L'aderenza alla terapia rappresenta uno dei fattori principali di successo clinico soprattutto per i pazienti con patologie croniche come quelli con I.R.C. Spesso però, sono numerosi i fattori che la compromettono e il medico e l'infermiere sono chiamati ad intervenire per rafforzare l'alleanza terapeutica con il paziente al fine di migliorare l'aderenza. L'indagine condotta in questo studio su 56 dei 70 pazienti emodializzanti nell'ospedale S. Vito di Torino, ha permesso di individuare quali elementi influenzano la loro aderenza alla terapia farmacologica domiciliare, soprattutto nell'ambito delle conoscenze sui farmaci assunti e del supporto informativo fornito dai medici e gli infermieri del Centro Dialisi. La dimenticanza nell'assumere la terapia si è rivelata tra i fattori principali di non aderenza, seguita dalla posologia delle prescrizioni farmacologiche poco coniugabili con lo stile di vita del paziente. I risultati del questionario somministrato agli assistiti, hanno dimostrato che la conoscenza da parte del paziente sulla terapia assunta e un minor numero di farmaci prescritti, influenzano positivamente la sua aderenza. A1 contrario, la chiarezza delle informazioni ricevute dal personale sanitario non è risultata significativa in questo ambito. Abstract

In the present multicenter study, 120 pts who had been treated by both hemodialysis (HD) and continuous ambulatory peritoneal dialysis (CAPD) for at least 6 months each, were invited to answer questions on 34 matters, to compare symptoms and their well-being while on the two treatments. Patients were invited to choose HD or CAPD and indicate the reasons for their choice. For 28 patients the first treatment was HD and for 92 CAPD. Abstract

Polyomavirus BK (BKV) reactivation can occur in immunodeficient patients. Few studies on BKV infection in patients with systemic lupus erytematosus (SLE) nephritis are available. Aim of this study was to analyse the prevalence of BKV infection by quantifying viral load and to investigate the association with clinical and histological parameters indicating duration, type and activity of SLE. BKV-DNA was evaluated by polymerase chain reaction in serum (sBKV) and urine (uBKV) specimens from 40 patients with SLE nephritis and 29 healthy controls. Renal function, urinary activity, clinical index of SLE activity [SLE Disease Activity Index (SLEDAI) score], CD4+/CD8+ ratio, histological classes and duration of SLE nephritis were compared according to the BKV-DNA-positivity. sBKV was present in 15% of SLE patients and in 13.8% of controls; uBKV in 32% of SLE patients and in 17.2% of controls. There was no significant difference in terms of kidney function, urinary activity, SLEDAI score, presence of anti-dsDNA antibodies, CD4+/CD8+ ratio and BKV viremia and/viruria, as well as there was no significant correlation between SLEDAI score, anti-dsDNA antibodies titers and median viral load. Duration of nephropathy tended to be shorter in patients with BKV viremia and/or viruria; proteinuria/creatininuria ratio tended to be higher in patients with positive sBKV and uBKV. BKV-DNA-positivity tended to be more frequent in patients treated with an immunosuppressive agent versus those on steroid treatment. Reactivation of BKV infection can occur in patients with SLE, although prevalence data do not significantly differ from those obtained in the control group. The trend toward an association between BKV infection and degree of proteinuria and less duration of SLE nephritis could indicate a major susceptibility to develop BKV infection in more active phases of the disease. The role of BKV reactivation in terms of clinical parameters and histological pattern, as well as the role of therapeutic protocols in the onset of BKV reactivation and, conversely, the therapeutic implication of BKV reactivation in SLE patients remain to be defined and should be addressed in further studies on a larger number of patients. Abstract

