1. INFILTRAÇÃO DO SISTEMA NERVOSO CENTRAL POR LINFOMA DE HODGKIN SIMULANDO MENINGIOMA

Abstract

Objetivos: Descrever ocorrência rara de infiltração do sistema nervoso central (SNC) por linfoma Hodgkin (LH) clássico simulando meningioma e fazer uma revisão de literatura sobre este evento. Materiais e métodos: Relato de caso com revisão de informações de prontuário e revisão de dados da literatura. Resultados: Homem, 33 anos, com diagnóstico de LH clássico subtipo celularidade mista, estadio IIA, há 7 anos. Os tratamentos incluíram quimioterapia (QT) de 1ª linha com 6 ciclos de BEACOPP padrão e radioterapia (RT) de campo envolvido. Findado tratamento, houve abandono de seguimento por 53 meses com readmissão por recaída tardia histologicamente comprovada de LH clássico com massa bulky paravertebral lombar e infiltração pulmonar e isquiosacral (estadio IVX). Tratamento de 2ª linha consistiu de QT com esquema ICE (ifosfamida, carboplatina e etoposideo). Paciente recusou terapia de consolidação por autotransplante e aceitou RT de áreas bulky, após o que ocorreu nova perda de seguimento. Após 6 meses, paciente cursou com disfagia, cefaléia e convulsões tônico-clônicas. Foram identificadas adenomegalias cervicais e paralisia facial central à direita. Estudo por ressonância nuclear magnética de crânio evidenciou volumosa (10 × 8,8 × 4 cm) lesão expansiva frontal extra-axial com invasão do seio sagital superior e sinais de hipertensão intracraniana. Para a elucidação, biópsia da lesão em SNC foi realizada. Achados de biópsia de congelação favoreceram o diagnóstico de meningioma e a ressecção completa da massa foi realizada. Ao anatomopatológico da lesão de SNC evidenciou-se neoplasia fusocelular, sem atipias, permeada por linfócitos pequenos e células atípicas com núcleo grande e hipercromático, considerando-se as hipóteses de LH ou meningioma rico em células linfoplasmocitárias. À imuno-histoquímica revelou marcação CD30+, CD15+, PAX5+, Receptor de Progesterona e Somatostatin Receptor 2 negativos em grandes células sugestivas de células de Hodgkin, achados compatíveis com LH, e semelhantes aos encontrados em nova biópsia de adenomegalia cervical. Paciente permanece em seguimento irregular com adesão insatisfatória à terapia com dexametasona e brentuximabe vedotina, havendo, por isto, a progressão de déficits neurológicos com comprometimento de status performance, logo, cuidados com a intenção não curativa. Discussão: O envolvimento do SNC pelo LH é considerado raro e a incidência estimada é de 0,02-0,07% dos casos. Quando da infiltração do SNC, alteração do estado mental, déficits neurológicos e cefaleia são as alterações mais encontradas. Fundamental a comprovação diagnóstica por meio de biópsia mediante os diagnósticos diferenciais a serem considerados. As evidências sugerem uma variedade de terapias, incluindo RT para envolvimento isolado de SNC e RT associada a QT contendo agentes com penetração em SNC. Metotrexato intratecal, ressecção completa do tumor e autotransplante são também descritos. A infiltração do SNC por LH compromete grandemente o prognóstico e a sobrevida global estimada de 29 a 43 meses. Conclusão: O envolvimento do SNC por LH é raro. A ocorrência de alterações neurológicas não previstas em associação a LH primariamente refratário ou com recaída pode representar cenários em que a suspeita de infiltração do SNC deva ser considerada. A raridade da ocorrência em associação com a ausência de terapia específica torna o prognóstico sombrio.

Leading Cities is a global review of the state of city leadership and urban governance today. Drawing on research into 202 cities in 100 countries, the book provides a broad, international evidence base grounded in the experiences of all types of cities. It offers a scholarly but also practical assessment of how cities are led, what challenges their leaders face, and the ways in which this leadership is increasingly connected to global affairs. Arguing that effective leadership is not just something created by an individual, Elizabeth Rapoport, Michele Acuto and Leonora Grcheva focus on three elements of city leadership: leaders, the structures and institutions that underpin them, and the tools used to drive change. Each of these elements are examined in turn, as are the major urban policy issues that leaders confront today on the ground. The book also takes a deep dive into one particular example of tool or instrument of city leadership – the strategic urban plan.

