1. Feedback mechanisms between precipitation and dissolution reactions across randomly heterogeneous conductivity fields

Abstract

Our study investigates interplays between dissolution, precipitation, and transport processes taking place across randomly heterogeneous conductivity domains and the ensuing spatial distribution of preferential pathways. We do so by relying on a collection of computational analyses of reactive transport performed in two-dimensional systems where the (natural) logarithm of conductivity is characterized by various degrees of spatial heterogeneity. Our results document that precipitation and dissolution jointly take place in the system, with the latter mainly occurring along preferential flow paths associated with the conductivity field and the former being observed at locations close to and clearly separated from these. High conductivity values associated with the preferential flow paths tend to further increase in time, giving rise to a self-sustained feedback between transport and reaction processes. The clear separation between regions where dissolution or precipitation takes place is imprinted onto the sample distributions of conductivity which tend to become visibly left skewed with time (with the appearance of a bimodal behavior at some times). The link between conductivity changes and reaction-driven processes promotes the emergence of non-Fickian effective transport features. The latter can be captured through a continuous-time random-walk model where solute travel times are approximated with a truncated power law probability distribution. The parameters of such a model shift towards values associated with increasingly high non-Fickian effective transport behavior as time progresses.

Abstract

This work explores a probabilistic modeling workflow and its implementation targeting CO2 generation rate and CO2 source location by the occurrence of carbonate–clay reactions (CCRs) in three-dimensional realistic sedimentary basins. We ground our study on the methodology proposed for a one-dimensional case study and a single CCR formulation by Ceriotti et al. (2017) which includes a framework to account for thermodynamic parameter uncertainties. This methodology is here extended to a realistic three-dimensional sedimentary basin setting and transferred to encompass different types of CCRs, including two newly formulated CCRs which account for minerals typically observed in sedimentary environments. While testing the ability of the selected procedure to model diverse CCRs in three-dimensional realistic subsurface sedimentary systems, we quantitatively compare the impact of CCR formulation on the spatial distribution of CO2 source location, temperature and pressure compatible with CO2 gaseous generation, and CO2 generation rate in three-dimensional environments characterized by complex and non-uniform stratigraphy. The application of the procedure to various types of CCRs enables us to provide an insight into the impact of mineralogical composition on the activation temperature and pressure and the amount of CO2 released by the different CCR mechanisms. Finally, we show the implementation of the proposed probabilistic framework to define scenarios associated with various levels of probability to be used as the input and boundary conditions for CO2 migration and transport models in the subsurface.

Abstract

Galactosemia is an inborn error of galactose metabolism that occurs mainly as the outcome of galactose-1-phosphate uridyltransferase (GALT) deficiency. The ability to assess galactose oxidation following administration of a galactose-labeled isotope (1-13C-galactose) allows the determination of galactose metabolism in a practical manner. We aimed to assess the level of galactose oxidation in both healthy and galactosemic Brazilian children. Twenty-one healthy children and seven children with galactosemia ranging from 1 to 7 years of age were studied. A breath test was used to quantitate 13CO2 enrichment in exhaled air before and at 30, 60, and 120 min after the oral administration of 7 mg/kg of an aqueous solution of 1-13C-galactose to all children. The molar ratios of 13CO2 and 12CO2 were quantified by the mass/charge ratio (m/z) of stable isotopes in each air sample by gas-isotope-ratio mass spectrometry. In sick children, the cumulative percentage of 13C from labeled galactose (CUMPCD) in the exhaled air ranged from 0.03% at 30 min to 1.67% at 120 min. In contrast, healthy subjects showed a much broader range in CUMPCD, with values from 0.4% at 30 min to 5.58% at 120 min. The study found a significant difference in galactose oxidation between children with and without galactosemia, demonstrating that the breath test is useful in discriminating children with GALT deficiencies.

