This work presents an unsupervised method for automatically constructing and expanding topic taxonomies using instruction-based fine-tuned LLMs (Large Language Models). We apply topic modeling and keyword extraction techniques to create initial topic taxonomies and LLMs to post-process the resulting terms and create a hierarchy. To expand an existing taxonomy with new terms, we use zero-shot prompting to find out where to add new nodes, which, to our knowledge, is the first work to present such an approach to taxonomy tasks. We use the resulting taxonomies to assign tags that characterize merchants from a retail bank dataset. To evaluate our work, we asked 12 volunteers to answer a two-part form in which we first assessed the quality of the taxonomies created and then the tags assigned to merchants based on that taxonomy. The evaluation revealed a coherence rate exceeding 90% for the chosen taxonomies. The taxonomies' expansion with LLMs also showed exciting results for parent node prediction, with an f1-score above 70% in our taxonomies.

This work proposes the Two-headed DragoNet, a Transformer-based model for hierarchical multi-label classification of financial transactions. Our model is based on a stack of Transformers encoder layers that generate contextual embeddings from two short textual descriptors (merchant name and business activity), followed by a Context Fusion layer and two output heads that classify transactions according to a hierarchical two-level taxonomy (macro and micro categories). Finally, our proposed Taxonomy-aware Attention Layer corrects predictions that break categorical hierarchy rules defined in the given taxonomy. Our proposal outperforms classical machine learning methods in experiments of macro-category classification by achieving an F1-score of 93\% on a card dataset and 95% on a current account dataset.

Hypoxia-inducible factor pathway genes are linked to adaptation in both human and nonhuman highland species. EPAS1, a notable target of hypoxia adaptation, is associated with relatively lower hemoglobin concentration in Tibetans. We provide evidence for an association between an adaptive EPAS1 variant (rs570553380) and the same phenotype of relatively low hematocrit in Andean highlanders. This Andean-specific missense variant is present at a modest frequency in Andeans and absent in other human populations and vertebrate species except the coelacanth. CRISPR-base-edited human cells with this variant exhibit shifts in hypoxia-regulated gene expression, while metabolomic analyses reveal both genotype and phenotype associations and validation in a lowland population. Although this genocopy of relatively lower hematocrit in Andean highlanders parallels well-replicated findings in Tibetans, it likely involves distinct pathway responses based on a protein-coding versus noncoding variants, respectively. These findings illuminate how unique variants at EPAS1 contribute to the same phenotype in Tibetans and a subset of Andean highlanders despite distinct evolutionary trajectories.

Introduction: The prevalence of overweight and obesity has increased in the last decades, posing significant health and economic impacts globally. These conditions are related to several non-communicable diseases, including cardiovascular disease, type II diabetes and cancer. This study estimated the disease burden and healthcare costs associated with overweight and obesity in the adult population in mainland Portugal, in 2018. Method: Burden of disease was measured in disability-adjusted life years (DALY) following Global Burden of Disease (GBD) methodology. DALY were calculated as the sum of years of life lost (YLL) and years lived with disability (YLD). The analyses included morbidity, mortality and related costs directly related to overweight and obesity, as well as the attributable morbidity, mortality and related costs of of 25 selected diseases related to obesity (DrO). A prevalence-based cost analysis was conducted a from the perspective of the public National Health Service, including costs related to inpatient, outpatient care and pharmacological treatment. Results: In 2018, total DALY amounted to 260,943, with 75% due to premature death (196,438 YLL) and 25% due to disability (64,505 YLD). The economic burden of overweight and obesity was estimated at approximately € 1,148 million. Of these, approximately € 13.3 million (1%) were costs related to the treatment of obesity, and the remaining were costs of DrO attributed to overweight and obesity. Outpatient care corresponded to 43% of total costs, pharmacological treatment 38%, and inpatient care 19%. Cardiovascular and cerebrovascular diseases were the largest contributor to total costs (38%), followed by type II diabetes (34%). Conclusion: Overweight and obesity incur a large disease and economic burden to the public healthcare sector, representing approximately 0.6% of the country´s gross domestic product and 5.8% of public health expenditures.

8. Treatment ineffectiveness towards Haemonchus contortus is highly prevalent in sheep and goat farms of North-Eastern Italy

Abstract Background Anthelmintic resistance (AR) is a global threat to grazing livestock farming. In Italy, anthelmintic efficacy remains high compared to other European countries, but many parts of the country haven’t been investigated yet. Local veterinary practitioners from Trentino and Veneto regions reported suspected inefficacy towards anthelmintic drugs in some of their farms, prompting a study on AR in sheep and goat farms of northern Italy. The study aimed to assess anthelmintic effectiveness using genus-specific faecal egg count reduction tests (FECRT), to detect differences in treatment response among nematode genera involved in the infection. Results Twelve farms (6 sheep and 6 goat farms) were included based on clinical suspicion of AR. Treatments were carried out with either benzimidazoles (BZ) or macrocyclic lactones (ML) Treatment was effective in 3/6 goat trials, with reduced effectiveness to BZ in two farms and to ML the last one. In sheep farms (6/6), effectiveness was consistently and more severely insufficient. Ineffectiveness was particularly high towards Haemonchus contortus, while Oesophagostomum/Chabertia maintained susceptibility in nearly all trials. Trichostrongylus/Teladorsagia exhibited intermediate results. Conclusions This study reveals diminished efficacy of both BZ and ML in small ruminant farms in north-eastern Italy, an area previously lacking data on the topic, except for goats in South Tyrol. Variability in treatment responses among nematode genera support suspicions of AR, and further concerns are raised by the prevalence of treatment ineffectiveness against the highly pathogenic Haemonchus contortus. This finding underscores the urgent need for comprehensive AR monitoring in the area and improved management practices to prevent further resistance development and protect livestock health.

