Bow Hunter's syndrome is a rare cause of posterior circulation ischemia, produced by the mechanical and reversible occlusion of the vertebral artery during cephalic rotation. Diagnosis requires clinical suspicion and careful inspection of images with three-dimensional reconstruction. The study of choice is dynamic digital subtraction angiography (DSA). Treatment alternatives are: medical, surgical or endovascular. We report the case of an 8-year-old boy with recurrent infarctions of the posterior circulation secondary to the dissection of the vertebral artery, in association with an occipital bone spur. Dynamic DSA was negative. Conservative initial management was elected with cervical immobilization and anticoagulation, but due to persistence of symptoms, surgical decompression was decided. The patient did not repeat symptoms postoperatively and returned to his usual life. This is the first case reported to our knowledge of a surgical pediatric patient with asymptomatic atypical compression of VA secondary to BHS, whose dynamic angiography was negative, suggesting an alternative mechanism of the syndrome.

Dental pulp stem cells (DPSCs) are multipotent stem cells derived from neural crest and mesenchyme and have the capacity to differentiate into multiple cell lineages. It has already been demonstrated that DPSCs differentiate into melanocyte-like cells but only when cultivated in a specific melanocyte differentiating medium. In this study we have shown, for the first time, that DPSCs are capable of spontaneously differentiating into mature melanocytes, which display molecular and ultrastructural features of full development, including the expression of melanocyte specific markers and the presence of melanosomes up to the terminal stage of maturation. We have also compared the differentiating features of DPSCs grown in different culture conditions, following the timing of differentiation at molecular and cytochemical levels and found that in all culture conditions full development of these cells was obtained, although at different times. The spontaneous differentiating potential of these cells strongly suggests their possible applications in regenerative medicine.

Fast pyrolysis is a thermochemical conversion process that has gained increased attention over the last decade since it offers an effective way to produce valuable bio-oil as a green alternative to crude oil in the production of liquid fuels, platform chemicals and materials. A clear understanding of how the feedstock composition impacts on the distribution and quality of pyrolysis products is important for both the optimization of the process and the design of bio-oil upgrading strategies. To address this knowledge gap, the conversion of two different types of biomass (i.e., spruce wood and wheat straw) into bio-oil has been experimentally investigated and the obtained results are described in the present work. In more detail, the pyrolysis experiments were performed in a bench-scale fluidized bed reactor at 500 °C and at atmospheric pressure, using a feed particle size smaller than 1 mm and a gas residence time of about 1 s, which allowed to achieve relatively high yields of the liquid product for the two investigated feedstocks (72% for spruce wood and 44% for wheat straw). Results highlight that the biomass composition significantly affects the distribution and the composition of pyrolysis products.

The potential of the fluidized bed gasification as a sustainable waste-to-fuel technology to handle the growing amount of sewage sludge (SS) generate from wastewater treatment plants located in the Campania region has been assessed in the present work. To this aim, the properties of SS relevant to its thermochemical conversion have been extensively investigated and the obtained results are briefly described in the present paper. Preliminary results from the gasification of SS in small-scale fluidized bed reactor are also presented and discussed, with a special focus on the composition of residual bottom ashes.

One of the first analysis items is the study of the f radiative decays, which allows us to investigate the nature of light pseudoscalar and scalar mesons. An integrated luminosity of 100 pb−1 is expected for next KLOE run, resulting in 3 × 108 f collected. A detailed simulation and full event reconstruction of both signal and background events indicates a sensitivity reach of 1 ÷ 2% on the branching ratios. Some preliminary studies on real data are also presented, showing a good agreement with MonteCarlo distributions

Objectives: Cancer stem cells make up a subpopulation of cells within tumours that drive tumour initiation, growth and recurrence. They are resistant to many current types of cancer treatment, causing failure of such therapeutic approaches, including chemotherapy and radiotherapy. In the study described here, anti-proliferative effects of 3-O-methylfunicone (OMF), a metabolite from Penicillium pinophilum, were investigated on human breast cancer MCF-7 cells and cancer stem cells selected as mammospheres derived from MCF-7s. Materials and methods: Stemness markers were analysed on isolated mammospheres showing positive expression of CD24, CD29, CD44, CD133, CD184 and CD338. Cell proliferation and apoptosis were analysed by flow cytometry and RT-PCR. Cell colony formation assays were performed to evaluate colony formation of mammospheres. Results and conclusion: OMF treatment affected both MCF-7 and mammosphere growth, inducing apoptosis. In addition, OMF strongly reduced stemness markers and survivin, hTERT and Nanog-1 gene expression. Growth of colonies in soft-agar was significantly affected by OMF treatment, too. Lastly, we tested ability of MCF-7 cells to form mammospheres after treatment with OMF or cisplatin, demonstrating that OMF treatment resulted in drastic reduction in number of mammospheres. These results introduce OMF as an effective molecule in suppressing breast cancer stem cells.

