2. Sustaining a Rural School-University Partnership: A Twenty-Two Year Retrospective of an After-School Tutoring Program

- Abstract
In this paper, we describe a rural school-university 22 year partnership. The partnership serves students with disabilities and students at-risk for being classified with a disability. Despite personnel changes, the undergraduate teacher candidates have consistently provided intense instruction to their assigned students. Electronic surveys were sent to school and university professionals affiliated with the undergraduate course "Introduction to the Exceptional Learner" to determine why the partnership has lasted so long. Follow-up focus group sessions were held, and data from the sessions were collected and analyzed for a deeper review of identified themes. The data analysis and theme development supported the following reasons for the longstanding partnership: (a) academic gains, (b) interpersonal benefits, and (c) specific program components.

3. Scaling up in Rural Schools Using Positive Behavioral Interventions and Supports

- Abstract
Many educators struggle with the challenges of effective behavior management. In rural schools, this frequently means that educators are struggling to do more, with little or no additional resources. Positive behavioral interventions and supports (PBIS) is one approach to proactive behavior management. In order to facilitate PBIS in a regional area, researchers implemented a week-long training and year-round ongoing coaching for representative members of participating schools/districts to create a plan for implementation suited to the needs of their schools/districts. This article provides a brief overview of school-wide positive behavioral interventions and supports with a particular focus on one rural school's successful implementation.

Blister-like aneurysms (BLA) are a rare half-dome-shaped aneurysm, with a broad-based appearance, originating more often from a non-branching site of the supraclinoid internal carotid artery (ICA). They are sometimes difficult to recognize due to their morphological changes and high tendency to rupture. BLAs are often undiagnosed and may be detected only after repeated angiograms in the case of acute subarachnoid hemorrhage (SAH). BLA are life-threatening and no consensus has so far been reached on the best management strategy. We describe a patient with a BLA in the left ICA successfully treated by flow-diverting device using intraoperative abciximab bolus, with successful results. Recently, the use of flow-diverting device (FDD), has offered a very promising option for treatment even for ruptured BLA, with high long-term occlusion and low complication rates. However, double antiplatelet therapy is required and still represents a major constraint in a case of SAH, so that standardized multicenter studies are still needed. [ABSTRACT FROM AUTHOR]

I fibromi uterini rappresentano il più comune tumore benigno del tratto genitale femminile in età riproduttiva. Sebbene siano spesso del tutto asintomatici, in circa la metà dei casi possono essere responsabili di sintomi debilitanti quali menometrorragia, dismenorrea, anemia, dolore, pressione pelvica, disturbi urinari, costipazione, dispareunia ed infertilità. Diversi approcci sono oggi disponibili per il trattamento dei fibromi uterini, sia farmacologici che chirurgici; tra questi l’MRgFUS rappresenta, in casi selezionati, una tecnica sicura, efficace e non invasiva in grado di preservare la fertilità della donna. Tale tecnica combina la tecnologia della risonanza magnetica (RM), in grado di fornire un imaging multiplanare e delle mappe termiche per il monitoraggio in tempo reale della temperatura raggiunta nella sede del trattamento, a quella degli ultrasuoni focalizzati ad alta intensità (HIFU), capaci di indurre una termoablazione tissutale del target prefissato. Presentiamo la nostra esperienza nel trattamento dei fibromi uterini sintomatici mediante MRgFUS.

Uterine fibroids, the most common benign tumor in women of childbearing age, may cause symptoms including pelvic pain, menorrhagia, dysmenorrhea, pressure, urinary symptoms, and infertility. Various approaches are available to treat symptomatic uterine fibroids. Magnetic Resonance-guided Focused Ultrasound Surgery (MRgFUS) represents a recently introduced noninvasive safe and effective technique that can be performed without general anesthesia, in an outpatient setting. We review the principles of MRgFUS, describing patient selection criteria for the treatments performed at our center and we present a series of five selected patients with symptomatic uterine fibroids treated with this not yet widely known technique, showing its efficacy in symptom improvement and fibroid volume reduction.