Background: B19 virus infection with persistent anaemia has been reported in organ transplant recipients. Detection of B19 virus DNA in serum is the best direct marker of active infection. Objective: The present study evaluated the incidence and clinical role of active B19 virus infection in renal transplant recipients presenting with anaemia. Study design: Forty-eight such recipients were investigated by nested PCR on serum samples. The controls were 21 recipients without anaemia. Active HCMV infection was also investigated as a marker of high immunosuppression. Results and conclusions: In 11/48 (23%) patients B19 virus DNA was demonstrated in serum versus only 1/21 (5%) of the controls. Ten of these 11 patients had already been seropositive at transplantation and active infection occurred in eight of them during the first 3 months after transplantation. The remaining patient experienced a primary infection 9 months after transplantation. Eight (73%) of these 11 patients displayed a concomitant HCMV infection and four (36%) showed increasing serum creatinine levels but none developed glomerulopathy; 3/11 (27%) recovered spontaneously from anaemia whereas 8/11 (73%) needed therapy. In conclusion, the relatively high occurrence (23%) of B19 virus infection in patients presenting with anaemia, suggests that it should be considered in the differential diagnosis of persistent anaemia in renal transplant recipients. Presence of the viral DNA should be assessed early from transplantation and the viral load should be monitored to follow persistent infection and better understand the relation between active infection and occurrence of anaemia, and to assess the efficacy of IVIG therapy and/or immunosuppression reduction in clearing the virus.

Abstract

Kidney cells are the common host for JC virus (JCV) and BK virus (BKV). Reactivation of JCV and/or BKV in patients after organ transplantation, such as renal transplantation, may cause hemorrhagic cystitis and polyomavirus-associated nephropathy. Furthermore, JCV and BKV may be shed in the urine after reactivation in the kidney. Rearranged as well as archetypal non-coding control regions (NCCRs) of JCV and BKV have been frequently identified in human samples. In this study, three JC/BK recombined NCCR sequences were identified in the urine of a patient who had undergone renal transplantation. They were designated as JC-BK hybrids 1, 2, and 3. The three JC/BK recombinant NCCRs contain up-stream JCV as well as down-stream BKV sequences. Deletions of both JCV and BKV sequences were found in these recombined NCCRs. Recombination of DNA sequences between JCV and BKV may occur during co-infection due to the relatively high homology of the two viral genomes. [ABSTRACT FROM AUTHOR]

Abstract

This paper describes the development of four Reverse Transcriptase-Polymerase Chain Reaction (RT-PCR) assays devised to evaluate lytic (UL123, immediate-early; UL54, early; UL65, late; and UL99, true late) human cytomegalovirus (HCMV) transcripts. Subsequently, the assays have been validated evaluating the HCMV lytic gene expression in 28 samples (peripheral blood leukocytes, PBLs) from 14 renal transplant recipients. Although the assessment of HCMV transcriptional profile could be useful to evaluate viral reactivation state, further studies on dynamics of viral transcripts in different clinical settings are needed to determine the role of transcriptional profiling in virological monitoring and therapeutic management in immunocompromised patients. Abstract

Human cytomegalovirus (HCMV) is a widespread agent causing a life-long persistent and generally asymptomatic infection in immunocompetent individuals. In contrast, immunocompromised subjects are the most susceptible group to experience HCMV disease. First genes to be expressed during the replication cycle are the immediate early (IE) genes, the products of which have pleiotropic effects on host cell metabolism. Aim of this study was to compare two set of primers in the optimization and standardization of a RT-PCR assay for qualitative detection of mRNA encoded by the IE gene UL123 (IE1). The RT-PCR assays were then used to evaluate the UL123 gene expression in 29 peripheral blood leukocyte (PBL) samples obtained from 14 renal transplant recipients. In particular, 21/29 (72.4%) were positive with set A of primers vs. 24/29 (82.8%) with set B. Only one sample were negative with set B and positive with set A. Twenty-four of 29 samples (82.8%) were pp65-antigaenemia positive: 21 mRNA-UL123 positive with set A vs. 22 with set B; all viraemia-positive patients were mRNA-UL123 positive with both set A and B. Five of 29 samples were pp65-antigaenemia negative: 1 mRNA-UL 123 positive with set A vs. 2 with set B; all of them were viraemia-negative. These two RT-PCR assays could provide a reliable, rapid and sensitive system enabling the detection and identification of UL123 transcripts and could be usefully employed to study the pathogenesis of HCMV-related diseases.
