Abstract

Background: Hypertension management in older patients represents a challenge, particularly when hospitalized. Objective: The objective of this study is to investigate the determinants and related outcomes of antihypertensive drug prescription in a cohort of older hospitalized patients. Methods: A total of 5671 patients from REPOSI (a prospective multicentre observational register of older Italian in-patients from internal medicine or geriatric wards) were considered; 4377 (77.2%) were hypertensive. Minimum treatment (MT) for hypertension was defined according to the 2018 ESC guidelines [an angiotensin-converting-enzyme-inhibitor (ACE-I) or an angiotensin-receptor-blocker (ARB) with a calcium-channel-blocker (CCB) and/or a thiazide diuretic; if >80 years old, an ACE-I or ARB or CCB or thiazide diuretic]. Determinants of MT discontinuation at discharge were assessed. Study outcomes were any cause rehospitalization/all cause death, all-cause death, cardiovascular (CV) hospitalization/death, CV death, non-CV death, evaluated according to the presence of MT at discharge. Results: Hypertensive patients were older than normotensives, with a more impaired functional status, higher burden of comorbidity and polypharmacy. A total of 2233 patients were on MT at admission, 1766 were on MT at discharge. Discontinuation of MT was associated with the presence of comorbidities (lower odds for diabetes, higher odds for chronic kidney disease and dementia). An adjusted multivariable logistic regression analysis showed that MT for hypertension at discharge was associated with lower risk of all-cause death, all-cause death/hospitalization, CV death, CV death/hospitalization and non-CV death. Conclusions: Guidelines-suggested MT for hypertension at discharge is associated with a lower risk of adverse clinical outcomes. Nevertheless, changes in antihypertensive treatment still occur in a significant proportion of older hospitalized patients.

Abstract

Coronavirus disease 2019 (COVID-19) is a newly recognized infectious disease which can lead to acute respiratory distress syndrome requiring ventilatory support and intensive care unit admission. The aim of our study is to evaluate the performance of two non-invasive respiratory function indices (the ROX index and the SatO2/FiO2 ratio), as compared to the traditional PaO2/FiO2 ratio, in predicting a clinically relevant composite outcome (death or intubation) in hospitalized patients for COVID-19 pneumonia. Four hospital centers in Northern Italy conducted an observational retrospective cohort study during the first wave of COVID-19 pandemic. Four hundred and fifty-six patients with COVID-19 pneumonia admitted to medical or sub-intensive wards were enrolled. Clinical, laboratory, and respiratory parameters, for the calculation of different indices, were measured at hospital admission. In medical wards (Verona and Padua) the PaO2/FiO2 ratio, ROX index and SatO2/FiO2 ratio were able to predict intubation or death with good accuracy (AUROC for the PaO2/FiO2 ratio, ROX index and SatO2/FiO2 ratio of 75%, 75% and 74%, respectively). Regarding sub-intensive wards (Milan and Mantua), none of the three respiratory function indices was significantly associated with the composite outcome. In patients admitted to medical wards for COVID-19 pneumonia, the ROX index and the SatO2/FiO2 ratio demonstrated not only good performance in predicting intubation or death, but their accuracy was comparable to that of the PaO2/FiO2 ratio. In this setting, where repeated arterial blood gas tests are not always feasible, they could be considered a reliable alternative to the invasive PaO2/FiO2 ratio.

5. Evaluation of LiverRisk score as predictor of liver fibrosis and mortality in patients with HCV-related hepatitis treated with direct acting antivirals.