Background: Colon capsule endoscopy (CCE) represents a new diagnostic, endoscopic technology for colonic exploration. Current protocols of preparation led to discordant rates of adequate cleansing level or CCE excretion. Aim: To evaluate the effect of a new regimen of bowel preparation for CCE on colon cleansing levels and on rate of capsule excretion. Study: 60 patients were prospectively enrolled. The new regimen of preparation consisted of a split regimen of PEG administration and of a 45 mL dose of sodium phosphate (NaP). Four senna tablets and a low-residue diet were also included. CCE excretion rate, colon cleansing, and accuracy were assessed. Results: Forty-six patients were included in the final analysis, 13 patients (22%) being excluded because of preparation protocol deviations and one due to CCE technical failure (2%). At CCE, bowel preparation was rated as good in 78% of patients, fair in 20% and poor in 2%. CCE excretion rate occurred in 83% of patients. CCE sensitivity and specificity for significant findings was 100% and 95%, respectively. Conclusions: The combination of a split-dose of PEG solution with a low dose of NaP boosters resulted in high rates of adequate cleansing level and CCE excretion. © 2010 Editrice Gastroenterologica Italiana S.r.l.

The mobility of selected heavy metals in trace concentrations was investigated in a standard OECD soil irrigated with the effluent of a real municipal wastewater treatment plant. While Cd, Cu and Ni accumulation-migration patterns were mainly influenced by the mobility of colloids generated from soil organic and inorganic matter, Zn mobility was more influenced by the wastewater content of dissolved organic matter and by its salinity. Metal accumulation caused by interaction with colloids resulted in contamination peaks both in different zones of the soil column and in the leaching solution. The release of metals in the leachate was correlated to the contemporary release of silicates from kaolinite and dissolved organic matter, identified through UV absorbance and chemical oxygen demand monitoring. The hypothesized colloidal mobilization was confirmed by spectroscopic studies. The highly heterogeneous complexes of organic and inorganic molecules responsible for metal transport through soil appeared to be structured in highly stable micellar aggregates.

Background New generation videoendoscopes potentially may visualize duodenal villi. This study compared endoscopic findings with this type of instrument to the histopathologic evaluation of duodenal villi. Methods A total of 191 patients underwent upper endoscopy for the purpose of obtaining duodenal biopsy specimens. The findings were assessed independently by 3 experienced observers by using a commercially available, high-resolution, high-magnifying (×2) videoendoscope. The duodenal villous profile was determined by endoscopic magnification and by endoscopic magnification after filling the duodenum with water. With both endoscopic magnification and endoscopic magnification after filling the duodenum with water, villous patterns were scored as the following: definitely present, partially present, or definitely absent. Villous patterns also were histopathologically scored as the following: normal, partial villous pattern, or total villous atrophy. Results Interobserver variability was excellent (κ=0.93). The concordance between either endoscopic magnification or endoscopic magnification after filling the duodenum with water and histology was 100% for presence/absence of villi. The sensitivity, the specificity, and the positive and negative predictive values of endoscopic magnification for detection of any villous abnormality were 95%, 99%, 95%, and 99%, respectively; the respective values of endoscopic magnification after filling the duodenum with water were 95%, 98%, 92%, and 99%. Conclusions High-resolution magnifying upper endoscopy can reliably predict the presence or the absence of duodenal villi.

