INTRODUCTION: The various rectal endoluminal radiation techniques all have steep, but different, dose gradients. In rectal contact brachytherapy (CXB) doses are typically prescribed and reported to the applicator surface and not to the gross tumor volume (GTV), clinical target volume (CTV) or organs at risk (OAR), which is crucial to understand tumor response and toxicity rates. To quantify the above-described problem, we performed a dose modeling study using a fixed prescription dose at the surface of the applicator and varied tumor response scenarios.METHODS: Endorectal ultrasound-based 3D-volume-models of rectal tumors and the rectal wall were used to simulate the delivered dose to GTV, CTV and the rectal wall layers, assuming treatment with Maastro HDR contact applicator for rectal cancer with a fixed prescription dose to the applicator surface (equivalent to 3 × 30 Gy CXB) and various response scenarios.RESULTS: An identical prescribed dose to the surface of the applicator resulted in a broad range of doses delivered to the GTV, CTV and the uninvolved intestinal wall. For example, the equieffective dose in 2 Gy per fraction (EQD2) D90% of the GTV varied between 63 and 231 Gy, whereas the EQD2 D2cc of the rectal wall varied between 97 and 165 Gy.CONCLUSION: Doses prescribed at the surface are not representative of the dose received by the tumor and the bowel wall. This stresses the relevance of dose reporting and prescription to GTV and CTV volumes and OAR in order to gain insight between delivered dose, local control and toxicity and to optimize treatment protocols.

PURPOSE: Even though High Dose Rate (HDR) brachytherapy has good treatment outcomes in different treatment sites, treatment verification is far from widely implemented because of a lack of easily available solutions. Previously it has been shown that an imaging panel (IP) near the patient can be used to determine treatment parameters such as the dwell time and source positions in a single material pelvic phantom. In this study we will use a heterogeneous head phantom to test this IP approach, and simulate common treatment errors to assess the sensitivity and specificity of the error-detecting capabilities of the IP. METHODS AND MATERIALS: A heterogeneous head-phantom consisting of soft tissue and bone equivalent materials was 3D-printed to simulate a base of tongue treatment. An High Dose Rate treatment plan with 3 different catheters was used to simulate a treatment delivery, using dwell times ranging from 0.3 s to 4 s and inter-dwell distances of 2 mm. The IP was used to measure dwell times, positions and detect simulated errors. Measured dwell times and positions were used to calculate the delivered dose. RESULTS: Dwell times could be determined within 0.1 s. Source positions were measured with submillimeter accuracy in the plane of the IP, and average distance accuracy of 1.7 mm in three dimensions. All simulated treatment errors (catheter swap, catheter shift, afterloader errors) were detected. Dose calculations show slightly different distributions with the measured dwell positions and dwell times (gamma pass rate for 1 mm/1% of 96.5%). CONCLUSIONS: Using an IP, it was possible to verify the treatment in a real-istic heterogeneous phantom and detect certain treatment errors. (c) 2022 The Au-thors. External beam radiotherapy with photon beams is a highly accurate treatment modality, but requires extensive quality assurance programs to confirm that radiation therapy will be or was administered appropriately. In vivo dosimetry (IVD) is an essential element of modern radiation therapy because it provides the ability to catch treatment delivery errors, assist in treatment adaptation, and record the actual dose delivered to the patient. However, for various reasons, its clinical implementation has been slow and limited. The purpose of this report is to stimulate the wider use of IVD for external beam radiotherapy, and in particular of systems using electronic portal imaging devices (EPIDs). After documenting the current IVD methods, this report provides detailed software, hardware and system requirements for in vivo EPID dosimetry systems in order to help in bridging the current vendor-user gap. The report also outlines directions for further development and research. In vivo EPID dosimetry vendors, in collaboration with users across multiple institutions, are requested to improve the understanding and reduce the uncertainties of the system and to help in the determination of optimal action limits for error detection. Finally, the report recommends that automation of all aspects of IVD is needed to help facilitate clinical adoption, including automation of image acquisition, analysis, result interpretation, and reporting/documentation. With the guidance of this report, it is hoped that widespread clinical use of IVD will be significantly accelerated.

