Számos klinikai vizsgálat igazolta hatékonyságát, kombinálva különböző csontpótló anyagokkal.Elkészítéséhez a páciens saját vérét, alvadásgátló hozzáadása nélkül a kidolgozott protokoll szerint centrifugálják. A fo- lyamateredményeként leukocytákban és trombocytákban gazdag fibrin-alvadék nyerhető, mely extracelluláris mátrixként szolgál. A fehérvérsejtek és vérlemezkék növekedési faktorai lassan (2–3 hét) szabadulnak fel, így kedvezően befolyásolják a sebgyógyulást. A szerzők áttekintik a főbb irodalmi adatokat, és beszámolnak a PRF fogászati, dentoalveoláris és maxilofaciális sebészeti alkalmazásának lehetőségeiről (implantológia, csontpótlás, dentoalveoláris sebészet, parodontológia, arcüregzárás, sinus elevatio, osteonecrosis, endodontia).

Concrete is considered the most anthropogenic material used in the construction sector worldwide. It is associated with consuming massive amounts of energy and the depletion of natural resources, based on the increasing Egyptian urban expansion by establishing new cities to face the population growth challenges and achieve the national development strategy. The importance of applying the Circular Economy (CE) for concrete materials in the building sector became a robust key for reducing conventional concrete (CC) materials and addressing the building materials' future challenges. This study investigates the benefits of Green Concrete (GC) materials and their potential for supporting the principles of achieving circularity for concrete materials in the Egyptian building sector. Furthermore, develop a conceptual framework for using GC from the building scale perspective in two new Egyptian cities. The study attempts to answer how GC materials help achieve a circular economy and the potential benefits of integrating different CE strategies for using GC in the Egyptian building sector. The evidence-based solutions (EBS) methodology was used for collecting and analyzing data for assessing the environmental impacts based on reducing the natural resources consumption, recycling, and reusing waste products in the Egyptian building sector. Case studies are used to provide in-depth insights into the practicalities of GC. Applying the before-and-after (BAA) technique for two building models highlights the challenges and opportunities for substituting CC with GC to assess the interactive factors for achieving CE and applying sustainability. The results showed valuable insights into the potential for using GC to support the CE and have a strong ability to reduce natural resources consumption and construction waste stream, which leads to close the loop and achieving circularity in the Egyptian building sector, and recommended that Design for Recycling (DfR) strategy is the most need for improving the using of GC in the building sectors.Keywords, Green Concrete, Circular Economy, Evidence-based Solutions, Egyptian building sector

Összefoglaló. A cherubismus ritka, autoszomális dominánsan öröklődő megbetegedés. A fibroossealis elváltozások csoportjába tartozik. Jellemzője az állcsontok szimmetrikus duzzanata, a típusos radiológiai elváltozások és az SH3BP2-gén mutációja. Szövettanilag nem különül el az óriássejtes granulomától. A csontelváltozások és a fibroticus szövet felszaporodása pubertás előtt kezdődik, ezután stagnálás vagy visszafejlődés következik be. A magyar orvosi irodalomban a szerzők elsőként tárgyalják három testvér kórtörténete alapján a cherubismust. A diagnózist a hasonló klinikai tünetek, a típusos kórlefolyás, a szinte azonos radiológiai kép, a szövettan és a genetikai elváltozások biztosítják. A testvérek és az anya csíravonalában kimutatott azonos mutáció akkor is megfelel egy dominánsan öröklődő szindrómának (például cherubismusnak), ha a betegség az anyában klinikailag nem manifesztálódott, de genetikailag igen. A szerzők összefoglalják a kórkép kezelési lehetőségeit: a sebészi (excochleatio, ,,decountouring”, esetleg reszekció) és a gyógyszeres (biszfoszfonát, kalcitonin, szteroid stb.) terápiát. Egyezik a véleményük azokéval, akik azon az állásponton vannak, hogy a beavatkozásokkal várni kell, és meg kell figyelni a betegeket a várható regresszió miatt. Cherubism is a rare autosomal, dominant bone disorder, characterised by symmetrical expansion of the jaws along the typical radiological and genetic (SH3BP2 mutation) features. It belongs to the heterogenous group of fibro-osseous lesions. Its histology is the same as that of giant-cell granuloma. The bone lesions and fibrous tissue expansion increase before puberty and regress thereafter. For the first time in Hungarian medical literature, the authors discuss the condition of cherubism in the case of three siblings. The diagnosis of these three siblings is supported by the clinical, radiological, microscopic and genetic data. In all three, the bone lesions and fibrous tissue expansion increased before puberty and stabilized thereafter. The radiological results and the molecular findings were nearly identical. The identical mutation shown in the germ lines of the three siblings and the mother correspond to a dominantly inherited syndrome (e.g., cherubism) even if the condition did not manifest in the mother. The authors summarize the treatment options of the disease: surgical (excochleation, decountouring, in rare case resection) and drug (bisphosphonate, calcitonin, steroid, etc.) therapy. They agree with those who are of the opinion that interventions should wait and the patients should be observed („wait and see”) for the expected regression. In their own cases, only excochleation of the growing tumor was performed, mainly for cosmetic reasons and to secure the tissue. Orv Hetil. 2022; 163(11): 446–452.