Abstract Introduction: Hemorrhagic cystitis (HC) is a common complication after allogeneic hematopoietic stem cell transplantation (HSCT), characterized by inflammation and bleeding of the bladder. Hyperbaric oxygen therapy (HBOT) has been shown to be effective in the treatment of radiation-induced HC. However, the optimal treatment for HC after allogeneic HSCT has not yet been established. Furthermore, limited research has been conducted on the use of HBOT in this setting. This study aimed to evaluate the effectiveness and safety of HBOT in patients with late-onset HC after allogeneic HSCT. Methods: Twenty-five-year (1998–2022) retrospective analysis performed in all consecutive patients with confirmed late-onset HC after allogeneic HSCT treated with HBOT at two centers in Portugal. Medical records were reviewed for clinical and laboratory features, primary indications for allogeneic HSCT, conditioning regimen, and treatment strategies for HC. Patients received 100% oxygen at 2.1–2.5 atmosphere absolute pressure (ATA) for 70–90-minute periods, once daily, five times per week. Complete clinical response was defined as the absence of macroscopic hematuria sustained for at least 2 weeks, and partial response was described as a downgrading in the severity of HC. Purpose – This study aims to evaluate the presence of abnormal returns due to stock splits or reverse stock splits in the Brazilian capital market context. Design/methodology/approach – The event study technique was used on data from 518 events that occurred in a 30-year period (1987–2016), comprising 167 stock splits and 351 reverse stock splits. Findings – The results revealed the occurrence of abnormal returns around the time the shares began trading stock splits or reverse stock splits at a statistical significance level of 5%. The main conclusion is that stock split and reverse stock split operations represent opportunities for extraordinary gains and may serve as a reference for investment strategies in the Brazilian stock market. Originality/value – This study innovates by including reverse stock splits, as the existing literature focuses on stock splits, and by testing two distinct “zero” dates that of the ordinary general meeting that approved the share alteration and the “ex” date of the alteration, when the shares were effectively traded, reverse split or split.

The hypoxic ventilatory response (HVR) is the increase in breathing in response to reduced arterial oxygen pressure. Over several decades, studies have revealed substantial population-level differences in the magnitude of the HVR as well as significant inter-individual variation. In particular, low HVRs occur frequently in Andean high-altitude native populations. However, our group conducted hundreds of HVR measures over several years and commonly observed low responses in sea-level populations as well. As a result, we aimed to determine the normal HVR distribution, whether low responses were common, and to what extent variation in study protocols influence these findings. We conducted a comprehensive search of the literature and examined the distributions of HVR values across 78 studies that utilized step-down/steady-state or progressive hypoxia methods in untreated, healthy human subjects. Several studies included multiple datasets across different populations or experimental conditions. In the final analysis, 72 datasets reported mean HVR values and 60 datasets provided raw HVR datasets. Of the 60 datasets reporting raw HVR values, 35 (58.3%) were at least moderately positively skewed (skew > 0.5), and 21 (35%) were significantly positively skewed (skew > 1), indicating that lower HVR values are common. The skewness of HVR distributions does not appear to be an artifact of methodology or the unit with which the HVR is reported. Further analysis demonstrated that the use of step-down hypoxia versus progressive hypoxia methods did not have a significant impact on average HVR values, but that isocapnic protocols produced higher HVRs than poikilocapnic protocols. This work provides a reference for expected HVR values and illustrates substantial inter-individual variation in this key reflex. Finally, the prevalence of low HVRs in the general population provides insight into our understanding of blunted HVRs in high-altitude adapted groups. KEY POINTS: The hypoxic ventilatory response (HVR) plays a crucial role in determining an individual's predisposition to hypoxia-related pathologies. There is notable variability in HVR sensitivity across individuals as well as significant population-level differences. We report that the normal distribution of the HVR is positively skewed, with a significant prevalence of low HVR values amongst the general healthy population. We also find no significant impact of the experimental protocol used to induce hypoxia, although HVR is greater with isocapnic versus poikilocapnic methods. These results provide insight into the normal distribution of the HVR, which could be useful in clinical decisions of diseases related to hypoxaemia. Additionally, the low HVR values found within the general population provide insight into the genetic adaptations found in populations residing in high altitudes.

SPIDER is the full-scale prototype of the ion source of the ITER Heating Neutral Beam Injector, where negative ions of Hydrogen or Deuterium are produced by a RF generated plasma and accelerated with a set of grids up to ~100 keV. The Power Supply System is composed of high voltage dc power supplies capable of handling frequent grid breakdowns, high current dc generators for the magnetic filter field and RF generators for the plasma generation. During the first 3 years of SPIDER operation different electrical issues were discovered, understood and addressed thanks to deep analyses of the experimental results supported by modelling activities. The paper gives an overview on the observed phenomena and relevant analyses to understand them, on the effectiveness of the short-term modifications provided to SPIDER to face the encountered issues and on the design principle of long-term solutions to be introduced during the currently ongoing long shutdown., Comment: 8 pages, 12 figures. ITER envisages the use of two heating neutral beam injectors plus an optional one as part of the auxiliary heating and current drive system. The 16.5 MW expected neutral beam power per injector is several notches higher than worldwide existing facilities. A Neutral Beam Test Facility (NBTF) was established at Consorzio RFX, exploiting the synergy of two test beds, SPIDER and MITICA. SPIDER is dedicated to developing and characterizing large efficient negative ion sources at relevant parameters in ITER-like conditions: source and accelerator located in the same vacuum where the beam propagates, immunity to electromagnetic interferences of multiple radio-frequency (RF) antennas, avoidance of RF-induced discharges on the outside of the source. Three years of experiments on SPIDER have addressed to the necessary design modifications to enable full performances. The source is presently under a long shut-down phase to incorporate learnings from the experimental campaign. Parallelly, developments on MITICA, the full-scale prototype of the ITER NBI featuring a 1 MV accelerator and ion neutralization, are underway including manufacturing of in-vessel components, while power supplies and auxiliary plants are already under final testing and commissioning. Integration, commissioning and tests of the 1MV power supplies are essential for this first-of-kind system, unparalleled both in research and industry field. The integrated test to confirm 1MV output by combining invertor systems, DC generators and transmission lines extracted errors/accidents in some components. To realize a concrete system for ITER, solutions for the repair and the improvement of the system were developed. Hence, NBTF is emerging as a necessary facility, due to the large gap with existing injectors, effectively dedicated to identify issues and find solutions to enable successful ITER NBI operations in a time bound fashion.