Abstract

The LiverRisk score has been recently developed and validated as a predictor of liver fibrosis and liver-related outcomes in the general population. This score has never been evaluated as predictor of liver fibrosis and outcomes in secondary care, such as in patients with chronic liver diseases. The aim of this study was to evaluate the role of the LiverRisk score as a predictor of liver fibrosis and mortality in patients with HCV-related hepatitis treated with direct acting antivirals (DAA). patients were enrolled retrospectively, outpatients with chronic hepatitis C treated with DAA between 2015 and 2017 were included consecutively. Patients were followed-up until September 2023. The exclusion criteria were: liver transplantation before DAAs and presence of HCC. Patient characteristics and LiverRisk score were collected before starting the DAA. The data for the calculation of LiverRisk score were collected the same day the fibroscan was performed. The primary endpoint was a liver stiffness ≥10 kPa. Area under the receiver operating characteristic (AUROC) curve was evaluated for assessing the discrimination ability of Liver Risk score. Overall mortality was assessed at the end of follow-up. in this ongoing study, 136 patients of our center with chronic hepatitis C treated with DAA were enrolled. In this population, 51% were men, the mean age was 65.3±12.2 years, 65.4% were genotype 1, 59.6% had liver cirrhosis, the mean liver stiffness measurement was 15.9 KPa (3.5-48.8), sustained virological response (SVR) was 95.5% and the mean follow-up was of 59 months. Coinfection with hepatitis B virus (HBV) was present in 8.8%. Discrimination ability of the LiverRisk score in the prediction of liver stiffness ≥10KPa was very good as shown by an AUROC of 0.848 (95% confidence interval [CI] = 0.767-0.930; p=0.000). During follow up 21 patients (15.4%) died. LiverRisk score was associated with the risk of all cause of mortality (Hazard Ratio = 1.154; 95% CI = 1.01–1.318; p = 0.035). the liver risk score is a good predictor of fibrosis and mortality in HCV patients treated with DAA. Abstract

There is a lack of evidence about the possible role of the family environment in the occurrence of the bullying phenomenon among school-age children. We carried out a systematic review of the literature to investigate the positive and negative family determinants associated with the bullying phenomenon. Potential classes of determinants-e.g. interparental conflict and/or parenting behaviours-were studied by searching peer-reviewed literature published between the January 2008 and December 2018 on PubMed/MEDLINE, SCOPUS and ISI Web of Science. At the end of the eligibility process, only 3 studies met all the inclusion criteria, and were then included in the narrative synthesis. Mother destructive profile and excessive infant crying resulted significantly associated with child emotional insecurity and conduct/mood problems at the age of 5-6 years, respectively. Moreover, a decrease of medically unexplained symptoms in adolescent was observed after a one-year family functioning therapy. This body of evidences should raise the interest of the researchers in order to invest in depth the family unit impact on the bullying phenomenon in school environment.

Abstract

Introduzione: Il 5-fluorouracile (5-FU) ed il suo profarmaco capecitabina, attivo per via orale, è usato nel trattamento di numerosi tumori solidi, tra cui carcinomi mammari, colo-rettali e della regione testa-collo. Nonostante la sua efficacia, a suo carico sono stati associati fenomeni di grave tossicità correlati all’enzima diidropirimidina deidrogenasi (DPD), responsabile del suo metabolismo,la cui carenza determina un profondo ritardo nell’eliminazione dell’agente chemioterapico.Pertanto la presenza di polimorfismi nella regione codificante del gene DPYD può determinare lo sviluppo di gravi tossicità di III-IV grado, come: nausea, vomito, diarrea, stomatiti, leucopenia, neutropenia, piastrinopenia, anemia e sindrome mano-piede.

Abstract

Results of a laboratory research programme on the rate of dissolution of selenitic gypsum are reported and discussed. The tested gypsum samples have been recovered from boreholes drilled in a thick layer outcropping on the left bank of Disueri reservoir located near Gela in Sicily. The tests carried out demonstrate that the rate of dissolution Rd depends essentially upon the velocity v of water flowing over the surface of the gypsum. Values of Rd range from 0.94 to 3 kg/(m2 • day) for v values from 0.05 to 0.20 m/s. Such high rates can account both for the rapid evolution of the existing cavities and for the creation of new caves within the gypsum mass subsequent to the impounding of the reservoir, and for the ineffectiveness of the grout curtains and of the waterproof facing of the outcropping front of the gypsum layer intended to prevent reservoir water from entering the gypum mass.

Abstract

L’applicazione della tipologia produttiva del fuori suolo nella viticoltura da tavola rappresenta oggi, a distanza di un decennio dall’inizio della sperimentazione condotta in Sicilia, una valida opportunità di diversificazione dei sistemi produttivi nel comparto della viticoltura da tavola. In Sicilia, tale tipologia si è sviluppata con la finalità di ottenere precocità di raccolta, di ampliare il calendario di offerta e di incrementare le produzioni per unità di superficie rispetto ai sistemi “tradizionali” in serra. Gli impianti di vite in fuori suolo realizzati in questi ultimi anni testimoniano un interesse sempre maggiore verso l’impiego di questa tipologia produttiva. L’attività di ricerca è oggi rivolta, inoltre, a verificare la possibilità di realizzare un ciclo produttivo in fuori suolo da luglio a novembre per ottimizzare le potenzialità offerte dall’ambiente “serra” e per offrire varietà a maturazione precoce, apprezzate dal consumatore, in epoca tardiva.

