Abstract

The Observatorio Astrof\'isico de Javalambre (OAJ{\dag}1) in Spain is a young astronomical facility, conceived and developed from the beginning as a fully automated observatory with the main goal of optimizing the processes in the scientific and general operation of the Observatory. The OAJ has been particularly conceived for carrying out large sky surveys with two unprecedented telescopes of unusually large fields of view (FoV): the JST/T250, a 2.55m telescope of 3deg field of view, and the JAST/T80, an 83cm telescope of 2deg field of view. The most immediate objective of the two telescopes for the next years is carrying out two unique photometric surveys of several thousands square degrees, J-PAS{\dag}2 and J-PLUS{\dag}3, each of them with a wide range of scientific applications, like e.g. large structure cosmology and Dark Energy, galaxy evolution, supernovae, Milky Way structure, exoplanets, among many others. To do that, JST and JAST are equipped with panoramic cameras under development within the J-PAS collaboration, JPCam and T80Cam respectively, which make use of large format (~ 10k x 10k) CCDs covering the entire focal plane. This paper describes in detail, from operations point of view, a comparison between the detailed cost of the global automation of the Observatory and the standard automation cost for astronomical facilities, in reference to the total investment and highlighting all benefits obtained from this approach and difficulties encountered. The paper also describes the engineering development of the overall facilities and infrastructures for the fully automated observatory and a global overview of current status, pinpointing lessons learned in order to boost observatory operations performance, achieving scientific targets, maintaining quality requirements, but also minimizing operation cost and human resources., Comment: Global Observatory Control System GOCS

2. Analysing the region of the rings and small satellites of Neptune

Abstract

The ring system and small satellites of Neptune were discovered during Voyager 2 flyby in 1989 (Smith et al.1989). In this work we analyse the diffusion maps which can give an overview of the system. As a result we found the width of unstable and stable regions close to each satellite. The innermost Galle ring, which is further from the satellites, is located in a stable region, while Lassel ring (W = 4000 km) has its inner border in a stable region depending on its eccentricity. The same happens to the Le Verrier and Adams rings , they are stable for small values of the eccentricity. They can survive to the close satellites perturbation only for values of e < 0.012. When the solar radiation force is taken into account the rings composed by 1{$\mu$}m sized particles have a lifetime of about $10^4$ years while larger particles (10{$\mu$}m in radius) can survive up to 10 5 years. The satellites Naiad, Thalassa and Despina can help replenish the lost particles of the Le Verrier, Arago and Lassel rings, while the ejecta produced by Galatea, Larissa and Proteus do not have enough velocity to escape from the satellite gravity.

Abstract

Fast radio bursts (FRBs) are extraordinary astrophysical phenomena characterized by short radio pulses that last only a few milliseconds, yet their power can surpass that of 500 million suns. To date, most detected FRBs originate from beyond our galaxy. However, if an FRB were to originate within the Milky Way, it could be detected using small antennas. In this paper, we propose a compact and ad-hoc antenna array designed for the efficient detection and localization of FRBs within the Milky Way. The antenna operates within the 1200–1800 MHz range and consists of three sub-arrays placed in an L-shape for source localization, occupying a total volume of 8 0 × 2 5 × 6 cm 3 . Each sub-array consists of 4 miniaturized, dual-polarized, half-space radiation antenna elements, forming a one-dimensional array that allows shaping the radiation pattern to match the form of the Milky Way without exhibiting grating lobes. A prototype was constructed and characterized to validate the design. The measured results exhibit good agreement with the simulations. In addition to having a custom elongated radiation pattern, the array has attractive merits, such as low reflections at the input ports, high radiation efficiency, and a distribution that inhibits the existence of phase ambiguities, thus facilitating source localization. 12. Dynamic effects in CdTe quantum-dot LEDs