Abstract Background Mechanical stimulation (MS) significantly increases the release of adenine and uracil nucleotides from bone marrow-derived mesenchymal stem cells (BM-MSCs) undergoing osteogenic differentiation. Released nucleotides acting via ionotropic P2X7 and metabotropic P2Y6 purinoceptors sensitive to ATP and UDP, respectively, control the osteogenic commitment of BM-MSCs and, thus, bone growth and remodelling. Yet, this mechanism is impaired in post-menopausal (Pm)-derived BM-MSCs, mostly because NTPDase3 overexpression decreases the extracellular accumulation of nucleotides below the levels required to activate plasma membrane-bound P2 purinoceptors. This prompted us to investigate whether in vitro MS of BM-MSCs from Pm women could rehabilitate their osteogenic commitment and whether xenotransplantation of MS purinome-primed Pm cells promote repair of critical bone defects in an in vivo animal model. Methods BM-MSCs were harvested from the neck of femora of Pm women (70 ± 3 years old) undergoing total hip replacement. The cells grew, for 35 days, in an osteogenic-inducing medium either submitted (SS) or not (CTR) to MS (90 r.p.m. for 30 min) twice a week. Increases in alkaline phosphatase activity and in the amount of osteogenic transcription factors, osterix and osteopontin, denoted osteogenic cells differentiation, while bone nodules formation was ascertain by the alizarin red-staining assay. The luciferin-luciferase bioluminescence assay was used to quantify extracellular ATP. The kinetics of the extracellular ATP (100 µM) and UDP (100 µM) catabolism was assessed by HPLC. The density of P2Y6 and P2X7 purinoceptors in the cells was assessed by immunofluorescence confocal microscopy. MS-stimulated BM-MSCs from Pm women were xenotransplanted into critical bone defects drilled in the great trochanter of femora of one-year female Wistar rats; bone repair was assessed by histological analysis 10 days after xenotransplantation. Results MS-stimulated Pm BM-MSCs in culture (i) release 1.6-fold higher ATP amounts, (ii) overexpress P2X7 and P2Y6 purinoceptors, (iii) exhibit higher alkaline phosphatase activity and overexpress the osteogenic transcription factors, osterix and osteopontin, and (iv) form larger bone nodules, than CTR cells. Selective blockage of P2X7 and P2Y6 purinoceptors with A438079 (3 µM) and MRS 2578 (0.1 µM), respectively, prevented the osteogenic commitment of cultured Pm BM-MSCs. Xenotransplanted MS purinome-primed Pm BM-MSCs accelerated the repair of critical bone defects in the in vivo rat model. Conclusions Data suggest that in vitro MS restores the purinergic cell-to-cell communication fostering the osteogenic differentiation and osteointegration of BM-MSCs from Pm women, a strategy that may be used in bone regeneration and repair tactics.

A pandemia exigiu adaptações imediatas no trabalho escolar cujas repercussões ainda não são bem conhecidas. No artigo, exploram-se os efeitos do confinamento nos modos de trabalho dos professores, considerando a utilização intensiva das tecnologias digitais e as oportunidades de aprendizagem profissional durante esse período. Recorre-se aos dados de um questionário respondido por professores do ensino básico e secundário entre julho e setembro de 2020. Identificam-se aprendizagens profissionais indissociáveis de mudanças nas dinâmicas pedagógicas e didáticas. Constata-se que a adaptação e adoção de novas práticas pelos professores variou em função do género, das habilitações, do nível de ensino, do tempo de serviço, entre outras variáveis.

Introdução: Citomegalovírus (CMV) é um evento frequente no transplante alogênico de células hematopoéticas (Alo-TCH), cuja primo infecção ocorre, em geral, no período da infância. A imunossupressão aumenta o risco de reativação e está associada a alta morbidade e mortalidade nessa população. Esse estudo tem por objetivo avaliar se a reativação de CMV na população pediátrica apresenta comportamento semelhante a reativação na população adulta submetida a Alo-TCH. Métodos: Estudo de coorte prospectivo em indivíduos submetidos a TCH em uma única instituição entre 2013 e 2024. Pacientes foram testados por PCR quantitativo de CMV (Taqman Sistem – artus CMV Qiagen) no plasma para detecção de DNAemia (limite de detecção 31 UI/mL). A testagem iniciou na primeira semana pós transplante. Pacientes menores de 18 anos foram classificados como pediátricos. Os dados foram analisados usando SPSS 21 e a significância estatística definida foi p < 0.05. Foram usados testes Qui-Quadrado, Mann Whitney e Pearson, considerando as características das variáveis. Kaplan-Meier foi usado para avaliação de incidência. O estudo foi aprovado pelo Comitê de Ética em Pesquisa Institucional. Resultados: Foram realizados 287 TCH alogênico, dos quais 40 (14%) pacientes pediátricos e 247 adultos. As medianas de idade dos pediátricos e adultos foram de 9 (1‒16) e 46 (18‒75) anos, respectivamente. A mediana do tempo de follow-up da coorte foi de 335 dias. As principais indicações para o Alo-TCH nos pediátricos foram LLA (42% × 17%; p < 0,01), anemia aplástica (20% × 4%; p < 0,01) e LMA (17,5 × 41%; p < 0,01) em comparação com os adultos. Em relação ao TCH, o tipo de doador e a fonte entre pediátricos e adultos foram respectivamente: doadores não aparentados (NAP) 45% x 23% e haploidênticos (Haplo) 40% × 34% (p < 0,01), com predomínio da fonte de medula óssea 70% × 39% (p < 0,01). Até o D+180, 19 pediátricos e 94, adultos tiveram reativação por CMV. A incidência de reativação por CMV até o D+100 e até o D+180 ocorreu em 49% x 35% e 52% x 42% (p = 0,12) respectivamente entre pediátricos e adultos. A mediana de dias pós TCH para reativação foi de D+23 × D+35 dias (p = NS), e 58% × 40% das reativações aconteceram até o D+30 (p = NS), entre pediátricos e adultos. As medianas da carga viral inicial e a de pico foram 73 × 162 UI/mL (p < 0,01) e 487 × 4180 UI/mL (p < 0,01), respectivamente. O tempo entre a primeira carga viral positiva e o clareamento foi de 28 × 38 dias (p = NS). Comparando as características de casos de reativação em pediatria versus adultos, houve diferença quanto a doença de base LLA (42% x 18), AA (26% x 6%) e LMA (26% × 40%) (p < 0,05); tipo de doador (NAP e Haplo foram 47% × 28% e 37% × 29%; p = 0,08) e fonte de células utilizada (medula óssea: 77% × 66%; p = 0,05). Conclusão: Comparando as duas populações, observou-se que o TCH pediátrico mais frequentemente foi realizado com doadores alternativos comparando com adultos. Houve uma incidência de reativação de CMV ao redor de 50% até o D+180, porém a diferença na incidência de CMV entre a população pediátrica e a adulta não foi estatisticamente significativa. 50% das reativações em pediatria ocorreram até o D+30. No entanto, a cinética viral indicou que as cargas virais iniciais e de pico foram significativamente menores nos pediátricos, porém sem diferença significativa no clareamento viral. A população pediátrica apresentou alta reativação de CMV, com reativações precoces em uma coorte com alta frequência de doadores alternativos.

