Mineral and bone disorder biomarkers ‘normal ranges’ are controversial. The aim of the study was to evaluate the association between serum calcium (Ca), phosphate (P), intact parathyroid hormone (iPTH), and 25(OH) vitamin D levels and mortality risk, in a chronic kidney disease (CKD) grade (G) 3b-4 cohort. The Uruguayan National Renal Healthcare Program (NRHP-UY) CKD patients’ cohort, included between 1 October 2004 and 1 March 2020 and followed-up until 1 March 2021, was analyzed with the Ethics Committee approval. A total of 6473 patients were analyzed: 56% men, median age 73 (65–79) years, 55% on CKD G3b. At the end of the follow-up, 2459 (37.7%) patients had died (6.4/100 patient–year). There were iPTH data on 2013 patients (younger, with lower estimated glomerular filtration rate (eGFR) and lesser comorbidities). By bivariate Cox analysis the lowest death risk was observed with mean Ca between 9.01 and 10.25 mg/dl, P between 2.76 and 4.0 mg/dl, iPTH ≤ 105 pg/ml, and 25(OH) vitamin D >10 ng/ml. The multivariate Cox regression mortality risk adjusted to age, sex, CKD etiology, diabetes, smoking, cardiovascular comorbidity, blood pressure, proteinuria, eGFR, renin-angiotensin system blockers and vitamin D treatments, serum Ca, P, iPTH, and 25(OH) vitamin D (n = 964) showed that a higher mortality risk was associated with p > 4.00 mg/dl (HR 1.668, CI 95%: 1.201–2.317), iPTH >105 pg/ml (HR 1.386, CI 95%: 1.012–1.989), and 25(OH) vitamin D ≤ 10 ng/ml (HR 1.958, CI 95%: 1.238–3.098) and a lower mortality risk with 1,25(OH)2 vitamin D treatment (HR 0.639, CI 95%: 0.451–0.906). These data may contribute to the precise G3b-4 CKD-MBD biomarkers levels definition.

Los pacientes con enfermedad renal crónica avanzada (ERCA) presentan menor capacidad funcional. El deterioro funcional se acentúa en los estadios más avanzados de la Enfermedad Renal Crónica (ERC) lo que aumenta la morbi-mortalidad. El objetivo del presente trabajo fue valorar el estado funcional de los pacientes con ERCA en tratamiento con diálisis en el Centro de Nefrología del Hospital de Clínicas, en el período comprendido de mayo a agosto del 2018. Es un estudio transversal, observacional, descriptivo. Los datos fueron extraídos por revisión de historia y entrevista presencial. Se evaluó el estado funcional y actividad física mediante: fuerza de prensión, test “up and go”; índice de Barthel. La presencia de deterioro cognitivo se determinó a través del “minimental state test examination”, y la calidad de vida percibida por el paciente, por medio del cuestionario SF-36 adaptado ERC. La población estudiada fue n= 47, edad media 58.5 ± 15.3 (DE) años. Modalidad hemodiálisis (29/47: 61,7%) siendo el resto pacientes en diálisis peritoneal (18/47: 38,3%). La mayoría (26) tuvieron fuerza conservada (prensión) y velocidad de marcha independiente (37) (rápidos por up and go). La presencia de menor capacidad funcional (debilidad o lentitud) se relacionó con menor masa magra, menores niveles de albúmina (3.9 ±0.52 vs 4.2 ±0.27 p 0.023), hemoglobina(10.7± 1.7 vs 11.7 ± 1.7 p 0.045), vitamina D ( 22.7 ± 14.5 vs 22.3 ± 8.1 p 0.917 ) y mayores niveles de hormona paratiroidea (308.1 ± 316.9 vs 378.4 ± 264.5 p 0.422). Concluimos que el Deterioro del estatus funcional en pacientes con ERC en terapia de sustitución renal (TSR) se relaciona con la disminución en la masa magra, menor nivel de albúmina, anemia y déficit de vitamina D. The presence of cognitive impairment was determined through the “mini mental state test examination”, and the quality of life perceived by the patient, through the adapted SF-36 ERC questionnaire. The studied population was n = 47, mean age 58.5 ± 15.3 (SD) years. Hemodialysis Group (29/47: 61.7%) and peritoneal dialysis Group (18/47: 38.3%). Most (26) had conserved strength (grip) and independent gait speed (37) (up and go fast). The presence of lower functional capacity (weakness or slowness) was related to lower lean mass, lower levels of albumin (3.9 ± 0.52 vs 4.2 ± 0.27 p 0.023), hemoglobin (10.7 ± 1.7 vs 11.7 ± 1.7 p 0.045), vitamin D ( 22.7 ± 14.5 vs 22.3 ± 8.1 p 0.917) and higher levels of parathyroid hormone (308.1 ± 316.9 vs 378.4 ± 264.5 p 0.422). We conclude that the Impairment of functional status in CKD patients on renal replacement therapy (RRT) is related to a decrease in lean mass, lower albumin level, anemia, and vitamin D deficiency.

