Giant-cell tumor of the skull is extremely rare. Surgery is the main treatment for this disease, but not all cases are suitable for complete resection. In this report, we present the clinical features of a case of giant-cell tumor of temporal bone that demonstrated good outcome after radiation therapy (RT) using volumetric-modulated arc therapy (VMAT). The patient was a 55-year-old man with giant-cell tumor of temporal bone who received surgery as the first treatment. Three months after the initial surgery, the tumor regrew, and the patient received surgical resection again. Although second partial resection was undergone, it regrew. Therefore, 36 months after initial surgery, RT was conducted. The prescribed dose was 54 Gy in 1.8 Gy fractions using VMAT. The tumor began to shrink from 4 months after the initiation of RT and kept shrinking slowly and gradually. At the last follow-up, there was no evidence of local recurrence. There was no report about VMAT for giant-cell tumor of the skull, and no report revealed the radiographic details after recent radiation techniques. Therefore, this case report was meaningful in describing the details and response during and after VMAT for giant-cell tumor of temporal bone. The adjuvant RT using VMAT seemed to demonstrate a sufficient local control benefit without severe adverse effects in our case with giant-cell tumor of temporal bone.

Introduction: Giant cell tumor (GCT) of the bone, although benign, poses significant challenges due to its locally aggressive nature and high recurrence rates post-surgical intervention. Among skeletal GCTs, those affecting the distal radius present unique difficulties, especially when fungating masses encase vital structures such as blood vessels., Case Report: We present a case of a 28-year-old male with a recurring GCT of the distal radius, where limb-preserving surgery was successfully performed despite encasement of the radial artery. Surgical excision followed by wrist arthrodesis utilizing ulnar centralization resulted in satisfactory outcomes, emphasizing the importance of tailored surgical approaches for optimal functional preservation. Background: Liquid nitrogen (LN) has been used as an adjuvant cryotherapy for bone tumors including giant-cell tumor of the bone (GCTB) to remove residual tumor cells after curettage. This study evaluated variables related to the efficacy of LN-based cryoablation in the context of adjuvant treatment of GCTB using porcine femur bone model. Methods: A porcine femur bone model was adopted to simulate intralesional cryotherapy. A LN-holding cavity (point 1, nadir) in the medial epicondyle, 4 holes (points 2–5) in the shaft situated 5, 10, 15, and 20mm away from the proximal edge of the cavity, and 2 more holes (points 6 and 7) in the condyle cartilage (10 and 20mm away from the distal edge of the cavity) were made. The cooling rate was compared between the 5 points. The cellular morphological changes and DNA damage in the GCTB tissue attributable to LN-based cryotherapy were determined by H&E stain and TUNEL assay. Cartilage tissue at points 6 and 7 was examined for the extent of tissue injury after cryotherapy. Results: The temperature kinetics at points 1, 2 reached the reference target and were found to be significantly better than the reference (both p < 0.05). The target temperature kinetics were not achieved at points 4 and 5, which showed a significantly lower cooling rate than the reference (both p < 0.001) without reaching the –60°C target. Compared with untreated samples, significantly higher proportion of shrunken or apoptotic cells were found at points 1–3; very small proportion were observed at points 4, 5. Significantly increased chondrocyte degeneration was observed at point 6, and was absent at point 7. Conclusion: The cryotherapy effective range was within 5mm from nadir. Complications were restricted to within this distance. The cooling rate was unchanged after three repeated cycles of cryotherapy. [ABSTRACT FROM AUTHOR]

Giant-cell tumour (GCT) of the bone is a benign neoplasm which commonly occurs in the distal femur, proximal tibia, and distal radius. The proximal humerus is a relatively rare site of occurrence, with a prevalence rate of 4%. We report a case of a 40 years-old male with a recurrent GCT of the proximal humerus. Wide excision and hemiarthroplasty of the shoulder was performed. However, the tumour recurred. Asymptomatic metastasis to the right lower lobe of the lung was found on plain radiograph. We suggest that multiple surgeries would increase the risk of local recurrence of GCT that will lead to pulmonary metastasis.