Abstract

In this work the electrical and electroluminescence properties CdTe nanocrystal films were analysed. The structure consisted of a multilayer of CdTe nanocrystals deposited by the layer-by-layer technique, sandwiched between an ITO anode and an aluminium cathode. The first part of this work was dedicated to structural and process improvement. Earlier devices, produced through a layer-by-layer (LbL) manual procedure, had an average thickness of 30nm per nanocrystal monolayer, with a roughness near 30% of the overall thickness. Electrical tests showed current densities over 100 mA/cm^2, with a frequent occurrence of pinholes and short-circuits that caused erratic sample behaviour and device rupture. SEM and AFM microscopic analysis showed that the nanoparticles were aggregated in 15-20nm thick clusters bound by polymer. A high porosity and non- uniformity of the multilayer was observed, explaining the formation of short-circuits. Luminescence was obtained from a very small number of samples, and with a very short duration that did not make spectral analyses possible. A robotic arm was programmed to carry out the LbL deposition, in an attempt to reduce the inhomogeneities of the multilayer. A key factor was the introduction of a special routine to remove the samples from the solutions. The sample withdrawal was designed to be in the vertical at a rate of 1.18 mm/s. The idea was to use gravity and the surface tension of the aqueous solutions to remove all the excess liquid from the surface. Additionally, poly(ethylenimine) (PEI) was eliminated from the process to improve homogeneity. These modifications produced multilayers with a thickness of 3nm per layer, average roughness below 5nm and CdTe packing density of 27%. Electrical measurements showed a stabilisation of the current-voltage (I-V) characteristics. A significant improvement in luminescence occurrence frequency and intensity was also achieved, enabling first spectral analyses. Once a reliable manufacturing procedure was developed, the electrical characterisation commenced with the analyses of samples with a different number of layers, operated in air. A field dependency of the I-V curves was found. Optimal performance was obtained from 30-layer samples, and this number of layers was adopted for subsequent analyses. Best samples showed external quantum efficiencies of 0.51%, with a photometric response of 0.8 lm/W and peak brightness of 1.42 cd/m^2. However, current and electroluminescence (EL) degradation with voltage and operation time were found in the device. Single carrier devices revealed a barrier for electron injection higher than predicted by the band diagram of the structure. The presence of an aluminium oxide layer at the multilayer/cathode interface was postulated, and confirmed through experiments in nitrogen. It was proposed that the growth of this oxide layer is the cause of device degradation during operation in air. However, it was demonstrated that the presence of the oxide favoured radiative recombination prior to degradation, with device efficiencies nearly 10 times higher than in devices without the oxide film. This was justified through three effects: charge accumulation at both sides of the oxide, field concentration across the oxide barrier and a reduction in leakage current. Unequal behaviour of samples with different electrode materials revealed that charge injection was the limiting mechanism for current flow, with a current onset field in the range of 2-3x10^7 V/m. Fowler-Nordheim plots showed that field emission was responsible for hole and electron injection into the device. Also, Fowler-Nordheim plots provided evidence of the dynamic nature of the cathode oxidation process. Dielectric breakdown of the aluminium oxide barrier at a rupture field of 3x10^7 V/m was found as a possible triggering mechanism of oxide layer growth. It was found that a critical field around 4.5x10^7 V/m caused irreversible loss of photoluminescence (PL) of the nanocrystals. This loss was attributed to an avalanche effect within the multilayer. The operational range for the devices is then found to be between 2x10^7 V/m and 4.5x10^7 V/m.

Abstract

Interleukin 4 (IL-4) suppresses the interleukin 2 (IL-2) induced lymphokine-activated killer (LAK) cell development from human peripheral blood mononuclear cells (PBMC). Suppression is observed at high (1,000 U ml-1) as well as low (10 U ml-1) concentrations of IL-2. IL-4 needs to be present at the beginning of the IL-2 culture to exert the suppressive effect. IL-4 also inhibits the development of CD25 (Tac) antigen on the PBMC cultured in IL-2. Interleukin 1 (IL-1) can reverse the suppressive effect of IL-4 on LAK induction when added at the early phase of the IL-2 culture. IL-1 enhances IL-2 induced LAK development, which may partially explain the reversion of IL-4 inhibition by IL-1. IL-1 also reverses the inhibitory effect of IL-4 on the development of CD25 antigen expression, although IL-1 alone does not enhance the induction of CD25 expression in PBMC cultured by IL-2. Furthermore, IL-4 suppresses IL-2 induced IL-1 production in PBMC. Thus, suppression of CD25 may be a pathway for the suppression of LAK induction. The expression of CD56 is not directly associated with the expression of LAK activity. IL-4, IL-1 or combination of the two cytokines has no effect on IL-2 induced expression of CD56. These results indicate that IL-4 has an antagonistic effect and IL-1 has a synergistic effect on IL-2-induced LAK development.

