PURPOSE As cancer surgery restarts after the first COVID-19 wave, health care providers urgently require data to determine where elective surgery is best performed. This study aimed to determine whether COVID-19–free surgical pathways were associated with lower postoperative pulmonary complication rates compared with hospitals with no defined pathway. PATIENTS AND METHODS This international, multicenter cohort study included patients who underwent elective surgery for 10 solid cancer types without preoperative suspicion of SARS-CoV-2. Participating hospitals included patients from local emergence of SARS-CoV-2 until April 19, 2020. At the time of surgery, hospitals were defined as having a COVID-19–free surgical pathway (complete segregation of the operating theater, critical care, and inpatient ward areas) or no defined pathway (incomplete or no segregation, areas shared with patients with COVID-19). The primary outcome was 30-day postoperative pulmonary complications (pneumonia, acute respiratory distress syndrome, unexpected ventilation). RESULTS Of 9,171 patients from 447 hospitals in 55 countries, 2,481 were operated on in COVID-19–free surgical pathways. Patients who underwent surgery within COVID-19–free surgical pathways were younger with fewer comorbidities than those in hospitals with no defined pathway but with similar proportions of major surgery. After adjustment, pulmonary complication rates were lower with COVID-19–free surgical pathways (2.2% v 4.9%; adjusted odds ratio [aOR], 0.62; 95% CI, 0.44 to 0.86). This was consistent in sensitivity analyses for low-risk patients (American Society of Anesthesiologists grade 1/2), propensity score–matched models, and patients with negative SARS-CoV-2 preoperative tests. The postoperative SARS-CoV-2 infection rate was also lower in COVID-19–free surgical pathways (2.1% v 3.6%; aOR, 0.53; 95% CI, 0.36 to 0.76). CONCLUSION Within available resources, dedicated COVID-19–free surgical pathways should be established to provide safe elective cancer surgery during current and before future SARS-CoV-2 outbreaks.

First, the Mediterranean sea has a profound impact on the Atlantic ocean circulation and, consequently, on the global thermohaline conveyor belt. Maps of the 5 Mediterranean salty water tongue exiting from the Gibraltar strait at intermediate depths and spreading throughout the Atlantic interior are well known since the 1950s. Through direct pathways to the Atlantic polar regions or through indirect mixing processes, the salty Mediterranean water preconditions the deep convection cells of the polar Atlantic. There the North Atlantic Deep Water is formed which successively spreads throughout 10 the world ocean constituting the core of the global thermohaline circulation. Even more importantly, the Mediterranean Sea is a laboratory basin for the investigation of processes of global importance, being much more amenable to observational surveys because of its location in mid-latitude and its dimensions. Both the western and eastern basins in fact possess closed thermohaline circulations analogous to the 15 global conveyor belt. A unique upper layer open thermohaline cell connects the eastern to the western basin and, successively, to the north Atlantic through the Gibraltar strait. In it, the Atlantic water entering into Gibraltar in the surface layer, after travelling to the easternmost Levantine basin, is transformed into one of the saltiest water masses through air–sea heat and moisture fluxes. This is the salty water which, crossing the 20 entire basin in the opposite direction below the surface Atlantic water, finally exits from the Gibraltar strait at mid-depths. Both the western and eastern basins are endowed with deep/intermediate convection cells analogous to the polar Atlantic deep convection cells or to the intermediate mode water ones. Deep and intermediate water masses are therefore formed in differ25 ent sites of the entire basin. Because of their easily accessible locations, these convection cells are much more amenable to direct observational surveys and mooring arrays