A joint working group was created by the American Association of Physicists in Medicine (AAPM), the European Society for Radiotherapy and Oncology (ESTRO), and the Australasian Brachytherapy Group (ABG) with the charge, among others, to develop a set of well-defined test case plans and perform calculations and comparisons with model-based dose calculation algorithms (MBDCAs). Its main goal is to facilitate a smooth transition from the AAPM Task Group No. 43 (TG-43) dose calculation formalism, widely being used in clinical practice for brachytherapy, to the one proposed by Task Group No. 186 (TG-186) for MBDCAs. To do so, in this work a hypothetical, generic high-dose rate (HDR)A generic HDRThe local dose differences among MC codes are comparable to the statistical uncertainties of the reference datasets for the "source centered in water" and "source displaced" test cases and for the clinically relevant part of the unshielded volume in the "source centered in applicator" case. Larger local differences appear in the shielded volume or at large distances. Considering clinically relevant regions, global dose differences are smaller than the local ones. The most disadvantageous case for the MBDCAs is the one including the shielded applicator. In this case, ACUROS agrees with MC within [-4.2%, +4.2%] for the majority of voxels (95%) while presenting dose differences within [-0.12%, +0.12%] of the dose at a clinically relevant reference point. For ACE, 95% of the total volume presents differences with respect to MC in the range [-1.7%, +0.4%] of the dose at the reference point.The combination of the generic source and generic shielded applicator, together with the previously developed test cases and reference datasets (available in the Brachytherapy Source Registry), lay a solid foundation in supporting uniform commissioning procedures and direct comparisons among treatment planning systems for HDR

Tratamentos braquiterápicos são comumente realizados conforme o relatório da American Association of Physicists in Medicine (AAPM), Task Group report TG-43U1, o qual define o formalismo para cálculo de dose absorvida na água e não considera a composição dos materiais, densidades, dimensões do paciente e o efeito dos aplicadores. Estes efeitos podem ser significantes, conforme descrito pelo recente relatório da AAPM, Task Group report TG- 186, que define diretrizes para que sistemas de planejamento modernos, capazes de considerar as complexidades descritas acima, sejam implementados. Esta tese tem como objetivo contribuir para o aumento da exatidão dos planejamentos de tratamento braquiterápicos, seguindo as recomendações do TG-186 e indo além do mesmo. Um software foi desenvolvido para integrar planejamentos de tratamento e simulações pelo método de Monte Carlo (MC); modelos acurados, CAD-Mesh, foram utilizados para representar aplicadores braquiterápicos; Grandezas utilizadas para reportar dose absorvida, Dw,m (dose para água no meio) e Dm,m (dose para o meio no meio), foram calculadas para um tratamento de cabeça e pescoço, considerando a teoria para pequenas (SCT small cavity theory) e grandes cavidades (LCT large cavity theory); a componente da dose em razão do movimento da fonte foi avaliada para tratamentos de próstata e ginecológicos. Perfis de velocidade obtidos na literatura foram utilizados; medidas de velocidade de uma fonte braquiterapica foram realizadas com uma câmera de alta taxa de aquisição. Cálculos de dose obtidos usando MC (incluindo a composição e densidade dos tecidos, ar e o aplicador) mostram sobredoses de aproximadamente 5% dentro do volume alvo, em um tratamento ginecológico, quando comparados aos resultados obtidos com um meio homogêneo de água. Por sua vez, subdoses de aproximadamente 5% foram observadas ao considerar a composição dos tecidos e regiões com ar em um tratamento intersticial de braço. Um aplicador cilíndrico oco resultou na sobredose observada no caso ginecológico, ressaltando a necessidade de modelos acurados para representar os aplicadores. Os modelos CAD-Mesh utilizados incluem um aplicador Fletcher-Williamson, com blindagem, e um balão deformável para irradiação de mama. Os resultados obtidos com estes modelos são equivalentes aos obtidos com modelos geométricos convencionais. Este recurso pode ser conveniente para aplicadores complexos e/ou quando o projeto dos aplicadores for disponibilizado pelo fabricante. Cálculos de dose, com a composição real dos tecidos humanos, podem