Se analizó retrospectivamente el Registro del Programa de Salud Renal, para relacionar hiper-tensión arterial (Presión arterial promedio de la evolución ≥ 140/90 mm Hg) con la evolución de los pacientes asistidos en el período de estudio (1/7/2011 a 30/6/2016).B. Se planificó, ejecutó y evaluó una intervención educativa a pacientes con Enfermedad Renal Crónica estadios IV-V, respecto al control de presión arterial, consumo de sodio, y ejercicio físico. Resultados:A - En la población global, 374 individuos con tiempo de seguimiento > 3 meses, se encontraron 123 hipertensos y su supervivencia fue significativamente menor que el grupo normotenso. En la subpoblación en etapa IV-V (59 pacientes) la diferencia no fue significativa. El análisis estadístico de las Evaluaciones previas y posteriores a la intervención educativa evidenció una mejoría en la identificación del contenido de sodio del alimento seleccionado (test McNemar, p< 0.05) y la Encuesta de Satisfacción evidenció alta aceptación. En conclusión, la hipertensión arterial se asoció a mayor riesgo de ingreso a diálisis o fallecimiento y la intervención educativa fue exitosa, lo que alienta a difundir la experiencia, para mejorar el auto-cuidado y la evolución de los pacientes. The aims of the study were to evaluate the relationship between blood pressure and outcome in Chronic Kidney Disease (CKD) patients assisted at the Departamento de Nefrología del Hospital de Clínicas and perform an educational intervention to improve adherence to treatment.The study was conducted in two stages: A. A retrospective analysis (data from the Renal Health Program Registry), was performed to correlate blood pressure with outcome of patients assisted in the study period (1/7/2015-30/6/2016).B. An educational activity to CKD patients (stage IV-V) was planned, implemented and evaluated with the aim of improving the Blood Pressure control, sodium intake, and physical exercise. Results. Objetivo: evaluar la frecuencia de acidosis metabólica en la enfermedad renal crónica (ERC) y su impacto en la evolución. Material y método: se realizó un estudio retrospectivo de pacientes del Programa de Salud Renal del Uruguay (octubre de 2004 a octubre de 2011) con dos controles separados por seis o más meses y al menos un dato de bicarbonatemia. Se analizaron: creatininemia, proteinuria, bicarbonatemia venosa y tratamiento alcalinizante. Se consideró evento final el ingreso a diálisis o trasplante y/o fallecimiento. La enfermedad renal crónica (ERC) tiene elevada prevalencia y morbimortalidad cardiovascular (CV). Objetivos: analizar la prevalencia de factores de riesgo y tratamientos en la población del Programa de Salud Renal del Uruguay (PSRU) y su asociación con eventos CV y supervivencia. Método: estudio retrospectivo de la cohorte del registro del PSRU, que representa 58% de la población del país, entre 29/9/2006 y 31/12/2014. Los criterios de inclusión son: personas ³ 20 años, con alteraciones renales por más de tres meses, filtrado glomerular estimado (FGe) < 60 ml/min/1,73 m² o proteinuria >300 mg/día o albuminuria >30 mg/día en diabéticos, con seis o más meses en control. Se incluye registro de nuevos eventos cardiovasculares (NECV), ingreso a tratamiento de sustitución renal (TSR) y fallecimientos. Resultados: se incluyeron 8.407 individuos, edad 68 ± 14 años, 56% hombres, 66,6% > 65 años. Se observó elevada prevalencia de factores de riesgo CV. Se reportaron 2.245 NECV no fatales en 1.439 individuos (18,9%), tasa 10,1 por 100 pacientes-año (pac-año). Fallecieron 1.380 pacientes, 32,7% de causa CV. La tasa de ingreso a tratamiento de sustitución renal (TSR) fue de 1,94, la de mortalidad global de 6,2 y de causa CV de 2,03 por 100 pac-año. Se demostró la asociación de NECV y mortalidad con factores de riesgo tradicionales y vinculados a ERC, así como disminución con control glucídico y tratamiento con inhibidores de la enzima convertidora de angiotensina (IECA). Conclusiones: la población con ERC presenta múltiples factores de riesgo CV con elevada morbimortalidad, lo que amerita detección precoz y tratamiento. Objectives: the aim of the study was to analyze risk factors and treatments in the Uruguayan National Renal Healthcare Program (NRHP) and their association with cardiovascular events (CVE) and survival. Methods: this is a cohort study of patients included in the Uruguayan NRHP Registry, from 29/9/2006 to 31/12/2014. The inclusion criteria were age ³ 20 years, kidney disease for more than 3 months, estimated glomerular filtration rate (eFG) < 60 ml/min/1.73 m² and/or proteinuria >300 mg/day or albuminuria >30 mg/day in diabetics, with ³ 6 months under surveillance. The end-points were a new CV event, renal replacement therapy (RRT) or death. Results: 8.407 patients were included, mean age 68 ± 14 years, 56% males, 66,6% >65 years. Cardiovascular risk factors were highly prevalent. 2.245 new CVE were registered in 1.439 (18.9%) patients who survived (10.1/100 patient-year (pt-yr). 1.380 patients died, 32.7% of them from a CVE. RRT rate was 1.94 / 100 pt-yr, global mortality rate 6.2 and CV mortality rate 2.03 / 100 pt-yr. Introducción: El Programa Nacional de Salud Renal (PNSR) mostró que la enfermedad renal crónica (ERC) puede estabilizarse en la evolución. Objetivo: evaluar el tamizaje de Enfermedad Renal Crónica presuntiva (ERCp) en población ambulatoria de una Clínica Preventiva con tirilla reactiva para proteinuria (TPu) y determinación de creatinina. Método. Estudio observacional, descriptivo, de corte transversal, entre 1/1/2008 y 31/12/2012 en 83.912 personas que se realizaron Carné de Salud (edad media =34.4años). Se consideró proteinuria positiva (Pu+) si TPu ≥1+ o ≥ 0.3 g/l. Se realizó TPu a todos y dosificación de creatinina para estimación Filtrado Glomerular (TFGe) en subpoblación con factores de riesgo (FR) como hipertensión o diabetes. Se evaluó proteinuria según edad y presencia o no de FR. En 11.161 individuos con determinación de creatinina se estimó el TFGe por fórmula CKD-EPI, (edad media=44.7años) se estimó prevalencia de ERCp mediante Pu+ TFGe90% en 70 años, donde cobra importancia la TFGe: 21.9% en con FR. Conclusiones: La población del Carné de Salud es útil para el tamizaje de ERCp temprana. La nefropatía IgA es frecuente en Uruguay y su tratamiento conflictivo. Objetivo: analizar la incidencia, presentación clínica, histología, tratamiento y evolución de la nefropatía IgA en Uruguay. Material y método: se realizó un análisis retrospectivo de datos