Abstract

Maple syrup urine disease (MSUD) is an autosomal recessive disease associated with high levels of branched-chain amino acids. Children with MSUD can present severe neurological damage, but liver transplantation (LT) allows the patient to resume a normal diet and avoid further neurological damage. The use of living related donors has been controversial because parents are obligatory heterozygotes. We report a case of a 2-year-old child with MSUD who underwent a living donor LT. The donor was the patient's mother, and his liver was then used as a domino graft. The postoperative course was uneventful in all three subjects. DNA analysis performed after the transplantation (sequencing of the coding regions of BCKDHA, BCKDHB, and DBT genes) showed that the MSUD patient was heterozygous for a pathogenic mutation in the BCKDHB gene. This mutation was not found in his mother, who is an obligatory carrier for MSUD according to the family history and, as expected, presented both normal clinical phenotype and levels of branched-chain amino acids. In conclusion, our data suggest that the use of a related donor in LT for MSUD was effective, and the liver of the MSUD patient was successfully used in domino transplantation. Routine donor genotyping may not be feasible, because the test is not widely available, and, most importantly, the disease is associated with both the presence of allelic and locus heterogeneity. Further studies with this population of patients are required to expand the use of related donors in MSUD.

Abstract

Introducción: Existen pocos estudios que hayan evaluado la eficacia y la seguridad de los sustitutivos de comidas para perder peso. Objetivo: Evaluar la eficacia y la seguridad de un programa de pérdida de peso que incluía productos sustitutivos de comidas. Métodos: Se evaluó el efecto de una dieta hipocalórica versus una dieta hipocalórica que incluía sustitutivos de comidas en 55 voluntarios con sobrepeso u obesidad grado I. Los sujetos fueron distribuidos en un grupo control (dieta hipocalórica) o un grupo de intervención (dieta hipocalórica con sustitutivos de comidas) durante 8 semanas. Se registraron datos antropométricos, hábito tabáquico y nivel de actividad física. También se realizaron extracciones sanguíneas para evaluar cambios bioquímicos al inicio del estudio, a las 4 semanas y al final del estudio. Resultados: El grupo control perdió 3,97 kg de media, mientras que en el grupo intervención se observó una pérdida de 4,44 kg, no siendo estas diferencias estadísticamente significativas entre grupos. Otros parámetros antropométricos como el perímetro de la cintura y el perímetro de la cadera también disminuyeron en ambos grupos, aunque sin diferencias entre grupos. Se observó una disminución estadísticamente significativa (p = 0,041) en los valores de triglicéridos, aunque también sin diferencias entre grupos. Discusión: Los sustitutivos de comidas, dentro de un programa dietético controlado, fueron tan eficaces y seguros para perder peso y modificar otros parámetros antropométricos como el tratamiento dietético convencional sin sustitutivos.Background: There are scarce data about the efficacy and security of meal replacement products as a strategy to weight loss. Aim: To evaluate the efficacy and safety of a weight loss program that includes meal replacement products. Methods: We evaluated the effect of a hypocaloric diet versus a hypocaloric diet that includes meal replacement products in 55 overweight or type I obese patients. Patients were distributed to a control group (hypocaloric diet) or an intervention group (hypocaloric diet with meal replacement products) during 8 weeks. We registered anthropometric data, smoke habit and level of physical activity. We also studied biochemical parameters at the beginning of the study, at 4th week of the study and at the end of the same. Results: The control group lost 3.97 kg, while in the intervention group we observed a loss of 4.44 kg. These differences were not statistically significant between groups. Other anthropometric parameters as waist and hip perimeters diminished also in both groups, without differences between them. We observed also a statistically significant decrease (p = 0.041) in the values of triglycerides, without differences between groups again. Discussion: Meal replacement products were as effective and safe to lose weight and to modify other anthropometric parameters in a controlled dietetic program as a conventional dietetic treatment without meal replacement products.

