Many studies have examined the effects of benzene on testosterone. The purpose of this study was to evaluate the possible correlation between the blood levels of benzene and the levels of testosterone.The study involved a group of 148 subjects. For every worker have been made out a blood sample for the evaluation of benzene and testosterone levels and an urine analysis for the evaluation of the levels of trans, trans-muconic acid and S-phenylmercapturic acid. We estimated the Pearson correlation coefficient between the variables in the sample and the urinary metabolites, age, length of service, gender, BMI. For the analysis of the major confounding factors it was performed a multiple linear regression.The Pearson correlation coefficiet showed: 1. a significant inverse correlation between the S-phenyl mercapturic acid and free testosterone; 2. a significant direct correlation between trans-trans muconic acid and BMI. After dividing the sample according to the median of blood benzene (161.0 ng / L), Pearson correlation coefficient showed a significant inverse correlation between the S-phenyl mercapturic acid and free testosterone in the group with values below this median.Our results, to be considered preliminary, suggest that occupational exposure to low levels of benzene, present in urban pollution, affect the blood levels of testosterone. These results need to be confirmed in future studies, with the eventual possibility of including more specific fertility tests.

The fervent urbanization process coupled with the climate changes has been generating a series of more frequent and intense floods all over the world. Although flood risk can never be completely eradicated, its impacts require to be reduced by either improving the modelling of urban drainage systems and deepening the knowledge of flood produced by extreme storm rainfalls. The detailed study of urban drainage networks plays a fundamental role not only on problems related to flooding phenomena that are repeated with increasing frequency, but also on issue related to water quality of run-off (Piro et al., 2012). The objective of the study is to demonstrate the rainfall tracking prediction can be accurately performed in areas where radar measurements are not available, by using a dense network of rain gauges. The results of numerous storm tracking studies reveal that the choice of the hyetograph feature is a very difficult task (Hindi et al., 1977; Felgate et al., 1975; Shaw, 1983). In this study the storm tracking was studied on the basis of the method proposed by Diskin (1990). The methodology proposed has been applied to data from a network of rain gauges distributed over an area of about 1,600 km2 around the city of London, UK. The results demonstrate how rain gauges, that are more approachable than radars for either economical and practical reasons, are very useful in forecasting the movements of storm events in the monitored area.

The aim of our study is to compare liver damage in "outdoor" environment technicians, a category occupationally exposed, and in "indoor" workers.We studied 142 male technicians of the environment exposed to urban pollution and 142 male "indoor" workers not exposed. We compared mean and standard deviation of the following liver parameters: glutamic oxaloacetic transaminase (GOT), glutamic pyruvic transaminase (GPT), gamma-glutamyl-traspeptidasi (γGT) and alkaline phosphatase (PHA), total bilirubin (TB) and direct (DB). We made the two groups comparable for age, length of service, BMI, alcohol consumption and smoking habits, and excluded the workers who presented confounding factors.We found statistically significant differences about the levels of γGT, PHA, GPT and albumin between the "outdoor" workers exposed and the "indoor" control group. In the outdoor group we observed statistically significant values, GPT (51.8 ± 30.6 I.U./l vs. 30 ± 22.3 I.U./l; p = 0.000), γ-GT (42.2 ± 29.4 I.U./l vs. 22.4 ± 20.7 I.U./l; p = 0.000) and PHA (75.7 ± 20.6 I.U./l vs. 59.1 ± 19.6 I.U./l; p= 0.000) compared to the unexposed group. No statistically significant difference emerged between the averages for the values of GOT (25.3 ± 20.7 I.U./l vs. 26 ± 17.7 I.U./l; p = 0.736) in two groups.It clearly emerges that the contaminants may alter the values of liver tests after prolonged exposure.

