1. Fundus Autofluorescence in Posterior and Panuveitis—An Under-Estimated Imaging Technique: A Review and Case Series

Abstract: Fundus autofluorescence (FAF) is a prompt and non-invasive imaging modality helpful in detecting pathological abnormalities within the retina and the choroid. This narrative review and case series provides an overview on the current application of FAF in posterior and panuveitis. The literature was reviewed for articles on lesion characteristics on FAF of specific posterior and panuveitis entities as well as benefits and limitations of FAF for diagnosing and monitoring disease. FAF characteristics are described for non-infectious and infectious uveitis forms as well as masquerade syndromes. Dependent on the uveitis entity, FAF is of diagnostic value in detecting disease and following the clinical course. Currently available FAF modalities which differ in excitation wavelengths can provide different pathological insights depending on disease entity and activity. Further studies on the comparison of FAF modalities and their individual value for uveitis diagnosis and monitoring are warranted.

Abstract: ObjectivesTo determine the risk of mortality and need for aortic valve replacement (AVR) in patients with low-flow low-gradient (LFLG) aortic stenosis (AS).MethodsA longitudinal multicentre study including consecutive patients with severe AS (aortic valve area [AVA] < 1.0 cm2) and normal left ventricular ejection fraction (LVEF). Patients were classified as: high-gradient (HG, mean gradient ≥ 40 mmHg), normal-flow low-gradient (NFLG, mean gradient < 40 mmHg, indexed systolic volume (SVi) > 35 ml/m2) and LFLG (mean gradient < 40 mmHg, SVi ≤ 35 ml/m2).ResultsOf 1,391 patients, 147 (10.5%) had LFLG, 752 (54.1%) HG, and 492 (35.4%) NFLG. Echocardiographic parameters of the LFLG group showed similar AVA to the HG group but with less severity in the dimensionless index, calcification, and hypertrophy. The HG group required AVR earlier than NFLG (p < 0.001) and LFLG (p < 0.001), with no differences between LFLG and NFLG groups (p = 0.358). Overall mortality was 27.7% (CI 95% 25.3–30.1) with no differences among groups (p = 0.319). The impact of AVR in terms of overall mortality reduction was observed the most in patients with HG (hazard ratio [HR]: 0.17; 95% CI: 0.12–0.23; p < 0.001), followed by patients with LFLG (HR: 0.25; 95% CI: 0.13–0.49; p < 0.001), and finally patients with NFLG (HR: 0.29; 95% CI: 0.20–0.44; p < 0.001), with a risk reduction of 84, 75, and 71%, respectively.ConclusionsParadoxical LFLG AS affects 10.5% of severe AS, and has a lower need for AVR than the HG group and similar to the NFLG group, with no differences in mortality. AVR had a lower impact on LFLG AS compared with HG AS. Therefore, the findings of the present study showed LFLG AS to have an intermediate clinical risk profile between the HG and NFHG groups.

Abstract: Aims To assess the progression of the disease and evolution of the main echocardiographic variables for quantifying AS in patients with severe low-flow low-gradient (LFLG) AS compared to other severe AS subtypes. Methods and results Longitudinal, observational, multicenter study including consecutive asymptomatic patients with severe AS (aortic valve area, AVA < 1.0 cm²) and normal left ventricle ejection fraction (LVEF ≥ 50%). Patients were classified according to baseline echocardiography into: HG (high gradient; mean gradient ≥ 40 mmHg), NFLG (normal-flow low-gradient; mean gradient < 40 mmHg, indexed systolic volume (SVi) > 35mL/m2), or LFLG (mean gradient < 40 mmHg, SVi ≤ 35 mL/m²). AS progression was analyzed by comparing patients’ baseline measurements and their last follow-up measurements or those taken prior to aortic valve replacement (AVR). Of the 903 included patients, 401 (44.4%) were HG, 405 (44.9%) NFLG, and 97 (10.7%) LFLG. Progression of the mean gradient in a linear mixed regression model was greater in low-gradient groups: LFLG vs. HG (regression coefficient 0.124, P = 0.005) and NFLG vs. HG (regression coefficient 0.068, P = 0.018). No differences were observed between the LFLG and NFLG groups (regression coefficient 0.056, P = 0.195). However, AVA reduction was slower in the LFLG group compared to the NFLG (P < 0.001). During follow-up, in conservatively-managed patients, 19.1% (n = 9) of LFLG patients evolved to having NFLG AS and 44.7% (n = 21) to having HG AS. In patients undergoing AVR, 58.0% (n = 29) of LFLG baseline patients received AVR with a HG AS. Conclusion LFLG AS shows an intermediate AVA and gradient progression compared to NFLG and HG AS. The majority of patients initially classified as having LFLG AS changed over time to having other severe forms of AS, and most of them received AVR with a HG AS.