Summary Background : Helicobacter pylori infection may persist after both first- and second-line current treatments. Aim : To assess the efficacy of a third-line, culture-guided treatment approach for the eradication of H. pylori. Methods : Patterns of resistance were analysed in H. pylori isolates from 94 consecutive patients in whom H. pylori infection had persisted after two eradication attempts. Using the epsilometer test, susceptibility analysis was performed for amoxicillin, clarithromycin, metronidazole, tetracycline and levofloxacin. Patients were then treated with a culture-guided, third-line regimen: 89 patients with a 1-week quadruple regimen including omeprazole, bismuth, doxycycline and amoxicillin, and five patients with a 1-week triple regimen containing omeprazole, amoxicillin and levofloxacin or clarithromycin. Results : Ninety-four subjects (100%) were resistant to metronidazole, 89 (95%) to clarithromycin, 29 (31%) to levofloxacin and five (5%) to tetracycline. No resistance to amoxicillin was found in any patient. Overall, H. pylori eradication was obtained in 90% of subjects. The quadruple regimen was effective in 81 patients (92% by per protocol and 91% by intention-to-treat analysis). Four patients (80%, both per protocol and intention-to-treat analysis) were H. pylori-negative after the triple regimen. Conclusions : A culture-guided, third-line therapeutic approach is effective for the eradication of H. pylori. Furthermore, the 1-week doxycycline- and amoxicillin-based quadruple regimen is a good third-line ‘rescue’ treatment option.

TRAP-1 is a mitochondrial heat shock protein (HSP), recently identified in Saos-2 osteosarcoma cells adapted to mild oxidative stress induced by diethylmaleate (DEM). TRAP-1 mRNA expression is increased in DEM-adapted cells as well as in tumor cells resistant to 5-fluorouracil and to platin derivatives. Since a strong decrease of TRAP-1 protein levels, upon cisplatin treatment, is observed only in controls but not in the DEM-adapted counterpart, a possible role for this protein in the development of resistant phenotypes could be hypothesized. To characterize the protective role of TRAP-1 against oxidative stress and apoptosis, stable transfectants were generated and characterized for their response to different stress types. These stable clones expressing constitutively high TRAP-1 levels: (i) are more resistant to H2O2-induced DNA damage and to apoptosis by cisplatin; (ii) contain higher reduced glutathione (GSH) levels than control cells; and (iii) do not release the apoptosis-inducing factor into the nucleus upon cisplatin treatment. Furthermore, high TRAP-1 levels interfere with caspase 3 activation. These results confirm the anti-apoptotic role of TRAP-1, and suggest that increased expression of this mitochondrial HSP in DEM-adapted and chemoresistant cells could be part of a pro-survival signaling pathway aimed to evade toxic effects of oxidants and anticancer drugs.

The KLOE detector, mainly designed to measure the R ( ϵ′ ϵ ) parameter of CP violation in KL decay to an absolute precision of ≈ 10−4, is completed and will start data taking by spring 1999 at the e+ e− Φ-factory DAΦNE in Frascati. The primary K-meson physics and the experimental requirements at DAΦNE are summarized. The design strategies, the salient features and the main physics performances of all components of the experimental apparatus are reviewed.

KLOE ,Neutral Kaons - Abstract
We have measured the ratio R=Gamma(K_L -> gamma gamma)/ \Gamma(K_L -> 3 pi^0) using the KLOE detector. From a sample of ~ 10^9 phi-mesons produced at DAFNE, the Frascati phi-factory, we select ~ 1.6 10^8 K_L-mesons tagged by observing K_S -> pi^+ pi^- following the reaction e^+ e^- -> phi -> K_L K_S. From this sample we select 27,375 K_L -> gamma gamma events and obtain R = (2.79 \pm 0.02_{stat} \pm 0.02_{syst}) \times 10^{-3}. Using the world average value for BR(K_{L} -> 3 pi^0), we obtain BR(K_{L} -> gamma gamma) = (5.89 \pm 0.07 \pm 0.08) \times 10^{-4} where the second error is due to the uncertainty on the 3 pi^0 branching fraction.