Over the last few years, some hemocomponents have been used advantageously in clinical neurosurgical practice, not systemically via transfusion but topically as a sealant (fibrin glue). This has diverted the attention of many authors to the role of platelets in the healing process. The combination of hyper-concentrated platelets and fibrin glue (fibrinogen, XIII factor, fibronectin) with activated thrombin produces a platelet gel that can be easily applied to "difficult" wounds. This topical use of hemocomponents has gained an important role in regenerative medicine. The authors have considered the possibility of using a preparation with a high autologous platelet concentration applied in addition to autologous bone during vertebral postero-lateral fusion. The aim of the procedure is to induce a higher rate of vertebral fusion. Between November 2007 and November 2008, 14 patients (9 men and 5 women, mean age 58.9) underwent laminectomy, vertebral stabilization and postero-lateral fusion. The number of vertebral levels involved in stabilization was: 1 in 2 patients, 2 in 5 patients, 3 in 5 patients, 4 in 1 patient and 5 in 1 patient. Platelet gel was obtained by taking 16 ml of peripheral venous blood from the patient. For this procedure two patented test tubes were used for each patient, with a capacity of 8 m each. These make up the REGEN-THT(®) (Thrombocyte Harvesting Tube) system that makes it possible to obtain 8 ml of autologous platelet gel in 40-45 min. The addition of Ca gluconate and ethanol at 95% makes it possible to obtain a preparation of plasma rich in platelets and activated thrombin with a platelet concentration five times superior to the haematic one. The platelet gel is combined with fragments of autologous bone and synthetic bone during surgical operation. To allow a comparative assessment of the degree of fusion achieved with and without application of the platelet preparation in each patient, it was arbitrarily decided to use it in only one half of the operative field. All patients underwent serial CT scans 3 and 6 months after surgery as well as plain X-rays to evaluate bone fusion. The reconstructed CT images, especially in sagittal and axial planes, permitted an evaluation of the degree of vertebral fusion and "bone growth". The fusion rate was calculated measuring the increment of bone density on CT images, by means of an evaluation of the ROI (HU) in the newly formed bone, and comparing bone density within the bone callus formed by autologous and synthetic bone alone in the one to which the platelet preparation had been added. A good rate of fusion was observed in all patients. Furthermore, a comparative analysis of ROI at 3 and 6 months after surgery demonstrated a high increase in the fusion rate during the first 3 months after surgery. After 6 months the differences in ROI between the two sides had balanced out. However, at 6-month follow-up examination, bone density in the half of the surgical field in which platelet gel had been added to autologous-heterologous bone was higher in comparison to the contralateral one. Bony neoformation after posterior-lateral arthrodesis is well-evident 3 months after surgery and usually continues gradually for the following 18-24 months. The autologous platelet preparation used seems to accelerate bony deposition and to promote tissue healing, increasing bone density at the level of posterior-lateral arthrodesis. Moreover, this preparation has low production costs and is easy to apply.

The aim of this study was to evaluate in scientific literature the more frequent work injuries among the occupational categories examined (construction workers, healthcare workers, night workers and shift work, drivers) and to identify occupational stressors that increase the risk of injury.The research was conducted through review of the scientific literature between 1990 and 2014. Two hundred articles were found of which 42 selected in the following categories: construction workers, healthcare workers, shift workers, lorry drivers.The movement of machinery and working at height are due to injury for building sector and they were positively correlated with physical (r = 0,206) and mental (r = 0,254) stress. In health workers the injuries are more frequently bruises and sprains (50%). Studies showed a higher rate of injury in a group of shift workers compared to a control group on a rota basis (p0.0001). Road accidents for drivers represent the most frequent cause of injury of which 13% comes from falling asleep, while 31% from distractions (OR = 3.16; CI = 1:22 to 8:24).Nurses have frequent injuries due to bruises, lumbago, punctures with needles and surgical wounds. Construction workers often suffer serious injury derived from falls at height, handling machinery. For workers on a rota basis, injuries are often related to lack of sleep. Road accidents in drivers are the most common injuries. A synergistic action finalized to promote health and safety organization, to ensure a work environment more secure, is advisable.

A significant relationship between exposure to cadmium and PAHs from urban air pollution and blood pressure has already been found. The aim of the study is to evaluate if other urban pollutants maintain a similar relationship in outdoor workers.349 subjects of both sexes were enrolled in the study; we evaluated values of environmental and biological monitoring of respirable dust, arsenic, lead, benzene, toluene, xylene and we measured blood pressure (BP) in upright and in supine position; to analyze data we used multiple linear regression. Also we valued our results in the light of those we have obtained in previous research.Multiple linear regression showed no significant results that demonstrate a relationship between BP and these pollutants.The results of our study lead to assume that exposure to urban pollutants studied has not connection with blood pressure, and to retain valid the relationship previously observed for cadmium and PAHs, which seem to be the only pollutants that can affect blood pressure.

The aim of the study is to evaluate the occupational risks among food service workers and cooks. During the occupational risks assessment the following risk factors must be evaluated: musculoskeletal disorders, chemical risk (cleaning kitchen work surface, dishes, utensils ecc.) biological risk (contact with foods or biological agents) cancerogenic risk (by baking smoke inhalation), and psycho-social stress. In this study the preventive measures and protective equipment to prevent health hazards for these workers have been evaluated (i.e. aspiration hood, adapted ventilation, chosen of less harmful methods of baking, ecc.). In particular the performance of rigid behavioural norms and hygienic procedures is very important for cooks and food service workers to reduce the risk of occupational infections.

The aim of this study is to estimate the exposure to Cd in a group of roadmen compared to a group of drivers by environmental monitoring and biological monitoring of blood and urinary Cd. It aims to evaluate whether exposure to Cd can affect plasma cortisol.The initial sample was composed by 130 employees of Municipal Police Force (roadmen and drivers). After administration of a clinical-anamnestic questionnaire were excluded 50 subjects who presented confounding factors and 30 subjects who weren't comparable. On a final sample of 25 drivers and 25 roadmen were conducted environmental monitoring of Cd, biological monitoring of urinary and blood Cd and assessment of cortisol.Personal exposure to Cd in air was significantly higher in roadmen compared to drivers (p = 0.015). Urinary and blood Cd were significantly increased in roadmen compared to drivers (p = 0.18 and p = 0.025). There weren't significant differences in plasma cortisol between drivers and roadmen (p0.05). The multiple linear regression analysis shows that urinary and blood Cd are dependent on the environment Cd levels and on the job (driver or roadmen) (p = 0.014 and p = 0.012) and plasma cortisol isn't dependent on blood and urinary Cd.The results show a higher concentration of Cd in roadmen both in air and urine and they don't indicate an influence of exposure to low doses of Cd on plasma cortisol.

