BACKGROUND: Male hypogonadism is a frequent complication of diabetes mellitus (DM) type 2; therefore, a study of its clinical and epidemiological characteristics is of interest. AIMS: Assessment of clinical and epidemiological characteristics of hypogonadism in men with diabetes mellitus type 2. METHODS: A full-design, cross-sectional, screening, multicenter, non-interventional study included men with diabetes mellitus type 2. The study was conducted from November 2017 to January 2019.Medical history assessment, sexological testing, estimation of luteinizing hormone (LH), sex hormone-binding globulin; total testosterone and glycated hemoglobin were performed. BACKGROUND:Patients with endogenous hypercortisolism have reduced bone mineral density (BMD) and trabecular bone score (TBS) that are the causes of secondary osteoporosis and low-traumatic fractures. It is well known that radical treatment (neurosurgery or radiosurgery) of Cushings disease leads to a decline of cortisol levels in all body fluids to normal values. However, it is still uncertain whether bone tissue structure, and particularly its microarchitecture, does recover in remission of the disease. AIMS:To evaluate an influence of hormone activity (presence or absence of remission) in patients with Cushing's disease on changes of bone structure measurements in accordance with DXA values (TBS, BMD, T- and Z-scores), as well as significance of such changes in 12 and 24 months after neurosurgical treatment. MATERIALS AND METHODS:In patients with confirmed active Cushing's disease (ACTH-producing pituitary adenoma) (n = 44) and in control group of healthy volunteers (n = 40), BMD in lumbar spine (L1-L4) and simultaneously TBS, in cut-off points before neurosurgical treatment (in both groups) and in 12 and 24 months after it (only in patients), were assessed. We diagnosed presence or absence of disease remission at cut-offs. All measurements were performed using a GE iDXA device (GE Healthcare Lunar, Madison, Wisconsin, USA). The TBS was calculated simultaneously from taken BMD scans, blinded to clinical outcome using TBS iNsight software v2.1 (Medimaps, Merignac, France). The activity of Cushings disease was evaluated using late-night salivary cortisol (LNSC, at 23:00). To determine the differences in DXA and TBS values before and after neurosurgical intervention depending on remission occurrence, covariate analysis (ANCOVA) was applied. RESULTS:There were found significant changes in TBS, BMD and T-score values in 12 months after neurosurgical treatment associated with presence or absence of disease remission (p = 0.039, 0.046 and 0.048, respectively). No differences in Z-score as well as in all measurements in 24 months, that might be associated with remission occurrence, were revealed. The gain in all DXA measurements (including TBS) during 24 months of observation period was statistically significant when analyzing data using Students paired t-test. However, the values corresponding to the age references had not been achieved for the specified time interval. CONCLUSIONS:Patients with Cushings disease have lower TBS values. In remission conditions TBS is getting significantly higher. The increase in BMD and TBS occurs during 24 months after achieving remission of Cushings disease but doesnt lead to a full restoration of normal bone mass and microstructure throughout observation period of 24 months.