Abstract: BACKGROUND: Among patients with stable coronary disease and moderate or severe ischemia, whether clinical outcomes are better in those who receive an invasive intervention plus medical therapy than in those who receive medical therapy alone is uncertain. METHODS: We randomly assigned 5179 patients with moderate or severe ischemia to an initial invasive strategy (angiography and revascularization when feasible) and medical therapy or to an initial conservative strategy of medical therapy alone and angiography if medical therapy failed. The primary outcome was a composite of death from cardiovascular causes, myocardial infarction, or hospitalization for unstable angina, heart failure, or resuscitated cardiac arrest. A key secondary outcome was death from cardiovascular causes or myocardial infarction. RESULTS: Over a median of 3.2 years, 318 primary outcome events occurred in the invasive-strategy group and 352 occurred in the conservative-strategy group. At 6 months, the cumulative event rate was 5.3% in the invasive-strategy group and 3.4% in the conservative-strategy group (difference, 1.9 percentage points; 95% confidence interval [CI], 0.8 to 3.0); at 5 years, the cumulative event rate was 16.4% and 18.2%, respectively (difference, -1.8 percentage points; 95% CI, -4.7 to 1.0). Results were similar with respect to the key secondary outcome. The incidence of the primary outcome was sensitive to the definition of myocardial infarction; a secondary analysis yielded more procedural myocardial infarctions of uncertain clinical importance. There were 145 deaths in the invasive-strategy group and 144 deaths in the conservative-strategy group (hazard ratio, 1.05; 95% CI, 0.83 to 1.32). CONCLUSIONS: Among patients with stable coronary disease and moderate or severe ischemia, we did not find evidence that an initial invasive strategy, as compared with an initial conservative strategy, reduced the risk of ischemic cardiovascular events or death from any cause over a median of 3.2 years. The trial findings were sensitive to the definition of myocardial infarction that was used. Abstract: Background Fiber is an important part of a healthy diet and is known to attenuate postprandial glycemia. Orange pomace (OP) is a by-product of orange juice (OJ) production and is a rich source of fiber. Objective Two separate studies determined the impact of added OP to 100% OJ on postprandial glycemic response compared with sugar-matched OJ or whole orange fruit (WOF). Methods Study 1 included 17 adults [65% female, age 39.3 ± 3.1 y, and BMI (in kg/m2) 24.6 ± 0.7], and study 2 included 45 different adults (47% female, age 25.1 ± 4.3 y, and BMI 22.5 ± 1.6). Studies were conducted at separate locations using a randomized, 3-arm, crossover design to test the glycemic response to sugar-matched OJ, OJ with 5 g fiber from OP (OPF), or WOF. The primary outcomes were 2-h glucose incremental area under the curve (iAUC) in study 1, analyzed by repeated measures ANOVA, and maximum glucose concentration (Cmax) in study 2, analyzed using PROC MIXED (ANCOVA). Glucose and insulin concentrations were measured at fasting and multiple time points over 2 h after test product consumption (study 1, serum; study 2, plasma). Results In study 1, glucose iAUC was not significantly lower in OPF compared to the OJ or WOF (825 ± 132 compared with 920 ± 132 and 760 ± 132 mg · min · dL-1, respectively, P = 0.57 for both). In study 2, glucose iAUC was significantly lower in WOF compared with OPF and OJ (689 ± 70.7 compared with 892 ± 70.7 and 974 ± 70.7 mg · min · dL-1, P = 0.02 and 0.001, respectively). Data from both studies indicated OPF reduced Cmax compared with OJ and that the reductions were comparable to WOF (study 1: OPF, 115 ± 4.06 compared with OJ, 124 ± 4.06 and WOF, 114 ± 4.06 mg · dL-¹, P = 0.002 and 0.75, respectively; study 2: OPF, 128 ± 1.92 compared with OJ, 136 ± 1.92 and WOF, 125 ± 1.92 mg · dL-¹, P = 0.001 and 0.28, respectively). Conclusion Data from both studies demonstrated no significant effect of OPF on postprandial iAUC compared with OJ. However, adding OP into OJ attenuates the postprandial glucose Cmax, and the responses were comparable to WOF in healthy adults.