Otoacoustic emissions are signals that originate from the cochlea, measuring them can be considered an objective method in the assessment of auditory function. In our study we wanted to examine their efficiency among normal audiological exams.We examined 14 workers exposed to continuous aircraft noise. They underwent ENT examination, metric eardrum test, hearing test tone audiometry system powered by 1 dB, spontaneous otoacoustic emissions and evoked otoacoustic emissions. For inclusion in the study the patients had to show bilateral normoacusia at the audiometry.The variation of the audiometric tracks was statistically significant (p0.05) at a frequency of 4 kHz. As to SNR parameter (relationship between sound/noise) we noticed a tendency of the transient-evoked otoacustic emission (TEOAE) to shift their spectral content, in particular a decrease in the high frequency response and on the other hand an increase at low frequencies. A similar trend for shifting in latency of otoacoustic waves emissions after exposure to chronic noise.The measurement of evoked transient otoacoustic emissions is well tolerated by the workers and provides objective results, so it is a more effective and objective method then the tone audiometry.

Everyday outdoor workers are exposed to chemicals including the polycyclic aromatic hydrocarbons (PAHs). The aim of the study is to evaluate the relationship between hydroxypyrene and blood pressure in outdoor workers.374 subjects of both sexes entered the study. We evaluated the assay of urinary hydroxypyrene (1-HOP); their blood pressure (BP) was measured in upright position and in the supine position and the correlation between the levels of 1-HOP and BP was assessed using Pearson test and linear multiple regression.The results obtained show that in the total sample the levels of urinary 1-OHP are negatively associated with systolic and diastolic BP in supine position and in upright position. The significance is maintained despite the stratification of the sample according to age; in non-smokers we found a negative relationship between 1-OHP and differential blood pressure, both systolic and diastolic.The results of the study suggest that occupational exposure to PAHs may be able to significantly influence the blood pressure probably acting on the autonomic nervous system.

The carpal tunnel syndrome (CTS) is a common working pathology. The CTS diagnosis is not so easy because neurophysiological investigations are necessary. The purpose of this study is to evaluate the working risks and the presence of CTS signs using a focused anamnestic and clinical procedure.We evaluated the working risks of CTS in a population of 65 male workers of a steel industry (average age 25.1 and seniority 2.82), performing 5 different tasks. The Borg Scale was used to evaluate the subjective muscle effort. Diurnal paresthesia (V1FG), nocturnal paresthesia (V1FN), hypersensitivity (V1IS), hypostenia (V1IT) were considered.We identified three main working risks repetitiveness of single actions, static and prolonged posture of the truck, muscle effort. 21.5% of the workers showed at least one of the three symptoms;13.8% showed one or more symptoms; the clinical objectivity was observed in 18.5%. A relation between V1FG, V1FN, clinical objective linked to the workers age of the task 1 (p =0.035) is showed.To prevent CTS, the use of the Borg Scale associated with identification of the risks and with the anamenstic-clinical investigation is useful to discriminate people at risk of CTS.

Aim of this study is evaluating alteration of neuro-immune-endocrine parameters in exposed workers and estimating whether urban pollution can modify them.Literature research. Different categories of exposed workers were included in the study (7287) and compared with controls (8054). To calculate results Effect Size (ES) and confidence interval were used.A correlation between urban pollution exposition and some neurogenic mediators and metabolites alterations was demonstrated; blood values can be alterated by toxicity of benzene and by xenobiotic metabolites' mechanism; follicle stimulating hormone is significantly increased in exposed versus controls.High heterogeneity and literature limitations, together with results of this meta analysis, induce to believe that is necessary to deepen the research about urban pollution effects on these parameters.

The exposure to Cd has been linked to many disease and liver is the major target organ of Cd toxicity following acute or chronic exposure.No study has ever evaluated the association between liver function tests and exposure to Cd present in urban air in outdoor workers.In order to assess the relationship between the exposure to urban Cd and the associated changes in liver functionality markers, we conducted a study on 110 municipal policemen divided into 2 groups according to their tasks: 60 traffic policemen and 50 drivers, underwent tests for the determination of the urinary values of Cd correlated with the blood levels of transaminases and ?-GT. Moreover we investigated the exposure to Cd in the air through a personal dosimetry on a subgroup of 40 subjects (20 traffic policemen 20 police drivers).The results showed that the average personal exposure to air Cd was significantly higher in traffic policemen than drivers. Furthermore, in the subset of 40 subjects and in the total sample of 110 subjects, the mean values of urinary Cd were significantly higher in traffic policemen rather than in the police drivers. No significant difference was observed in the mean values of liver functionality in both groups.Due to the few studies on the effects of the exposure to Cd in big cities, the results of the present study offers useful information about occupational exposure to Cd on the road and in the car and the lack of effects on liver function.