Illegal migration often results in dramatic loss of lives that can be avoided with timely Search and Rescue (SAR) interventions; such SAR services require accurate information such as on sea-state and operational tools such as trajectory forecasting for floating objects at sea. The maritime transport of oil crossing this region accounts for 25% of the global maritime traffic and for nearly 7% of the world oil accidents over the last 25 years. In combination with localized oil extraction plants existing in the shelf zones this situation presents a serious threat to both the open-sea and coastal-zone habitats, with consequent impacts on local economic activities as tourism and fisheries, impacts on ecosystems and losses in revenue. In the case of both accidental/deliberate oil spills or drifting-vessel emergency, an operative response chain must include both the detection and the trajectory prediction steps, that take advantage of the most appropriate methodologies and data availability such as: updated meteorological information, near-surface current measurements, and hydrodynamic models with oil spill weathering processes modules. Indeed, both the knowledge of initial positions and an accurate, effective and prompt prediction of their future pathways are of fundamental importance to optimize response activities, shortening the intervention time and increasing their efficacy. In the particular case of oil spills, the knowledge of spill trajectories is very important to anticipate impacts on economic and environmental assets on threatened coasts; the provision of accurate information to decision makers together with training in best practices and damage recovery methods need to be adopted in order to minimize spill impacts.

The flow through the Grado-Marano lagoon inlets is tidally driven: more than 90% of the energy is due to the astronomical forcing [1]. The second source of variability is due to the Adriatic seiches accounting for 7% of the total kinetic energy. Local winds are responsible of the variability in the ,residual currents (tides and seiches removed). NE Bora wind increases the residual inflow into the lagoon through the easternmost inlet and favouring the outflow through the westernmost inlet. Such a behaviour is similar to what has been reported for another Northern Adriatic lagoon, namely the Venetian lagoon.

The distribution of the chlorophyll a concentration ([Chl a]) in the Mediterranean Sea, mainly obtained from satellite surface observations or from scattered in situ experiments, is updated by analyzing a database of fluorescence profiles converted into [Chl a]. The database, which includes 6790 fluorescence profiles from various origins, was processed with a specific quality control procedure. To ensure homogeneity between the different data sources, 65 % of fluorescence profiles have been intercalibrated on the basis of their concomitant satellite [Chl a] estimation. The climatological pattern of [Chl a] vertical profiles in four key sites of the Mediterranean Sea has been analyzed. Climatological results confirm previous findings over the range of existing [Chl a] values and throughout the principal Mediterranean trophic regimes. They also provide new insights into the seasonal variability in the shape of the vertical [Chl a] profile, inaccessible through remote-sensing observations. An analysis based on the recognition of the general shape of the fluorescence profile was also performed. Although the shape of [Chl a] vertical distribution characterized by a deep chlorophyll maximum (DCM) is ubiquitous during summer, different forms are observed during winter, thus suggesting that factors affecting the vertical distribution of the biomass are complex and highly variable. The [Chl a] spatial distribution in the Mediterranean Sea mimics, on smaller scales, what is observed in the global ocean. As already evidenced by analyzing satellite surface observations, midlatitude- and subtropical-like phytoplankton dynamics coexist in the Mediterranean Sea. Moreover, the Mediterranean DCM variability appears to be characterized by patterns already observed on the global scale.

- Abstract
This paper is the outcome of a workshop held in Rome in November 2011 on the occasion of the 25th anniversary of the POEM (Physical Oceanography of the Eastern Mediterranean) program. In the workshop discussions, a number of unresolved issues were identified for the physical and biogeochemical properties of the Mediterranean Sea as a whole, i.e., comprising the Western and Eastern sub-basins. Over the successive two years, the related ideas were discussed among the group of scientists who participated in the workshop and who have contributed to the writing of this paper. Three major topics were identified, each of them being the object of a section divided into a number of different sub-sections, each addressing a specific physical, chemical or biological issue: 1. Assessment of basin-wide physical/biochemical properties, of their variability and interactions. 2. Relative importance of external forcing functions (wind stress, heat/moisture fluxes, forcing through straits) vs. internal variability. 3. Shelf/deep sea interactions and exchanges of physical/biogeochemical properties and how they affect the sub-basin circulation and property distribution. Furthermore, a number of unresolved scientific/methodological issues were also identified and are reported in each sub-section after a short discussion of the present knowledge. They represent the collegial consensus of the scientists contributing to the paper. Naturally, the unresolved issues presented here constitute the choice of the authors and therefore they may not be exhaustive and/or complete. The overall goal is to stimulate a broader interdisciplinary discussion among the scientists of the Mediterranean oceanographic community, leading to enhanced collaborative efforts and exciting future discoveries. © Author(s) 2014.

