The Center for Disease Control and Prevention estimates that 75% of reported cases of traumatic brain injury (TBI) are mild, where chronic pain and depression are 2 of the most common symptoms. In this study, we used a murine model of repeated mild TBI to characterize the associated pain hypersensitivity and affective-like behavior and to what extent microglial reactivity contributes to these behavioral phenotypes. Male and female C57BL/6J mice underwent sham or repeated mild traumatic brain injury (rmTBI) and were tested for up to 9 weeks postinjury, where an anti-inflammatory/neuroprotective drug (minocycline) was introduced at 5 weeks postinjury in the drinking water. Repeated mild traumatic brain injury mice developed cold nociceptive hypersensitivity and negative affective states, as well as increased locomotor activity and risk-taking behavior. Minocycline reversed negative affect and pain hypersensitivities in male but not female mice. Repeated mild traumatic brain injury also produced an increase in microglial and brain-derived neurotropic factor mRNA transcripts in limbic structures known to be involved in nociception and affect, but many of these changes were sex dependent. Finally, we show that the antiepileptic drug, gabapentin, produced negative reinforcement in male rmTBI mice that was prevented by minocycline treatment, whereas rmTBI female mice showed a place aversion to gabapentin. Collectively, pain hypersensitivity, increased tonic-aversive pain components, and negative affective states were evident in both male and female rmTBI mice, but suppression of microglial reactivity was only sufficient to reverse behavioral changes in male mice. Neuroinflammation in limbic structures seems to be a contributing factor in behavioral changes resulting from rmTBI.

Introduction: There is increased clinical use of gabapentin, these trends appear to chronologically align with the opioid overdose crisis and particularly with the increased restrictions on opioid prescription. Objectives: The study aimed to assess the clinical variables, pain quality, and severity among patients with opioid use disorder as the main substance with and without gabapentin comorbid use. Methods: The research involved 60 patients diagnosed with substance use disorder, divided into two groups: opioid use as the main substance and opioid use with comorbid gabapentin. All participants were assessed using a semi-structured clinical addiction interview, the structured clinical interview for diagnostic and statistical manual of mental disorders, the Addiction Severity Index, urine drug test, McGill Pain Questionnaire, and Douleur Neuropathique en 4 Questionnaire, in one interview setting. Results: The study showed that pain relief was the main desirable effect in the gabapentin group, with 66.2% of the group experiencing it. Euphoria was the desirable effect in the opioid group, while overdose was more common in the gabapentin group. Respiratory complications were also more common in the Gabapentin group. Conclusion: The study concluded that patients with comorbid gabapentin use disorder had a higher frequency of overdose, history of pain, and significant neuropathic pain, while less abstinence. A previous study reported mild changes on the neurologic examination after administration.Investigate the effects of gabapentin on anxiety, sedation, compliance, and neurologic examination in 2 age groups of cats.Thirty‐one young cats and 12 geriatric cats perceived by their owners to be healthy and neurologically normal.Prospective double‐blinded clinical crossover study. Assessment of baseline sedation and anxiety was performed before initial neurologic examinations and after gabapentin administration (100 mg/cat). Assessments were repeated 90 to 120 minutes after administration. Ease of handling pregabapentin and postgabapentin was assessed in the younger cats. All examinations were performed by a board‐certified veterinary neurologist and scoring of examinations was performed by a different, masked board‐certified neurologist.Sixteen cats (50%) in the younger cohort and 6 cats (50%) in the geriatric cohort exhibited an increase in their overall neurologic examination score postgabapentin administration, mainly through new or progressive postural reaction deficits and gait changes. Anxiety and sedation scores were significantly changed in the geriatric population (P < .01, P = .004, respectively); however, only sedation scores were significantly increased in the younger cats after gabapentin administration (P = .004).All study participants showed mild neurologic changes after gabapentin administration, most markedly noted in the geriatric population. Background Methods Results Conclusion Gabapentinoids are commonly prescribed to control neuropathic pain of lumbar radiculopathy. Few trials have compared the efficacy of gabapentin (GBP) and pregabalin (PGB). Therefore, the authors conducted a meta‐analysis to compare the difference in effect between GBP and PGB in lumbar radiculopathy patients.Articles which were published between January 1, 1960 and May 31, 2023 were investigated via Cochrane Central Register of Controlled Trials, Embase, Google Scholar, and MEDLINE. This meta‐analysis was conducted on patients with lumbar radiculopathy. Gabapentin was used as an intervention, and pregabalin as a comparison. As outcomes, pain rating scales including visual analog scale (VAS) and numeric pain rating scale (NRS), and number of adverse events (dizziness and sedation) were obtained.PGB showed statistically significant improvement in pain scale (VAS and NRS) in short‐term follow‐up (6 weeks or less) compared to GBP. (Total mean difference of −0.31) However, in the long‐term follow‐up (6 weeks to 12 weeks), there was no difference in pain reduction effect between two groups. The incidence of AEs showed no difference between two groups.Based on this article, the existing evidence suggests that PGB was more effective in reducing pain of lumbar radiculopathy compared to GBP at the short‐term follow‐up, but there was no difference in the long‐term follow‐up. Introduction: surgical procedures can lead to severe anxiety and stress in patients scheduled for surgery. Anxiety can cause stimulation leads to cardiovascular changes. Use of certain drugs like GABA mimetic like gabapentin are effective in control of anxiety. This will cause decrease in the level of cathacolamine release from adrenal gland cremafinn cell. Aims and objectives: To assess the level of the anxiety of the patient, to assess the level of the sedation levels of the patients, and to assess the hemodynamic responses produced by the body of the patients. Material and methods: This Prospective - Randomized study was conducted in the Anaesthesia Department in Rama Medical College Hospital and research centre, Kanpur from 25TH AUGUST 2022 TO 25TH MARCH 2024. The study was done on 120 patients who participated in the study. The samples were divided into two groups, 60 in each group, Group C (received clonidine) and Group G (received Gabapentin). Baseline vital signs such blood pressure, pulse rate, respiration rate, and oxygen saturation were recorded and these were recorded again after the intubation. Result: Anxiety score was significantly better in clonidine (group C) as compared with gabapentin (group G). Patient who received Gabapentin have more raise in SPO2 level than clonidine. Oral Gabapentin attenuated increase in heart rate, SPO2 and MAP than oral gabapentin. Conclusion: showed that oral clonidine can be used safely and efficiently to obtund the hemodynamic alterations related with laryngoscopy and endotracheal intubation. It was discovered that clonidine was more successful than gabapentin at reducing hemodynamic reactions to the procedures of intubation and laryngoscopy. [ABSTRACT FROM AUTHOR]