Aim: Root hydraulic conductivity is one of the main factors that control water flow in the soil-plant system and ultimately affect crop irrigation requirements. This work sets out to estimate root water conductivity for three self-rooted or grafted table grape cultivars.Methods and results: We evaluated root water conductivity of the cultivars ‘Black Magic’, ‘Matilde’ and ‘Victoria’ grafted onto 1103 Paulsen rootstock or self-rooted. Measurements were performed on two-year-old table grapes grown in pots filled with pumice. Root water conductivity was determined by placing the pots in a pressure chamber and increasing pressures from 0.05 to 0.30 MPa, at intervals of 0.05 MPa. Plant growth in terms of shoot and root dry matter was also evaluated, as well as leaf and root area.Conclusion: Root water conductivity differed according to cultivar and grafting. The ‘Victoria’ cultivar had higher root water conductivity than the other two, which differed little between them. The grafted plants showed higher root water conductivity than the self-rooted plants, except the plants of the cultivar ‘Matilde’, whose root water conductivity for the grafted and self-rooted plants was almost the same. It was also observed that the higher root water conductivity in relation to cultivar and grafting changed with increasing water flux and was constant for high water flux.Significance and impact of the study: Estimation of root hydraulic conductivity helps to determine the water consumption of the cultivars investigated, whether self-rooted or grafted, as well as the amount of irrigation water to apply to vineyards.

Abstract

Coronavirus disease 2019 (COVID-19) is a newly recognized infectious disease which can lead to acute respiratory distress syndrome requiring ventilatory support and intensive care unit admission. The aim of our study is to evaluate the performance of two non-invasive respiratory function indices (the ROX index and the SatO2/FiO2 ratio), as compared to the traditional PaO2/FiO2 ratio, in predicting a clinically relevant composite outcome (death or intubation) in hospitalized patients for COVID-19 pneumonia. Four hospital centers in Northern Italy conducted an observational retrospective cohort study during the first wave of COVID-19 pandemic. Four hundred and fifty-six patients with COVID-19 pneumonia admitted to medical or sub-intensive wards were enrolled. Clinical, laboratory, and respiratory parameters, for the calculation of different indices, were measured at hospital admission. In medical wards (Verona and Padua) the PaO2/FiO2 ratio, ROX index and SatO2/FiO2 ratio were able to predict intubation or death with good accuracy (AUROC for the PaO2/FiO2 ratio, ROX index and SatO2/FiO2 ratio of 75%, 75% and 74%, respectively). Regarding sub-intensive wards (Milan and Mantua), none of the three respiratory function indices was significantly associated with the composite outcome. In patients admitted to medical wards for COVID-19 pneumonia, the ROX index and the SatO2/FiO2 ratio demonstrated not only good performance in predicting intubation or death, but their accuracy was comparable to that of the PaO2/FiO2 ratio. In this setting, where repeated arterial blood gas tests are not always feasible, they could be considered a reliable alternative to the invasive PaO2/FiO2 ratio. Abstract

Background: Prevalence of mobile device addiction has increased over the years; both women and men have assimilated the mobile phone as a central component of their personal existence: integrating it into their lifestyle or becoming so dependent on it that life without it has become unimaginable. Smartphones generate radio-frequency electromagnetic fields. While short-term exposure in adults was considered quite safe, effects of long-term exposure or exposure during pregnancy on fetuses or during breastfeeding on newborns are not well studied yet. The objective of the present study was to investigate the prevalence and usage characteristics of smartphones among a sample of pregnant women, and promote the correct and conscious use of the smartphone. Methods: A cross-sectional study was conducted, with a questionnaire administered during childbirth classes and - after the questionnaire administration - an educational intervention focused on promoting the correct and conscious use of smartphones was carried out by psychologists and psychotherapists. Results: The findings of our study suggest that a significant number of the participants suffered addiction to mobile phone usage, but were not aware of it. More than two third of the sample (67.2%) have not changed their smartphone use habits since the beginning of their pregnancy and even more significant data shows that almost all future moms (98.3%) never speak with their doctor about smartphone use during pregnancy. Conclusions: Data collected suggest a lack of attention to the proposed topic, especially in relation to pregnancy. It seems necessary to sensitize future mothers on this topic. The promotion of a more conscious and controlled use of electronic devices can help reduce the radiation to which the unborn child may be exposed, but has a fundamental role even after birth, to ensure an adequate psychomotor and relational development of the child and do not affect, due to uncontrolled use of smartphones, the mother-child relationship.

