- Abstract
Introduction: This study investigated attachment networks in a sample of Italian young adults. Attachment networks were defined in terms of attachment functions, attachment strength, the presence of a primary figure, and full-blown attachments. Method: Participants were 405 young adults, and we studied the effects of the demographic variables of gender, romantic status (whether single, involved in a romantic relationship for less or more than 24 months) and employment (whether university students or workers) on the structure of attachment networks. Participants were asked to answer the WHO-TO questionnaire, and derived indexes were analyzed using mixed ANOVAs, linear and logistic regression techniques. Results: Results indicated that while friends still had great importance in the network, partners were acquiring increasing relevance; at the same time, parents, and particularly mothers, remained central figures, particularly for the secure base function. Regarding the demographic variables, we observed that women reported stronger bonds with their mothers than men did, while the importance of friends was higher for men than for women. Additionally, our study supports previous findings underlining the importance of romantic partners in this phase of life, with participants involved in romantic relationships for longer than 24 months showing a fully developed attachment bond with their partners. Finally, for workers, the transfer of functions from the family-of-origin to external figures seemed to be fostered. Discussion: In conclusion, young Italian young adults go through a phase of intensive restructuring of attachment bond networks, particularly in relation to the consolidation of romantic relationships and work commitments.

4. A Smartphone-app intervention for Stress Reduction and Well-being Enhancement

- Abstract
Work-related stress is one of the biggest health hazards in the workplace. In the last years increasing attention has been paid to workplace safety and mental health issues, particularly in identifying new approaches to improve them with the help of m-health solutions. The following study aims to test the efficacy of a smartphone-based intervention, based on the principles of mindfulness and positive psychology, in reducing perceived stress and psychosomatic malaise and in increasing the resources of Psychological Capital, life satisfaction and mindfulness, in a sample of university students. Employing a controlled randomized trial, 98 students were randomly assigned to an intervention group (n=46), who used the app, and a control (n=52) group. Results showed that the smartphone-based intervention has significantly reduced perceived stress and psychosomatic malaise and increased the level of the Psychological Capital related to hope, resilience, and optimism.

- Abstract
Motor regions atrophy has been traditionally assumed to account for selective deficits of verb (V) vs. noun (N) processing in amyotrophic lateral sclerosis (ALS) patients via action semantic impairment (Embodied Cognition Theory, ECT). Nonetheless, accounts pertaining to both motor region sensitivity to the morpho-phonological structure of Vs and executive functioning (EF) contribution (task difficulty) have not been evenly endorsed. This study aimed at assessing neurocognitive underpinnings of N and V processing in ALS patients. ALS patients were compared to healthy participants (HPs) on tasks evaluating N and V semantic and post-semantic processing. Effects of semantic motor content (actionality) of Ns and Vs and of argument structure complexity of Vs were assessed. EF measures were put into relation to language tasks in patients. Both groups performed worse in V than in N lexical retrieval. Patients had difficulties in retrieving low-actionality Vs, being also sensitive to both transitive Vs and those with 2 and 3 arguments. Patients differed form HPs on both object- and action-semantic measures. EF measures were highly related to patients’ V-retrieval performance. ECT-framed explanations for N > V discrepancies in ALS patients are inconsistent with the present results. Indeed, ALS patients showed semantic deficits that are not limited to the action domain and retrieved more easily high-actionality Vs. N > V discrepancies in ALS patients would thus reflect a magnification of a differential processing demand for Vs and Ns, which is likely intrinsic to the neurocognitive system. Nonetheless, ALS patients being sensitive to argument structure complexity of Vs might imply motor regions involvement in V post-semantic processing, possibly at the lemma level.