Abstract

Chickenpox, an exanthematous disease caused by the varicella zoster virus, is a frequent and common entity in childhood. Its clinical course is generally benign and self-limited. Despite this, it can be associated with complications at different levels. Up to 6% of them, occur at musculoskeletal level, constituting rare but important group of complications in terms of severity. Objetive: The aim of this case report is to show a rare complication with an uncommon location of chickenpox in children. Case description: We present the case and evolution of a 4-year-old schoolboy who developed pyomyositis of the right pectoralis mayor muscle during a chickenpox infection., La varicela, enfermedad exantemática causada por el virus varicela zóster, es una entidad frecuente y común a lo largo de la infancia. Su curso clínico es generalmente benigno y autolimitado. Pese a ello, puede asociar complicaciones a distintos niveles. Hasta un 6% de las mismas se producen a nivel musculoesquelético, constituyendo un grupo de complicaciones relativamente poco frecuente, pero importante en cuanto a severidad(1,2). Objetivo: El objetivo de este caso clínico es mostrar una rara complicación de la varicela en niños, que cuenta además con una localización infrecuente. Descripción del caso: A continuación, presentamos el caso y la evolución de un escolar de 4 años que durante una infección por varicela presenta piomiositis del músculo pectoral mayor derecho.

Abstract

Acute myeloid leukemia (AML) is a complex disease, and its treatment needs to be adjusted to the risk, which is conferred by cytogenetics and molecular markers. Cytarabine is the main drug to treat AML, and it has been suggested that the genotype of cytarabine metabolizing enzymes may have a prognostic relevance in AML. Here we report the association between the 5 '-nucleotidase, cytosolic II (NT5C2) rs10883841, cytidine deaminase (CDA) rs2072671 and rs532545 genotypes and the clinical outcome of 477 intermediate-risk cytogenetic AML patients receiving cytarabine-based chemotherapy. Patients younger than 50 years old with the NT5C2 rs10883841 AA genotype had lower overall survival (OS) (p: .003; HR 2.16, 95% CI 1.29-3.61) and lower disease-free survival (DFS) (p: .002; HR 2.45, 95% CI 1.41-4.27), associated to a higher relapse incidence (p: .010; HR 2.23, 95% CI 1.21-4.12). Interestingly, subgroup analysis showed that the negative effect of the NT5C2 rs10883841 AA genotype was detected in all subgroups except in patients with nucleophosmin mutation without high ratio FLT-3 internal tandem duplication. CDA polymorphisms were associated with the complete remission rate after induction chemotherapy, without influencing OS. Further studies are warranted to determine whether this pharmacogenomic approach may be helpful to individualize AML treatment.

