1. Tropifexor plus cenicriviroc combination versus monotherapy in nonalcoholic steatohepatitis: Results from the phase 2b TANDEM study.

Abstract: BACKGROUND AND AIMS: With distinct mechanisms of action, the combination of tropifexor (TXR) and cenicriviroc (CVC) may provide an effective treatment for NASH. This randomized, multicenter, double-blind, phase 2b study assessed the safety and efficacy of TXR and CVC combination, compared with respective monotherapies. APPROACH AND RESULTS: Patients (N = 193) were randomized 1:1:1:1 to once-daily TXR 140 μg (TXR 140 ), CVC 150 mg (CVC), TXR 140 μg + CVC 150 mg (TXR 140 + CVC), or TXR 90 μg + CVC 150 mg (TXR 90 + CVC) for 48 weeks. The primary and secondary end points were safety and histological improvement, respectively. Rates of adverse events (AEs) were similar across treatment groups. Pruritus was the most frequently experienced AE, with highest incidence in the TXR 140 group (40.0%). In TXR and combination groups, alanine aminotransferase (ALT) decreased from baseline to 48 weeks (geometric mean change: -21%, TXR 140 ; -16%, TXR 140 + CVC; -13%, TXR 90 + CVC; and +17%, CVC). Reductions in body weight observed at week 24 (mean changes from baseline: TXR 140 , -2.5 kg; TXR 140 + CVC, -1.7 kg; TXR 90 + CVC, -1.0 kg; and CVC, -0.1 kg) were sustained to week 48. At least 1-point improvement in fibrosis stage/steatohepatitis resolution without worsening of fibrosis was observed in 32.3%/25.8%, 31.6%/15.8%, 29.7%/13.5%, and 32.5%/22.5% of patients in the TXR 140 , CVC, TXR 140 + CVC, and TXR 90 + CVC groups, respectively. CONCLUSIONS: The safety profile of TXR + CVC combination was similar to respective monotherapies, with no new signals. TXR monotherapy showed sustained ALT and body weight decreases. No substantial incremental efficacy was observed with TXR + CVC combination on ALT, body weight, or in histological end points compared with monotherapy.

2. The building of an institutional liver transplant registry: opportunities and challenges

Abstract: To improve current data systems for institutional decision-making, the Adult Liver Transplant Registry was established at the Hospital Italiano de Buenos Aires, Argentina. This article describes its design and implementation and reports on the outcomes for patients transplanted since its January 2020 launch. A multidisciplinary team designed the registry by identifying key variables from a literature review while considering balance between data depth and feasibility. Rigorous quality control measures were enforced, including monthly audits and staff training. Benchmark indicators for post-transplant outcomes were established. As of November 2023, the registry included 136 transplants. Its implementation and maintenance were straightforward, with no significant difficulties encountered. Cirrhosis was the predominant indication (77%) for transplant. Only one living donor transplantation was performed. Post-transplant results generally aligned with benchmarks, but rates of biliary complications slightly exceeded the recommended thresholds. The one-year post-transplant survival rate was 87%. The successful registry implementation provides a robust framework for research, treatment management, and patient care enhancement within a liver transplant unit.