Background: Bone tissue is a non-classical endocrine organ, which produces at least two hormones: fibroblast growth factor 23 (FGF-23) and decarboxylated osteocalcin (OC). In addition to this, recent studies demonstrate that specific proteins involved in the paracrine regulation of bone remodelling can be measured in peripheral serum samples and may serve as additional biomarkers of bone metabolism. Aims: to evaluate the serum levels of biomarkers related to endocrine and paracrine function of bone tissue in patients with Cushing’s disease (CD) and acromegaly. Materials and methods: The study was conducted according to the cross-sectional case-control type. Fasting serum samples were taken between 8–10 a.m. from patients with CD, acromegaly and age-, sex- and BMI-matched healthy volunteers and stored at -40° C. Commercially available kits for enzyme-linked immunosorbent assay (ELISA) were used to determine the serum levels of FGF-23, co-factor (co-receptor) Klotho, cathepsin K, sclerostin, P1NP. Insulin-like growth factor-1 (IGF-1) was measured by the immunochemiluminescence assay, late-night (11 p.m.) salivary cortisol (LNSC) was evaluated using the electrochemiluminescence method. Non-parametric tests (the Kruskal-Wallis test and the Mann-Whitney test) were used to assess the differences between the groups of patients. Results: The study includes 78 patients, (37.6 years old, 95% CI 34.75–40.46): 29 patients with CD (group 1), 22 – with acromegaly (group 2), and 27 healthy individuals (group 3), matched by sex, age and BMI (p = 0.432, 0.373 and 0.725 between groups, respectively). We do not recommend population screening for diabetes insipidus (DI) (B3). We recommend to perform diagnostic testing for central diabetes insipidus (CDI) in patients who underwent neurosurgery, after skull and brain trauma, subarchnoid hemorrhage (B3). We recommend excluding thirst impairment during all stages of diagnostic assessment (С3). We recommend excluding DI in cases of persistent hypotonic polyuria: excretion of more than 3 L. or more than 40 mL/kg of urine daily; urine osmolality less than 300 mOsm/kg or urinary specific gravity less than 1004 g/L in all urine samples or during Zimnitsky test (В3). After hypotonic polyuria is confirmed, we recommend excluding of the main causes of nephrogenic diabetes insipidus (NDI) (B3). We recommend simultaneous measurement of urine osmolality and blood osmolality/sodium level in order to confirm DI. Blood hyperosmolality (more than 300 mOsm/kg) and/or hypernatremia with low urine osmolality (less than 300 mOsm/kg) confirms DI (B2). If testing does not reveal these findings, we recommend performing a fluid deprivation test to exclude primary polydipsia (PP) (B2). Desmopressin test is recommended to distinguish CDI and NDI (B2). In cases of CDI we recommend to perform head MRI with contrast (B3). In cases of NDI we recommend assessing renal structure and function and possible electrolyte disturbances (C3). In cases of PP we recommend to refer a patient to psychiatrist (B3). We recommend treating CDI with synthetic vasopressin analogue – desmopressin (B1). We recommend an individual approach in choosing desmopressin dosage form (B2). As the initial dose is difficult to predict when starting desmopressin treatment, we recommend titrating the dosage using two approaches: “the average dose” and “as required” (C4). We recommend educating the patients to ensure knowledge of the features of various desmopressin dosage forms (C4). To decrease the risk of water intoxication, we recommend educating the patients to the water intake regimen adherence (С4). When CDI is accompanied by thirst impairment, we recommend titrating the dose in a clinical setting, with assessment of blood sodium, bodyweight and/or urine volume (C4).

The presented paper is a third revision of the clinical recommendations for the treatment of morbid obesity in adults. Morbid obesity is a condition with body mass index (BMI) ≥40 kg / m2 or a BMI ≥35 kg / m2 in the presence of serious complications associated with obesity. The recommendations provide data on the prevalence of obesity, its etiology and pathogenesis, as well as on associated complications. The necessary methods for laboratory and instrumental diagnosis of obesity are described in detail. In this revision of the recommendations, the staging of prescribing conservative and surgical methods for the treatment of obesity are determined. For the first time, a group of patients with obesity and type 2 diabetes mellitus is selected, in whom metabolic surgery allows a long-term improvement in the control of glycemia or remission of diabetes mellitus.

Aim. To estimate the trabecular bone score (TBS) for evaluation of fracture probability in order to make decisions about starting osteoporosis treatment in patients with type 2 diabetes mellitus (T2DM). Materials and methods. We obtained the bone mineral density (BMD) and trabecular bone score (TBS) using dual energy X-ray absorptiometry (iDXA) in patients with T2DM (with and without a history of osteoporotic fractures) versus the control group. Before and after TBS measurements we assessed the ten-year probability of fracture using the Fracture Risk Assessment Tool (FRAX). Results. We enrolled 48 patients with T2DM, including 17 with a history of low-traumatic fracture, 31 patients without fractures and 29 subjects of a control group. BMD was higher in patients with T2DM compared to the control group at L1–L4 (mean T-score 0.44, 95% CI -3.2 – 4.9 vs mean T-score 0.33, 95% CI -2.9 – 3.0 in a control group p=0.052) and Total Hip (mean T-score 0.51, 95% CI -2.1 – 3.0 vs mean T-score -0.03, 95% CI -1.4 – 1.2 in a control group p=0,025). The TBS and 10-year probability of fracture (FRAX) was not different in patients with T2DM versus the control group. However, when the TBS was entered as an additional risk factor, the 10-year probability of fracture became higher in patients with T2DM (10-year probability of fracture in T2DM- 8.68, 95% CI 0.3-25.0 versus 6.68, 95% CI 0.4–15.0 in control group, p=0.04). Among patients with diabetes with and without fractures the FRAX score was higher in subjects with fractures, but no difference was found in regards to BMD or TBS. Entering BMD and TBS values into the FRAX tool in subjects with diabetes and fractures decreased the FRAX score. However, patients with low-traumatic fractures should be treated for osteoporosis without a BMD, TBS or FRAX assessment. Conclusion. TBS improves the results of FRAX assessment in patients with T2DM and should be entered while evaluating FRAX in patients with T2DM. However, additional research is needed to develop a more sensitive tool to evaluate fracture risk in patients with T2DM.

