According to the International Bruxism Consensus, bruxism refers to the activity of the masticatory muscles reflecting contraction disorders, regardless of whether it is during sleep (SB) or an awake (AB) state. The objective of the present study was to evaluate the activity of the masseter muscle by surface electromyographic (sEMG) recordings. This study was performed on 20 participants with self-reported “possible bruxism” (study group) and 20 participants with no self-reported bruxism (control group); all participants underwent an evaluation of the masseter muscle activity using the dia-BRUXO device, which provides numerical parameters regarding sEMG (the total duration and the type of bruxism specific events, the effort made by the masticatory muscles during the recording period, and the personal bruxism index of each participant). Participants from the study group presented more clenching events during AB, three times more frequent than the control group (p = 0.002, Mann–Whitney U test); for SB, the frequency of clenching and grinding events was comparable within the study group, being more frequent than for the control group; the mean value of the effort index was higher for AB (1.177%) than SB (0.470%) and the same for the duration index, with a mean value of 2.788% for AB and 1.054% for SB. All participants from the control group presented reduced values for all acquired parameters. Overall, the personal bruxism index in AB was approximately four times higher for the study group (2.251%) compared to the control group (0.585%) (p < 0.005, Mann–Whitney U test). Similar values were obtained for SB. All participants with “possible bruxism” from the study group presented a higher activity of the masseter muscle, which is specific for bruxism, thus being defined as “definite bruxism”.

Medical databases usually contain a significant volume of images, therefore search engines based on low-level features frequently used to retrieve similar images are necessary for a fast operation. Color, texture, and shape are the most common features used to characterize an image, however extracting the proper features for image retrievals in a similar manner with the human cognition remains a constant challenge. These algorithms work by sorting the images based on a similarity index that defines how different two or more images are, and histograms are one of the most employed methods for image comparison. In this paper, we have extended the concept of image database to the set of frames acquired following wireless capsule endoscopy (from a unique patient). Then, we have used color and texture histograms to identify very similar images (considered duplicates) and removed one of them for each pair of two successive frames. The volume reduction represented an average of 20% from the initial data set, only by removing frames with very similar informational content.

This study assessed the learning perception of undergraduate medical students on three types of teaching (classical/online/hybrid), in relation to coping strategies, stress, and social support, in the context of the SARS-CoV-2 pandemic. Additionally, we explored gender differences and the perceived usefulness of teaching. 201 students (48 men, 153 women, mean age = 22.900, SD = 2.830) participated in the study. They answered a Multidimensional Scale of Perceived Social Support, a Brief Cope Scale, a Student-life Stress Inventory, a visual analog scale for usefulness, and a survey collecting their perceptions about learning across teaching types. Results point out a preference for classical teaching, followed by the hybrid and online formats. Online teaching was identified as more advantageous in terms of time management and seeking information. Denial and substance use were statistically associated with poor communication, bad time using, and impaired learning. Both stress and social support had ambivalent associations with learning perceptions. Gender differences were limited to behavioral disengagement and higher social support perceived by women. Although classical teaching was globally perceived as the most useful, online teaching was considered desirable by male urban respondents. These data may contribute to the strategic growth and refinement of web-based teaching methods in medical universities.

This study aimed to investigate the relationship between somatic symptom disorder (SSD) and sleep disorders, following three research questions: (1) How are these disorders correlated? (2) What are the comorbidities reported in these patients? and (3) What are the most effective pharmacological and non-pharmacological treatments for both conditions? PubMed, Scopus, OVID, Medline, and ProQuest databases were searched for relevant articles published between 1957–2020. Search terms included “somatic symptoms disorder”, “sleep disorders”, “insomnia”, “somatoform”, “somatization”, “therapeutic”, “psychotherapy”, and alternative, formerly used terms for SSD. Forty papers were finally included in the study. Prevalence of insomnia in SSD patients ranged between 20.4–48%, with this being strongly correlated to somatic symptoms and psychosocial disability. The most relevant comorbidities were generalized anxiety disorder, depression, fatigue, negative mood, substance use, orthorexia, alexithymia, anorexia, weight loss, poor eating habits, and acute stress disorder. Patients receiving antidepressant therapy reported significant improvements in insomnia and somatic symptoms. In terms of non-pharmacological interventions, cognitive-behavioral therapy (CBT) showed improvements in sleep outcomes, while the Specialized Treatment for Severe Bodily Distress Syndromes (STreSS) may represent an additional promising option. Future research could include other medical and psychosocial variables to complete the picture of the relationship between sleep disorders and somatic symptoms.