10. Avocado Consumption for 12 Weeks and Cardiometabolic Risk Factors: A Randomized Controlled Trial in Adults with Overweight or Obesity and Insulin Resistance

Abstract: Diets emphasizing unsaturated fat and high fiber are associated with reducing cardiometabolic risk factors. Avocados are rich in MUFA and PUFA fats and fiber.Assess replacement of carbohydrate energy with avocado energy for 12 wk on glucose homeostasis and cardiometabolic risk factors in self-selecting free-living adults who are overweight or with obesity and have insulin resistance.In a single-center, randomized, 2-arm, controlled, 12-wk parallel trial, adults [n = 93; male/female: 39/54; mean ± SD age: 42 ± 12 y; BMI: 32.6 ± 3.9 (in kg/m2); HOMA-IR: 2.7 ± 1.7] were counseled to exchange avocado (AV) or control food (C; low fat, low fiber, energy matched) for carbohydrate food in their usual diet for 12 wk. The primary outcome was the change in Matsuda Insulin Sensitivity Index (MISI) after 12-wk interventions. Secondary outcomes were changes in fasting and post-oral glucose tolerance test glycemic variables, fasting lipids, endothelial activation and inflammation markers. Automated Self-Administered 24-h Dietary Assessment Tool captured weekly dietary intake. Intervention effects were mainly determined by ANCOVA using PC-SAS version 9.4.Dietary total, MUFA, and PUFA fat; fiber; and vegetable intake were higher in the AV group compared with the C group (P 0.05), and no change in body weight or composition was observed (P 0.05). Differences between the changes in MISI after AV compared with C were not different (Δ0-12 wk, P = 0.1092). Differences in fasting insulin (Δ0-12 wk, P = 0.0855) and improved glycated hemoglobin (Δ0-12 wk, P = 0.0632) after AV compared with C were suggested. C-reactive protein was significantly lower after AV compared with C at 12 wk (P = 0.0418). Select biomarkers of endothelial activation and lipoproteins by NMR were also influenced by AV compared with C food intake.Avocado intake was associated with a healthier dietary pattern and trends favoring improved glucose control and reduced biomarkers of cardiometabolic risk when replacing avocado energy for carbohydrate energy in free-living adults who are overweight or with obesity and have insulin resistance. Abstract: OBJECTIVES: Apple pomace (AP) is a byproduct of apple juice production and is a rich source of fiber. The goal of the present study was to determine the impact of adding AP to 100% apple juice (AJ) on the postprandial glycemic response compared to sugar matched AJ or whole apple fruit (WAF) in healthy human subjects. METHODS: Fifty-eight healthy subjects (aged 25 ± 3 years, BMI 22.9 ± 1.4 kg m(−2), mean ± SD) participated in a randomized, 3-arm, cross-over clinical trial to test the glycemic response to AJ, 235 g, AJ with 5 g fiber from AP added (APF, 235 g total beverage weight) or whole Red Delicious apple with skin (WAF, 230 g edible portion with skin). Fiber content of APF was matched to WAF and all study products were matched for available carbohydrates (AJ/APF/WAF, 24 g). Blood samples were collected to measure glucose and insulin concentrations at fasting (0 min) and multiple time points over 2 h after consuming study product. The primary end point was the difference in maximal glucose concentrations (C(max)) among study products. RESULTS: Results indicated that Cmax values were not significantly different between 3 products (AJ −121.2 ± 1.8 mg/dL vs. APF- 120.8 ± 1.8 mg dL-1 vs. WAF 121.3 ± 1.8 mg dL-1, respectively, P > 0.05). Time to reach maximal glucose concentration (T max) was delayed in APF compared to AJ and WAF (33.2 ± 0.8 min vs. 29.9 ± 0.8 and 29.6 ± 0.8, respectively, P 0.05. CONCLUSIONS: This study demonstrated that adding 5 g of fiber from AP into 235 g of AJ did not attenuate maximal postprandial glucose concentration. However, times to reach maximal glucose and insulin concentrations were significantly delayed in the group receiving pomace fiber added to apple juice compared to apple juice alone or whole apple fruit. Abstract: Resistant starch (RS) is a variant of starch that is indigestible by human enzymes and has been acknowledged for multiple physiological benefits including attenuation of postprandial glycemia when incorporated into foods. Distarch phosphate is a RS type 4 (RS4) containing phosphodiester cross-links within and between starch molecules. Considering the importance of RS to human health, the present study aimed to investigate the dose response effect of a novel RS4 (potato-derived distarch phosphate - (VERSAFIBE 1490™) on acute postprandial glycemic responses compared with energy/available carbohydrate and sugar-matched control. The study was designed as a controlled, single-center randomized, single-blinded, cross-over trial, in which 31 healthy adults consumed a baked breakfast cereal bar containing 0, 10 or 20 g RS4 followed by serial blood samples over two hours to determine glucose and insulin concentrations and calculate the incremental area under the curve (iAUC). Results suggest that the addition of RS4 did not reduce iAUC glucose or insulin responses significantly (P > 0.05) at the doses provided. No significant changes in the glucose or insulin maximum concentration (Cmax) and time to reach maximal glucose and insulin concentrations (Tmax) were observed (P > 0.05). Overall, this particular RS4 did not affect measures of glycemia in healthy individuals at doses provided in ready-to-eat baked-good.