Sociologists and philosophers say technology is a double edged sword, on one hand it increases the potential of human senses, while on the other hand it forces man to adopt rhythms and conditions not natural but artificial, with consequences for his physical and mental health. Modern Cognitive Ergonomic tries to analyze to technology, interacting with mental functions, causes stress and it tries to offer the most appropriate technological solutions for human work. From our analysis it emerges that the old media have an influence on mood causing episodes of insomnia, irritability, anxiety, etc., even if in a way that cannot be completely evaluated. Contrariwise, a controlled use of the web can improve the communication and the interacting abilities in patients with psychosocial distress and low social skills. Nielsen's Heuristics are a great tool to value methods of informatical and media interfaces.

Chronic venous disorder is a public health problem that affects the western industrialized countries. The aim of this study is to evaluate the etiology and prevalence of venous disease of the lower limb in workers, and to identify some risk factors using a detailed and systematic analysis of the literature from 1964 to 2011. There is an important relationship between standing position at work and venous disease. The prolonged orthostatic position of the body implies: venostasis, high pressure and risks of blood clots and thrombosis; in standing workers there is an overproduction of reactive oxygen species (ROS) with oxidation of the components of cell membranes, endothelial damage and increase in vascular permeability. Other risk factors were investigated: sitting during work time, weight lifting-moving and exposure to heat sources, the data suggest that this risk factors are less important than orthostatic body position. Age, sex and familiarity are relevant as the extra-occupational risk factors. For a more accurate study of the role of the prolonged orthostatic position on the development of venous disease in the lover limb all authors should define exactly the population, the role and the length of standing time at work. They should also set a universal language to define the correct standing position (ie. within 1 m2 or steps) and time (ie. one hour or 50%-70% of work time). Attention should be given to prevention, to use early therapeutic measures in view of mortality as a consequence of venous disease, of the high social costs related to the loss of working days, of medical care and of residual disability.

Aim of the study is to evaluate whether occupational exposure to low doses of pollutants present in the air of the city selected for the study could cause alterations in peripheral blood counts in workers of the Municipal Police with outdoor tasks vs workers with indoor tasks. 279 non smoker males were enrolled and divided on the basis of their different kind of task. The dosage of air pollutants was carried out through the use of personal air samplers on a representative group of workers. Data obtained were subject to statistical evaluation consisting of Homogeneity of variance test, ANOVA univariate test with post hoc Bonferroni correction, Jonckheere-Terpstra test and multiple linear regression analysis. The differences were considered statistically significant when p values were lower than 0.05. Mean levels of RBC, HB, MCHC, WBC and neutrophil cells were significantly higher in traffic policemen and police drivers compared to controls (workers with indoor tasks). Mean levels of MCV, MCH and lymphocytes were significantly lower in traffic policemen and police drivers compared to controls. These results were confirmed by Multiple linear regression test and Jonckheere-Terpstra test. The results suggest that prolonged occupational exposure to low doses of traffic pollutants can alter some lines of the hematopoietic system in exposed workers.

Asbestos is a very relevant medical and social issue, because of its potential dangerous effects on human health. It's present indeed in different environments, because it was widely used in the past. Asbestos is classified as first-class carcinogen, and so labelled with R 45 risk phrase. Exposure to asbestos fibers (both occupational and not) can cause diseases involving mainly the respiratory system. We point out the methods for a correct assessment of the risk from asbestos, with particular reference to current legislation and to the main preventive measures that have to be taken in case of presence of asbestos in a building. A correct prevention has to be carried out through education and information of workers and the use of specific check-lists.

The aim of this study is to examine if urban pollution can modify indices of genotoxic damage.Research of articles inherent in the evaluation of these alterations. Differents categories of exposed workers are included in the study (cases, in number of 7287) and compaired with no exposed workers (controls, in number of 8054). To calculate the result of meta-analysis the Effect Size (ES) was used and the confidence interval was calculated.In this category we evaluated 54 studies and we observed significantly differences in the complex.Starting from the individual variables analysis investigated, between exposed and not exposed group, we underline the utility to deepen the studies concerning of these parameters, to appreciate their benefits and to safeguard health of exposed workers.

The few studies in literature about the relationship between venous diseases and work show that the posture assumed while working could promote the occurrence of venous disease in lower limbs. We compared male workers belonging to different occupational categories, matched for age, BMI and traditional risk factors for venous diseases, in order to assess the prevalence of venous disease and occupational and not-occupational postural risk factors. We found that maintaining a standing position for more than 50% of the shift appears the most important occupational risk factor in provoking the observed higher prevalence of venous disease in the workers studied. It can be assumed the venous diseases are often determined by occupational factors which could be main or concomitant causes. The identification of preventive measures to apply in workplaces, such as better organization of work, targeted examinations and therapeutic indications as the prescription of elastic stockings, is very important.