One of the most frequent diseases all over the world is represented by dental caries, hence it is of utmost importance to detect them as early as possible, in order to prevent massive tooth decay. Next to clinical examination, radiographic images are essential in identifying caries, especially for lesions located on the contact surface between the posterior teeth. Incipient caries are difficult to identify, sometimes even for specialists, so segmentation and detection through various image processing techniques may be useful. This paper presents an analysis of edge and texture parameters in X-rays containing caries, based on Sobel and Canny operators, Gabor filters and local binary pattern (LBP) operator. Our study set consisted in 80 X-rays from Bitewing Radiography Caries Detection Challenge 2015. Final results align with literature results and confirm the fact that caries segmentation in X-rays is a difficult task, with an average of less than 35% edge pixels correctly identified and poor results for texture segmentation; they also motivate us to refine our work in search of new algorithms and methods for caries segmentation and detection.

Currently, barnyardgrass [Echinochloa crus-galli (L.) P.Beauv.] is considered a very dangerous weed for agriculture. It’s spread is achieved both by adapting to the crop and by the high degree of dissemination. The observations found this weed harms all spring crops, including maize. Controlling them can be relatively easy, including herbicides. In order to promote specific weed management (SWM) have been found useful information on the morphological variability of the species. Panicle characters and kernels showed how plant adaptation held in these conditions. Analysis and measurements showed that the panicle had an average length of 10-14 cm. Panicle formed a number of 16-20 branches with 200-600 spikelets, which weighting 0.4-1.0 g. The mass of a thousand grains was of 1.7-2.0 g. The higher frequency had the spikelets with a length of 2.6-2.8 mm and width of 1.2 -1.4 mm. Regarding the length, the highest frequency was 3.1-3.3 mm. The awn length was between 0.1- 30.0 mm. The correlations obtained between the different characters were positive. Thus, the length of panicle and the spikelets number was close (r = 0.551***), as well as of the length to the width spikelet (r = 0.420 ***). Correlation between glume length and awn length was low (r = 0.132), and between total spikelets weight with TSW (thousand spikelet weight), rising (I = 0.503). The present study demonstrated the wide possibilities of weed adaptation to conditions offered by maize crop.

Wild oat – Avena fatua L. has experienced widespread in Southern territory, Argeş County. Characters of panicles and kernels highlights how adaptation weed held in these conditions. The analyzes and measurements showed that the panicle has an average of 20 cm, formed 5 knots ramifications, with a total of 40 spikelets. The average length of the glume was 21 mm and of the kernels was 15 mm. Kernels formed in the panicle were 71, they weighted 1.5 g, and the thousand kernels of weight (TKW) was 21 g. The average length of the awns measured in the bottom portion was 11.8 mm, and 21.1 mm in the portion from the top. Correlations obtained between various characters were positive and significant. Thus, the length of panicle with total number of spikelets was the closest (r = 0.829***). Length of glume with kernel length (r = 0.366***), the length of kernel with awn length (r = 0.375***) and kernel weight/panicle with TKW (thousand kernels weight)(r = 0.334***), values were slightly lower. The present study demonstrated the wide possibilities of that weed, recently adapted into winter wheat crop.

Introduction The role of digoxin (cardiac glycoside) in controlling the heart rate (HR) for the treatment of atrial fibrillation (AF) patients has not been explored in depth. Methods To contribute to the limited data, our team conducted retrospective analysis of the clinical records of 1444 AF patients. We divided the AF patients into two groups, wherein group 1 patients were treated with beta-blockers (BB), low-dose digoxin, and an anticoagulant (vitamin K antagonist/factor-IIa inhibitor/factor-Xa inhibitor), and group 2 patients were treated with just BB and an anticoagulant. Our objectives were to compare the impact of combination therapy of BB and digoxin on the resting HR in patients with permanent AF and the patients’ quality of life (QOL) at periodic intervals. Results The findings of our study showed a better control of the resting HR rate (Conclusion Our findings are indicative of the favorable clinical outcomes that resulted from the addition of a low-dose of digoxin to the AF treatment regimen. However, larger studies/trials elucidating the outcomes of AF patients treated with the dual rate control therapy are required, to clarify the role of digoxin, guide the choice of agents, and standardize the AF treatment protocol.