Abstract

Weakly interacting massive particles (WIMPs) may interact with a virtual pion that is exchanged between nucleons. This interaction channel is important to consider in models where the spin-independent isoscalar channel is suppressed. Using data from the first science run of the LUX-ZEPLIN dark matter experiment, containing 60 live days of data in a 5.5 tonne fiducial mass of liquid xenon, we report the results on a search for WIMP-pion interactions. We observe no significant excess and set an upper limit of 1.5 × 10−46 cm2 at a 90% confidence level for a WIMP mass of 33 GeV/c2 for this interaction.

Abstract

Endotracheal intubation revolutionized anesthesia and allowed prolonged surgical procedures to be performed. It also made positive pressure ventilation possible in patients in respiratory insufficiency and being managed in intensive care units. But successful intubation required the development of laryngoscopes. A variety of blades were developed to be used in different sets of patients. A laryngoscope came to be known as a symbol of the specialty of anesthesiology. But it was not always safe, nor successful, necessitating more sophisticated instruments. Video laryngoscopes were introduced with a sigh of relief for the anesthesiologists in difficult airway cases. Many variations of video laryngoscopes with slight differences have been marketed. But cases of video laryngoscope related injuries have been reported. This paper gives an over-view of the possible mechanism and preventive measures.

DESCRIÇÃO DO MÉTODO DE COLETA DE EVIDÊNCIAS: Reunião consensual e multidisciplinar com a participação de médicos reumatologistas, fisiatra e ortopedista para elaboração do texto final. O texto inicial tomou por base o documento do Colégio Americano de Reumatologia "Guidelines for the management of rheumatoid arthritis" e também o texto das "Diretrizes do Ministério da Saúde para o Diagnóstico e Tratamento da Artrite Reumatóide", acrescido da experiência de reumatologistas, e complementado por extensa revisão bibliográfica, com prioridade para meta-análises. Os participantes, divididos em três grupos, produziram o texto básico, cujas recomendações foram submetidas à revisão e à aprovação pelo conjunto dos participantes. Abstract

Dual-phase xenon time projection chamber (TPC) detectors have demonstratedsuperior search sensitivities to dark matter over a wide range of particlemasses. To extend their sensitivity to include low-mass dark matterinteractions, it is critical to characterize both the light and chargeresponses of liquid xenon to sub-keV nuclear recoils. In this work, we report anew nuclear recoil calibration in the LUX detector $\textit{in situ}$ usingneutron events from a pulsed Adelphi Deuterium-Deuterium neutron generator. Wedemonstrate direct measurements of light and charge yields down to 0.45 keV(1.4 scintillation photons) and 0.27 keV (1.3 ionization electrons),respectively, approaching the physical limit of liquid xenon detectors. Wediscuss the implication of these new measurements on the physics reach ofdual-phase xenon TPCs for nuclear-recoil-based low-mass dark matter detection.

Abstract

We present the results from combining machine learning with the profile likelihood fit procedure, using data from the Large Underground Xenon (LUX) dark matter experiment. This approach demonstrates reduction in computation time by a factor of 30 when compared with the previous approach, without loss of performance on real data. We establish its flexibility to capture nonlinear correlations between variables (such as smearing in light and charge signals due to position variation) by achieving equal performance using pulse areas with and without position-corrections applied. Its efficiency and scalability furthermore enables searching for dark matter using additional variables without significant computational burden. We demonstrate this by including a light signal pulse shape variable alongside more traditional inputs, such as light and charge signal strengths. This technique can be exploited by future dark matter experiments to make use of additional information, reduce computational resources needed for signal searches and simulations, and make inclusion of physical nuisance parameters in fits tractable.

Abstract

We report here the results of a nonrelativistic effective field theory (EFT) WIMP search analysis using LUX data. We build upon previous LUX analyses by extending the search window to include nuclear recoil energies up to ∼180 keVnr, requiring a reassessment of data quality criteria and background models. In order to use an unbinned profile likelihood statistical framework, the development of new analysis techniques to account for higher-energy backgrounds was required. With a 3.14×104 kg·day exposure using data collected between 2014 and 2016, we find our data is compatible with the background expectation and set 90% C.L. exclusion limits on nonrelativistic EFT WIMP-nucleon couplings, improving upon previous LUX results and providing constraints on a EFT WIMP interactions using the {neutron,proton} interaction basis. Additionally, we report exclusion limits on inelastic EFT WIMP-isoscalar recoils that are competitive and world-leading for several interaction operators.