Abstract: Introducción: cada vez más comunidades rurales expresan preocupación por posible daño a la salud por los agroquímicos. A partir de conocer la presencia en el cuerpo humano de glifosato es posible confeccionar, junto a los actores locales, un modelo sistémico que evidencie los condicionantes socioambientales de la salud. Materiales y métodos: Investigación-Acción Participativa. Muestreo probabilístico trietápico (manzanas, casas, familia) de French (Partido de 9 de Julio; Provincia Buenos Aires) para la medición de factores ambientales y autoinforme de enfermedad oncológica. Análisis de glifosato en orina por cromatografía líquida acoplada a espectrometría de masa en tándem. Entrevistas en profundidad a casos positivos para determinar la vía de exposición. Confección de un modelo conceptual para el análisis de la complejidad sistémica. Resultados: del total de las 46 manzanas de French, 23 manzanas fueron incluidas con muestreo sistemático y en 76 casas (50%) una persona fue seleccionada al azar. En el 21,8% de los hogares refirieron antecedentes de enfermedad oncológica. El 13% de la población (IC 95%: 6,5-23) presentó glifosato cuantificable en orina en junio de 2023. La exposición laboral se descartó en todos los casos, siendo las principales fuentes autorreferidas: “la descarga de agroquímicos en el galpón cercano”, “la cerealera”, “la deriva” y “los camiones mosquito que pasan por la calle”, variables pertenecientes a condicionantes económicos y culturales. Emergió una red de actores, a la espera de la problematización sociopolítica de los resultados mediada por el Honorable Concejo Deliberante (HCD) y las Organizaciones no Gubernamentales (ONG), que equilibren el sistema con fuerzas de cuidado. Discusión: estudio de alta validez externa. Se requiere informatizar sistemas de vigilancia comunitarios tanto para la sospecha de exposición como de posibles enfermedades relacionadas con agroquímicos; crear circuitos de derivación de muestras a laboratorios de alta complejidad y diseñar estrategias múltiples sobre los condicionantes para cuidar de posibles daños por exposición crónica. Conclusión: la presencia de glifosato en la orina se debió a exposición ambiental; expresa una vía de absorción pasiva, involuntaria y crónica de contaminantes ambientales que resulta de la actividad agropecuaria de French que no es antagonizada por fuerzas de cuidado.

Abstract: Health registries are organized systems that collect data on individuals with a particular disease, condition or exposure. The aim of this narrative review was to provide an integrated perspective from the Research Department at the Hospital Italiano de Buenos Aires, Argentina, on how health registries can be used as a bridge between healthcare, research, education, innovation and development while addressing ethical challenges. The review includes a description of the experience of a registry implemented at our institution, which has provided healthcare for 170 years, and is committed to support, education and research. We focus on the potential of health registries to provide better value healthcare by reducing healthcare costs and improving health outcomes and quality of care, and to improve medical knowledge. However, we also acknowledge and discuss the challenges that accompany these achievements, such as that of ethical issues. Through effective collaboration and integration with other healthcare stakeholders, health registries can be a powerful tool to promote better health.

Abstract: Background & Aims: Malnutrition, sarcopenia, and frailty are prevalent in cirrhosis. We aimed to assess the correlation between assessment tools for malnutrition, sarcopenia, and frailty in patients on the liver transplant (LT) waiting list (WL), and to identify a predictive model for acute-on-chronic liver failure (ACLF) development. Methods: This prospective single-center study enrolled consecutive patients with cirrhosis on the WL for LT (May 2019-November 2021). Assessments included subjective global assessment, CT body composition, skeletal muscle index (SMI), ultrasound thigh muscle thickness, sarcopenia HIBA score, liver frailty index (LFI), hand grip strength, and 6-minute walk test at enrollment. Correlations were analyzed using Pearson's correlation. Competing risk regression analysis was used to assess the predictive ability of the liver- and functional physiological reserve-related variables for ACLF. Results: A total of 132 patients, predominantly with decompensated cirrhosis (87%), were included. Our study revealed a high prevalence of malnutrition (61%), sarcopenia (61%), visceral obesity (20%), sarcopenic visceral obesity (17%), and frailty (10%) among participants. Correlations between the assessment tools for sarcopenia and frailty were poor. Sarcopenia by SMI remained prevalent when frailty assessments were not usable. After a median follow-up of 10 months, 39% of the patients developed ACLF on WL, while 28% experienced dropouts without ACLF. Multivariate analysis identified MELD-Na, SMI, and LFI as independent predictors of ACLF on the WL. The predictive model MELD-Na-sarcopenia-LFI had a C-statistic of 0.85. Conclusions: The poor correlation between sarcopenia assessment tools and frailty underscores the importance of a comprehensive evaluation. The SMI, LFI, and MELD-Na independently predicted ACLF development in WL. These findings enhance our understanding of the relationship between sarcopenia, frailty, and ACLF in patients awaiting LT, emphasizing the need for early detection and intervention to improve WL outcomes. Impact and implications: The relationship between sarcopenia and frailty assessment tools, as well as their ability to predict acute-on-chronic liver failure (ACLF) in patients on the liver transplant (LT) waiting list (WL), remains poorly understood. Existing objective frailty screening tests have limitations when applied to critically ill patients. The correlation between sarcopenia and frailty assessment tools was weak, suggesting that they may capture different phenotypes. Sarcopenia assessed by skeletal muscle index, frailty evaluated using the liver frailty index, and the model for end-stage liver disease-Na score independently predicted the development of ACLF in patients on the WL. Our findings support the integration of liver frailty index and skeletal muscle index assessments at the time of inclusion on the WL for LT. This combined approach allows for the identification of a specific patient subgroup with an increased susceptibility to ACLF, underscoring the importance of early implementation of targeted treatment strategies to improve outcomes for patients awaiting LT.