Variability studies of weeds in agricultural fields are useful in promoting integrated management measures. It was found that a higher variability of any species can be controlled more easily by various methods. And if C.intybus species was a significant spread in agricultural fields, while variability information they are fewer. Weed, being adapted white luvicsoil the south of the country has shown that inflorescence have diameters between 3 and 5 cm, with a ligulae flower number between 13 and 25 pieces. Their modal values were 3.8-4.0 cm in diameter and 15-16 ligulae flowers number in head (capitula). Fruit s (achene) were formed in a number between 1 and 15 in the head, with a length between 1.5 and 2.6 mm, and the width between 0.7 and 1.1 mm. Modal values of these dimensions of fruits were 7-9 pieces/ head, with 1.9-2.0 length and 0.9 mm width. Correlations between the various characters showed positive results, and that means the plant have a good adaptation to the various vegetation conditions.

Eosinophilic gastroenteritis is an uncommon condition characterized by focal or diffuse infiltration of eosinophils in the gastrointestinal tract in the absence of secondary causes. The pathogenesis of this condition is not well understood and its clinical presentation depends on the segment and layer of the gastrointestinal tract affected. The definition of eosinophilic gastroenteritis may be difficult, as the normal ranges of eosinophil numbers in normal and abnormal gastric and intestinal mucosa are not standardized. We present the case of a 59-year-old male who came to the hospital with hypovolemic shock and lethargy secondary to severe diarrhea. Laboratory analysis was significant for peripheral eosinophilia, and pathology from both the duodenum and colon showed marked eosinophilic infiltration.

Small bowel polyps present in images acquired by wireless capsule endoscopy are more difficult to detect using computer-aided diagnostic (CAD) systems. We aimed to identify the optimum morphological characteristics that best describe a polyp and convert them into feature vectors used for automatic detection of polyps present in images acquired by wireless capsule endoscopy (WCE). We prospectively included 54 patients with clinical indications for WCE. Initially, physicians analyzed all images acquired, identifying the frames that contained small bowel polyps. Subsequently, all images were analyzed using an automated computer-aided diagnostic system designed and implemented to convert physical characteristics into vectors of numeric values. The data set was completed with texture and color information, and then analyzed by a feed forward back propagation artificial neural network (ANN) trained to identify the presence of polyps in WCE frames. Overall, the neural network had 93.75% sensitivity, 91.38% specificity, 85.71% positive predictive value (PPV) and 96.36% negative predictive value (NPV). In comparison, physicians' diagnosis indicated 94.79% sensitivity, 93.68% specificity, 89.22% PPV and 97.02% NPV, thus showing that ANN diagnosis was similar to that of human interpretation. Computer-aided diagnostic of small bowel polyps, based on morphological features detection methods, emulation and neural networks classification, seems efficient, fast and reliable for physicians.

A 2-year-old boy presented with an abdominal mass and was diagnosed as Churg-Strauss syndrome (CSS). There was no history of asthma. He developed fatal gastro-intestinal disease, despite treatment with corticosteroids and cyclophosphamide. CSS is extremely rare in young children and gastro-intestinal involvement might carry a worse prognosis than in adults.

Our aim was to identify potential correlations between activated hepatic stellate cells (HSCs) and immune system's cells in patients with viral C hepatocellular carcinoma, by quantifying the percentage of activated HSCs, T-lymphocytes, natural killer cells and B-lymphocytes, in three distinct regions: tumor, transition area and the vicinity tissue (2-5 mm).We prospectively included 20 samples prelevated at necropsy from patients with HCC and C viral infection. We assessed the percentage of alpha-smooth muscle actin (α-SMA), CD45RO, NK1 and CD20 expression using immunohistochemistry and a semi-quantitative scoring method.We found an inverse correlation between the number of α-SMA-positive HSCs and the number of NK1-positive cells in tumor (p=0.0007), in the transition area/tumor capsule (p=0.024) and in the vicinity tissue (p=0.038). Regarding T-lymphocytes, we have also identified an inverse correlation with the number of α-SMA-positive HSCs in tumor (p=0.0036), in the transition area/tumor capsule (p=0.034) and in the vicinity tissue (p=0.047). We found no correlation between the number of activated HSCs and the number of CD20-positive cells in all three examined areas.The analysis of HSCs activity within specified areas of tumoral liver tissue may lead to new perspectives in early diagnosis of relapses and in the development of future neoadjuvant therapies.