Abstract

La configuración del deporte actual y de nuestra sociedad hacen que no solo las mujeres se vean afectadas por las ideologías sexistas, también los hombres se ven bajo el influjo de estereotipos, roles y tabúes muy asentados en nuestra cultura patriarcal. Objetivos: Esta investigación pretende como objetivo principal conocer y analizar las experiencias de chicos que practican gimnasia rítmica, ya sea de manera competitiva o recreativa, y de sus familiares más cercanos. Como objetivos secundarios, se procurará realizar un seguimiento de la situación de estos chicos para contrastar los datos previos y realizar propuesta de mejora para la situación de este colectivo. Métodos: La selección de los participantes se llevó a cabo atendiendo a los criterios de inclusión/ exclusión, que fueron: 1) chicos que practiquen, ya sea a nivel competitivo o recreativo, la modalidad de gimnasia rítmica, y 2) pertenecientes a la Comunidad Autónoma de Andalucía. Se llevaron a cabo diez entrevistas semi- estructuradas para la recogida de los datos, tres a gimnastas y siete a familiares. Para el tratamiento de los datos, se utilizó la técnica de análisis del contenido. Las entrevistas fueron grabadas y transcritas con el consentimiento de los participantes. Resultados y discusión: Los gimnastas se ven muy influenciados por su entorno social más próximo para comenzar a practicar y desarrollar una adherencia a esta modalidad deportiva. También indican en sus respuestas que en los comienzos se sentían raros y observados. Y aunque para la mayoría de los gimnastas, las chicas y los chicos tengan estilos diferenciados, ambos géneros son merecedores de realizar gimnasia rítmica por igual. Por otro lado, los familiares cercanos (padres y madres) no se sorprenden cuando sus hijos les dicen que quieren practicar gimnasia rítmica, y no suelen presentar ningún problema, aunque padres y hermanos suelen mostrar mayor rechazo. También la mayoría de los padres y algunas madres piensan que la gimnasia rítmi, The configuration of the current sport and our society means that not only women are affected by sexist ideologies, but also men are under the influence of stereotypes, roles and taboos that are highly established in our patriarchal culture. Aim: The main objective of this research is to know and analyze the experiences of boys who practice Rhythmic Gymnastics, either competitively or recreationally, and of their closest relatives. As secondary objectives, an effort will be made to follow up the situation of these boys to contrast the previous data and make a proposal for improvement for the situation of this group. Methods: The selection of the participants was carried out according to the inclusion/ exclusion criteria that were: 1) Boys who practice, either at a competitive or recreational level, the Rhythmic Gymnastics modality, and 2) belonging to the Autonomous Community of Andalusia. Ten semi-structured interviews were conducted to collect data, three with gymnasts and six with relatives. For data treatment, the content analysis technique was used. The interviews were recorded and transcribed with the consent of the participants. Results & discussion: Gymnasts are greatly influenced by their closest social environment to start practicing and develop an adherence to this sport. They also indicate in their responses that at the beginning they felt strange and observed. And although for most gymnasts, girls and boys have different styles, both genres are worthy of performing Rhythmic Gymnastics alike. On the other hand, close relatives (fathers and mothers) are not surprised when their children tell them that they want to practice this modality, and they don’t usually present any problems, although parents and siblings tend to show greater rejection. Also, most fathers and some mothers think that Rhythmic Gymnastics is a markedly feminine sport, which society associates with a homosexual orientation. The situation of this group is very improvable, and although it, A configuração do esporte atual e da nossa sociedade significa que não apenas as mulheres são afetadas por ideologias sexistas, mas também os homens estão sob a influência de estereótipos, papéis e tabus muito estabelecidos em nossa cultura patriarcal. Objectivos: O principal objectivo desta pesquisa é conhecer e analisar as experiências de meninos que praticam ginástica rítmica, de forma competitiva ou recreativa, e de seus familiares mais próximos. Como objectivos secundários, será feito um esforço para monitorar a situação desses meninos para contrastar os dados anteriores e fazer uma proposta de melhoria para a situação desse grupo. Métodos: A seleção dos participantes foi realizada de acordo com os critérios de inclusão/ exclusão que foram: 1) meninos que praticam, em nível competitivo ou recreativo, a modalidade de ginástica rítmica; e 2) pertenecentes à Comunidade Autônoma da Andaluzia. Foram realizadas dez entrevistas semiestructuradas para coletar dos dados, três com ginastas e seis com familiares. Para o tratamento dos dados, foi utilizada a técnica de análise de conteúdo. As entrevistas foram gravadas e transcritas com o consentimento dos participantes. Resultados e discussão: As ginastas são grandemente influenciadas por seu ambiente social mais próximo para començar a praticar e desenvolver uma adesão a ese esporte. Eles também indicam em suas respostas que a principio se sentiram estranhos e observados. E, embora para a maioria das ginastas, meninas e meninos, tenham estilos diferentes, ambos os géneros são dignos de realizar ginástica rítmica. Por outro lado, parentes próximos (país e mães) não se surprendem quando os filos lhes dizem que desejam praticar ginástica rítmica e geralmente não apresentam problemas, embora país e irmãos tendam a mostrar maior rejeição. 38. Tratamiento quirurgico de fractura cervical en el hospital Regional Rancagua: analisis de un periodo de 4 anos