Abstract: Because wearable technology is ubiquitous, it is important to determine validity and reliability not only in a laboratory setting, but applied environments where the general population utilizes the devices. The purpose of this study was to 1) determine intra-rater reliability of visual step count outdoors, 2) determine validity of commercially available wearable technology devices in this setting, and 3) report test-retest reliability of commercial devices during hiking and trail running. Individuals (N = 20) completed 5-min hikes and trail runs on a 200-m section of trail while wearing the following devices: Fitbit Surge 2, Garmin Vivosmart HR+, Leaf Health Tracker, Polar A360, Samsung Gear 2, Spire Activity Tracker, and Stryd Power Meter. Abstract: Anaplastic large cell lymphoma associated with breast implants is receiving increased attention. Most cases have been reported in Europe, North America (USA and Canada), Australia and New Zealand. Fewer cases have been reported in Latin America (including Mexico), Africa and Asia. This report was delivered during our national plastic surgery meeting in Cancun in May 2017. Before the meeting, two participants reviewed the literature. The review was performed using the following information sources: PubMed, Embase, Cochrane, Fisterra, Google Scholar and LILACS, with entries from 1980 to August 2015 in several languages (English, Spanish, French and Portuguese). The results were revealed during the meeting to the other participants. The consensus was divided into two parts. The first part included an open-ended question regarding the incidence and prevalence of the problem. The second part included clinical scenarios with different items that were rated by the participants. After this activity, accordance among the responses was evaluated. Seven cases were reported during the meeting (3 from Mexico, 3 from Chile and 1 from Argentina). Fifty percent of the participants reported consulting with guidelines and clinical centers to help with potential cases. Most agreed that further studies must be done in cases of chronic seroma where the capsule plays an important role. A current debate exists about the incidence of this problem in Latin America because we did not report the same number of cases as Europe, Australia or North America. More studies are required to determine the differences among reports in Latin America. Most representatives agreed that further studies must be done. Concern is increasing, and the problem is known. Other factors involved may be considered, and the problem must not be ignored. This journal requires that authors assign a level of evidence to each article. The goal of the present study was to determine the impact of the addition of OP to 100% orange juice (OJ) on the postprandial glycemic response compared to the glycemic response to a sugar matched OJ or whole orange fruit (WOF). METHODS: Forty-five healthy subjects (aged 25 ± 4 years, BMI 23 ± 2 kg m(−2), mean ± SD) participated in a randomized, 3-arm, cross-over clinical trial to test the glycemic response to OJ, 250 g, OJ with 5 g fiber from OP added (OPF, 257 g total beverage weight or Navel variety whole orange (WOF, 227 g edible portion). The fiber level was chosen because it is similar to the amount found in sugar-matched weight of WOF. All 3 study products were matched for available carbohydrates (OJ/OPF/WOF, 19.3 g). Blood samples were collected and glucose and insulin concentrations were measured at fasting (0 min) and at multiple time points over 2 h after consuming study product. The primary end point was to assess and compare maximal glucose concentrations (C(max)) among study products. RESULTS: OPF and WOF significantly attenuated glucose C(max) compared to OJ (127.7 ± 1.9 and 125.1 ± 1.9 mg dL(−1) vs. 136.1 ± 1.9 mg dL(−1), respectively, P 0.05. Abstract: A variety of wearable technology devices report heart rate. Heart rate sensing smart bras are manufactured for females who participate in activity, however accuracy has not been determined. The purpose was to determine the validity of heart rate measures in three commercially available sports bras during walking and running. Twenty-four healthy females completed bouts of treadmill exercise. The Adidas Smart sports bra, Berlei sports bra, and Sensoria Fitness biometric sports bra were tested. Participant perception of each garment was obtained immediately after the participant divested the sports bra. The Adidas Smart sports bra was valid only during rest (Intraclass correlation Coefficient [ICC] = 0.79, mean absolute percentage error [MAPE] = 4.5%, Limits of Agreement [LoA]=−8 to 8). The Berlei sports bra was valid across all conditions (ICC = 0.99, MAPE = 0.66%, LoA = −19 to 19), and the Sensoria biometric bra was valid during rest and walking (ICC = 0.96, MAPE = 1.9%, LoA = −15 to 12). Perception of the smart sports bras was higher for the Adidas Smart sports bra and Sensoria Fitness sports bra, and lower for the Berlei sports bra. Sports bra manufacturers designing wearable technology garments must consider the ability of returning accurate biometric data while providing necessary function and comfort to females engaging in physical activity.