Objective: The visceral compartment is a surrogate for visceral adipose tissue. Cross-sectional visceral compartment area (VCA) has been approximated from waist circumference using a circular model. However, the two-dimensional shape of the abdomen is rarely circular. This study validated an elliptical model of cross-sectional total abdominal area (TAA), subcutaneous adipose tissue (SAT) area, and VCA at the L4–L5 level. Research Methods and Procedures: We analyzed magnetic resonance images (MRIs) at the level of the L4–L5 intervertebral space from 35 subjects with a wide range of abdominal adiposity. Waist circumference, abdominal thickness (midline sagittal diameter), abdominal width (coronal diameter at one-half of abdominal thickness), and abdominal SAT thickness at four sites (front, back, right, and left) were measured from MRI images using an image analysis software. The same anatomical regions were also estimated from anthropometrics purely by geometric formulae of circular and elliptical models. A simple linear regression model was used to interpret the association strength between anthropometric estimates and MRI measures. Results: Estimated TAA by either model was strongly related to MRI TAA (r2 = 0.98, p < 0.0001). The SAT and VCA by MRI analysis showed a stronger association with calculation from an elliptical model (r2 = 0.95 and 0.88, respectively; p < 0.001) than a circular model (r2 = 0.69 and 0.25, respectively; p < 0.001). The absolute prediction residuals and variances were significantly smaller with an elliptical model than a circular model (p < 0.0001). Discussion: An elliptical anthropometric model might be superior to a circular model to estimate abdominal SAT and VCA.

We studied aspects of metabolism in subcutaneous adipose tissue (SAT) in 40 human immunodeficiency virus (HIV)-infected subjects with and without lipodystrophy and in healthy control subjects. HIV-infected subjects without lipodystrophy had less SAT and visceral adipose tissue (VAT). Glycerol release was higher in both HIV-infected groups, especially those without fat redistribution. Tumor necrosis factor (TNF) release from SAT and serum soluble TNF receptor 2 concentrations were significantly higher in HIV-infected individuals with lipodystrophy. The absolute production of acylation-stimulating protein (ASP) and the percentage conversion of the complement protein to ASP were significantly lower in HIV-infected subjects with lipodystrophy. Further studies are needed to dissect the factors that mediate lipoatrophy in HIV infection.

Liver innervation comprises sympathetic, parasympathetic and peptidergic nerve fibers, organized as either afferent or efferent nerves with different origins and roles. Their anatomy and physiology have been studied in the past 30 years, with different results published over time. Hepatocytes are the main cell population of the liver, making up almost 80% of the total liver volume. The interaction between hepatocytes and nerve fibers is accomplished through a wealth of neurotransmitters and signaling pathways. In this short review, we have taken the task of condensing the most important data related to how the nervous system interacts with the liver and especially with the hepatocyte population, how it influences their metabolism and functions, and how different receptors and transmitters are involved in this complex process.

Hepatic stellate cells (HSCs) play a crucial role in liver fibrosis, following inflammatory processes within the parenchyma. Their activation pathways give way to a cascade of phenomena which are potentially dangerous for the liver metabolism at a cellular level. The changes towards fibrosis pave the way for the evolution of hepatitis into cirrhosis, the most important etiological entity of hepatocellular carcinoma. In this review, we try to cover a few of the basic aspects of the intricate mechanisms that govern the complex activation of HSCs, their involvement in carcinogenesis and how these molecular targets may become valuable in the future therapeutic approaches for primary liver carcinomas.