El tumor de células gigantes espinal es una neoplasia de estirpe benigna y comportamiento local agresivo, de presentación rara en la población pediátrica. El dolor asociado al compromiso neurológico es la presentación típica en niños. La resección amplia del tumor y la descompresión de los elementos neurales asociada a fusión instrumentada permiten obtener una columna estable, preservar o restaurar la función neural y evitar la recidiva tumoral. Se presentan dos pacientes con tumor de células gigantes espinal en esqueleto inmaduro, sometidos a resección y estabilización mediante artrodesis instrumentada, sin recidiva en el posoperatorio alejado. La presentación clínica, la edad de los pacientes y los resultados quirúrgicos a largo plazo hacen de extremo valor el reporte de estos casos en esqueletos inmaduros.

Primary patellar tumors are highly unusual. Most are benign neoplasms with giant-cell tumors being the most common, followed by chondroblastomas and aneurysmal bone cysts. Intralesional curettage and bone grafting is the treatment of choice for most giant-cell tumors in the patella. The use of adjuvants can reduce the high recurrence rate. This is a case report of a giant-cell tumor in a 61-year-old man who was diagnosed with a pathological fracture in the patella after minimal trauma. Extended curettage through an osteotomy made in the medial cortical of the patella was performed. The tumor cavity was filled with high viscosity bone cement and the medial cortical was repositioned. Histological analysis showed mononuclear cells and numerous multinucleated giant cells, confirming the diagnosis. Twenty-four months after surgery, the patient was asymptomatic and there was no evidence of local recurrence. Epidemiology, symptomatology, imagenology, histopathology, treatment options and prognoses of giant-cell tumors of the patella are discussed herein. [Arch Clin Exp Surg 2017; 6(2.000): 112-116]

Giant cell tumors are rare, locally aggressive non-odontogenic osteolytic tumors associated with high rates of local recurrence. Treatment modalities are subject to considerable controversy, with successful outcomes hinging on achieving complete tumor elimination through thorough curettage. A 78-year-old male referred in December 2023 for a persistent mucosal lesion in the right maxilla under a removable denture. Clinical examination revealed a well-defined erythematous nodular lesion measuring approximately 3 cm along its long axis, localized on ridge quadrant 1. Biopsy confirmed the diagnosis of giant cell tumor. Although complete resection with healthy margins may be justified for aggressive lesions, it often results in significant morbidity and requires immediate defect reconstruction. Some studies suggest favorable long-term outcomes with guided bone regeneration (GBR). The bone lamina technique uses a xenogeneic cortical bone membrane to maintain space and promote bone healing. This surgical approach promotes bone healing through the mechanical support and biological properties of the lamina. Although giant-cell tumor (GCT) of the bone was originally classified as a benign tumor, metastasis has been reported. The radiographic features usually comprise parenchymal solitary or multiple nodules that are round-to-oval nodular opacities of homogeneous density in patients with GCT. However, the patient described in this case presented with a hypervascular mass with feeding vessels and hemothorax, which are common features of pulmonary arteriovenous malformation. To the best of our knowledge, cases of pulmonary metastases presenting as a pulmonary arteriovenous malformation have not been reported. Here, we report a case of giant-cell tumor of the bone that exhibited histologically benign pulmonary metastases and mimicked an arteriovenous malformation.