Hypogonadism in men, defined as a reduction in serum testosterone in combination with characteristic symptoms and/or signs (described in detail later), is common in diabetes mellitus (DM). These recommendations do not cover the whole range of pathologies that cause the development of testosterone deficiency (hypogonadism), but focus on its clinical variants and characteristic for men with diabetes. The recommendations provide data on the prevalence of hypogonadism in diabetes, its etiology. In the section "diagnostics" the features of anamnesis of patients with hypogonadism with diabetes, the necessary methods of physical and laboratory examination are presented in detail. The risk factors and clinical consequences of hypogonadism are separately examined. In the section "choice of treatment methods", there are possible treatment options for such patients using various androgenic therapies, taking into account the needs of the man, maintaining his reproductive function and risk factors. Particular attention is paid to indications, contraindications and risk factors for androgen therapy in men with diabetes, especially in old age. With this in mind, principles for monitoring the treatment are developed. Based on a large number of studies, favorable effects of androgen replacement therapy in men with hypogonadism and diabetes have been demonstrated.

Science will never stop in its search for new solutions. The process of investigating and predicting by scientific theories is the process of identifying and managing patterns (sets of repetitive models, stable combinations and sequences). Over the years, the problem of iodine deficiency remains important for global health. Despite all the measures have been taken to eliminate this disease, now it is not possible to optimize iodine deficiency diagnosis and treatment all over the world. So, previously published in Clinical and experimental thyroidology journal article Evaluating of significance of thyroglobulin (Tg) level in blood as a biomarker of iodine deficiency disorders severity in Uzbekistan is incredibly relevant and undoubtedly requires detailed analysis. The analysis and comments to provided research is the subject of current article.

The aim of this study was to investigate the relationship between the dynamics of body weight and sleep disorders in the treatment of obesity.Materials and methods. The study included 200 obese patients: 83 men and 117 women.Results. Complaints about problems sleeping (snoring, hypersomnia, insomnia, etc.) were present in 78% of obese patients. 89 patients were under the observation of an endocrinologist for 7 ± 1 months, they were divided into three matched by age, sex and BMI groups: patients with obstructive sleep apnea syndrome (OSAS) (n = 42), the second – with the syndrome of insomnia (n = 19), the third (control group) – patients without sleep disorders (n = 24). After treatment of obesity the weight loss in patients with insomnia syndrome was -2.5 [-4; 0]kg, in patients with OSAS -7 [-18; -2] kg, in patients without sleep disorders -6.5 [-12; -2.25] kg. Clinically significant weight reduction was reached in 25 (59.5%) patients with OSAS; 3 (16%) – with insomnia syndrome; 15 (62.5%) – without sleep disorders.Conclusion. Thus, the insomnia syndrome essentially influences the obesity treatment results – most of patients with this sleep disorder (81.2%) do not achieve clinically significant weight loss. The presence and severity of breathing disorders during sleep do not prevent weight loss. However, with the regular use of CPAP-therapy in patients with OSAS has a tendency of greater reduction of body weight.

Russian guidelines for diagnostic of thyroid nodules gained some actual questions: necessity of ultrasound (US)-screening of the thyroid cancer, indications for fine needle aspiration and exam of calcitonin, necessity of unification of US andcytopathology classification for signs of thyroid nodules.