Összefoglaló. Bevezetés: Magyarországon 100 000 olyan értelmi sérült él, akik speciális ellátásra szorulnak. Előzőleg beszámoltunk az akut fogászati ellátás eredményeiről és problémáiról. Ahhoz azonban, hogy a teljes fogászati rehabilitáció felé lépéseket lehessen tenni, ismerni kell az ilyen betegek fogászati állapotát. Célkitűzés: Statisztikailag releváns, nagyobb beteganyagon felmérni az értelmi sérült betegek fogászati állapotát, összehasonlítva mind az orvosi rehabilitációs kezelésben részesülő betegek (főleg testi fogyatékosok), mind az általános populáció adataival. Módszer: A fogászati állapot felmérésére két betegcsoport állt rendelkezésünkre. Összesen 325 beteg statusát rögzítettük. A vizsgáltakat 3 csoportba osztottuk: enyhe (36), közepes (247) és súlyos (42) értelmi sérültek. Az értelmi sérült betegek fogászati állapotát DMF-T-indexszel mértük, és összehasonlítottuk az orvosi rehabilitációs kezelésben részesülő betegek és az általános populáció adataival is. Eredmények: A 325 szellemi sérült össz-DMF-T-indexének átlaga ± szórás: 11,04 ± 7,35; a carieses fogak számának átlaga: D = 3,66 ± 4,61; a hiányzó fogak számának átlaga: M = 5,22 ± 5,74. A fogmegtartó kezelés minimális volt: átlag F = 2,16 ± 3,12. Azoknak az értelmi sérülteknek, akik intézetben élnek, rosszabb a fogazati állapotuk, mint azoknak, akik családban vannak. Következtetések: Az eredményekből látszik, hogy az értelmi sérültek fogai elhanyagoltabbak, mint a normálpopuláció esetében. Introduction: There are 100,000 mentally disabled individuals in Hungary requiring special care. Previously, we reported the results and problems of acute dental care. However, in order to take the appropriate measures toward holistic dental rehabilitation, clarity regarding the condition of those mentally disabled individuals is necessary. Objective: To measure the dental health of mentally disabled individuals based on a large sample of patients, to compare the results with both differently-abled (i.e., primarily physically disabled) individuals and the general population. Methods: There were two available patient cohorts for the measurement of dental health. With the two cohorts combined, we recorded data about 325 patients. We split the patients into three groups: mild (36), medium (247), and severe (42) mental disability. We used DMF-T index to measure dental health status, and compared the results with those of physically disabled and the general population. Results: The mean ± deviation of DMF-T index of the disabled patients was 11.04 ± 7.35; the mean of decayed teeth: D = 3.66 ± 4.61; the missing teeth: M = 5.22 ± 5.74; the filled teeth: F = 2.16 ± 3.12. Mentally disabled patients living in institutions showed worse dental health than those living with families. Conclusions: The results show that the teeth of mentally disabled patients are more neglected than those of the general population. To support the monitoring and reporting processes during implementation of the Sustainable Development Goals, well-developed, commonly recognized Earth observations and geospatial data, methods, innovations, committed professionals, and strong sustainability policies are necessary. This article informs the readers on the Earth observation and geoinformation developments and innovations, and on the engagement of profession, academy and governance to support implementation of the Sustainable Development Goals in Hungary. Description, analyses and critical assessments are given on the elements selected from Hungarian sustainable-oriented Earth observation and geospatial novelties: (a) Working Group for Sustainable Development mission and national sustainability-policy, (b) international partnerships, domestic activities and achievements, (c) status of the professional education, (d) spatial databases and services to support implementation of the sustainable development, (e) a case study on the internationally recognized soil geoinformation system, (f) national Earth Observation Information System and perspectives of its applications for monitoring the sustainability. The article conclusion strongly advises the Hungarian realization of (a) institutionalization of the Earth observation and geospatial tools and capacity for sustainable development, (b) their use in integration with statistical data, (c) establishment of national spatial information infrastructure and (d) development and spreading of the use of big data.