Abstract

This paper presents a novel technique for mitigating electrode backgrounds that limit the sensitivity of searches for low-mass dark matter (DM) using xenon time projection chambers. In the Large Underground Xenon (LUX) detector, signatures of low-mass DM interactions would be very low-energy () scatters in the active target that ionize only a few xenon atoms and seldom produce detectable scintillation signals. In this regime, extra precaution is required to reject a complex set of low-energy electron backgrounds that have long been observed in this class of detector. Noticing backgrounds from the wire grid electrodes near the top and bottom of the active target are particularly pernicious, we develop a machine learning technique based on ionization pulse shape to identify and reject these events. We demonstrate the technique can improve Poisson limits on low-mass DM interactions by a factor of 1.7-3 with improvement depending heavily on the size of ionization signals. We use the technique on events in an effective 5 tonne·day exposure from LUX’s 2013 science operation to place strong limits on low-mass DM particles with masses in the range . This machine learning technique is expected to be useful for near-future experiments, such as LUX-ZEPLIN and XENONnT, which hope to perform low-mass DM searches with the stringent background control necessary to make a discovery.

Abstract

The Large Underground Xenon (LUX) dark matter search was a 250-kg active mass dual-phase time projection chamber that operated by detecting light and ionization signals from particles incident on a xenon target. In December 2015, LUX reported a minimum 90% upper C.L. of 6×10-46 cm2 on the spin-independent WIMP-nucleon elastic scattering cross section based on a 1.4×104 kg·day exposure in its first science run. Tension between experiments and the absence of a definitive positive detection suggest it would be prudent to search for WIMPs outside the standard spin-independent/spin-dependent paradigm. Recent theoretical work has identified a complete basis of 14 independent effective field theory (EFT) operators to describe WIMP-nucleon interactions. In addition to spin-independent and spin-dependent nuclear responses, these operators can produce novel responses such as angular-momentum-dependent and spin-orbit couplings. Here we report on a search for all 14 of these EFT couplings with data from LUX's first science run. Limits are placed on each coupling as a function of WIMP mass.

We present a comprehensive analysis of electronic recoil vs nuclear recoil discrimination in liquid/gas xenon time projection chambers, using calibration data from the 2013 and 2014-2016 runs of the Large Underground Xenon experiment. Abstract

This paper presents a novel technique for mitigating electrode backgroundsthat limit the sensitivity of searches for low-mass dark matter (DM) usingxenon time projection chambers. In the LUX detector, signatures of low-mass DMinteractions would be very low energy ($\sim$keV) scatters in the active targetthat ionize only a few xenon atoms and seldom produce detectable scintillationsignals. In this regime, extra precaution is required to reject a complex setof low-energy electron backgrounds that have long been observed in this classof detector. Noticing backgrounds from the wire grid electrodes near the topand bottom of the active target are particularly pernicious, we develop amachine learning technique based on ionization pulse shape to identify andreject these events. We demonstrate the technique can improve Poisson limits onlow-mass DM interactions by a factor of $2$-$7$ with improvement dependingheavily on the size of ionization signals. Abstract

Dual-phase xenon detectors, as currently used in direct detection dark matter experiments, have observed elevated rates of background electron events in the low energy region. While this background negatively impacts detector performance in various ways, its origins have only been partially studied. In this paper we report a systematic investigation of the electron pathologies observed in the LUX dark matter experiment. We characterize different electron populations based on their emission intensities and their correlations with preceding energy depositions in the detector. By studying the background under different experimental conditions, we identified the leading emission mechanisms, including photoionization and the photoelectric effect induced by the xenon luminescence, delayed emission of electrons trapped under the liquid surface, capture and release of drifting electrons by impurities, and grid electron emission. We discuss how these backgrounds can be mitigated in LUX and future xenon-based dark matter experiments.

Abstract

Two-neutrino double electron capture is a process allowed in the standard model of particle physics. This rare decay has been observed in 78Kr, 130Ba and more recently in 124Xe. In this publication we report on the search for this process in 124Xe and 126Xe using the full exposure of the large underground xenon (LUX) experiment, in a total of 27769.5 kg-days. No evidence of a signal was observed, allowing us to set 90% C.L. lower limits for the half-lives of these decays of 2.0 × 1021 years for 124Xe and 1.9 × 1021 years for 126Xe.

