Objective: To evaluate the accuracy of dental rotational movements using clear aligners with different attachment configurations. Materials and Methods: This retrospective study analysed 212 teeth from 89 patients undergoing Invisalign treatment. Digital models were analysed after the virtual treatment plan (ST1) and after the first treatment phase (ET1) to evaluate the effective clinical rotational movement. The rotational movements of incisors, canines, and bicuspids were measured using data from the Clincheck Movements Table. ST1 and ET1 were compared to determine the actual rotational movement achieved (ST1‐ET1). The presence or absence of attachments (rectangular or optimized) on teeth was analysed. The accuracy of rotational movements among attachment types was compared using the Kruskal‐Wallis test. Multiple linear regressions were conducted with accuracy as the dependent variable and tooth type, gender, and age as predictors. Results: Optimized attachments had the highest median accuracy (70%), followed by rectangular (65%), and without attachment (63%), with no significant differences (p =.5). There were no significant differences across age groups, genders, or tooth types. Baseline accuracy was 68.62% (95% CI: 56.03–81.20, p <.001). Age was a significant predictor (estimate = −0.30, 95% CI: −0.58 – −0.03, p =.032), indicating decreased accuracy with increasing age. The model's R2 was 0.046, with an adjusted R2 of 0.003, indicating minimal variance explained. Conclusion: The addition of attachment configurations to clear aligners improves rotational accuracy, but not significantly. Objectives: The treatment of class II patients with mandibular retrusion often involves the use of a Herbst appliance, due to its efficiency and reduced need for compliance. Despite skeletal benefits, undesired dental effects, caused by anchorage loss, could reduce mandibular advancement, especially after the pubertal peak. Although the period between cervical vertebral maturation (CVM) 3 and CMV4 is considered the best choice to maximize the orthopedic outcome, clinicians cannot always treat patients during this ideal time window, as they present at their first visit later. The goal of this study is to evaluate whether the combination of an acrylic splint Herbst appliance with skeletal anchorage and elastic chains in both the upper and lower arch (Skeletal Therapy Manni Telescopic Herbst 4: STM4) could improve the efficiency of functional treatment also in CVM 5–6 patients. Methods: Lateral cephalograms of 10 consecutively treated patients taken at the beginning and at the end of the Herbst phase were analyzed and compared with those of 10 untreated patients of the same sex, age, malocclusion, and skeletal maturation. Results: According to the Pancherz analysis, the treated group of patients showed the significant advancement of the pogonion (4.75 mm), with a reduction in ANB (−3.3°) and Wits (−4.15 mm). The combination of miniscrews and elastic ligatures allowed the labial tipping of the upper incisors (+6.65°) and limited the labial flaring of the lower ones (+3.05°), maintaining the overjet needed for mandibular advancement. Conclusions: The STM4 technique might be an effective protocol for skeletal class II correction after the pubertal peak since it significantly reduces unfavorable dental compensations, increasing the skeletal effects and the esthetic outcome of the orthopedic treatment. To evaluate the quantitative effects of palatal Temporary Anchorage Device (TAD)-supported appliances for the distalisation of maxillary molars in Class II patients.

