7. Video-assisted thoracic surgery and anatomical lung resections. Where do we stand? National survey by the Spanish Society of Thoracic Surgery

Abstract

Introduction: The objective of this survey is to find out the cumulated experience and the current situation of video-assisted thoracic surgery (VATS) for anatomical lung resections in Spain. Methods: This is a descriptive study performed from two independent surveys designed through the Survey Monkey(R)web platform. The first survey was aimed at 53 thoracic surgery departments from the public and state-assisted national health system. The second survey, of a personal nature, was directed at 315 thoracic surgeons in active service, including physicians at their residency program. The surveys were kept operative from 18/11/2014 to 15/01/2015. Results: The first survey was answered by 32 (60%) departments and the second by 167 (53%) professionals. A total of 29 (91%) of the thoracic surgery departments represented recognized having some level of experience in this technique. However, a great proportion of departments, 15 (52%), counted less than 100 procedures and the cumulated time of experience was lower than 5 years in 19 (66%) departments. Among all the individual respondents, 126 (77%) admitted having performed the procedure at some point. Of those without any experience, at least 36 (95%) of them recognized that future training in this technique is one of their future professional objectives. Conclusions: Waiting for future prospective national registries contribute further information about the expansion of this technique in our country, the results of the current survey show, up to now, the best reflection of clinical practice and opinion of the surgeons involved in the development of VATS.(C) 2016 AEC. 46. Carbohydrate-Based Polycarbonates. Synthesis, Structure, and Biodegradation Studies

Abstract

Linear homo- and co-polycarbonates have been prepared from the sugar-based diols 2,3,4-tri-O-methyl-l-arabinitol and 2,3,4-tri-O-methylxylitol. Statistical co-polycarbonates were obtained from molar feed ratios of sugar:bisphenol A of 1:1 and 2:1. The polycarbonates were stable up to ~280 °C with maximum decomposition rate above 345 °C. The arabinitol-based homopolymer was found to be semicrystalline whereas the xylitol-based one was amorphous. All the copolymers were amorphous with Tg ranging between 60 and 80 °C. Both homo- and co-polycarbonates showed high resistance to chemical hydrolysis; however, they were enzymatically degraded in different degree. The xylitol-based polycarbonates were the more easily degraded by lipase B from Candida antarctica in acetonitrile at 70 °C&sbd;this enzyme showing a high selectivity toward the configuration of the sugar-based moiety.

47. Synthesis and Characterization of Linear Polyamides Derived from <SCP>l</SCP>-Arabinitol and Xylitol

Abstract

The synthesis and characterization of a new series of AABB-type polyamides based on l-arabinitol and xylitol are described. These linear polyamides were obtained by reaction of the aldaric acids or aliphatic dicarboxylic acids, with sugar-based diamine or aliphatic diamines. The pentaric diacids were activated as their pentachlorophenyl esters, and the diamino sugars were used as dihydrochlorides. Aliphatic diamines were used either as their activated trimethylsilyl derivatives or as the free bases. All the polyamides were soluble in the usual organic solvents, and those fully sugar-based were also soluble in water. All of them were very hygroscopic, especially those based on xylitol. DSC and X-ray powder diffraction studies showed that l-arabinitol-based polyamides were more crystalline than those derived from xylitol.

Abstract

Polycondensation of 1,6-diamino-1,6-dideoxy-2,3,4,5-tetra-O-methyl-d-mannitol or 1,6-diamino-1,6-dideoxy-2,3,4,5-tetra-O-methyl-l-iditol with aromatic and aliphatic dicarboxylic acids has been used to prepare a series of regio- and stereoregular AABB-type polyamides. We also describe the preparation of 1,6-diamino-1,6-dideoxy-3,4-O-isopropylidene-d-mannitol and 1,6-diamino-1,6-dideoxy-2,5-di-O-methyl-3,4-O-isopropylidene-d-mannitol and their polycondensation reactions with the same dicarboxylic acids. The obtained polyamides were gummy solids that were optically active and had a pronounced affinity for water, although most of them were not soluble in this solvent. These polyamides were characterized by viscosimetry, gel-permeation chromatography, elemental analysis, and IR and NMR spectroscopies.

49. Carbohydrate-Based Copolymers. Hydrolysis of Copoly(ester amide)s Containing <SCP>l</SCP>-Arabinose Units

Abstract

The in vitro hydrolysis of a series of copoly(ester amide)s abbreviated as PVGAn, prepared previously by random copolymerization of 1-amino-1-deoxy-2,3,4-tri-O-methyl-5-O-[(pentachlorophenoxy)succinyl]-l-arabinitol hydrochloride and 5-amino-1-O-[(pentachlorophenoxy)glutaryl]pentanol hydrochloride, has been performed in phosphate buffer at 37 °C and evaluated by weight loss and intrinsic viscosity. The copoly(ester amide) PVGA46, containing about 50% of the sugar-based monomer, was water-soluble. Its solubility in water displayed a temperature dependence that was studied and related with the hydrolysis process. The degradation of the aliphatic copoly(ester amide) was greatly enhanced with increasing amount of arabinose−succinyl monomer incorporated in the polymer chain. The presence of small amounts of this monomer was enough to produce a noticeable increase in polymer degradability. Spectroscopic investigations of the hydrolysis products provided evidence for succinimide ring formation, supporting the general mechanism proposed for the hydrolysis of poly(ester amide)s containing four-carbon diacid units in their structure.

Abstract

Copoly(ester amide)s, abbreviated as PVGAn, have been prepared by random copolymerization of 1-amino-1-deoxy-2,3,4-tri-O-methyl-5-O-[(pentachlorophenoxy)succinyl]-l-arabinitol hydrochloride and 5-amino-1-O-[(pentachlorophenoxy)glutaryl]pentanol hydrochloride by the active ester polycondensation method. The molar ratios of the two comonomers in the feed varied from 1/99 to 50/50. The two monomeric units are randomly distributed within the copolymers, as assessed by NMR analysis, and the degree of randomness and average sequence lengths of the different copoly(ester amide)s have been experimentally determined. Thermal properties depend on the copoly(ester amide) composition, since it was found that the melting and the decomposition temperatures decreased as the molar fraction of carbohydrate-based monomer unit increased, whereas the glass-transition temperature increased in parallel.