apresentar diferenças significativas em razão da grandeza adotada. Diferenças entre Dm,m e Dw,m (SCT ou LCT) chegam a 14% em razão da composição do osso. A metodologia adotada (SCT ou LCT) resulta em diferenças de até 28% para o osso e 36% para os dentes. A componente de dose de trânsito também pode levar a diferenças significativas, uma vez que baixas velocidades ou movimentos uniformemente acelerados foram descritos na literatura. Considerando a pior condição e sem incluir nenhuma correção no tempo de parada, a dose de trânsito pode chegar a 3% da dose prescrita para um caso ginecológico, com 4 cateteres, e até 11.1% da dose prescrita para um tratamento de próstata, com 16 cateteres. A dose de trânsito para a fonte avaliada (velocidade obtida experimentalmente) não é uniformemente distribuída e pode levar a sub ou sobredoses de até 1.4% das doses comumente prescritas (310 Gy). Os tópicos estudados são relevantes para tratamentos braquiterápicos e podem contribuir para o aumento de sua acurácia. Os efeitos estudados podem ser avaliados com o uso do software, associado a um código MC, desenvolvido. Brachytherapy treatments are commonly performed using the American Association of Physicists in Medicine (AAPM) Task Group report TG-43U1 absorbed dose to water formalism, which neglects human tissue densities, material compositions, body interfaces, body shape and dose perturbations from applicators. The significance of these effects has been described by the AAPM Task Group report TG-186 in published guidelines towards the implementation of Treatment Planning Systems (TPS) which can take into account the above mentioned complexities. This departure from the water kernel based dose calculation approach requires relevant scientific efforts in several fields. This thesis aims to improve brachytherapy treatment planning accuracy following TG-186 recommendations and going beyond it. A software has been developed to integrate clinical treatment plans with Monte Carlo (MC) simulations; high fidelity CAD-Mesh geometry was employed to improve brachytherapy applicators modelling; different dose report quantities, Dw,m (dose to water in medium) and Dm,m (dose to medium in medium), were obtained for a head and neck case using small cavity theory (SCT) and large cavity theory (LCT); the dose component due to the source moving within the patient was evaluated for gynecological and prostate clinical cases using speed profiles from the literature. Moreover, source speed measurements were performed using a high speed camera. Dose calculations using MC showed overdosing around 5% within the target volume for a gynecological case comparing results obtained including tissue, air and applicator effects against a homogeneous water phantom. On the other hand, the same comparison showed underdosing around 5% when including tissue and air composition for an interstitial arm case. A hollow cylinder applicator was responsible for the overdosing observed for the gynecological case highlighting the importance of accurate applicator modelling. The evaluated CAD-Mesh applicators models included a Fletcher- Williamson shielded applicator and a deformable balloon used for accelerated partial breast irradiation. Results obtained were equivalent to ones obtained with conventional constructive solid geometry and may be convenient for complex applicators and/or when manufacturer CAD models are available. Differences between Dm,m and Dw,m (SCT or LCT) are up to 14% for bone in a evaluated head and neck case. The approach (SCT or LCT) leads to differences up to 28% for bone and 36% for teeth. Differences can also be significant due to the source movement since some speed profiles from literature show low source speeds or uniform accelerated movements. Considering the worst case scenario and without include any dwell time correction, the transit dose can reach 3% of the prescribed dose in a gynecological case with 4 catheters and up to 11.1% when comparing the average prostate dose for a case with 16 catheters. The transit dose for a high speed (measured with a video camera) source is not uniformly distributed leading to over and underdosing, which is within 1.4% for commonly prescribed doses (310 Gy). The main subjects evaluated in this thesis are relevant for brachytherapy treatment planning and can improve treatment accuracy. Many of the issues described in here can be assessed with the software, coupled with a MC code, developed in this work.

