Purpose. Diabetes mellitus is a global disease in which a reduced insulin production or effect affects the blood sugar metabolism. The incidence of the illness is rising steadily. An elevated blood sugar level implies changes in the vascular walls and, in the long term, a reduced blood flow to some ar- eas of the retina As a result, growth factors such as VEGF are overexpressed, which can lead to vitreous haemorrhage and traction-related retinal detachment via the formation of new blood vessels. The aim of this work is to review the literature to discuss the risk factors relevant for the development and progression of diabetic retinopathy. Material and Methods. We carried out a differentiated liter- ature review regarding the risk factors relevant to the devel- opment and/or progression of diabetic retinopathy based on a detailed description of the different forms of diabetes. The different risk factors were discussed and evaluated in the context of the different stages of diabetic retinopathy. Results. The risk of retinal conditions increases with the du- ration of the underlying diabetic disease and a significantly higher blood pressure. In insulin-dependent type 1 diabetics, optimising the time in range can reduce the risk. In type 2 diabetics, whose disease is often associated with obesity and increased blood lipid levels, an increase in physical activity can be useful in addition to optimal blood sugar control. All factors that reduce blood circulation, such as nicotine consumption or an increase in intraocular pressure, are to be regarded as risk factors. The first visible ocular signs of diabetic retinopathy are outpouching of the vascular walls (microaneurysms), the number of which correlates with the risk of progression of the retinal disease. Conclusion. It is crucial to understand the systemic and ocular risk factors to reduce the risk of developing or progressing diabetic retinopathy. A clear risk factor that can be reduced is nicotine. ZusammenfassungDie Behandlung orbitaler Verletzungen gehört zum Alltag der medizinischen Akutversorgung. Bei 10 – 25% aller Gesichtsverletzungen ist die Orbita beteiligt. Dieser Beitrag gibt einen Überblick über die möglichen Pathologien nach Orbitatrauma sowie das primärdiagnostische Vorgehen und die therapeutischen Optionen.

The continuous recording of eye injuries caused by fireworks around New Year's Eve over 6 years allows us to investigate injury numbers, injury patterns, and accident histories in a year to year comparison. In order to relieve the burden on hospitals during the COVID-19 pandemic, a ban on the sale of firework articles was implemented in Germany for 2020/2021 and 2021/2022. We investigate the impact of these measures on the number of fireworks-related eye injuries and analyze whether this led to an increase in serious injuries and an association with increased use of homemade pyrotechnics or pyrotechnics potentially not approved in Germany.We used our online questionnaire to collect anonymized data on patients, accident history, and, since 2017/2018 information on the acquisition of firework articles.Our analysis included data from 2151 affected individuals. While data of about 500 injured were entered per year before the pandemic, this number decreased to 79 in 2020/2021 and 193 in 2021/2022. The proportion of severe eye injuries requiring inpatient care ranged from 21% to 26% in the years before the pandemic but ranged from 27% to 34% in the pandemic years. At the same time, under the sales ban the proportion of fireworks that were homemade or unofficially purchased increased from 3% to nearly 10%. In terms of absolute numbers, however, there were 67 accidents involving unofficially purchased pyrotechnics compared with 1675 incidents involving officially purchased or unidentified fireworks, which can be explained by the high proportion (about 50%) of injured bystanders.The absolute number of patients with eye injuries caused by pyrotechnics decreased under the pandemic conditions from about 500 to 79 or 193. The use of fireworks designated as unofficially purchased was proportionally low even under the sales ban, and plays a minor role compared to injuries with officially purchased fireworks articles.HINTERGRUND UND ZIEL: Die kontinuierliche Erfassung von Augenverletzungen durch Pyrotechnik in den Tagen um Silvester über 6 Jahre ermöglicht uns, Verletzungszahlen, Verletzungsmuster und Unfallhergänge im Jahresvergleich zu untersuchen. Zur Entlastung der Krankenhäuser wurden in Deutschland im Rahmen der COVID-19-Pandemie für die Jahreswechsel 2020/21 und 2021/22 ein Verkaufsverbot für Pyrotechnikartikel und Versammlungsbegrenzungen umgesetzt. Wir untersuchen, welchen Einfluss diese Maßnahmen auf die Anzahl von feuerwerksbedingten Augenverletzungen hatten. Während vor der Pandemie pro Jahr Daten von rund 500 Verletzten eingegeben worden waren, sank diese Zahl auf 79 am Jahreswechsel 2020/21 und 193 2021/22. Der Anteil schwerer, stationär zu versorgender Augenverletzungen lag in den Jahren vor der Pandemie zwischen 21 und 26 %, in den Pandemiejahren 2020 bis 2022 hingegen bei 27 % und 34 %. Gleichzeitig stieg unter dem Verkaufsverbot der Anteil von Pyrotechnikartikeln, die nach Aussagen der Patienten selbst gebaut oder inoffiziell erworben waren, von 3 % auf knapp 10 % an. Bei den absoluten Zahlen jedoch stehen 67 Unfälle mit nicht offiziell erworbener Pyrotechnik 1675 Zwischenfällen mit offiziell erworbenen oder nicht benennbaren Feuerwerkskörpern gegenüber. In etwa der Hälfte der Fälle konnte zum auslösenden Pyrotechnikartikel keine Aussage gemacht werden, was erklärbar ist durch den hohen Anteil (rund 50 %) verletzter Unbeteiligter.Die absolute Zahl der Augenverletzungen durch Pyrotechnik sank unter den Pandemiebedingungen von rund 500 auf 79 bzw. 193. Die Nutzung von Feuerwerkskörpern, die als nicht offiziell erworben bezeichnet wurden, war auch unter dem Verkaufsverbot anteilsmäßig gering und spielt im Vergleich zu Verletzungen mit „offiziell erworbener“ Pyrotechnik eine untergeordnete Rolle.