Surface patterns of the low-frequency current in a 20 × 30 km region in front of the Venice Lagoon were analysed from a 13-month-long HF radar data set. Surface circulation was related to prevalent wind regimes in the area and to the tidal flow through the lagoon inlet. Three different categories of wind-forcing were defined: bora (NE wind), sirocco (SE wind), and finally the category containing all other wind directions and calms (winds lower than 3 m/sec). Mean flow and vorticity spatial distributions were discussed for different wind conditions. The coastal area about 5 km wide is characterized by a flow field with maximum vorticity. Outside the coastal boundary layer the interior flow is part of the Adriatic basin-wide cyclonic circulation. Two counter-rotating vortices of the dimension of about 45 km were evidenced in the average flow field in front of the lagoon inlet (Malamocco inlet) in all situations except for the bora. The vortex-pair is probably associated with the tidal flow through the inlet. The bora wind induces a strong southward coastal jet detached from the coast by about 5 km homogenizing the flow and eliminating the dipole. The average coastal flow pattern in calm wind conditions was also analysed as a function of the strong inflow/outflow (currents in the inlet > 0.7 m s-1) from the lagoon inlet. In both cases the vorticity pattern is similar, with the negative vorticity to the left of the inlet and positive to the right looking seaward.

In order to study the role of the Bari canyon system as pathway for sediment and water coming from the Northern Adriatic, three moorings consisting of sediment traps and current meters, were deployed near the bottom, at 600 m depth, for one year. Two moorings were deployed within the canyon (B and C) and one on the adjacent open slope (A). Principal Component Analysis (PCA) on current data indicates that for moorings A and C the major variance occurs in the along- and the across-slope (along canyon axis) direction, respectively; while for mooring B the variance was almost equally between both components. A very good correlation between the major variance component and temperature was found showing 3-day and 10-day fluctuations mainly during the convection and spreading phase.

The feasibility of monitoring the suspended particle concentration (SPC) and fluxes (PF) with Acoustic Doppler Current Profilers (ADCP) was tested in the Venice Lagoon inlets. The backscatter recorded by boat- and bottom-mounted instruments was converted into SPC estimates with a calibration procedure based on multiple water sample collection and ADCP transects. Time series of PF across the inlets were derived from discharge and SPC data allowing for the calculation of a preliminary budget of the exchanges between the lagoon and the open sea.

Long-term variations of the water flow through three Venice lagoon inlets are analysed seeking for the possible impact of structures under construction for the mobile gates for defending Venice against flooding. Time-series of monthly mean flow and standard deviations show long-term variations, mainly on the seasonal time-scale. An important increase in the water flow variance due to an increase in amplitudes of major tidal constituents in the Chioggia inlet can probably be explained as a consequence of the structures constructed recently that narrowed the channel.

Lo studio della circolazione costiera e dello scambio di flussi dacqua tra la laguna di Venezia ed il mare aperto attraverso le sue bocche di porto è stato uno dei temi principali di un progetto di ricerca, finanziato dal CORILA (Consorzio Ricerche Lagunari) negli anni 2001-2004 Un aspetto preso in considerazione in questa ricerca è il ruolo fondamentale che le correnti marine superficiali giocano nellambiente costiero per la loro influenza nellecosistema, e come il campo di corrente adiacente alle bocche, nella fascia costiera fuori della laguna, interviene nellefficienza dei flussi attraverso le bocche nel rinnovare le acque della laguna. Lo studio della circolazione superficiale e del suo impatto sulle caratteristiche ambientali richiede lacquisizione di lunghe serie di dati di corrente di alta qualità, che spesso risultano carenti a causa delle limitazioni nelle tecniche di misura disponibili. Una nuova tecnica di misura basata su una rete di sistemi Radar HF per produrre le mappe della corrente marina superficiale al largo della bocca di porto di Malamocco, è stata utilizzata, permettendo lanalisi della completa serie temporale dal 2001 al 2003. Questa tecnologia, già ampiamente utilizzata negli Stati Uniti e sperimentata per la prima volta in Italia dal gruppo di ricerca del CNR ISMAR di Venezia, in collaborazione con lOGS di Trieste ha permesso di superare le difficoltà fino ad ora incontrate nello studio delle correnti in ampie aree di mare. La circolazione costiera é stata studiata con lutilizzo di radar costieri ad alta frequenza (25-30 MHz). Il radar utilizza due sistemi di antenne ricetrasmittenti posizionate sulle isole di Lido e di Pellestrina, più una terza antenna Rx/Tx installata sulla piattaforma Acqua Alta, 8 miglia al largo delle bocche di porto. Con questa configurazione é stata coperta dalle misure una zona larga 20km davanti alla bocca di Malamocco, con una risoluzione spaziale di 1km, ottenendo le serie temporali desiderate.

