Objective: A single-nucleotide polymorphism of the growth hormone 1 gene, GH1IVS4+90A>T (rs2665802), associated with short stature and a polymorphism of the growth hormone receptor gene, exon 3 deleted variant, associated with increased responsiveness to growth hormone have been reported previously. We aimed to investigate the frequency of both polymorphisms and their correlation to height in Turkish short children. Also, we aimed to evaluate the effect of exon 3 deleted variant on response to 1-year growth hormone therapy. Materials and Methods: Children with idiopathic isolated growth hormone deficiency (n = 39) and with idiopathic short stature (n = 10) and 50 control subjects were evaluated for anthropo-metric parameters, annual growth velocity, and annual height gain. Growth hormone receptor gene polymorphisms were analyzed via multiplex polymerase chain reaction; growth hormone 1 gene polymorphism was analyzed via polymerase chain reaction and single-strand confor-mation polymorphism techniques. Results: The frequency of genotypes carrying the “A” allele was not significantly higher in the idiopathic isolated growth hormone deficiency group than in the idiopathic short stature and control groups (P = .03 for each). The exon 3 deleted variant genotype was significantly lower in the idiopathic short stature group compared to the control group (P = .01). There was no effect of exon 3 deleted variant, on response to the first-year growth hormone therapy. Conclusion: In Turkish population, no correlation was found between the “A” allele of GH1IVS4+90A>T polymorphism and idiopathic isolated growth hormone deficiency and short stature, and a significant negative correlation was found between exon 3 deleted variant and idiopathic short stature and short stature. Meta-analysis has shown a modest improvement in first-year growth response to recombinant human GH (r-hGH) for carriers of the exon 3-deleted GH receptor (GHRd3) polymorphism but with significant interstudy variability. The associations between GHRd3 and growth response to r-hGH over 3 years in relation to severity of GH deficiency (GHD) were investigated in patients from 14 countries. Treatment-naïve pre-pubertal children with GHD were enrolled from the PREDICT studies (NCT00256126 and NCT00699855), categorized by peak GH level (peak GH) during provocation test: ≤4 μg/l (severe GHD; n=45) and >4 to n=49) and genotyped for the GHRd3 polymorphism (full length (fl/fl, fl/d3, d3/d3). Gene expression (GE) profiles were characterized at baseline. Changes in growth (height (cm) and SDS) over 3 years were measured. Der in der Bevölkerung mit einer Allelfrequenz von 25-32% vorkommende Wachstumshormonrezeptor-Deletion-Exon 3-linebreak Polymorphismus (GHRd3) verfügt im Vergleich zur normalen Rezeptorform mit Expression von Exon 3 (GHRfl) über eine erhöhte Bioaktivität. Die Tatsache, dass Wachstumshormon sowie Wachstumshormonrezeptor lokal im Brustdrüsengewebe gebildet werden und ihre Expression bei malignen Erkrankungen der Brustdrüse gesteigert ist, führt zu der Vermutung, dass das endogene Wachstumshormon einen ähnlichen wachstumsfördernden Effekt über die GHRd3-Isoform auf das Brustdrüsengewebe ausüben und somit zur Entstehung von Brustkrebs beitragen könnte. Ziel war es, eine mögliche Assoziation des GHRd3-Polymorphismus mit dem Auftreten von Brustkrebs zu untersuchen und dabei eine weitere genetische Einflussgröße für die Erkrankung zu detektieren. Es wurden 125 Probandinnen mit positiver Brustkrebsanamnese sowie 125 Probandinnen ohne bisherige maligne Erkrankungen und ohne Brustkrebs in der Familienanamnese als Kontrollkollektiv rekrutiert. Zur Bestimmung des GHRd3-Polymorphismus wurde eine LightCyclertexttrademark-PCR mit Schmelzkurvenanalyse durchgeführt. Klinische Daten wie Alter, Erkrankungsalter, Raucheranamnese etc. wurden mittels Fragebogen erhoben. Die Messung der IGF-1-linebreak Serumkonzentration wurde mit AMP-IGF-I-ELISA nach standardisiertem Protokoll durchgeführt. Die statistische Auswertung erfolgte mithilfe einer Software des Helmholtz Zentrum München, einer Software De Finetti Generator der MEB Universität Bonn sowie mit SPSS. Die GHRd3-Allelfrequenz bei Probandinnen mit Brustkrebs lag bei 30,4% und im Kontrollkollektiv bei 28%. 50,4% der Probandinnen mit positiver Brustkrebsanamnese waren homozygot für den Genotyp fl/fl, 38,4% heterozygot für fl/d3 und 11,2% homozygot für d3/d3. In der Kontrollgruppe zeigten 52% den Genotyp fl/fl, 40% den Genotyp fl/d3 und 8% den Genotyp d3/d3. Es wurde eine Odds Ratio für Brustkrebs von Trägern mindestens eines GHRd3-Allels im Vergleich zu Trägern ohne GHRd3-Allel von 1,066 (95%-KI 0,649-1,75; p=0,80) errechnet. Probandinnen mit Brustkrebs und Kontrollen unterschieden sich im Hinblick auf die mittlere IGF-1-Serumkonzentration (p=0,26), dem BMI (P=0,4) und der Einnahme von Hormonersatztherapie (p=0,41) nicht signifikant voneinander.