Many studies showed a connection between exposition to high levels of urban pollution (especially to particulate and traffic noise) and the onset of even deadly cardiovascular diseases.Meta-analysis of case-control design.The aim of this study is to estimate the association between cardiovascular effects and occupational exposition to atmospheric pollution in urban environment.Biomedcentral, MEDLINE/ PubMed, MEDLINE/ National Library of Medicine (NLM), MEDLINE Plus, Nioshtic-2, Scopus, TOXNET/Toxline, unpublished studies known by the authors and acts of national and international conferences between 1988 and May 2010 included. There has not been any kind of language or typological restriction. CRITERIA OF ELEGIBILITY: The research on cardiovascular effects includes control cases of workers exposed to urban pollution, compared with non-exposed subjects.The selected studies present outdoor workers exposed to urban pollution (drivers and petrol pump attendants) and a control group of indoor workers (managers, university students and other selected subjects).The evidences (independently token from two different authors) have been grouped in two classes, the first one formed by continuous variables (systolic blood pressure, diastolic blood pressure, heart rate, total cholesterol, HDL cholesterol, LDL cholesterol, triglycerides) and the second one by discontinuous variables (electrocardiographic abnormalities prevalence, hypertension prevalence, hypercholesterolemia prevalence). The studies related to both classes and presenting more than one variable for each class have been included and used, in order to elaborate the results. We calculated heterogeneity in each variable (ES calculation for continuous variables and OR calculation for discontinuous variables).On 378 publications, we have selected and included 16 articles. The variables show statistically irrelevant differences between exposed group and control group, except regarding the triglycerides.The controlled studies are limited and characterized by a non-homogeneous evaluation of both expositional times of workers to urban pollution and of professional expositional values.Considering the heterogeneity and the lack of studies, it is impossible at the moment to document effects on the cardiovascular system in workers exposed to urban pollution. The results of this meta-analysis only suggest the association of urban pollution with alteration of triglycerides blood levels (referring to just three studies). As reported in scientific literature on this subject, it is necessary to conduct a future professional investigation on this subject with more qualified and homogeneous studies.

Our research group has conducted several studies to investigate the effects of occupational risks on the cardiovascular system of exposed workers. In these studies, groups of workers exposed to possible professional cardiovascular risk factors were compared with controls with respect to some parameters (blood pressure, ECG, etc.). The groups were made comparable by age, length of service, traditional cardiovascular risk factors and possible confounding factors outside work. Four studies investigated the effects of noise exposure, three studies the effects of exposure to urban stressors, other studies the presence of other cardiovascular risk factors in various occupational categories (manual workers, agricultural workers, workers exposed to ionizing radiation, workers in rotogravure factory). The results of our studies confirm the presence of cardiovascular risk factors in various professions and the consequent need to take develop preventive strategies in workplaces to prevent the onset of cardiovascular diseases associated with these risk factors.

The aim of our study is to determine the effectiveness in reducing workplace accidents in agriculture using preventive interventions including awareness and effective knowledge of hazards with workers who followed training-informative programs. On the basis of the data reported in the 11 studies three categories we identified: Reduction in the number of workplace accidents: comparison between groups; Reduction in the number of workplace accidents: before-after comparison; Self-rating of injuries risk reduction. The second category showed significant statistical differences in the subgroup who followed preventive intervention compared to subgroup who did not follow this intervention. The third category showed a significant statistical increase of the Consciousness Index of the agricultural workers who followed the training-informative programs. The results suggested the need to use assessment models more suitable to the nature of the examined variables. A promising area is the evaluation of knowledge of risks achieved by workers exposed to formative-informative programs.

The aim of the study is to evaluate whether occupational exposure to urban stressors could cause alterations in thyroid hormones plasma levels (TSH, FT3 and FT4) in female outdoor workers vs. administrative.The study has been performed on an initial sample of 198 female subjects divided in two groups (occupationally exposed and not exposed to urban stressors). After excluding the subjects with the main confounding factors, female outdoor workers and administrative staff were matched by age, working life and drinking habit.In 40 female outdoor workers mean plasma TSH levels were significantly higher compared to 40 administrative staff. The distribution of TSH values in outdoor workers and in administrative staff was significant. Plasma FT3 and FT4 levels were not significantly different in the two groups. Our results suggest that exposure to urban stressors (chemical, physical and psycho-social), can alter the plasma concentration of TSH.According to our previous research, plasma TSH levels may be used as an early biological marker of chronic exposure to urban stressors, in occupational set even before the onset of the related disorders.

With the legislative decree 81/08 and s.m.i. it's explicitly defined a model of management and corporate organization that can contribute to prevent security risks in work environments. The realization of the model is not obligatory, but desirable because the result of its implementation is a decrease of company's risks and costs for safety. Our study group has developed the structure of an organizational and management model for corporate safety and the tools necessary for its realization. The realization of a model is structured in various phases: initial exam, safety policy, planification, implementation, monitoring, system retest and improvement. Such a model, in continuous evolution, is based on the responsibilities of the different corporate figures through an accurate analysis of the measured risks and the measures adopted.

