Neglect is a severe neuropsychological/neurological deficit that usually develops due to lesions of the posterior inferior parietal area of the right hemisphere and is characterized by a lack of attention to the left side. Our case is a proven right-handed, 30-year-old female patient with a low-grade glioma, which was located in the temporo-opercular region and also in the superior temporal gyrus of the right hemisphere. Upon presurgical planning, the motor, language, and visuospatial functions were mapped. In order to achieve this, the protocol for routine magnetic resonance imaging and navigated transcranial magnetic stimulation has been expanded, accordingly. Absztrakt: Bevezetés: Jelenleg nem áll rendelkezésünkre specifikus kuratív kezelés vagy vakcina az új koronavírus-2 (COVID–19, SARS-CoV-2) kezelésére. A súlyos citokinvihar-szindrómával társuló COVID–19-fertőzés kezelésére sürgősen szükséges alternatív kezelési megoldást találni. Célkitűzés: A convalescens plazmaterápia alkalmazása más, súlyos vírusfertőzések kezelésében a túlélés javulását eredményezte, és ígéretes terápiás megközelítés lehet súlyos COVID–19-betegek számára. Módszer: Két kritikus állapotú, COVID–19-fertőzött beteg convalescens friss fagyasztott plazmával történő kezelését mutatjuk be. Eredmények: A plazmaterápia megkezdésekor mindkét beteg gépi lélegeztetésre szorult, és antivirális kezelésben, valamint maximális szupportív ellátásban részesült. A COVID–19-fertőzésből nemrég felépült és a súlyos akut légúti distressz szindrómát okozó koronavírus-2 elleni immunglobulin-G-t termelő donorokból 3 × 200 ml convalescens plazma transzfúzióját alkalmaztuk. A convalescens plazma transzfúzióját követően mindkét betegnél javult az oxigénszaturáció, és a gyulladásos értékeket jelző paraméterek csökkenést mutattak. A convalescens plazma adását megelőző paraméterekhez viszonyítva növekedett a lymphocytaszám, és csökkent az interleukin-6-szint. Mindkét betegnél 2 héten belül megszüntethető volt a gépi lélegeztetés. Nem észleltünk a convalescens plazma adása során súlyos mellékhatást. Következtetések: Megfigyelésünk alapján a convalescens plazmával történő terápia jól tolerálható volt, és javíthatja a betegek gyógyulási arányát. A convalescens plazmával való kezelés optimális dózisának, időpontjának és klinikai előnyeinek meghatározására további nagy, kontrollált vizsgálatok szükségesek. Közleményünk megírásának célja az első két, hazánkban convalescens plazmával sikeresen kezelt, kritikus állapotú COVID–19-fertőzött beteg kórtörténetének bemutatása. Absztrakt: A repetitív transcranialis mágneses stimulációs eljárás egy olyan neuromodulációs technika, melynek gyorsan fejlődő területe jelentheti az egyik biztonságos, alternatív megközelítési módot bizonyos mentális zavarok, elsősorban a terápiarezisztens major depresszív zavar kezelésében. Célunk a transcranialis mágneses stimulációs kezelés jelenlegi kutatási és klinikai alkalmazási lehetőségeinek bemutatása a szakirodalmi adatok és a klinikai gyakorlat áttekintése alapján. Mióta ismertté vált, hogy külső mágneses forrással neuronok depolarizálhatók, mind a neurológia, mind a pszichiátria az eljárás lehetséges klinikai használatát kutatja. Napjainkig a módszer a pszichiátria területén belül a major depresszív zavar és a kényszeres zavar kezelésében engedélyezett, de egyéb betegségek (például szkizofrénia, bipoláris zavar) kezelésében is kutatják lehetséges alkalmazását. A transcranialis mágneses stimulációs rendszerek fejlesztésének következő lépcsőjét jelenti a mágnesesrezonancia-felvétel alapján, helymeghatározó algoritmusok segítségével, valós időben navigált mágneses stimulációs készülékek kidolgozása. Az úgynevezett neuronavigációs rendszerek lehetővé teszik a depresszió kialakulásáért felelős idegi hálózatok pontos célzását, így a dorsolateralis praefrontalis kéreg excitabilitásának fokozását a bal féltekén és csökkentését a jobb féltekén. Az eljárás kevés kontraindikációval bír, és a megfelelő körültekintéssel választott indikáció és betegpopuláció esetén mellékhatások is alacsony számban jelentkeznek. A transcranialis mágneses stimulációs kezelés mára bizonyítottan hatékony módszerré vált bizonyos mentális zavarok, elsősorban a terápiarezisztens major depresszív zavar kezelésében. A jövőben a neuronavigációs neuromodulációs kezelések, azon belül is a repetitív transcranialis mágneses stimulációs kezelés térhódítása várható a pszichiátria és a neurológia területén. Jelenleg Magyarországon több centrumban is végeznek különböző indikációkban mágneses stimulációs kezeléseket, ám az eljárás mentális zavarokban történő alkalmazásának finanszírozási befogadása és beépítése a mindennapi klinikai gyakorlatba még várat magára. IntroductionThe goal of brain tumor surgery is the maximal resection of neoplastic tissue, while preserving the adjacent functional brain tissues. The identification of functional networks involved in complex brain functions, including visuospatial abilities (VSAs), is usually difficult. We report our preliminary experience using a preoperative planning based on the combination of navigated transcranial magnetic stimulation (nTMS) and DTI tractography to provide the preoperative 3D reconstruction of the visuospatial (VS) cortico-subcortical network in patients with right parietal lobe tumors.Material and MethodsPatients affected by right parietal lobe tumors underwent mapping of both hemispheres using an nTMS-implemented version of the Hooper Visual Organization Test (HVOT) to identify cortical areas involved in the VS network. DTI tractography was used to compute the subcortical component of the