Abstract

We present a novel analysis technique for liquid xenon time projection chambers that allows for a lower threshold by relying on events with a prompt scintillation signal consisting of single detected photons. The energy threshold of the LUX dark matter experiment is primarily determined by the smallest scintillation response detectable, which previously required a twofold coincidence signal in its photomultiplier arrays, enforced in data analysis. The technique presented here exploits the double photoelectron emission effect observed in some photomultiplier models at vacuum ultraviolet wavelengths. We demonstrate this analysis using an electron recoil calibration dataset and place new constraints on the spin-independent scattering cross section of weakly interacting massive particles (WIMPs) down to 2.5 GeV/c2 WIMP mass using the 2013 LUX dataset. This new technique is promising to enhance light WIMP and astrophysical neutrino searches in next-generation liquid xenon experiments.

Abstract

We report here methods and techniques for creating an improved model that reproduces the scintillation and ionization response of a dual-phase liquid and gaseous xenon time projection chamber. Starting with the recent release of the Noble Element Simulation Technique (NEST v2.0), electronic recoil data from the β decays of 3H and 14C in the Large Underground Xenon (LUX) detector were used to tune the model, in addition to external data sets that allow for extrapolation beyond the LUX data-taking conditions. This paper also presents techniques used for modeling complicated temporal and spatial detector pathologies that can adversely affect data using a simplified model framework. The methods outlined in this report show an example of the robust applications possible with NEST v2.0 framework and how it can be modified to produce a final, detector-specific, electronic recoil model. This example provides the final model for LUX and detector parameters that will used in the new analysis package, the LUX Legacy Analysis Monte Carlo Application (LLAMA), for accurate reproduction of the LUX data. As accurate background reproduction is crucial for the success of rare-event searches, such as dark matter direct detection experiments, the techniques outlined here can be used in other single-phase and dual-phase xenon detectors to assist with accurate ER background reproduction.

Abstract

We report here methods and techniques for creating an improved model that reproduces the scintillation and ionization response of a dual-phase liquid and gaseous xenon time projection chamber. Starting with the recent release of the Noble Element Simulation Technique (NEST v2.0), electronic recoil data from the β decays of 3H and 14C in the Large Underground Xenon (LUX) detector were used to tune the model, in addition to external data sets that allow for extrapolation beyond the LUX data-taking conditions. This paper also presents techniques used for modeling complicated temporal and spatial detector pathologies that can adversely affect data using a simplified model framework. The methods outlined in this report show an example of the robust applications possible with NEST v2.0 framework and how it can be modified to produce a final, detector-specific, electronic recoil model. This example provides the final model for LUX and detector parameters that will used in the new analysis package, the LUX Legacy Analysis Monte Carlo Application (LLAMA), for accurate reproduction of the LUX data. As accurate background reproduction is crucial for the success of rare-event searches, such as dark matter direct detection experiments, the techniques outlined here can be used in other single-phase and dual-phase xenon detectors to assist with accurate ER background reproduction.

Abstract

We present the results of a direct detection search for mirror dark matter interactions, using data collected from the Large Underground Xenon experiment during 2013, with an exposure of 95 live-days×118 kg. Here, the calculations of the mirror electron scattering rate in liquid xenon take into account the shielding effects from mirror dark matter captured within the Earth. Annual and diurnal modulation of the dark matter flux and atomic shell effects in xenon are also accounted for. Having found no evidence for an electron recoil signal induced by mirror dark matter interactions we place an upper limit on the kinetic mixing parameter over a range of local mirror electron temperatures between 0.1 and 0.9 keV. This limit shows significant improvement over the previous experimental constraint from orthopositronium decays and significantly reduces the allowed parameter space for the model. We exclude mirror electron temperatures above 0.3 keV at a 90% confidence level, for this model, and constrain the kinetic mixing below this temperature.

Abstract

The scattering of dark matter (DM) particles with sub-GeV masses off nuclei is difficult to detect using liquid xenon-based DM search instruments because the energy transfer during nuclear recoils is smaller than the typical detector threshold. However, the tree-level DM-nucleus scattering diagram can be accompanied by simultaneous emission of a bremsstrahlung photon or a so-called "Migdal" electron. These provide an electron recoil component to the experimental signature at higher energies than the corresponding nuclear recoil. The presence of this signature allows liquid xenon detectors to use both the scintillation and the ionization signals in the analysis where the nuclear recoil signal would not be otherwise visible. We report constraints on spin-independent DM-nucleon scattering for DM particles with masses of 0.4-5  GeV/c^{2} using 1.4×10^{4}  kg day of search exposure from the 2013 data from the Large Underground Xenon (LUX) experiment for four different classes of mediators. This analysis extends the reach of liquid xenon-based DM search instruments to lower DM masses than has been achieved previously.

