value of prostate lesions (Pearson's r = 0.557, p < 0.01). A positive correlation was also found in the comparison between PSA values and SUVmax (Pearson's r = 0.34, p < 0.05). Conclusions: In our study, [68Ga]Ga-PSMA-11 PET/CT scans detected the extended disease in more than half of the patients. Locating disease beyond the prostate gland allowed better informed clinical decisions and modified treatment. A positive correlation was found between intraprostatic SUVmax values and the ISUP group of prostate cancer. High-risk patients had SUVmax values that were significantly higher than those of low-risk patients. The correlation between the Gleason score and SUVmax value can be explained by the increased intensity of PSMA expression as the tumor grade increases. [ABSTRACT FROM AUTHOR]

26. First line therapy of metastatic urothelial bladder cancer

Abstract: This manuscript reviewes recent knowledge regarding first line therapy of metastatic urothelial bladder cancer. Bladder cancer is on the 10th place in the world by its incidence, and more prevalent in men. Patients with metastatic urothelial cancer should be classified into one of the two groups: cisplatin eligible and cisplatin-ineligible. Cisplatin-eligible can be treated with cisplatin based regimens and have better outcome. Cisplatin-ineligible patients (40-50%) are cisplatin-ineligible patients are primarily those with creatinine clirence les than 50 ml/min, the ones with certain comorbities and/or poor ECOG performance status, and, as an alternative, can be treated with carboplatin which is less effective. After the diagnosis of metastatic bladder cancer has been confirmed, it is necessary to choose one of the cisplatin based chemotherapy regimens. However, one should have in mind that cisplatin can cause certain side effects such as nephrotoxic, neurotoxic and ototoxic effects. A minority of patients are not eligible for any platinum-containing chemotherapy regimen. Besides chemotherapy regimens, checkpoint inhibitors (CPIs)-PD-1 and PD-L1 inhibitors play an important role in first-line therapy of metastatic urothelial cancer. NCCN guidelines have included avelumab, pembrolizumab and atezolizumab in a first-line systemic therapy. Recently, Javelin Bladder 100 study has confirmed a positive impact of avelumab as a maitenance therapy in cisplatin-eligible and cisplatin-ineligible patients, which is why the combination of chemotherapy and avelumab is nowadays deemed to be the best therapeutic option., Ovaj rukopis daje pregled nedavnih saznanja o prvoj liniji terapije metastatskog urotelnog raka mokraćnog mjehura. Rak mokraćnog mjehura je na 10. mjestu u svijetu po učestalosti, a češći je u muškaraca. Bolesnike s metastatskim urotelnim rakom svrstava se u dvije skupine: prihvatljivi za liječenjem cisplatinom i nepodobni za liječenje cisplatinom. Bolesnici prihvatljivi za liječenje cisplatinom mogu se liječiti režimima temeljenim na cisplatini i imati bolji ishod. Bolesnici koji ne ispunjavaju uvjete za liječenje cisplatinom, njih oko 40-50%, su oni s određenim komorbitetima i/ili lošim ECOG statusom te se mogu liječiti karboplatinom koja je, međutim, manje učinkovita. Nakon potvrde dijagnoze metastatskog urotelnog raka mokraćnog mjehura, potrebno je odabrati jedan od režima kemoterapije na bazi cisplatine. Međutim, treba imati na umu da cisplatina može uzrokovati nuspojave poput nefrotoksičnosti, neurotoksičnosti i ototoksičnosti. Danas u u prvoj liniji terapije važnu ulogu igraju inhibitori kontrolnih točaka (CPI), PD-1 i PD-L1. Smjernice NCCN-a uključuju avelumab, pembrolizumab i atezolizumab u sustavnu terapiju prve linije. Nedavno je studija ″Javelin bladder 100″ potvrdila pozitivan utjecaj avelumaba kao terapije održavanja u bolesnika koji ispunjavaju uvjete za liječenje cisplatinom i koji ne ispunjavaju uvjete za liječenje cisplatinom, zbog čega se kombinacija kemoterapije i avelumaba trenutno smatra najboljom terapijskom opcijom.