Introduction. The prevalence of obesity and associated comorbidities is comparable to the scale of the epidemic. Along with the change in lifestyle, pharmacotherapy is necessary to lead obesity patients to a healthier state.Aim. The evaluation of efficacy and safety of Reduxin® (sibutramine + microcrystalline cellulose) application among patients with alimentary obesity and comorbidities in routine clinical practice. Analysis of results of Primavera program.Materials and methods. The observation program Primavera was conducted since November 2012 to July 2015 under the auspices ofEndocrinology Research Centre and the Russian Association of Endocrinologists. The program "Primavera" was attended by 3095 doctors of various specialties and 98.774 patients being treated in 1272 hospitals in 142 Russian cities. The average age of the patients was 39.4 ± 10.4 years, the ratio of male / female – 17.7%/82,3%, the average body mass – 99.1±14.28 kg, the average BMI – 35.7±4.41 kg/m2, the average waist circumstance – 105.7±13.7 cm. The Duration of Reduxine® treatment was determined by the attending physician and ranged from 3 months to 1 year.Results. The duration of therapy was 3 months for 3% of patients, 6 months for 59.3% of patients, 12 months for 37.7% of patients. The BMI reducing dynamics during 3, 6, 12 months was 3.4±1.53 kg/m2 (average 9.5±4.28 kg), 5.4±2.22 kg/m2 (15.0±6.22 kg), 7.2±3.07 kg/m2 (20.0±8.62 kg) respectively. The average waist circumference reduction for 3, 6 and 12 months was 6.3±4.31 sm, 10.6±6.30 sm, 16.0±8.94 sm, respectively. It was shown that the weight loss during prolonged (more than six months) Reduxine therapy under the supervision of a physician was associated with decreased levels of systolic and diastolic blood pressure and had no affect on heart rate. Adverse events were reported for 4.1% of patients, according to doctors, the cause-effect relationship with the use of Reduxine can be estimated as probable or definite for 1.9% of patients, possible – for 1.7%.Conclusions. Observation programs allow to implement the principles of active monitoring of the efficacy and safety of the drug in the current clinical practice and to develop a skill of reasonable prescribing. In Primavera program it was shown that the use of Reduxine® (sibutramine+ microcrystalline cellulose) leads to loss of body weight and doesn’t lead to serious adverse effects.

The project of Russian clinical practice guidelines for diagnosis and treatment of differentiated thyroid cancer is dedicated to the management of patients with differentiated thyroid cancer. The guideline modifications 2016 include the following matters: indication for fine-needle aspiration biopsy, calcitonin screening, standards for biopsy results, indication for suppressive therapy and thyroid replacement therapy, targeted therapy in patients with radioiodine-refractory differentiated thyroid cancer.

This review will provide an overview of a novel phenomenon in medicine and endocrinology known as microchimerism (MC). MC is defined as the long-term presence of genetically distinct populations of cells in peripheral blood and tissues of individual. This condition may be caused by the transplacental bidirectional cell trafficking between mother and fetus during pregnancy. Other sources are the passage from a twin or a vanished twin and iatrogenic interventions such as organ transplantation and blood transfusion. Long-term consequences of MC and its influence on woman's health are under active investigation. Recent studies have shown a broad spectrum of its biological effects either beneficial or adverse. MC has been suggested to play a key role in the pathogenesis of autoimmune diseases. It is assumed that microchi-meric cells could initiate a "graft versus host" or "host versus graft" reactions. MC has been extensively studied in autoimmune thyroid diseases (AITD). MC could explain the higher prevalence of AITD in females and frequent manifestation during the postpartum period. It is proposed that microchimeric cells could reduce the risk of cancer development and be a part of tissue repair. MC has also been investigated in papillary thyroid cancer. Further studies of this phenomenon are mandatory to get more insights about its role in thyroid cancer and AITD development. This could provide novel therapeutic, preventive or prognostic perspectives regarding these diseases.