Abstract

Various dark matter models predict annual and diurnal modulations of dark matter interaction rates in Earth-based experiments as a result of the Earth's motion in the halo. Observation of such features can provide generic evidence for detection of dark matter interactions. This paper reports a search for both annual and diurnal rate modulations in the LUX dark matter experiment using over 20 calendar months of data acquired between 2013 and 2016. This search focuses on electron recoil events at low energies, where leptophilic dark matter interactions are expected to occur and where the DAMA experiment has observed a strong rate modulation for over two decades. By using the innermost volume of the LUX detector and developing robust cuts and corrections, we obtained a stable event rate of 2.3±0.2 cpd/keVee/tonne, which is among the lowest in all dark matter experiments. No statistically significant annual modulation was observed in energy windows up to 26 keVee. Between 2 and 6 keVee, this analysis demonstrates the most sensitive annual modulation search up to date, with 9.2σ tension with the DAMA/LIBRA result. We also report no observation of diurnal modulations above 0.2 cpd/keVee/tonne amplitude between 2 and 6 keVee.

DESCRIÇÃO DO MÉTODO DE COLETA DE EVIDÊNCIAS: Reunião consensual e multidisciplinar com a participação de médicos reumatologistas, fisiatra e ortopedista para elaboração do texto final. O texto inicial tomou por base o documento do Colégio Americano de Reumatologia "Guidelines for the management of rheumatoid arthritis" e também o texto das "Diretrizes do Ministério da Saúde para o Diagnóstico e Tratamento da Artrite Reumatóide", acrescido da experiência de reumatologistas, e complementado por extensa revisão bibliográfica, com prioridade para meta-análises. 27. Death prognosis factors of chronic heart failure in the elderly in Burkina Faso

Abstract

Background: Heart failure is the main reason for hospitalization in the cardiology department of Yalgado Ouédraogo University Hospital. The main causes of heart failure are arterial hypertension and myocardial ischemia. Mortality is high during hospitalization and can reach 48%. The follow-up of patients after hospitalization is difficult with many lost to follow-up. The medium and long term prognosis of chronic heart failure is not known in our country. Objective: Identify death prognosis factors of heart failure in elderly subjects aged at least 60 years old. Patients and Method: We conducted a retrospective, cross-sectional study that consisted of systematic inclusion from hospitalization registers of patients of both sexes aged at least 60 years, hospitalized in the cardiology department CHU Yalgado Ouédraogo from January 1 to December 31, 2016. We assessed the type of heart failure, electrocardiographic and echocardiographic parameters, comorbidities, etiologies of heart failure as well as the intra-hospital evolution during this hospitalization until discharge. After including these patients, they were called to participate in the study (which ran from December 1 to 31, 2020) by giving informed consent. We collected follow-up data (since discharge from the first hospitalization, i.e., one consultation each month for at least 42 months) from the patients’ individual follow-up diaries (rehospitalization, improvement, therapeutic compliance). We then performed a complete clinical examination, an electrocardiogram, a cardiac echography, and a biological workup including blood count, creatinine level, liver function tests, blood glucose and blood ionogram. Results: We included 87 elderly subjects representing 53.05% of all patients received for chronic heart failure in the cardiology department in 2016. The mean age was 71.49 ± 8.46 years and the sex ratio was 2. High blood pressure was the main cardiovascular risk factor in 62.07% of cases. In-hospital mortality was 20.69%, and factors associated with it included male sex (OR= 11.60 p < 0.01), renal dysfunction on admission (OR= 1.30 p= 0.03), and cardiogenic shock developed during hospitalization (OR= 10.30 p < 0.01). Mortality at approximately 4 years was 67.47% with a mean time to death of 13.60 months. No independent factor was found to predict death after discharge. Conclusion: The prognosis of heart failure in the elderly is poor in our context. The in-hospital death prognosis facteur are renal dysfunction, cardiogenic shock, An early and adapted management of this age group will improve survival.