Abstract: Simple Summary Previous studies have shown heavy intake of different alcoholic beverages affects prostate cancer (PCa) clinical outcomes differently. However, the intake patterns of specific alcoholic beverages for PCa status are understudied. The study's objective is to evaluate intake patterns of total alcohol and three types of alcoholic beverage (beer, wine, and spirits) by PCa risk and aggressiveness status. This study included 10,029 men with European ancestry (4676 non-PCa men and 5353 PCa patients). We found PCa patients had a similar total heavy alcohol intake compared with non-PCa men. However, PCa patients were likely to drink more wine and spirits than non-PCa men. Patients with aggressive PCa drank more beer but not wine and spirits. Interestingly, heavy wine intake was inversely associated with PCa aggressiveness. These findings suggest that the intake patterns of specific alcoholic beverages differ by PCa status, and this information might help develop personalized alcohol intervention for PCa patients. Abstract: Background: Testicular germ cell tumors (TGCT), histologically classified as seminomas and nonseminomas, are believed to arise from primordial gonocytes, with the maturation process blocked when they are subjected to DNA methylation reprogramming. SNPs in DNA methylation machinery and folate-dependent one-carbon metabolism genes have been postulated to influence the proper establishment of DNA methylation. Methods: In this pathway-focused investigation, we evaluated the association between 273 selected tag SNPs from 28 DNA methylation-related genes and TGCT risk. We carried out association analysis at individual SNP and gene-based level using summary statistics from the Genome Wide Association Study meta-analysis recently conducted by the international Testicular Cancer Consortium on 10,156 TGCT cases and 179,683 controls. Results: In individual SNP analyses, seven SNPs, four mapping within MTHFR, were associated with TGCT risk after correction for multiple testing (q = 0.05). Queries of public databases showed that three of these SNPs were associated with MTHFR changes in enzymatic activity (rs1801133) or expression level in testis tissue (rs12121543, rs1476413). Gene-based analyses revealed MTHFR (q = 8.4 × 10-4), methyl-CpG-binding protein 2 (MECP2; q = 2 × 10-3), and ZBTB4 (q = 0.03) as the top TGCT-associated genes. Stratifying by tumor histology, four MTHFR SNPs were associated with seminoma. In gene-based analysis MTHFR was associated with risk of seminoma (q = 2.8 × 10-4), but not with nonseminomatous tumors (q = 0.22). Conclusions: Genetic variants within MTHFR, potentially having an impact on the DNA methylation pattern, are associated with TGCT risk. Abstract: Purpose: Radiation therapy is a possible treatment strategy for patients with testicular seminoma after orchiectomy in clinical stage I or II disease. Little is known about the outcome of patients who experience a relapse after radiation therapy. Methods and Materials: Data from 61 patients who relapsed after adjuvant or curative radiation therapy from 17 centers in 11 countries were collected and retrospectively analyzed. Primary outcomes were disease-free and overall survival. Secondary outcomes were time to relapse, stage at relapse, treatment for relapse, and rate of febrile neutropenia during chemotherapy for relapse. Results: With a median follow-up of 9.9 years (95% confidence interval [CI], 7.5-10.9), we found a 5-year disease-free survival of 90% (95% CI, 79-95) and a 5-year overall survival of 98% (95% CI, 89-100). Sixty-six percent of patients had stage III disease at time of relapse and 93% of patients fell into the good prognosis group per the International Germ Cell Cancer Collaborative Group classification. The median time to relapse after radiation therapy was 15.6 months (95% CI, 12-23). Twenty-two (36%) patients relapsed more than 2 years after radiation therapy and 7 (11.5%) patients relapsed more than 5 years after radiation therapy. One-third of relapses was detected owing to patients' symptoms, whereas two-thirds of relapses were detected during routine follow-up. The majority (93%) of cases were treated with cisplatin-based chemotherapy. The rate of febrile neutropenia during chemotherapy was 35%. Five patients experienced a second relapse. At last follow-up, 55 patients (90%) were alive without disease. Only 1 patient died owing to disease progression. Conclusions: Cisplatin-based chemotherapy for patients with seminoma who have relapsed after treatment with radiation therapy alone leads to excellent outcomes. 35. Prostate cancer and environmental exposure to toxic metal(loid)s: impact on prostate specific antigen (PSA)

