Gamma-Hydroxybutyrate (GHB) overdoses cause respiratory depression, coma, or even death. Symptoms and severity of poisoning depend on blood-concentrations and individual factors such as tolerance. A retrospective case study was conducted, evaluating GHB intoxication cases. GHB-concentrations in blood and urine were determined by gas chromatography-mass spectrometry (GC-MS) along with, in part, via enzymatic assay. GHB-concentrations, demographic data, and additional drug use, as well as specific clinical information, were evaluated. The correlation between GHB-levels in blood and associated symptoms were examined. In total, 75 cases originating from the Emergency Departments (EDs) of Hamburg and surrounding hospitals were included. Fifty-four of the patients (72%) were male. The mean GHB-concentration in blood was 248 mg/L (range 21.5-1418 mg/L). Out of the group with detailed clinical information (n = 18), the comatose group (n = 10/18) showed a mean of 244 mg/L (range 136-403 mg/L), which was higher than that of the somnolent and awake patients. Of the comatose collective, 70% (n = 7) showed co-use of one or more substances, with the additional use of cocaine being the most frequently detected (n = 5). In conclusion, a moderate dose-effect relationship was observed, although, there was some overlap in dosage concentration levels of GHB in awake and comatose patients. In GHB-intoxication cases, co-use was common as were clinical effects such as acidosis, hypotension, and impact on the heart rate. Timely analytical determination of the GHB-concentration in blood could support correct diagnosis of the cause of unconsciousness.

ObjectivesGamma-hydroxybutyrate (GHB) is a drug of abuse with central depressing effects, which may cause coma with a GCS score as low as 3. A rapid diagnosis ‘GHB intoxication’ may prevent unnecessary diagnostic work-up and may lead to guided, less invasive, treatment. The aim of this study was to evaluate if ED physicians’ clinical evaluation were sufficient for diagnosis in patients with suspected GHB-intoxication.MethodsPatients presenting at the ED with a GCSResultsA total of 506 patients were included, 100 patients tested positive for GHB and 406 patients tested negative for GHB. Sensitivity and specificity of the ED physicians compared with the confirmation test to diagnose GHB intoxications were 63% (95% CI 52 to 73) and 93% (95% CI 90 to 95), respectively. The positive predictive value was 67% (95% CI 60 to 77) and the negative predictive value was 92% (95% CI 88 to 94).ConclusionPhysicians underestimate the presence of GHB intoxication and can fail to diagnose GHB intoxication based on clinical observations alone. In the future, a rapid reliable initial analytical GHB test in addition to clinical judgement could be valuable to reduce false negative diagnosis.

Objective The objective of this study was to determine if supportive care without endotracheal intubation in the emergency department (ED) was safe in the absence of complications in gamma-hydroxybutyrate (GHB)/gamma-butyrolactone (GBL) intoxicated patients with a decreased Glasgow Coma Scale (GCS) score. Methods This was a retrospective chart review of patients presenting to a Dutch tertiary urban ED with a reduced level of consciousness related to alleged GHB/GBL intoxication between April 2011-December 2014. Primary endpoint was major adverse events, defined by: upper airway obstruction not resolved with mayo tube or nasopharyngeal airway, hypoxia not resolved with 15 l of oxygen delivered via non-rebreathing mask, bradypnea not resolved after stimulation, intubation, bradycardia not resolved after intravenous atropine bolus, hypotension for which inotropes were started. Results Data of 209 patients were retrieved. Major adverse events were reported in five patients (2.4%; 95% CI: 0.8-5.5). Intubation with subsequent ICU admission was required for 1.4% of patients (95% CI: 0.3-4.1). The most frequently seen minor adverse events (N = 209) were: airway obstruction (22%), hypothermia (14.8%), hypoxia (12.9%), bradycardia (8.1%), hypotension (6.7%), bradypnea (5.7%), vomiting (5.3%). There were no deaths. None of the patients had signs of aspiration pneumonia or returned to our ED due to complications. Conclusion Our study suggests that conservative airway management for patients with a decreased GCS due to suspected GHB intoxication may be safe. Major adverse events were present in 2.4% of patients, only 1.4% of patients required intubation. All minor adverse events were managed effectively with conservative treatment.