Background: Delirium is thought to be common across various settings of care; however, still little research has been conducted in rehabilitation. Aim: We investigated the prevalence of delirium, its features and motor subtypes in older patients admitted to rehabilitation facilities during the three editions of the “Delirium Day project”. Methods: We conducted a cross-sectional study in which 1237 older patients (age ≥ 65 years old) admitted to 50 Italian rehabilitation wards during the three editions of the “Delirium Day project” (2015 to 2017) were included. Delirium was evaluated through the 4AT and its motor subtype with the Delirium Motor Subtype Scale. Results: Delirium was detected in 226 patients (18%), and the most recurrent motor subtype was mixed (37%), followed by hypoactive (26%), hyperactive (21%) and non-motor one (16%). In a multivariate Poisson regression model with robust variance, factors associated with delirium were: disability in basic (PR 1.48, 95%CI: 1.17–1.9, p value 0.001) and instrumental activities of daily living (PR 1.58, 95%CI: 1.08–2.32, p value 0.018), dementia (PR 2.10, 95%CI: 1.62–2.73, p value < 0.0001), typical antipsychotics (PR 1.47, 95%CI: 1.10–1.95, p value 0.008), antidepressants other than selective serotonin reuptake inhibitors (PR 1.3, 95%CI: 1.02–1.66, p value 0.035), and physical restraints (PR 2.37, 95%CI: 1.68–3.36, p value < 0.0001). Conclusion: This multicenter study reports that 2 out 10 patients admitted to rehabilitations had delirium on the index day. Mixed delirium was the most prevalent subtype. Delirium was associated with unmodifiable (dementia, disability) and modifiable (physical restraints, medications) factors. Identification of these factors should prompt specific interventions aimed to prevent or mitigate delirium.

- Abstract
La pandemia di Covid-19 ha creato le condizioni per un “maelstrom” di problemi, clinici ed etici, sia per quanto riguarda le scelte, la cura e l’assistenza erogate dagli operatori sanitari ai pazienti, sia, in particolare, per il rispetto dei diritti dei pazienti alla fine della vita. Il Comitato per l’Etica di Fine Vita ha effettuato uno studio per valutare quanto fossero stati rispettati i diritti dei morenti durante le fasi più acute della pandemia di Covid-19 (da marzo a dicembre 2020). Attraverso le opinioni degli operatori, i dati raccolti evidenziano criticità in merito alla dimensione umanistica e più specificamente etica della cura, senza la quale si rischia di compromettere la dignità del morire delle persone assistite e si rischia di avere setting di cura più stressanti e critici per gli operatori sanitari.

- Abstract
Pathological narcissism implies a fragile self-view. The psychological effects of ego-relevant events in people high in pathological narcissism, however, are still uncertain. The study examined the effects of pathological narcissism on psychological reactions to ego-relevant events occurring in private or public settings. Participants (N = 410) completed measures of pathological narcissistic traits, and then they took part in a scenario-based experimental session. They were randomly assigned to four conditions: ego-threatening vs. ego-fostering events in public vs. private settings. Self-esteem and affective states before and after the experimental manipulation were measured. Results showed that vulnerable and grandiose manifestations of pathological narcissism affect differently psychological reactions to ego-relevant events. Vulnerable narcissism made people particularly sensitive to ego-threatening and ego-fostering events, especially when occurring in public settings. Grandiose narcissism was linked to a reduction in emotional responses to ego-relevant events. Findings suggest that self- and affective reactions to ego-relevant events depend on narcissistic prevailing manifestations, and that public exposure has a key role in vulnerable narcissism.

- Abstract
Background: Delirium is thought to be common across various settings of care; however, still little research has been conducted in rehabilitation. Aim: We investigated the prevalence of delirium, its features and motor subtypes in older patients admitted to rehabilitation facilities during the three editions of the “Delirium Day project”. Methods: We conducted a cross-sectional study in which 1237 older patients (age ≥ 65 years old) admitted to 50 Italian rehabilitation wards during the three editions of the “Delirium Day project” (2015 to 2017) were included. Delirium was evaluated through the 4AT and its motor subtype with the Delirium Motor Subtype Scale. Results: Delirium was detected in 226 patients (18%), and the most recurrent motor subtype was mixed (37%), followed by hypoactive (26%), hyperactive (21%) and non-motor one (16%). In a multivariate Poisson regression model with robust variance, factors associated with delirium were: disability in basic (PR 1.48, 95%CI: 1.17–1.9, p value 0.001) and instrumental activities of daily living (PR 1.58, 95%CI: 1.08–2.32, p value 0.018), dementia (PR 2.10, 95%CI: 1.62–2.73, p value < 0.0001), typical antipsychotics (PR 1.47, 95%CI: 1.10–1.95, p value 0.008), antidepressants other than selective serotonin reuptake inhibitors (PR 1.3, 95%CI: 1.02–1.66, p value 0.035), and physical restraints (PR 2.37, 95%CI: 1.68–3.36, p value < 0.0001). Conclusion: This multicenter study reports that 2 out 10 patients admitted to rehabilitations had delirium on the index day. Mixed delirium was the most prevalent subtype. Delirium was associated with unmodifiable (dementia, disability) and modifiable (physical restraints, medications) factors. Identification of these factors should prompt specific interventions aimed to prevent or mitigate delirium.