In the past the role of women was mainly that of wife and mother, over the centuries, this role has changed: women assert themselves in every social field and workplace.Analyze the history of women's work, from prehistory to the present day highlighting the evolution of women's role over the centuries. Identify which may be the different occupational stressors, in particular the organizational and psycho-social ones, to which is submitted a working woman.The authors have analyzed the most relevant data from literature, in particular on the issue of mental health.Provide the main forms of prevention that have to be implemented.The main challenge for women today, is to balance work outside the home and housework. This added to job stressors, may interfere with the psychological and physical wellbeing of the worker.

The aim of the study is to evaluate if "outdoor" workers exposed to chemical, physical and psycho-social urban stressors could have alterations of systolic and diastolic blood pressure at rest and heart disease investigated by electrocardiogram compared with a group of "indoor" workers not exposed. Among non-smoking, the mean values of systolic blood pressure at rest were significantly higher in exposed compared to non exposed (p = 0.002) and frequency of electrocardiographic abnormalities was significantly higher in exposed (20.8%) than not exposed (10.4%, p = 0.02). Among exposed, the values of systolic blood pressure at rest were significantly higher in smokers than non-smokers (p = 0.002). Among non-exposed, the frequency of electrocardiographic abnormalities was significantly higher in smokers than non-smokers (p = 0.002). The study results suggest that chronic exposure to chemical, physical and psycho-social stressors can be a cardiovascular risk factor in addition to cigarette smoking in a manner determined by the categories of outdoor workers.

In the last thirty years, several studies have demonstrated a correlation between exposure to noise and the development of alterations and/or pathologies in organs and apparatus, apart from the auditory one. Exposure to noise can induce biochemical, physiological, or psychosocial modifications that can remain either inside the range of biological normality or alter and compromise the psychophysical well-being of the individual depending on the extent of the functional alterations of organic systems and psychosocial functions; both the reversibility and duration of the alterations and the adaptation could be related to the recovery capabilitiy of the human organism and to the environmental condition. Among the factors that can influence the effects of noise we can find: intrinsic characteristics of the physical insult (pressure, sound intensity, emission frequency), extrinsic characteristics (duration of the exposure, way of emission, presence of impulsive components, masking effects), spectral characteristics (infrasounds, ultrasounds, time of recovery) genetic factors (susceptibility) and acquired factors of the human organism (acoustic isolation of the living houses and the social necessity for the industrial activities that generate noise). Other factors that influence the damage are the surprise effect, the semantic content and the identification of the noise source. It can be hypothesized that the extra-auditory effects of noise show themselves through a series of nervous circuits that use the autonomic nervous system and interfere on neuro-immune-endocrine parameters. The organs which are more studied are: cardiovascular, gastro-enteric and endocrine systems, nervous system and psychological effects, respiratory system, fetus and effects on reproductive system, immune system and genetic material.

The monitoring of health conditions in workers exposed to asbestos for prolonged periods is a problematic issue. The Occupational Health physician being required to carry out his activity of prevention of the medical consequences and protection of the workers can use various instruments among which there is the evaluation of lung function. We have performed a meta-analysis on the scientific evidence on this matter in order to identify significant alterations of the lung function parameters in subjects professionally exposed to asbestos compared with controls. By the means of a bibliographic search and the application of selected inclusion and exclusion criteria, we identified 21 articles presenting data from controlled studies. Our results show that the number of controlled trials is very small compared to the totality of studies performed on this matter (1 to 70 ratio) and that the number of the subjects included in the majority of the analyzed studies is oddly divided in the experimental and control group. For this and other reasons the analyzed studies are very heterogeneous. Despite these difficulties, the meta-analysis evidences that some parameters of respiratory functioning are more likely to be influenced by asbestos exposure taking into account the smoking habit and that other parameters are sinergically influenced by both asbestos exposure and smoking. Our meta-analysis points out the necessity to define a common methodology about the technical and operatives procedures in the evaluation of the effects on lung function measures determined by an occupational exposure to asbestos and a thorough examination of the intensity of abnormality levels that could allow using the values obtained from the above mentioned measures in a discriminating diagnostic way.

According to scientific literature, burnout can be described as a multi-dimensional syndrome with three fundamental dimensions: emotional exhaustion, depersonalization and reduced personal accomplishment.Since attention for this phenomenon is increasing, with a large number of articles published in scientific journals in recent years, we set out to evaluate the scientific evidence on this topic by applying the meta-analytical method.All comparative studies performed on burnout were identified and examined in order to compute the Effect Size of the results. Forty studies complied with the inclusion criteria of this analysis for a total of 2,031 burnout cases versus 1,950 controls.Our results revealed a significant disparity in the methods of selecting subjects with burnout. This problem emphasizes the lack of a standardized definition of burnout as a univocal clinical entity and, consequently, the lack of a unanimously agreed procedure for the identification of patients to be included in this diagnostic category.The wide heterogeneity between studies thus considerably reduces the possibility of comparing them and generalizing the results.