Abstract: Introduction and Objectives: there is insufficient data regarding bacterial infections in patients with cirrhosis to support recommendations for empiric antibiotic treatments, particularly in Latin America. This study aimed to evaluate bacterial infection's clinical impact and microbiological characteristics, intending to serve as a platform to revise current practices. Materials and Methods: multicenter prospective cohort study of patients with cirrhosis and bacterial infections from Argentina and Uruguay. Patient and infection-related information were collected, focusing on microbiology, antibiotic susceptibility patterns, and outcomes. Results: 472 patients were included. Spontaneous bacterial infections and urinary tract infections (UTIs) were registered in 187 (39.6%) and 116 (24.6%) patients, respectively, representing the most common infections. Of the 256 culture-positive infections, 103 (40.2%) were caused by multidrug-resistant organisms (reaching 50% for UTI), and 181 (70.7%) received adequate initial antibiotic treatment. The coverage of cefepime and ceftriaxone was over 70% for the empirical treatment of community-acquired spontaneous infections, but ceftazidime´s coverage was only 40%. For all UTI cases and for healthcare-associated or nosocomial spontaneous bacterial infections, the lower-spectrum antibiotics that covered at least 70% of the isolations were imipenem and meropenem. During hospitalization, a second bacterial infection was diagnosed in 9.8% of patients, 23.9% required at least one organ support, and 19.5% died. Conclusions: short-term mortality of bacterial infections in patients with cirrhosis is very high, and a high percentage were caused by multidrug-resistant organisms, particularly in UTIs. The information provided might serve to adapt recommendations, particularly related to empirical antibiotic treatment in Argentina and Uruguay. 27. Sarcopenia HIBA score predicts sarcopenia and mortality in patients on the liver transplant waiting list

Abstract: Abstract Sarcopenia is a prevalent condition that predicts prognosis in patients awaiting liver transplantation (LT). The gold standard for the diagnosis of sarcopenia is the assessment of the muscular area at L3 with computed tomography (CT) scan (skeletal muscle index [SMI]), but the routine use of CT scan is limited in clinical practice. Thus, we designed a single‐center observational study aimed to evaluate the clinical factors associated with the presence of sarcopenia by SMI, and to build a score capable of predicting or excluding the presence of sarcopenia in patients on the LT waiting list (WL). Binary logistic regression analysis was performed to establish the factors independently associated with sarcopenia, and the Sarcopenia Hospital Italiano de Buenos Aires (HIBA) score was built from the resulting model after internal validation analysis by bootstrapping and correction for optimism. The predictive capability of mortality on the WL was evaluated with competing risk regression analysis. A total of 215 patients with cirrhosis on the LT WL were included. The independent factors associated with the presence of sarcopenia were male sex (odds ratio [OR]: 6.09, p < 0.001), body mass index (OR: 0.74, p < 0.001), Child Pugh (OR: 1.44, p < 0.001), and the ratio creatinine/Cystatin C (OR: 0.03, p = 0.007). The Sarcopenia HIBA score constructed with these variables showed an area under the curve of 0.862. During follow‐up, 77 (36%) patients underwent LT, 46 (21%) died, and 92 (43%) remained alive. After adjusting for Model for End‐Stage Liver Disease–Sodium, Sarcopenia HIBA score was an independent predictor of WL mortality (subhazard ratio: 1.19; 95% confidence interval 1.01–1.40; p = 0.042). Sarcopenia HIBA score is an easy‐to‐use, objective, and reliable diagnostic and predictive tool that can be useful to improve the prognostic evaluation and allow identifying a group of patients with a higher risk of death while awaiting LT.