Abstract

El batolito Cerro Aspero-Alpa Corral (CA-AC) es un cuerpo granítico que aflora en el extremo sur de las Sierras Pampeanas Orientales, en la provincia de Córdoba, República Argentina, cubriendo una superficie cercana a los 440 km2. Es un cuerpo intrusivo postcinemático de probable edad paleozoica. Las rocas encajantes son metamorfitas correspondientes al Precámbrico Superior-Paleozoico Inferior constituidas principalmente por esquistos micáceos, gneises y migmatitas que localmente alternan con pequeños bancos de anfibolitas. Este trabajo intenta caracterizar desde el punto de vista petrográfico y geoquímico un área central de este cuerpo de 150 km2, ubicada entre 32°34'-32°42'LS y 64°43'-64°52'LW. Fueron reconocidas tres facies graníticas en base a sus características mineralógicas, texturales, modo y quimismo. La facies porfídica con megacristales de feldespato potásico es la facies arealmente preponderante, aunque también se encuentran bien representados diques de micro-leucogranitos y granitos con fenocristales de biotita. Son todos monzogranitos con un quimismo subalcalino y peraluminoso. Su mineralogía muestra proporciones variables de los componentes esenciales y micas, con fases menores representados por óxidos de Fe-Ti, apatito (Ap), titanita (Ti) y allanita (Aln). Enclaves microgranulares abundantemente distribuidos en la facies porfídica, proveen valiosa información para interpretar la evolución del magma que los aloja. El alto contenido de Ca de las muestras de este cuerpo plantea una importante diferencia al compararlas con las del batolito de Achala, cuerpo granítico de mayor envergadura ubicado al norte de CA-AC. También afloran dentro del batolito CA-AC pegmatitas y aplitas, así como brechas portadoras de fluorita y venas de cuarzo, estériles o mineralizadas.

Abstract

The pathogenic molecular mechanisms underlying the insurgence of nasal polyps has not been completely defined. In some patients, these lesions can have a recurrence after surgery removal, and the difference between recurrent and not recurrent patients is still unclear. To molecularly characterize and distinguish between these two classes, a cohort of patients affected by nasal polyposis was analysed. In all patients we analysed the p63 isoform expression using fresh tissues taken after surgery. Moreover, confocal immunofluorescence analysis of fixed sections was performed. The results show high ΔNp63 expression in samples from the nasal polyps of patients compared to the normal epithelia. Analysis of the expression level of the TAp63 isoform shows differential expression between the patients with recurrence compared to those not recurring. The data, considered as the ΔN/TAp63 ratio, really discriminate the two groups. In fact, even though ΔNp63 is expressed in non-recurrent patients, the resulting ratio ΔN/TAp63 is significantly lower in these patients. This clearly indicates that the status of TAp63 expression, represented by the ΔN/TAp63 ratio, could be considered a prognostic marker of low recurrence probability. In these samples we also investigated the expression of OTX2 transcription factor, known to be a selective activator of TAp63, detecting a significant correlation. Database analysis of HNSCC patients showed increased survival for the patients presenting OTX2 amplification and/or overexpression. These results, together with the fact that TAp63 can be selectively upregulated by HDAC inhibitors, open the possibility to consider local treatment of recurrent nasal polyps with these molecules.

Abstract

Background Ticagrelor (TICA), a potent P2Y12 receptor antagonist is recommended to patients with acute coronary syndromes. However, TICA exposes to a risk of bleeding, which management is challenging because platelet transfusion is ineffective and no antidote is available. We hypothesized that in a milieu containing ADP, epinephrine (EPI), a current vasopressor drug, could counter the antiplatelet effects of TICA and restore platelet functions, via α2A-adrenergic receptors and Gz-protein pathway. Objective To assess in vitro the efficacy of EPI in restoring platelet aggregation inhibited by TICA and investigate the underlying mechanisms. Method Using lumi-aggregometry of washed platelets, we evaluated aggregation and secretion induced by EPI alone or in combination with ADP. VASP dephosphorylation and Akt phosphorylation were quantified to determine AMPc and PI3k pathway contribution, respectively. The role of P2Y1 receptor in restoration of platelet aggregation was determined using MRS2179, a specific inhibitor, and calcium mobilization measurement. Results TICA inhibited ADP-induced platelet aggregation (5 [2–17] vs 59 [54–73]%, P Conclusion In combination with ADP, EPI restored platelet aggregation inhibited by TICA trough inhibition of cAMP pathway and activation of PI3 K pathway, thus allowing the P2Y1 receptor signaling and subsequent calcium mobilization by ADP. This proof-of-concept study must be challenged in vivo for the management of bleeding in ticagrelor-treated patients.