network, consisting of the three branches of the superior longitudinal fasciculus (SLF). The 3D reconstruction of the VS network was used to plan and guide the safest surgical approach to resect the tumor and avoid damage to the network. We retrospectively analyzed the cortical distribution of nTMS-induced errors, and assessed the impact of the planning on surgery by analyzing the extent of tumor resection (EOR) and the occurrence of postoperative VSAs deficits in comparison with a matched historical control group of patients operated without using the nTMS-based preoperative reconstruction of the VS network.ResultsTwenty patients were enrolled in the study (Group A). The error rate (ER) induced by nTMS was higher in the right vs. the left hemisphere (p=0.02). In the right hemisphere, the ER was higher in the anterior supramarginal gyrus (aSMG) (1.7%), angular gyrus (1.4%) superior parietal lobule (SPL) (1.3%), and dorsal lateral occipital gyrus (dLoG) (1.2%). The reconstruction of the cortico-subcortical VS network was successfully used to plan and guide tumor resection. A gross total resection (GTR) was achieved in 85% of cases. After surgery no new VSAs deficits were observed and a slightly significant improvement of the HVOT score (p=0.02) was documented. The historical control group (Group B) included 20 patients matched for main clinical characteristics with patients in Group A, operated without the support of the nTMS-based planning. A GTR was achieved in 90% of cases, but the postoperative HVOT score resulted to be worsened as compared to the preoperative period (p=0.03). The comparison between groups showed a significantly improved postoperative HVOT score in Group A vs. Group B (p=0.03).ConclusionsThe nTMS-implemented HVOT is a feasible approach to map cortical areas involved in VSAs. It can be combined with DTI tractography, thus providing a reconstruction of the VS network that could guide neurosurgeons to preserve the VS network during tumor resection, thus reducing the occurrence of postoperative VSAs deficits as compared to standard asleep surgery.

At present, neither specific curative treatment nor vaccines for novel coronavirus 2019 (COVID-19) are available. There is an urgent need to look for alternative strategies for COVID-19 treatment especially in the case of severe and/or critically ill patients with cytokine release syndrome (CRS).Convalescent plasma proved to increase survival rates in other severe viral infections. Therefore, convalescent plasma could be a promising treatment option for severe COVID-19 patients.In our article, we present the first two critically ill Hungarian patients with COVID-19 infection treated with convalescent fresh frozen plasma.At the time of plasma therapy both patients were on mechanical ventilation and received antiviral agents and a full scale of supportive care. Each patient received 3 × 200 mL of convalescent plasma of recently recovered donors with sufficient novel anti-coronavirus IgG titers. Subsequent to convalescent plasma infusion, oxygenization improved and inflammatory markers decreased in both individuals. As compared to pretransfusion, lymphocyte counts increased and interleukin-6 level lessened. Both patients were weaned from mechanical ventilation within 2 weeks of treatment. No severe adverse effects were observed.Our experience indicates that convalescent plasma therapy is well tolerated and could potentially improve clinical outcomes. Optimal dose and timing as well as precise assessment of clinical benefit of convalescent plasma therapy will need further investigation in larger, well-controlled trials. The rapidly evolving field of repetitive transcranial magnetic stimulation as a neuromodulational technique may mean a safe, alternative approach to the management of several mental disorders, especially treatment-resistant major depressive disorder. Our aim is to describe the current role of transcranial magnetic stimulation in research and routine clinical practice, based on the literature and clinical protocols. Since the discovery, that an outer magnetic source can depolarize neurons, both neurology and psychiatry seek the method's possible clinical utility. To date, in the field of psychiatry, the method is only approved in the treatment of major depressive disorder and obsessive-compulsive disorder, but research continues to find application in other mental disorders (schizophrenia, bipolar disorder), too. The next step in the evolution of repetitive transcranial magnetic stimulation is based on magnetic resonance guided, real-time navigation with the help of positioning algorithms. The so-called neuronavigational systems make precise aiming of neuronal circuits responsible for the development of depression, thus increasing the excitability of the left dorsolateral prefrontal cortex and decreasing it on the right hemisphere. The method has few contraindications, and the occurrence of side effects can be minimized by carefully selected patient population. For today, transcranial magnetic stimulation became an evidence-based, effective treatment for some mental disorders, especially treatment-resistant major depressive disorder. It is to be assumed that in the future neuronavigational neuromodulation techniques, including repetitive transcranial magnetic stimulation, will be widely used in the field of psychiatry and neurology. Magnetic stimulation is currently available in a number of centres in Hungary, but the financial approval and the implementation of this neuromodulation method for treating mental disorders in the everyday clinical practice are still in progress.
