In the light of the increasing importance of cardiovascular cross-sectional imaging in current guidelines, the goal of this study is to provide a comprehensive overview of cardiovascular imaging (CVI) offered by radiological institutions across Germany.Data were extracted from the national certification program database of the German Roentgen Society (DRG) from 2015-2021. A nationwide online survey among radiology institutes (university hospitals, non-university hospitals, and private practices) was conducted for 2019, and data was extracted from the European Society of Cardiovascular Radiology (ESCR) registry. The data collection's key points included the number of centers and individuals certified for CVI, the number of cardiac CT and MRI examinations performed, the reporting habits, and the participation in the ESCR registry.71 centers and 1278 persons, both with a substantial increase since 2015, were registered and certified by the DRG to perform CVI. According to the survey, a total of 69,286 CT and 64,281 MRI examinations were performed annually. Data from the survey and the ESCR registry indicated that reporting was mostly done solely by radiologists or, to a lesser degree, in joint consensus meetings with non-radiologists. The overall participation rate in the ESCR registry was 48 % among the survey's participants.This comprehensive analysis demonstrates that high-quality CVI by radiologists is available nationwide. Antibiotic carrier particles of variable size might influence mechanic properties within impacted thermodisinfected and native cancellous bone different. Herafill®G containing calciumsulfate and calciumcarbonate provides high local concentrations of gentamicin being important for revision surgery in infected joint replacements. Native and thermodisinfected cancellous bone derived from 6 to 7 months old piglets was used for in vitro impaction bone grafting and supplemented each with Herafill®G granules of two different sizes. Introduction Morphology and mechanic properties of impacted cancellous bone are affected by carrier substances which provide high local concentrations of antibiotics. Methods Bone chips were taken from the femoral head of 6–7 months old piglets. One half was thermodisinfected and the other remained native. Ten specimens each were mixed with Herafill® antibiotic pellets and a control group of each 10 specimens respectively was examined. The cancellous bone was impacted according to Exeter technique and the implants were cemented. The distribution of the particles and the pores were defined with three dimensional computertomographic scan and shear force resistance was measured until failure. Results Shear force resistance was not measured significantly less for thermodisinfected (2.7 Nm) compared with native bone (3.5 Nm) and addition of antibiotic pellets reduced shear force resistance in both groups since this was significant for the native group. The average pore volume of the native bone specimens appeared significant smaller compared to the thermodisinfected group (p = 0.011) and the pore volume showed a negative correlation with shear force resistance (p = 0.044). Pore volume around the pellets was found significantly increased and it appeared smaller for native bone. The number of pellets located next to the implant showed a negative correlation with shear force resistance (p = 0.034) and the negative correlation increased for pellets below the tip of the shaft model (p = 0.024). Conclusion Adding antibiotic pellets to native and thermodisinfected impacted cancellous bone increased pore volume since the area around the pellets showed increased porosity which correlated with reduced shear force resistance. Computertomographic three dimensional measurement of porosity might predict shear force resistance of impacted cancellous bone and improve impaction of bone grafting intraoperatively.

Zusammenfassung Hintergrund Mit in Kraft treten des neuen Strahlenschutzgesetzes und der neuen Strahlenschutzverordnung in Deutschland ergaben sich viele neue Änderungen für die Radiologie in Bezug auf die alte Strahlenschutzverordnung und Röntgenverordnung. Ziel der Arbeit Die wesentlichen Änderungen in der Radiologie in Hinblick auf die Bereiche Anzeige- und Genehmigungsverfahren, Teleradiologie, Früherkennung, Forschung und Radon am Arbeitsplatz wurden zusammengefasst. Methode Es wurden die Änderungen von neuem Strahlenschutzgesetz und neuer Strahlenschutzverordnung zur alten Strahlenschutzverordnung und Röntgenverordnung herausgearbeitet. Dabei lag der Schwerpunkt auf Bereichen, die über die Arbeitsabläufe in der klinischen Routine hinausgehen. Ergebnisse und Diskussion Die Anforderungen beim Anzeige- und Genehmigungsverfahren wurden erhöht. So ist hier beispielsweise der Nachweis zu erbringen, dass ein Medizinphysik-Experte hinzugezogen werden kann. Durch Festlegung von Bearbeitungsfristen seitens der zuständigen Behörden können Bearbeitungsverfahren beschleunigt und mehr Planungssicherheit geschaffen werden.