The expression of matrix metalloproteinase-2 (MMP-2) and tissue inhibitor of matrix metalloproteinase-3 (TIMP-3) in giant-cell tumor of bone (GCT), and the correlation of their expression with the clinicopathologic features and prognosis were investigated. A total of 70 GCT patients treated in our hospital from September, 2013 to September, 2015, were selected, and the tumor and para-carcinoma tissues were obtained by surgery. The expression levels of MMP-2 and TIMP-3 in GCT and para-carcinoma tissues were detected via semi-quantitative polymerase chain reaction (PCR) and western blot analysis, as well as immunohistochemical staining. Moreover, the clinicopathological data of the GCT patients were collected to study the correlation of MMP-2 and TIMP-3 with clinicopathological features and prognosis of GCT. The results of semi-quantitative PCR and western blot analysis revealed that the expression level of MMP-2 in tissues of the 70 GCT patients was significantly higher than that in para-carcinoma tissues, and the difference was statistically significant (P<0.01), while the expression level of TIMP-3 was obviously lower than that in para-carcinoma tissues (P<0.01). The results of immunohistochemical staining revealed that the positive expression rate of MMP-2 was 57.6% in GCT tissues and 18.9% in para-carcinoma tissues, while that of TIMP-3 was 63.2% in GCT tissues and 13.8% in para-carcinoma tissues, and the differences were statistically significant (P<0.01). The expression levels of MMP-2 and TIMP-3 were correlated with the diameter of tumor, clinical staging, lymph node metastasis and relapse of GCT (P<0.01), but were not correlated with the age and sex of GCT patients (P>0.05). There was a negative correlation between MMP-2 and TIMP-3 expression levels (r=−0.258, P<0.05). The expression levels of MMP-2 and TIMP-3 are closely related to the clinicopathological features and prognosis of patients, which can be used as one of the clinical examination indexes of GCT and also provide new insights for the clinical treatment of GCT. Background:Resection of the distal end of the radius is indicated in the treatment of locally aggressive primary benign and malignant bone tumors.The aim of this study was to evaluate the technique of osteoarticular allograft reconstruction of the distal radius after wide excision of a giant-cell tumor.Methods: We analyzed 15 patients retrospectively who had reconstruction of the distal aspect of the radius with use of an osteoarticular allograft, between 1981 and 2005 after excision of a giant-cell tumor with a minimum follow-up of 2 years (range: 26–125 months, median: 45 months). Results: 15 consecutive patients with a Campanacci grade 3 giant-cell tumor of the distal radius formed the study population. Three patients had a local recurrence at 8, 14 and 18 months. Non-union of the osteotomy line was diagnosed 6 months after surgery in one case and needed bone grafting. Distal radio–ulnar joint instability was observed in nine cases. Subchondral bone alterations and joint narrowing were present in all cases but were painful in only one patient. Five patients needed a revision of the osteoarticular allograft, at an average of 5.4 years (range: 0.8 to 12 years) after the initial reconstruction. The reason for the revision was a fracture of the allograft in four patients and recurrence of the tumor in one. Of the fifteen patients in whom the osteoarticular allograft survived, five patients reported no functional limitation, eight had limitation in the ability to perform strenuous activities, and two had limitation in the ability to perform moderate activities. The average range of motion of the wrist was 35 degrees of dorsiflexion, 30 degrees of volar flexion, 10 degrees of radial deviation, 14 degrees of ulnar deviation, 55 degrees of supination, and 70 degrees of pronation.Conclusion: Reconstruction of the distal aspect of the radius with use of an osteoarticular allograft was associated with a low rate of recurrence of the tumor, a moderately high rate of revision, and little pain in association with common activities, good function, and a moderate range of motion.

