OBJECTIVE: Caudal block is a type of regional anesthesia suitable for inguinal hernia surgery in children. Our goal was to determine the effectiveness of caudal block combined with general anesthesia in providing intraand postoperative analgesia and its effect on hemodynamic stability and drug consumption. PATIENTS AND METHODS: Prospective, randomized controlled study included 78 boys, 3-5 years, with an indication for inguinal herniorrhaphy, divided into groups G (general anesthesia, n=39) and G+C (general anesthesia + caudal block, n=39). We monitored hemodynamic parameters intraoperatively, postoperative pain, and total consumption of all medicaments in the perioperative period. The monitoring of complications and side effects of drugs was also carried out. RESULTS: Boys in group G had statistically significantly higher values of heart rate in the 5th minute (p<0.01), in the 25th minute (p<0.01), and after awakening from anesthesia (p<0.01). We obtained similar results with systolic and diastolic pressure values in the 5th minute (p<0.01), 15th minute (p<0.01), 25th minute (p<0.01), before awakening from anesthesia (p<0.01) and after awakening (p<0.01). They also had significantly statistically higher total consumption of propofol, fentanyl, and acetaminophen (p<0.01). Boys in group G+C had significantly lower postoperative pain scores: initially (p<0.01), after 2 hours (p<0.01), and after 5 hours (p<0.01). No complications occurred in this group. CONCLUSIONS: In children, the combination of general anesthesia with caudal block, compared to general anesthesia only, is more effi- cient in suppressing visceral pain, leading to better hemodynamic stability, and reducing the consumption of medicines in the perioperative period. OBJECTIVE: Laryngoscopy and endotracheal intubation (EI) often provoke a marked sympathetic response, which leads to tachycardia and hypertension. The aim of this study was to investigate the effect of different doses of remifentanil on the cardiovascular response to laryngoscopy and EI. PATIENTS AND METHODS: 100 patients were included in this randomized study. The participants were divided into four groups of 25 patients each. The patients in the control group did not receive remifentanil. The patients from other three groups received remifentanil prior to induction of anesthesia at doses of 0.5 µg/kg, 1 µg/kg, and 1.5 µg/kg. Hemodynamic parameters were measured before and after administration of remifentanil, after induction of anesthesia and one minute after EI. RESULTS: After administration of remifentanil and induction of anesthesia, a decrease in arterial pressure and heart rate occurred in most patients. After EI, an increase in arterial pressure and heart rate was observed in most patients. The largest increase was recorded in the group of patients who did not receive remifentanil. The best hemodynamic response was observed in patients who received 1 and 1.5 µg/kg of remifentanil. CONCLUSIONS: Remifentanil at the doses of 1-1.5 µg/kg is absolutely safe for co-induction of anesthesia with thiopental. Background and Objectives: The moment of birth represents a complex physiological process that is followed by adaptive changes in the vital systems of the newborn. Such reactions have their positive but also negative effects. The aim of this research was to determine the difference in laboratory values of stress indicators in newborn children delivered by cesarean section (CS) with general and spinal anesthesia. We attempted to make a recommendation about the optimal type of anesthesia based on our results., Materials and Methods: The study was performed on 150 healthy term newborns delivered by urgent or planned CS. Samples for adrenocorticotropic hormone (ACTH), cortisol, triglycerides, and interleukin-6 (IL-6) were analyzed., Results: Leukocyte numbers, triglycerides, and blood sugar values were normal for the newborns' age, with statistically significantly lower values of blood sugar and triglycerides in newborns delivered by CS in spinal anesthesia ( p < 0.005) compared to general anesthesia. There were no significant differences in ACTH, cortisol, and IL-6 levels between those newborns delivered via CS after spinal or general anesthesia., Conclusions: In cases where vaginal delivery is not possible, when CS is indicated, the use of well-controlled spinal anesthesia is followed by lower degrees of metabolic, inflammatory, and stress responses and better vitality of the baby upon birth., Competing Interests: The authors declare no conflicts of interest.