22. LINFOMA DE CÉLULAS DO MANTO COM ACOMETIMENTO EXTENSO GÁSTRICO E EM PALATO, SEM ENVOLVIMENTO NODAL AO DIAGNÓSTICO

Introdução: O Linfoma de Células do Manto (LCM), neoplasia linfoide de células B maduras, corresponde a aproximadamente 6% dos Linfomas Não-Hodgkin. Caracteriza-se por um comportamento heterogêneo, variando de casos indolentes a agressivos, com uma sobrevida média de 3 a 5 anos após o diagnóstico. Apresenta-se comumente com sintomas B e acometimento nodal extenso, com ou sem infiltração da medula óssea, e manifestações extranodais podem ocorrer em até 90% dos casos, com trato gastrointestinal em até 60%. Entretanto, o acometimento extranodal em mais de um sítio, sem envolvimento nodal ou da medula óssea é uma apresentação rara da doença. Objetivo: Descrever um caso de LCM com envolvimento do trato gastrointestinal, especificamente palato e estômago, atendido no Hospital Municipal São José de Joinville, com discussão de sua apresentação clínica e evolução do tratamento. Resultados: Homem, 75 anos, ex-tabagista, hipertenso, diabético tipo 2, com histórico de câncer de próstata tratado com radioterapia, encaminhado por quadro de fraqueza, tontura, anemia, sudorese noturna, epigastralgia e perda ponderal de 20 kg em 4 meses, na ausência de linfonodomegalia ou esplenomegalia. Havia realizado endoscopia que revelou mucosa gástrica com aspecto congesto e múltiplas erosões, lesão elevada e ulcerada em corpo gástrico (40mm), em transição de corpo proximal e fundo gástrico (30mm) e alterações endoscópicas sugestivas de aspecto infiltrativo em região bulbar, além de tomografia de abdome com acentuado espessamento parietal do antro gástrico. Apresentava ainda lesão eritematosa em palato, com sangramento discreto na alimentação. Biópsia das lesões gástrica e de palato revelou infiltração por linfócitos pequenos, e perfil imuno-histoquímico CD20+, Bcl2+, CD3-, CD23-, CD5+, Ki-67 em 60%, ciclina D1+ nas duas amostras. H. Pylori foi negativo na imuno-histoquímica. Apresentava MIPIc de alto risco, e iniciou-se o tratamento com R-CVP (Rituximabe, Ciclofosfamida, Vincristina e Prednisona), atualmente em curso, com resolução aparente de lesão em palato e ausência de sangramento ou outros sintomas gástricos. Aguarda o término do tratamento, endoscopia e PET-CT de controle de remissão. Discussão: O LCM trata-se de uma neoplasia hematológica com prognóstico desfavorável. Afeta, principalmente, indivíduos masculinos acima dos 60 anos, e apesar de sua heterogeneidade clínica, a apresentação mucosa, sobretudo em mais de um sítio, é incomum, especialmente na ausência de acometimento nodal. Avaliação criteriosa histológica e imunoistoquímica é essencial no diagnóstico diferencial, com exclusão de linfoma de zona marginal (LZM) extranodal de tecido linfoide associado à mucosa (MALT). A correta caracterização neste caso foi essencial tratamento, prognóstico e seguimento. A positividade de Ciclina D1 possibilitou a determinação de LCM, e em caso de dúvida diagnóstica a expressão imuno-histoquímica de SOX11 ou a presença do rearranjo IGH::CCND1 - t(11;14) por FISH, ambas esperadas no LCM, poderiam complementar os métodos disponíveis no caso. Conclusão: Apesar dos avanços em diagnóstico e tratamento, LCM ainda representa um desafio na prática médica, por sua heterogeneidade e resposta mais limitada aos tratamentos disponíveis, o que reforça a correlação clínica com métodos diagnósticos para seu manejo correto.