Abstract: The increased prevalence of anorexia nervosa reported in non-Western societies inevitably raises the issue of the influence of cultural factors in the genesis and the patterns of this disorder. Anorexia nervosa is not a straightforward Western culture-bound syndrome, although an influence of Western ideals of thinness does exist. The illness seems more related to rapid cultural shifts, either societal or individual, such as those occurring in the migratory process. Migrants and their children have to face the acculturation process and may experience a culture-clash. The pathology can also fulfil a positive acculturative function. This is a case study concerning a second-generation Chinese girl born in France presenting with anorexia nervosa. This case leads us to raise the issue of the choice of diagnostic criteria in relation to cultural background. We will also discuss the impact of the family’s migratory history on the construction of identity in adolescence. Finally we will explore the specific features of care provision for anorexia nervosa in a transcultural setting.

Abstract: En este trabajo de tesis hemos identificado los defectos puntuales en películas, micro- y nanoestructuras de GaN formados por la incorporación de impurezas, que participan (o inhiben) en la generación de ferromagnetismo (FM) en este material. Para realizar este estudio se sintetizaron películas, micro- y nanoestructuras de GaN impurificas con Mn, O y Cu usando la técnica de evaporación térmica (TE). Para identificar los defectos puntuales en estos materiales se realizaron diversos estudios, tales como: catodoluminiscencia (CL), espectroscopia de fotoelectrones emitidos por rayos X (XPS) y microscopía electrónica de transmisión de alta resolución (HRTEM). Su respuesta magnética se estudió usando un sistema SQUID operado a 100 K. Nuestros resultados mostraron que la temperatura de síntesis del GaN influyó significativamente tanto en la morfología de las micro- y nanoestructuras obtenidas, así como en la concentración de las impurezas incorporadas. Además de que la síntesis del GaN en un ambiente rico en N propició la generación de defectos tipo vacancias de galio (VGa). La incorporación de oxígeno en las nanoestructuras de GaN generó múltiples fallas de apilamiento atómico, las cuales además de reducir su calidad cristalina inhibieron sus propiedades ferromagnéticas incluso al ser codopadas con Mn. De acuerdo al estudio por XPS, las impurezas de oxígeno se incorporaron en la red de forma intersticial (Oi) y substitutional (ON). Los resultados de CL mostraron que esta impureza genera una emisión luminiscente de 2.68 eV, asociada a una transición entre niveles electrónicos producidos por defectos puntuales tipo ON (donador) y Oi (aceptor). Por otro lado, hemos encontrado que las impurezas de Mn y Cu se incorporan en la red cristalina del GaN en forma substitutional (MnGa, CuGa), en estado químico divalente (+2). En las muestras impurificadas con Mn se observó una banda ancha con varias componentes centradas en 1.9, 2.6 y 2.8 eV asociadas a transiciones entre estados introducidos en la banda prohibida por los iones de Mn y las bandas de conducción y valencia. Una de las conclusiones más importantes de este trabajo de tesis es que la incorporación de impurezas sustitucionales de oxígeno (ON) promueve un incremento en la concentración de defectos tipo vacancias de nitrógeno (VN) en el GaN que inhiben la generación de FM. En cambio la presencia de impurezas tipo MnGa y CuGa incrementa la generación de defectos tipo VGa. The magnetic response of the samples was studied by SQUID at 100 K. Results indicate that deposition temperature during synthesis of GaN significantly influenced the morphology of the obtained structures and the concentration percentage of the incorporated impurity. Besides, the synthesis of GaN in an N-rich environment generated gallium vacancy defects (VGa). The incorporation of O into GaN nanowires generated multiple stacking fault defects that diminished the purity of the GaN crystal structure. We also observed that O inhibits the generation