To date, batteries are the most widely used energy storage devices, fulfilling the requirements of different industrial and consumer applications. However, the efficient use of renewable energy sources and the emergence of wearable electronics has created the need for new requirements such as high-speed energy delivery, faster charge-discharge speeds, longer lifetimes, and reusability. This leads to the need for supercapacitors, which can be a good complement to batteries. However, one of their drawbacks is their lower energy storage capability, which has triggered worldwide research efforts to increase their energy density. With the introduction of novel nanostructured materials, hierarchical pore structures, hybrid devices combining these materials, and unconventional electrolytes, significant developments have been reported in the literature. This paper reviews the short history of the evolution of supercapacitors and the fundamental aspects of supercapacitors, positioning them among other energy-storage systems. The main electrochemical measurement methods used to characterize their energy storage features are discussed with a focus on their specific characteristics and limitations. High importance is given to the integral components of the supercapacitor cell, particularly to the electrode materials and the different types of electrolytes that determine the performance of the supercapacitor device (e.g., storage capability, power output, cycling stability). Current directions in the development of electrode materials, including carbonaceous forms, transition metal-based compounds, conducting polymers, and novel materials are discussed. The synergy between the electrode material and the current collector is a key factor, as well as the fine-tuning of the electrode material and electrolyte.

The increasing demand for nanofiltration processes in drinking water treatment, industrial separation and wastewater treatment processes has highlighted several shortcomings of current state-of-the-art thin film composite (TFC NF) membranes, including limitations in chemical resistance, fouling resistance and selectivity. Polyelectrolyte multilayer (PEM) membranes provide a viable, industrially applicable alternative, providing significant improvements in these limitations. Laboratory experiments using artificial feedwaters have demonstrated selectivity an order of magnitude higher than polyamide NF, significantly higher fouling resistance and excellent chemical resistance (e.g., 200,000 ppmh chlorine resistance and stability over the 0-14 pH range). This review provides a brief overview of the various parameters that can be modified during the layer-by-layer procedure to determine and fine-tune the properties of the resulting NF membrane. The different parameters that can be adjusted during the layer-by-layer process are presented, which are used to optimize the properties of the resulting nanofiltration membrane. Substantial progress in PEM membrane development is presented, particularly selectivity improvements, of which the most promising route seems to be asymmetric PEM NF membranes, offering a breakthrough in active layer thickness and organic/salt selectivity: an average of 98% micropollutant rejection coupled with a NaCl rejection below 15%. Advantages for wastewater treatment are highlighted, including high selectivity, fouling resistance, chemical stability and a wide range of cleaning methods. Additionally, disadvantages of the current PEM NF membranes are also outlined; while these may impede their use in some industrial wastewater applications, they are largely not restrictive. The effect of realistic feeds (wastewaters and challenging surface waters) on PEM NF membrane performance is also presented: pilot studies conducted for up to 12 months show stable rejection values and no significant irreversible fouling. We close our review by identifying research areas where further studies are needed to facilitate the adoption of this notable technology.

The goals of the present investigation are to study and to model pale lager beer dealcoholization via reverse osmosis (RO). Samples were dealcoholized at a temperature of 15 ± 1 °C. An Alfa Laval RO99 membrane with a 0.05 m 2 surface was used. The flux values were measured during the separations. The ethanol content, extract content, bitterness, color, pH, turbidity, and dynamic viscosity of beer and permeate samples were measured. The initial flux values were determined using linear regression. The initial ethanol flux (JEtOH 0) values were calculated from the initial flux values and the ethanol content values. A 2 P full factorial experimental design was applied, and the factors were as follows: transmembrane pressure (TMP): 10, 20, 30 bar; retentate flow rate (Q): 120, 180, 240 L/h; JEtOH 0 was considered as the response. The effect sizes of the significant parameters were calculated. The global maximum of the objective function was found using a self-developed Grid Search code. The changes in the analytical parameters were appropriate. The TMP had a significant effect, while the Q had no significant effect on the JEtOH 0. The effect size of the TMP was 1.20. The optimal value of the factor amounted to TMP = 30 bar. The predicted JEtOH 0 under the above conditions was 121.965 g/m 2 h.