del Registro Uruguayo de Glomerulopatías (RUG) e historias clínicas con estricta confidencialidad; se registró edad, sexo, fecha de biopsia renal, presentación clínica, presión arterial (PA), proteinuria, hematuria y creatininemia (al inicio y en última evolución registrada), histología, tratamientos y evolución. Análisis estadístico uni y multivariado. Aprobado por el Comité de Ética del Hospital de Clínicas. Resultados: se estudiaron 341 pacientes con nefropatía IgA, confirmada por histología, en el período comprendido entre el 1º de enero de 1985 y el 31 de diciembre de 2009; 65% hombres, edad media 31 ± 13 años y mediana de seguimiento de 52 meses (1-271); 14% ingresaron a diálisis y 1,2% fallecieron. La presentación clínica más frecuente fue con alteraciones urinarias asintomáticas (AUA) (42%), (mayor en el período 2000-2009, chi2 p < 0,05). Al inicio, la proteinuria era 1,7 ± 1,9 g/l; creatininemia 1,6 ± 1,8 mg/dl, y 32% tenían PA ≥ 140/90 mmHg. Se observaron semilunas en 48%. Recibieron inhibidores de la enzima convertidora de angiotensina/antagonistas de los receptores de la angiotensina II (IECA/ARAII) 56% e inmunosupresores, 52%. La PA disminuyó significativamente en la evolución. En el análisis univariado, la creatininemia inicial fue ≥ 2,5 mg/dl, proteinuria, semilunas y proliferación endocapilar se asociaron a ingreso a diálisis o fallecimiento, pero en el análisis multivariado (regresión de Cox) solo fue significativo el nivel de creatininemia. Conclusiones: la presentación clínica de nefropatía IgA ha cambiado en la última década, siendo más frecuente las AUA. La creatininemia ≥ 2,5 mg/dl se asocia a peor supervivencia renal, probable “punto de no retorno”. Abstract IgA nephropathy is a frequent condition in Uruguay and treating it is controversial. Objective: to analyse incidence, clinical presentation, histology, treatment and evolution of nephropathy in Uruguay. Method: we conducted a retrospective study of data kept at the Uruguayan Registry of Glomerulopathies and medical records under strict confidentiality. Age, sex, date of the kidney biopsy, clinical presentation, blood pressure, proteinuria, hematuria and creatininemia (initial and in the last evolution recorded), histology, treatment and evolution were registered. Single-variate and multivariate statistic analysis were applied. The study was approved by the Ethics Committee of the Clínicas University Hospital. Results: Three hundred and forty one patients with IgA nephropathy - confirmed with histopathology analysis - were studied. Sixty five per cent were men, average age was 31 ± 13 year old and follow-up median 52 months (1-271); 14% started with dialysis and 1.2% died. The most common clinical presentation was asymptomatic urinary alterations (42%), (and this was more evident in the 2000-2009 period, chi2 p < 0.05). At the beginning, proteinuria was 1.7 ± 1.9 g/l; creatininemia 1.6 ± 1.8 mg/dl, and blood pressure was PA ≥ 140/90 mmHg in 32% of patients. Crescent formations were observed in 48%. Fifty six per cent of them received angiotensin-converting enzyme (ACE) / angiotensin receptor blockers and 52% of patients received immunosuppressants. Blood pressure significantly dropped in the evolution. In the single-variate