(1) Objectives: The purpose of this review was to compare the effects of combining skeletal anchorage and Class II devices, both from an overall perspective and individually for each type of appliance, considering as main outcomes the vertical dimensions and the inclination of the mandibular and maxillary incisors. (2) Materials and Methods: A search without time restrictions was performed up to February 2024 in PubMed, PubMed Central, Scopus, and Medline for randomized controlled trials, as well as prospective and retrospective cohort studies, considering Class II patients treated with and without skeletal anchorage. The effect measure used for the meta-analytic evaluation was the standardized mean difference (SMD). The SMD calculation was obtained by subtracting the mean values of T1–T0 for each individual treatment and then calculating the SMD between the treatments involved. The meta-analysis was performed using the standardized mean difference of the mean difference of the T1–T0 change in the outcome between the different treatments evaluated as the effect size. (3) Results: A total of 1217 documents were initially retrieved. According to the PRISMA protocol, 18 studies comparing different skeletal anchorage protocols (upper/lower miniscrews and miniplates), combined with four appliances (Herbst, Forsus, Carriere Motion, and elastics), were included in the analysis. No significant difference in skeletal divergence was found between groups from an overall point of view (SMD: 0.19 (−0.48 to 0.83) according to the random-effects model). A statistically significant reduction in IMPA° was found in patients treated with temporary anchorage devices (TADs) (SMD of 5.58 (3.40 to 7.75)), except for the elastics group (SMD: 3.76 (−0.91 to 8.43)). The effect on the upper incisors' inclination appeared to be strictly dependent on the type of anchorage (TADs in one or both of the arches). Some limitations must be considered when interpreting the results: the small number of studies included and the heterogeneity among them are among the limitations, and the temporal disparity among some studies; the ages of the patients were not always comparable; and, finally, the clinical relevance of the effects of TADs is sometimes questionable. (4) Conclusions: The vertical dimension seems not to be significantly affected by skeletal anchorage; instead, the proclination of mandibular incisors is generally reduced when TADs are used. The aim of this study was to compare midfacial soft tissue assessment and horizontal width changes of fresh socket implants in immediate and delayed prosthetic restorations. Patients requiring extractions in the maxillary esthetic zone were recruited. In one group, 30 implants were placed using an immediate loading approach; in the second group, 30 implants were loaded after a period of 3 months. At the 4-year follow-up, statistically significant differences (P < .001) were found. Immediate implants restored on the day of surgery demonstrated more stable midfacial soft tissue levels and bone volume maintenance than delayed implants. [ABSTRACT FROM AUTHOR]

Purpose: To evaluate whether 5.0 × 5.0 mm dental implants with a novel nanostructured calciumincorporated titanium surface could be an alternative to implants of at least 10.0 mm long placed in bone augmented with bone substitutes in posterior atrophic jaws. Materials and methods: Forty patients with atrophic posterior (premolar and molar areas) mandibles with 5.0 mm to 7.0 mm bone height above the mandibular canal, and 40 patients with atrophic maxillas with 4.0 mm to 6.0 mm below the maxillary sinus, were randomised according to a parallel group design to receive between one and three 5.0 mm implants or one to three at least 10.0 mmlong implants in augmented bone at two centres. All implants had a diameter of 5.0 mm. Mandibles were vertically augmented with interpositional bovine bone blocks covered with resorbable barriers. Implants were placed after 4 months. Maxillary sinuses were augmented with particulated porcine bone via a lateral window covered with resorbable barriers, and implants were placed simultaneously. All implants were submerged and loaded after 4 months with provisional prostheses. Four months later, definitive screw-retained or provisionally cement metal-ceramic or zirconia prostheses were delivered. Patients were followed to 3 years post-loading and the outcome measures were: prosthesis and implant failures, any complication, and peri-implant marginal bone level changes. Results: Seven patients dropped out before the 3-year evaluation (two short mandibles, one short maxilla, two augmented mandibles and two augmented maxillae). In mandibles, two grafted patients were not prosthetically rehabilitated because of multiple complications and two implants failed in the same patient (the second was a replacement implant) vs