Item does not contain fulltext, BACKGROUND: Several reports have demonstrated the impact of the COVID-19 pandemic on the management and outcome of patients with ST-segment elevation myocardial infarction (STEMI). The aim of the current analysis is to investigate the potential gender difference in the effects of the COVID-19 pandemic on mechanical reperfusion and 30-day mortality for STEMI patients within the ISACS-STEMI COVID-19 Registry. METHODS: This retrospective multicenter registry was performed in high-volume primary percutaneous coronary intervention (PPCI) centers on four continents and included STEMI patients undergoing PPCIs in March-June 2019 and 2020. Patients were divided according to gender. The main outcomes were the incidence and timing of the PPCI, (ischemia time ≥ 12 h and door-to-balloon ≥ 30 min) and in-hospital or 30-day mortality. RESULTS: We included 16683 STEMI patients undergoing PPCIs in 109 centers. In 2020 during the pandemic, there was a significant reduction in PPCIs compared to 2019 (IRR 0.843 (95% CI: 0.825-0.861, p < 0.0001). We did not find a significant gender difference in the effects of the COVID-19 pandemic on the numbers of STEMI patients, which were similarly reduced from 2019 to 2020 in both groups, or in the mortality rates. Compared to prepandemia, 30-day mortality was significantly higher during the pandemic period among female (12.1% vs. 8.7%; adjusted HR [95% CI] = 1.66 [1.31-2.11], p < 0.001) but not male patients (5.8% vs. 6.7%; adjusted HR [95% CI] = 1.14 [0.96-1.34], p = 0.12). CONCLUSIONS: The COVID-19 pandemic had a significant impact on the treatment of patients with STEMI, with a 16% reduction in PPCI procedures similarly observed in both genders. Furthermore, we observed significantly increased in-hospital and 30-day mortality rates during the pandemic only among females. Trial registration number: NCT 04412655.

The aim of the current analysis is to evaluate the age-related effects of the COVID-19 pandemic on mechanical reperfusion and 30-day mortality for STEMI within the registry ISACS-STEMI COVID-19. METHODS: This retrospective multicenter registry was performed in high-volume PPCI centers on four continents and included STEMI patients undergoing PPCI in March-June 2019 and 2020. Patients were divided according to age (< or ≥75 years). The main outcomes were the incidence and timing of PPCI, (ischemia time longer than 12 h and door-to-balloon longer than 30 min), and in-hospital or 30-day mortality. RESULTS: We included 16,683 patients undergoing PPCI in 109 centers. In 2020, during the pandemic, there was a significant reduction in PPCI as compared to 2019 (IRR 0.843 (95%-CI: 0.825-0.861, p < 0.0001). We found a significant age-related reduction (7%, p = 0.015), with a larger effect on elderly than on younger patients. Furthermore, we observed significantly higher 30-day mortality during the pandemic period, especially among the elderly (13.6% vs. 17.9%, adjusted HR (95% CI) = 1.55 [1.24-1.93], p < 0.001) as compared to younger patients (4.8% vs. 5.7%; adjusted HR (95% CI) = 1.25 [1.05-1.49], p = 0.013), as a potential consequence of the significantly longer ischemia time observed during the pandemic. CONCLUSIONS: The COVID-19 pandemic had a significant impact on the treatment of patients with STEMI, with a 16% reduction in PPCI procedures, with a larger reduction and a longer delay to treatment among elderly patients, which may have contributed to increase in-hospital and 30-day mortality during the pandemic.

Abstract

ObjectiveThe initial data of the International Study on Acute Coronary Syndromes - ST Elevation Myocardial Infarction COVID-19 showed in Europe a remarkable reduction in primary percutaneous coronary intervention procedures and higher in-hospital mortality during the initial phase of the pandemic as compared with the prepandemic period. The aim of the current study was to provide the final results of the registry, subsequently extended outside Europe with a larger inclusion period (up to June 2020) and longer follow-up (up to 30 days).MethodsThis is a retrospective multicentre registry in 109 high-volume primary percutaneous coronary intervention (PPCI) centres from Europe, Latin America, South-East Asia and North Africa, enrolling 16 674 patients with ST segment elevation myocardial infarction (STEMI) undergoing PPPCI in March/June 2019 and 2020. The main study outcomes were the incidence of PPCI, delayed treatment (ischaemia time >12 hours and door-to-balloon >30 min), in-hospital and 30-day mortality.ResultsIn 2020, during the pandemic, there was a significant reduction in PPCI as compared with 2019 (incidence rate ratio 0.843, 95% CI 0.825 to 0.861, p75 years) (p=0.015), and was not related to the peak of cases or deaths due to COVID-19. The heterogeneity among centres was high (pConclusionPercutaneous revascularisation for STEMI was significantly affected by the COVID-19 pandemic, with a 16% reduction in PPCI procedures, especially among older patients (about 20%), and longer delays to treatment, which may have contributed to the increased in-hospital and 30-day mortality during the pandemic.Trial registration numberNCT04412655.