Although researchers have valued dichotomization because it can simplify conducting and presenting analyses, statisticians have criticized the general use of this procedure because it reduces power and can lead to invalid results. Despite this, researchers continue to regularly dichotomize continuous measures, as evidenced by the presence of median splits and other forms of artificial categorization in the published literature. This suggests that researchers do not find the currently presented arguments against dichotomization compelling enough to avoid using the procedure. In response, this article provides a new, more pragmatic examination of the effects of dichotomization focused on how this practice can impede the pursuit of researchers’ professional and personal goals. Specifically, we present an examination of the effects of dichotomization phrased in terms of the sensitivity, specificity, positive predictive value, and negative predictive value of hypothesis tests, showing that dichotomization makes it more likely researchers will publish false results and reduces their ability to detect true findings in their data.

- Abstract
Financial well-being is a desirable state as it benefits individuals, families, organizations, and society, and these benefits reach beyond the financial domain. We assessed financial well-being as two components (current financial stress and expected future financial security) and used data from a representative sample of adults in the United Kingdom (n = 411). Our study provides novel insights based on preregistered hypotheses, method, and analysis plan on the Open Science Framework. We hypothesized that both executive functioning and financial self-efficacy are positively related to financial well-being via positive financial behaviors. We also hypothesized that executive functioning moderated the indirect relation of financial self-efficacy with financial well-being, and that financial self-efficacy moderated the indirect relation of executive functioning with financial well-being. As predicted, results showed that financial self-efficacy was strongly positively related to financial well-being via positive financial behaviors. Our results did not show that executive functioning was related to financial well-being via positive financial behaviors, nor that executive functioning or financial self-efficacy operated as moderators. This study provides possible strategies for financial practitioners and service providers, among others, to help individuals and families better their financial behaviors and their financial well-being.

- Abstract
A relevant portion of patients with disease caused by the severe acute respiratory syndrome coronavirus 2 (COVID-19) experience negative outcome, and several laboratory tests have been proposed to predict disease severity. Among others, dramatic changes in peripheral blood cells have been described. We developed and validated a laboratory score solely based on blood cell parameters to predict survival in hospitalized COVID-19 patients. We retrospectively analyzed 1,619 blood cell count from 226 consecutively hospitalized COVID-19 patients to select parameters for inclusion in a laboratory score predicting severity of disease and survival. The score was derived from lymphocyte- and granulocyte-associated parameters and validated on a separate cohort of 140 consecutive COVID-19 patients. Using ROC curve analysis, a best cutoff for score of 30.6 was derived, which was associated to an overall 82.0% sensitivity (95% CI: 78–84) and 82.5% specificity (95% CI: 80–84) for detecting outcome. The scoring trend effectively separated survivor and non-survivor groups, starting 2 weeks before the end of the hospitalization period. Patients’ score time points were also classified into mild, moderate, severe, and critical according to the symptomatic oxygen therapy administered. Fluctuations of the score should be recorded to highlight a favorable or unfortunate trend of the disease. The predictive score was found to reflect and anticipate the disease gravity, defined by the type of the oxygen support used, giving a proof of its clinical relevance. It offers a fast and reliable tool for supporting clinical decisions and, most important, triage in terms of not only prioritization but also allocation of limited medical resources, especially in the period when therapies are still symptomatic and many are under development. In fact, a prolonged and progressive increase of the score can suggest impaired chances of survival and/or an urgent need for intensive care unit admission.