Ziel der Arbeit Die wesentlichen Änderungen in Hinblick auf die arbeitstägliche Routine in der Radiologie wurden zusammengefasst. Methode Die Änderungen und Neuerungen von neuem Strahlenschutzgesetz und neuer Strahlenschutzverordnung zur alten Strahlenschutzverordnung und Röntgenverordnung wurden herausgearbeitet. Der Hauptschwerpunkt lag hierbei in der Bedeutung für die Arbeitsabläufe in der klinischen Routine. Ergebnisse und Diskussion Die neue Strahlenschutzgesetzgebung beinhaltet eine Reihe von Anordnungen, die dem Strahlenschutz, der Qualitätssicherung und der Dosisoptimierung entscheidende Werkzeuge zu deren Umsetzung in die Hand legen. Dies geht aber auch mit einem zeitlichen und personellen Mehraufwand einher.

The purpose of this study was to assess the diagnostic accuracy of computed tomography pulmonary angiogram (CTPA) including dual energy and reconstruction of iodine maps for diagnosing CTEPH. This method for detecting embolisms and perfusion failures was compared with V/Q-SPECT. An additional purpose was to compare the applied radiation dose of both techniques. 71 patients (49 women) with suspected CTEPH were included in this prospective study. The patients received a V/Q-SPECT and a dual-energy CTPA. Iodine maps were reconstructed from the data set. CTPA and the iodine maps were read by an experienced radiologist unaware of the clinical information as well as the results of the V/Q-SPECT. Results were compared to the V/Q-SPECT. DLP and the applied amount of radionuclides (MAA, Technegas) were obtained for comparison of radiation dose. For the diagnosis of CTEPH, the sensitivity of DECT was 1.000, specificity 0.966, PPV 0.867 and NPV 1.000, respectively. There was not a considerable difference in the x-ray exposure between the DECT examination and the V/Q-SPECT (1.892 mSv vs. 1.911 mSv; p = 0.6115). Both examination modalities were highly consistent regarding the classification of pathological segments (1177/1278 segments, 92,09 %, κ = 0,5938). This study presents the DECT, in combination with reconstructed iodine maps, as a potential alternative to the current imaging technique of first choice, V/Q-SPECT. Zudem wurde die Übereinstimmung der CTPA mit der Ventilations-/Perfusionsszintigrafie (V/Q-SPECT) bei der Detektion von Embolien und Perfusionsausfällen untersucht. Ein weiteres Ziel war es, die applizierte Strahlenexposition beider Techniken miteinander zu vergleichen. 71 Patienten (49 Frauen) mit dem Verdacht auf eine CTEPH wurden in diese prospektive Studie eingeschlossen. Die Patienten erhielten eine V/Q-SPECT und eine Dual-Energy-CTPA und hieraus rekonstruierte Jodkarten. Die DECT erzielte eine Sensitivität von 1,000, eine Spezifität von 0,966, einen PPV von 0,867 und einen NPV von 1,000. Zwischen den beiden Untersuchungsmodalitäten gab es, in Bezug auf die Strahlenexposition, nahezu keinen Unterschied (1,911 mSv vs. 1,892 mSv). Die Untersuchungsmodalitäten zeigten im Hinblick auf Perfusionsdefizite eine hohe Übereinstimmung (1177/1278 Segmente, 92,09 %; κ = 0,5938). Die DECT präsentierte sich, in Kombination mit den Jodkarten, als potenzielle Alternative zum gegenwärtigen bildgebenden Verfahren der ersten Wahl, der V/Q-SPECT. Evaluation of the dose values of a polytrauma whole-body CT examination used in clinical practice with regard to the 2016 updated diagnostic reference levels and reduction of the mean exposure levels using simple optimization steps. In each case, 100 exposure values before and after dose optimization were compared with the old and new diagnostic reference levels. The grayscale values and the signal-to-noise ratio (SNR) were determined for the lung, the