14. MALATTIA DI ERDHEIM–CHESTER: RARE LOCALIZZAZIONI SPLANCNO- ED INTRA-CRANICHE IN PAZIENTE CON STADIO AVANZATO DI MALATTIA

SCOPO DEL LAVORO Presentiamo il caso di una donna di 63 anni con storia di pseudotumor orbitae e diagnosi tardiva di Malattia di Erdheim-Chester (MEC) con coinvolgimento multisistemico e rare localizzazioni a livello splancno- ed intracranico. MATERIALI E METODI La paziente, a causa della comparsa di astenia progressivamente ingravescente, vomito e disfagia per i solidi veniva condotta al PS dove eseguiva ECG ed esami ematochimici e veniva ricoverata con la diagnosi di disidratazione e squilibrio elettrolitico. L’esame obiettivo evidenziava pallore e disidratazione di cute e mucose, ipotrofia muscolare e proptosi bulbare bilaterale da pseudotumor orbitario infiammatorio cronico con evidenti secrezioni purulente. La paziente veniva quindi sottoposta a Esofago-Gastro-Duodenoscopia (che non ha mostrato significativi reperti) e TC total body (Siemens SOMATOM Definition AS+ - 128 strati). Nel sospetto di una patologia sistemica a carattere fibrosclerotico-istiocitario, veniva richiesta una rivalutazione immunoistochimica su un prelievo bioptico eseguito, oltre un anno addietro, nel seno mascellare destro, che aveva evidenziato tessuto fibrovascolare con infiltrato flogistico linfocitario e raccolte di istiociti schiumosi. Venivano inoltre eseguite RM encefalo con apparecchiatura da 1,5 Tesla (GE Signa HDxt) prima e dopo mdc e.v. e PET total body con ¹⁸F-FDG. Durante la degenza è stata somministrata antibioticoterapia sistemica e topica oculare con aminoglicoside, steroidi a basse dosi ed analgesici, con progressivo miglioramento del quadro infiammatorio locale orbitario e degli indici sistemici di flogosi. RISULTATI La TC total body ha evidenziato segni di diffuso interessamento del distretto cranio-encefalico, di rimaneggiamento osseo diffuso e di coinvolgimento polmonare e retroperitoneale. La RM encefalo mostrava la presenza di tessuto di intensità intermedia in T1 e bassa in T2, dotato di intensa impregnazione post-contrastografica, a localizzazione transcompartimentale splancno- ed intra-cranica (meningi, ependima, plessi corioidei, seno cavernoso, spazi perivasali, orbite e seni paranasali): in particolare a livello orbitario erano interessati bulbi oculari, nervi ottici, muscolatura estrinseca e ghiandole lacrimali; di rilievo inoltre il coinvolgimento, a livello intracranico, dei plessi corioidei in corrispondenza del corno temporale del ventricolo laterale di sinistra e dei recessi del IV ventricolo e, a livello dello splancnocranio, del canale carotideo, del foro lacero posteriore e l’infiltrazione perivasale intorno alla carotide interna nei suoi tratti intrapetroso ed intracavernoso. La PET meglio definiva l’interessamento multisistemico. La valutazione immunoistochimica degli elementi istiocitari risultava positiva al CD68 e negativa al CD1a/S100. CONCLUSIONI I dati dell’analisi immunoistochimica hanno permesso di confermare la diagnosi di MEC, una rara istiocitosi a cellule non-Langerhans, caratterizzata da infiltrazione tissutale di istiociti schiumosi e coinvolgimento multisistemico. La diagnosi di ECD è spesso insidiosa e tardiva, nonostante la presenza di alcuni tipici segni clinici e radiologici, non sempre riconosciuti e ricondotti a questa patologia. In particolare l’interessamento del SNC si riscontra in circa il 50% dei pazienti affetti. Nel nostro caso la paziente è giunta alla nostra osservazione in uno stadio ormai avanzato di malattia con localizzazioni infrequenti a livello intracranico: in particolare estremamente rari risultano le caratteristiche del coinvolgimento orbitario, l’infiltrazione perivasale e la localizzazione nei plessi corioidei.

Presentiamo i risultati preliminari ottenuti con la prima istallazione italiana di un’apparecchiatura per la neurochirurgia funzionale mediante ultrasuoni focalizzati ad alta intensità su guida RM (trans-cranial MRI-guided Focused Ultrasound Surgery, tcMRgFUS); si tratta tra l’altro della prima apparecchiatura tcMRgFUS al mondo mai installata su uno scanner RM da 1,5T. Tale apparecchiatura è destinata a trattamenti di neurochirurgia funzionale di pazienti con tremore essenziale (Essential Tremor, ET), Parkinson idiopatico (Parkinson Disease, PD) prevalentemente tremorigeno e dolore neuropatico (Neuropa- thic Pain, NP). Sanno presentati i risultati ottenuti con i primi pazienti sottoposti ad intervento di neurochirurgia funzionale mediante tcMRg- FUS analizzando le difficoltà tecniche affrontate per l’ottimizzazione sia delle sequenze di studio intraoperatorie utilizzate come guida per un preciso targeting che di quelle utilizzate per la termometria RM. Sebbene si tratti di un’esperienza preliminare, il successo ottenuto con i primi trattamenti da noi effettuati, pone le basi affinché in futuro, gli innovativi trattamenti di neurochirurgia funzionale mediante tcMRgFUS (attualmente eseguiti in pochi centri al mondo utilizzando esclusivamente scanner RM da 3T), potranno essere eseguiti efficacemente anche con i più diffusi scanner RM operanti a 1,5T.