In order to facilitate a smooth transition for brachytherapy dose calculations from the American Association of Physicists in Medicine (AAPM) Task Group No. 43 (TG-43) formalism to model-based dose calculation algorithms (MBDCAs), treatment planning systems (TPSs) using a MBDCA require a set of well-defined test case plans characterized by Monte Carlo (MC) methods. This also permits direct dose comparison to TG-43 reference data. Such test case plans should be made available for use in the software commissioning process performed by clinical end users. To this end, a hypothetical, generic high-dose rate (HDR) (192)Ir source and a virtual water phantom were designed, which can be imported into a TPS.A hypothetical, generic HDR (192)Ir source was designed based on commercially available sources as well as a virtual, cubic water phantom that can be imported into any TPS in DICOM format. The dose distribution of the generic (192)Ir source when placed at the center of the cubic phantom, and away from the center under altered scatter conditions, was evaluated using two commercial MBDCAs [Oncentra(®) Brachy with advanced collapsed-cone engine (ACE) and BrachyVision ACUROS™ ]. Dose comparisons were performed using state-of-the-art MC codes for radiation transport, including ALGEBRA, BrachyDose, GEANT4, MCNP5, MCNP6, and PENELOPE2008. The methodologies adhered to recommendations in the AAPM TG-229 report on high-energy brachytherapy source dosimetry. TG-43 dosimetry parameters, an along-away dose-rate table, and primary and scatter separated (PSS) data were obtained. The virtual water phantom of (201)(3) voxels (1 mm sides) was used to evaluate the calculated dose distributions. Two test case plans involving a single position of the generic HDR (192)Ir source in this phantom were prepared: (i) source centered in the phantom and (ii) source displaced 7 cm laterally from the center. Datasets were independently produced by different investigators. MC results were then compared against dose calculated using TG-43 and MBDCA methods.TG-43 and PSS datasets were generated for the generic source, the PSS data for use with the ace algorithm. The dose-rate constant values obtained from seven MC simulations, performed independently using different codes, were in excellent agreement, yielding an average of 1.1109 ± 0.0004 cGy/(h U) (k = 1, Type A uncertainty). MC calculated dose-rate distributions for the two plans were also found to be in excellent agreement, with differences within type A uncertainties. Differences between commercial MBDCA and MC results were test, position, and calculation parameter dependent. On average, however, these differences were within 1% for ACUROS and 2% for ace at clinically relevant distances.A hypothetical, generic HDR (192)Ir source was designed and implemented in two commercially available TPSs employing different MBDCAs. Reference dose distributions for this source were benchmarked and used for the evaluation of MBDCA calculations employing a virtual, cubic water phantom in the form of a CT DICOM image series. The implementation of a generic source of identical design in all TPSs using MBDCAs is an important step toward supporting univocal commissioning procedures and direct comparisons between TPSs.

Dose calculation in high dose rate brachytherapy with (192)Ir is usually based on the TG-43U1 protocol where all media are considered to be water. Several dose calculation algorithms have been developed that are capable of handling heterogeneities with two possibilities to report dose: dose-to-medium-in-medium (Dm,m) and dose-to-water-in-medium (Dw,m). The relation between Dm,m and Dw,m for (192)Ir is the main goal of this study, in particular the dependence of Dw,m on the dose calculation approach using either large cavity theory (LCT) or small cavity theory (SCT). A head and neck case was selected due to the presence of media with a large range of atomic numbers relevant to tissues and mass densities such as air, soft tissues and bone interfaces. This case was simulated using a Monte Carlo (MC) code to score: Dm,m, Dw,m (LCT), mean photon energy and photon fluence. Dw,m (SCT) was derived from MC simulations using the ratio between the unrestricted collisional stopping power of the actual medium and water. Differences between Dm,m and Dw,m (SCT or LCT) can be negligible (1%) for some tissues e.g. muscle and significant for other tissues with differences of up to 14% for bone. Using SCT or LCT approaches leads to differences between Dw,m (SCT) and Dw,m (LCT) up to 29% for bone and 36% for teeth. The mean photon energy distribution ranges from 222 keV up to 356 keV. However, results obtained using mean photon energies are not equivalent to the ones obtained using the full, local photon spectrum. This work concludes that it is essential that brachytherapy studies clearly report the dose quantity. It further shows that while differences between Dm,m and Dw,m (SCT) mainly depend on tissue type, differences between Dm,m and Dw,m (LCT) are, in addition, significantly dependent on the local photon energy fluence spectrum which varies with distance to implanted sources.