Introducción: La gravedad, las implicancias neurológicas y el alto costo del tratamiento de la fractura cervical, hacen de esta lesión un tema de suma importancia. Esta injuria provoca graves limitaciones e invalidez a los afectados, en su mayoría en plena actividad laboral, impactando en lo médico, social y económico. Objetivos: Describir los pacientes tratados quirúrgicamente de fractura cervical traumática en el Hospital Regional Rancagua, por el equipo de Neurocirugía dentro de un periodo de 4 años. Materiales y Métodos: Se evaluó-24 pacientes con antecedente de fractura cervical el mecanismo del trauma, el tiempo transcurrido desde el ingreso hospitalario hasta la cirugía, nivel y compromiso de la lesión medular, tipo de abordaje quirúrgico, complicaciones médicas, quirúrgicas y seguimiento post-operatorio, entre otros. Resultados: 5 mujeres y 19 hombres, edad promedio 39 años (rango de edad 14 a 75 años), principales mecanismos de trauma fueron los accidentes automovilísticos-atropello (58%). El nivel medular más frecuentemente lesionado fue C4-C5 (25%). En el 50% de los pacientes se clasificó de entrada como Frankel A, por lo que un 50% de todos los pacientes ingresaron tetrapléjicos, y de estos, un 33% egresó tetraparéticos. Del ingreso a-cirugía hubo un tiempo de espera promedio 5 días. Dentro de las complicaciones médicas la causa respiratoria (46%) fue la más frecuente, necesitando 64% de estos pacientes ventilación mecánica. Se registró 1 infección de herida operatoria donde el abordaje fue posterior. El Índice de Barthel promedio de 14 pacientes fue de 49 puntos. De 6 pacientes con puntaje Cero, 85% de estos ingresaron como Frankel A y 50 % egresaron tetrapléjicos. Los pacientes se rehabilitaron en promedio de 2.5 meses. Discusión: Logramos objetivar nuestra realidad. La técnica quirúrgica fue prácticamente uniforme entre los pacientes, sin complicaciones quirúrgicas post operatorias graves. Sin embargo, existen grandes falencias en la demora de la estabilización quirúrgica como también en rehabilitación neurológica y apoyo bio-psico-social del paciente al alta. Pareciera existir cierta relación entre el bajo puntaje obtenido por el Índice de Barthel y el mal pronostico neurológico, siendo necesario proponer a futuro un estudio prospectivo dirigido directamente a evaluar dicha eventual correlación. Interesa por último, estudiar el impacto GES ejercido desde el 2007 hasta la fecha, relativo a la resolutividad de esta patología. This injury causes severe limitations and disability to those affected, mostly in full working activity, impacting on the medical, social and economic. Objectives: To describe patients surgically treated for traumatic cervical fracture at the Rancagua Regional Hospital, by the team of Neurosurgery Department, within a period of 4 years. Materials and Methods: We evaluated 24 patients with an history of cervical fracture mechanism of trauma, the time from hospital admission to surgery, and commitment level of spinal cord injury, type of surgery, medical and surgical complications and follow-up post -operation, among others. Results: 5 women and 19 men, average age 39 years (ranging age from 14 to 75 years), major trauma mechanisms were motor vehicle accidents (58%). The most frequently injured spinal level was C4-C5 (25%). 50% of patients were classified as Frankel A at the admission, so that 50% of all patients admitted were quadriplegics, and of these, 33% were discharged as tetraparétic. Admission to the surgery were an average of 5 days. Within cause respiratory medical complications (46%) was the most frequent, requiring 64% of these patients mechanical ventilation. There was only 1 wound infection where the approach was posterior. The average Barthel Index of 14 patients was 49 points. Conclusions: We were able to objectify our reality. The surgical technique was nearly uniform among patients without severe postoperative surgical complications. However, there are major misstatments in the delayed surgical stabilization as well as neurological rehabilitation and bio-psycho-social discharged patient's support. Certain relation seemed to exist between the low rating obtained by Barthel's Index and the poor neurological outcome, being necessary to propose to future a market study directed directly to evaluating the above mentioned eventual correlation. Is interessting in studying the GES (Garantías Explicitas en SaludHealth explicit guaranties)-impact exerted from 2007 to date on the resoluteness of this disease. Abstract