Abstract: Prostate cancer is among the most commonly diagnosed malignancies in men and its incidence increases with age. In addition to family history, race, and ethnicity, various external risk factors may contribute to its increased risk. Arsenic (As), cadmium (Cd), lead (Pb) and mercury (Hg) are pervasive environmental contaminants and several experimental and epidemiological studies linked exposure to these metal(loid)s with an increased risk of prostate cancer. However, research on the impact of low-level exposure is scanty and largely inconclusive. Prostate specific antigen (PSA), combined with a digital rectal examination of prostate, has been routinely used for early detection of prostate cancer in spite of its insufficient specificity for clinically important cancers. Moreover, several studies in presumably healthy men indicated that various external factors can affect PSA levels, including metal(loid) exposure. Therefore, we aimed to evaluate the role of environmental exposure to As, Cd, Pb and Hg in prostate cancer risk and their impact on serum PSA. Blood As, Cd, Pb, and Hg and serum PSA were measured in 62 prostate cancer patients and 30 control men with no occupational exposure to metals. Prostate cancer patients had significantly higher serum PSA and blood Hg than control subjects. The results of Spearman’s correlation and a simple linear regression analysis in all 92 subjects showed a positive association between blood Hg and PSA. However, after control for the impact of the remaining elements in the models of multiple regression, this association was lost. Although we did not confirm a linear association between measured metal(loid)s and PSA in conditions of environmental exposure, further investigation in larger cohorts, taking into consideration possible synergistic or non-linear mixture effects, are warranted.

39. Urinary profiles of free amino acids and metal(loid)s in testicular cancer

Abstract: Testicular germ cell tumours (TGCT) are the most common malignancy in men aged 15–35 years and its incidence is increasing worldwide. Tumour markers commonly used in the diagnosis and prognosis of TGCT have limited sensitivity and specificity and additional, minimally invasive biomarkers for disease management are needed. Metabolomic-based profiling of amino acids in urine may identify changed metabolites as potential biomarkers of testicular cancer. Impaired metal(loid) homeostasis may increase susceptibility to cancer development. We compared a quantitative profile of 30 amino acids and 18 metal(loid)s in urine samples between 86 men with newly diagnosed TGCT and 68 control subjects, aged 18 to 55 years. The concentration of free amino acids was determined by GC-MS using Phenomenex EZ:faast kit for sample preparation, multi-element analysis was performed by ICP-MS, and all results were corrected for creatinine levels. Testicular cancer patients had significantly higher levels of aspartic acid (Asp), manganese (Mn) and zinc (Zn), and significantly lower levels of threonine (Thr), serine (Ser), histidine (His) and cobalt (Co) when compared to control subjects. Linear discriminant analysis using all 30 amino acids successfully separated subjects with TGCT from control subjects and the discrimination rate was 86%. Metabolic pathway analysis (MetaboAnalyst 4.0) showed that Asp, Thr, Ser, and His were related to alterations in 12 metabolic pathways and those with the highest impact were metabolism of alanine, aspartate and glutamate (0.26) and glycine, Thr and Ser (0.23). Our results indicated that disorders in the citric acid cycle, the urea cycle and amino acid metabolism play a role in the pathogenesis of testicular cancer. The impaired urinary profile of Mn, Co and Zn may be highly relevant with regard to their role in amino acids synthesis, antioxidant defense and genetic repair processes. 40. Role of F-18-choline PET/CT in staging of prostate cancer

Abstract: Introduction: The aim of this study was to evaluate the value of F-18 choline PET/CT in initial staging of high-risk prostate cancer patients. Subjects and methods: Hundred consecutive biopsy confirmed prostate cancer patients were scanned with F-18-choline PET/CT from February 2012 to April 2018. Mean PSA value at referral was 48, 4 ng/ml (range 4, 3 to 280, 6). Functional imaging was proposed in high risk patients and after equivocal conventional imaging and bone scintigraphy findings. Imaging was performed after administration of F-18 choline (2- 3 MBq/kg, mean 209 MBq). All patients had a minimum 6-month follow-up. Results: There were 24 patients without metastases, while the majority of patients (76%) had a choline scan positive for metastatic disease. Eight patients had a single positive pelvic lymph node, with a short axis diameter shorter than 1 cm. 56 patients had FCH positive metastastic disease in lymph nodes only (mean SUV 6, 4), 25 in lymph nodes and/or skeleton (mean SUV 10, 7). In sixteen patients seminal vesicles’ infiltration was found. Majority of our patient population had a combination of radiotherapy and hormonal therapy, with the mean PSA levels after six months significantly lower (2, 1 ng/ml). Conclusion: F-18-choline PET/CT provides clinicians with valuable information in staging of high-risk prostate cancer. It has an important impact on therapeutic strategy, providing additional data necessary for the appropriate and individual patient management.