Gamma-hydroxybutyrate (GHB) is a central nervous system depressant primarily used as a recreational drug of abuse, but also for the treatment of narcolepsy with cataplexy in adult patients and as an adjuvant for control of alcohol withdrawal syndrome. The main aim of this review is to summarise updated knowledge about GHB pharmacokinetics and pharmacodynamics, acute poisoning, and clinical features of GHB withdrawal syndrome, its diagnosis and medical treatment. The most common clinical signs and symptoms of acute poisoning include sleepiness to deep coma, bradycardia, hypotension, and respiratory failure. Therapy is essentially supportive and based on continuous monitoring of vital signs. GHB withdrawal syndrome shares patterns with other withdrawal syndromes such as alcohol withdrawal and is sometimes difficult to distinguish, especially if toxicological tests are GHB-negative or cannot be performed. There are no official detoxification protocols for GHB withdrawal syndrome, but its therapy is based on benzodiazepine. When benzodiazepine alone is not effective, it can be combined with barbiturates or antipsychotics. Information about abuse and distribution of GHB and its precursors/analogues among the general population is still limited. Their prompt identification is therefore crucial in conventional and non-conventional biological matrices, the latter in particular, to clarify all the issues around this complex molecule., Gama-hidroksibutirat (GHB) depresiv je središnjega živčanog sustava koji se ponajviše zlorabi kao rekreativna droga, ali se primjenjuje i za liječenje narkolepsije s katapleksijom u odraslih bolesnika te kao pomoćni lijek u kontroli sindroma odvikavanja od alkohola. Ljudi i endogeno proizvode GHB, budući da je on prekursor gama-aminomaslačne kiseline (GABA), jednoga od glavnih neurotransmitera s inhibicijskim učinkom na živčani sustav. GHB ima ulogu neuromodulatora u GABA sustavu i uglavnom djeluje preko GABAB receptora. Za zloporabu se čestu kupuju GBH prekursori/analozi gama-butirolakton (GBL) i 1,4-butanediol (1,4-BD) u obliku otapala namijenjenih legalnoj industrijskoj uporabi. Oni, međutim, nisu endogeni. Glavni je cilj ovoga preglednog rada bio sažeti najnovije spoznaje o farmakokinetici i farmakodinamici GHB-a, akutnom trovanju i o kliničkim značajkama sindroma odvikavanja od GHB-a, njegovoj dijagnozi i liječenju. Akutno je trovanje GHB-om donekle i dijagnostički izazov jer su znakovi i simptomi uobičajeni i za depresive središnjega živčanog sustava. Najčešći su klinički znakovi akutnoga trovanja pospanost (sve do duboke kome), bradikardija i hipotenzija te respiratorno zatajenje. Liječenje je u osnovi potporno, uz stalno praćenje vitalnih znakova. Sindrom odvikavanja od GHB-a dijeli značajke s drugim sindromima odvikavanja (npr. od alkohola ili benzodiazepina). Ponekad ih je vrlo teško razlikovati, ponajviše kada su testovi na GHB negativni ili nije moguće napraviti toksikološku analizu. Ne postoje službeni protokoli za detoksikaciju od GHB-a ili njegovih prekursora/analoga. Liječenje se temelji na primjeni benzodiazepina te, ovisno o težini simptoma, sekundarno barbituratima ili antipsihoticima.

The authors reviewed case reports of patients presenting to an advanced medical assessment and resuscitation service at 15 music events over 22 days from June 2018 through March 2019 around Australia. Event size ranged from 4,000 to 57,500 participants. Events observed had a mean patient presentation rate (PPR) of 0.83% (SD = 0.59%) and mean transport to hospital rate (TTHR) of 1.89 (SD = 0.92) per 10,000. Two-hundred and twenty-one cases were reviewed and tabulated for descriptive analysis.Lower rates of traumatic injuries were seen compared to other case reports, and minor procedures represented a minor but important part of the team’s workload. Methylenedioxymethamphetamine (MDMA) use was reported by 33.0% of patients on the day of presentation; almost one-half of these reported a co-ingestion. Patients presenting after using MDMA were more likely to have an elevated temperature. Eight percent of patients presented with temperature above 38°C. Patients with an initial temperature above 38°C were more likely to require hospitalization. On-site electrocardiograph (ECG), blood gas, ultrasound, and urinalysis were found to be useful in decision support. In total, 29.8% of patients required sedation during their encounter; 2.7% required rapid sequence induction at the event. Mean observation time was 44 minutes, with longer observation required in MDMA and hallucinogen-related presentations.