A giant cell tumor of the bone is among the most common bone tumors encountered by orthopedic surgeons. These benign and aggressive tumors are most commonly present around the knee joint; however, rare cases may involve the distal fibula. An 18-year-old man presented with a painless swelling of the lateral aspect of the left ankle. Clinical examination, radiologic evaluation, and biopsy confirmed the diagnosis of a giant cell tumor of the distal fibula. The patient was treated with resection of the distal fibula followed by reconstruction using an ipsilateral proximal fibula graft. A giant cell tumour (GCT) is a benign and locally aggressive tumour that is usually observable in a skeletally mature patient involving the end of long bones. The reported incidence of this tumour in a skeletally immature patient is extremely rare. However, we report one such case in the distal radius of a seven-year-old female patient. Having presented with painful swelling of the right distal forearm, she underwent clinical and radiological examination, and a diagnosis of distal radius GCT was made. The tumour was treated with curettage, fibular graft, and synthetic bone graft. This case report shows the importance of including GCT in children as a differential diagnosis. Giant-cell tumor (GCT) is a type of bone tumor that is usually benign. In this study, we investigate the clinicopathological features of GCT and identify unreported characteristics of such disease in Iranian patients. In this study, 50 GCT patients had been treated at a referral orthopedic hospital from March 2003 through March 2013. The patients' sex, age, location of tumor, presenting symptoms, treatment methods, and recurrence rate were determined. To confirm the reliability of the initial diagnosis, hematoxylin and eosin (H-E) staining and P63 immunostaining were used. In this sample, 52 % of patients were male and 48 % were female. Among these patients, the most common symptom was pain. The most frequent surgical technique used for this group was the curettage, graft, and en bloc method. The lowest rate of tumor recurrence was observed in this technique. With the p63 immunostaining method, 27 cases were weakly positive and 14 cases were positive, while 9 cases were highly positive with episodes of recurrence. Of all body parts, GCT most commonly affects the knee. In our patients, the recurrence rate was 16 %, and all episodes of recurrence were observed in highly positive p63 patients. Background. Total shoulder arthroplasty is a standard operative treatment for a variety of disorders of the glenohumeral joint. Successful outcome of total shoulder arthroplasty depends on a well-designed and a well-executed physical therapy program. The rehabilitation program needs to respect a logical sequence: tissue healing, joint mobility and muscle strength. Aims. The purpose of this study was to emphasize the best therapeutic options to functionally rehabilitate a shoulder that was surgically replaced after a giant-cell tumor excision. Methods. The case report describes a 24-year-old female patient who presented in September 2013 to the Clinical Rehabilitation Hospital in Cluj-Napoca for rehabilitation after undergoing total shoulder arthroplasty for a giant-cell bone tumor located in the proximal region of the right humerus. Although they are considered benign in 90% of the cases, giant-cell tumors show a tendency for significant bone destruction, local recurrence, and occasional metastasis in the lung or lymph nodes. The objective examination of the patient at her first hospital admission: normal weight (BMI=19 kg/m2), two postoperative scars in the anterior region of the right arm with minimal sensitivity when palpated, hypotonia and hypotrophy of the right upper limb, spontaneous pain at the mobilization of the right shoulder and limited active and passive range of motion (especially for flexion, abduction and external rotation), pain on the VAS = 30 mm, Constant Shoulder Score = 42, DASH score = 35, SPADI score = 53%. In our hospital, she underwent the current Neer protocol for postoperative total shoulder arthroplasty rehabilitation, which is widely used and is based on the basic science of soft tissue and bone healing. Results. The functional outcome was good, as the range of motion and strength of the upper right limb improved after attending our physical therapy program. Conclusions. When a well-performed surgical procedure is supplemented with a well-designed and frequently monitored therapy program, an excellent outcome of shoulder replacement should be expected. Giant-cell tumor (GCT) is often more aggressive when located in the distal radius, and wide resection is then the gold-standard. No single reconstruction protocol is recommended, and the technique depends upon the surgeon's preferences. The aim of the present review was to