Abstract

We report two cases in which moderate and intense rocuronium-induced neuromuscular block was reversed intraoperatively with low sugammadex doses in order to facilitate electromyographic evaluation of facial nerve function during surgery of the parotid gland and the middle ear. Acceleromyography was used to assess reversal of neuromuscular block before starting electromyography monitoring. Rocuronium-induced neuromuscular block was reversed with sugammadex 0.22mgkg(-1) when the TOF ratio was 0.14 in the first patient, and with sugammadex 2mgkg(-1) during intense block (PTC 0) in the second patient. In each case, appropriate neuromuscular function (TOF ratio≥0.9) was established soon after sugammadex administration, and electromyographic evaluation of facial nerve was successfully conducted. The use of rocuronium and sugammadex, coupled with objective neuromuscular monitoring with acceleromyography, assured complete restoration of neuromuscular function and created the optimal conditions for the surgical team.

Abstract

The effect of the incorporation of several lithium salts on the electrical and mechanical properties of polyurethane rigid (PUR) foams was investigated. Different amounts of lithium chloride (LiCl), lithium perchlorate (LiClO4), and lithium trifluoromethanesulfonate (LiCF3SO3) were added to the polyuretanic precursor. The salts affected the cellular microstructures and consequently the mechanical properties of the composite. Composite foams containing an amount of LiCl greater than 2 wt% showed low-surface resistivity (∼106 Ω), whereas the LiClO4 and LiCF3SO3 composites showed, in all range of filler percentage analyzed, high-surface resistivity values (∼1011 Ω). This behavior was related to the different interactions between PUR and lithium salts, as confirmed by FTIR/Attenuated Total Reflectance analysis. Only the LiCl was able to create a motion of the ions Li+ and Cl− along the polyurethanic chains, because LiCl was completely dissociated. On the contrary, LiClO4 and LiCF3SO3 that affected the macromolecular structure of the polymeric network did not permit the formation of polyurethanic channels, where the ions could move, thus creating a charge motion. Abstract

Acute pancreatitis (AP) is a rare and life-threatening manifestation of juvenile systemic lupus erythematosus (JSLE). The objective of this study was to evaluate the prevalence and clinical features of AP in our JSLE population. AP was defined according to the presence of abdominal pain or vomiting associated to an increase of pancreatic enzymes and/or pancreatic radiological abnormalities. Of note, in the last 26 years, 5367 patients were followed up at our Pediatric Rheumatology Unit and 263 (4.9%) of them had JSLE diagnosis (ACR criteria). AP was observed in 4.2% (11/263) of JSLE patients. The median of age of the JSLE patients at AP diagnosis was 12.4 years (8.8—17.9). All of them had lupus disease activity at AP onset. Three patients were receiving corticosteroids before AP diagnosis. Interestingly, 10/11 JSLE patients fulfilled preliminary guidelines for macrophage activation syndrome, three of them with macrophage hemophagocytosis in bone marrow aspirate and hyperferritinemia. The hallmark of this syndrome is excessive activation and proliferation of T lymphocytes and macrophages with massive hypersecretion of proinflammatory cytokines and clinically it is characterized by the occurrence of unexplained fever, cytopenia and hyperferritinemia. AP treatment was mainly based on intravenous methylprednisolone. Four JSLE patients with AP died and two developed diabetes mellitus. In conclusion, AP was a rare and severe manifestation in active pediatric lupus. The association between AP and macrophage activation syndrome suggests that the pancreas could be a target organ of this syndrome and that pancreatic enzyme evaluation should also be carried out in all patients.

Abstract

Juvenile systemic lupus erythematosus (JSLE) and autoimmune hepatitis (AIH) are both autoimmune disorders that are rare in children and have a widespread clinical manifestation. A few case reports have shown a JSLE-AIH associated disorder. To our knowledge, this is the first study that simultaneously evaluated the prevalence of JSLE-AIH in a large JLSE and AIH population in groups of Hepatology and Rheumatology of a tertiary Paediatric University Hospital. In a 24-year period, 228 patients were diagnosed with JSLE (ACR criteria). In the same period, 252 patients were diagnosed with AIH according to the International Autoimmune Hepatitis Group. In this article, we present the demographic data, clinical features, laboratory exams and treatment of four children with both the diseases. The prevalence was 1.8% in JSLE population and was 1.6% in AIH population. The current median age was 15.5 years and three were females. In three of them, the diagnosis of AIH preceded JSLE. All of them had increased liver enzymes with a characteristic liver biopsy of AIH and responded to the combination of prednisone, azathioprine and antimalarial drugs. In conclusion, the presence of AIH-JSLE associated disorder was rarely observed. The liver biopsy could be necessary in patients with JLSE with a persistent increase of liver enzymes. Abstract