Abstract: Introduction and Objectives: Fungi, archaea and protozoa are the least known members of the gut microbiome, but they could represent a niche for biomarkers discovery to risk-stratify MAFLD patients. This study aimed to identify gut metatranscriptomic signatures in MAFLD patients from Argentina. Materials and Methods: Stool samples, diet, demographic and clinical data were obtained from 33 biopsy-proven MAFLD patients (12 simple steatosis (SS) / 21 steatohepatitis (SH)) and 19 healthy volunteers (HV). Abstract: Introduction and Objectives: Selecting an empiric antibiotic treatment in patients with cirrhosis and spontaneous bacterial infections is challenging. It is of paramount importance to have local epidemiological data to maximize pathogen coverage while minimizing the unnecessary use of broad-spectrum antibiotics. This study aimed to describe the patterns of antibiotic resistance of spontaneous bacterial infections according to the site of acquisition. Materials and Methods: Analysis of the multicenter prospective cohort study of cirrhotic patients with bacterial infections in Argentina and Uruguay (NCT03919032). Only culture-positive spontaneous infections were included in this study: spontaneous bacterial peritonitis (SBP), spontaneous bacterial empyema (SBE), and spontaneous bacteremia (SB). We estimated the proportion of infections that were sensitive to various antibiotics according to where the infection was acquired: community-acquired (CA), healthcare-associated (HCA), or nosocomial (NOS). Approximately 80% coverage is advisable for empiric treatments in stable patients and 90% for critically-ill patients. Results: The main cohort included 472 patients, of whom 97 presented culture-positive spontaneous infections and were included: with 57 (59%) SBP, 34 (35%) SB, and 4 (6%) SBE. Regarding the site of acquisition, 43% were CA, 36% NOS, and 21% HCA. Gram-positive and negative bacteria were found in 53% and 47% of the infections. The most frequent isolations were Streptococcus spp (26%), E coli (20), K pneumonia (15%), S Aureus (10%), E. faecium (6%) and E. faecalis (4%). Multidrug-resistant organisms (MDROs) were isolated in 35% of the patients. As shown in the table, cefepime and ceftriaxone offer the most rational coverage for CA and HCA infections, and imipenem or meropenem for NOS infections. However, in critically-ill patients, broader-spectrum antibiotics are needed to achieve a coverage closer to 90% (table). Conclusions: We present, for the first time in our region, evidence-based recommendations for the empirical treatment of spontaneous bacterial infections. Prior colonization and/or infections by MDROs might refine even more the antibiotic selection and should be explored.