Achalasia is rare in the pediatric age group and in most cases it is idiopathic with no family history. Familial achalasia is very rare. This report describes two families with achalasia: in one, six children were affected while in the other a brother and a sister had Allgrove’s syndrome (triple-A syndrome consisting of achalasia, adrenal insufficiency, and alacrima). Familial achalasia suggests that it is hereditary and may be transmitted as an autosomal recessive trait. The management of achalasia in children is still controversial. With the recent advances in minimal invasive surgery, laparoscopic Heller’s myotomy is the procedure of choice in the management of achalasia in children.

Psychological stress, as defined by modern science, appears when an individual balances existing threats, being real or imaginary, and after confronting them with his or hers current resources, and finds the later to be inadequate. In practice, this occurs when experiences are judged to be exceeding adaptive capacities by an individual, therefore over-soliciting the mental defense mechanisms and coping capabilities. The above definition and context excludes psychiatric disorders that may impair the integration capacities of an individual. In the following chapter, psychiatric disorders resulting from stressful events are also excluded. Chronic psychosocial stress is possibly the most complex and powerful harming agent, as its repetitive nature enables it to constantly follow a patient during the onset and the course of chronic disease. The idea that stressful events in a life of a person can influence his or hers response to illness is an idea that appeared from ancient times, despite a lack of scientific evidence. However, recent advances in the field of psychoneuroimmunology led to a new perspective on the issue. The scientific community devoted its attention to designing experiments and studies in order to document the effects and interactions between psychological and physical stress, and the well-being of a living organism. Within the last two decades, several studies revealed new interactions between endocrine response systems, such as the hypothalamic-pituitary-adrenal axis (HPA) or the sympathetic-adrenal-medullary (SAM) system, and various pathologies. Moreover, it has been proven that the central nervous system and especially the autonomous component exert regulatory effects on the immunity of the organism. The two regulatory bodies are deeply interconnected, numerous links at various levels critically determining the end function and outcome of the immune response. Liver disease was demonstrated to be heavily influenced by perceived chronic stress during several human and animal studies. The effects of chronic viral hepatitis on liver parenchyma are well known, however the idea that real-life events, independent from the disease itself, can influence the course of this disease, is relatively new.

Hepatocellular carcinoma (HCC) represents the fifth most common cancer worldwide, while being the third leading cause of death by cancer. The primary risk factor for HCC seems to be liver cirrhosis. A large majority of these patients have a history of viral hepatitis.We selected a study lot consisting of 244 patients diagnosed with HCC, admitted between 2006 and October 2009 in the Emergency County Hospital of Craiova, Romania along with an age and gender matched control group, consisting of patients with no history of HCC or other malignancies. We interviewed all subjects regarding their alcohol consumption and background environment. All subjects underwent hepatitis B surface antigen (Hbs Ag) and anti-HCV antibodies (Anti-HCV Atb) serological determinations.The study group consisted of 148 males and 96 females. Liver cirrhosis (LC) was present in 84% of the study lot, 10% associated viral B hepatitis (HBV) and 6% viral C hepatitis (HCV), with no signs of LC. We found LC to be an important risk factor for HCC (RR 6.53, CI 95% 3.18-13.38). The RR and 95% CI of HCC were 4.51 (2.48-8.21) for HbsAg positivity. We noticed a strong correlation (Chi-square test, p0.001) between the rural environment and the association with LC. HVB was also more present in patients coming from rural areas (p0.01). Alcohol intake was present in 89% of the whole lot, being more correlated with the presence of LC as well as with HbsAg positivity (RR 9.165, CI 95% 4.43-18.92).Cirrhosis proved to be the primary risk factors for HCC. We underline the fact that HCC was found to be directly associated with viral hepatitis, without evident LC. Further studies are needed in order to establish if intensified HCC screening, especially in rural areas, is required in patients with newly diagnosed viral hepatitis. The increased prevalence of HBV infections might encourage HBV vaccinations as an efficient tool to prevent HCC.