of FM even in samples codoped with Mn. According to XPS results we identified that O incorporates into the GaN lattice as an interstitial (Oi) and substitutional (ON) impurity. CL spectra showed the generation of a luminescent component centered at 2.68 eV associated to the transitions between electronic states produced by ON and Oi point defects. On the other hand, we found that Mn and Cu impurities incorporate in the crystalline lattice of GaN as substitutional defects (MnGa, CuGa) in divalent state (+2). In CL spectra, a broad band with multiple components centered at 1.9, 2.6 and 2.8 eV was associated to transitions between states introduced by Mn ions in the forbidden gap and the valence (conduction) band of GaN. An important conclusion of this thesis is that ON impurities increase the generation of nitrogen vacancies (VN) that inhibit the generation of FM. Meanwhile, the presence of MnGa and CuGa increased the formation of VGa defects. We also proved that VGa participates in the generation of FM in GaN independently of sample morphology (film, micro or nanostructure). The magnetic and structural defect properties observed in GaN were correlated to those observed for MnO nanowires, also synthesized by TE over GaN microcrystals. Finally, by CL we found that MnO has similar defects to those observed for GaN codoped with Mn and O. In particular, these MnO nanowires presented a clear FM response

Abstract: Systemic lupus erythematosus (SLE) patients display an increased risk of cardiovascular disease (CVD). With the improved clinical management of other classical severe manifestation of the disease, CVD is becoming one of the most relevant complications of SLE, and it is an important factor causing morbidity and mortality. Several immune constituents have been shown to be involved in the pathogenesis of atherosclerosis and endothelial damage in SLE patients, including specific circulating cell populations, autoantibodies, and inflammatory mediators. In this review, we summarize the presentation of CVD in SLE and the role of the autoimmune responses present in SLE patients in the induction of atherogenesis, endothelial impairment and cardiac disease. Additionally, we discuss the utility of these immune mediators as early CVD biomarkers and targets for clinical intervention in SLE patients.

Abstract: Objective We compared the 1-year predictive value of several inflammatory and non-inflammatory biomarkers in ACS patients. Methods In 610 patients (73.0% male) – 36.0% unstable angina (UA) and 64.0% NSTEMI – we assessed high-sensitivity C-reactive protein (hs-CRP), interleukins 6, 10 and 18, soluble CD40 ligand, P- and E-selectin, NT-proBNP, fibrinogen and cystatin C at hospital admission. Two outcomes at 1-year follow up were selected for analysis: (1) all-cause death, MI, UA, or coronary revascularization, and (2) all-cause death, and non-fatal MI. The effect of biomarker levels on endpoints was examined by the Cox proportional hazards model, and their discrimination ability with the C statistic (AUC). Results Of 549 patients (90.0%) who completed the 1-year follow up, 206 (37.5%) and 54 (8.9%) reached the first and second composite endpoints, respectively. None of the biomarkers studied improved prediction of the first endpoint. However, considered as continuous variables, and in combination, NT-proBNP and fibrinogen, increased the AUC from 0.64 (95% CI 0.55–0.72) to 0.73 (95% CI 0.64–0.81; p =0.02) for prediction of the second endpoint. Cut-off values for NT-proBNP and fibrinogen, regarding best sensitivity and specificity for prediction of the secondary endpoint were 1043.9ng/L and 4.47mg/dL, respectively. For these cut-off points, sensitivity, specificity, positive predictive value and negative predictive value were 40.5% vs 59.5%, 83.3% vs 67.1%, 18.8% vs 14.9% and 93.5% vs 94.4% for NT-proBNP and fibrinogen, respectively. Conclusion In ACS patients, inflammatory biomarkers offer modest incremental information to that provided by clinical risk markers. Fibrinogen and NT-proBNP measurements, however, improve cardiovascular risk prediction.