determine the recurrence rate of GTC of the distal radius after intralesional treatment, to assess the results, advantages and complications of the various surgical techniques, and to draw up a decision-tree for surgical indications. The review of literature was performed in the main healthcare databases, searching for studies that reported results of wide resection and reconstruction of distal radius GCT. Local recurrence rates, metastasis rates, reconstruction techniques and respective results and complications were evaluated and analyzed. Sixteen studies were selected, for a total population of 226 patients; 6.0% and 0.9% experienced local recurrence and lung metastasis, respectively. Arthroplasty with non-vascularized or vascularized ipsilateral fibula were the most common techniques and were associated with the highest satisfaction rates: 86.4% and 88.0%, respectively. Arthroplasty with allograft presented a MusculoSkeletal Tumor Society (MSTS) score of 79.2% and arthroplasty with custom-made prosthesis presented an MSTS score of 81.8%. Arthrodesis was performed in 46 cases, with an MSTS score of 82.7%. Arthroplasty techniques are the most common in literature; they are used in patients who wish to conserve joint motion. Reconstruction with non-vascularized fibula seems to provide the best results, with lower morbidity. Giant cell tumour of the tendon sheath (GCTTS) is a rare benign soft tissue tumour with no known cause. It is more prevalent in the hand than in the ankle and foot. It appears as a non-painful, perceptible enlargement. Although pre-operative imaging and fine-needle aspiration cytology (FNAC) corroborate suspicion, histology following surgical resection is used to confirm the diagnosis. Due to its rarity, a case of giant cell tumour (GCT) of the extensor tendon sheath of the left ring finger is reported here. A 39-year-old female presented with a six-month history of painless swelling over left ring finger. The swelling was spontaneous, slowly progressive and painless. On clinical examination, a 1.5 cm x 1 cm firm swelling was seen on the dorsal surface of the left ring finger extending from the distal portion of the middle phalanx to the proximal half of the distal phalanx. The swelling was well-defined, smooth, firm, and uniform in consistency. The swelling was movable sideways with no attachment to the bone when examined clinically. X-ray of the hand showed soft tissue mass without the involvement of the bone. Soft tissue mass was seen on ultrasonography. An excisional biopsy was done. Histopathology showed typical features of GCTTS. Our case is a rare example of GCTTS in a single digit of the hand. Furthermore, considering its high recurrence risk, the tumour should be totally excised. Abstract: A 19-year-old woman was admitted to the hospital with a complaint of right wrist pain. A plain radiograph showed an osteolytic lesion in the right distal radius. Surgical treatment with curettage and bone grafting was performed under the clinical diagnosis of a giant-cell tumor. However, local recurrence was noticed 6 months after the treatment, and the curettage and bone graft were performed again. The histology of the recurrent lesion revealed a malignant giant-cell tumor that was entirely composed of highly atypical stromal cells and tumor giant cells. After a retrospective review of the primary lesion, it was found to also contain a small focus of pleomorphic cells within the conventional giant-cell tumor. Immunohistochemical and genetic analysis revealed p53 overexpression in the tumor giant cells and loss of heterozygosity of p53 gene only in the recurrent tumor; thus, a diagnosis of secondary malignant giant-cell tumor was finally made. The patient underwent systemic chemotherapy along with curettage and cryosurgery using liquid nitrogen. It is now 1 year and 4 months after the last treatment, and the patient is well and alive with no evidence of the disease. Giant-cell tumors of the distal ulna are frequently recurrent and difficult to treat. A Darrach-type resection can lead to functional impairments of the wrist and hand. A case of a 42-year-old man, who underwent a distal ulna arthroplasty for recurrent giant-cell tumor of the distal ulna, is presented. The patient had two previous operations in an attempt to curette the cavity, either alone or filled with allograft bone chips. En bloc resection and distal ulnar implant arthroplasty provided a good functional outcome without any evidence of tumor recurrence at a 2-year follow-up evaluation. [Copyright &y& Elsevier]