Abstract: Introduction and Objectives: The functional dynamics of the gut microbiome and its interactions with the human transcriptome represent a niche for non-invasive biomarkers to risk-stratify MAFLD. This study aimed to identify gut transcriptomic signatures associated with MAFLD in Argentina. Materials and Methods: Stool samples, diet, demographic and clinical data were obtained from 33 biopsy-proven MAFLD patients (12 simple steatosis -SS- and 21 steatohepatitis -SH-) and 19 healthy volunteers (HV). Abstract: Background & Aims: Two recently developed composite models, the alpha-fetoprotein (AFP) score and Metroticket 2.0, could be used to select patients with hepatocellular carcinoma (HCC) who are candidates for liver transplantation (LT). The aim of this study was to compare the predictive performance of both models and to evaluate the net risk reclassification of post-LT recurrence between them using each model’s original thresholds. Methods: This multicenter cohort study included 2,444 adult patients who underwent LT for HCC in 47 centers from Europe and Latin America. A competing risk regression analysis estimating sub-distribution hazard ratios (SHRs) and 95% CIs for recurrence was used (Fine and Gray method). Harrell’s adapted c-statistics were estimated. The net reclassification index for recurrence was compared based on each model’s original thresholds. Nuestro objetivo fue estimar la proporción de pacientes con EHGNA que teniendo riesgo indeterminado o alto de fibrosis según scores no-invasivos se someten a estudios adicionales de estadificación. Materiales y métodos: Estudio de corte transversal multicéntrico de pacientes con EHGNA evaluados por hepatólogos del 1 de junio del 2018 al 31 de julio del 2018. Se calcularon FIB-4 y NAFLD fibrosis score en todos los pacientes y se consideró que, si al menos uno de los scores sugería riesgo indeterminado o alto de fibrosis, el paciente debería haberse sometido a estudios adicionales de estadificación. Resultados: Se incluyeron 238 pacientes. La mediana de tiempo entre el diagnóstico de EHGNA y la inclusión en el estudio fue de 12.2 (IQR 3.0-36.5) meses. En total, 128 (54%) pacientes tuvieron al menos una evaluación con score no-invasivo que sugería riesgo indeterminado o alto de fibrosis. Sin embargo, solamente 72 (56%) se realizaron estudios para confirmar el grado de fibrosis (elastografía, biopsia, etc.). Las principales barreras encontradas por los médicos para aplicar los algoritmos de estadificación fueron relacionadas a cobertura de salud y a costos. Conclusiones: Un alto porcentaje de pacientes con EHGNA tiene riesgo indeterminado o alto de fibrosis, de acuerdo con los resultados obtenidos con scores no invasivos. Sin embargo, solamente la mitad se realizó estudios adicionales, evidenciando una baja adherencia a las recomendaciones actuales. Abstract: Acute-on-chronic liver failure (ACLF) is one of the main causes of death on the waiting list. Liver transplantation (LT) is the only curative treatment for patients with ACLF and therefore it should be considered in all cases. However, the applicability of LT in patients with ACLF is challenging, given the scarcity of donors and the high short-term mortality of these patients. Organ allocation has traditionally been prioritised according to the model for end-stage liver disease (MELD) system. However, the accuracy of MELD score is limited in patients with ACLF. In this article, the authors review the outcomes of patients with ACLF before and after LT, highlighting its clinical course, the feasibility of LT in the sickest patients, the role of the organ allocation system, and possible indicators of futility.

Abstract: Liver transplantation is an extremely complex procedure performed in an extremely complex patient. With a successful technique and acceptable long-term survival, a new challenge arose: overcoming donor shortage. Thus, living donor liver transplant and other techniques were developed. Aiming for donor safety, many liver transplant units attempted to push the viable limits in terms of size, retrieving smaller and smaller grafts for adult recipients. With these smaller grafts came numerous problems, concepts, and definitions. The spotlight is now aimed at the mirage of hemodynamic changes derived from the recipients prior alterations. This article focuses on the numerous hemodynamic syndromes, their definitions, causes, and management and interconnection with each other. The aim is to aid the physician in their recognition and treatment to improve liver transplantation success.

