It is since the beginning of the so-called ‘digital revolution’ in the 1950s that technological tools have been developed to simplify and optimise traditional, time-consuming, and laborious anamnestic collection for many physicians. In recent years, more and more sophisticated ‘automated’ anamnestic collection systems have been developed, to the extent that they can actually enter daily clinical practice. This article not only provides a historical overview of the evolution of such tools, but also explores the ethical and medico-legal implications of the transition from traditional to digital anamnesis, including the protection of data confidentiality, the preservation of the communicative effectiveness of the doctor–patient dialogue and the safety of care in patients with poor digital and health literacy.

Abstract In forensic commingled contexts, when the disarticulation occurs uniquely at the atlantoaxial joint, the correct match of atlas and axis may lead to the desirable assembly of the entire body. Notwithstanding the importance of this joint in such scenarios, no study has so far explored three-dimensional (3D) methodologies to match these two adjoining bones. In the present study, we investigated the potential of re-associating atlas and axis through 3D–3D superimposition by testing their articular surfaces congruency in terms of point-to-point distance (Root Mean Square, RMS). We analysed vertebrae either from the same individual (match) and from different individuals (mismatch). The RMS distance values were assessed for both groups (matches and mismatches) and a threshold value was determined to discriminate matches with a sensitivity of 100%. The atlas and the corresponding axis from 41 documented skeletons (18 males and 23 females), in addition to unpaired elements (the atlas or the axis) from 5 individuals, were superimposed, resulting in 41 matches and 1851 mismatches (joining and non-joining elements). No sex-related significant differences were found in matches and mismatches (p = 0.270 and p = 0.210, respectively), allowing to pool together the two sexes in each group. Pneumonia is a common disease affecting Alpine chamois. However, little is known concerning the etiological agents involved. We investigated whether Mycoplasma spp. infection occurs in Alpine chamois and describe the microscopic lesions associated with Mycoplasma-associated bronchopneumonia in this species. Lung tissues obtained from 45 chamois with gross evidence of pneumonia were analysed. The histological lesions and the presence of lungworms within the lungs were evaluated blindly. The presence of Mycoplasma spp. was assessed by immunohistochemistry (Mycoplasma bovis and Mycoplasma mycoides subsp. mycoides) and by end-point PCR. M. bovis was detected by immunohistochemistry and confirmed by PCR and sequencing in 6/45 (13%) cases, while all lungs were negative for M. mycoides subsp. mycoides. A significant association was found between the detection of M. bovis and the presence of severe lungworms infection in the examined lungs. We report for the first time M. bovis as a bacteria associated with verminous pneumonia in chamois.

Abstract Forensic anthropologists dealing with personal identification (PI) of human remains have recently stressed the need to explore the potential of “secondary identifiers” for identifying victims who died in particular events for whom images often represent the main antemortem data available. Being the face the part most exposed in images, characteristics as pigmented skin lesions (PSLs), can be crucial if combined with other input. Since no data is available on frequencies and distribution of facial PSLs in the general population, this study aims at systematically collecting such data to verify their potential in PI and to open a debate on the aid that “secondary identifiers”, regardless of their specific nature, can give to the identification of the deceased in specific forensic contexts. A retrospective analysis on three-dimensional facial models of 1039 Italian subjects (from 4 to 84 years old) was conducted to examine the incidence of PSLs discriminated according to size and position in well-defined facial areas. From the collected data we developed a probabilistic approach providing the likelihood ratio (LR) for two settings: (1) the relative frequencies of nevi in the various facial areas, providing the deriving compound probability of owning a certain facial PSLs pattern; and (2) codes describing the facial nevi pattern of each individual of our population, thus testing their uniqueness and so their potential in PI. The calculated LRs mostly proved high identifying strength, particularly when provided by the compound probability-based approach. Data on incidence and position of facial nevi, their generated codes, and the probabilistic approach here presented, all constitute a starting point for advancing secondary identifiers. Nonetheless, although this preliminary study proved facial PSLs as valuable and potentially useful for identification, their significance and validity should be interpreted with caution as we are still at the first theoretical step clearly based on ideal conditions, and thus further investigations are due on the limitations of their use in practical identifying settings. Therefore, being this systematic study only a preliminary one in its nature, it is recommended not to use this kind of approach until further studies will test its validity in several practical conditions.