Introdução: O monitoramento com PCR quantitativo do gene de fusão BCR-ABL é essencial para a caracterização de resposta ao tratamento com inibidores de tirosina quinase (TKI) em leucemia mieloide crônica (LMC). Enquanto o acesso a TKI em primeira e segunda linha no sistema público é relativamente amplo, o acesso ao PCR é heterogêneo, com diversos centros de referência apresentando monitoramento insuficiente frente às recomendações internacionais. Objetivo: Analisar o acesso ao PCR para monitoramento de tratamento com TKI e sua adequação em relação à diretriz de monitoramento da European LeukemiaNet (ELN) de 2023 no primeiro ano de (3, 6 e 12 meses), bem como o impacto do acesso em troca de tratamento e sobrevida em LMC. Metodologia: Análise retrospectiva de vida real com pacientes com diagnóstico de LMC em fase crônica, com seguimento ativo entre 2020 e 2024 no Hospital Municipal São José (Joinville-SC). Resultados: Foram incluídos 115 pacientes, 50,4% sexo feminino, com mediana de idade ao diagnóstico de 50 anos (intervalo: 16-77). Ao diagnóstico, 1% possuíam Hasford e 6% Sokal de alto risco. Foram realizados 917 PCR, com média anual de 1,15 testes por paciente, e média de 1,46 testes no 1ºano de tratamento. Todos os pacientes possuíam ao menos um PCR. No 1ºano de tratamento, 22 pacientes realizaram ≥ 3 PCR (18 com Resposta Molecular Maior, RMM, 82%), e 93 pacientes realizaram ≤ 2 PCR no 1ºano (50 com RMM, 54%). Todos os 115 pacientes iniciaram o tratamento com imatinibe (IM), e 67 (58%) obtiveram RMM em algum momento da terapia. O monitoramento de BCR-ABL ocorreu de acordo com as recomendações da ELN 2023 em 22 pacientes (19%). Sessenta pacientes (52%) receberam TKI de segunda linha (41 com Nilotinibe e 19 com Dasatinibe), com mediana de 31 meses após o diagnóstico. RMM foi atingida em segunda linha em 25 pacientes (61%) com Nilotinibe e 10 pacientes com Dasatinibe (53%). TKI de terceira linha e quarta linha foram utilizados em 7 e 1 pacientes, respectivamente, e TCTH alogênico em 3 pacientes. Com mediana de seguimento de 96 meses, foram observados 3 óbitos, com probabilidade de sobrevida global em 5 anos de 97%, e sobrevida livre de eventos (SLE) de 72%. Discussão: O estudo evidencia as limitações de acompanhamento via PCR quantitativos no SUS em relação às diretrizes da ELN, com a média de testes inferior aos preconizados no primeiro ano de TKI, influenciando na avaliação da eficácia do tratamento e na possibilidade de detecção precoce de recaída ou refratariedade. Pacientes com ≥ 3 testes no 1ºano de tratamento apresentaram maior taxa de RMM em relação àqueles com ≤ 2 PCRs, pela possibilidade de detecção precoce de falhas terapêuticas. Em nosso serviço, a maioria dos pacientes depende de vouchers fornecidos pelos laboratórios para o monitoramento, com número limitado de testes custeados pela instituição. São limitações do estudo sua natureza retrospectiva, com possível viés de sobrevida considerando o período de inclusão de pacientes e a mediana de seguimento, bem como dependência de revisão de prontuário médico para levantamento de dados. Conclusão: A adesão e o monitoramento do tratamento com TKI são centrais ao controle da LMC. Os dados apresentados evidenciam que uma testagem adequada poderia aprimorar os desfechos em nossa população dentro do SUS.

24. IMPACTO DOS FATORES SOCIOECONÔMICOS NA DOAÇÃO DE SANGUE NO BRASIL: UMA COMPARAÇÃO COM O CENÁRIO MUNDIAL

Introdução: Segundo o Ministério da Saúde, cerca de 1,4% da população brasileira doa sangue, adequando-se à recomendação da Organização Mundial de Saúde (OMS) de 1-3% da população de cada país ser doadora. Porém, observam-se disparidades na captação de sangue no Brasil, deixando hemocentros em alerta de estoque. Para ampliar o recrutamento de potenciais doadores, é imprescindível compreender as barreiras e fatores socioeconômicos envolvidos, além de avaliar condições determinantes no processo de doação. Objetivo: Analisar os principais fatores socioeconômicos envolvidos no processo de doação de sangue e o perfil predominante dos doadores e não doadores. Materiais e métodos: Revisão integrativa de literatura a partir dos descritores em saúde (Socioeconomic Factors, blood donation, Blood donors) nos últimos 10 anos, nas bases de dados PubMed, Scielo e LILACS. Resultados: No período, encontramos 108 artigos publicados. Destes, 102 foram excluídos por não se enquadrarem nos critérios de elegibilidade. Seis foram incluídos nesta revisão, avaliando fatores socioeconômicos da doação de sangue no Brasil (n = 4), nos Estados Unidos (n = 1) e na União Europeia (n = 1) por meio de inquérito populacional e análises secundárias de resultados obtidos. No Brasil, a prevalência de doadores constitui-se por homens, brancos, de meia-idade, com nível educacional e socioeconômico médio-elevado. Condições médicas, falta de tempo e de informação mostraram-se os maiores empecilhos para a doação. Nos Estados Unidos, a idade, etnia e localização geográfica foram os fatores mais influentes na doação, sendo os indivíduos jovens, brancos, nativos e moradores de locais, com maior acesso a centros de coleta. Indivíduos do sexo masculino, graduados e praticantes de atividade física também se mostraram parte deste grupo. Na Europa, evidenciou-se o predomínio do ato entre indivíduos homens (apesar da diminuição da disparidade de gênero ao longo dos anos), casados, de meia-idade, empregados e de maior nível de escolaridade, além do aumento no número de doadores provenientes de zonas rurais. Discussão: Homens, de maneira geral, doam sangue mais frequentemente do que mulheres, o que pode ser atribuído às diretrizes de doação que garantem segurança do doador, como peso mínimo, gravidez e níveis de hemoglobina, que diminuem a chance de novas doações pelas mulheres. Maiores índices de escolaridade se traduzem em melhores taxas de doação, devido à disseminação de conhecimento e conscientização eficaz sobre a doação de sangue. Já menores condições financeiras, implicam em doações menos frequentes. No Brasil, negros doam menos, o que reflete o cenário do país, onde a etnia negra possui menor status social e econômico. Por mais que a taxa de doação de indivíduos de meia idade seja a maior, observa-se constante aumento no número de doadores de sangue mais jovens. Os doadores tendem a ser empregados, residentes de regiões urbanas, além de casados e possuírem boa autopercepção de saúde. Conclusão: Os resultados revelam importantes fatores a serem considerados para o aumento da doação de sangue no Brasil. A realização de campanhas direcionadas aos potenciais doadores, juntamente à políticas públicas, podem oportunizar a diversificação deste perfil, incluindo mulheres, jovens e estudantes, além de diminuir as disparidades envolvidas na doação de sangue.