Membrane-based beverage dealcoholization is a successful process for producing low- and non-alcoholic beer and represents a fast-growing industry. Polyamide NF and RO membranes are commonly applied for this process. Polyelectrolyte multilayer (PEM) NF membranes are emerging as industrially relevant species, and their unique properties (usually hollow fiber geometry, high and tunable selectivity, low fouling) underlines the importance of testing them in the food industry as well. To test PEM NF membranes for beer dealcoholization at a small pilot scale, we dealcoholized filtered and unfiltered lager beer with the tightest available commercial polyelectrolyte multilayer NF membrane (NX Filtration dNF40), which has a MWCO = 400 Da, which is quite high for these purposes. Dealcoholization is possible with a reasonable flux (10 L/m 2 h) at low pressures (5-8.6 bar) with a real extract loss of 15-18% and an alcohol passage of ~100%. Inorganic salt passage is high (which is typical for PEM NF membranes), which greatly affected beer flavor. During the dealcoholization process, the membrane underwent changes which substantially increased its salt rejection values (MgSO 4 passage decreased fourfold) while permeance loss was minimal (less than 10%). Effluents of anaerobic processes still contain valuable components, among which volatile fatty acids (VFAs) can be regarded and should be recovered and/or used further in applications such as microbial electrochemical technology to generate energy/energy carriers. To accomplish the separation of VFAs from waste liquors, various membrane-based solutions applying different transport mechanisms and traits are available, including pressure-driven nanofiltration (NF) and reverse osmosis (RO) which are capable to clarify, fractionate and concentrate salts and organics. Besides, emerging techniques using a membrane such as forward osmosis (FO) and supported liquid membrane (SILM) technology can be taken into consideration for VFA separation. In this work, we evaluate these four various downstream methods (NF, RO, FO and SILM) to determine the best one, comparatively, for enriching VFAs from pH-varied model solutions composed of acetic, butyric and propionic acids in different concentrations. The assessment of the separation experiments was supported by statistical examination to draw more solid conclusions. Accordingly, it turned out that all methods can separate VFAs from the model solution. The highest average retention was achieved by RO (84% at the applied transmembrane pressure of 6 bar), while NF provided the highest permeance (6.5 L/m 2 hbar) and a high selectivity between different VFAs.

Galacto-oligosaccharides (GOS) are prebiotic compounds widely used for their health-promoting effects. Conventionally, GOS is produced by the enzymatic conversion of lactose in stirred tank reactors (STR). The high operational costs associated with enzyme inactivation and removal might be reduced by the application of enzyme membrane reactors (EMR). In this study, we aimed to assess the potential of continuous GOS production by EMR using soluble Biolacta N5, a Bacillus circulans -derived commercial enzyme preparation. The steady-state performance of the EMR equipped with an ultrafiltration module was investigated as function of residence time (1.1-2.8 h) and enzyme load (17-190 U·g -1 ) under fixed operational settings of temperature (50 °C), pH (6.0), lactose feed concentration (300 g·kg -1 ), and recirculation flow-rate (0.18 m 3 ·h -1 ). Results indicate that the yield of oligosaccharides with higher degree of polymerization (DP3-6) in STR (approx. 38% on total carbohydrate basis) exceeds that measured in EMR (ranging from 24% to 33%). However, a stable catalytic performance without a significant deterioration in product quality was observed when operating the EMR for an extended period of time (> 120 h). Approx. 1.4 kg of DP3-6 was produced per one gram of crude enzyme preparation over the long-term campaigns, indicating that EMR efficiently recovers enzyme activity.

Enzymatic hydrolysis of soybean milk proteins with cysteine protease papain was performed in an advanced bioreactor, operated with batch mode. In soybean milk protein hydrolysis reaction, enzyme and substrate ratio and reaction temperature were varied, ranging from 0.029:100-0.457:100 and 30-60 °C, respectively. The degree of hydrolysis of soybean milk proteins was increased with increase of enzyme and substrate (soybean milk protein) ratio. However, the degree of hydrolysis was increased due to change of reaction temperature from 30 °C to 60 °C with enzyme and substrate ratio 0.229:100 and was reduced when hydrolysis reaction was performed with enzyme and substrate ratio 0.11:100 at hydrolysis temperature 60 °C. Antioxidant capacity of enzyme-treated milk had a similar trend with degree of hydrolysis. In a later exercise, a membrane bioreactor was adopted for continuous production of antioxidant and antibacterial peptides from soybean milk. The membrane bioreactor was operated for 12 h with constant feeding. Ceramic-made tubular membrane with a pore size 20 nm was used. Application of static turbulence promoter in a membrane separation process was investigated and its positive effects, with respect to higher permeate flux and lower energy consumption in filtration process, were proven. Antioxidant capacity and antibacterial activity against Bacillus cereus of enzyme-hydrolyzed milk and permeate from membrane were confirmed.