Adhesive capsulitis (АС) has an incidence of 20% in diabetic patients. The tightening of the shoulder capsule ("freezing") can cause gradually increasing limitation in active and passive range of motion (ROM) of shoulder. Consequences of the increasing limitations are reduced quality of life and patient disability. It is thought that AC is a self-limiting disorder that resolves in 1?2.5 years in most cases. However, new clinical data indicate both long-term persistent pain and residual loss of motion in 10% of patients without diabetes and in 85% of patients with diabetes. In this review, we summarize the results from different clinical trials in which risk factors and pathogenesis of AC in diabetic patients as well as the diagnosis and efficacy of various methods for the treatment of AC were examined.

Aim. Evaluation of the itopride (Ganaton?, Abbot) therapy efficacy in the management of gastrointestinal (GI) symptoms and gastric motor function (GMF) in type 1 diabetis mellitus (T1DM) patients (pts) in the presence of GMF dysfunction and other forms of diabetic autonomic neuropathy (DAN). Materials and Methods. The total of 34 patients with T1DM, GMF dysfunction and DAN were selected for randomized, prospective, open-label, comparative study. The duration of the study was 6 weeks. The study group (17 pts) received itopride 150 mg total daily. The control group (17 pts) did not receive any treatment for GMF. А questionnaire was used for the assessment of gastrointestinal (GI) symptoms. Gastric emptying velocity was evaluated with 13C-octanoate breath test. Results. As a result of itopride therapy there was a statistically significant decrease in the amount of time (T1/2) needed for the gastric emptying. Aims. The aim of this study was to estimate the prevalence rate of concomitant pathology and its influence on leading causes of death in patients withtype 2 diabetes mellitus according to clinical charts and pathologist's reports. Materials and methods. We have studied the database of Moscow City Hospital №50 in order to pick out confirmed cases of type 2 DM, treated inthe period from 2006 to 2008 years (302 patients, 9,97%). Prevalence rate of concomitant pathology and leading causes of death were then carefullystudied on this ground. Results. We examined clinical charts of 302 patients with type 2 DM - 219 female (72.5%) at the age of 76 [70;80] and 83 male at theage of 75 [68;80]. Cardiovascular pathology and cerebrovascular disease (acute cerebrovascular event and/or postinfarction encephalic cysts,discirculatory encephalopathy) (50.66%) showed high prevalence. Respiratory system diseases (25.8%), excessive body weight and obesity (21.5%),gallstone disease (19.86%), malignant neoplasm (16.2%), prostatic hyperplasia (found in 35 male patients, 42.17%), gynecologic pathology (foundin 23 female patients, 10.5%) and infectious inflammatory diseases of kidneys and urinary tract (8.6%) were also disclosed. Leading causes of deathwere found to be acute cerebrovascular events (28.8%), postinfarction cardiosclerosis (23.18%), acute/recurring myocardium infarction (19.54%)and malignant neoplasm (14.57%). High polypathy prevalence was discovered in studied cohort, and in one third of cases patients perished fromcombination of concurrent diseases.Conclusion: High prevalence rate of intercurrent diseases and polymorbidity in patients with type 2 diabetes mellitus substantiate the need for thoroughexamination at different stages of medical care, treatment of existing malfunctions, as well as preventive measures against complications.