- Abstract
Background: Patients suffering from stroke in the acute/post-acute phases often present with depressive mood — which negatively impacts on patients’ prognosis. However, psychometric evaluation of mood in acute stroke patients may be challenging due to cognitive deficits. Tools investigating emotional states via a vertical analogue line may overcome language/visuo-spatial disorders. This study thus aimed at (a) investigating the clinical usability of a Visual Analogue Mood Scale (VAMS) in acute stroke patients and (b) investigating the interplay between mood and cognition in this population. Methods: Forty-one acute stroke patients were compared to 41 age-, education- and sex-matched healthy participants (HPs) on the VAMS and on cognitive measures (mental performance in acute stroke, MEPS). A control line bisection (LB) task was administered to control for potential visuo-spatial deficits in patients. Results: Patients reported higher depression levels than HPs (lower VAMS scores); this between-group difference stayed significant when covarying for LB scores. MEPS scores discriminated patients from HPs; among cognitive measures, only the Clock drawing test (CDT) was positively associated with VAMS scores. Lesion side did not affect patients’ mood state; however, disease duration was inversely related to VAMS scores. Discussion: The VAMS proved to be a suitable tool for assessing mood in acute stroke patients, as being independent from post-stroke cognitive sequelae. The CDT might represent an adequate measure of depression-induced, post-stroke cognitive efficiency decrease. Mood disorders might occur and thus should be adequately addressed also in post-acute phases — likely due to longer hospitalization times and regression of anosognosic features.

- Abstract
Background: Working memory (WM) abilities are frequently impaired in neurological disorders affecting fronto-parietal cortical/sub-cortical structures. WM deficits negatively influence interventional outcomes and everyday functioning. This study thus aimed at the following: (a) developing and standardizing an ecologically valid task for WM assessment (Ice Cream Test, ICT); (b) validating and norming a novel WM test (Digit Ordering Test, DOT), as well as providing updated norms for digit span (DS) tasks, in an Italian population sample; (c) introducing a novel scoring procedure for measuring WM. Methods: One-hundred and sixty-eight Italian healthy participants—73 male, 95 females; age: 48.4 ± 19.1 (18–86); education: 12.1 ± 4.8 (4–21)—underwent a thorough WM assessment—DOT, ICT, and both forward and backward DS tasks (FDS, BDS). The ICT requires participants to act as waiters who have to keep track of customers’ orders. For each task, WM and total (T) outcomes were computed, i.e., the number of elements in the longest sequence and that of recalled sequences, respectively. Norms were derived via the equivalent score (ES) method. Results: DS ratios (DSRs) were computed for both WM/S and T outcomes on raw DS measures (BDS divided by FDS). Age and education significantly predicted all WM tasks; sex affected FDS and DSR-T scores (males > females). WM measures were highly internally related. Discussion: The present work provides Italian practitioners with a normatively updated, multi-component, adaptive battery for WM assessment (WoMAB) as well as with novel outcomes which capture different WM facets—WM capacity and attentive monitoring abilities.

Nielsen et al. (1) argue that Van Doesum et al. (2) need to consider three points for their interpretation of a positive association between individual-level social mindfulness (SoMi) and environmental performance (EPI) at the country level (3). The association is weaker when 1) it is controlled for GDP and 2) when the data of three countries are removed; also, 3) the data do not address the association between SoMi and individual-level environmental concern. We discuss these points in turn.

Although researchers have valued dichotomization because it can simplify conducting and presenting analyses, statisticians have criticized the general use of this procedure because it reduces power and can lead to invalid results. Despite this, researchers continue to regularly dichotomize continuous measures, as evidenced by the presence of median splits and other forms of artificial categorization in the published literature. This suggests that researchers do not find the currently presented arguments against dichotomization compelling enough to avoid using the procedure. In response, this article provides a new, more pragmatic examination of the effects of dichotomization focused on how this practice can impede the pursuit of researchers’ professional and personal goals. Specifically, we present an examination of the effects of dichotomization phrased in terms of the sensitivity, specificity, positive predictive value, and negative predictive value of hypothesis tests, showing that dichotomization makes it more likely researchers will publish false results and reduces their ability to detect true findings in their data.

- Abstract
Humans are social animals, but not everyone will be mindful of others to the same extent. Individual differences have been found, but would social mindfulness also be shaped by one's location in the world? Expecting cross-national differences to exist, we examined if and how social mindfulness differs across countries. At little to no material cost, social mindfulness typically entails small acts of attention or kindness. Even though fairly common, such low-cost cooperation has received little empirical attention. Measuring social mindfulness across 31 samples from industrialized countries and regions (n = 8,354), we found considerable variation. Among selected country-level variables, greater social mindfulness was most strongly associated with countries' better general performance on environmental protection. Together, our findings contribute to the literature on prosociality by targeting the kind of everyday cooperation that is more focused on communicating benevolence than on providing material benefits.