aortic arch and the liver. A visual assessment of the image quality was performed by two radiologists on the basis of a Likert scale (0 - non-diagnostic, 1 - poor visualization, 2 - moderate visualization, 3 - good visualization, 4 - excellent visualization) for CT examinations both before and after optimization. The acquired exposure values after dose optimization were below the old and new diagnostic reference levels (1319.98 ± 463.16 mGy · cm) while the mean value of the exposure values before optimization (1774.96 ± 608.78 mGy · cm) exceeded the current diagnostic reference levels. The measured grayscale values (HU) were (before versus after optimization): lung - 833 HU vs. - 827 HU (p = 0.43), aortic arch 341 HU vs. 343 HU (p = 0.70) and liver 68 HU vs. 67 HU (p = 0.35). After dose optimization the SNR in the lung was minimally higher, while it was minimally lower in the two other regions than before the optimization. Visual assessment of the image quality showed almost identical values with 3.85 evaluation points before and 3.82 evaluation points after dose optimization (p = 0.57). Due to the updating of the diagnostic reference levels, an analysis of the own exposure values is necessary in order to be able to detect high values promptly and to initiate appropriate measures for dose reduction. Jeweils 100 Expositionswerte vor und nach der Dosisoptimierung wurden mit den alten und den neuen DRW verglichen. Die Graustufenwerte und das Signal-Rausch-Verhältnis (SNR) wurden für die Lunge, den Aortenbogen und die Leber bestimmt. Durch 2 Radiologen erfolgte eine visuelle Beurteilung der Bildqualität anhand einer Likert-Skala (0 – nicht diagnostisch, 1 – schlecht sichtbar, 2 – moderat sichtbar, 3 – gut sichtbar, 4 – exzellent sichtbar) sowohl für die CT-Untersuchungen vor als auch nach der Optimierung. Die ermittelten Expositionswerte nach der Dosisoptimierung lagen unterhalb der alten und neuen DRW (1319,98 ± 463,16 mGy · cm), während der Mittelwert der Expositionswerte vor der Optimierung (1774,96 ± 608,78 mGy · cm) die aktuellen DRW überschritt. Die gemessenen Graustufenwerte (HE) betrugen (vor gg. nach der Optimierung) Lunge – 833 HE gg. – 827 HE (p = 0,43), Aortenbogen 341 HE gg. 343 HE (p = 0,70) und Leber 68 HE gg. 67 HE (p = 0,35) (gg = gegenüber). Das SNR war nach der Dosisoptimierung in der Lunge minimal höher, in den beiden anderen Regionen minimal geringer als vor der Optimierung. Die visuelle Beurteilung der Bildqualität wies mit 3,85 Bewertungspunkten vor und 3,82 Bewertungspunkten nach der Dosisoptimierung (p = 0,57) relativ identische Werte auf. Anlässlich der Aktualisierung der DRW ist eine Analyse der eigenen Expositionswerte unumgänglich, um Überschreitungen zeitnah erkennen und geeignete Maßnahmen zur Dosisreduktion einleiten zu können. Fabry disease (FD) is an X-linked multi-organ disorder of lysosomal metabolism with cardiac disease being the leading cause of death. Identifying early FD-specific pathologies is important in the context of maximum therapeutic benefit in these stages. Therefore, the aim of this study was to investigate the value of quantitative cardiac T1 mapping as a potential disease-specific surrogate. 