The most difficult to examine segment of the digestive tract is the small bowel. However, examining the distal portions that follow the duodeno-jejunal junction is vital when trying to diagnose an important number of pathologies, most of them with vital implications for the patient. Until recently, standard diagnostic techniques were limited to radiological barium-enhanced investigations, “push-enteroscopy” and intraoperatory enteroscopy, selective arteriography, scintigraphy or computer tomography. All these methods were either too intrusive, such as the case with intraoperatory enteroscopy, or did not provide sufficient data for an accurate diagnostic. Technological progresses in recent years allowed for the rapid development of novel, more patient-friendly and diagnostically competent investigation techniques dedicated to this particular GI segment. Videocapsule endoscopies (VCE), along with balloon (single or double) enteroscopy are currently considered the two most valuable tools available to the gastroenterologist. (Iddan et al, 2000; Swain P, 2003) The VCE system is currently the most widespread investigation technique for the small bowel, due to its convenience of use and slightly better affordability. It has been tested in a large number of studies, being compared to all other investigatory techniques, currently a large number of high-impact papers existing on the subject. It is estimated that over one million such devices have been swallowed over the course of almost ten years since its first introduction on a global scale.

Survivors of childhood malignancies are known to be at an increased risk for developing a variety of secondary cancers. Primary adenocarcinoma of the colon is very rare in children and adenocarcinoma of the colon occurring as a secondary malignancy in children is much rarer. A boy with a history of successfully treated embryonal rhabdomyosarcoma developed adenocarcinoma of the colon as a secondary cancer. The boy presented with a solid mass of the left cheek at 3 years of age. The mass was excised and histological examination showed embryonal rhabdomyosarcoma. He was treated with multi-agent chemotherapy and local radiotherapy, which resulted in complete remission. Four years later, he presented with recurrent colicky abdominal pain and bleeding per rectum and was found to have intussusceptions. Colonoscopy revealed a tumor in the transverse colon, which was biopsied and proved to be an adenocarcinoma. The boy underwent excision followed by chemotherapy using an adult colon cancer regimen. He is currently off chemotherapy for 2 years with no evidence of the disease. We report a rare case of colon cancer after treatment of rhabdomyosarcoma. Colorectal adenocarcinoma must be kept in mind as a secondary neoplasm following treatment for early childhood malignancies although it is extremely rare.

Urine bacteriological examination is one of the most frequently asked for examinations, in ambulatory as well as in hospitals, regarding specialty prophyle. The present study comprises a group of 3971 pacients that were sent to the laboratory for uroculture and showed a percentage of only 21.85% significant urocultures--the etiological groups most frequently isolated being Gram-negative bacilli--96.05%, respectively Escherichia sp. 76.95%, Klebsiella sp. 14.68%, Proteus sp. 4.66%. Out of the Gram-positive bacteria we encountered only group B streptococcus and Staphylococcus epidermidis. Colimycine, phosphomycine, fluorurated quinolones were the most active on the Gram-negative flora, and the Gram-positive cocci were most frequently sensitive to amikacine, amoxycyline and ceftazidim.

“Psychosocial stress” is an increasingly common concept in the challenging and highly-demanding modern society of today. Organic response to stress implicates two major components of the stress system, namely the hypothalamic-pituitary-adrenal axis and the sympathetic nervous system. Stress is anamnestically reported by patients during the course of disease, usually accompanied by a decline in their overall health status. As the mechanisms involving glucocorticoids and catecholamines have been deciphered, and their actions on immune cell function deeper understood, it has become clear that stress has an impact on hepatic inflammatory response. An increasing number of articles have approached the link between psychosocial stress and the negative evolution of hepatic diseases. This article reviews a number of studies on both human populations and animal models performed in recent years, all linking stress, mainly of psychosocial nature, and the evolution of three important liver-related pathological entities: viral hepatitis, cirrhosis and hepatocellular carcinoma.