Цель. Изучение распространенности синдрома диабетической стопы и выявление факторов, приводящих к развитию данной патологии у больных сахарным диабетом на ЗПТ. Материалы и методы. Группу составили 60 пациентов (31 пациент на гемодиализе (ГД) и 29 на перитонеальном диализе (ПД)). В контрольную группу были включены 49 пациентов с тХПН без нарушения углеводного обмена, получающие ЗПТ в тех же диализных центрах. Для выявления признаков медиокальциноза Монкеберга (кальциноза медии артерий нижних конечностей) проводилась рентгенография голеней, стоп в двух проекциях. Оценилвалась пульсовая волна в артериях нижних конечностей. Проводилось дуплексное сканирование артерий (при выявлении признаков хронического облитерирующего заболевания артерий нижних конечностей (ХОЗАНК) вышеупомянутыми методами), рентгеноконтрастная ангиография артерий нижних конечностей (для решения вопроса о необходимости и определения предполагаемого объема проведения эндоваскулярной ангиопластики). Оценка гемодинамических изменений во время процеду- ры диализа проводилась по следующим параметрам. САД в артерии плеча, САД в артерии голени, САД в пальцевой артерии. Результаты. На момент первичного осмотра частота периферической полинейропатии составила 85%. Диабетическая остеартропатия была выявлена на момент включения в исследование у 2 пациентов (3,3%) с СД. У 9 больных СД и 4 лиц без нарушения углеводного обмена зафиксировано прогрессирование периферической полинейропатии. снижение САД как в артериях плеча, голени, так и голени и I пальца (в мм рт. ст.) было достоверно более выражено к моменту t/2 ГД как в группе с СД, так и без СД (рис. 7). Отмечено, что большинство больных с нейроишемической формой СДС в изучаемой группе имело сочетание двух признаков ? ХОЗАНК и ТрГ. Выявлено также, что доля пациентов с нейроишемической формой СДС была достоверно больше в подгруппе с ХОЗАНК и ТрГ ? 7/9, по сравнению с подгруппой с ХОЗАНК, но без ТрГ ? 1/10. Заключение. У пациентов на заместительной почечной терапии с сахарным диабетом частота встречаемости периферической полинейропатии, хронических облитерирующих заболеваний артерий нижних конечностей значимо выше по сравнению с таковой у пациентов без нарушения углеводного обмена. После начала заместительной почечной терапии у пациентов на гемодиализе отмечается увеличение частоты развития нейроишемической формы синдрома диабетической стопы. Транзиторная интрадиализная гипотензия способствует развитию нейроишемической формы синдрома диабетической стопы у пациентов с хроническими облитерирующими заболеваниями артерий нижних конечностей.

Цель. Изучение влияния компенсации углеводного обмена и терапии аторвастатином на показатели липидного спектра и атеросклеротическое воспаление у пациентов с СД 2 типа. Материалы и методы. В исследование были включены 26 пациентов с СД 2 типа. Критериями включения больных в исследование были HbA1c ? 6,5% и ХС ЛПНП ? 3 ммоль/л. Пациенты при помощи открытой блоковой рандомизации с использованием таблицы случайных чисел были разделены на две группы: основная группа: 14 пациентов на протяжении 3 мес получали аторвастатин в дозе 20 мг в сутки в 1 прием; контрольная группа: 12 пациентов на протяжении исследования аторвастатин (а также другие препараты из этой группы) не получали. Опеределяли гликированный гемоглобин, ХС, триглицериды, хс-ЛПВП, хс-ЛПНП, апо-А1, апо-В. Результаты. В результате коррекции сахароснижающей терапии через 12 нед в обеих группах отмечено значимое улучшение состояния углеводного обмена; при этом группы не отличались между собой по характеру сахароснижающей терапии. В контрольной группе через 12 нед отмечено статистически значимое повышение лишь уровня ХС ЛПВП с, в то время как уровени ХС ЛПНП, ТГ, ХС и апо В не изменились. Заключение. Гиполипидемическая терапия аторвастатином в дозе 20 мг позволяет существенно снизить уровень ОХС и ХС ЛПНП, что сопровождается выраженным снижением коэффициентов атерогенности. В результате терапии также отмечается снижение уровня ТГ и повышение уровня ХС ЛПВП. Улучшение компенсации углеводного обмена ведет к повышению уровня ХС ЛПВП и снижению коэффициентов атерогенности, однако не влияет на уровень ОХС, ХС ЛПНП и ТГ.

В настоящее время среди лиц обоего пола, находящихся в репродуктивном периоде, резко повысилась распространенность заболеваний пародонта. По данным ВОЗ (1990), наличие заболеваний пародонта (по сравнению с осложнениями кариеса) увеличивает в 5 раз развитие функциональных расстройств зубочелюстной системы, приводящих к потере зубов. Общеизвестно, что у больных СД пародонтит встречается практически в 100% случаев и характеризуется агрессивным характером течения. Данный патологический процесс имеет уникальную морфологическую структуру, значительно отличающуюся от воспалительных заболеваний пародонта у лиц без СД. Частота и тяжесть указанного процесса имеет прямую корреляционную зависимость от длительности нарушений углеводного обмена.