analysis, initial creatininemia was ≥ 2.5 mg/dl, proteinuria, crescent formations and endocapillary proliferation were associated to starting dialysis or death, although in the multivariate analysis (Cox regression) only the level of creatininemia was significant. Conclusions: the clinical presentation of IgA nephropathy has changed in the past decade, being the asymtomatic urinary alterations the most frequent. Creatininemia ≥ 2.5 mg/dl is associated to a worse renal survival, probably the “point of no return”. Resumo A nefropatia IgA é frequente no Uruguai e seu tratamento conflitivo. Objetivo: analisar a incidência, apresentação clínica, histologia, tratamento e evolução da nefropatia IgA no Uruguai. Material e método: uma análise retrospectiva dos dados do Registro Uruguaio de Glomerulopatias (RUG) e prontuários médicos com absoluta confidencialidade foi realizada; foram registrados idade, sexo, data da biopsia renal, apresentação clínica, pressão arterial (PA), proteinúria, hematúria e creatininemia (no inicio e na última evolução registrada), histologia, tratamentos e evolução. Análise estatística uni e multivariada. Aprovado pelo Comitê de Ética do Hospital das Clínicas. Resultados: foram estudados 341 pacientes com nefropatia IgA, confirmada por histologia, no período 1º de janeiro de 1985 - 31 de dezembro de 2009; 65% eram homens, idade média 31 ± 13 anos e mediana de seguimento de 52 meses (1-271); 14% ingressaram a diálise e 1,2% faleceu. A apresentação clínica mais frequente foi presença de alterações urinárias assintomáticas (AUA) (42%), (maior no período 2000-2009, chi2 p < 0,05). No inicio, a proteinuria era 1,7 ± 1,9 g/l; creatininemia 1,6 ± 1,8 mg/dl, e 32% apresentaram PA ≥ 140/90 mmHg. Foram observadas crescentes celulares em 48%. Receberam inibidores da enzima conversora da angiotensina/antagonistas dos receptores da angiotensina (IECA/ ARA) 56% e imunossupressores, 52%. A PA diminuiu significativamente na evolução. Na análise univariada, a creatininemia inicial foi ≥2,5 mg/dl, proteinúria, crescentes celulares e proliferação endocapilar estavam associados ao ingresso à diálise e ao falecimento, porém na análise multivariada (regressão de Cox) somente o nível de creatininemia foi significativo. Conclusões: a apresentação clínica da nefropatía IgA mostrou mudanças na última década, sendo mais frequente as AUA. A creatininemia ≥ 2,5 mg/dl está associada a pior supervivência renal, provável “ponto de não retorno”.

It ranges from an asymptomatic state to a severe condition associated with extreme elevations in creatine kinase and acute renal failure. Reported etiologies of rhabdomyolysis include alcohol abuse, drugs, muscle trauma and muscle overexertion. Less common causes include muscle enzyme deficiencies, electrolyte abnormalities, infectious causes, toxins and endocrine disorders. Hypokalemia is a rare cause of rhabdomyolysis. We report six patients aged 31 to 57 years (three women) with a severe hypokalemic rhabdomyolysis, secondary to chronic diarrhea in two patients, treatment with loop diuretics in one and Gitelman syndrome in three. Rhabdomyolysis may be underdiagnosed in the context of hypokalemia, because the neuromuscular symptoms can be attributed solely to the electrolyte disorder.