Objectives To assess the histomorphometric outcomes obtained in randomized clinical trials (RCTs) with different biomaterials used for maxillary sinus augmentation (MSA). Materials and Methods A search of the existing medical literature until October 1, 2019, was performed. Inclusion criteria were (a) RCTs assessing a two‐stage MSA from the lateral approach using autologous bone or biomaterials for grafting and (b) reported histomorphometric outcomes based on crestal bone core biopsy samples. The Bayesian method was used to perform pairwise meta‐analyses and network meta‐analysis (NMA). The primary outcome, the new bone percentage (NB %), was calculated as mean differences with 95% credible intervals. The interventions were ranked by their posterior probability by calculating the surface under the cumulative ranking curve values. Results Thirty‐four RCTs (842 MSAs) were included in the analysis with a normal healing period (5–8 months). All comparisons were presented in a league table. On the basis of the ranking probability, the most effective bone grafting material for NB% was bovine xenograft + bone marrow concentrate (BMC) (81%), followed by bovine xenograft + platelet‐rich plasma (PRP) (77%), bioactive glass ceramic + autologous bone 1:1 (70%), nanocrystalline hydroxyapatite in silica gel (70%), and bioactive glass ceramic (70%). Autologous bone graft alone took the twelfth position with 57%. Conclusion Within the limitations of the present NMA, the analysis did not confirm autologous bone alone as the gold standard for MSA and showed superiority of composite grafts such as bovine xenograft + BMC after 5–8 months of healing.

Abstract The challenges posed by the growth of urbanization in Egypt and the development of new cities play an essential role in applying the circular economy (CE) in the construction materials sector and the priorities for promoting sustainable construction activities in the future. Therefore, the construction sector has many adverse environmental impacts on energy and natural resources consumption. Starting from materials production, operation until disposal to landfills. Consequently, the industry is considered one of the most consumers of non-renewable resources and producer of CO2 emissions. On the other hand, applying Nature-based solutions (NbS) to enhance sustainability by protecting the ecosystems and maintaining economic benefits plays a vital role, especially for new Egyptian cities. The research aims to investigate the role of applying NbS for achieving CE in construction materials and eliminate its negative impact in the scope of three factors: green building materials, waste management systems, renewable energy use. The current research attempts to answer how NbS can improve the CE and reduce environmental impacts of the construction materials sector. Therefore, the SOWT analysis investigated the strengths, opportunities, weaknesses, and threats of using the NbS strategies for three different construction sites in Egypt. Furthermore, the survey questionnaire was applied to identify the interactions between the parameters derived from 40 participants such as consultants, architecture engineers, civil engineers, site engineers, project managers and review the previous research efforts. As a result, a conceptual framework was created for the construction materials considering reduce, reuse, recycle, recovery, and disposal, to identify the impact of the implementation of NbS on achieving sustainable development strategies in the Egyptian construction sector. The result showed that the NbS could effectively promote the construction sector and achieve environmental and economic benefits, which consequently help the transition to CE. Therefore, there is the necessity for developing new sustainable policies and cooperation between public and private sectors to support the investments of sustainable strategies in the construction materials market and increase Egyptian society's awareness of the benefits of NbS in economic, environmental, and social aspects. Septic tanks without proper construction and insulation entail a significant risk to the environment. In this study, the environmental impacts of a permeably designed septic tank on shallow groundwater contamination are investigated, and changes in water quality in the period after its elimination in 2014 are assessed. For the purpose of evaluating the pollution level of the site, 10 monitoring wells were installed around the septic tank in 2012 and long-term monitoring was carried out. Analytical measurements revealed a significant level of groundwater contamination in the operational period of the septic tank. Extremely high concentrations of NH4+ (>90 mg/L) were observed in the closest monitoring wells, and in most of the wells, concentrations exceeded the relevant contamination limit. δD and δ18O isotopic ratios of monitoring wells within 1 m from the septic tank indicate continuous recharge of sewage water originating from deeper aquifers. The groundwater dome resulting from the wastewater discharge exceeded 1.1 m, within a distance of 25 m. Statistical analyses also revealed significant changes in water quality depending on the monitoring well location from the septic tank. In the period after the septic tank elimination, considerable changes have been detected. Following the cessation of the wastewater discharge, the groundwater dome around the septic tank disappeared; therefore, differences in groundwater levels have decreased from more than 1 m to a few cm. Significant positive changes were detected in the water quality parameters investigated after the dismantling of the septic tank. Five years after the cessation of the pollutant supply, concentrations still exceeded the contamination limit in most of the monitoring wells, indicating slow decontamination processes with a permanently high level of pollution.