Resume Introduction Au Burkina Faso, la stimulation cardiaque introduite en octobre 2000 a pris de l'ampleur au fil du temps. Afin d’evaluer l'efficacite du programme de stimulation nous nous sommes proposes d’evaluer la qualite de vie des patients porteurs d'un pacemaker. Objectif general .- Etudier l'effet du pacemaker sur la qualite de vie des patients Patients et methodes Il s'est agi d'une etude transversale a visee descriptive des porteurs d'un pacemaker depuis au moins six mois au centre hospitalier universitaire Yalgado Ouedraogo et au centre medical Schiphra. Le questionnaire AQUAREL a ete adapte a notre contexte pour etablir un score de la qualite de vie. Resultats L’âge moyen de la population d’etude etait de 68 ans avec une predominance feminine a 53,3 %. Le bloc atrio-ventriculaire complet etait l'indication principale de la stimulation dans 55 %. La duree moyenne de l'implantation etait de 41,6 mois avec des extremes de 8 et 128 mois. Il s'agissait d'une primo-implantation chez 79,1 % des patients contre 18,3 % pour un premier changement de boitier et 2,5 % pour un deuxieme changement de boitier. Une stimulation double chambre a ete realisee dans 65 % des cas. Le mode DDD etait le plus utilise suivi de VVIR. Le score moyen de la qualite de vie etait de 90,12. Les facteurs predictifs negatifs de la qualite de vie etaient l’âge et le sexe feminin. Une correlation entre le score de qualite de vie et l’âge, l'HTA et les dyslipidemies avait ete retrouvee (p 0,05). Conclusion.- Les patients porteurs d'un pacemaker ont une bonne qualite de vie. Cependant, la qualite de vie n'est pas correlee au nombre de pacemaker, ni la duree et le mode de stimulation.

Abstract

We estimate the amount of 37 Ar produced in natural xenon via cosmic-ray-induced spallation, an inevitable consequence of the transportation and storage of xenon on the Earth’s surface. We then calculate the resulting 37 Ar concentration in a 10-tonne payload (similar to that of the LUX-ZEPLIN experiment) assuming a representative schedule of xenon purification, storage, and delivery to the underground facility. Using the spallation model by Silberberg and Tsao, the sea-level production rate of 37 Ar in natural xenon is estimated to be 0.024 atoms / kg / day . Assuming the xenon is successively purified to remove radioactive contaminants in 1-tonne batches at a rate of 1 tonne / month , the average 37 Ar activity after 10 tons are purified and transported underground is 0.058 − 0.090 μ Bq / kg , depending on the degree of argon removal during above-ground purification. Such cosmogenic 37 Ar will appear as a noticeable background in the early science data, while decaying with a 35-day half-life. This newly noticed production mechanism of 37 Ar should be considered when planning for future liquid-xenon-based experiments.

Abstract

BACKGROUND: Greater understanding of international cancer survival differences is needed. We aimed to identify predictors and consequences of cancer diagnosis through emergency presentation in different international jurisdictions in six high-income countries. METHODS: Using a federated analysis model, in this cross-sectional population-based study, we analysed cancer registration and linked hospital admissions data from 14 jurisdictions in six countries (Australia, Canada, Denmark, New Zealand, Norway, and the UK), including patients with primary diagnosis of invasive oesophageal, stomach, colon, rectal, liver, pancreatic, lung, or ovarian cancer during study periods from Jan 1, 2012, to Dec 31, 2017. Data were collected on cancer site, age group, sex, year of diagnosis, and stage at diagnosis. Emergency presentation was defined as diagnosis of cancer within 30 days after an emergency hospital admission. Using logistic regression, we examined variables associated with emergency presentation and associations between emergency presentation and short-term mortality. We meta-analysed estimates across jurisdictions and explored jurisdiction-level associations between cancer survival and the percentage of patients diagnosed as emergencies. FINDINGS: In 857 068 patients across 14 jurisdictions, considering all of the eight cancer sites together, the percentage of diagnoses through emergency presentation ranged from 24·0% (9165 of 38 212 patients) to 42·5% (12 238 of 28 794 patients). There was consistently large variation in the percentage of emergency presentations by cancer site across jurisdictions. Pancreatic cancer diagnoses had the highest percentage of emergency presentations on average overall (46·1% [30 972 of 67 173 patients]), with the jurisdictional range being 34·1% (1083 of 3172 patients) to 60·4% (1317 of 2182 patients). Abstract

Se destaca la importancia de las actividades de reparación y mantenimiento (RyM) que contribuyen a prolongar la vida útil de los bienes como una componente clave en transición a una economía circular (EC) y sostenible. Se analiza empíricamente el impacto del Impuesto al Valor Agregado (IVA) en las actividades de RyM y se realiza un análisis crítico de su tratamiento fiscal en la Ley del IVA (LIVA) y el régimen de beneficios fiscales existentes en la misma. Sobre esa base se formula una propuesta de reforma del IVA centrada en beneficios tasa 0 y exenciones. Se cuantifica el impacto y su coste fiscal, así como vías para compensar ese coste fiscal.