INTRODUCTION: Epithelial-to-mesenchymal transition (EMT) is a key event in renal fibrosis. The aims of the study were to evaluate acidosis induced EMT, transforming-growth-factor (TGF) β1 role and citrate effect on it. METHODS: HK2 cells (ATCC 2290) were cultured in DMEM/HAM F12 medium, pH 7.4. At 80% confluence, after 24 hr under serum free conditions, cells were distributed in three groups (24 hours): A) Control: pH 7.4, B) Acidosis: pH 7.0 and C) Calcium citrate (0.2 mmol/L) + pH 7.0. Change (Δ) of intracellular calcium concentration, basal and after Angiotensin II (10-6M) exposition, were measured to evaluate cellular performance. EMT was evaluated by the expression of α-smooth muscle actin (α-SMA) and E-cadherin by immunocytochemistry and/or Western blot. TGF-β1 secretion was determined by ELISA in cell supernatant. RESULTS: At pH 7.0 HK2 cells significantly reduced E-cadherin and increased α-SMA expression (EMT). Supernatant TGF-β1 levels were higher than in control group. Calcium citrate decreased acidosis induced EMT and improved cells performance, without reduction of TGF-β production. CONCLUSIONS: Acidosis induces EMT and secretion of TGF-β1 in tubular proximal cells in culture and citrate improves cellular performance and ameliorates acidosis induced EMT. INTRODUÇÃO: A transição epitélio-mesenquimal (TEM) é um evento chave na fibrose renal. Os objetivos do estudo foram avaliar se o citrato seria capaz de reverter a TEM induzida por acidose, e qual seria o papel do fator de crescimento transformador (TGF) β1 neste evento. MÉTODOS: Células de túbulo proximal (HK2) foram cultivadas em meio DMEM-F12, pH 7,4. Após confluência, as células foram distribuídas em três grupos A) controle: pH 7,4, B) Acidose: pH 7,0 e C) Acidose: pH 7,0 + citrato de cálcio (0,2 mmol/L). A variação na concentração de cálcio intracelular, antes e após a adição de angiotensina II (10-6M) foi medida para avaliar o desempenho celular. TEM foi avaliada pela expressão de α-actina de músculo liso (α-SMA) e E-caderina por imunocitoquímica e/ou de Western blot. A secreção de TGF-β1 foi determinada por ELISA no sobrenadante. RESULTADOS: Em pH 7,0, houve redução significante na expressão de E-caderina e aumento de α-SMA indicando a presença de TEM e a concentração de TGF-β1 foi maior do que no grupo controle. O citrato de cálcio melhorou TEM induzida pela acidose e a resposta das células à angiotensina II, sem redução do TGF-β. CONCLUSÕES: Acidose induz TEM e secreção de TGF-β1 em células tubulares proximais em cultura e o citrato melhora o desempenho celular e a TEM induzida por acidose.

Resumen: Arch Med Interna 2012 - 34(1):03-11 Mediante revisión de PubMed y opinión de expertos los autores concluyeron que proteinuria y albuminuria son marcadores de diagnóstico de enfermedad renal crónica (ERC), son indicadores de progresión y por lo tanto un objetivo terapéutico y además son marcadores de riesgo de muerte y enfermedad CV tanto en población general como en pacientes con ERC. Estos resultados se confirmaron utilizando la tira reactiva, el cociente albuminuria/creatininuria (A/C) o el cociente proteinuria/creatininuria (P/C) en muestra de orina con referencia a la proteinuria en orina de 24 hs (gold standard). El índice A/C parece ser el método ideal porque será el más fácil de estandarizar, pero su costo es mayor y el índice P/C es buen predictor a un costo menor. Las tiras reactivas tienen 29% de falsos positivos pero si somos estrictos en la recolección de la muestra de orina y la lectura es automatizada se reduce la posibilidad de error y la variabilidad de resultados. Se establecen por consenso las etapas preanalíticas, analíticas y post-analíticas de la medición de proteinuria y albuminuria en laboratorio para precisar mejor el diagnóstico y evolución de ERC. ABSTRACT: Arch Med Interna 2012 - 34(1):03-11 Through PubMed review and opinion of experts the authors concluded that proteinuria and albuminuria are diagnostic markers of chronic kidney disease (CKD), are indicators of progression and therefore a therapeutic goal, and are also risk markers of death and CV disease both in the general population and in patients with CKD. These results were confirmed by using the dipstick, proteinuria/creatininuria (P/C) or albuminuria/creatininuria (A/C) ratios in single urine sample with reference to the 24 hours (gold standard) collected urine. The A/C ratio seems to be the ideal method because it will be the easiest to standardize, but their cost is higher, and the P/C ratio is a good predictor at one lower cost. The dipstick test have 29% false positive but if we are strict in collecting the urine sample and the reading is automated it would reduce the possibility of error and the variability of results. Pre-analytical, analytical and post-analytical stages of the proteinuria and albuminuria measurements in laboratory were established by consensus to clarify the diagnosis and evolution of CKD.