We report the results on the radiative decays phi --> eta'gamma, phi --> pi(0)pi(0)gamma and phi --> etapi(0)gamma obtained with the KLOE detector at the DAPhiNE phi-factory, using a sample of similar to 16 pb(-1). The value of the branching ratio for the phi --> eta'gamma decay is related to the s (s) over bar and gluon content of the eta'. A fit to the two-pseudoscalar invariant mass for the pi(0)pi(0)gamma and etapi(0)gamma final states gives the contribution of the direct terms phi --> f(0)gamma, phi --> sigmagamma and phi --> a(0)gamma, allowing us to investigate the nature of these light scalar mesons. We present a study of the reaction e+e- --> pi+pi-pi0 at the phi peak W=M(phi)=1019.4 MeV observed with the KLOE detector at Dafne. The reaction is dominated by phi production and decay, e+e- --> phi --> pi+pi-pi0. From a fit to the Dalitz plot density distribution we obtain the rho-meson parameters for its three charge states. We also find the relative amplitudes for phi --> rhopi and phi -> pi+pi-pi0 and the cross-section for e+e- --> omega pi0 with omega --> pi+pi-.

In the fixed energy environment of the $e^{+}e^{-}$ collider DA$\Phi$NE, KLOE can measure the cross section of the process $e^{+}e^{-} \to$ hadrons as a function of the hadronic system energy using the radiative return. At energies below 1 GeV, $e^{+}e^{-} \to \rho \to \pi^{+}\pi^{-}$ is the dominating hadronic process. KLOE phi meson - Abstract
We have measured the radiative decays phi --> etagamma, phi --> eta'gamma selecting pi(+)pi(-)gammagammagamma final state in a sample of similar to 5 x 10(7) phi-mesons produced at the Frascati phi-factory DAPhiNE. We obtain Gamma(phi --> eta'gamma)/Gamma(phi --> etagamma) = (4.70 +/- 0.47 +/- 0.31) x 10(-3). From this result we derive new accurate values for the branching ratio BR(phi --> eta'gamma) = (6.10 +/- 0.61 +/- 0.43) x 10(-5) and the mixing angle of pseudoscalar mesons in the flavour basis phi(P) = (41.8(-1.6)(+1.9))degrees.

The stringent requirements of the KLOE experiment on dimensions, efficiency, resolution and transparency of its drift chamber led to building a tracking device which can be considered innovative from many points of view. Details of the design and construction of this detector, together with preliminary results from the test of its full scale prototype, constructed and operated under a test beam at CERN, are presented and discussed.

The quadrupole tile calorimeters of KLOE (QCAL) are two compact detectors placed close to the interaction point and surrounding the focusing quadrupoles. Their purpose is to increase the hermeticity of KLOE calorimetry. Each QCAL consists of a sampling structure of lead plates and scintillator tiles with wavelength shifter (WLS) fibers and mesh photomultiplier readout arranged in 16 azimuthal sectors. The arrangement of WLS fibers allows the measurement of the longitudinal position of the showers from time of flight. In this paper we describe the QCAL design and assembly and present preliminary results obtained with both cosmic rays and photons from KL decays. The time and energy calibration procedures are also discussed in detail.

We have measured the branching ratio BR(phi-->pi(0)pi(0)gamma) with the KLOE detector using a sample of similar to5x10(7) phi decays. phi mesons are produced at DAPhiNE, the Frascati phi-factory. We find BR(phi-->pi(0)pi(0)gamma) = (1.09+/-0.03(stat)+/-0.05(syst)) x 10(-4). We fit the two-pion mass spectrum to models to disentangle contributions from various sources. KLOE KS meson - Abstract
We have measured the ratio R-S(pi) = Gamma(K-S --> pi(+)pi(-)(gamma))/Gamma(K-S --> pi(0)pi(0)) with the KLOE detector at the DAPhiNE e(+)e(-) collider. This measurement is fully inclusive with respect to the pi(+)pi(-)gamma final state. The sample of over 106 two-pion decays of tagged K-S mesons allows a statistical error as low as similar to 0.1% to be obtained. The accuracy is limited by systematic uncertainties, which are estimated primarily from data. We find R-S(pi) = 2.236 +/- 0.003(stat) +/- 0.015(syst).

The main aim of the KLOE experiment at DA Phi NE, the Frascati phi -factory, is to study CP violation in the K-0-(K) over bar (0) system. Requirements on shower detection are very stringent. An hermetic, lead-scintillating fiber sampling calorimeter has been chosen and built. A review of the methods used to calibrate and reconstruct energy and timing is reported in this paper. Emphasis is given to the calibration procedures developed using the 2.4 pb(-1) collected in 1999. An energy resolution of 5.7% rootE/GeV is achieved together with a linearity in energy response better than 1% above 50 MeV. A time resolution of similar to 54 ps/ rootE/GeV is also measured on samples of radiative Bhabha and Phi decays. (C) 2001 Elsevier Science B.V. Ail rights reserved.