El desarrollo del pensamiento científico-técnico constituye una exigencia de la formación de los profesionales universitarios de especialidades de las ciencias técnicas. El objetivo de este artículo es exponer las manifestaciones peculiares de las características del pensamiento científico-técnico que deben formarse en estudiantes de Ingeniería Hidráulica de acuerdo a los estilos de aprendizaje activo, reflexivo, teórico y pragmático. Se utilizaron los métodos de la sistematización de la teoría y la práctica, el inductivo-deductivo, el hipotético-deductivo, el test de estilo de aprendizaje de Honey-Alonso y técnicas experimentales basadas en la solución de problemas. La utilización de técnicas de correlación permitió establecer relaciones entre los estilos de aprendizaje y las características del pensamiento científico-técnico en la muestra de sujetos estudiada.

Atomistic Molecular Dynamics (MD) simulations probe structural and dynamical features related to system functionality. However, while simulations in homogenous aqueous environments are routine, dehydrated model assemblies are a challenge with systems investigated in-silico needing careful consideration; simulated systems potentially differing markedly despite seemingly negligible changes in procedure. Here we propose an in-silico protocol to model proteins in lyophilised and weakly hydrated amorphous states that is both more experimentally representative and routinely applicable. Since the outputs from MD align directly with those accessed by neutron scattering, the efficacy of the simulation protocol proposed is shown by validating against experimental neutron data for apoferritin and insulin. This work also highlights that without cooperative experimental and simulative data, development of simulative procedures using MD alone would prove most challenging.

For several years, the European Commission has been investing in transnational mobility projects, namely Erasmus+, for the continuing professional development of teaching and non-teaching staff of schools for the sole purpose of improving the quality of education, as well as to establish networks of communications where teachers and schools can share their best practices and experiences. Although necessarily nestled in a proactive and positive school culture, these projects also have an impact on teachers professionally, but also on a personal level. This article therefore aims at studying how these international teacher professional development experiences change teachers’ biographies and their narrative expressions and possibly transform how they identify themselves personally, as European citizens, and professionally as European teachers. Empirical data collected through a multiple case study in Portugal appears to confirm that a considerable shift in professional and personal identity occurs through the participation of Erasmus+ KA1 projects for teacher professional development.

Cardiovascular diseases are the leading cause of death globally. The objective of this study was to estimate the 10-year cardiovascular risk in the Portuguese population using the new Systematic Coronary Risk Evaluation 2. Data from the first National Health Examination Survey from 2015 were used. Inclusion criteria were age between 40 and 69 years, absence of pregnancy, available information on sex, age, smoking status, systolic blood pressure, total cholesterol, and high-density lipoprotein cholesterol. Participants who had an acute myocardial infarction or a stroke, had diabetes, chronic kidney disease, or reported taking medication for these conditions were excluded from the analysis. The prevalence of high and very high cardiovascular risk was stratified by sex, age group, marital status, education level, occupational activity, degree of urbanization of the area of residence, health region, and income quintile. The sample consisted of 2817 individuals. In Portugal, in 2015, 36.7% (95% CI: 34.2 - 39.3) and 6.1% (95% CI: 4.8 - 7.4) of the individuals aged between 40 and 69 years had a high and a very high risk of having a cardiovascular disease in the following 10 years, respectively. In 2015, there was a high percentage (42.8%) of the Portuguese population aged 40 to 69 years in high or very high risk of developing cardiovascular disease (fatal and non-fatal) in the following 10 years. A possible explanation may be the high prevalence of risk factors for cardiovascular disease in Portugal.

Introduction: Exposure to ambient air pollution may play a role in the onset of common mental disorders like depressive and anxiety disorders. The association of long-term exposure to particles smaller than 10 μm (PM10) with these diseases remains unclear. This study aimed to estimate the association of long-term exposure to PM10 with mental well-being and the frequency of probable diagnosis of common mental disorders. Methods: A nationally representative cross-sectional study was done in mainland Portugal. Long-term exposure was estimated through one-year average concentrations of PM10, calculated with data from the Portuguese Environment Agency, attributed individually considering individuals’ postal codes of residence. The mental well-being and the probable diagnosis of common mental disorders were ascertained through the five-item Mental Health Inventory scale. Linear and Robust Poisson regression models were computed to estimate change percentages, prevalence ratios (PR), and their 95% confidence intervals (95% CI). Results: The median (interquartile range) concentration of PM10 was 18.6 (15.3 - 19.3) μg/m3. The mental well-being score was 72 (56 - 84) points, on a scale from 0 to 100. A probable diagnosis of common mental disorders was found in 22.7% (95% CI: 20.0 to 25.6). Long-term exposure to PM10 was associated with a non-statistically significant decrease in the mental well-being score [for each 10 μg/m³ increment in one-year average PM10 concentrations, there was a 2% (95% CI: -8 to 4) decrease], and with a non-statistically significant increase in the common mental health frequency (PR = 1.012, 95% CI: 0.979 to 1.045). Conclusion: We did not find statistically significant associations between long-term exposure to PM10 and mental well-being or the frequency of probable diagnosis of common mental disorders. These results may be explained by the reduced variability in the exposure values, given the geographical distribution and functioning of the network of air quality monitoring stations. This study contributes with evidence for low levels of air pollutants, being one of the first to adjust for individual and aggregate-level variables. Moreover, to the best of our knowledge, this was the first nationally representative, population-based study conducted on the Portuguese population using real-life data. Maintaining a robust and nationwide air quality monitoring network is essential for obtaining quality exposure data.

Introduction and objectives: Increasing evidence shows that newborn weight loss is significantly influenced by the type of labour among other factors, implying a wider understanding of weight loss and its context to intervene accordingly. Given the importance of breastfeeding and the impact of infant formula use on its success, we aimed to understand the impact of the application of nomogram weight loss tools in initiating infant formula. Methods: This study included 206 newborns in a nonrandomised clinical trial, who were divided into two groups. In the intervention group, weight loss was managed through the use of the “Early Weight Loss Nomogram”, rather than with clinical experience associated with weight loss percentage, the standard type of care. We analysed the characteristics of mothers, newborns, and labours and their influence on each group. Results: We found that the introduction of formula milk due to weight loss tended to diminish (61.3% vs. 38.7%, p = 0.082) and nurses’ suggestions dropped to zero in the intervention group. The intervention group reported a longer period of exclusive breastfeeding, up to nearly six months of age. The babies who were not breastfed during the first hour of life were more often supplemented (78.6% vs. 21.4%, p < 0.001) and those with skin-on-skin contact were given a lower proportion of formula milk (69.8% vs. 30.2%, p < 0.001). Discussion: Application of the nomogram allowed the introduction of formula milk in more selected cases. This tool should be applied in healthcare units because it increases health professionals’ awareness of the criteria for formula supplementation.