Abstract

Background & Aims: Bacterial infections can trigger the development of organ failure(s) and acute-on-chronic liver failure (ACLF). Geographic variations in bacteriology and clinical practice could lead to worldwide differences in ACLF epidemiology, phenotypes and associated outcomes. Herein, we aimed to evaluate regional differences in bacterial infection-related ACLF in patients with cirrhosis admitted to hospital. Methods: This post hoc analysis included 1,175 patients with decompensated cirrhosis (with bacterial infection on admission or nosocomial infection) from 6 geographic regions worldwide. Clinical, laboratory and microbiological data were collected from the diagnosis of infection. Patients were followed-up for organ failure(s) and ACLF development according to the EASL-CLIF criteria from enrolment to discharge/death. Results: A total of 333 patients (28%) had ACLF at diagnosis of infection, while 230 patients developed ACLF after diagnosis of infection, resulting in an overall rate of bacterial infection related-ACLF of 48%, with rates differing amongst different geographic regions (38% in Southern Europe vs. 75% in the Indian subcontinent). Abstract

Introduction: In the present paper, the authors have discussed anti-aging strategies which aim to slow the aging process and to delay the onset of age-related diseases, focusing on nutrient sensing pathways (NSPs) as therapeutic targets. Indeed, several studies have already demonstrated that both in animal models and humans, dietary interventions might have a positive impact on the aging process through the modulation of these pathways. Areas covered: Achieving healthy aging is the main challenge of the twenty-first century because lifespan is increasing, but not in tandem with good health. The authors have illustrated different approaches that can act on NSPs, modulating the rate of the aging process. Expert opinion: Humanity’s lasting dream is to reverse or, at least, postpone aging. In recent years, increasing attention has been devoted to anti-aging therapies. The subject is very popular among the general public, whose imagination runs wild with all the possible tools to delay aging and to gain immortality. Some approaches discussed in the present review should be able to substantially slow down the aging process, extending our productive, youthful lives, without frailty.

Le scuole italiane svolgono un ruolo fondamentale nel compito di includere i più giovani tra i migranti nella nostra società. Il contributo vuole presentare la complessità del fenomeno dal punto di vista quantitativo, della loro distribuzione, della provenienza, della differenza di genere, dell'anzianità di arrivo e cerca di presentare alcuni approcci al fenomeno da parte degli insegnanti.

Abstract

In Sicilia, a fronte di un incremento dell’estensione dello screening organizzato del cervicocarcinoma, non corrisponde un aumento dell’adesione. Nei mesi di gennaio-giugno 2016 è stata condotta un’indagine trasversale somministrando un questionario standardizzato a un campione di 365 donne, per descrivere le ragioni del fenomeno e promuovere interventi mirati. Per le variabili di atteggiamento e conoscenza è stato utilizzato l’Health Belief Model, che ha esplorato la percezione della donna della suscettibilità (rischio di sviluppare il cancro), della gravità della patologia e dei benefici derivanti dal pap-test. Dai risultati emerge che il 66,6% delle intervistate ha effettuato un pap- test nell’ultimo triennio (valore nazionale 79,2%); il consiglio da parte di un operatore sanitario e la suscettibilità percepita sono associati a una maggiore adesione allo screening. Il potenziamento dei programmi organizzati di screening e l’intervento attivo del medico di medicina generale rappresentano strategie efficaci di promozione dello screening.