one patient who lost a short implant and crown 2 years after loading. In maxillas one short implant failed with its provisional crown 3 months post-loading. There were no statistically significant differences in prostheses (difference in proportion = 0.001; 95% CI: -0.12 to 0.13; P = 1.000) and implant failures (difference in proportion = 0.00; 95% CI: -0.13 to 0.13; P = 1.000) up to 3 years after loading. Significantly, more complications occurred at mandibular grafted sites: 17 augmented patients were affected by complications vs eight patients treated with short implants in mandibles (difference in proportion = 0.43; 95% CI: 0.13 to 0.64; P = 0.008). In the maxilla, six sinus-lifted patients vs two patients treated with short implants were affected by complications; the difference not being statistically significant (difference in proportion = 0.21; 95% CI: -0.05 to 0.45; P = 0.232). Patients with mandibular short implants lost on average 1.10 mm of peri-implant bone at 3 years and patients with 10.0 mm or longer mandibular implants lost 1.39 mm. Patients with maxillary short implants lost on average 1.04 mm of periimplant bone at 3 years and patients with 10 mm or longer maxillary implants lost 1.43 mm. Longer implants showed a greater bone loss up to 3 years after loading than short implants both in maxillae (mean difference: -0.39 mm; 95% CI: -0.70 to -0.07 mm; P = 0.017) and in mandibles (mean difference: -0.29 mm; 95% CI: -0.53 to -0.05 mm; P = 0.020). Conclusions: Three years after loading, 5.0 mm × 5.0 mm implants achieved similar results than longer implants placed in augmented bone. Short implants might be a preferable choice to bone augmentation especially in posterior mandibles since the treatment is faster, cheaper and associated with less morbidity. The aim of this study was to evaluate buccal-lingual bone remodeling after fresh socket implant placement and immediate loading at 3 years of follow-up by cone beam computed tomography (CBCT). A total of 96 implants were placed in fresh sockets in anterior maxillary regions and immediately loaded. The sockets were divided into two groups related to buccal bone thickness: group A, with a buccal bone thickness > 1 mm, and group B, with a thickness ≤ 1 mm. The CBCT scans were performed before tooth extractions and 3 years after implant placement, and measurements were assessed. At 3 years, all fresh sockets in both groups presented considerable buccal-palatal crestal reduction. In both groups, statistically significant (P < .05) bone loss was found between time points. There was no statistically significant difference in bone reduction between groups at 3 years of follow-up. Nevertheless, sockets with vertical axis presented more bone volume reduction than those with axis parallel to buccal bone after implant placement. [ABSTRACT FROM AUTHOR]

Purpose: To evaluate clinical safety and effectiveness of a novel calcium-incorporated titanium implant (Xpeed, MegaGen Implant Co. Limited, Gyeongbuk, South Korea). Materials and methods: In total, 60 patients were randomised to receive one to six titanium implants in the maxilla with either calcium-incorporated (Xpeed) or control resorbable blasted media (RBM) surfaces, according to a parallel group design at two centres. Implants were submerged and exposed at three different endpoints in equal groups of 20 patients at 12, 10 and 8 weeks, respectively. Within 2 weeks, implants were functionally loaded with provisional or definitive prostheses. Outcome measures were prosthesis failures, implant failures, any complications and peri-implant marginal bone level changes. Results: A total of 30 patients received 45 calcium-incorporated implants and 30 patients were given 42 control titanium implants. Five years after loading, eight patients dropped-out from the Xpeed group and nine left the RBM group. No prosthesis or implant failures occurred. Two patients were affected by three complications in the Xpeed group vs five patients from the RBM group, who experienced eight complications; the difference between groups being not statistically significant different (P = 0.187; difference in proportions = 14.7%; 95% CI: -10.7% to 39.4%). Five years after loading patients with Xpeed implants lost on average 1.19 ± 0.48 mm of peri-implant marginal bone vs 1.43 ± 0.98 mm of patients with RBM implants, the difference being not statistically significant (P = 0.35; mean difference: -0.23 mm; 95% CI: -0.73 to 0.27 mm). Conclusions: Both implant surfaces provided good clinical results and no significant difference was found when comparing titanium implants with a nanostructured calcium-incorporated surface with implants with an RBM surface. Purpose: To evaluate the efficacy of short (5 or 6 mm long) dental implants vs 10 mm or longer implants placed in crestally lifted sinuses. Materials and methods: Twenty partially