International audience; BACKGROUND:Rituximab plus chemotherapy has been shown to be effective in patients with advanced-stage, previously untreated follicular lymphoma; nevertheless, most patients will have a relapse. Combination immunotherapy with lenalidomide and rituximab is an immunomodulatory regimen that has shown promising activity in patients with indolent B-cell non-Hodgkin's lymphoma.METHODS:We conducted this multicenter, international, phase 3 superiority trial to evaluate rituximab plus lenalidomide, as compared with rituximab plus chemotherapy, in patients with previously untreated follicular lymphoma. Patients were randomly assigned to receive one of the two regimens, followed by maintenance monotherapy with rituximab. Treatment with rituximab plus lenalidomide consisted of 18 cycles of the two drugs, followed by rituximab maintenance therapy every 8 weeks for 12 cycles (six additional doses). Treatment with rituximab plus chemotherapy consisted of the investigator's choice of one of three rituximab-based regimens, followed by maintenance monotherapy with rituximab every 8 weeks for 12 cycles. The primary end points were complete response (confirmed or unconfirmed) at 120 weeks and progression-free survival.RESULTS:A total of 1030 patients were randomly assigned to receive rituximab plus lenalidomide (513 patients) or rituximab plus chemotherapy (517 patients). The rate of confirmed or unconfirmed complete response at 120 weeks was similar in the two groups: 48% (95% confidence interval [CI], 44 to 53) in the rituximab-lenalidomide group and 53% (95% CI, 49 to 57) in the rituximab-chemotherapy group (P=0.13). The interim 3-year rate of progression-free survival was 77% (95% CI, 72 to 80) and 78% (95% CI, 74 to 82), respectively. A higher percentage of patients in the rituximab-chemotherapy group had grade 3 or 4 neutropenia (32% vs. 50%) and febrile neutropenia of any grade (2% vs. 7%), and a higher percentage of patients in the rituximab-lenalidomide group had grade 3 or 4 cutaneous reactions (7% vs. 1%).CONCLUSIONS:Among patients with previously untreated follicular lymphoma, efficacy results were similar with rituximab plus lenalidomide and rituximab plus chemotherapy (with both regimens followed by rituximab maintenance therapy). 47. UGT1A1 genotype influences clinical outcome in patients with intermediate-risk acute myeloid leukemia treated with cytarabine-based chemotherapy

Abstract

The treatment of acute myeloid leukemia (AML) is adjusted according to cytogenetic risk factors and molecular markers. Cytarabine remains the main drug to treat AML, and several studies have explored the prognostic relevance of the genotype of cytarabine metabolizing enzymes in AML. Glucuronidation has been identified to be relevant in the cytarabine clearance, but there are still few data concerning the clinical impact of genetic polymorphisms known to condition the activity of UDP-glucuronosyl transferases in AML patients. Here we report the association between the UGT1A1 rs8175347 genotype and the clinical outcome of 455 intermediate-risk cytogenetic AML patients receiving cytarabine-based chemotherapy. Patients with the UGT1A1*28 homozygous variant (associated to a lower UGT1A1 activity) had a lower overall survival (OS) (25.8% vs. 45.5%; p: 0.004). Multivariate analysis confirmed this association (p: 0.008; HR: 1.79; 95% CI: 1.16-2.76). Subgroup analysis showed the negative effect of the UGT1A1*28 homozygous genotype on OS in women (14.8% vs. 52.7%; p: 0.001) but not in men. This lower OS was associated with longer neutropenia after consolidation chemotherapy and with higher mortality without previous relapse, suggesting an association between a low glucuronidation activity and mortal toxic events.