Beta‐hydroxybutyrate (BHB) is a synthetic ketone body used as an adjuvant energy substrate in the treatment of patients with metabolic cardiomyopathy. A medication prescribing error led to the administration of the general anesthetic sodium gamma‐hydroxybutyrate (GHB) instead of sodium BHB in a liver transplant recipient with propionic acidemia and cardiomyopathy, causing acute coma. A 15‐year‐old boy suffering from neonatal propionic acidemia underwent liver transplantation (LT) for metabolic decompensation and cardiomyopathy (treated with cardiotropic drugs and BHB) diagnosed a year previously. The patient had been rapidly extubated after LT, and was recovering well. Eight days after LT, the patient suddenly became comatose. No metabolic, immunological, hypertensive, or infectious complications were apparent. The brain magnetic resonance imaging and electroencephalography results were normal. The coma was soon attributed to a medication prescribing error: administration of GHB instead of BHB on day 8 post‐LT. The patient recovered fully within a few hours of GHB withdrawal. The computerized prescription system had automatically suggested the referenced anesthetic GHB (administered intravenously) instead of the non‐referenced ketone body BHB, triggering coma in our patient. A computerized prescription system generated a medication prescribing error for a rare disease, in which the general anesthetic GHB was mistaken for the nonreferenced energy substrate BHB.

Background Gamma hydroxybutyrate (GHB) is an illicit drug commonly used in music festival, party and 'chemsex' settings. Most people who use GHB do so occasionally, without dependent use or withdrawal symptoms. However, a minority of users experience harms including unconsciousness and respiratory collapse in overdose. Adverse interactions can also occur when GHB is used with other drugs (eg methamphetamine), necessitating assessment, management or onward referral by general practitioners. Objective This article describes the use of GHB, with a contemporary update on principles of assessment and management in general practice, brief intervention and harm-minimisation strategies, and indicators for referral to a specialist in dependent use. Discussion The assessment and management of individuals with GHB-related harms in general practice is supported by an awareness of the context of use, familiarity with targeted harm-minimisation advice and cognisance of markers of risk indicating onward referral to specialist addiction services when appropriate.

Narcolepsy is a chronic and debilitating sleep disorder characterized by cataplexy and excessive daytime sleeping. Gamma-hydroxybutyrate (GHB) has been widely used to treat narcolepsy, and new findings have been published in recent years.A meta-analysis was conducted to assess the efficacy and tolerability of GHB treatment in adults with narcolepsy.A systematic search of PubMed, Cochrane, Embase, Web of Science, and clinical-trials.gov from inception to June 2018 was performed. Change in daily diaries and polysomnographic data of narcoleptic patients were defined as the efficacy outcomes. The tolerability and acceptability outcomes were the rates of adverse events and dropping out for adverse effects or other reasons.Fifteen randomized controlled trials involving 2104 participants were identified. GHB was found to improve cataplexy attacks (P = 0.001), subjective daytime sleepiness (P 0.0001), daytime sleep latency (P 0.0001), inadvertent naps/sleep attacks (P 0.00001), effective rates (Clinical Global Impression of change) (P 0.00001), hypnagogic hallucinations (P = 0.004), sleep paralysis (P = 0.004), stage 1 sleep (P = 0.04), slow wave sleep (P = 0.003), REM sleep (P = 0.0006), sleep shifts (P = 0.005), nocturnal awakenings (P = 0.004), quality of nocturnal sleep (P 0.00001), chin muscle activity, and quality of life, but had no effect on stage 2 sleep (P = 0.88). GHB was less well tolerated than placebo because of side effects that occurred in a dose-dependent fashion (RR = 6.08; 95% CI = 2.18 to 16.97; P = 0.0006).GHB was effective in improving narcolepsy-cataplexy and related symptoms in adults but was less well tolerated than placebo because of dose-dependent side effects.

Gamma-hydroxybutyrate (GHB) and its various pro-drugs are GABA-B central nervous system depressants more commonly known as "G" and "liquid ecstasy" among those who consume. Due to the depressant quality of the substance and its convenience in online marketplaces, it has become a more commonly abused drug, characteristically among people involved in the "club scene" and among men who have sex with men. As the dose of GHB necessary to obtain feelings of euphoria is relatively low combined with the substance's rapid metabolism, GHB is nearly undetectable through traditional emergency department urinary drug testing. Consequentially, although the treatment of GHB intoxication and withdrawal is mainly supportive, it is difficult for providers to determine whether GHB is the cause of a patient's symptoms. Moreover, when GHB is combined with other substances, treatment becomes more complicated. The author presents a case of GHB intoxication and withdrawal in a patient with an extensive polysubstance use history demonstrating not only the difficulty of diagnosis but also a timeline outlining the interventions taken from arrival to discharge with a successful outcome faster than the conventional timeline of GHB withdrawal and recovery.