MRgFUS is a new noninvasive thermal ablation method, consisting of a high focused ultrasound beam and a MR scanner for imaging and real-time thermal mapping. For decades, therapeutic transcranial ultrasound (tcMRgFUS) was assumed impossible. Novel technology using high-power phased array transducers and multiple channel driving electronics enabled a sharp focal point in the planned target through the intact skull allowing the use of this innovative technology for functional neurosurgery. Preliminary results have been published with systems operating with 3T MR units on patients with neurologic disorders such as essential tremor, tremor dominant idiopathic Parkinson's disease and neuropathic pain. In this talk the preliminary results achieved with the world-first tcMRgFUS system operating with a 1,5T MR (first Italian tcMRgFUS installation ever) will be presented.

Scopo del Lavoro Presentiamo i risultati preliminari ottenuti con la prima istallazione italiana di un’apparecchiatura per la neurochirurgia funzionale mediante ultrasuoni focalizzati ad alta intensità su guida RM (trans-cranial MRI-guided Focused Ultrasound Surgery, tcMRgFUS); si tratta tra l’altro della prima apparecchiatura tcMRgFUS al mondo mai installata su uno scanner RM da 1,5T. Tale apparecchiatura è attualmente certificata (CE ed FDA) per trattamenti di neurochirurgia funzionale in pazienti con tremore essenziale (Essential Tremor, ET), Parkinson idiopatico (Parkinson Disease, PD) unilaterale prevalentemente tremorigeno e dolore neuropatico (Neuropathic Pain, NP). Materiali e Metodi Sono stati arruolati pazienti con disordini neurologici resistenti a terapia medica ed indicazione a trattamento di neurochirurgia funzionale. Prima del trattamento è stata raccolta una dettagliata anamnesi e ciascun paziente è stato sottoposto ad un esame clinico-neurologico e neurofisiologico completo. I pazienti eleggibili sono stati sottoposti ad esame TC ed RM di screening per escludere controindicazioni al trattamento. I trattamenti tcMRgFUS sono stati eseguiti da un gruppo multidisciplinare guidato da un neurochirurgo di comprovata esperienza in neurochirurgia funzionale e da un neuroradiologo, ottimizzando i parametri di trattamento caso per caso. Risultati Sebbene si tratti di un’esperienza preliminare, il successo ottenuto con i primi trattamenti da noi effettuati, pone le basi affinché in futuro, gli innovativi trattamenti di neurochirurgia funzionale mediante tcMRgFUS (attualmente eseguiti in pochi centri al mondo utilizzando esclusivamente scanner RM da 3T), potranno essere eseguiti efficacemente anche con i più diffusi scanner RM operanti a 1,5T. Nei trattamenti fino ad oggi eseguiti non sono state registrate complicanze intra- o post-trattamento. I pazienti trattati hanno pressoché immediatamente ottenuto un beneficio clinico e sono stati dimessi dopo 48h dal trattamento. Conclusioni Poter eseguire interventi di neurochirurgia funzionale con una tecnica non invasiva, che non fa uso di radiazioni ionizzanti, che consente di verificare l’efficacia clinica del trattamento prima ancora di indurre una lesione cerebrale permanente e tutto questo utilizzando i più comuni scanner RM operanti a 1,5T rappresenta un enorme passo avanti sia per la radiologia interventistica che per la neurochirurgia funzionale.