Long-term care insurance (LTCI) plays a crucial role in providing substantial aid in non-self-sufficient situations and complementing existing state protection mechanisms. With an aging population and increasing demand for healthcare services LTC policies have become indispensable. While individual LTCI policies face adoption challenges, group insurances offer a more streamlined alternative. However, realizing the full potential of these insurances necessitates targeted legislative intervention to improve accessibility and ensure sustainability. This article explores the evolution of LTCI policies in Italy, offering an overview of the current landscape and highlighting the socio-economic and medico-legal factors shaping the present scenario. By providing this analysis, we seek to offer insights into the dynamic evolution of LTCI policies and the crucial role of legislative measures in enhancing their effectiveness and accessibility.

Contextualización: el agua es indispensable para el desarrollo de las plantas y su respuesta al déficit hídrico varía según las especies y las etapas de crecimiento de estas. Vacío de conocimiento: se desconoce la magnitud del efecto del déficit hídrico del suelo en el crecimiento de las plantas jóvenes de café (Coffea arabica L.) evaluado en el tiempo. Propósito: se realizó una investigación tendiente a identificar el efecto del déficit hídrico del suelo y por consiguiente la sensibilidad a la sequía en plantas jóvenes de café. Metodología: se utilizó el método de riego deficitario. Se plantaron colinos de café variedad Cenicafé 1 de 3 meses de edad en materos cilíndricos y se acondicionaron por cuatro meses en casas de malla en Cenicafé, Planalto y posterior al acondicionamiento, por 140 días se evaluó el efecto en el crecimiento y la materia seca de las plantas sometidas a cinco rangos de potencial mátrico del suelo. Los potenciales iban desde -20±10 kPa (testigo) hasta-600±100 kPa Resultados y conclusiones: los resultados mostraron que el déficit hídrico del suelo afecta el crecimiento de las plantas jóvenes de café (C. arabica), lo que podría definir a C. arabica en esta etapa como sensible a la sequía. Las plantas sometidas al menor estrés y a sequía extrema presentaron una disminución del 30% y 57% en materia seca, respectivamente. Para las variables de crecimiento, el efecto del déficit hídrico se evidenció en las evaluaciones de los 77, 113 y 140 días de aplicación de tratamientos. En todos los casos, los resultados mostraron diferencias entre la sequía extrema y el testigo de hasta 30 cm en altura y diámetro ecuatorial, 0.9 m2 de área foliar, 132 hojas, 3 cruces y 66 nudos. Los modelos matemáticos que explicaron el efecto de la sequía en la masa seca fueron de tipo logarítmico. Se identificó que para condiciones climáticas y de suelo similares a las del estudio, las plantas jóvenes de café inician el deterioro a los 40 días de sequía. Con un potencial mátrico del suelo cercano a -40 kPa, por un tiempo mantenido de 120 días, las plantas de café pueden perder un 10% de masa seca y cuando el potencial supera los -90 kPa se pueden dar pérdidas mayores en materia seca y disminuciones en el número de cruces entre los 77 y 113 días.

In aquaculture, Lactococcus garvieae is a common fish pathogen that can cause significant economic losses in several fresh and saltwater species. Despite the extensive range of hosts, L. garvieae infection in sea bass (Dicentrarchus labrax) has rarely been reported. During the summer of 2023, an outbreak occurred in an inland farm in the Gulf of Follonica (Tuscany, Italy). Fish of various sizes were affected, showing apathy, inappetence, erratic swimming and eye lesions, while the mortality was low (2–3% per month). Anatomopathological examinations suggested a septicaemic infection characterised by melanosis, diffuse redness (skin and fins), paleness (gills and internal organs), haemorrhages and splenomegaly. Seventy swabs from the viscera of 14 subjects were collected and colonies similar to Streptococcus spp. grew from all the samples. Lactococcus garvieae was identified via the biochemical tests, API20STREP, MALDI-TOF, 16S rDNA and whole genome sequencing. Genetical characterisation revealed remarkable differences between this isolate and the strains previously isolated in Italian fish farms. Feed treatments with flumequine and erythromycin were ineffective. Considering the limited effects of antimicrobials, preventive measures, such as vaccination and biosecurity, should be implemented.

Negli anni Settanta ci fu un improvviso risveglio dell’attenzione sulle lettere, i diari e le memorie dei soldati semplici e della gente comune. Il saggio descrive i protagonisti, le tappe e le caratteristiche di questa rivoluzione di prospettiva sull’evento sconvolgente che ha aperto il secolo ventesimo.