Purpose: Multiple options have been reported for reconstruction of Campanacci grade 3 giant-cell tumor of the distal radius after resection. In 1990, the senior author described an allograft reconstruction combined with a Sauve-Kapandji procedure after distal radius resection and reported preliminary results in 3 patients. The purpose of this study was to assess with validated patient outcome surveys the intermediate to long-term outcomes of all patients treated with this surgery and to analyze their functional results and document tumor control. Methods: All cases of distal radius osteoarticular allograft combined with the Sauve-Kapandji reconstruction for Campanacci grade 3 giant-cell tumors performed from 1986 to 2000 by a single surgeon were evaluated by clinical and radiologic examinations; the Disabilities of the Arm, Shoulder, and Hand questionnaire; the Short Form–36; and Mayo Wrist scores. Results: Nine consecutive patients (7 women, 2 men) with an average age at follow-up evaluation of 42 years and with a Campanacci grade 3 giant-cell tumor of the distal radius formed the study population. Clinical follow-up time after reconstruction averaged 7 years. Examination showed an average of 51° of extension and 19° of flexion of the wrist and 63° of supination and 79° of pronation of the forearm. Grip strength measured in 5 patients averaged 23 kg. The Disabilities of the Arm, Shoulder, and Hand questionnaire; the Short Form–36; and the Mayo Wrist scores averaged 15, 72, and 73, respectively. These scores indicate modest functional impact. No patient had tumor recurrence, metastases, nonunion, or proximal ulna instability. Conclusions: En bloc resection of a Campanacci grade 3 giant-cell tumor of the distal radius followed by reconstruction with an osteoarticular allograft and a Sauve-Kapandji procedure with autogenous bone graft results in a reasonable functional outcome at intermediate to long-term follow-up evaluation. Giant-cell tumour of bone (GCTOB) is a benign, locally aggressive, primary bone tumour. Involvement of the distal radius accounts for between 10 and 12% of cases of GCTOB, with the bones of the hand and wrist being rarely affected. GCTOB most commonly affects skeletally mature patients between the ages of 20 and 40 years, with the peak incidence being in the third decade. Women are affected slightly more commonly than men. GCTOB involving the bones of the hand most commonly occurs in a central location, which differs from the usual eccentric location seen in GCTOB at other sites. The radiographic features of GCTOB in the hand and wrist are presented. The role of bone scintigraphy, computed tomography and magnetic resonance imaging is discussed. Evaluation of the postoperative patient is also addressed, including the role of dynamic contrast-enhanced magnetic resonance imaging. Summary: Giant-cell tumor of bone (GCTB) and giant-cell tumor of soft tissue (GCTST) are tumors that contain a prominent osteoclastlike giant-cell component. The precise relationship between these morphologically similar tumors is unclear, and the cellular mechanism whereby giant cells accumulate within these and other locally aggressive tumors is uncertain. In this study, we have examined the cytochemical, functional, and molecular phenotype of the mononuclear and multinucleated components of GCTB and GCTST. Giant cells in GCTB and GCTST exhibited an osteoclast phenotype expressing tartrate-resistant acid phosphatase and vitronectin receptor and being capable of lacunar resorption. The mononuclear stromal cells derived from GCTB and GCTST exhibited an osteoblast phenotype, expressing alkaline phosphatase, and the receptor activator for nuclear factor κB ligand (RANKL), a factor that is essential for osteoclast formation. These cells also expressed osteoprotegerin (OPG), an inhibitor of osteoclastogenesis, and TRAIL, a receptor that binds OPG. Lacunar resorption by giant cells isolated from GCTB and GCTST was inhibited by OPG, zoledronate, and calcitonin. These findings indicate that the mononuclear and giant-cell components of GCTB and GCTST have similar phenotypic features and that the accumulation of osteoclasts in these giant-cell–rich tumors occurs by a RANKL-dependent process. RANKL expression by osteoblastlike mononuclear stromal cells in these tumors stimulates osteoclast formation and resorption; this would account for the osteolysis associated with these giant-cell–rich tumors. Inhibitors of osteoclast formation and activity are likely to be effective in controlling the osteolysis associated with GCTB and possibly other giant-cell–rich lesions. [Copyright &y& Elsevier]

A case of giant-cell tumor of the third metacarpal bone is reported, treated with resection and reconstruction by iliac graft. At a 5 years follow-up, the cosmetic and functional results of the hand are good. [Copyright &y& Elsevier]