KLOE is acquiring and analyzing hundreds of terabytes of data, stored as tens of millions of files. In order to simplify the access to these files, a URI-based mechanism has been put in place. The KID package is an implementation of that mechanism and is presented in this paper.

- Abstract
Treatment of chronic heart failure (CHF) is strongly focused on evidence-based medicine. However, large trials are often far away from the “real world” of geriatric patients and their messages are poorly transferable to the clinical management of CHF elderly patients. Precipitating factors and especially non-cardiac comorbility may decompensate CHF in the elderly. More importantly, drugs of first choice, such as Angiotensin Converting Enzyme inhibitors and β-blockers, are still underused and effective drugs on diastolic dysfunction are not available. Poor adherence to therapy, especially due to cognitive disorders and depression, worsens the management. Electrical therapy is indicated, but attention to the older age groups with reduced life expectancy has to be paid. Physical exercise, stem cells, gene therapy and new devices are encouraging, but definitive results are not available yet. Palliative care plays a key role in the end-stage of the disease. Follow-up of CHF elderly patient is very important but tele-medicine is the future. Finally, self-care management, caregiver training and multidimensional team represent the critical point of the treatment of CHF elderly patients.

The KLOE experiment's major physics goal is to measure the R( ϵ′ ϵ ) , with a precision of 10−4, via both the traditional double ratio method and the kaons' quantum interferometry. The experiment has started taking data at DAfNE, the f-factory at the Frascati National Laboratory (LNF) of INFN in Italy, beginning in April 1999. Integrated luminosity exceeding 100 pb−1 (108 f's) are expected to be delivered for end 1999/early 2000. A description of the detector's design and of its performances is presented. At the moment of writing the experiment has collected its first collisions for an integrated luminosity of 220 nb−1; the peak luminosity reached has been of 2 × 1030 cm−2sec−1.

- Abstract
KLOE, a new state of the art detector recently commissioned for physics operation at DAFNE, has many innovative interesting features, especially in the DAQ and Trigger areas. Custom electronics assert a trigger in a 2 microseconds decision time and distributes it to the FEE with a 50 ps time resolution. Data are read out using 10 front-end data acquisition chains and sent to a farm of online servers through a FDDI Gigaswitch. This design of the KLOE DAQ system allows us to manage an input data rate as high as 50 Mbyte/s, and is completely scalable by extending the number of computers connected to the switch

46. Human CD34+ stem cells produce bone nodules in vivo

Objectives: The aim of this study was to select and provide enough stem cells for quick transplantation in bone engineering procedures, avoiding any in vitro expansion step. Materials and Methods: Dental germ pulp, collected from 25 healthy subjects aged 13–20 years, were subjected to magnetic‐activated cell sorting to select a CD34(+) stem cell population capable of differentiating into pre‐osteoblasts. These cells were allowed to adhere to an absorbable polylactic–coglycolic acid scaffold for 30 min, without any prior expansion, and the CD34(+) cell‐colonized scaffolds were then transplanted into immunocompromised rats, subcutaneously. Results: After 60 days, analysis of recovered transplants revealed that they were formed of nodules of bone, of the same dimensions as the original scaffold. Bone‐specific proteins were detected by immunofluorescence, within the nodules, and X‐ray diffraction patterns revealed characteristic features of bone. In addition, presence of platelet endothelial cell adhesion molecule and von Willebrand factor immunoreactivity were suggestive of neo‐angiogenesis and neovasculogenesis taking place within nodules. Importantly, these vessels were HLA‐1(+) and, thus, clearly human in origin. Conclusions: This study indicates that CD34(+) cells obtained from dental pulp can be used for engineering bone, without the need for prior culture expanding procedures. Using autologous stem cells, this schedule could be used to provide the basis for bone regenerative surgery, with limited sacrifice of tissue, low morbidity at the collection site, and significant reduction in time needed for clinical recovery.