Abstract: It remains elusive whether some of the associations identified in genome-wide association studies of prostate cancer (PrCa) may be due to regulatory effects of genetic variants on CpG sites, which may further influence expression of PrCa target genes. To search for CpG sites associated with PrCa risk, here we establish genetic models to predict methylation (N = 1,595) and conduct association analyses with PrCa risk (79,194 cases and 61,112 controls). We identify 759 CpG sites showing an association, including 15 located at novel loci. Among those 759 CpG sites, methylation of 42 is associated with expression of 28 adjacent genes. Among 22 genes, 18 show an association with PrCa risk. Overall, 25 CpG sites show consistent association directions for the methylation-gene expression-PrCa pathway. We identify DNA methylation biomarkers associated with PrCa, and our findings suggest that specific CpG sites may influence PrCa via regulating expression of candidate PrCa target genes.

Abstract: Background: Polygenic hazard scores (PHS) can identify individuals with increased risk of prostate cancer. We estimated the benefit of additional SNPs on performance of a previously validated PHS (PHS46). Materials and method: 180 SNPs, shown to be previously associated with prostate cancer, were used to develop a PHS model in men with European ancestry. A machine-learning approach, LASSO-regularized Cox regression, was used to select SNPs and to estimate their coefficients in the training set (75,596 men). Performance of the resulting model was evaluated in the testing/validation set (6,411 men) with two metrics: (1) hazard ratios (HRs) and (2) positive predictive value (PPV) of prostate-specific antigen (PSA) testing. HRs were estimated between individuals with PHS in the top 5% to those in the middle 40% (HR95/50), top 20% to bottom 20% (HR80/20), and bottom 20% to middle 40% (HR20/50). PPV was calculated for the top 20% (PPV80) and top 5% (PPV95) of PHS as the fraction of individuals with elevated PSA that were diagnosed with clinically significant prostate cancer on biopsy. Results: 166 SNPs had non-zero coefficients in the Cox model (PHS166). All HR metrics showed significant improvements for PHS166 compared to PHS46: HR95/50 increased from 3.72 to 5.09, HR80/20 increased from 6.12 to 9.45, and HR20/50 decreased from 0.41 to 0.34. By contrast, no significant differences were observed in PPV of PSA testing for clinically significant prostate cancer. Conclusions: Incorporating 120 additional SNPs (PHS166 vs PHS46) significantly improved HRs for prostate cancer, while PPV of PSA testing remained the same.

Abstract: Genetic models for cancer have been evaluated using almost exclusively European data, which could exacerbate health disparities. A polygenic hazard score (PHS1) is associated with age at prostate cancer diagnosis and improves screening accuracy in Europeans. Here, we evaluate performance of PHS2 (PHS1, adapted for OncoArray) in a multi-ethnic dataset of 80,491 men (49,916 cases, 30,575 controls). PHS2 is associated with age at diagnosis of any and aggressive (Gleason score ≥ 7, stage T3-T4, PSA ≥ 10 ng/mL, or nodal/distant metastasis) cancer and prostate-cancer-specific death. Associations with cancer are significant within European (n = 71,856), Asian (n = 2,382), and African (n = 6,253) genetic ancestries (p < 10−180). Comparing the 80th/20th PHS2 percentiles, hazard ratios for prostate cancer, aggressive cancer, and prostate-cancer-specific death are 5.32, 5.88, and 5.68, respectively. Within European, Asian, and African ancestries, hazard ratios for prostate cancer are: 5.54, 4.49, and 2.54, respectively. PHS2 risk-stratifies men for any, aggressive, and fatal prostate cancer in a multi-ethnic dataset.