Abstract

Summary T helper 9 (Th9) cells and interleukin (IL)-9 are involved in the pathogenesis of several autoimmune diseases. The exact role of IL-9 and Th9 cells in patients with systemic sclerosis (SSc) have not yet been studied adequately. IL-9, IL-9R, transcription factor PU.1 (PU.1), IL-4, thymic stromal lymphopoietin (TSLP) and transforming growth factor (TGF)-β expression were assessed in skin and kidney biopsies of SSc patients and healthy controls (HC) by immunohistochemistry (IHC). The cellular source of IL-9 was also analysed by confocal microscopy analysis. Peripheral IL-9-producing cells were also studied by flow cytometry. The functional relevance of IL-9 increased expression in SSc was also investigated. Our results demonstrated a strong expression of IL-9, IL-9R, IL-4, TSLP and TGF-β in skin tissues of patients with both limited and diffuse SSc. IL-9 expression was observed mainly in the context of skin infiltrating mononuclear cells and keratinizing squamous epithelium. IL-9 over-expression was also observed in renal biopsies of patients with SSc. IL-9 producing cells in the skin were identified as Th9 cells. Similarly, Th9 cells were expanded and were the major source of IL-9 among SSc peripheral blood mononuclear cells (PBMC), their percentage being correlated directly with the modified Rodnan skin score. Infiltrating mononuclear cells, mast cells and neutrophils expressed IL-9R. In in-vitro studies stimulation with rIL-9 significantly induced NET (neutrophil extracellular traps) release by dying cells (NETosis) in neutrophils, expansion of mast cells and increase of anti-systemic scleroderma 70 (Scl70) production by B cells. Our findings suggest that Th9 cells and IL-9 could be implicated in the pathogenesis of SSc.

Abstract

Background: The aim of this study was to analyse the nutraceutical properties of table green olives Nocellara del Belice, a traditional Mediterranean food. The Mediterranean Diet has as key elements olives and extra virgin olive oil, common to all Mediterranean countries. Olive oil is the main source of fat and can modulate oxidative stress and inflammation, whereas little is known about the role of olives. Moreover, emerging evidences underline the association between gut microbiota and food as the basis of many phenomena that affect health and delay or avoid the onset of some age-related chronic diseases. Methods: In order to show if table green olives have nutraceutical properties and/or probiotic effect, we performed a nutritional intervention, administering to 25 healthy subjects (mean age 38,3), 12 table green olives/day for 30 days. We carried out anthropometric, biochemical, oxidative stress and cytokines analyses at the beginning of the study and at the end. Moreover, we also collected fecal samples to investigate about the possible variation of concentration of Lactobacilli, after the olives consumption. Result: Our results showed a significant variation of one molecule related to oxidative stress, malondialdehyde, confirming that Nocellara del Belice green olives could have an anti-oxidant effect. In addition, the level of interleukin-6 decreased significantly, demonstrating how this food could be able to modulate the inflammatory response. Moreover, it is noteworthy the reduction of fat mass with an increase of muscle mass, suggesting a possible effect on long time assumption of table olives on body mass variation. No statistically significant differences were observed in the amount of Lactobacilli, although a trend towards an increased concentration of them at the end of the intervention could be related to the nutraceutical effects of olives. Conclusion: These preliminary results suggest a possible nutraceutical effect of daily consumption of green table olives Nocellara del Belice. To best of our knowledge, this is the first study performed to assess nutraceutical properties of this food. Of course, it is necessary to verify the data in a larger sample of individuals to confirm their role as nutraceuticals.

Abstract

Ageing is characterized by a decline in all systemic functions. A greater susceptibility to apoptosis and senescence may contribute to proliferative and functional impairment of endothelial progenitor cells. They play an important role in neo-angiogenesis and endothelial repair. Vascular ageing is associated with changes in the structure and functions of vessels' wall. There are many possible causes of this damage. For sure, inflammation and oxidative stress play a fundamental role in the pathogenesis of endothelial dysfunction, commonly attributed to a reduced availability of nitric oxide. Inflammageing, the chronic low-grade inflammation that characterizes elderly people, aggravates vascular pathology and provokes atherosclerosis, the major cardiovascular disease. Nutraceutical and molecular biology represent new insights in this field. In fact, the first could represent a possible treatment in the prevention or delay of vascular ageing; the second could offer new possible targets for potential therapeutic interventions. In this review, we pay attention on the causes of vascular ageing and on the effects of nutraceuticals on it.