Objective: To analyse the use of psychoactive drugs in a health and welfare centre and compare this use with current guidelines. Method: A cross-sectional study of the drug treatment regime of hospitalised patients in a health and welfare centre was carried out. Information was obtained from prescriptions and the clinical histories of patients in the centre. The following variables were assessed: demographic data, treatment with psychoactive drugs, date of commencing treatment, dosage, drug combinations, indication and total number of drugs analysed. The results were compared with the literature and current prescription guidelines. Results: 45 of the 70 patients analysed were taking psychoactive drugs: 51.1% were being treated with neuroleptic drugs, 42.2% with antidepressants, and 6.7% with anxiolytic agents. 62% were women. The overall mean age was 80.3 years old. The average number of psychoactive drugs administered to each patient was 1.6 and the average number of total drugs prescribed was 10.5. The most frequently administered psychoactive drugs were risperidone, lorazepam and citalopram. The most frequent association was neuroleptic drugs with benzodiazepine. The indication and dosage prescribed were appropriate according to the data sheet, although some inappropriate prescription practices were observed. Conclusions: The results of the study would recommend controlling the duration of treatment with benzodiazepine, confirming the diagnosis of states of depression and correctly monitoring the associations between psychoactive drugs.

Abstract

Gallbladder cancer is a relatively uncommon malignancy. Its presentation is similar to that of gallstone disease and sometimes with non-specific symptoms. Laparoscopic cholecystectomy has become the method of choice for removing the gallbladder in most benign conditions. Occasionally, unsuspected gallbladder carcinoma is encountered in association with laparoscopic cholecystectomy. Overall gallbladder cancers have a poor prognosis, despite surgery or adjuvant therapies. However, in selected cases, a favourable outcome can be expected and the less favourable predicted outcome can be improved. Management of patients with gallbladder cancer in different situations is discussed: gallbladder cancer noted post-operatively on final pathology, gallbladder cancer noted after removal of the gallbladder and opening of the specimen at the time of surgery, difficulty encountered at the time of dissection and resultant suspicion of gallbladder cancer, and diagnosis of extensive disease at initial placement of the laparoscope.

Abstract

Context: Breakthrough pain is common in patients with cancer and is a significant cause of morbidity in this group of patients. Objectives: The aim of this study was to characterize breakthrough pain in a diverse population of cancer patients. Methods: The study involved 1000 cancer patients from 13 European countries. Patients were screened for breakthrough pain using a recommended diagnostic algorithm and then questioned about the characteristics and management of their pain. Results: Of the 1000 patients, 44% reported incident pain, 41.5% spontaneous pain, and 14.5% a combination. The median number of episodes was three a day. The median time to peak intensity was 10 minutes, with the median for patients with incident pain being five minutes (P < 0.001). The median duration of untreated episodes was 60 minutes, with the median for patients with incident pain being 45 minutes (P = 0.001). Eight hundred six patients stated that pain stopped them doing something, 66 that it sometimes stopped them doing something, and only 107 that it did not interfere with their activities. Patients with incident pain reported more interference with walking ability and normal work, whereas patients with spontaneous pain reported more interference with mood and sleep. As well, 65.5% of patients could identify an intervention that improved their pain (29.5%, pharmacological; 23%, nonpharmacological; 12%, combination). Regarding medications, 980 patients were receiving an opioid to treat their pain, although only 191 patients were receiving a transmucosal fentanyl product licensed for the treatment of breakthrough pain. 33. Intérêt d’une check-list per-anesthésique spécifique : étude prospective, en simple-insu