The recently updated guidelines for dosimetry in brachytherapy in TG-186 have recommended the use of model-based dosimetry calculations as a replacement for TG-43. TG-186 highlights shortcomings in the water-based approach in TG-43, particularly for low energy brachytherapy sources. The Xoft Axxent is a low energy (50 kV) brachytherapy system used in accelerated partial breast irradiation (APBI). Breast tissue is a heterogeneous tissue in terms of density and composition. Dosimetric calculations of seven APBI patients treated with Axxent were made using a model-based Monte Carlo platform for a number of tissue models and dose reporting methods and compared to TG-43 based plans.A model of the Axxent source, the S700, was created and validated against experimental data. CT scans of the patients were used to create realistic multi-tissue/heterogeneous models with breast tissue segmented using a published technique. Alternative water models were used to isolate the influence of tissue heterogeneity and backscatter on the dose distribution. Dose calculations were performed using Geant4 according to the original treatment parameters. The effect of the Axxent balloon applicator used in APBI which could not be modeled in the CT-based model, was modeled using a novel technique that utilizes CAD-based geometries. These techniques were validated experimentally. Results were calculated using two dose reporting methods, dose to water (Dw,m) and dose to medium (Dm,m), for the heterogeneous simulations. All results were compared against TG-43-based dose distributions and evaluated using dose ratio maps and DVH metrics. Changes in skin and PTV dose were highlighted.All simulated heterogeneous models showed a reduced dose to the DVH metrics that is dependent on the method of dose reporting and patient geometry. Based on a prescription dose of 34 Gy, the average D90 to PTV was reduced by between ~4% and ~40%, depending on the scoring method, compared to the TG-43 result. Peak skin dose is also reduced by 10%-15% due to the absence of backscatter not accounted for in TG-43. The balloon applicator also contributed to the reduced dose. Other ROIs showed a difference depending on the method of dose reporting.TG-186-based calculations produce results that are different from TG-43 for the Axxent source. The differences depend strongly on the method of dose reporting. This study highlights the importance of backscatter to peak skin dose. Tissue heterogeneities, applicator, and patient geometries demonstrate the need for a more robust dose calculation method for low energy brachytherapy sources.

Há diversas enfermidades radiossensíveis epidermotrópicas, como a micose fungóide e a síndrome de Sézary, neoplasias cutâneas originadas de linfócitos do tipo T, que apresentam grande possibilidade de erradicação quando tratadas com feixes de elétrons com energia entre 4 e 10 MeV, conforme apontam diversos estudos. No entanto, esta técnica de tratamento apresenta inúmeras dificuldades práticas, pois a doença dissemina-se por todo o corpo do paciente tornando necessário um grande campo de radiação e deposição de energia limitada à profundidade da pele. A fim de obter uma distribuição de dose uniforme, muitas técnicas já foram desenvolvidas. Com base em estudos anteriores e guiados pelo protocolo n. 23 da American Association of Physicists in Medicine (AAPM), o presente trabalho desenvolveu placas espalhadoras e degradadoras de energia e realizou toda a dosimetria (computacional e experimental), a fim de fornecer subsídios para a implementação da técnica de tratamento Total Skin Electron Therapy (TSET) no Serviço de Radioterapia do Hospital das Clínicas de São Paulo. O programa MCNP4C baseado no método de Monte Carlo foi utilizado para reconstruir o espectro energético do acelerador Varian Clinac 2100C, por meio de medidas experimentais de percentual de dose em profundidade (PDP) e perfis radiais de dose. Com estes dados, foi possível efetuar simulações computacionais para a seleção de materiais, mediante análise da distribuição radial e axial de dose, produção de raios-X e a atenuação do feixe, além da simulação de placas espalhadoras e degradadoras de energia, a serem posicionadas na saída do acelerador. Os resultados das simulações foram validados por meio de medidas experimentais a fim de obter um grande campo de radiação com 200 cm x 80 cm que atendesse as especificações do protocolo da AAPM. There are many radiosensitive epidermotropics diseases such as mycosis fungoids and the syndrome of Sézary, coetaneous neoplasics originated from type T lymphocytes. Several studies indicate the eradication of the disease when treated with linear accelerators emitting electron beams with energies between 4 to 10 MeV. However, this treatment technique presents innumerable technical challenges since the disease in general reaches all patient´s body, becoming necessary not only a very large field size radiation beam, but also deliver superficial doses limited to the skin depth. To reach the uniformity in the dose distribution, many techniques had already been developed. Based on these previous studies and guided by the report n. 23 of the American Association of Physicists in Medi-cine (AAPM), the present study developed an energy scattering and degrading plates and made dosimetry (computational and experimental), supplying subsidies for a future installation of Total Skin Electron Therapy (TSET) at the Serviço de Radioterapia do Hospital das Clínicas de São Paulo. As part of the plates design, first of all, the energy spectrum of the 6 MeV electron beam of the VARIAN 2100C accelerator was reconstructed through Monte Carlo simulations using the MCNP4C code and based on experimental data. Once the spectrum is built, several materials were analyzed for the plates design based on radial and axial dose distribution, production of rays-x and dose attenuation. The simulation results were validated by experimental measurements in order to obtain a large field of radiation with 200 cm x 80 cm that meets the specifications of the AAPM protocol.