Background Patients with chronic abdominal pain consult gastroenterologists frequently, requiring a large number of examinations. Aim To assess the diagnostic yield of capsule endoscopy in patients with chronic abdominal pain of unknown origin and negative diagnostic work-up. Patients From January 2002 to September 2004, 16 patients (10 female; mean age 42.7 years) who complained of chronic abdominal pain were referred to our unit for capsule endoscopy. Methods Chronic abdominal pain was defined as continuous or almost continuous, for at least 3 months and without criteria for other gastrointestinal disorders. All patients had a previous diagnostic work-up including abdominal ultrasonography, oesophagogastroduodenoscopy, colonoscopy and small bowel follow through, performed within 2 months. Results Capsule endoscopy was normal in 12 patients (75%). Small bowel abnormalities were found in three patients, but were considered irrelevant. In one patient (6.3%), capsule endoscopy revealed ileal erosions and inflammation and was retained in a stricture undetected by radiology. This patient underwent elective surgery which revealed an ileal carcinoid neoplasm. Conclusions Capsule endoscopy identified a specific cause of chronic abdominal pain in only one patient. Capsule endoscopy is a safe procedure but does not seem to play an important role in the evaluation of patients with chronic abdominal pain of unknown origin.

Our purpose was to assess the potential of ultrasonography (US) in the detection of in-transit or satellite metastases.Following a review of the relevant literature, we present the results of a retrospective study based on 2,000 malignant melanoma patients with complete case records. Of these, we selected 600 patients who had a thick melanoma (1 mm) at presentation but were clinically free of in-transit or satellite melanoma metastases during follow-up. All patients underwent periodic clinical and imaging investigations, as well as US examination of the site of the surgical wound and surrounding soft tissues.US raised the suspicion of in-transit or satellite metastases in 63 patients. A total of 95 lesions were identified. Average lesion diameter was 0.7 mm, and only four were larger than 1 cm. All suspected lesions were confirmed by surgery, follow-up or US-guided fine-needle aspiration (FNA) with 22-gauge needles using a freehand technique and exploiting the capillarity principle. In this series, there were apparently no false positive or false negative US results although inclusion criteria precluded correct evaluation of possible false negatives. Minimum lesion diameter allowing sonographic detection appears to be around 0.4 mm. US features of in-transit metastases have been well documented. They usually appear as solid lesions, hypoechoic relative to the surrounding subcutaneous fat and with relatively well-defined and regular contours and good US transmission. Internal structure is fairly homogeneous, and sometimes millimetresized fluid areas can be appreciated inside. Larger metastatic lesions may exhibit internal vascular signals at power Doppler imaging. These findings in dermatological sonography are almost exclusive of metastases but may also be seen in glomangioma, which, however, has intense intralesional vascularity. US-guided FNA plays an important role in diagnosis of metastases from malignant melanoma. Of the 32 nodules that were cytologically sampled, a definitive or most probable diagnosis of metastasis was made for nodules with a mean diameter of 0.7 mm (minimum 0.5 mm).Sonography of soft tissues surrounding the original site of a malignant melanoma should be more widely used and associated with US-guided FNA biopsy.

Background Upper endoscopy is not routinely performed to directly detect abnormalities of the duodenal villi. The reliability of the immersion technique for assessment of duodenal villi was evaluated in a series of patients with dyspepsia. Methods A total of 396 patients who were to undergo standard EGD for dyspeptic symptoms were enrolled. Patients with suspected malabsorption were excluded. By performing a "modified immersion technique," duodenal villi were scored as the following: definitely present, partially present, or definitely absent. Three duodenal biopsy specimens were obtained from each patient, and villi also were scored histologically as the following: normal, partial villous pattern, or total villous atrophy. Results Sensitivity, specificity, and positive and negative predictive values of the modified immersion technique for detection of total villous atrophy were 100%, 99.7%, 85.7%, and 100%, respectively. Sensitivity, specificity, and positive and negative predictive values of modified immersion technique for detection of partial villous patterns were 75%, 99.5%, 60%, and 99.7%, respectively. Sensitivity, specificity, and positive and negative predictive values for modified immersion technique detection of any villous abnormality (partial or total villous atrophy) were 90.9%, 99.5%, 83.3%, and 99.7%, respectively. Conclusions During standard EGD, duodenal evaluation by modified immersion technique can reliably detect abnormalities of duodenal villi. This simple diagnostic technique may be performed routinely during endoscopic exploration of duodenum.