Recent years have seen the emergence of new technologies that exploit nanoscale evaporation, ranging from nanoporous membranes for distillation to evaporative cooling in electronics. Despite the increasing depth of fundamental knowledge, there is still a lack of simulation tools capable of capturing the underlying non-equilibrium liquid–vapour phase changes that are critical to these and other such technologies. This work presents a molecular kinetic theory model capable of describing the entire flow field, i.e. the liquid and vapour phases and their interface, while striking a balance between accuracy and computational efficiency. In particular, unlike previous kinetic models based on the isothermal assumption, the proposed model can capture the temperature variations that occur during the evaporation process, yet does not require the computational resources of more complicated mean-field kinetic approaches. We assess the present kinetic model in three test cases: liquid–vapour equilibrium, evaporation into near-vacuum condition, and evaporation into vapour. The results agree well with benchmark solutions, while reducing the simulation time by almost two orders of magnitude on average in the cases studied. The results therefore suggest that this work is a stepping stone towards the development of an accurate and efficient computational approach to optimising the next generation of nanotechnologies based on nanoscale evaporation. Three-dimensional bioprinting is a rapidly evolving technology that holds the promise of addressing the increasing demand for organs, tissues, and personalized medicine. By employing computer-aided design and manufacturing processes, 3D bioprinting allows for the precise deposition of living cells, biomaterials, and biochemicals to create functional human tissues and organs. The potential applications of this technology are vast, including drug testing and development, disease modeling, regenerative medicine, and ultimately, organ transplantation. However, as with any groundbreaking technology, 3D bioprinting presents several ethical, legal, and regulatory concerns that warrant careful consideration. As the technology progresses towards clinical applications, it is essential to address these challenges and establish appropriate frameworks to guide the responsible development of 3D bioprinting. This article, utilizing the Arksey and O’Malley scoping review model, is designed to scrutinize the bioethical implications, legal and regulatory challenges, and medico-legal issues that are intertwined with this rapidly evolving technology.

Abstract Computed tomography (CT) is considered the gold standard technique for the assessment of trauma patients with suspected involvement of the eye and orbit. These traumas can result in dramatic consequences to visual function, ocular motility, and aesthetics. CT is a quick and widely available imaging modality, which provides a detailed evaluation of the orbital bony and soft tissue structures, an accurate assessment of the globes, and is used to guide the patients’ treatment planning. For a timely and accurate diagnosis, radiologists should be aware of fracture patterns and possible associated complications, ocular detachments and hemorrhages, and different appearances of intraorbital foreign bodies. This educational review aims to describe all post-traumatic orbital abnormalities that can be identified on CT, providing a list of tips and a diagnostic flowchart to help radiologists deal with this complex condition.

Healthcare in the third millennium is largely delivered through systems involving the use of the technological devices and services, foremost among them telemedicine. For the adequate delivery of digital medicine services, however, it is necessary for users to be digitally literate, that is, able to consciously make use of technology. In order to understand how relevant digital literacy is in determining the effectiveness of e-Health services, we performed a traditional literature review on 3 major databases by combining the terms “Digital Literacy” and “Computer Literacy” with the terms “Telemedicine” and “Telehealth”. Starting from an initial library of 1,077 papers, we selected 38 articles. At the outcome of the search, we found that digital literacy is a pivotal element in conditioning the effectiveness of telemedicine and digital medicine services in general, however, with some limitations.

IntroductionX-linked adrenoleukodystrophy (X-ALD) is the most common inherited peroxisomal disorder caused by variants in the ABCD1 gene. The main phenotypes observed in men with X-ALD are primary adrenal insufficiency, adrenomyeloneuropathy, and cerebral ALD (cALD). Cerebral ALD consists of a demyelinating progressive cerebral white matter (WM) disease associated with rapid clinical decline and is fatal if left untreated. Hematopoietic stem cell transplantation is the standard treatment for cALD as it stabilizes WM degeneration when performed early in the disease. For this reason, early diagnosis is crucial, and several countries have already implemented their newborn screening programs (NBS) with the assessment of C26:0-lysophosphatidylcholine (C26:0-LPC) values as screening for X-ALD.MethodsIn June 2021, an Italian group in Lombardy launched a pilot study for the implementation of X-ALD in the Italian NBS program. A three-tiered approach was adopted, and it involved quantifying the values of C26:0-LPC and other metabolites in dried blood spots with FIA-MS/MS first, followed by the more specific ultra-performance liquid chromatography-tandem mass spectrometry (UHPLC-MS/MS) technique and, finally, the genetic confirmation via focused NGS.DiscussionGenetically confirmed patients are set to undergo a follow-up protocol and are periodically evaluated to promptly start a specific treatment if and when the first signs of brain damage appear, as suggested by international guidelines. A specific disease monitoring protocol has been created based on literature data and personal direct experience.ConclusionThe primary aim of this study was to develop a model able to improve the early diagnosis and subsequent follow-up and timely treatment of X-ALD.EthicsThe study was approved by the local ethics committee.