Abstract

The nature of dark matter and properties of neutrinos are among the most pressing issues in contemporary particle physics. The dual-phase xenon time-projection chamber is the leading technology to cover the available parameter space for Weakly Interacting Massive Particles (WIMPs), while featuring extensive sensitivity to many alternative dark matter candidates. These detectors can also study neutrinos through neutrinoless double-beta decay and through a variety of astrophysical sources. A next-generation xenon-based detector will therefore be a true multi-purpose observatory to significantly advance particle physics, nuclear physics, astrophysics, solar physics, and cosmology. Abstract

The projected sensitivity of the LUX-ZEPLIN (LZ) experiment to two-neutrino and neutrinoless double β decay of 134 Xe is presented. LZ is a 10-tonne xenon time-projection chamber optimized for the detection of dark matter particles and is expected to start operating in 2021 at Sanford Underground Research Facility, USA. Its large mass of natural xenon provides an exceptional opportunity to search for the double β decay of 134 Xe , for which xenon detectors enriched in 136 Xe are less effective. For the two-neutrino decay mode, LZ is predicted to exclude values of the half-life up to 1.7 × 10 24 years at 90% confidence level (CL) and has a three-sigma observation potential of 8.7 × 10 23 years, approaching the predictions of nuclear models. Abstract

LUX-ZEPLIN is a dark matter detector expected to obtain world-leading sensitivity to weakly-interacting massive particles interacting via nuclear recoils with a ∼7-tonne xenon target mass. This paper presents sensitivity projections to several low-energy signals of the complementary electron recoil signal type: 1) an effective neutrino magnetic moment, and 2) an effective neutrino millicharge, both for pp-chain solar neutrinos, 3) an axion flux generated by the Sun, 4) axionlike particles forming the Galactic dark matter, 5) hidden photons, 6) mirror dark matter, and 7) leptophilic dark matter. Abstract

Background Extracorporeal membrane oxygenation (ECMO) was frequently used to treat patients with severe coronavirus disease-2019 (COVID-19)-associated acute respiratory distress (ARDS) during the initial outbreak. Care of COVID-19 patients evolved markedly during the second part of 2020. Our objective was to compare the characteristics and outcomes of patients who received ECMO for severe COVID-19 ARDS before or after July 1, 2020. Methods We included consecutive adults diagnosed with COVID-19 in Paris–Sorbonne University Hospital Network ICUs, who received ECMO for severe ARDS until January 28, 2021. Characteristics and survival probabilities over time were estimated during the first and second waves. Pre-ECMO risk factors predicting 90-day mortality were assessed using multivariate Cox regression. Results Characteristics of the 88 and 71 patients admitted, respectively, before and after July 1, 2020, were comparable except for older age, more frequent use of dexamethasone (18% vs. 82%), high-flow nasal oxygenation (19% vs. 82%) and/or non-invasive ventilation (7% vs. 37%) after July 1. Respective estimated probabilities (95% confidence intervals) of 90-day mortality were 36% (27–47%) and 48% (37–60%) during the first and the second periods. After adjusting for confounders, probability of 90-day mortality was significantly higher for patients treated after July 1 (HR 2.27, 95% CI 1.02–5.07). ECMO-related complications did not differ between study periods. Conclusions 90-day mortality of ECMO-supported COVID-19–ARDS patients increased significantly after July 1, 2020, and was no longer comparable to that of non-COVID ECMO-treated patients. Failure of prolonged non-invasive oxygenation strategies before intubation and increased lung damage may partly explain this outcome.

Abstract

In Burkina Faso, cardiac stimulation was introduced in October 2000 has grown over time.In orderto evaluate the effectiveness of stimulation on life, we proposed to evaluate the quality of life of patients with a pacemaker. GENERAL OBJECTIVE: to study the effect of the pacemaker on the quality of life of patients PATIENTS AND METHODS: This was a cross-sectional study aimed at describingthe wearers of a pacemaker for at least six months at the Yalgado OUEDRAOGO University Hospital and the Schiphramedical center.The AQUAREL questionnaire has been adapted to our context to establish a quality-of-life score.The mean age of the study population was 68 years with a female predominance (53.3%). Complete atrioventricular block was the main indication of stimulation in 55%. The average duration of implantation was 41.6 months with extremes of 8 and 128 months. The primo implantation was found in 79.1 % of patients against 18.3 % for a first change of case and2.5 % for a second change of housing. Double chamber stimulation was performed in 65% of cases. DDD mode was the most used followed by VVIR. The average quality of life score was 90.12. Negative predictors of quality of life were age and female.A correlation between quality-of-life score and age, hypertension and dyslipidemia was found (p0.05). On the other hand, no correlation between the number of implantation, the duration of implantation and the mode of stimulation was found (p0.05).patients with a pacemaker have a good quality of life. However, quality-of-life is not correlated with the number of pacemakers, nor the duration and mode of stimulation.