Item does not contain fulltext, OBJECTIVE: The initial data of the International Study on Acute Coronary Syndromes - ST Elevation Myocardial Infarction COVID-19 showed in Europe a remarkable reduction in primary percutaneous coronary intervention procedures and higher in-hospital mortality during the initial phase of the pandemic as compared with the prepandemic period. The aim of the current study was to provide the final results of the registry, subsequently extended outside Europe with a larger inclusion period (up to June 2020) and longer follow-up (up to 30 days). METHODS: This is a retrospective multicentre registry in 109 high-volume primary percutaneous coronary intervention (PPCI) centres from Europe, Latin America, South-East Asia and North Africa, enrolling 16 674 patients with ST segment elevation myocardial infarction (STEMI) undergoing PPPCI in March/June 2019 and 2020. The main study outcomes were the incidence of PPCI, delayed treatment (ischaemia time >12 hours and door-to-balloon >30 min), in-hospital and 30-day mortality. RESULTS: In 2020, during the pandemic, there was a significant reduction in PPCI as compared with 2019 (incidence rate ratio 0.843, 95% CI 0.825 to 0.861, p<0.0001). This reduction was significantly associated with age, being higher in older adults (>75 years) (p=0.015), and was not related to the peak of cases or deaths due to COVID-19. The heterogeneity among centres was high (p<0.001). Furthermore, the pandemic was associated with a significant increase in door-to-balloon time (40 (25-70) min vs 40 (25-64) min, p=0.01) and total ischaemia time (225 (135-410) min vs 196 (120-355) min, p<0.001), which may have contributed to the higher in-hospital (6.5% vs 5.3%, p<0.001) and 30-day (8% vs 6.5%, p=0.001) mortality observed during the pandemic. Western societies face unprecedented challenges after the pandemic. One of these is related to the possibility, accelerated and enhanced by the restrictions introduced by governments during the emergency, of structuring social systems in which the relational dimension appears less and less central. Replacing it is the illusion of being able to build a 'comfortable society' in which technology makes human life increasingly disintermediated. The world of work, over which hovers the utopia of reduced fatigue thanks to technology and the remoteisation of work, does not escape this danger. The essay addresses these issues by emphasising their strong anthropological challenge.

17. Gender equality attitudes of Muslim migrants in Italy

Abstract

In European public debates, Islam is often described as having a determinant role on gender inequality. However, Islamic community in Italy is significantly diversified and gender-related attitudes are affected by the different contexts of origin. By using multipurpose Istat data, we estimate a synthetic index of gender equality attitudes and conduct multivariate empirical analyses on migrant Muslim men and women. Results show that the heterogeneity among migrant groups persists after controlling for other factors. In addition, more religious Muslims are on average more opposed to gender equality. However, the role of religious communal integration is more important than that of individual religiosity.

Abstract

The body of literature on the fertility of migrants in Europe has grown significantly in recent decades. The focus has mostly been on analysing their actual behaviours, and reflection on other crucial aspects extensively analysed in studies on natives has been lacking. In particular, differences between migrants’ reproductive decision-making process and that of natives are currently understudied in the European context. We will focus on the association between demographic and socio-economic characteristics and fertility intentions, comparing native Italian and migrant women in Italy. We apply logistic regression models to analyse factors associated with strong intentions to have a child (or positive fertility intention) and strong intentions not to have a child (or negative fertility intention) in the short term. We use data from the Social Condition and Integration of Foreign Citizens survey on households with at least one foreign member and from the Italian Gender and Generation Survey, both carried out at the national level by the National Institute of Statistics (ISTAT). Net of controlled covariates, migrant women have both higher positive and lower negative fertility intentions within a 3-year time frame than native women do. However, the patterns of fertility intentions are different for natives and migrants as a function of age, educational level, marital status, parity, homeownership and, notably, labour market status. Focusing only on migrant women, we observe that fertility intentions vary by age at arrival and time since migration and that there are significant differences by country and geographical area of birth.