16 consecutive FD patients (9 female, 7 male; median age: 54 years, IQR 17) and 16 control patients (9 female, 7 male; median age: 52 years, IQR 20) were investigated at 1.5 Tesla. Native T1 mapping was performed using a modified look locker inversion recovery sequence (MOLLI) and native T1 times were measured within the septal myocardium at the midventricular short-axis section. Also functional parameters, left ventricular morphology, presence of late-gadolinium enhancement, cTnI- and Lyso-Gb3-Levels were evaluated. The median native septal T1 time for FD was 889.0 ms and 950.6 ms for controls (p 0.003). LGE and positive cTnI values (0.26 ± 0.21) were present in 5 FD patients (31.25 %), and left ventricular hypertrophy (LVH) was present in 4 FD patients (25.00 %). The 4 cTnI and 8 Lyso-Gb3 positive FD patients had significantly lower native T1 values (p 0.05, respectively p 0.01). Assuming a T1 cut-off value of 900 ms for the identification of increased cardiac lipid deposit, 9 patients with FD (56.25 %) had pathologic values (4 patients cTnI and 8 patients Lyso-Gb3 positive). Moreover, native septal T1 showed a good negative correlation to Lyso-Gb3 (r = - 0.582; p = 0.018). A pathologic cardiac native T1 time obviously reflects cardiac involvement in the scope of FD at tissue level. Fortschr Röntgenstr 2019; 191: 932 - 939.ZIEL: Morbus Fabry (FD) ist eine X-chromosomale Multiorganerkrankung des lysosomalen Metabolismus, wobei die kardiale Beteiligung die Haupttodesursache der Erkrankung darstellt. Deswegen ist es wichtig, möglichst frühe Erkrankungsmanifestationen zu detektieren, um einen maximalen therapeutischen Nutzen zu erzielen. Das Ziel unserer Studie war es, die Wertigkeit des nativen T1-Mappings als krankheitsspezifisches Äquivalent zu untersuchen. 16 konsekutive FD-Patienten (9 weiblich, 7 männlich; Altersmedian 54 Jahre; IQR 17) und 16 Kontrollpatienten (9 weiblich, 7 männlich; Altersmedian 52 Jahre, IQR 20) wurden mit einem 1,5-Tesla MRT-System untersucht. Das native T1-Mapping wurde als modifizierte Look-Locker-Sequenz (MOLLI) durchgeführt, die Messungen erfolgten im septalen linksventrikulären Myokard auf mittventrikulären Kurzachsenschnitten. Zudem wurden die linksventrikuläre Funktion und Morphologie, das Vorhandensein einer Kontrastmittelspätanreicherung sowie cTnI- und Lyso-Gb3-Laborwerte ausgewertet. Die mediane native septale T1-Zeit bei FD-Patienten betrug 889,0 und 950,6 für die Kontrollgruppe (p 0,003). 5 (31,25 %) Patienten hatten eine Kontrastmittelspätanreicherung und positive cTnI-Werte, 4 Patienten (25,0 %) eine linksventrikuläre Hypertrophie. Die 5 cTnI- und die 8 Lyso-Gb3-positiven Patienten hatten signifikant niedrigere native T1-Zeiten (p 0,05, respektive p 0,01). Unter der Annahme eines Grenzwertes von 900 ms für die Detektion eines erhöhten, zellulären Lipidgehalts zeigten 9 Patienten (56,25 %) pathologische Werte. Davon waren 8 Patienten Lyso-Gb3- und 4 Patienten cTnI-positiv. Zudem zeigte sich eine gute negative Korrelation der nativen T1-Zeit zu den Lyso-Gb3-Werten (r = – 0,582; p = 0,018). Offensichtlich reflektiert eine pathologische native T1-Zeit eine kardiale Beteiligung bei FD-Patienten. BACKGROUND: In the EuroNet Pediatric Hodgkin Lymphoma (EuroNet-PHL) trials, decision on Waldeyer's ring (WR) involvement is usually based on clinical assessment, that is, physical examination and/or nasopharyngoscopy. However, clinical assessment only evaluates mucosal surface and is prone to interobserver variability. Modern cross-sectional imaging technology may provide valuable information beyond mucosal surface, which may lead to a more accurate WR staging. PATIENTS, MATERIALS, AND METHODS: The EuroNet-PHL-C1 trial recruited 2102 patients, of which 1752 underwent central review including reference reading of their cross-sectional imaging data. In 14 of 1752 patients, WR was considered involved according to clinical assessment. In these 14 patients, the WR was re-assessed by applying an imaging-based algorithm considering information from 18 F-fluorodeoxyglucose positron emission tomography, contrast-enhanced computed