Abstract

This paper presents a novel technique for mitigating electrode backgrounds that limit the sensitivity of searches for low-mass dark matter (DM) using xenon time projection chambers. In the LUX detector, signatures of low-mass DM interactions would be very low energy ($\sim$keV) scatters in the active target that ionize only a few xenon atoms and seldom produce detectable scintillation signals. In this regime, extra precaution is required to reject a complex set of low-energy electron backgrounds that have long been observed in this class of detector. Noticing backgrounds from the wire grid electrodes near the top and bottom of the active target are particularly pernicious, we develop a machine learning technique based on ionization pulse shape to identify and reject these events. We demonstrate the technique can improve Poisson limits on low-mass DM interactions by a factor of $2$-$7$ with improvement depending heavily on the size of ionization signals. We use the technique on events in an effective $5$ tonne$\cdot$day exposure from LUX's 2013 science operation to place strong limits on low-mass DM particles with masses in the range $m_{\chi}\in0.15$-$10$ GeV. Abstract

Anesthetic management of patients with mitochondrial disorders is challenging due to their rare nature, and difficulties to develop evidence-based protocols for general anesthesia. We report a case of a 52-year-old male with hypertension, congenital deafness, optic neuritis, hypogonadism and myotonia, who developed severe refractory hypotension and bradycardia, and intractable severe lactic acidosis during induction of general anesthesia with fentanyl, propofol, and succinylcholine. The surgery had to be postponed and supportive management started. After extensive work-up, the patient was diagnosed with probable mitochondrial dysfunction. Patient returned for surgery under a different anesthetic regimen consisting of ketamine, fentanyl, ephedrine and dexmedetomidine; yielding a successful perioperative course and highlighting potential alternative strategies to manage anesthesia in patients with such disorders.

Abstract

The Large Underground Xenon (LUX) dark matter search was a 250-kg active mass dual-phase time projection chamber that operated by detecting light and ionization signals from particles incident on a xenon target. In December 2015, LUX reported a minimum 90% upper C.L. of 6e-46 cm^2 on the spin-independent WIMP-nucleon elastic scattering cross section based on a 1.4e4 kg*day exposure in its first science run. Tension between experiments and the absence of a definitive positive detection suggest it would be prudent to search for WIMPs outside the standard spin-independent/spin-dependent paradigm. Recent theoretical work has identified a complete basis of 14 independent effective field theory (EFT) operators to describe WIMP-nucleon interactions. In addition to spin-independent and spin-dependent nuclear responses, these operators can produce novel responses such as angular-momentum-dependent and spin-orbit couplings. Abstract

The LUX-ZEPLIN (LZ) experiment will enable a neutrinoless double β decay search in parallel to the main science goal of discovering dark matter particle interactions. We report the expected LZ sensitivity to 136 Xe neutrinoless double β decay, taking advantage of the significant ( > 600 kg) 136 Xe mass contained within the active volume of LZ without isotopic enrichment. After 1000 live-days, the median exclusion sensitivity to the half-life of 136 Xe is projected to be 1.06 × 10 26 years (90% confidence level), similar to existing constraints. We also report the expected sensitivity of a possible subsequent dedicated exposure using 90% enrichment with 136 Xe at 1.06 × 10 27 years.

Here, the calculations of the mirror electron scattering rate in liquid xenon take into account the shielding effects from mirror dark matter captured within the Earth. Annual and diurnal modulation of the dark matter flux and atomic shell effects in xenon are also accounted for. Having found no evidence for an electron recoil signal induced by mirror dark matter interactions we place an upper limit on the kinetic mixing parameter over a range of local mirror electron temperatures between 0.1 and 0.9 keV. This limit shows significant improvement over the previous experimental constraint from orthopositronium decays and significantly reduces the allowed parameter space for the model. We exclude mirror electron temperatures above 0.3 keV at a 90% confidence level, for this model, and constrain the kinetic mixing below this temperature.