Background: Traditional surgery for uterine fibroids treatments (e.g. myomectomy, hysterectomy) offers very invasive therapeutic approaches, which not always preserve reproductive potential of the woman. MRgFUS (MR guided Focused UltraSound) is a new and non-invasive technique for uterine fibroids treatment, not requiring hospitalization and recovery time [1]. On June 2011 and July 2012 the first treatments were started at HSR-Giglio Hospital in Cefalù and at University Hospital (DIBIMEF) in Palermo. An initial assessment of MRgFUS treatment was made by computing the thermally-ablated volume of uterine fibroid. This volume was evaluated considering the NPV (Non Perfused Volume) on a post-treatment MR dataset acquired with contrast medium. Nowadays, the used approach is a time-expensive and operator-dependent manual segmentation procedure. In this paper, the preliminary results of the two Sicilian facilities are showed and a semi- automatic segmentation approach is proposed, based on multi-seed region-growing technique, to calculate the NPV. The realized approach gives a quantitative evaluation in the post-treatment phase. Materials and Methods: The MRgFUS procedures, regarding women affected by single/multiple uterine fibroid, were performed using the ExAblate 2000 and 2100 (InSightec, Haifa, Israel), which is fully integrated with a 1.5 Tesla MR Scanner (GE Medical System, Milwaukee, WI). For NPV manual and semi-automatic evaluation we have used the sagittal or coronal images acquired after the treatment with a gadolinium-based contrast medium (FSPGR+FS+C protocol). The proposed semi-automatic approach in based on multi-seed region-growing, where it is possible to individualize the processing steps: • pre-processing filtering; • region-growing segmentation; • post-processing filtering; • NPV computation. Results: Among all 23 performed treatments, 69.57% have obtained a NPV, manually evaluated, bigger than 60%, successful treatment threshold, and the 65.22% > 70% and 47.83% > 80%. The evaluation of our segmentation approach was performed by calculating Jaccard and Dice similarity indexes and specificity and sensitivity values. In order to obtain the above indexes, the results of the proposed region-growing approach were compared with the manual segmentation. Preliminary segmentation tests obtained the following results: • Jaccard Index: 87.83%; • Dice Index: 94.12%; • Sensitivity: 91.59%; • Specificity: 89.72%. Conclusion: The MRgFUS provides an important new non-invasive and effective treatment for uterine fibroids. The proposed multi-seed region-growing segmentation approach allows to estimate the NPV in a semi-automatic and operator-independent way. Obtained segmentation indexes show the effectiveness of the implemented technique.

Parkinson’s disease (PD) is a progressive neurodegenerative disorder, characterized by motor and non-motor symptoms due to the degeneration of the pars compacta of the substantia nigra (SNc) with dopaminergic denervation of the striatum. Although the diagnosis of PD is principally based on a clinical assessment, great efforts have been expended over the past two decades to evaluate reliable biomarkers for PD. Among these biomarkers, magnetic resonance imaging (MRI)-based biomarkers may play a key role. Conventional MRI sequences are considered by many in the field to have low sensitivity, while advanced pulse sequences and ultra-high-field MRI techniques have brought many advantages, particularly regarding the study of brainstem and subcortical structures. Nowadays, nigrosome imaging, neuromelanine-sensitive sequences, iron-sensitive sequences, and advanced diffusion weighted imaging techniques afford new insights to the non-invasive study of the SNc. The use of these imaging methods, alone or in combination, may also help to discriminate PD patients from control patients, in addition to discriminating atypical parkinsonian syndromes (PS). A total of 92 articles were identified from an extensive review of the literature on PubMed in order to ascertain the-state-of-the-art of MRI techniques, as applied to the study of SNc in PD patients, as well as their potential future applications as imaging biomarkers of disease. Whilst none of these MRI-imaging biomarkers could be successfully validated for routine clinical practice, in achieving high levels of accuracy and reproducibility in the diagnosis of PD, a multimodal MRI-PD protocol may assist neuroradiologists and clinicians in the early and differential diagnosis of a wide spectrum of neurodegenerative disorders.

Pituitary carcinomas (PCa) are extremely rare, indistinguishable from pituitary adenomas on histopathological grounds and have a poor prognosis. Most PCa start as PRL or ACTH secreting tumors in males, with relapsing invasive behaviour, refractoriness to medical and radiotherapy and increasing hormonal levels. The presence of distant metastases is still required for the diagnosis of PCa. The association with genetic endocrine diseases must be taken into account, since it adds further risk of evolution towards malignancy. Intradural spinal metastases have also been reported, so a complete craniospinal MR evaluation is recommended, when clinically indicated. We report a case of PCa, associated with MEN1 syndrome, with evidence of meningeal spread to the tentorium cerebelli, clival dura and spinal drop metastases mimicking spinal nerves schwannomas.

- Abstract
In past years non-invasive clinical applications of magnetic resonance-guided focused ultrasound for the treatment of neurological disorders have been hampered by technical limitations that today have been finally overcome. In 2015, for the first time in the world, the very first treatments have been performed in Italy by the use of an affordable 1.5T magnetic resonance unit. The clinical results obtained to date and all the future possible applications are very promising and pave the way towards safe and effective treatment options once unthinkable.