Zusammenfassung Hintergrund Weltweit entstehen an Feiertagen schwere Augenverletzungen durch Feuerwerkskörper. Die Datenlage für Deutschland ist bislang unklar. Fragestellung Wie häufig entstehen solche Verletzungen in Deutschland, wer ist gefährdet, wie schwer sind die Verletzungen, welche Therapien erfordern sie, und wie häufig treten Begleitverletzungen auf? Methode Deutschlandweite standardisierte Online-Umfrage an bettenführenden, notdienstleistenden Augenkliniken und Auswertung der Ergebnisse über 3 Jahre. Ergebnisse Die Beteiligung konnte von 2016 bis 2019 von 41 auf 51 Augenkliniken gesteigert werden. Von 1356 Patienten war mehr als ein Drittel (33–39 %) minderjährig, rund 60 % 25 Jahre oder jünger und rund 60 % Bystander. Ein Viertel der Verletzungen war schwer und erforderte eine stationäre Therapie. Begleitverletzungen am Partnerauge, an Händen und Gesicht waren bei Minderjährigen häufiger als bei Erwachsenen. Zwei Register aus Finnland über 20 Jahre und aus den Niederlanden über 10 Jahre haben die Auswirkungen von Öffentlichkeitsarbeit und gesetzlichen Regelungen untersucht. Die Häufigkeit von Feuerwerksverletzungen in diesen beiden Ländern wurde in den letzten 10 Jahren auf etwa die Hälfte reduziert, indem die Anzahl der zulässigen Stunden für private Feuerwerke und strengere Vorschriften für die Verwendung von Feuerwerkskörpern festgelegt wurden. Zudem wurde die Öffentlichkeitsarbeit verstärkt und die Verwendung von Schutzbrillen empfohlen und erleichtert. Obwohl mit diesen Maßnahmen die Gesamtzahl der Verletzungen zurückgegangen ist, scheint die Schwere der Augenverletzungen in den Niederlanden möglicherweise aufgrund des höheren Gehalts an Schwarzpulver in Feuerwerkskörpern zuzunehmen. Orbital trauma is a common medical emergency with potentially sight-threatening complications. Due to the confined orbital conditions, any direct injury or space-consuming lesion may cause serious tissue damage. Possible complications are orbital fractures, foreign body injuries, development of orbital compartment syndrome and traumatic optic neuropathy. Clinical signs include periorbital hematoma or emphysema, subconjunctival hemorrhage, proptosis, decreased ocular mobility, decreased vision, increased intraocular pressure and relative afferent pupillary defect. Ophthalmic status and low-dose CT-imaging provide key information regarding the therapeutic decision. Treatment options include surgical exploration, reconstruction or decompression or a conservative approach with anti-inflammatory or anti-glaucomatous medication. Rapid interdisciplinary diagnostics and therapy is critical for early detection and prevention of irreversible functional loss respectively. Considering the frequent association with life-threatening comorbidities diagnosis may be complicated.

Background: On festive days worldwide eyes are severely injured by fireworks. The data on the number and severity are to date not registered in Germany. Objective: How frequent are firework-induced injuries in Germany, who are the affected, how serious are the injuries, which forms of treatment are necessary and how frequent are accompanying injuries? Method: A German nationwide online-based survey was carried out in all inpatient eye departments and data over the last 3 consecutive years were descriptively analyzed. Results: From New Years Eve 2016/2017, when 41 eye departments sent in data, the participation could be increased to 51 eye departments in 2018/2019. More than one third (33–39%) of all 1356 patients over 3 years were minors, 60% were younger than 25 years old and roughly 60% of patients were injured as bystanders or in an unclear situation. In total 25% of all eye injuries were considered severe and required inpatient treatment. Accompanying injuries of the other eye, the face and hands were more frequent in minors than in adults.