In the present study, the environmental hazards of an abandoned and unrecultivated liquid waste disposal are investigated with a special focus on soil and shallow groundwater contamination. After a period of operation from 1994 to 2010, when the wastewater collection of the municipality was regulated, the disposal site was subsequently decommissioned without further action. Eight monitoring wells have been established in the disposal basins and in the surrounding area to determine the contamination of the site. Sampling took place in the summers of 2020 and 2021. The results of the analysis of the soil and water samples collected showed a high level of contamination in the area. In the borehole profile of the infiltration basin, a well-developed leachate nitrate profile was observed, with a concentration above 3000 mg/kg NO3−. The soil phosphate content was also significant, with a value of over 1900 mg/kg in the upper 40 cm layer. Extremely high concentrations of ammonium (>45 mg/L) and organic matter (>90 mg/L) were detected in the groundwater of the basins, indicating that contaminated soil remains a major source of pollutants more than 10 years after closure. For all micro- and macroelements present in detectable concentrations, a significant increase was observed in the infiltration basin. Our results have revealed that the surroundings are also heavily contaminated. NO3− concentrations above the contamination limit were measured outside the basins. Recultivation of liquid waste disposal sites of similar characteristics is therefore strongly recommended.

Initial optimization of a series of novel imidazo[1,5-a]quinoxaline compounds originated from a heuristic approach combining two known structural moieties towards α5-GABA

The growing significance of renewable energy resources has been one of the most important processes in the field of energy management for the past two decades. This is due to phenomena related to environmental protection, security of supply, economic and rural development, as well as to the abundance of financial incentives (EU grants, constructive national off-take regime). As a consequence of this, the paper concentrates on the general and territorial characteristics of the EU financial instruments for Hungary in the 2007–2013 budgetary period with the objective to enhance the dissemination of renewable energy resources. As an outcome of our research, three very important conclusions were drawn. First of all, less developed regions were more active in submitting proposals though the more developed regions reached higher success rates. Secondly, it was noticed that the “identity” of the successful tenderers influenced the average volume of proposals to a considerable extent. Thirdly, in the case of the successful proposals, the development level and size (of settlements) of the territorial units (districts) had a direct impact on the identity of the applicants and on the average size of proposals.

Abstract: Introduction: Dental care for mentally disabled people poses a growing challenge for healthcare. In Hungary, the number of mentally disabled people needing special dental care is ca. 100 000. Aim: The aim of our retrospective analysis is to provide a summary of the demographic data and the treatment outcomes of patients with mental disorders treated at the Department of the Oral and Maxillofacial Surgery of the Semmelweis University in the past five years. Method: Dental care for patients with a severe level of mental disability can be carried out in general anaesthesia only. At Semmelweis University, in the Oral and Maxillofacial Department, 1717 mentally disabled adults received dental care during the past five years. (Patients included people with a mild, medium or severe level of mental disability, patients with Down’s syndrome, autism, epilepsy or panic disorder.) Results: The single biggest achievement seems to be the fact that the issue of acute dental care and oral surgery has basically been settled. A workable relationship has been forged with foundations and organizations dealing with the problems of these patients. It has been realized, however, that in the case of mentally disabled patients there is an enormous need for prevention and ongoing care. Conclusions: Up to now no survey has been carried out in Hungary with the aim of objectively revealing the dental care needs of these patients. Internationally, however, several surveys have been published. It can be stated on the basis of these that both caries frequency and the presence of parodontal diseases increase in correlation with age and the level of disability. Oral hygiene is insufficient, patients or their caretakers do not get proper information, only a few of them receive adequate training and they are not motivated to keep up oral health. Dental care, except for tending acute cases, is not satisfactory. For the sake of prevention, cooperation is needed with non-governmental organizations, foundations, special education teachers and psychiatrists specialized in this field. Evolutionary potential games provide an analogy for equilibrium statistical physics under suitable conditions that is broken if cyclic components are added. Using Monte Carlo simulations we study the effect of weak cyclic perturbations on the equilibrium in a system of players located on a square lattice while the dynamics is controlled by a logit rule. The pair interaction is composed of a symmetric three-strategy coordination game of unit strength, a weak self-dependent term, and a cyclic (rock–paper–scissors) component. The self-dependent component favors the first (rock) strategy which dominates the system behavior at low noises if the strength of the cyclic component is below a threshold value. In the opposite case the predator of the first strategy (paper) rules the behavior at low noises while the first strategy can occasionally form growing domains (bursts) diminished after the appearance of the third one. The variation of noise modifies the invasion velocities as well as the rate of nucleation processes and yields drastic changes in the strategy frequencies, fluctuations, average payoffs, and spatio-temporal patterns.