Two cases of the diffuse type of giant-cell tumor of the tendon sheath (GCTTS) are described. Both tumors arose in the vicinity of large joints of the lower extremity, showing similar clinical and radiological features. Histologically, a proliferation of polygonal mononuclear cells was seen, together with osteoclastlike giant cells, foam cells, and siderophages. The tumors were poorly delineated, displaying an infiltrative pattern into the neighboring soft tissues. Immunohistochemically, strong expression of vimentin, neuron-specific enolase, A[sub 1]-antitrypsin, and CD68 was found in both mono- and multinucleated tumor cells. At the ultrastructural level, mononuclear cells revealed a diverse morphology, displaying features of histiocytelike and fibroblastlike cells, with the former being more numerous. Scarce neurosecretorylike granules, made up of electrondense membrane-bound material, were found in the cytoplasm of the mononuclear cells. Cytogenetic analysis of one case shows the presence of a clonal population with 47 chromosomes and two different translocations, t(2;3) and der(8) t(8;12). A correlated histologic and radiographic study of nine giant-cell tumors, six aneurysmal bone cysts, and one combined lesion is presented. Clinical findings and plain radiographic appearances were found to overlap. Angiographically, the giant-cell tumors were richly vascularized, with a marked intratumoral contrast uptake, occasional irregular tumor vessles, a prominent peritumoral arterial net-work, and early draining veins. Microscopic examination revealed fine, capillary-like and somewhat larger, angulated, sinusoid vessels, and occasional small, bloodfilled cysts within the tumor. Conspicuous arteries, veins, and capillaries surrounded the tumor. The aneurysmal bone cysts at angiography were predominantly avascular in the interior, surrounded by a thin hyperemic zone in the peritumoral tissue, with sometimes early filling of veins. Microscopically, the aneurysmal bone cysts showed large, anastomosing, bloodfilled vascular channels, and a moderate number of capillaries within the solid, fibrous areas and the granulation tissue. It is concluded that angiography, particularly with a subtraction technique, may be a valuable diagnostic method in the pre-operative differentiation of giant-cell tumors from aneurysmal bone cysts. Saturable, specific, high-affinity calcitonin receptors were demonstrated in cultured neoplastic mononuclear spindle cells from a giant-cell tumor of the sacrum of a 38-year-old woman. The receptor was analyzed by autoradiography andI-calcitonin binding assay. Binding reversibility ofI-calcitonin to the cells was not complete and the structural specificity was indicated by the inability of unrelated hormones to compete with calcitonin. The 24 000 receptors/cell and dissociation constant ( K) of 8.0×10 M, calculated from linear Scatchard plots, suggested the existence of a single class of calcitonin binding sites in the neoplastic mononuclear cells. Flow-cytometric analysis in the primary culture showed that mononuclear cells consisted of mononuclear round cells of monocyte/macrophage lineage, which express more calcitonin receptors than neoplastic mononuclear spindle cells. Administration of calcitonin caused morphological and physiological alterations, resulting in involutional or irregular cytoplasmic shapes and inhibition of DNA synthesis in neoplastic mononuclear cells accompanied by the escape phenomenon. Cells preincubated with caleitonin showed a decrease inI-calcitonin binding activity, which could account for the escape phenomenon. The decrease inI-calcitonin binding was rapid, but the recovery was not observed for 24 h after elimination of calcitonin. This decrease may be caused by the disappearance of residual receptors or by a decrease in calcitonin affinity. The calcitonin-induced morphological changes and the inhibition of DNA synthesis of cells were revealed to be mediated by caleitonin receptors. [ABSTRACT FROM AUTHOR]

The dedifferentiated giant-cell tumor of the bone is a very rare variant of the giant-cell tumor (GCT). We report the clinical, radiographic and histological findings of a dedifferentiated GCT in which the dedifferentiated component consisted of small round cells. We also comment on previously reported cases of dedifferentiated GCT, discuss the clinical implications of this dual histology, and analyze the information published about the coexistence of similar genetic abnormalities in GCT and small round cell tumors of the bone. Abstract: Primary hyperparathyroidism is common, particularly in postmenopausal women. Since the introduction of routine automated serum calcium assays and the development of assays for intact 1–84 parathyroid hormone, the diagnosis is usually made fortuitously in asymptomatic patients or during evaluation for osteoporosis. As a result, many physicians have no experience with the clinical manifestations. Here, we describe the case of a 70-year-old man with bone pain, multiple brown tumors, and severe hypercalcemia (4mmol/L). The diagnostic pitfalls raised by these symptoms are illustrated.