tomography, and/or magnetic resonance imaging. For verification purposes, the imaging-based algorithm was applied to 100 consecutive patients whose WR was inconspicuous on clinical assessment. RESULTS: The imaging-based algorithm confirmed WR involvement only in four of the 14 patients. Of the remaining 10 patients, four had retropharyngeal lymph node involvement and six an inconspicuous WR. Applying the imaging-based algorithm to 100 consecutive patients with physiological appearance of their WR on clinical assessment, absence of WR involvement could be confirmed in 99. However, suspicion of WR involvement was raised in one patient. CONCLUSIONS: The imaging-based algorithm was feasible and easily applicable at initial staging of young patients with Hodgkin lymphoma. The aim of this study was to assess effects of balloon pulmonary angioplasty (BPA) on right ventricular (RV) mechanical dysfunction in patients with inoperable chronic thromboembolic pulmonary hypertension (CTEPH) via MRI.MRI at 1.5 Tesla and right heart catheterization were performed before and 6 months after BPA in 30 CTEPH patients (mean age 63.4 ± 10.6 years; 17 female). Feature-tracking strain analysis, including global longitudinal (GLS), circumferential (GCS), and radial (GRS) strain, was performed and compared with right ventricular function, myocardial remodelling (assessed by native T1 times), and pulmonary haemodynamics (mean pulmonary arterial pressure and pulmonary vascular resistance).RVEF (35.9% to 48.4%) increased and mPAP (42.1 mmHg to 33.1 mmHg) and PVR (551.8 to 377.7 dyn∙s/cmRV mechanical dysfunction, pulmonary haemodynamics, and myocardial remodelling are markedly improved by BPA. Moreover, RV strain values showed good correlations with RV function, pulmonary haemodynamics, and myocardial remodelling. Therefore, strain analysis might provide new insights regarding therapy outcome, monitoring, and prognosis.

On 31 December 2018, the new Radiation Protection Regulation came into effect in Germany and made the new Radiation Protection Act more concrete. The old Radiation Protection Regulation and X‑ray Regulation have thereby been replaced.The substantial modifications regarding the practical daily routine in radiology are summarized.Modifications and innovations of the New Radiation Protection Act and Regulation compared to the old Radiation Protection Regulation and X‑ray Regulation and accordances were evaluated. Thereby the main focus was in the relevance for workflow in clinical routine.The new legislation contains a number of regulations that provide crucial tools for implementation of radiation protection, quality assurance, and dose optimization. However, this also requires additional time and personnel.

The entry of the new Radiation Protection Act and new Radiation Protection Regulation into force in Germany created many changes for radiology with regard to the old Radiation Protection Regulation and X‑ray Regulation.The substantial modifications in radiology regarding the areas of approval and notification procedures, teleradiology, screening, research and radon in the workplace are summarized.Changes in the new Radiation Protection Act and Regulation compared to the old Radiation Protection Regulation and X‑ray Regulation were evaluated. Thereby, the focus was on areas beyond the workflow in clinical routine.The requirements for the approval and notification procedure have increased. For example, proof must be provided that a medical physics expert can be consulted. The establishment of deadlines for the process by the responsible authorities may accelerate the procedure and create planning certainty.