The purpose of this clinical study is to evaluate the preventive bypass indications, the kind of bypass to use, and the timing of postoperative internal carotid artery (ICA) occlusion in the treatment of complex aneurysms.This study enrolled 10 patients suffering from complex aneurysms treated using several kind of extra-intracranial bypass on the basis of preoperative careful assessment of cerebrovascular flow. This assessment is based on the balloon test occlusion (BTO), from which three different conditions emerge. According to the Department of Neurosurgery these three different conditions require three different kinds of bypass: in case of depleted flow a short STA-MCA bypass is necessary, in case of intermediate flow a long arterial graft bypass is required, whereas in case of high flow a long venous graft bypass is mandatory.Bypass patency was found in six out of seven patients who underwent surgery for giant ICA aneurysms, as well as in other patients. One of the patients died for a septic shock, another had a neck hematoma that required surgical evacuation. At mean follow-up no further neurological deficits were found.The severe complications due to the combined approaches can be reduced thanks to the indications obtained by BTO concerning the kind of bypass to perform and to the timing of the ICA occlusion.

The aim of the study is to evaluate whether occupational exposure to night work could cause alterations in the levels of plasmatic cortisol. The interest toward this argument arises form several studies in scientific literature referring the presence of an alteration in the synthesis and release of cortisol in workers exposed to night work. We studied a population of workers employed in night security service and monitoring service of alarm systems in different museums compared to a control group not performing shift-work and/or night work. The exposed and control subjects were compared by age, length of service, smoking habit (n. cigarettes per day), habitual consumption of alcoholic drinks (n. glass of wine/beer per day). We evaluated the levels of plasmatic cortisol on 50 workers exposed to night work, all males of whom 30 smokers and 20 non-smokers and on 50 controls of whom 30 smokers and 20 non-smokers.

Lifestyle's alterations are hazardous for health. On one hand they produce a high rate of mortality and disease, on the other hand they cause a reduction of work outcomes and an increase of occupational accidents with important consequences for both worker's health and his/her financial status. The aim of this study is to review the scientific literature for possible relationships between mental health and lifestyle alterations of young workers. We considered as lifestyle factors the attitudes towards: smoking, alcohol consumption, eating, use of medications and doping substances, physical activity and sleeping. From our study it clearly emerges the existence of correlation between lifestyle habits and mental health; in fact behavioural alterations can produce problems of mental health and vice versa. Furthermore, some work peculiarities can lead to psychic disturbances and/or to unhealthy habits which can themselves cause negative effects on working activity. It is very important for young workers to understand that unhealthy behaviours, which can be corrected, are hazardous in terms of health and safety for both the single worker and the collectivity and that those behaviours can enhances the other working risks. Because there is a close interaction between mental health and lifestyles, it would be necessary a careful promotion of mental health on workplaces and to take all the preventive measures, with particular regard for those related to the work organization, in order to reduce the onset, exacerbation and unmasking of mental disorders and psychological difficulties. In working environment, the occupational health physician and his relationship with the patient are of fundamental importance. During preventive and periodic medical examinations, the occupational health physician should take detailed information on young worker's habits regarding smoking, alcohol consumption, eating, physical activity, sleeping, pharmacological abuse and possible presence of mental disorders and furthermore he/she should actively take part in the information and education process of the worker.

The aim of the study is to evaluate the occupational risks among food service workers and cooks. During the occupational risks assessment the following risk factors must be evaluated: musculoskeletal disorders, chemical risk (cleaning kitchen work surface, dishes, utensils ecc.) biological risk (contact with foods or biological agents) cancerogenic risk (by baking smoke inhalation), and psycho-social stress. In this study the preventive measures and protective equipment to prevent health hazards for these workers have been evaluated (i.e. aspiration hood, adapted ventilation, chosen of less harmful methods of baking, ecc.). In particular the performance of rigid behavioural norms and hygienic procedures is very important for cooks and food service workers to reduce the risk of occupational infections.

The GABA(B) receptor agonists display a number of pharmacological effects including central muscle relaxation, decreased self-administration of cocaine and narcotic drugs, antinociception, cognitive impairment as well as enhancement of synaptic plasticity. The main relationships between intrathecal or intracerebral baclofen and the Central Nervous System (CNS) are reviewed with particular attention to actions on pain, epilepsy and basal ganglia regulation. Since baclofen may be involved in synaptic plasticity and the development of neuronal pathways, the main issues of this field are reviewed with particular attention to the effects of baclofen on the developing brain. The role of baclofen in the regulation of movement has not been clearly understood, but recent findings support its important involvement in globus pallidus and subthalamic nucleus. The neuroprotective action of baclofen in cerebral ischemia is a matter of debate. The effects of baclofen in cognition and attention are another important issue because patients with chronic intrathecal baclofen (ITB) administration often present with impairment of cognitive functions. Drug craving and its improvement after baclofen administration is also reviewed. Finally, the clinically interesting results on the regulation of food intake and blood pressure are highlighted. The preliminary experience on the effects in cortical neuron viability at different concentrations of ITB is reported.