Neste artigo, apresenta-se uma pesquisa que explora possíveis efeitos da formação pedagógica de professores do ensino superior, tendo em conta os desafios que marcam hoje a docência neste nível de ensino. Tais desafios decorrem quer da expansão e diversificação da população estudantil quer das mudanças curriculares e pedagógicas – nomeadamente, aquelas associadas ao Processo de Bolonha. Com base na análise de um conjunto de portefólios produzidos por 14 professores portugueses no quadro de um curso de pós-graduação em pedagogia do ensino superior, consideram-se as suas motivações para a formação, a relação que estabelecem com a docência e as reverberações da formação no respetivo exercício da profissão académica, bem como a (re)configuração dos seus entendimentos sobre finalidades e conteúdos da formação académica. Conclui-se evidenciando possíveis contributos da formação pedagógica na valorização do trabalho docente e no reforço da autonomia no exercício da docência, em paralelo com a centralidade das respetivas dimensões coletivas. Estes elementos, no seu conjunto, abrem possibilidades para equacionar os efeitos da formação pedagógica no reequilíbrio da tensão entre investigação e ensino e na afirmação da profissão académica como um bem público.

Category theory plays a special character in mathematics - it unifies distinct branches under the same formalism. Despite this integrative power in math, it also seems to provide the proper foundations to the experimental physicist. In this work, we present another application of category in physics, related to the principle of relativity. The operational construction of (inertial) frames of reference indicates that only the movement between one and another frame is enough to differentiate both of them. This fact is hidden when one applies only group theory to connect frames. In fact, rotations and translations only change coordinates, keeping the frame inert. The change of frames is only attainable by boosts in the classical and relativistic regimes for both Galileo and Lorentz (Poincar\'e) groups. Recently, random lasing in complex networks has shown efficient lasing over more than 50 localised modes, promoted by multiple scattering over the underlying graph. If controlled, these network lasers can lead to fast-switching multifunctional light sources with synthesised spectrum. Here, we observe both in experiment and theory high sensitivity of the network laser to the spatial shape of the pump profile, with mode intensity variation of up to 280% for a non-homogeneous 7% pump decrease. We solve the nonlinear equations within the steady state ab-initio laser theory (SALT) approximation over a graph and we show selective lasing of around 90% of the top modes, effectively programming the spectrum of the lasing networks. In our experiments with polymer networks, this high sensitivity enables control of the lasing spectrum through non-uniform pump patterns. We propose the underlying complexity of the network modes as the key element behind efficient spectral control opening the way for the development of optical devices with wide impact for on-chip photonics for communication, sensing and computation., Comment: 22 pages, 14 figures

Nanodroplets have demonstrated potential for the range detection of hadron radiotherapies. Our formulation uses superheated perfluorobutane (C4F10) stabilized by a poly(vinyl-alcohol) shell. High-LET (linear energy transfer) particles vaporize the nanodroplets into echogenic microbubbles. Tailored ultrasound imaging translates the generated echo-contrast into a dose distribution map, enabling beam range retrieval. This work evaluates the response of size-sorted nanodroplets to carbon-ion radiation. We studied how thesize of nanodroplets affects their sensitivity at various beam-doses and energies, as a function of concentration and shell cross-linking. First, we show the physicochemical characterization of size-isolated nanodroplets by differential centrifugation. Then, we report on the irradiations of the nanodroplet samples in tissue-mimicking phantoms. We compared the response of large (≈900 nm) and small (≈400 nm) nanodroplets to different carbon-ions energies and evaluated their dose linearity and concentration detection thresholds by ultrasound imaging. Additionally, we verified the beam range detection accuracy for the nanodroplets samples. All nanodroplets exhibited sensitivity to carbon-ions with high range verification precision. However, smaller nanodroplets required a higher concentration sensitivity threshold. The vaporization yield depends on the carbon-ions energy and dose, which are both related to particle count/spot. These findings confirm the potential of nanodroplets for range detection, with performance depending on nanodroplets’ properties and beam parameters.

Purpose: Higher education institutions (HEIs) must take on a leadership role in building a sustainable world, given their responsibility for preparing future professionals and leaders worldwide and considering the role they provide to society. To accomplish this goal, HEIs need to holistically embody sustainability in everything they develop. This study aims to help HEIs in this purpose by developing a method to integrate sustainability into the strategic planning process in these institutions. Design/methodology/approach: In the first stage, the method was developed based on papers selected through a systematic literature review. The proposed method was then applied in a Brazilian HEI to validate and adjust it. Findings: A method that adopts a participatory process to integrate sustainability into HEIs' strategic planning was proposed. Practical implications: This study provides university leaders with a simple and practical method to aid with elaborating on strategic plans for holistic sustainability integration. Originality/value: This study uniquely applied a framework called "HEIs sustainability action archetypes" as the foundation for selecting sustainable objectives, goals and actions to be integrated into these institutions' strategic planning.

This paper is devoted to discussing the topological structure of the arrow of time. In the literature, it is often accepted that its algebraic and topological structures are that of a one-dimensional Euclidean space $\mathbb{E}^1$, although a critical review on the subject is not easy to be found. Hence, leveraging on an operational approach, we collect evidences to identify it structurally as a normed vector space $(\mathbb{Q}, |\cdot|)$, and take a leap of abstraction to complete it, up to isometries, to the real line. During the development of the paper, the space-time is recognized as a fibration, with the fibers being the sets of simultaneous events. The corresponding topology is also exposed: open sets naturally arise within our construction, showing that the classical space-time is non-Hausdorff. Os temas foram divididos em seis capítulos (Capítulo 1-Prevenção de úlceras nos pés de pessoas com diabetes; Capítulo 2-Alívio da pressão de úlceras nos pés de pessoas com diabetes; Capítulo 3-Classificações das úlceras do pé diabético; Capítulo 4-Pé diabético e a doença arterial periférica; Capítulo 5-Infecção e o pé diabético; Capítulo 6-Neuroartropatia de Charcot). A versão atual das Diretrizes sobre pé diabético apresenta importantes recomendações para prevenção, diagnóstico, tratamento e seguimento dos pacientes com pé diabético, oferecendo um guia objetivo para prática médica.