31. Vorticity Patterns Offshore of the Venetian Lagoon from HF Radar Observations

- Abstract
A network of high frequency (HF) radar systems was used to produce maps of surface velocity offshore of the Malamocco inlet during the 12-month period from November 2001 through October 2002. Inspection of the sub-tidal residual flow fields revealed frequent occurrences of small-scale (~10 km) eddy structures both north and south of the inlet. In addition, several monthly averaged velocity maps indicate the presence of a persistent meander in the mean current patterns. A more objective technique, based on vorticity, was developed to search the data set for the presence of eddies near the inlet and to separate those features from the larger-scale meander pattern. The vorticity at scales of 5-10 km was computed each hour using the low-passfiltered data set and year-long vorticity statistics were investigated at selected locations surrounding the inlet. The vorticity histograms and temporal variability changed significantly as a function of location relative to the Malamocco inlet. Both north and south of the inlet, the mean vorticity was weakly negative reflecting the larger-scale meander pattern whose crest is aligned, approximately, with the inlet. However, the range of vorticity north of the inlet was much greater than it was south of the inlet. A minor peak (or shoulder) in the year-long histogram of vorticity north of the inlet highlighted frequent occurrences of strong (~ 5 × 10-5 s-1) positive vorticity events in that area. A similar statistical behaviour was not seen south of the inlet. This observation was used to define a conditional sampling criteria that lead to a map of the typical flow field associated with strong eddy events. The vorticity time series were also used to investigate the relationship of eddy events to external forcing parameters. Strong winds, for instance, acted to destroy vorticity in the surface current field since they lead to a strong but horizontally uniform response in the surface currents. The roles of sea level variability and offshore current velocity were also investigated.

- Abstract
Located between subtropical and mid-latitude climates, the Mediterranean region acts as a transition area and is very sensitive to global climate change: As global temperatures have risen, the Mediterranean region has warmed, particularly in the west and in the summer, where surface temperatures in some locations increased at a rate greater than 0.4°C per decade during the second half of the twentieth century. Model simulations give a collective picture of substantial drying and warming of the Mediterranean region in the future, especially summer, with average precipitation expected to decrease by 25-30% and temperatures expected to rise by 4°-5°C by the end of this century, approximately. These changes will likely be accompanied by increased intensity in heat waves and dry spells along with important changes in Mediterranean water masses, which will become warmer and saltier. This future climate change can pose very serious problems to ecosystems and human societies.

- Abstract
A series of seasonal cruises and Eulerian current measurements in the Otranto Strait were carried out to study the biogeochemical characteristics of the strait and to estimate the exchange of water, dissolved and particulate matter between the Adriatic and the Ionian Sea. From the biogeochemical distributions obtained, the role for the Adriatic Sea in supporting the N:P ratio anomaly of the Mediterranean is hypothesized. Examining the bottom oxygen distribution, the deep ventilation of the Eastern Mediterranean seems to follow a more complex pathway than previously hypothesized Fluxes computation shows a net outflow through the strait for nitrate, phosphate and silicate, while a slight net inflow was obtained for POC and PN.