Abstract

Introduction L’efficacite des check-lists n’est plus a demontrer en medecine pour ameliorer la securite des patients [1] . En dehors de la check-list officielle de la HAS realisee au bloc operatoire, nous avons cherche a evaluer dans cette etude prospective, en simple-aveugle, la pertinence d’une check-list specifiquement dediee aux 3 phases critiques de l’anesthesie generale. Materiel et methodes Nous avons elabore une check-list reprenant plusieurs items a rechercher avant l’induction anesthesique (elements du dossier medical, materiel de suppleance), juste apres l’intubation (installation patient, reglage alarmes, rechauffage) et juste avant le reveil (decurarisation, analgesie). Pour valider la pertinence de cette check-list, nous avons choisi d’evaluer un certain nombre d’items (donnees patient, aspiration fonctionnelle, bougie de Frova, insufflateur manuel, stethoscope et medicaments d’urgence en salle, canule de guedel en place, protection des yeux, pression du ballonnet verifiee, alarmes EtCO2 reglee, rechauffage patient) de facon anonyme dans notre centre sur une periode de 3 mois : operateur unique ne participant pas a l’anesthesie, recueil en peroperatoire. Les resultats sont exprimes en mediane [interquartile] ou nombre ( %). Resultats Sur la periode d’etude, 51 patients (H/F = 21/30 ; 58 [43–69] ans ; ASA I (42 %), II (46 %), III (10 %), IV (2 %)) operes d’une chirurgie digestive, orthopedique, gynecologique ou vasculaire ont ete inclus. Aucun des elements evalues ne manquait dans 47 % des cas (24 patients). Parmi les 27 patients restant, 1 (78 %), 2 (18,5 %) ou 3 (3,5 %) items faisaient defaut. Dans 29 % des cas (n = 15), l’alarme de l’EtCO2 n’etait pas reglee, suivie par l’absence de verification de la pression du ballonnet de la sonde d’intubation (10 cas = 20 %), l’absence d’insufflateur manuel (3 cas = 6 %), l’absence de bilan preoperatoire (2 cas), l’absence de rechauffage (2 cas), une aspiration non fonctionnelle (1 cas) et l’absence de verification des criteres d’intubation difficile (1 cas). Aucune consequence nefaste n’etait notee. Discussion Cette etude pilote montre la pertinence de l’utilisation systematique de check-lists specifique au cours des 3 phases critiques de l’anesthesie generale. En effet, une telle check-list aurait permis d’optimiser la prise en charge des patients dans 53 % des cas. Ce travail est poursuivi pour obtenir un collectif plus important.

Abstract

This paper on trucked air cargo versus an environmentally-sustainable intermodal freight system is from the proceedings of 14th International Conference on Urban Transport and the Environment in the 21st Century, which was held in Malta in 2008. The authors focus on travel times, operating costs, and total costs, using a case study at the Tuscan Industrial Zones in Italy. They stress that the use of the proposed freight intermodal system can be a good solution only if it is accepted by transport operators. The paper first identifies the Tuscan Industrial Zones (Italy), which are characterized by an important air cargo export, and defines the destination airports, to give a graphic description about ways that they are linked and to determinate current travel times and operating costs. The authors then propose a new intermodal system called “Metrocargo,” which realizes a rapid road-railway modal exchange that evaluate travel times and operating costs based on the regional railway lines that link the demand origin and the destination points. They also quantify the time and monetary values for both trucked air cargo and rail transport.

Abstract

To analyse the use of psychoactive drugs in a health and welfare centre and compare this use with current guidelines.A cross-sectional study of the drug treatment regime of hospitalised patients in a health and welfare centre was carried out. Information was obtained from prescriptions and the clinical histories of patients in the centre. The following variables were assessed: demographic data, treatment with psychoactive drugs, date of commencing treatment, dosage, drug combinations, indication and total number of drugs analysed. The results were compared with the literature and current prescription guidelines.45 of the 70 patients analysed were taking psychoactive drugs: 51.1% were being treated with neuroleptic drugs, 42.2% with antidepressants, and 6.7% with anxiolytic agents. 62% were women. The overall mean age was 80.3 years old. The average number of psychoactive drugs administered to each patient was 1.6 and the average number of total drugs prescribed was 10.5. The most frequently administered psychoactive drugs were risperidone, lorazepam and citalopram. The most frequent association was neuroleptic drugs with benzodiazepine. The indication and dosage prescribed were appropriate according to the data sheet, although some inappropriate prescription practices were observed.The results of the study would recommend controlling the duration of treatment with benzodiazepine, confirming the diagnosis of states of depression and correctly monitoring the associations between psychoactive drugs.