The testis as a site for atherosclerotic changes has so far attracted little attention. We used the apolipoprotein E (ApoE)/low density lipoprotein (LDL) receptor deficient mouse model (KO) for atherosclerosis (20, 40, 60 and 87-week-old) to investigate whether Leydig cells or the capillary network are responsible for reduced serum testosterone levels previously observed in extreme ages of this model. In KO mice, overall testosterone levels were reduced whereas the adrenal gland-specific corticosterone was increased excluding a general defect of steroid hormone production. In addition to micro-CT investigations for bigger vessels, stereology revealed a reduction of capillary length, volume and surface area suggesting capillary rarefaction as a factor for diminished testosterone. Stereological analyses of interstitial cells demonstrated significantly reduced Leydig cell numbers and size. These structural changes in the testis occurred on an inflammatory background revealed by qPCR. Reduced litter size of the KO mice suggests hypo- or infertility as a consequence of the testicular defects. Our data suggest reduced testosterone levels in this atherosclerosis model might be explained by both, rarefication of the capillary network and reduced Leydig cell number and size. Thus, this study calls for specific treatment of male infertility induced by microvascular damage through hypercholesterolemia and atherosclerosis.

Background In head and neck squamous cell carcinoma (HNSCC), the occurrence of concurrent lung malignancies poses a significant diagnostic challenge because metastatic HNSCC is difficult to discern from second primary lung squamous cell carcinoma (SCC). However, this differentiation is crucial because the recommended treatments for metastatic HNSCC and second primary lung SCC differ profoundly. Methods We analyzed the origin of lung tumors in 32 patients with HNSCC using human papillomavirus (HPV) typing and targeted next generation sequencing of all coding exons of tumor protein 53 (TP53). Results Lung tumors were clearly identified as HNSCC metastases or second primary tumors in 29 patients, thus revealing that 16 patients had received incorrect diagnoses based on clinical and morphological data alone. Conclusion The HPV typing and mutation analysis of all TP53 coding exons is a valuable diagnostic tool in patients with HNSCC and concurrent lung SCC, which can help to ensure that patients receive the most suitable treatment.

Motive External vibration excitation might be key to many novel non-surgical interventions for pathologies in the musculoskeletal system and in other parts of the human organism. Lack of understanding about vibration patterns, their controllability, and reproducibility are three limitations of ongoing research. This study establishes a bovine vibration model and animal model replacements for future research. Methods We used biological samples ( n = 5 ) and one polyurethane sample of the bovine femur. Mechanical resonance was measured experimentally and analysed numerically by finite element method. Main results The experiments obtained 5 distinct mode shapes for the biological sample set, with standard deviation 7.5%. Finite element analysis of the biological samples can replicate experimental mode shape deflection. The use of polyurethane changes resonance character but results are also good approximations of the biological samples. Conclusions A model of the bovine femur with consistent resonance behaviour is presented with alternatives (polyurethane and finite element analysis) that can serve in reducing the number of necessary biological samples. Future work will be to adapt results to human anatomy. Of clinical interest will be to influence bone pathologies such as post-surgical non-union, or bone functionality as part of haematopoiesis and endocrine secretion.