В данном положении был представлен современный обзор всей доступной информации из наблюдательных, рандомизированных и доклинических исследований. В феврале 2004 г. Исполнительным комитетом Международной ассоциации по менопаузе было утверждено информационное послание, адресованное врачам-специалистам и научным работникам, занимающимся проблемами лечения и профилактики проявлений менопаузального синдрома у женщин старшей возрастной группы и использующих в повседневной практике ЗГТ.

Цель. Оценка тощаковой и стимулированной секреции инсулина во взаимосвязи с динамикой гликемии, кортизолемии и липидемии в ответ на оральную нагрузку глюкозой у женщин с СПКЯ с избыточной и нормальной массой тела; установление значения резистентности и чувствительности к инсулину, состояния функции бета-клеток в интегральном ответе инсулина на стимуляцию глю? козой у женщин с СПКЯ на фоне нормальной и избыточной массы тела; определение значимости избыточной массы тела или особенностей распределения жировой ткани на выраженность метаболических расстройств у женщин с СПКЯ; выявление особенностей атерогенных сдвигов липидов плазмы в зависимости от наличия избыточной массы тела или особенностей её распределения. Материалы и методы. В исследование включены 122 женщины с СПКЯ и 30 сопоставимых здоровых женщин. Критериями включения в исследование было подтверждение диагноза СПКЯ. Проводился расчет суррогатных индексов, позволяющих оценить инсулинорезистентность натощак (ИР), функцию бета-клеток и чувствительность к инсулину на основании опубликованных формул. Обследуемые женщины разделены на 4 группы: 1-я ? здоровые женщины, у которых индекс массы тела (ИМТ) был меньше 25 кг/м2; 2-я ? здоровые с индексом массы тела 25 кг/м2 и более; 3-я ? женщины с СПКЯ и ИМТ до 25 кг/м2; 4-я - СПКЯ и ИМТ более 25 кг/м2. Результаты. худые? женщины с СПКЯ отличались от здоровых женщин достоверным увеличением окружности талии и индекса талия-бедро (ИТБ) при отсутствии различий по массе тела, что является свидетельством тенденции к абдоминальной аккумуляции жира у них, т. е. проявлением метаболического синдрома и связанных с ним нарушений. Полные женщины с СПКЯ отличались от худых? с СПКЯ более высокой тощаковой и стимулированной инсулинемией, увеличением общей продукции инсулина за двухчасовой период ОП Т (по данным площади кривой инсулина) и не различались уровнями пиковой 30-минутной инсулинемии. Полные женщины с СПКЯ отличались от худых? женщин с СПКЯ повышением тощаковой и постнагрузочной гликемии. Выводы. Клинический фенотип СПКЯ с избыточной массой тела характеризовался сочетанием тощаковой ИР, сниженной инсулиночувствительностью (ИЧ), аддитивной гиперинсулинемией и нарушенной утилизацией глюкозы, высоким базальным уровнем кортизола и его супрессией после нагрузки глюкозой, дислипидемией с повышением содержания ХС, ХС ЛПОНП, ХС ЛПНП, тошаковых и постнагрузочных ТГ и снижением уровня ХС ЛПВП. Клинический фенотип СПКЯ с нормальной массой тела характеризовался тенденцией к абдоминальной аккумуляции жира при отсутствии избыточной массы тела, снижением ИЧ и метаболического клиренса глюкозы и повышением уровня двухчасового ОГТТ инсулина и тощаковой глюкозы, базальной гиперкортизолемией и его супрессией на фоне нагрузки глюкозой и дислипидемией в виде снижения уровня ХС ЛПВП.