Abstract

Couple formation and migration are the results of interrelated decision-making processes in the life cycle. Using data from the “Social Condition and Integration of Foreign Citizens (SCIF)” survey, conducted in Italy in 2011–2012 by Istat, we aim to investigate how the timing of migration events affects the type and timing of marriages in the destination country. Time-related models investigate the competing-risk transitions to endogamous and exogamous marriages with Italian spouses. Obtained results provide evidence of the complexity of today’s migrations, and they indicate the coexistence of various patterns among first-generation migrants in Italy, characterised by a plurality of origins, with different projects and behavioural models. The “interrelation of events” hypothesis explains the transitions to both endogamous and exogamous marriages among women, while men usually spend more time finding a partner and achieving economic stability. Despite this general picture, our analysis shows different and original pathways shaping transitions to marriage by reason of migration and considering a number of demographic and migratory characteristics

Abstract

In the Attentional Boost Effect (ABE), stimuli encoded with to-be-responded targets are later recognized more accurately than stimuli encoded with to-be-ignored distractors. While this effect is robust in young adults, evidence regarding healthy older adults and clinical populations is sparse. The present study investigated whether a significant ABE is present in bipolar patients (BP), who, even in the euthymic phase, suffer from attentional deficits, and whether the effect is modulated by age. Young and adult euthymic BP and healthy controls (HC) presented with a sequence of pictures paired with target or distractor squares were asked to pay attention to the pictures and press the spacebar when a target square appeared. After a 15-min interval, their memory of the pictures was tested in a recognition task. The performance in the detection task was lower in BP than in HC, in both age groups. More importantly, neither young nor adult BP exhibited a significant ABE; for HC, a robust ABE was only found in young participants. The results suggest that the increase in the attentional demands of the detection task in BP and in adult HC draws resources away from the encoding of target-associated stimuli, resulting in elimination of the ABE. Clinical implications are discussed.

Obiettivo di questo capitolo è di affrontare, sulla base dei dati dell’indagine campionaria dell’Istat su “Condizione e integrazione sociale dei cittadini stranieri”, i temi della mancata partecipazione scolastica, dell’intenzione di andare all’università e dell’uscita precoce dalla scuola dei giovani di cittadinanza straniera , allo scopo di valutarne l’associazione con alcune caratteristiche demografiche individuali, la situazione socio-economica familiare, l’inserimento scolastico e/o il livello medio di integrazione della famiglia. Dopo una rapida disamina della letteratura sulle determinanti della dispersione scolastica e dell’uscita precoce dal sistema formativo, le tre tematiche sopra indicate vengono analizzate, attraverso il ricorso alla regressione logistica, le caratteristiche ed i fattori determinanti per fornire sintetiche indicazioni sulle ricadute in termini di politiche sociali e scolastiche di inclusione.

According to Eurostat (European Commission 2014, 2016), Italy has one of highest quota in Europe of immigrant youths (aged 18-24) with at most a compulsory level of education, who left school before they have finished their course and have not attended any other course in the last four weeks. After a short presentation of the international literature on ethnic identity (section 2), we will introduce the survey and the method of analysis adopted (section 3). The maternal ethnic identity of children (aged 14-24) will be presented in section 4 and in the following one (section 5) the determinant of educational trajectories will be discussed with special attention to the role of maternal ethnic identity. In the final section, the main results will be synthetized and the possible political implications will be highlighted.

Abstract

Behavioral and psychological symptoms of dementia (BPSD) are commonly observed since the early stage of Alzheimer's disease (AD) associated with structural brain changes. It is conceivable that they may also relate to functional brain changes. This resting-state functional MRI (RS-fMRI) study investigated the alterations within functional brain networks of a cohort of AD patients at different clinical stages who presented with BPSD. One hundred one AD patients and 56 patients with amnestic mild cognitive impairment underwent a neuropsychological evaluation including the Neuropsychiatry Inventory-12 (NPI-12). All patients and 35 healthy controls (HS) underwent 3T-MRI. Factor analysis was used to extract the principal factors from NPI-12, while RS-fMRI data were processed using graph theory to investigate functional connectivity. Five factors were extracted from NPI-12. Sixty-two percent of patients showed BPSD and functional brain connectivity changes in various networks compared to those without BPSD and HS. These changes contributed to account for patients' BPSD. This work opens new perspectives in terms of nonpharmacological interventions that might be designed to modulate brain connectivity and improve patients' BPSD.