Abstract: RESUMEN Introducción y objetivos: El objetivo del estudio fue investigar si la evaluación no invasiva del índice de calcificación coronaria mediante tomografía computarizada cardiaca multidetector (TCMD) puede predecir la necesidad de una aterectomía rotacional (AR) electiva durante la intervención coronaria percutánea. Métodos: Se incluyeron pacientes diagnosticados de estenosis coronaria grave con placas moderadamente o gravemente calcificadas durante la angiografía coronaria. Esos pacientes se sometieron a la cuantificación del índice de calcificación coronaria con la escala de Agatston utilizando TCMD y posteriormente a intervención percutánea. Solo fueron tratadas con AR las lesiones que se consideraba que no era posible cruzar ni dilatar, según un protocolo de revascularización prediseñado. Ninguno de los operadores conocía de antemano los resultados de la TCMD. Según el protocolo del estudio, en el análisis estadístico se incluyeron variables clínicas, angiográficas y relacionadas con la puntuación Agatston. Durante el seguimiento se estudiaron los resultados a corto y largo plazo en ambos grupos. Resultados: Se analizaron 40 pacientes: 20 que recibieron AR y 20 con intervención coronaria percutánea convencional. La mayoría se incluyó después de un síndrome coronario agudo y tenían una anatomía coronaria compleja (puntuación media de la escala Syntax de 25 puntos). La creatinina y la puntuación de Agatston por lesión fueron los únicos factores predictivos de la AR. No se observaron diferencias significativas en el pronóstico dentro del hospital o a largo plazo. Un nuevo parámetro, el índice CAC-Cre, fue útil para predecir la necesidad de AR. Conclusiones: El análisis de la calcificación de las arterias coronarias mediante la puntuación de Agatston mejora la evaluación no invasiva de las placas coronarias complejas antes de la intervención coronaria percutánea. La puntuación de Agatston por lesión, la creatinina sérica y el índice CAC-Cre son parámetros útiles para predecir la necesidad de una AR electiva durante la intervención coronaria percutánea.

Abstract: ABSTRACT Introduction and objectives: This study aims to investigate if the non-invasive assessment of coronary calcium score using multislice cardiac computerized tomography (MSCT) may anticipate the need for elective rotational atherectomy (RA) during percutaneous coronary intervention. Methods: Patients were considered eligible for the study after receiving a diagnosis of severe coronary stenosis with moderate or severely calcified plaques during index coronary angiography. Those patients underwent the Agatston coronary artery calcium (CAC) score quantification using the MSCT and then underwent percutaneous intervention. Only those lesions considered non-crossable or non-dilatable according to a pre-specified revascularization protocol were treated with RA. All operators were blinded to the MSCT results. According to the study protocol, clinical, angiographic and Agatston-related variables were included in the statistical analysis. Short and long-term outcomes were investigated in both treatment groups during follow-up. Results: A total of 40 patients were included in the analysis: 20 underwent RA and 20 conventional percutaneous coronary interventions. Most patients were included after suffering from an acute coronary syndrome and had complex coronary anatomy (mean Syntax score, 25 points). The logistic regression analysis showed that creatinine levels and the per-lesion Agatston score were the only predictors of RA. No significant differences were observed regarding in-hospital or long-term procedural outcomes. A novel parameter, the CAC-Cre index, was found to be useful to anticipate the need for RA. Conclusion: Coronary artery calcification analysis using the Agatston score is a simple technique that improves the non-invasive assessment of complex coronary plaques prior to percutaneous coronary intervention. The per-lesion Agatston score, serum creatinine levels, and the CAC-Cre index may become useful parameters to anticipate the need for elective RAs during percutaneous coronary intervention.