edentulous patients with 5 to 7 mm of residual crestal height and at least 7 mm thickness below the maxillary sinuses as measured on computed tomography scans were randomised according to a parallel group design to receive either one to two 5 or 6 mm long implants (10 patients) or 10 mm long implants (10 patients) after crestal sinus lifting and grafting with anorganic bovine bone. Implants were left to heal submerged for 4 months and were loaded with reinforced acrylic provisional prostheses, replaced, after 4 months, by definitive provisionally cemented or screw-retained metal-ceramic or metal-composite prostheses. Outcome measures were: prosthesis and implant failures, any complications, radiographic peri-implant marginal bone level changes, and patient's satisfaction assessed by blinded assessors, when possible. All patients were followed up to 3 years after loading. Results: Two patients from the augmented group dropped out after the 1-year follow-up. No implant or prosthesis failure occurred. One short implant patient was affected by two complications vs one complication in the long implant group. There was no difference for patients experiencing complications between the two groups (difference in proportions = -0.03%; 95%CI: -0.32 to 0.27; P = 1.000). Short implants lost 0.89 ± 0.25 mm and long implants lost 1.08 ± 0.29 mm of periimplant marginal bone 3 years after loading, the difference between the two groups being not statistically significant (difference = -0.19 mm; 95%CI: -0.46 to 0.09; P = 0.165). All patients were fully or partially satisfied with function and fully satisfied with aesthetics. Conclusions: Both techniques achieved excellent results and no differences were observed between prostheses supported by one to two 5 or 6 mm long implants vs 10 mm long in posterior atrophic maxillae up to 3 years after loading; therefore it is up to clinicians to decide which procedure to use, although longer follow-ups with larger patient populations are needed to better understand if one of these procedures could be more effective in the long-term. Purpose: To compare implants with machined vs roughened surfaces placed flapless in totally edentulous jaws and immediately restored with metal-resin screw-retained cross-arch prostheses. Mandibles were rehabilitated with two implants (Fixed-on-2 or Fo2) and maxillae with three implants (Fixed-on-3 or Fo3). Materials and methods: Forty edentulous or to be rendered edentulous patients (20 in the mandible and 20 in the maxilla) were randomised to the machined group (20 patients: 10 mandibles and 10 maxillae) and to the roughened group (20 patients: 10 mandibles and 10 maxillae) according to a parallel group design. To be immediately loaded implants had to be inserted with a minimum torque of 60 Ncm. Outcome measures were prosthesis and implant failures, complications and peri-implant marginal bone level changes evaluated up to 1 year post-loading. Results: Flaps were raised in four patients from the machined group. Four prostheses on machined implants and three on roughened implants were delayed for loading because a sufficient insertion torque was not obtained. There were no dropouts 1 year after loading. Two maxillary machined implants were lost in two patients (difference in proportions = 0.10; 95% CI = -0.03 to 0.23; P (Fisher's exact test) = 0.487); one maxillary Fo3 prosthesis on machined implants and one mandibular Fo2 prosthesis on roughened implants had to be remade (difference in proportions = 0; 95% CI = -0.14 to 0.14; P (Fisher's exact test) = 1.000). Five patients with machined implants had six complications vs seven patients who had eight complications at roughened implants (difference in proportions = -0.10; 95% CI = -0.38 to 0.18; P (Fisher's exact test) = 0.731). There were no statistically significant differences for implant failures, prosthetic failures or complications between groups. There were no statistically significant differences for marginal peri-implant bone levels between the two groups (estimate of the difference = -0.06 mm; 95% CI = -0.23 to 0.10; P (ANCOVA) = 0.445), with both groups losing marginal bone in a statistically significant way (0.35 ± 0.23 mm for machined and 0.42 ± 0.27 mm for roughened surface). Conclusions: These preliminary results suggest that immediately loaded cross-arch prostheses can be supported by only two mandibular or three maxillary dental implants at least up to 1 year postloading, independently of the type of implant surface used. Longer follow-ups are needed to understand whether one of the two-implant surfaces is preferable. Intraoral digital impression is a viable alternative to conventional impression techniques and has been proven to be even more accurate than conventional materials. Nevertheless, there are cases in which the rheologic properties of conventional materials can be helpful.