Evolutionary potential games represent a set of biological and ecological models equivalent to multiparticle physical systems for a suitable dynamical rule. In these systems the pair interaction is described by a payoff matrix of two-player games possessing a wider class of interactions. Potential games satisfy criteria related to the Kirchhoff laws and have pure Nash equilibria. Using the bi-matrix formalism of game theory we show a simple method for checking the existence of potential which is related to the absence of cyclic components. It will be shown that potential exists if the game is orthogonal to a suitable set of cycling elementary games resembling voluntary matching pennies games. Relationships among these cyclic components and consequences of player’s equivalence are also discussed.

Urban ecosystem services provide a wide range of services to sustain life, social relation, health, etc., and address most of the challenges, including climate change and environmental pollution. While it is recognized that the urban ecosystem substantially contributes to human well-being in cities, there is less attention to consider the value of urban ecosystem service in urban planning and policymaking. This study analyzed the land use and land cover (LULC) dynamics of city of Dhaka over the past three decades (1990–2020) to evaluate the impact of LULC on ecosystem services value (ESV). The estimation of ESV in relation to LULC has been done using the globally used benefits transfer method (BTM). Findings of the study show that built-up area has increased by 188.35% from 1990 to 2020, with an average annual growth rate is about 6.28%The analysis of ESV shows that it has decreased by 59.55% (85 million USD) from 142.72 million USD in 1990 to 57.72 million USD in 2020 due to the development of the built-up area through conversion of agricultural land, waterbodies, and forest and vegetation land. This study also identified that waterbodies are the greatest contributor to ESV. The result on the elasticity of ESV in relation to LULC implies that about 1% transition in LULC would result in about 0.33% change in total ESV during the study period. We believe that the findings of this study would serve as a reference for the policy maker and urban planner to devise appropriate land use decision to ensure sustainable urban development of Dhaka.

Green walls and green dams are increasingly being considered as part of many national and international desertification initiatives. This paper studies the spatiotemporal evolution of the green dam in the Moudjbara region (Djelfa Province, Algeria), from 1972 to 2019, by using Landsat imagery, Land Change Modeler, and OpenLand package. The future evolution of pine plantations, for the year 2029, was also forecasted, based on an anthropogenic scenario (i.e., anthropogenic pressure is the main driver of the green dam destruction). Our findings revealed that the green dam project was successful for a few years, but, after that, pine plantations deteriorated significantly, due to forest harvesting, livestock overgrazing, and the proliferation of the pine caterpillar processionary, which destroyed most of the reforestation. Land change modeler predicted a huge degradation of pine plantations for the year 2029, and if the deforestation continues at the same rate, the green dam in the Moudjbara region will disappear during the next few decades. Being aware of this threat, the Algerian authorities are now planning to reforest more than 1.2 million ha under the latest rural renewal policy, by introducing new principles related to sustainable development, fighting desertification, and climate change adaptation. We strongly recommend moving away from the singular tree planting focus, to diversifying desertification control methods.

It belongs to the heterogenous group of fibro-osseous lesions. Its histology is the same as that of giant-cell granuloma. The bone lesions and fibrous tissue expansion increase before puberty and regress thereafter. For the first time in Hungarian medical literature, the authors discuss the condition of cherubism in the case of three siblings. The diagnosis of these three siblings is supported by the clinical, radiological, microscopic and genetic data. In all three, the bone lesions and fibrous tissue expansion increased before puberty and stabilized thereafter. The radiological results and the molecular findings were nearly identical. The identical mutation shown in the germ lines of the three siblings and the mother correspond to a dominantly inherited syndrome (e.g., cherubism) even if the condition did not manifest in the mother. The authors summarize the treatment options of the disease: surgical (excochleation, decountouring, in rare case resection) and drug (bisphosphonate, calcitonin, steroid, etc.) therapy. They agree with those who are of the opinion that interventions should wait and the patients should be observed ("wait and see") for the expected regression.