During the period 1998–2000, the Mediterranean Forecasting System Pilot Project, aiming to build a forecasting system for the physical state of the sea, has been carried out. A ship-of-opportunity programme sampled the Mediterranean upper ocean thermal structure by means of eXpendable Bathy-Thermographs (XBTs), along seven tracks, from September 1999 to May 2000. The tracks were designed to detect some of the main circulation features, such as the stream of surface Atlantic water flowing from the Alboran Sea to the Eastern Levantine Basin. The cyclonic gyres in the Liguro-Provenal Basin, the southern Adriatic and Ionian Seas and the anticyclonic gyres in the Levantine Basin were also features to be detected. The monitoring system confirmed a long-term persistence of structures (at least during the entire observing period), which were previously thought to be transient features. In particular, in the Levantine Basin anticyclonic Shikmona and Ierapetra Gyres have been observed during the monitoring period. In order to identify the major changes in the thermal structures and the dynamical implications, the XBT data are compared with historical measurements collected in the 1980s and 1990s. The results indicate that some thermal features are being restored to the situation that existed in the 1980s, after the changes induced by the so-called “Eastern Mediterranean Transient”

- Abstract
A novel description of the phenomenology of the Eastern Mediterranean is presented based upon a comprehensive pooled hydrographic data base collected during 1985-1987 and analyzed by cooperating scientists from several institutions and nations (the POEM project). Related dynamical process and modeling studies are also overviewed. The circulation and its variabilities consist of three predominant and interacting scales: basin scale, subbasin scale, and mesoscale. Highly resolved and unbiased maps of the basin wide circulation in the thermocline layer are presented which provide a new depiction of the main thermocline general circulation, composed of subbasin scale gyres interconnected by intense jets and meandering currents. Semipermanent features exist but important subbasin scale variabilities also occur on many time scales. Mesoscale variabilities modulate the subbasin scale and small mesoscale eddies populate the open sea, especially the south-eastern Levantine basin. Clear evidence indicates Levantine Intermediate Water (LIW) to be present over most of the Levantine Basin, implying that formation of LIW is not localized but rather is ubiquitous. The Ionian and Levantine basins are confirmed to form one deep thermohaline cell with deep water of Adriatic origin and to have a turnover time of one and a quarter centuries. Prognostic, inverse, box and data assimilative modeling results are presented based on both climatological and POEM data. The subbasin scale elements of the general circulation are stable and robust to the dynamical adjustment process. These findings bear importantly on a broad range of problems in ocean science and marine technology that depend upon knowledge of the general circulation and water mass structure, including biogeochemical fluxes, regional climate, coastal interactions, pollution and environmental management. Of global ocean scientific significance are the fundamental processes of water mass formations, transformations and dispersion which occur in the basin. © 1992.

This paper reviews the present state of knowledge of the Adriatic general circulation and its higher-frequency current variability. All studies show that the surface circulation consists of a large-scale cyclonic meander with several smaller cells embedded in it, and that the circulation system is modulated seasonally. Water mass analyses and direct current measurements indicate that the cyclonic meander also occurs in the intermediate layer, dominated by the current that inflows along the eastern coast. In the bottom layer, an outflowing current pressed against the western coast prevails. The circulation patterns are constantly perturbed by the higher-frequency current variations. Tidal currents modify the mean, residual flow rather weakly. On the other hand, winds have recently emerged as an important driving force, causing transient currents which may surpass in order of magnitude all the other contributions to the Adriatic current field. It has been found that the wind-driven currents are controlled by vorticity of the wind field, and that under the summer, stratified conditions, wind pulses may trigger oscillations of the inertia period. The physical processes occurring along the two opposing coasts of the Adriatic Sea differ markedly in their characteristics. Water exchange between the semienclosed basins of the eastern coast and the open sea is mainly forced by the local wind. Conversely, the shelf area along the western coast is dominated by the Po River outflow, which in winter mostly remains confined to a coastal boundary layer, whereas in summer it spreads to the open sea as well. Critical gaps in the present knowledge are identified, and ways of approaching them are suggested. Future work must specifically address the question of the relative importance of different forcing mechanisms in driving the Adriatic general circulation. Special attention must be paid to the exchange of water through the Otranto Strait and its relation to the rate of dense water formation in the Adriatic. The relative contribution of different water masses to the formation of the outflowing Adriatic water should be assessed. Combined modelling efforts, satellite data analysis and in situ measurements are suggested to study the rather poorly known mesoscale features and processes.