Abstract: La pandèmia del COVID-19, les crisis econòmiques de molts països en desenvolupament i la crisi ambiental van marcar l'inici de la dècada de 2020. Molts països van començar a tenir problemes de deute, un problema que a Llatinoamèrica en general va afectar en la dècada del ’80 i que a l'Argentina en particular ve assotant des de la dècada del 1970 d'ara endavant. Les típiques reestructuracions amb agents privats van significar quitacions de deute i el lliurament de nous bons. En tant que amb organismes internacionals com el Fons Monetari Internacional (FMI), els bescanvis van significar estirament dels terminis a canvi d'una sèrie de mesures econòmiques de tint liberal. La crisi climàtica que travessa el món en l'actualitat requereix d'accions concretes immediatament i per part de tots els actors: empreses, governs, organismes de crèdit, societat, etc. Sense perdre de vista això, molts països han de donar respostes a creixents necessitats socials que es plantegen diàriament. Donat aquest context es proposa un nou rol dels organismes de crèdit internacional, en aquest cas del FMI, en el qual es converteixin en actors climàtics actius. En el present treball es proposa el bescanvi de part del deute argentí per acció climàtica que resolgui al mateix temps el problema d'endeutament que tingui el país, però que al seu torn contribueixi a reduir els impactes ambientals. Per a justificar aquesta mesura, es mesura el valor que atorguen actualment alguns serveis ecosistèmics a la Conca Matanza-Riachuelo i en els aiguamolls del Delta de Paraná, i s'avalua el potencial de millora que poden arribar a obtenir en cas d'un sanejament en el cas de la Conca o una restauració en el cas dels aiguamolls., La pandemia del covid-19, las crisis económicas de muchos países en desarrollo y la crisis ambiental marcaron el inicio de la década de 2020. Muchos países comenzaron a tener problemas de deuda, un problema que en Latinoamérica en general afectó en la década del ’80 y que a Argentina en particular viene azotando desde la década del 1970 en adelante. Las típicas reestructuraciones con bonistas privados significaron quitas de deuda y la entrega de nuevos bonos. En tanto que con organismos internacionales como el Fondo Monetario Internacional (FMI), los canjes significaron estiramiento de los plazos a cambio de una serie de medidas económicas de tinte liberal. La crisis climática que atraviesa el mundo en la actualidad requiere de acciones concretas inmediatamente y por parte de todos los actores: empresas, gobiernos, organismos de crédito, sociedad, etc. Sin perder de vista eso, muchos países deben dar respuestas a crecientes necesidades sociales que se plantean a diario. Dado este contexto se propone un nuevo rol de los organismos de crédito internacional, en este caso del FMI, en el cual se conviertan en actores climáticos activos. En el presente trabajo se propone el canje de parte de la deuda argentina por acción climática que resuelva al mismo tiempo el problema de endeudamiento que tenga el país, pero que a su vez contribuya a reducir los impactos ambientales. Para justificar dicha medida, se mide el valor que otorgan actualmente algunos servicios ecosistémicos en la Cuenca Matanza-Riachuelo y en los humedales del Delta de Paraná, y se evalúa el potencial de mejora que pueden llegar a obtener en caso de un saneamiento en el caso de la Cuenca o una restauración en el caso de los humedales., The covid-19 pandemic, the economic crises in many developing countries and the environmental crisis marked the beginning of the 2020s. Many countries began to have debt problems, a problem that affected Latin America in general in the 1980s and Argentina, in particular, since the 1970s onwards. The typical restructurings with private bondholders meant debt write-downs and the issuance of new bonds. Meanwhile, with international organizations such as the International Monetary Fund (IMF), the swaps meant stretching the terms in exchange for a series of liberal economic measures. The climate crisis that the world is currently experiencing requires immediate and concrete actions by all actors: companies, governments, lending agencies, society, etc. Without losing sight of this, many countries must respond to the growing social needs that arise daily. Given this context, a new role is proposed for international lending agencies, in this case the IMF, in which they become active climate actors. This paper proposes a swap of part of Argentina's debt for climate action that would help to solve the country's debt problem, while at the same time helping to reduce environmental impacts. To justify this measure, we measure the value currently provided by some ecosystem services in the Matanza-Riachuelo Basin and in the wetlands of the Paraná Delta and evaluate the potential for improvement in the case of clean-up in the case of the Basin or restoration in the case of the wetlands.

44. Resistencia antimicrobiana y producción porcina de cría intensiva

Abstract: Introducción: el impacto de la resistencia antimicrobiana (RAM) generará un aumento de las muertes relacionadas de 10 millones anuales hacia 2050. El 70% de la dispensación de antimicrobianos (ATB) se utiliza en la agroveterinaria y no en salud humana. Es fundamental conocer la portación de RAM en trabajadores de cría de animales y en los animales, para acciones tempranas de salud pública. Métodos: bajo metodología PRISMA se realizó la búsqueda bibliográfica en distintas fuentes disponibles hasta octubre de 2020. Se priorizaron revisiones sistemáticas, metanálisis, ensayos clínicos y estudios observacionales para determinar la RAM en trabajadores de cría de cerdos. De 990 artículos identificados se incluyeron 8 estudios. Resultados: la tasa de colonización por Staphylococcus aureus resistente a la meticilina (SAMR) en trabajadores fue mayor que la de la población general. La prevalencia de SAMR fue significativamente mayor en trabajadores en contacto directo con animales y los de granjas de cría intensiva con respecto a los de extensiva. En cerdos, la prevalencia de RAM en cría intensiva fue significativamente mayor que la de los de cría extensiva. También fue significativa la asociación entre el suministro de antibióticos en la cría intensiva y la presencia de RAM. Las granjas de más de 1250 cerdos presentaron mayor prevalencia de RAM (p < 0,001). El fenotipo de SAMR en cerdos, trabajadores y el ambiente fue el mismo. Conclusiones: existe evidencia de asociación entre la producción agrícola de cría intensiva y la RAM en cerdos y trabajadores. No se encontraron estudios de vigilancia epidemiológica en la Argentina en trabajadores de cría de animales.