Since its introduction, chronic intrathecal baclofen (ITB) infusion has been proved to improve spasticity, spasms and related pain. In the literature, the reported clinical improvement is evident in more than 85% of the patients suffering from spasticity and in more than 66% of the patients suffering from spasms. Usually, the evaluation of spasticity is carried out by the Asworth Scale although there is not yet general accordance on the validity of this scale. It is possible that some of the scales used to assess the implanted patients are not sensitive enough to detect changes in the quality of life or functional outcome. After the pump's implantation, the overall care seems to be rather simple for a devoted team. The side effects are usually temporary but they can worry the patients. The most dangerous side effects are baclofen overdose and withdrawal syndrome. These complications are totally avoidable by adopting an approach attentive to the details regarding the patient, the device, and the procedure.

Mental illness, as anxiety, schizophrenia, obsessive-compulsive disorders, anorexia, have a huge impact on any working population. These disorders are a medical and social reality in strong increasing and they represent a great public interest. People suffering from a mental illness experience many inconveniences: a reduced possibility in choosing of the type of job, the necessity of a support, the difficulty in changing job, a reduction of the working hours, discriminations and abuse. This study explored the role of work in enhancing the economic and social integration of people with mental disorders. The importance of addressing specific issues (supported employment, vocational rehabilitation) related to the employment of persons with mental health problems has also been recognized.

This study was aimed to evaluate if workers exposed to environmental stressors, including the urban traffic noise, might show significant differences compared to a control group in neuro-psychological and emotional profile as well as neurophysiological functions. In particular if these differences could be evidenced by the application of the "oddball paradigm" for event related potential P300 component. The study consisted of the following examinations: (1) exposed workers vs. controls under the odd-ball paradigm and the Stroop test in baseline condition; (2) amplitude and latency ofP300 (in baseline condition and after administration of acute urban traffic noise and Stroop test). The research was carried on a sample of 81 volunteers: 39 workers exposed to environmental stressors and 42 controls. The phonometric measurements showed mean levels of noise due urban traffic like 74 dBAeq. In baseline condition significative differences in exposed workers vs. control were found in Raven's Matrices PM 38 (p = 0.002) and Arithmetic reasoning from WAIS-R (p = 0.0024). Attention capacities as measured by Digit Span Forward and Visual Search, emotional functioning as measured by state- and trait-anxiety test and mood profile were not different in the two groups. Either in baseline condition or after acute stimuli no significant changes were found in two groups concerning the odd-ball paradigm. Exposed workers showed a higher execution time at Stroop test compared to controls (p = 0.047). No differences were found in the number of errors at the Stroop test. Before the acute stimulus, P300 amplitude was significant higher in the exposed workers than in controls (p = 0.002) while the latency was not different between two groups. Both noise (p = 0.001) and Stroop test (p = 0.002) stimulation increased the P300 latency of the whole sample, without significant differences between exposed workers and controls. A significative decrease of P300 amplitude due noise both in the exposed workers (p = 0.001) and in controls (p = 0.012) was found, without significant difference between the two groups. These results are interpreted as follows: (1) there are chronic effects on cognitive functioning in the exposed group vs. controls in baseline condition, like showed by significant differences in Raven PM38 and WAIS-R; (2) the exposed workers have a smaller cognitive flexibility, as shown by the Stroop test results; (3) in baseline condition the greater P300 amplitude in exposed workers reflect a greater division of attentive resources vs. controls, probably linked to the chronic stimulation by environmental stressors, especially noise, to which these workers are exposed; (4) the effects on P300 latency and amplitude can document the physiological response both in the exposed and not exposed to the acute stimulus and that the lack of significative differences in P300 latency and amplitude may be due to adaptative response to acute stimuli in exposed too. Our results allow us to consider that in workers exposed to urban stressor, such as noise, there are effects on cognitive functioning, especially on attention, without auditory damages. The valuation of P300 might represent a valid diagnostic instrument to evaluate the effects on cognitive functions especially on attention, in workers chronically and acutely exposed to urban stressors.

Pathologies due to repetitive activity of the upper limb, well known in scientific literature as WMSDs (Work Related Musculoskeletal Disorders), are considerably increased in the last years. At the moment, there are no validated methods for the assessment of the work-related risk. This study compares two different methods proposed in literature for the assessment of the work-related risk, combining objective and subjective measures.

The three-dimensional microvascular structure of many organs can be adequately investigated only using the corrosion casting technique. We applied this method, consisting of an injection of low viscosity acrylic resin through the major vessels and the subsequent digestion of the organic component with strong alkali or acids, to the choroid plexus of the lateral ventricles in the rat, focusing on its structural vascular features. This approach allowed a qualitative morphological description of the choroid plexus of the lateral ventricles, revealing several aspects of their capillary architecture as well as the morphological details underlying its main functional activity, essential to cerebrospinal fluid turnover. Observation of the casts with scanning electron microscopy gave a detailed picture of the choroid plexus of the lateral ventricles of the rat and enabled us to distinguish four different regions, depending on the site that the lateral ventricles occupied: the anterior olfactory region, the main central region, the longer branch and the inferior horn. Each region mostly consisted of spiral capillaries and had specific characteristics. At high magnification, the casts revealed distinctive vascular specializations, such as numerous bulges regularly placed on the capillaries. This morphological investigation underpins a better comprehension of the pathological mechanisms involving the choroid plexus in the lateral ventricles.