Die Rekonstruktion knocherner Defekte bei Revisionseingriffen von Endoprothesen mit dem Impaction-Bone-Grafting bietet die Moglichkeit, eine langfristige knocherne Integration mit guten klinischen Ergebnissen zu gewahrleisten. Allogener nativer spongioser Knochen kommt haufig fur dieses Verfahren zur Anwendung. Diese Studie untersucht die Auswirkungen der Thermodesinfektion auf das Impaktionsverhalten von Spongiosa verschiedener Geometrie und die Zementverteilung. Die Spongiosa wurde aus den Femurkopfen 7 Monate alter Schweine gewonnen. Eine Halfte des Kopfes wurde jeweils thermodesinfiziert und die andere verblieb nativ. Es wurden Spongiosachips der Grose 3–5 mm, 5–8 mm und 8–10 mm hergestellt. Die Impaktion in einem Zylindermodell mit 30 mm Innendurchmesser erfolgte mit standardisierter Krafteinwirkung durch einen Impaktor mit einem Fallgewicht von 1450 g. Die optimale Dichteverteilung von Mischungen verschiedener Partikelgrosen des nativen im Vergleich zum thermodesinfizierten Knochen wurde mittels Computertomographie untersucht. Ebenso wurde die Verteilung von Luft, Wasser und Fett gemessen. Die jeweils beste Partikelkombination wurde fur die nachfolgende computertomographische Messung der Zementverteilung und der Kontaktflache zum Knochen in verschiedenen Bereichen des Schafts in jeweils 7 Untersuchungen verwendet. Als statistische Methoden kamen zweifaktorielle Varianzanalysen mit Messwiederholung und Bonferroni-Korrektur, der LSD-post-hoc-Test und der Mann-Whitney-U-Test zur Anwendung. Die Irrtumswahrscheinlichkeit wurde mit α = 5 % angegeben und die statistische Analyse erfolgte mit der Software SPSS® fur Windows. Hinsichtlich der Dichteverteilung der Spongiosa und der Kompakta ergab sich entlang des Schafts kein signifikanter Unterschied zwischen thermodesinfizierter und nativer Spongiosa (p > 0,05). Impaktierte native Spongiosa zeigte weniger Lufteinschlusse, resultierend in einer insgesamt besseren Feststoffdichteverteilung im Vergleich zum thermodesinfizierten Knochen (p 0,05) auf. Im distalen Schaftbereich zeigte sich das Zementvolumen in Verbindung mit dem nativen Knochen signifikant groser und proximal waren mehr Lufteinschlusse im thermodesinfizierten Knochen nachweisbar. Die Zementpenetrationsflachen waren fur die native Spongiosa insgesamt signifikant groser (p < 0,001). Die Impaktion thermodesinfizierter und nativer Spongiosa zeigte eine grosere Deformation des prozessierten Knochens ohne signifikanten Unterschied der maximal erreichbaren Dichte. Das Zementvolumen und die Penetration des Zements waren bei nativer und prozessierter Spongiosa proximal ausgepragter. Die Zementverteilung war fur den nativen Knochen nach distal signifikant weiterreichend. Die Stabilisierung des Schafts erscheint somit distal zunehmend von der Dichte der impaktierten Spongiosa abhangig zu sein und korreliert proximal mehr mit der Penetration des Zementes in die porosere Spongiosa.

Mesenchymal tumors in childhood comprise benign and malign entities and differ regarding typical sites of manifestation, age peak and clinical symptoms. In some cases biopsy is mandatory. If nephroblastoma is a possible diagnosis, biopsy must be avoided by all means in order to avoid biopsy tract metastasis. Imaging is used to narrow the differential diagnosis, describe the exact extent of the lesion and deliver complete staging. In pediatric patients sonography and MRI are the most important imaging modalities. Low-dose CT of the thorax serves for assessment of possible pulmonary metastases.

Background MRI is attractive for guiding and monitoring interventional procedures due to its high intrinsic soft tissue contrast and the possibility to measure flow and cardiac function. Methods Technical solutions have been developed for all procedural steps including imaging guidance, MR-safe catheters and instruments and patient monitoring. This has led to widening of the clinical applications. Interventional MRI is becoming increasingly important for the treatment of patients suffering from malignant diseases. The detectability of masses and consequently their accessibility for biopsy is higher, compared to other modalities, due to the high intrinsic soft tissue contrast of MRI. Temperature-dependent sequences allow for minimally invasive and tissue-sparing ablation (A-0 ablation). Conclusion Interventional MRI has become established in the clinical routine for a variety of indications, including biopsies and tumor ablation. Since the economic requirement of covering costs by reimbursement is met and interventional MRI decreases the mortality and morbidity of interventional procedures, broader application of interventional MRI can be expected in the clinical routine in the future.