Abstract

The clinical usefulness of two commercial peach extracts for SPT (by Lofarma SpA and ALK-Abellò, respectively) was compared in a multicenter study carried out in Italy. Peach allergic patients were tested with the two extracts in parallel and underwent the detection of IgE specific for all three peach allergens currently available (Pru p 1, Pru p 3, and Pru p4, respectively). The two extracts were almost identical in terms of sensitivity and specificity, being able to detect virtually all patients sensitized to stable peach allergens (lipid transfer protein [LTP] and, presumably, peamaclein) but scoring negative in patients exclusively sensitive to labile allergens (either PR-10 and/or profilin). Thus, the two extracts represent an excellent tool to carry out a preliminary component-resolved diagnosis of peach allergy at the first patient visit

Abstract

Numerous research has shown that there is direct relationship among high educational outcomes, employment, income and integration. Immigrants’ children do show, on average, lower educational performances than natives’ ones in addition of being more socio-economically disadvantaged. Countries’ success in helping and integrating immigrants’ children needs to find new educational policies and strategies. Using PISA survey data, we aim at providing further elements of discussion looking at “resilient” students. Given the strong impact of the students’ social, cultural and economic family background, we aim at analysing how “disadvantaged” students can overcome their socio-economic and cultural obstacles and achieve good educational performances. Applying multilevel logit models, we analyse the main determinants to be resilient students considering both individual motivations and school resources.

Abstract

School enrollment and early school leaving are crucial issues in many EU countries, with Italy playing a central role in the European context. According to Eurostat data, in 2011 Italy is ranked 28th for early leaving from education and training (ELET) among 33 countries. Large school dropout rates, less success in studies, loss of school years, and lack of concentration, which are directly involved in the labor market, are the main issues that children of immigrants in the Italian school face. This paper contributes to the growing literature on these issues and highlights its main characteristics and determinants. The main source of this work is the first (and only) survey on the Condition and Social Integration of Foreign Citizens in 2011-2012 of ISTAT (National Institute of Statistics in Italy) released in 2017. The survey collects data on families with at least one foreign citizen and provides unprecedented information on foreigners living in Italy. The data allowed us to associate each young immigrant student with parents’ characteristics (e.g. age, citizenship, degree of study, family type) and provided information about the type of schooling (e.g. loss of school years, perceived discrimination in class, amount of homework). Using logistic regression models, we analyzed three different dichotomous dependent variables: non-enrollment at school, intention to enroll in university and early school leaving, and training of young people of foreign citizenship. This paper evaluates their association with some individual demographic characteristics, family socio-economic context, school inclusion, and (or) the average level of family integration. The results show that the risk of not being enrolled in school is significantly lower for girls, increases with age, is higher in Southern Italy (i.e. double compared to the Northern area), is higher for those encountering problems with the language, and increases with age on arrival in Italy. Family characteristics play a decisive role, in that young foreigners living without parents exhibit a greater risk of not being enrolled in school, the same is true for those with low-educated parents. When analyzing the intention to enroll in university, it was also possible to use variables related to the study path of young foreigners. The intention of not going to university is clearly associated with school failure and with parents’ low interest in following their children’s school career. The analysis on early leaving from studies and training confirmed the afore-mentioned results. In particular, the risk of early leaving from studies and training affects young people who arrived in Italy between 13 and 17 years, especially Albanian or Chinese citizens. Even in this case the role of parents is decisive.

After a synthetic introductory analysis on the trends of legal and resident population in Italy according to different sources, such as census, post-census estimations and Municipal Population Registers, the paper aims at focusing on the migratory component of population dynamics. In particular, it underlines the recent changes occurred to foreign immigration (for example, the importance of family reunifications and the growth of asylum seekers) to foreign-origin presence (from first to second generation, and from foreigners to new Italians), as well as to Italian emigration.