Abstract: Prostate cancer is a highly heritable disease with large disparities in incidence rates across ancestry populations. We conducted a multiancestry meta-analysis of prostate cancer genome-wide association studies (107,247 cases and 127,006 controls) and identified 86 new genetic risk variants independently associated with prostate cancer risk, bringing the total to 269 known risk variants. The top genetic risk score (GRS) decile was associated with odds ratios that ranged from 5.06 (95% confidence interval (CI), 4.84-5.29) for men of European ancestry to 3.74 (95% CI, 3.36-4.17) for men of African ancestry. Men of African ancestry were estimated to have a mean GRS that was 2.18-times higher (95% CI, 2.14-2.22), and men of East Asian ancestry 0.73-times lower (95% CI, 0.71-0.76), than men of European ancestry. These findings support the role of germline variation contributing to population differences in prostate cancer risk, with the GRS offering an approach for personalized risk prediction. Abstract: Abstract Background Development of graphical/visual presentations of cancer etiology caused by environmental stressors is a process that requires combining the complex biological interactions between xenobiotics in living and occupational environment with genes (gene-environment interaction) and genomic and non-genomic based disease specific mechanisms in living organisms. Traditionally, presentation of causal relationships includes the statistical association between exposure to one xenobiotic and the disease corrected for the effect of potential confounders. Methods Within the FP6 project HENVINET, we aimed at considering together all known agents and mechanisms involved in development of selected cancer types. Selection of cancer types for causal diagrams was based on the corpus of available data and reported relative risk (RR). In constructing causal diagrams the complexity of the interactions between xenobiotics was considered a priority in the interpretation of cancer risk. Additionally, gene-environment interactions were incorporated such as polymorphisms in genes for repair and for phase I and II enzymes involved in metabolism of xenobiotics and their elimination. Information on possible age or gender susceptibility is also included. Diagrams are user friendly thanks to multistep access to information packages and the possibility of referring to related literature and a glossary of terms. Diagrams cover both chemical and physical agents (ionizing and non-ionizing radiation) and provide basic information on the strength of the association between type of exposure and cancer risk reported by human studies and supported by mechanistic studies. Causal diagrams developed within HENVINET project represent a valuable source of information for professionals working in the field of environmental health and epidemiology, and as educational material for students. Introduction Cancer risk results from a complex interaction of environmental exposures with inherited gene polymorphisms, genetic burden collected during development and non genomic capacity of response to environmental insults. In order to adopt effective preventive measures and the associated regulatory actions, a comprehensive investigation of cancer etiology is crucial. Variations and fluctuations of cancer incidence in human populations do not necessarily reflect environmental pollution policies or population distribution of polymorphisms of genes known to be associated with increased cancer risk. Tools which may be used in such a comprehensive research, including molecular biology applied to field studies, require a methodological shift from the reductionism that has been used until recently as a basic axiom in interpretation of data. The complexity of the interactions between cells, genes and the environment, i.e. the resonance of the living matter with the environment, can be synthesized by systems biology. Within the HENVINET project such philosophy was followed in order to develop interactive causal diagrams for the investigation of cancers with possible etiology in environmental exposure. Results Causal diagrams represent integrated knowledge and seed tool for their future development and development of similar diagrams for other environmentally related diseases such as asthma or sterility. In this paper development and application of causal diagrams for cancer are presented and discussed.

Abstract: Abstract The role of steroids in carcinogenesis has become a major concern in environmental protection, biomonitoring, and clinical research. Although historically oestrogen has been related to development of reproductive system, research over the last decade has confirmed its crucial role in the development and homeostasis of other organ systems. As a number of anthropogenic agents are xenoestrogens, environmental health research has focused on oestrogen receptor level disturbances and of aromatase polymorphisms. Oestrogen and xenoestrogens mediate critical points in carcinogenesis by binding to oestrogen receptors, whose distribution is age-, gender-, and tissue-specific. This review brings data about cancer types whose eatiology may be found in environmental exposure to xenoestrogens. Cancer types that have been well documented in literature to be related with environmental exposure include the reproductive system, breast, lung, kidney, pancreas, and brain. The results of our data mining show (a) a significant correlation between exposure to xenoestrogens and increased, gender-related, cancer risk and (b) a need to re-evaluate agents so far defined as endocrine disruptors, as they are also key molecules in carcinogenesis. This revision may be used to further research of cancer aetiology and to improvement of related legislation. Investigation of cancers caused by xenoestrogens may elucidate yet unknown mechanisms also valuable for oncology and the development of new therapies.