The Rivermead assessment of somatosensory performance (RASP) provides a quantitative assessment of somatosensory processing, suitable for brain-damaged patients suffering from stroke. It consists of seven subcomponents: Subtest 1 (sharp/dull discrimination), Subtest 2 (surface pressure touch), Subtest 3 (surface localization), Subtest 4 (sensory extinction), Subtest 5 (2-point discrimination), Subtest 6 (temperature discrimination), and Subtest 7 (proprioception). Overall, the RASP assesses 5 bilateral body regions: face (cheek), hand (palm and back), and foot (sole and back). This study aimed at providing normative data and cut-off scores for RASP subtests, for each body region, in a large Italian population sample. We present results from 300 healthy Italian individuals aged 19 to 98 years. Data represent a comprehensive set of norms that cover each subtest and each body region tested. Performance in Subtests 1, 5, and 6 decreased, for some body regions, with increasing age. Based on these results, norms were stratified for age (seven groups), with the pathological/non-pathological cut-off coinciding with the 5th percentile. Conversely, other results were not influenced by age; in such cases, a single error, in each body region, has to be considered indicative of pathological performance. This independent investigation of all subcomponents of the somatosensory system, for each body region, further confirms RASP’s potential in clinical practice, for neurological assessment, as well as in research settings.

- Abstract
Several studies have used text analysis software such as Linguistic Inquiry and Word Count (LIWC) to study how language modification reflects improvements in individuals’ psychophysical health. The aim of this preliminary single-case study was to evaluate the linguistic changes occurring during EMDR sessions in a patient suffering from anorexia nervosa. Each weekly session (N = 73) of an entire one-and-a-half-year treatment was divided into three different stages (pre-stimulation, stimulation and post-stimulation phase): the patient's speech was analysed at each phase. Results showed significant changes in the examined linguistic categories: affective processes, cognitive processes, biological processes and non-fluency words. These changes were observed before, during and after the bilateral stimulations. These results are discussed in the light of previous studies concerning the linguistic changes occurring in traumatic event disclosure. They suggest access and reprocessing of painful events during each session, especially when the clinician uses bilateral stimulation. It is possible to suppose the patient's progressive processing of the traumatic experience, followed by its integration into more adaptive mnemonic networks at the end of the therapeutic session. In conclusion, the observed linguistic changes could be representative of an adaptive resolution of adverse life experiences, allowing the patient to create new connections between thoughts, images and bodily sensations linked to the target memories. The clinical implications and limits of this single-case study are also discussed.

Background: Family caregivers of patients with dementia are at high risk of stress and burden, and quarantine due to the coronavirus disease 2019 (COVID-19) pandemic may have increased the risk of psychological disturbances in this population. The current study was carried out during the national lockdown declared in March 2020 by the Italian government as a containment measure of the first wave of the coronavirus pandemic and is the first nationwide survey on the impact of COVID-19 lockdown on the mental health of dementia informal caregivers.Methods: Eighty-seven dementia centers evenly distributed on the Italian territory enrolled 4,710 caregiver–patient pairs. Caregivers underwent a telephone interview assessing classical symptoms of caregiver stress and concern for the consequences of COVID-19 infection on patient’s health. We calculated prevalence of symptoms and regressed them on various potential stress risk factors: caregivers’ sociodemographic characteristics and lifestyle, patients’ clinical features, and lockdown-related elements, like discontinuity in medical care.Results: Approximately 90% of caregivers reported at least one symptom of stress, and nearly 30% reported four or more symptoms. The most prevalent symptoms were concern for consequences of COVID-19 on patient’s health (75%) and anxiety (46%). The main risk factors for stress were identified as a conflicting relationship with the patient and discontinuity in assistance, but caregiver’s female sex, younger age, lower education, and cohabitation with the patient also had an impact. Availability of help from institutions or private individuals showed a protective effect against sense of abandonment but a detrimental effect on concern about the risk for the patient to contract COVID-19. The only protective factor was mild dementia severity, which was associated with a lower risk of feeling isolated and abandoned; type of dementia, on the other hand, did not affect stress risk.Conclusion: Our results demonstrate the large prevalence of stress in family caregivers of patients with dementia during the COVID-19 pandemic and have identified both caregivers and situations at a higher risk of stress, which should be taken into account in the planning of interventions in support of quarantined families and patients.