Abstract: Background & Aims: Patients with hepatocellular carcinoma (HCC) are selected for liver transplantation (LT) based on pre-LT imaging ± alpha-foetoprotein (AFP) level, but discrepancies between pre-LT tumour assessment and explant are frequent. Our aim was to design an explant-based recurrence risk reassessment score to refine prediction of recurrence after LT and provide a framework to guide post-LT management. Methods: Adult patients who underwent transplantation between 2000 and 2018 for HCC in 47 centres were included. A prediction model for recurrence was developed using competing-risk regression analysis in a European training cohort (TC; n = 1,359) and tested in a Latin American validation cohort (VC; n=1,085). Results: In the TC, 76.4% of patients with HCC met the Milan criteria, and 89.9% had an AFP score of ≤2 points. The recurrence risk reassessment (R3)-AFP model was designed based on variables independently associated with recurrence in the TC (with associated weights): ≥4 nodules (sub-distribution of hazard ratio [SHR] = 1.88, 1 point), size of largest nodule (3–6 cm: SHR = 1.83, 1 point; >6 cm: SHR = 5.82, 5 points), presence of microvascular invasion (MVI; SHR = 2.69, 2 points), nuclear grade >II (SHR = 1.20, 1 point), and last pre-LT AFP value (101–1,000 ng/ml: SHR = 1.57, 1 point; >1,000 ng/ml: SHR = 2.83, 2 points). Wolber’s c-index was 0.76 (95% CI 0.72–0.80), significantly superior to an R3 model without AFP (0.75; 95% CI 0.72–0.79; p = 0.01). Four 5-year recurrence risk categories were identified: very low (score = 0; 5.5%), low (1–2 points; 15.1%), high (3–6 points; 39.1%), and very high (>6 points; 73.9%). The R3-AFP score performed well in the VC (Wolber’s c-index of 0.78; 95% CI 0.73–0.83). Conclusions: The R3 score including the last pre-LT AFP value (R3-AFP score) provides a user-friendly, standardised framework to design post-LT surveillance strategies, protocols, or adjuvant therapy trials for HCC not limited to the Milan criteria. Lay summary: Considering discrepancies between pre-LT tumour assessment and explant are frequent, reassessing the risk of recurrence after LT is critical to further refine the management of patients with HCC. In a large and international cohort of patients who underwent transplantation for HCC, we designed and validated the R3-AFP model based on variables independently associated with recurrence post-LT (number of nodules, size of largest nodule, presence of MVI, nuclear grade, and last pre-LT AFP value). The R3-AFP model including last available pre-LT AFP value outperformed the original R3 model only based on explant features. The final R3-AFP scoring system provides a robust framework to design post-LT surveillance strategies, protocols, or adjuvant therapy trials, irrespective of criteria used to select patients with HCC for LT.

Abstract: Background & Aims: Good outcomes after liver transplantation (LT) have been reported after successfully downstaging to Milan criteria in more advanced hepatocellular carcinoma (HCC). We aimed to compare post-LT outcomes in patients receiving locoregional therapies (LRT) before LT according to Milan criteria and University of California San Francisco downstaging (UCSF-DS) protocol and ‘all-comers’. Methods: This multicentre cohort study included patients who received any LRT before LT from Europe and Latin America (2000–2018). We excluded patients with alpha-foetoprotein (AFP) above 1,000 ng/ml. Competing risk regression analysis for HCC recurrence was conducted, estimating subdistribution hazard ratios (SHRs) and corresponding 95% CIs. Results: From 2,441 LT patients, 70.1% received LRT before LT (n = 1,711). Of these, 80.6% were within Milan, 12.0% within UCSF-DS, and 7.4% all-comers.