Congenital absence of the pericardium (CAP) is an unusual condition discovered, in most cases, incidentally but can potentially lead to fatal complications, including severe arrhythmias and sudden death. Recently, the use of modern imaging technologies has increased the diagnosis of CAP, providing important findings for risk stratification. Nevertheless, there is not yet consensus regarding therapeutic decisions, and the management of patients with CAP remains challenging. In this paper, we discuss the pathophysiological implication of CAP, review the current literature and explain the role of multimodality imaging tools for its diagnosis, management and treatment.

Treatment for pregnant women with psoriasis is limited by the lack of information typically related to clinical trials. While anti-tumor necrosis factor (TNF) drugs offer therapeutic benefits, their safety during pregnancy is a concern. Notably, certolizumab is comparatively safer than adalimumab, etanercept, infliximab, and golimumab according to the current recommendations. Thus, this study aimed to conduct a pharmacovigilance comparative analysis of maternal and neonatal outcomes associated with certolizumab versus other anti-TNF drugs by using data from EudraVigilance. A descriptive analysis was performed of Individual Case Safety Reports (ICSRs) associated with an anti-TNF drug and related to the pregnant patients with psoriasis from 2009 and 2023, focusing our analysis on the specific pregnancy outcomes and fetal/neonatal disorders. The most common pregnancy-related adverse event was spontaneous abortion, predominantly related to adalimumab and certolizumab. Certolizumab was also reported in cases of caesarean section, gestational diabetes, abortion, fetal death, fetal distress syndrome, pre-eclampsia, and premature separation of placenta. Generally, the findings from our study depicted a safety profile that overlapped for each anti-TNF drug, both in maternal/neonatal outcomes and other adverse events, suggesting no substantial differences between treatments. We advocate for further investigations before making concrete recommendations.

Lasers based on biological materials are attracting an increasing interest in view of their use in integrated and transient photonics. DNA as optical biopolymer in combination with highly-emissive dyes has been reported to have excellent potential in this respect, however achieving miniaturized lasing systems based on solid-state DNA shaped in different geometries to confine and enhance emission is still a challenge, and physico-chemical mechanisms originating fluorescence enhancement are not fully understood. Herein, a class of wavelength-tunable lasers based on DNA nanofibers is demonstrated, for which optical properties are highly controlled through the system morphology. A synergistic effect is highlighted at the basis of lasing action. Through a quantum chemical investigation, we show that the interaction of DNA with the encapsulated dye leads to hindered twisting and suppressed channels for the non-radiative decay. This is combined with effective waveguiding, optical gain, and tailored mode confinement to promote morphologically-controlled lasing in DNA-based nanofibers. This research aimed to analyze whether the innovative factors, financial and Corporate Governance criteria of publicly traded companies are related to the ability to acquire resources via Private Equity funds. Data from Brazilian companies from 2014 to 2018 were collected. A logistic regression with panel data was performed, using the Probit and Logit models. The observed result shows the positive influence of the innovation criteria: spending on R&D, environmental and social investments; financial criteria: ROE and size of investee companies and governance criteria: management and shareholder protection. The results are in line with the findings of the international literature and contribute by offering an objective, empirical analysis of the behavior of attractiveness variables in the environment of companies operating in countries of emerging economies.

50. Auto-extração ou plano duro no resgate da vítima de trauma no extra-hospitalar: scoping review

Introdução: Atualmente, as lesões traumáticas foram consideradas uma das dez principais causas morte no mundo constituindo um problema de saúde pública. Os acidentes de viação constituem 3,5% das mortes registadas em Portugal. As incidências destes sinistros potenciaram a necessidade de elaboração de medidas de prevenção e abordagem. Nesse sentido, a metodologia “ABCDE” e as técnicas de extração, como: autoextração e extração com recurso ao plano duro são hipóteses a considerar nestes cenários. Objetivo: Mapear e analisar a evidência disponível acerca da utilização do plano duro e da autoextração enquanto técnicas de extração no extra-hospitalar. Material e métodos: Revisão scoping segundo o protocolo do instituto Joanna Briggs, com frase de pesquisa "[(Trauma) OR (multiple trauma) OR (Spinal Cord Injuries) OR (Trauma, Nervous System)] AND [(Emergency Treatment) OR (Advanced Trauma Life Support Care) OR (First Aid) OR (Transportation of Patients) OR (Emergency Nursing) OR (Emergency Medical Services) OR (Emergency Medicine) OR (Emergency) OR (Emergency Responders) OR (Paramedics)] AND [(Immobilization) OR (Backboard) OR (Stability) OR (Board)]”. Realizada nas bases de dados MEDLINE, CINAHL e Cochrane Database of Sysematic Reviews com critérios de inclusão os artigos compreendidos entre 2018 e 2023 em Português, Espanhol e Inglês. Após a exclusão dos duplicados e da leitura do título, resumo e texto integral, a pesquisa resultou para análise 5 artigos. Resultados: Foram obtidos 865 artigos, tendo sido integrados 5 na revisão. A elevada incidência de acidentes de viação que requerem assistência desencadeou investigações, que denotam um agravamento das lesões da coluna cervical durante a abordagem e o transporte, afirmam que a taxa de mortalidade é superior nas vítimas imobilizadas por fatores, tais como, o tempo despendido no local. A autoextração, assim como, as extrações com o plano duro são aplicáveis, desde que obedeçam a critérios como: estado de consciência da vítima, cinemática do sinistro, estado clínico e sintomatologia. Conclusão: Constatou-se que ambas abordagens têm aplicabilidade, contudo, tendo em conta a inexistência de consenso e pouca evidência científica disponibilizada, devem-se desenvolver novas investigações, como: estudos longitudinais e experimentais para serem construídos protocolos de abordagem às vítimas de trauma, com vista à homogeneização cuidados nos diferentes contextos no extra-hospitalar.