Background A large proportion of HIV-infected patients on antiretroviral medication develop insulin resistance, especially in the context of fat redistribution. This study investigates the interrelationships among fat distribution, hepatic lipid content, and insulin resistance in HIV-infected men. Methods We performed a cross-sectional analysis of baseline data from 23 HIV-infected participants in three prospective clinical studies. Magnetic resonance spectroscopy was used to quantify hepatic lipid concentrations. Magnetic resonance imaging was used to quantify whole-body adipose tissue compartments: that is, subcutaneous adipose tissue (SAT) and visceral adipose tissue (VAT) volumes, as well as the intermuscular adipose tissue (IMAT) subcompartment and the omental-mesenteric adipose tissue (OMAT) and retroperitoneal adipose tissue (RPAT) subcompartments of VAT. The homeostasis model for assessment of insulin resistance (HOMA-IR) was calculated from fasting glucose and insulin concentrations. Results Hepatic lipid content correlated significantly with total VAT (r=0.62, P=0.0014), but not with SAT (r=0.053, P=0.81). In univariate analysis, hepatic lipid content was associated with the OMAT (r=0.67, P=0.0004) and RPAT (r=0.53, P=0.009) subcompartments; HOMA-IR correlated with both VAT and hepatic lipid contents (r=0.61, P=0.057 and r=0.68, P=0.0012, respectively). In stepwise linear regression models, hepatic lipid had the strongest associations with OMAT and with HOMA-IR. Conclusion Hepatic lipid content is associated with VAT volume, especially the OMAT subcompartment, in HIV-infected men. Hepatic lipid content is associated with insulin resistance in HIV-infected men. Hepatic lipid content might mediate the relationship between VAT and insulin resistance among treated, HIV-infected men.

BACKGROUND The high regional incidence of hepatocellular carcinoma (HCC) in South Africa also may be present in children of the region, although the link to hepatitis B (HBV) appears less clear. The objective of this study was to assess the incidence and probable causes of HCC in South African children. METHODS Data were obtained from seven participating pediatric oncology units and from the tumor registry to review hepatic tumors in children in South Africa. RESULTS One hundred ninety-four children (ages 0–14 years) presented with malignant primary hepatic tumors (1988–2003). One hundred twelve tumors (57%) were hepatoblastoma (HB), 68 tumors (35%) were hepatocellular carcinoma (HCC) (including 9 patients with the fibrolamellar variant, 6 of which occurred in black children), 10 tumors (5%) were sarcoma of the liver, and 4 tumors were lymphoma. The ratio of HB to HCC (1.67) was markedly lower compared with other reports, suggesting a greater prevalence of HCC. Correlation with population statistics indicated an incidence of 1.066 malignant liver tumors per year per 106 children age < 14 years (HB, 0.61 per 106 children; HCC, 0.39 per 106). Two-thirds of patients with HCC were positive for HBV surface antigen (HBsAg), and HCC occurred mostly in black African patients (93%). The mean age of onset was 1.47 years for HB and 10.48 years for HCC. A preponderance of males (3.5:1.0) was noted in the HBsAg-positive group that was not reflected elsewhere. Serum α-fetoprotein (AFP) levels were raised both in patients with HB (100%; most AFP levels were very high) and in patients with HCC (69%), although 15% of patients with HCC had low or normal AFP levels. CONCLUSIONS It appeared from the current results that HCC is more prevalent among children in South Africa compared with the children in more developed countries, although their rates were lower that the rates noted in adults. The lipodystrophy syndrome (adipose tissue redistribution and metabolic abnormalities) observed with highly active antiretroviral therapy (HAART) during human immunodeficiency virus (HIV) infection may be related to increased proinflammatory cytokine activity. We measured acute cytokine (TNF-α, IL-6, leptin), glycerol, and lactate secretion from abdominal subcutaneous adipose tissue (SAT), and systemic cytokine levels, in HIV-infected subjects with and without lipodystrophy (HIVL+ and HIVL–, respectively) and healthy non-HIV controls. Lipodystrophy was confirmed and characterized as adipose tissue redistribution in HIVL+ compared with HIVL– and controls, by dual-energy X-ray absorptiometry and by whole body MRI. TNF-α secretion from abdominal SAT and circulating levels of IL-6, soluble TNF receptors I and II, and insulin were elevated in HIVL+ relative to HIVL– and/or controls, particularly in HIVL+ undergoing HAART. In the HIV-infected group as a whole, IL-6 secretion from abdominal SAT and serum IL-6 were positively associated with visceral fat and were negatively associated with the relative amount of lower limb adipose tissue ( P < 0.01). Decreased leptin and increased lactate secretion from abdominal SAT were specifically associated with HAART. In conclusion, increased cytokine secretion from adipose tissue and increased systemic proinflammatory cytokine activity may play a significant role in the adipose tissue remodeling and/or the metabolic abnormalities associated with the HIV-lipodystrophy syndrome in patients undergoing HAART.