Abstract

There are scarce data about the efficacy and security of meal replacement products as a strategy to weight loss.To evaluate the efficacy and safety of a weight loss program that includes meal replacement products.We evaluated the effect of a hypocaloric diet versus a hypocaloric diet that includes meal replacement products in 55 overweight or type I obese patients. Patients were distributed to a control group (hypocaloric diet) or an intervention group (hypocaloric diet with meal replacement products) during 8 weeks. We registered anthropometric data, smoke habit and level of physical activity. We also studied biochemical parameters at the beginning of the study, at 4th week of the study and at the end of the same.The control group lost 3.97 kg, while in the intervention group we observed a loss of 4.44 kg. These differences were not statistically significant between groups. Other anthropometric parameters as waist and hip perimeters diminished also in both groups, without differences between them. We observed also a statistically significant decrease (p=0.041) in the values of triglycerides, without differences between groups again.Meal replacement products were as effective and safe to lose weight and to modify other anthropometric parameters in a controlled dietetic program as a conventional dietetic treatment without meal replacement products.

Abstract

To cope with the growing demand of moving freight, an innovative system for easily and quickly manage containers moved by trains, named Metrocargoreg, has been designed. The leading idea of such a system is to speed up the operations of loading/unloading containers on/from trains, with the goal of assimilating goods to passengers in transportation, thus increasing the flexibility of the whole system and, possibly, optimising the space allocation inside each terminal. In this framework, whereas the performances of such a system appear to be promising, no models to evaluate them have been proposed yet. Then, to achieve this task, in the paper a model of Metrocargoreg intermodal terminals based on timed Petri nets is presented, which allows to easily define a modular, performance-evaluation oriented representation of the considered system

Abstract

Gallbladder cancer is a relatively uncommon malignancy. Its presentation is similar to that of gallstone disease and sometimes with non-specific symptoms. Laparoscopic cholecystectomy has become the method of choice for removing the gallbladder in most benign conditions. Occasionally, unsuspected gallbladder carcinoma is encountered in association with laparoscopic cholecystectomy. Overall gallbladder cancers have a poor prognosis, despite surgery or adjuvant therapies. However, in selected cases, a favourable outcome can be expected and the less favourable predicted outcome can be improved. Management of patients with gallbladder cancer in different situations is discussed: gallbladder cancer noted post-operatively on final pathology, gallbladder cancer noted after removal of the gallbladder and opening of the specimen at the time of surgery, difficulty encountered at the time of dissection and resultant suspicion of gallbladder cancer, and diagnosis of extensive disease at initial placement of the laparoscope.

Abstract

Laparoscopic splenectomy has gained increasing acceptance in the surgical management of a variety of splenic disorders, in particular hematological diseases. In this series, we report our experience with 20 patients (male:female ratio of 4:16 with median age of 16 years, range 5-49 years) who underwent this procedure because of ITP in 9 cases, spherocytosis in 7 and Cooley disease, sickle cell anemia, dyserythropoietic and hemolytic anemia in one case each. The patient was placed in a supine position using a fourtrocars technique. We did not perform pre-operative splenic artery embolization in any case. Spleen lower pole and its posterolateral attachments were dissected first, using electrocautery and endoclips. Vascular hilar isolation was achieved with an EndoGIA stapler and the spleen was removed by morcelation within a retrieval bag (16 cases) or via a 4-5 cm left subcostal incision (4 cases). One patient required conversion to open technique (conversion rate 5 %), because of uncontrolled bleeding from splenic hilum. Mean operative time was 165 min (range 100-240 min), mean splenic size was 13.5 cm (range 11-20 cm), with weight ranging between 140 and 1060 g and estimated blood loss was 151 ml (75-280 ml). No patient required a blood transfusion. Median postoperative hospital stay was 4 days (range 3-8 days). Postoperative complications occurred in 2 patients (10%), with no mortality rate in this series. Regarding the low complication rate and the advantages of a small abdominal trauma in the postoperative period, such as less postoperative pain, faster hospital discharge and better cosmetic results, the laparoscopic approach for elective splenectomy in hematological disorders has a substantial benefit for the patient.