Abstract: Summary: During heart regeneration in the zebrafish, fibrotic tissue is replaced by newly formed cardiomyocytes derived from preexisting ones. It is unclear whether the heart is composed of several cardiomyocyte populations bearing different capacity to replace lost myocardium. Here, using sox10 genetic fate mapping, we identify a subset of preexistent cardiomyocytes in the adult zebrafish heart with a distinct gene expression profile that expanded after cryoinjury. Genetic ablation of sox10+ cardiomyocytes impairs cardiac regeneration, revealing that these cells play a role in heart regeneration. : Unlike adult mammals, zebrafish regenerate their heart after injury through proliferation of preexistent cardiomyocytes. Abstract: A 12-year-old boy suffered from a full-thickness traumatic amputation of his lower lip and chin following a horse bite. Microsurgical technique was used to reanastomose the inferior labial artery and a vein of the chin. The replanted flap remained viable, and the patient has done well despite some early problems with eating and drooling. The patient is now able to purse his lips and has regained sensation and the use of his orbicularis oris and musculus mentalis, even though no attempt was made to repair the motor nerves or sensory nerves. Because of the potential superior cosmetic and functional results following replantation, we recommend aggressive microsurgical attempts at arterial and venous anastomosis not previously described following traumatic amputation. The inferior labial artery may be considered for use as a nutrient artery for replantation and in future elective maxillofacial reconstruction and free-flap transfer.

Abstract: After myocardial infarction in humans, lost cardiomyocytes are replaced by an irreversible fibrotic scar. In contrast, zebrafish hearts efficiently regenerate after injury. Complete regeneration of the zebrafish heart is driven by the strong proliferation response of its cardiomyocytes to injury. Here we show that, after cardiac injury in zebrafish, telomerase becomes hyperactivated, and telomeres elongate transiently, preceding a peak of cardiomyocyte proliferation and full organ recovery. Using a telomerase-mutant zebrafish model, we found that telomerase loss drastically decreases cardiomyocyte proliferation and fibrotic tissue regression after cryoinjury and that cardiac function does not recover. The impaired cardiomyocyte proliferation response is accompanied by the absence of cardiomyocytes with long telomeres and an increased proportion of cardiomyocytes showing DNA damage and senescence characteristics. These findings demonstrate the importance of telomerase function in heart regeneration and highlight the potential of telomerase therapy as a means of stimulating cell proliferation upon myocardial infarction.

Abstract: Extensive scalping injuries offer a unique challenge for tissue coverage because of the wide expanse of bone and lack of deep soft tissue or significant perforating vessels. For smaller injuries, pedicle flaps offer ideal coverage. Larger defects can be covered by omental flaps. Coverage with a free muscle flap followed by split-thickness skin grafting offers optimal long-term coverage. Two new techniques are introduced. The wire-button technique offers stabilization, and the halo frame provides good support and protection for a new free-flap graft and may increase the success rate of flaps in patients with scalping injuries.

Abstract: This is the first case report of the clinical use of intraoperative streptokinase to promote free flap salvage. A latissimus dorsi free flap was mobilized to cover a scalping type injury. After 4 1/2 hours of ischemia and recurrent thrombosis, streptokinase was perfused into the thoracodorsal artery (7,500 units of streptokinase in 30 cc of normal saline). The free flap was exposed to this concentration of streptokinase for 10 minutes followed by drainage of the venous effluent in order to avoid possible deleterious systemic effects of the streptokinase. Good flow throughout the free flap resulted, and the flap remained viable, providing good coverage for the patient's skull. Controversies regarding the no-reflow phenomena and the use of various thrombolytic agents are discussed.

Abstract: Chronic upper extremity arterial insufficiency is rare. Consequently, major reports specifically limited to the topic are scarce, and the clinical experience is small. In addition, symptomatology, diagnostic criteria, and guidelines for surgical management remain ill-defined. In the lower extremities, however, in situ vein bypass has been attempted for nearly three decades. This technique offers many advantages over traditional revascularization methods. Although the procedure has become popular for the lower extremity, no report of its use in the upper extremity is found in the literature. We report what may be the first case in which in situ bypass was used in the upper extremity for a threatened limb secondary to diabetic occlusive vascular disease complicated by a previous shunt used for hemodialysis. Revascularization of the upper extremity using the in situ vein bypass technique may offer a new alternative to traditional methods of revascularization.