Цель. Оценка распространенности и характеристик ЭД у больных с малой длительностью СД 2 типа с целью установления доминирующего фактора в ее этиологии. Материалы и методы. Проведено скрининговое обследование репрезентативной выборки больных СД 2 типа мужчин в Нижнем Новгороде и различных районах Нижегородской области на предмет наличия осложнений СД в рамках Федеральной целевой программы ?Сахарный диабет?. У 29 пациентов, включанных в дальнейшее исследование произведена допплерография артерий полового члена и определение вибрационной, тактильной, температурной чувствительности полового члена с оценкой бульбокавернозного рефлекса. На основании полученных данных диагностировалась та или иная форма ЭД. Результаты. Из 29 пациентов ЭД выявлена у 8 (27%) мужчин. У 19 пациентов нарушений половой функции выявлено не было. Показатели гликированнного гемоглобина у пациентов с ЭД и без нарушений половой функции также достоверно не различались и составляли 8,7 (6,6-10,0) и 7,2% (6,9-8,8) соответственно, т. е. все пациенты находились в состоянии декомпенсации СД. В структуре ЭД у пациентов с малой длительностью СД преобладала нейрогенная форма, длительность заболевания которой была сопоставима с длительностью СД. ЭД, развившаяся именно вследствие СД, выявлена у 6 (20%) пациентов, но поскольку объем обследованной выборки невелик, реальная распространенность ЭД в данной группе пациентов может отличаться от полученной. Выводы. ЭД нередко является одним из первых выраженных клинических проявлений СД 2 типа у мужчин. В структуре ЭД, возникшей у пациентов в начале заболевания СД, преобладает нейрогенная форма. Развитие васкулогенной формы ЭД, выявляемой в начале заболевания СД, вероятнее всего связано с васкулопатией, обусловленной соматической патологией, возникшей до дебюта СД.

Цель. Изучение клинико-эпидемиологических особенностей инвалидности по зрению вследствие СД в г.Москве в течение 1994-2000 гг. Материалы и методы. Объектом исследования явились лица впервые (ВПИ) и повторно (ППИ) признанные инвалидами по зрению вследствие офтальмологических осложнений (00) СД в указанный период времени. Методом сплошного наблюдения проведена выкопировка актов освидетельствования этих лиц в специализированных офтальмологических бюро МСЭ Москвы. Методом интервью был проведен опрос освидетельствуемых лиц. Объем наблюдений составил 515 лиц ВПИ и 1462 лица ППИ. Результаты. Распространенность первичной инвалидности по зрению вследствие СД в популяции зарегистрированных больных СД составила в среднем (в 1994 ?2000 гг.) в Москве 47,6 на 100 тыс. больных. В 97% случаев лицам ВПИ по зрению при СД были установлены 1-я и 2-я группа инвалидности. Это свидетельствует о быстрой и тяжелой инвалидизации лиц с ОО СД. 27,2% лиц при первичном освидетельствовании и признании их инвалидами в связи с НО СД не были диагностированы ОО СД, которые давали основания для установления более тяжелой группы инвалидности. В 26,0% случаев была установлена 3-я группа инвалидности по соматическому заболеванию, а по состоянию органа зрения можно было установить 2-ю группу. В 1,2% случаев была установлена 2-я группа инвалидности по соматическому заболеванию, а по состоянию органа зрения могла быть установлена 1-я группа. Выводы. Показатели распространенности первичной инвалидности по зрению при СД в Москве на протяжении 1994-2000 гг. имеют тенденцию к снижению. Среди лиц ВПИ вследствие ОО СД больных трудоспособного возраста составляют 1/3. При первичном освидетельствовании больны: с ОО СД в 97% случаев больным устанавливание 1-я и 2-я группы инвалидности, что свидетельствует о тяжелой первичной инвалидизации. В настоящее время в Москве существует невыявленная первичная инвалидность по зрению при СД. При медико-социальной экспертизе больных НО СД следует учитывать офтальмологический статус этих лиц и проводить офтальмологическую МСЭ.

Диабетическая ретинопатия (ДР) является поздним осложнением сахарного диабета (СД) и одной из основных причин слепоты и снижения зрения у лиц трудоспособного возраста. В качестве скринингового метода использован метод прямой офтальмоскопии с обязательным использованием мидриатичеких капель, который проводился специально подготовленными эндокринологами, прошедшими 3-месячное обучение на базе офтальмологического отделения ЭНЦ РАМН. Проведение скрининга ДР врачами диабетологами является доступным и достаточно эффективным методом выявления ДР, в связи с чем уменьшается нагрузка на офтальмологов, улучшается контроль за течением СД и создается удобство для больного в сокращении числа визитов к врачу.