Abstract

We report a patient that presented two episodes of coma in the neonatal period, with severe metabolic acidosis and hypoglycemia, without ketosis. The urinary organic acid analysis showed increased amounts of 3-hydroxy-3-methyl-glutaric, 3-methylglutaconic, 3-methylglutaric and 3-hydroxyisovaleric acid. The deficiency of 3-hydroxy-3-methylglutaryl-CoA lyase was diagnosed by the clinical and metabolic features. This disease shows autosomal recessive inheritance and the treatment is done by a diet with restriction of protein (mainly leucine) and lipids, high in carbohydrate content, and the avoidance of fasting and carnitine supplementation.

Abstract

The objective of this study was to increase our understanding of the importance of members of the Streptococcus milleri (SM) group as respiratory pathogens, by studying the epidemiological and clinical features of thoracic infections caused by this group and comparing the epidemiology and prognosis of empyema caused by SM with cases of pneumococcal aetiology. The clinical histories and microbiology reports were reviewed in 27 cases of thoracic infection caused by SM over a period of 8 yrs. Cases of pneumococcal empyema that occurred during the same period were also analysed. Diagnoses were made of cases of empyema, including six with pneumonia and one with pulmonary abscess, three cases of pneumonia and two of mediastinitis. In 17 cases, SM was the only pathogen isolated. There was a history of instrument or surgical procedures on the digestive or respiratory tract in 59%. Secondary bacteraemia was documented in three cases. The treatment administered, a combination of antibiotics and surgery, was successful in 22 of 27 (81%) of cases. All strains were susceptible to penicillin. When the characteristics of the empyemas caused by monomicrobial SM infection were compared with those of pneumococcal aetiology from the same period of study, significant differences were found with respect to age, origin of the infection and the need for surgery. In conclusion, thoracic infections caused by Streptococcus milleri are largely pleural. They are polymicrobial in one-third of cases, commonly acquired in hospital and, in most patients, associated with major surgery and/or surgical procedures of the respiratory or digestive tract. The empyema frequently requires thoracotomy for complete resolution.

Abstract

In CAD-systems technical objects are given by set of points or analytic surface representations. Often the faces of these objects are unfair. In this paper energy oriented algorithms are developed to approximate a set of points by fair surfaces or to smooth automatically given unfair surfaces. To get linear (and fast) algorithms in both cases quadratic models of the bending energy of surfaces are used. Minimal values of these energy models lead to smooth and fairshaped surfaces.

The title of this workshop “Reverse Engineering” is to demonstrate that this meeting is above all concerned with “surface reconstruction or mathematical modelling using 3D points”. Nevertheless the authors are of the opinion that this is only a link in the chain of a more general task, which is: Manufacturing Technology. In this context, the aim of Sculptured Surfaces Metrology using Coordinate Measuring Machines and the HOLOS software package, specially developed for this application field, will be introduced.

Abstract

A retrospective study was carried out of 159 CT examinations acquired in 64 patients who underwent unilateral nephrectomy for renal-cell carcinoma (34 in the left-hand side and 30 in the right-hand side). Thirty-nine patients were examined twice in the three years following nephrectomy; 25 were examined three or more times over a 5-year period after nephrectomy. Acute or chronic inflammatory disease was demonstrated in 6 patients (4 abscesses were detected by means of clinico-radiologic signs and 2 by means of cytology after CT-guided needle biopsy). Twenty patients had local recurrences 2-7 cm phi: in 17 of them the diagnosis was confirmed by CT-guided percutaneous FNAB; 15 patients had liver metastases, in 3 of them associated with local recurrences; 26 patients were free of both local recurrences and distant metastases. The authors analyze the value of CT in the definition of post-nephrectomy anatomic alterations, in the early detection and staging of local recurrences, and in the differentiation between postoperative fibrosis and complications. Moreover, the value is stressed of CT-guided FNAB of suspicious lesions. Our study suggests the value of a methodical CT follow-up of asymptomatic post-nephrectomy patients. CT immediately after surgery is also recommended to serve as a baseline reference for subsequent examinations. In our series, CT was accurate in the early detection of both local recurrences and distant solitary metastases.