Lactose-based prebiotics are synthesized by enzymatic- or microbial- biotransformation of lactose and have unique functional values. In this comprehensive review article, the biochemical mechanisms of controlling osteoporosis, blood-lipid, and glucose levels by lactose-based prebiotics and symbiosis with probiotics are reported along with the results of clinical investigations. Interaction between lactose-based prebiotics and probiotics reduces osteoporosis by (a) transforming insoluble inorganic salts to soluble and increasing their absorption to gut wall; (b) maintaining and protecting mineral absorption surface in the intestine; (c) increasing the expression of calcium-binding proteins in the gut wall; (d) remodeling osteoclasts and osteoblasts formation; (e) releasing bone modulating factors; and (f) degrading mineral complexing phytic acid. Lactose-based prebiotics with probiotics control lipid level in the bloodstream and tissue by (a) suppressing the expressions of lipogenic- genes and enzymes; (b) oxidizing fatty acids in muscle, liver, and adipose tissue; (c) binding cholesterol with cell membrane of probiotics and subsequent assimilation by probiotics; (d) enzymatic-transformations of bile acids; and (e) converting cholesterol to coprostanol and its defecation. Symbiosis of lactose-based prebiotics with probiotics affect plasma glucose level by (a) increasing the synthesis of gut hormones plasma peptide-YY, glucagon-like peptide-1 and glucagon-like peptide-2 from entero-endocrine L-cells; (b) altering glucose assimilation and metabolism; (c) suppressing systematic inflammation; (d) reducing oxidative stress; and (e) producing amino acids. Clinical investigations show that lactose-based prebiotic galacto-oligosaccharide improves mineral absorption and reduces hyperlipidemia. Another lactose-based prebiotic, lactulose, improves mineral absorption, and reduces hyperlipidemia and hyperglycemia. Lactose-derived prebiotics provide wide ranges of gastrointestinal comforts. In this review article, the probable biochemical mechanisms through which lactose-derived prebiotics offer positive gastrointestinal health are reported along with the up-to-date results of clinical investigations; this might be the first review article of its kind, to the best of our knowledge. Lactose-derived prebiotics have unique biological and functional values, and they are confirmed as 'safe' by the Food and Drug Administration federal agency. Medical practitioners frequently recommend them as therapeutics as a pure form or combined with dairy-based products (yoghurt, milk and infant formulas) or fruit juices. The biological activities of lactose-derived prebiotics are expressed in the presence of gut microflora, mainly probiotics ( Lactobacillus spp. in the small intestine and Bifidobacterium spp. in the large intestine). Clinical investigations reveal that galacto-oligosaccharide reduces the risks of several types of diarrhea (traveler's diarrhea, osmotic diarrhea and Clostridium difficile associated relapsing diarrhea). Lactulose and lactosucrose prevent inflammatory bowel diseases (Crohn's disease and ulcerative colitis). Lactulose and lactitol reduce the risk of hepatic encephalopathy. Furthermore, lactulose, galacto-oligosaccharide and lactitol prevent constipation in individuals of all ages. This paper presents the results of the experimental and numerical investigation of interactions between surface flood flow in urban areas and the flow in below ground drainage systems (sewer pipes and manholes). An experimental rig has been set up at the Water Engineering Laboratory at the University of Sheffield. It consists of a full scale gully structure with inlet grating, which connects the 8 m(2) surface area with the pipe underneath that can function as an outfall and is also further connected to a tank so that it can come under surcharging conditions and cause outflow from the gully. A three-dimensional CFD (Computational Fluid Dynamics) model has been set up to investigate the hydraulic performance of this type of gully inlet during the interactions between surface flood flow and surcharged pipe flow. Preliminary results show that the numerical model can replicate various complex 3D flow features observed in laboratory conditions. This agreement is overall better in the case of water entering the gully than for the outflow conditions. The influence of the surface transverse slope on flow characteristics has been demonstrated. It is shown that re-circulation zones can form downstream from the gully. The number and size of these zones is influenced by the transverse terrain slope.
